Zusammenfassung Hintergrund Im Rahmen der Kontaktbeschränkungen aufgrund der COVID-19-Pandemie vom März 2020 wurde zur Aufrechterhaltung der Patientenversorgung eine videobasierte Sprechstunde eingeführt. Als Basis einer kontaktminimierenden Kommunikation wurde diese nach den Maßnahmen fortgeführt. Ziel der Arbeit Ziel dieser prospektiven Pilotstudie ist die Beurteilung hinsichtlich der Effektivität, der technischen Machbarkeit sowie der Steuerung von Patientenströmen sowohl unter Lockdown-Bedingungen sowie in der Zeit danach. Material und Methoden Die Auswertung erfolgte vom Beginn des ersten Lockdowns am 16.03.2020 bis zum Stichtag der zweiten einschränkenden Maßnahmen am 14.12.2020. Dokumentiert wurde die Qualität der Verbindungen bezüglich Ton und Bild. Weiterhin wurden die Konsequenzen aus den Gesprächen dokumentiert. Unterschieden wurde hierbei in 4 Kategorien: 1. keine erneute Vorstellung, 2. Wiedervorstellung in der Videosprechstunde, 3. operative Therapie und 4. Vorstellung zur klinischen Untersuchung. Ergebnisse Es erfolgten 236 Patientenvorstellungen mittels Videosprechstunde. Insgesamt erfolgten 182 (82 %) Gespräche ohne Einschränkungen. Bei 47 (21 %) Konsultationen handelte es sich um Erstvorstellungen. Bei 41 (18 %) Patienten erfolgte keine erneute Vorstellung. Bei 36 (16 %) Patienten wurde eine Wiedervorstellung in der Videosprechstunde geplant, bei 36 (16 %) Patienten erfolgte die direkte Einweisung zur Operation, und bei 105 (47 %) Patienten wurde eine Wiedervorstellung zur klinischen Untersuchung vereinbart. Diskussion Bei 40 % der Patienten konnte durch den Kontakt in der Videosprechstunde eine definitive Entscheidung gestellt werden. Auf der anderen Seite erfolgte bei 47 % der Patienten eine Vorstellung zur klinischen Untersuchung. Die Videosprechstunde ist eine sehr nützliche Maßnahme, um Patientenaufkommen zu leiten und den direkten Arzt-Patient-Kontakt sichtbar zu unterstützten.

Zusammenfassung Bei der Versorgung von lateralen Clavicula-Frakturen sollte auf eine instabile K‑Draht-Osteosynthese unbedingt verzichtet werden. Bei Vorliegen von sicheren Operationsverfahren (Plattenosteosynthese, Hybridversorgungen) sollten diese bevorzugt werden. Bei Durchführung primärer oder additiver K‑Draht-Osteosynthesen ist auf deren Sicherung z. B. durch Umbiegen zu achten, da es bei fehlerhafter Versorgung zu erheblichen Komplikationen, wie Pseudarthrose oder sekundären Dislokation des Materials, kommen kann.

Lateral clavicle fractures can be treated both conservatively and surgically depending on the fracture classification. Different surgical techniques have been described for the operative treatment. The selection of the appropriate technique is decisive for the functional outcome and healing process without complications.We report on a patient with a secondary dislocation of two Kirschner wires after Kirschner wire osteosynthesis. The secondary dislocation caused one of the wires to migrate into the mediastinum and pulmonary tissue, directly under the aortic arch. To prevent further migration with potential damage to surrounding structures, a uniportal video-assisted thoracoscopy was performed to retrieve the wire.The treatment of lateral clavicle fractures should be performed with bent Kirschner wires as they can otherwise lead to severe complications including the occurrence of pseudarthrosis or secondary migration of the material. Purpose Fibular mobility after suture button stabilization (SBS) of inferior tibiofibular joint (syndesmosis) injuries has been described. This effect is called the “flexible nature of fixation (FNF).” In this study, we aimed to quantify FNF in syndesmotic stabilization. Methods Postoperative bilateral computed tomography (CT) of ankle fractures with syndesmosis stabilization by SBS or syndesmotic screw (SYS) was retrospectively analyzed. The transverse offset (TO) and vertical offset (VO) were defined by evaluating the drill channels. The reduction outcome was evaluated by the side-to-side difference between the clear space and the anterior tibiofibular distance (antTFD). The calculated anterior tibiofibular distance (cal-antTFD) was calculated by subtracting the TO from the validated antTFD. Subsequently, a reevaluation of the reduction outcomes after SYS or SBS stabilization was performed using cal-antTFD. Results Sixty patients (44 with SBS and 16 with SYS stabilization) were analyzed. The intra-rater and inter-rater reliabilities for TO and VO were excellent (α > 0.92). SYS stabilization showed lower mean TO (− 0.02 mm; SD 0.14) and VO (0.11 mm; SD, 0.29 mm) than SBS stabilization (TO 1.16 mm, SD 1.4 mm; VO 0.2 mm, SD 0.8 mm; p = 0.001). The rate of malreduction according to cal-antTFD was higher than that of antFTD (p = 0.033). Conclusion The presented method, which evaluates the position of the tibial to the fibular drill channel, allowed the quantification of the “FNF.” The often described difference in the dynamic stabilization of SBS compared to the rigid stabilization by SYS could be objectified. Considering cal-antTFD illustrates that FNF potentially reduces the rate of malreduction in SBS stabilization.

Zusammenfassung Hintergrund Die proximale Humerusfraktur ist eine der häufigsten Frakturen des älteren Menschen. Während epidemiologische Faktoren gut untersucht wurden, ist der Einfluss einer proximalen Humerusfraktur auf die Morbidität, Mortalität und assoziierten Kosten unzureichend analysiert. Methode Auf der Basis von 4,1 Mio. GKV-Versicherten wurden für den Zeitraum 2012–2016 Patienten mit (Studienpopulation, SP) und ohne (Vergleichsgruppe, VG) proximale Humerusfraktur (pHF) in Hinblick auf Komorbidität, Rehospitalisierung, Mortalität, Medikamenten- und Heilmittelbedarf sowie Anzahl und Facharztkontakten verglichen. Ergebnis 6068 Patienten der SP erfüllten die Ein- und Ausschlusskriterien (Alter 69,4 ± 14,3 Jahre; m : w = 28,2% : 71,8%). 4781 Patienten (78,8%) erhielten eine operative, 1287 Patienten (21,2%) eine konservative Versorgung der pHF. The proximal humeral fracture is one of the most common fractures in the elderly. While epidemiological factors have been well studied, the influence of a proximal humeral fracture on morbidity, mortality and associated costs has not yet been adequately analysed.On a basis of 4.1 million insurance holders of the German public health insurance (GKV), patients with (study population, SP) and without (comparison group, VG) a proximal humeral fracture (pHF) were compared with regard to comorbidity, rehospitalisation, mortality, drug and aid needs as well as number of physician contacts. Study period was between 2012 and 2016.6068 patients of the SP met the inclusion and exclusion criteria (age 69.4 ± 14.3 years; male : female = 28.2% : 71.8%). 4781 patients (78.8%) received surgical, 1287 patients (21.2%) conservative treatment of the pHF. Rehospitalisations and visits to the general practitioner occurred more frequently in the SP vs. VG (p 0.01). Contacts with specialists after pHF varied according to specialty, as did newly occurring diseases. Typical specialities for preventive examinations were significantly less common (gynaecology p 0.01, pathology p 0.01, dermatology p 0.01). According to pHF, the costs of SP for drugs (2490.76 ± 1395.51 € vs. 2167.86 ± 1314.43 €; p = 0.04), medical therapies (867.01 ± 238.67 € vs. 393.26 ± 217.55 €; p 0.01) and aids (821.02 ± 415.73 € vs. 513.52 ± 368.76 €; p 0.01) were significantly above the VG. The two-year survival after pHF is lower in the SP than in the VG (p 0.01).The results show increased morbidity and mortality as well as medical costs after a proximal humeral fracture. Preventive examinations and treatments are rarer. Es wurden 2 Gruppen, Gruppe A mit gutem Outcome (∆CMS ≤ 15P.) und Gruppe B mit schlechtem Outcome (∆CMS ≥ 16P.), definiert. Weiterhin wurden im Vorfeld deskriptive Variablen inkl. Definition einer Komplikation festgelegt. Ergebnisse Es wurden 138 Patienten in die Studie eingeschlossen (Gruppe A: 91, Gruppe B: 47). Das durchschnittliche Follow-up beträgt 93,86 Monate ± 37,33 (36–167). Gruppe B hatte signifikant mehr Komplikationen. Weiterhin zeigte sich, dass Patienten mit einem schlechten Outcome bez. der Schulter (Gruppe B) auch einen signifikant niedrigeren EQ-VAS (78,9 ± 19,2 [20,0–100] vs. 70,4 ± 19,5 [15,0–98,0]; p = 0,008) und EQ-Index (0,91 ± 0,14 [0,19–1,00] vs. 0,82 ± 0,17 [0,18–1,00]; p Diskussion Zusammenfassend weisen in der vorliegenden Studie die Patienten mit einem schlechten Outcome der Schulterfunktion im CMS nach durchschnittlich mehr als 6 Jahren Follow-up eine signifikant niedrigere generelle Lebensqualität auf. Das schlechte Outcome ist auf eine signifikant höhere postoperative Komplikationsrate zurückzuführen. Dies war unabhängig von der vorliegenden Frakturmorphologie und vom verwendeten operativen Verfahren.

Surgical treatment of proximal humerus fracture is an established procedure. Postoperative complications have been shown to have a significant impact on shoulder-specific outcome. Little is known to date about an influence on injury-independent quality of life.The aim of this retrospective study is to analyse whether patients with a poor functional outcome after surgically treated proximal humerus fracture also show a reduced general quality of life in the medium term. Emphasis is placed on the analysis of patients with poor functional outcome due to postsurgical complications.Evaluation of all patients operated at one level 1 trauma centre with a proximal humerus fracture in the period 01.01.2005 to 31.12.2015 and follow-up using validated scores (Constant-Murley Score [CMS], EQ-5D). Two groups, group A with good outcome (∆CMS ≤ 15P.) and group B with poor outcome (∆CMS ≥ 16P.), were defined. Furthermore, descriptive variables including definition of a complication were defined in advance.138 patients were included in the study (group A: 91, group B: 47). The mean follow-up was 93.86 months ± 37.33 (36-167). Group B had significantly more complications. Furthermore, patients with poor outcome regarding the shoulder (group B) were also found to have significantly lower EQ-VAS (78.9 ± 19.2 [20.0-100] vs. 70.4 ± 19.5 [15.0-98.0]; p = 0.008) and EQ index (0.91 ± 0.14 [0.19-1.00] vs. 0.82 ± 0.17 [0.18-1.00]; p 0.001).In conclusion, in the present study, the patients with poor outcome of shoulder function in CMS have significantly lower overall quality of life after a mean of more than 6 years of follow-up. The poor outcome was due to a significantly higher postoperative complication rate. Zusammenfassung Hintergrund Die Corona-Pandemie stellt Krankenhäuser vor enorme finanzielle Herausforderungen. Am Beispiel einer Klinik für Orthopädie und Unfallchirurgie soll die Leistungsentwicklung in der stationären Versorgung der ersten 5 Wochen nach Beginn der gesetzgeberisch angeordneten Leistungsreduktion im Vorjahresvergleich sowie eine Bewertung der gesetzgeberisch festgelegten Kompensationsmaßnahmen bewertet werden. Methodik Anhand der Leistungszahlen wird ein Vergleich des Zeitraumes 16.03. bis 17.04.2019 und demselben Zeitraum 2020 durchgeführt. Veränderungen von Fallzahl, Casemix, Casemix-Index und Daymix-Index sowie den Belegungstagen werden erfasst. Auf diese Veränderungen werden die monetären Maßnahmen aus dem COVID-19-Krankenhausentlastungsgesetz angewendet und deren Auskömmlichkeit bewertet. Ergebnisse Im Vergleich zum Vorjahr kommt es im Beobachtungszeitraum zu einem Rückgang der stationären Aufnahmen von 307 Patienten. Demzufolge waren ein Rückgang des Casemix um 595 Punkte und der Belegungstage um 2320 Tage zu verzeichnen. Es ergibt sich ein Erlösrückgang von ca. 1,9 Mio. EUR. Die Leerbettenpauschale stellt die monetär größte Kompensation der Erlösausfälle dar. Sie beläuft sich auf ca. 1,3 Mio. EUR. Unter Berücksichtigung weiterer Unterstützung und einer Bereinigung um variable Kosten bleibt ein Fehlbetrag von 382.069 € in Bezug auf die stationären Leistungen für 5 Wochen. Diskussion Die Maßnahmen des Gesetzgebers stellen eine wichtige Stütze zur wirtschaftlichen Absicherung deutscher Krankenhäuser dar. Die fehlende Differenzierung der Maßnahmen nach Fachrichtung führt für Orthopädie und Unfallchirurgie zu einer nur unzureichenden Kompensation.

Purpose Numerous techniques have been described for the tibial-sided graft preparation in anterior cruciate ligament (ACL) reconstruction. The use of less suture material for graft preparation is thought to improve ingrowth and to reduce the risk for infection. At the same time, the suture construct should be strong enough to resist the surgeon’s pull during tensioning of the transplant. Methods In total, 39 fresh-frozen procine deep flexor tendons were used and prepared as four-strand grafts. In the three-suture group (n = 19), graft preparation was performed using three tibial-sided sutures, with each tendon end sutured separately. In the one-suture group (n = 20), a modified graft preparation using only one tibial-sided suture was applied. Each sample underwent load-to-failure testing (Nmax) after cyclic pre-loading. To estimate intraoperative tension forces acting on the tibial-sided suture constructs, the maximal tension force of 26 volunteers on such a construct was measured using a load cell. Results The biomechanical testing of the two different suture constructs showed a significantly higher load-to-failure for the three-suture group (711 N ± 91 N) compared to the one-suture group (347 N ± 24 N) (p = 0.0001). In both groups, the mode of failure was a tear of the suture in all samples. A failure of the suture–tendon interface was not observed in any case. The median maximal tension force on the construct applied by the 26 volunteers was 134 N (range 73–182 N). Conclusion The presented single-suture tendon graft preparation resisted to smaller failure loads than the conventional three-suture technique. However, no failures in the suture–tendon interface were seen and the failure loads observed were far beyond the tension forces that can be expected intraoperatively. Hence, the single-suture graft preparation technique may be a valuable alternative to the conventional technique.

The aim is to address core decompression and pathologies of the femoral head, treating them during the same procedure. Furthermore, radiation exposure will be reduced. Femoral head necrosis ARCO (Association Research Circulation Osseous) stages I–III. Progressive femoral head necrosis as ARCO stages IIIC–IV. Arthroscopically navigated core decompression of the femoral head using an established optoelectronic system with fluoro-free software module. First, hip joint arthroscopy was performed and further pathologies were treated. Second, core decompression was navigated by a navigation pointer and drill sleeve to reach the correct target point. After visualization, the procedure is repeated 3–5 times. Limited weight bearing of the operated leg (20 kg) for 10–14 days. Active or passive continuous motion machine for 4 weeks. Adjuvant postoperative indomethacin therapy for 10 days to reduce pain and bone marrow edema. From May 2018 to January 2019, 7 patients (male = 4; 40 ± 9 years) underwent arthroscopically navigated core decompression with 2 (29%) and 5 (71%) patients being classified as ARCO II and III, respectively. Preoperatively, all patients reported load-dependent pain. In all cases, we could identify synovitis, which results in soft tissue release and synovectomy. Furthermore, 4 of 7 patients had an additional labrum lesion, which is addressed by refixation or shrinking. Compared to the conventional technique, this fluoro-free navigation procedure allows more precise drilling. Moreover, additional pathologies, as found in all our cases, could be simultaneously addressed. The intraoperative radiation exposure for the patient and surgical team could also be reduced. Although arthroscopically assisted core decompression requires more preparation time, there are advantages over conventional surgery.

Background Cement augmentation has been suggested to increase the stability of screw anchoring in osteoporotic humeral fractures. Initial results are promising but may be jeopardized by cement leakage into the joint and difficult implant removal. Absorbable cement might have advantages in this regard, but it is unclear if the primary stability of both techniques is equivalent to each other. Therefore, this study aimed to compare its primary stability with that of non-absorbable cement augmentation. Methods Nineteen cadaveric humeri with two-part fracture models were treated with locking plate osteosynthesis and cement augmentation using either absorbable calcium phosphate cement (group 1) or polymethylmethacrylate (group 2). Fracture movement, stiffness, failure mode, and ultimate load under cyclic compressive loading were examined and compared between the groups. Findings The absolute and relative stiffness values in group 1 were significantly smaller than those in group 2 after 50 cycles (group 1: 114 ± 38 N/mm and 94 ± 8% vs. group 2: 188 ± 71 N/mm and 106 ± 9%; p50 = 0.022), 2000 cycles (group 1: 97 ± 34 N/mm and 81 ± 15% vs. group 2: 153 ± 47 N/mm and 88 ± 15%; p2000 = 0.028), and 5000 cycles (group 1: 98 ± 40 N/mm and 81 ± 22% vs. group 2: 158 ± 40 N/mm and 92 ± 16%; p5000 = 0.028). The failure load was not statistically significantly different between the groups. Interpretation Although the PMAA group showed higher values for absolute and relative stiffness, no statistically significant difference was found in the primary stability between absorbable and non-absorbable cement augmentation supporting plate osteosynthesis in proximal humeral fractures. In view of the potential advantages of bio-absorbable cement during the healing process, its use should be considered for the augmentation and stabilization of osteoporotic fractures.

With the regulation of the Saxon State Government and the Saxon State Ministry for Social Affairs and Social Responsibility on the modification of the Infection Protection Act of March 2020 coming into force, a video-based outpatient consultation was implemented to maintain patient care. In order to allow communication with minimized contact, this was continued after the lockdown.The aim of this prospective pilot study was to assess the effectiveness of a video-based outpatient consultation service, technical feasibility and control of patient flow under both lockdown and post-lockdown conditions.The initial evaluation was conducted up to 14 December 2020 when the second restrictive measures were implemented by the state government. The quality of the connections regarding sound and image was documented. Furthermore, the consequences of the conversations were documented. Distinctions were made in four categories: 1. no follow-up visit, 2. follow-up via video consultation, 3. operative intervention and 4. in-person follow-up visit for clinical examination.There were 236 video-based outpatient consultations, 182 (82%) consultations were without restrictions and 47 (21%) consultations were initial presentations. There were no follow-up consultations in 41 (18%) patients. Video-based follow-up was scheduled in 36 (16%) patients, direct referral for surgery in 36 (16%) patients, and in-person follow-up in 105 (47%) patients.In 40% of the patients a definite decision could be made by the initial video-based consultation alone. On the other hand, 47% of the patients needed in-person follow-up for a clinical examination. Thus, video consultation is a very useful measure to manage patient volume and visibly support direct doctor-patient contact.HINTERGRUND: Im Rahmen der Kontaktbeschränkungen aufgrund der COVID-19-Pandemie vom März 2020 wurde zur Aufrechterhaltung der Patientenversorgung eine videobasierte Sprechstunde eingeführt. Als Basis einer kontaktminimierenden Kommunikation wurde diese nach den Maßnahmen fortgeführt.Ziel dieser prospektiven Pilotstudie ist die Beurteilung hinsichtlich der Effektivität, der technischen Machbarkeit sowie der Steuerung von Patientenströmen sowohl unter Lockdown-Bedingungen sowie in der Zeit danach.Die Auswertung erfolgte vom Beginn des ersten Lockdowns am 16.03.2020 bis zum Stichtag der zweiten einschränkenden Maßnahmen am 14.12.2020. Dokumentiert wurde die Qualität der Verbindungen bezüglich Ton und Bild. Weiterhin wurden die Konsequenzen aus den Gesprächen dokumentiert. Unterschieden wurde hierbei in 4 Kategorien: 1. keine erneute Vorstellung, 2. Wiedervorstellung in der Videosprechstunde, 3. operative Therapie und 4. Vorstellung zur klinischen Untersuchung.Es erfolgten 236 Patientenvorstellungen mittels Videosprechstunde. Insgesamt erfolgten 182 (82 %) Gespräche ohne Einschränkungen. Bei 47 (21 %) Konsultationen handelte es sich um Erstvorstellungen. Bei 41 (18 %) Patienten erfolgte keine erneute Vorstellung. Bei 36 (16 %) Patienten wurde eine Wiedervorstellung in der Videosprechstunde geplant, bei 36 (16 %) Patienten erfolgte die direkte Einweisung zur Operation, und bei 105 (47 %) Patienten wurde eine Wiedervorstellung zur klinischen Untersuchung vereinbart.Bei 40 % der Patienten konnte durch den Kontakt in der Videosprechstunde eine definitive Entscheidung gestellt werden. Auf der anderen Seite erfolgte bei 47 % der Patienten eine Vorstellung zur klinischen Untersuchung. Die Videosprechstunde ist eine sehr nützliche Maßnahme, um Patientenaufkommen zu leiten und den direkten Arzt-Patient-Kontakt sichtbar zu unterstützten.

Die hier vorgestellte 1‑Faden-Armierung mit 4‑facher Semitendinosussehne fur Rekonstruktionen des vorderen Kreuzbands (VKB) bietet eine Moglichkeit zur Verkurzung der Operationszeit sowie zur Reduktion von Fremdmaterial. Jedoch sollte das Nahtkonstrukt stark genug sein, um dem Zug des Chirurgen beim Spannen des Transplantats zu widerstehen. Zur Testung vor klinischer Anwendung erfolgte eine biomechanische Untersuchung an frisch gefrorenen tiefen Flexorensehnen von Schweinen. Es erfolgte eine Aufteilung in eine 3‑Faden-Gruppe (n = 19) und in eine 1‑Faden-Gruppe (n = 20). Jede Probe wurde nach zyklischer Vorbelastung einer Load-to-failure-Prufung (Nmax) unterzogen. Um die einwirkenden intraoperativen Zugkrafte abzuschatzen, wurde zusatzlich die maximale Zugkraft von 26 Probanden gemessen. Bei allen Proben war die Versagensart ein Riss des Fadens. Es zeigte sich ein signifikant hohere Versagenslast fur die 3‑Faden-Gruppe (711 N ± 91 N) im Vergleich zur 1‑Faden-Gruppe (347 N ± 24 N; p = 0,0001). Die mittlere maximale Zugkraft auf das Konstrukt, die von den 26 Probanden aufgebracht wurde, betrug 134 N (Bereich: 73–182 N). Die beobachteten Versagenslasten lagen weit uber den intraoperativ zu erwartenden Zugkraften. Daher ist die 1‑Faden-Transplantatpraparationstechnik eine wertvolle Alternative zur konventionellen Technik.

Purpose Horizontal instability is a common problem after acromioclavicular joint injuries. The aim of this study was to evaluate if there is a difference regarding horizontal stability between an anatomical and a non-anatomical configuration of the double tunnel coraco-clavicular ligament reconstruction of the acromioclavicular joint. Methods Thirteen acromioclavicular joints of human cadaveric shoulders in ethanol-glycerin fixation were included in the study and underwent cyclic anterior and posterior translational testing at a load of 70 N using an electromechanical uniaxial testing machine. The shoulders were randomly assigned to the following groups: double coraco-clavicular tunnel technique in an anatomical configuration (DCTa) and double coraco-clavicular tunnel technique in an inverse configuration of the anatomical position (DCTb). The dislocation was recorded with a 3D optical measuring system. Results The total horizontal displacement (p10 = 0.0221; p5000 = 0.082) was significantly higher for the non-anatomical reconstruction (DCTb) compared to the anatomical reconstruction (DCTa) after every measured amount of cycles. The increase in displacement for DCTb group was overall higher than the increase in displacement for DCTa group but without significance. Conclusion Reconstruction of the CC ligaments in an anatomical configuration with two suture devices results in a significantly higher stability of the AC joint in the horizontal plane than reconstruction of the CC ligaments in a non-anatomical configuration. Based on the results of this biomechanical in vitro study, the use of a double coraco-clavicular reconstruction should focus on an anatomically correct position of the suture devices.

Periprosthetic joint infections (PJI) after hemiarthroplasty for geriatric femoral neck fractures are a devastating complication that results in serious morbidity and increased mortality. Identifying risk factors associated with early infection after HA for hip fractures may offer an opportunity to address and prevent this complication in many patients. The aim of this study was to evaluate preoperative risk factors for early PJI after HA in hip fracture patients. From January 2010 to December 2015, 312 femoral neck fractures (AO/OTA 31-B) in 305 patients were included in this single-center, retrospective study. PJI was defined according to the Centers for Disease Control (CDC) definition of deep incisional surgical site infection. Early infection referred to a postoperative period of 4 weeks. Binary univariable and multivariable regression analysis with backward elimination was applied to identify predictors of PJI. Median age of all patients was 83.0 (IQR 76–89) years. We identified 16 (5.1%) early PJI which all required surgical revision. Median length of in-hospital stay (LOS) was 20.0 (IQR 10–36) days after PJI compared to 10.0 (8–15) days without deep wound infection. In-hospital mortality was 30.8 vs. 6.6%, respectively. Preoperative CRP levels (OR 1.009; 95% CI 1.002–1.018; p = 0.044), higher BMI (OR 1.092; 95% CI 1.002–1.189; p = 0.044) and prolonged surgery time (OR 1.013; 95% CI 1.000–1.025; p = 0.041) were independent risk factors for PJI. Excluding infection following major revision due to mechanical complications identified preoperative CRP levels (OR 1.012; 95% CI 1.003–1.021; p = 0.007) and chronic glucocorticoid therapy (OR 6.314; 95% CI 1.223–32.587; p = 0.028) as risk factors, a clear trend was seen for higher BMI (OR 1.114; 95% CI 1.000–1.242; p = 0.051). A cut-off value at CRP levels ≥ 14 mg/l demonstrated a sensitivity of 69% and a specificity of 70% with a fair accuracy (AUC 0.707). Preoperative serum CRP levels, higher BMI and prolonged surgery time are independent predictors of early PJI. Excluding PJI secondary to major revision surgery revealed chronic glucocorticoid use as a risk factor apart from preoperative CRP levels.

Hintere verhakte Schulterluxationen sind seltene Verletzungen. Haufig werden sie bei der primaren Untersuchung ubersehen. Standardrontgenaufnahmen in 2 Ebenen konnen das Risiko, eine hintere Luxation zu ubersehen, deutlich minimieren. In der Literatur findet sich eine Vielzahl von Therapieoptionen. Parameter zur Wahl eines Therapieverfahrens sind Dauer der Luxation, Grose des Defekts und das Alter des Patienten. Die Beurteilung der Defektmorphologie gewinnt zunehmend an Bedeutung. Nach anatomischer Rekonstruktion sind gute bis sehr gute Ergebnisse mit stabilen Schultern zu erzielen. Bei alteren Patienten (>70) konnten inverse Prothesen den anatomischen Implantaten uberlegen sein.

We report the case of a 24-year-old female triathlon athlete with a snapping syndrome of both the distal semimembranosus and the semitendinosus tendon which has not been described so far. The semitendinosus tendon was harvested whereupon snapping has diminished. Tenoscopy and digital palpation showed the semimembranosus tendon to cause the snapping, as well. A careful step-by-step release of the anterior insertion of the tendon was performed with the result of a snapping-free range of motion of the patient's knee.

Horizontal instability—especially in the posterior plane—is a common problem after acromioclavicular joint injuries. The purpose was to compare the stability of a single coraco-clavicular tunnel technique and a double coraco-clavicular tunnel technique for coraco-clavicular ligament reconstruction and to examine the influence of an additional acromioclavicular cerclage on the horizontal stability in the acromioclavicular joint. 21 acromioclavicular joints of human cadaveric shoulders were randomly assigned to the following groups: single coraco-clavicular tunnel technique with horizontal augmented acromioclavicular cerclage (SCT + AC); double coraco-clavicular tunnel technique (DCT); double coraco-clavicular tunnel technique and acromioclavicular cerclage (DCT + AC). The specimens underwent cyclic horizontal testing and were recorded using a 3D optical measuring system. The displacement and the increase in displacement in relation to the displacement after precondition for SCT + AC were significantly higher after every measured amount of cycles than for DCT (p10 = 0.0023; p5000 = 0.0012) and DCT + AC (p10 = 0.0006; p5000 = 0.0012). There was no significant difference in the total displacement, or in the increase in total displacement between double coraco-clavicular tunnel reconstructed groups with and without additional acromioclavicular cerclage. Double coraco-clavicular tunnel technique with and without additional acromioclavicular cerclage results in a significant higher stability regarding the horizontal plane in comparison to single coraco-clavicular tunnel technique with acromioclavicular cerclage. Based on the results of this biomechanical in vitro study, the use of an additional acromioclavicular cerclage with single coraco-clavicular tunnel technique may not be indicated in most cases. The effect of an additional acromioclavicular cerclage seems to be negligible, at least in presence of a double-coraco-clavicular tunnel technique reconstruction. Techniques of AC joint reconstruction should focus on the use of double coraco-clavicular tunnel devices.

The aim of this systematic review is to show the feasibility and efficacy of operatively treated posttraumatic shoulder stiffness and to report on the implementation of a standardized treatment algorithm. In January 2016, a systematic literature search of Medline ( www.pubmed.com ) was performed to identify studies with cohorts of patients with operatively treated posttraumatic shoulder stiffness. Studies were included according to predefined inclusion and exclusion criteria. In total, nine studies with 191 patients were included. After a mean of 7 months (range: 3.7–12) of conservative treatment with pain medication and physiotherapy, surgery was performed. The mean follow-up was 24 months (range: 6–46). All studies showed an improvement in clinical scores or range of motion (ROM) on follow-up. To our knowledge, no previous systematic review of operatively treated posttraumatic shoulder stiffness has been published. Based on this systematic review, arthroscopic-guided or arthroscopic release of posttraumatic shoulder stiffness is an effective method to relieve pain and gain ROM. A clearly defined time until surgery does not exist and must be determined individually.

Background Due to the high anatomical variability and limited visualization of the scapula, optimal screw placement for baseplate anchorage in reversed total shoulder arthroplasty (rTSA) is challenging. Image quality plays a key role regarding the decision of an appropriate implant position. However, these data a currently missing for rTSA and were investigated in the present study. Furthermore, the rate of required K-wire changes for the central peg as well as post-implantation inclination and version were assessed. Methods In ten consecutive patients (8 female, 86 years, range 74–94) with proximal humeral fracture and indication for rTSA, an intraoperative 3D-scan of the shoulder with a 3D image intensifier (Ziehm Vision FD Vario 3D© [Ziehm Imaging GmbH, Nürnberg, Germany]) was performed after resection of the humeral head. Using the Vectorvision© Software (Brainlab AG, Feldkirchen, Germany), the virtual anatomy was compared to the visible anatomical landmarks. After implantation of the baseplate, a 3D scan was performed. All 3D scans included multiplanar reconstruction (MPR) and the cinemode to examine screw and baseplate placement. The rate of required K-wire changes was assessed. The intraoperative 3D image quality (modified visual analogue scale [VAS] and point system) was assessed before and after implantation of the glenoid component. Inclination and version were determined in post-implantation scans. Results The virtually presented anatomical landmarks always correlated to the anatomical visible points indicating an good virtual accuracy. The central K-wire position was corrected in three cases due to a deviation from the face plane technique position. The VAS was higher for the pre-implantation MPR (6.7, range 5–8) compared to the post-implantation acquired MPR (5.1, range 4–6; p = 0.0002). The point system showed a reduced quality in all subcategories, especially regarding the grading of the articular surfaces. The preoperative (7.9, range 6–9) and post-implantation (7.9, range 6–9) cinemode displayed no significant differences (p = 0.6). Conclusion The present study underlines the need for the improvement of 3D image intensifiers algorithms to reduce artifact associated impaired image quality to enhance the benefit of real-time intraoperative 3D scans and navigation.

Das Diagnosis-related groups(DRG)-System wurde 2004 verpflichtend in Deutschland eingefuhrt. „Case mix“ (CM), „case mix index“ (CMI) und die Fallzahl sind grundlegende Kennzahlen. Sowohl deren saisonale Verteilung und auch die Verteilung von Hauptdiagnosen und DRGs konnten zu logistischen Konsequenzen im Krankenhaus fuhren. Zwischen 2004 und 2013 wurden alle Hauptdiagnosen und DRGs der stationaren Patienten der unfallchirurgischen Klinik des Universitatsklinikums Leipzig erfasst. Die monatliche und saisonale Verteilung wurde mittels ANOVA analysiert. Die durchschnittliche monatliche Fallzahl betrug 265 ± 25, der durchschnittliche CM 388,50 ± 51,75, der CMI 1,46 ± 0,15 jeweils ohne signifikante saisonale Haufung (p > 0,1). Die Gehirnerschutterung war die haufigste Diagnose im beobachteten Zeitraum (3739 Falle) gefolgt von proximalen Humerusfrakturen (699). Signifikante Verteilungsunterschiede konnten fur Humeruskopffrakturen im monatlichen (p = 0,018) sowie jahreszeitlichen Vergleich (p = 0,006) mit einem Maximum im Winter gezeigt werden. Auch Radius- (p = 0,01) und Ausenknochelfrakturen (p ≤ 0,001) treten im Winter am haufigsten auf. Weichteilverletzungen der Schulter sind im Fruhjahr am seltensten (p = 0,04). DRGs weisen keine saisonale Verteilungshaufung auf (p > 0,1). Die signifikante Haufung von Verletzungen in bestimmten Monaten/Jahreszeiten sollte in Bezug auf logistische Konsequenzen in einer Klinik berucksichtigt werden (OP-Kapazitat, Personalanforderung). Zur Erstellung einer Bedarfsanalyse ist die Betrachtung der Diagnosen besser geeignet als die von DRGs.

Reconstruction of the coracoclavicular ligament functions to restore anatomic alignment of the clavicle and may improve biomechanical function and clinical outcomes. Improper placement of the coracoclavicular tunnel may inherently weaken the coracoid. The purpose of this study was to evaluate the feasibility and accuracy of navigated image-free placement of K-wires for coracoclavicular tunnel position in comparison to conventional drill guide-based placement. Eight human shoulder specimens were assigned for conventional technique with a coracoclavicular guide device (group CP) and the paired contralateral side for the navigated procedure (group NP) with an optoelectronic system with a fluoro-free software module. First-pass accuracy (%) and the K-wire trajectory (lateral–center orientation (LC), center–center (CC) orientation and medial–center orientation (MC) were measured. In all navigated K-wires a 100 % first-pass accuracy was observed. In three of the eight (37.5 %) specimens of the drill guide-based group, drilling had to be repeated. One of them had to be repeated twice, resulting in eight versus twelve drillings for the navigated versus conventional group, respectively (p = 0.021). K-wire trajectory showed an MC orientation in most of the specimen (n = 9, group NP 4, group CP 5). Image-free navigated coracoclavicular drilling for the repair of acromioclavicular joint dislocation has higher first-pass accuracy in comparison to conventional drill guide-based placement and, therefore, may enable a precise anatomic position of the drill holes and reduce the risk of an iatrogenic coracoid fracture.

The purpose of this study was to identify the rate of primary screw perforations after osteosynthesis of proximal humerus fractures with intra-operative 3D fluoroscopy and to analyse the rate of secondary screw perforations as well as complications, outcome, and revision surgeries after a minimum of 12 months. Thirty-three patients (20 female, 13 male, median age 67 years, range 35–85 years) with displaced proximal humerus fractures were included. After reduction and fixation, an intra-operative 3D fluoroscopy was performed to evaluate primary screw perforations (PS) and screws with “near perforation” (nPS). These screws were changed intra-operatively. Patients were followed-up for a minimum of 12 months. Clinical and radiological parameters, such as secondary screw perforation, secondary loss of reduction, or functional outcome, were investigated. In six patients (18.2%), humeral head screws were changed due to primary PS (n = 2) or nPS (n = 4) after the intra-operative 3D fluoroscopy. Follow-up revealed an adapted constant score (%CMS) of 76.2% after a mean follow-up of 17.7 months. Two secondary screw perforations were observed (6%). Loss of reduction was observed in eight patients (24.2%). The intra-operative 3D reveals a high rate of primary screw perforations or near perforations. Immediate change of these screws may lead to a lower rate of secondary screw perforations and, therefore, reduce post-operative complications.

Rund 30% aller Halswirbelsaulenverletzungen entfallen auf die obere Halswirbelsaule, wobei mit steigendem Lebensalter die Frakturen des zweiten Halswirbelkorpers prozentual zunehmen: Bezogen auf alle Verletzungen der Halswirbelsaule finden sich bei den unter 60-Jahrigen zu 21% Frakturen des zweiten Halswirbelkorpers, bei den 60- bis 75-Jahrigen zu 28% und bei den uber 75-Jahrigen zu 54% [1]. Auch die Art der Verletzung ist altersabhangig: Bei jungeren Patienten dominieren aufgrund der guten Knochenqualitat die ligamentaren Lasionen, bei alteren uberwiegen die knochernen Verletzungen.

Wie haufig sind ipsilaterale Begleitverletzungen bei proximalen Humerusfrakturen, wo sind sie lokalisiert und welchen Einfluss haben sie auf das klinische Ergebnis? Im Zeitraum von 1/2001 bis 3/2008 wurden in unserer Klinik 641 Patienten mit einer proximalen Humerusfraktur operativ versorgt. Davon bestanden bei 58 Patienten (9 %) Begleitverletzungen an der ipsilateralen oberen Extremitat und des Schultergurtels. Im Rahmen einer retrospektiven Studie konnten 36 Patienten (Altersdurchschnitt: 67 Jahre, 12 mannlich, 24 weiblich) im Mittel 38 Monate nach der Primarversorgung nachuntersucht werden (DASH-Score, Constant-Score). Die Versorgung der 58 proximalen Humerusfrakturen mit Begleitverletzungen erfolgte bei 38 Patienten mittels winkelstabiler Plattenosteosynthese, bei 12 Patienten mit einer Frakturprothese, bei 7 Patienten mit einem Nagel und bei einem Patienten mittels Schraubenosteosynthese. Frakturen des distalen Radius und Nervenverletzungen waren die haufigsten Begleitverletzungen, wobei Frakturen des distalen Radius ausschlieslich bei geringer Sturzenergie auftraten und Nervenverletzungen gehauft bei Luxationsfrakturen. Begleitverletzungen kamen besonders haufig bei Frauen jenseits des 60. Lebensjahres vor, waren bei Mannern aber homogen uber alle Altersstufen verteilt. Die 36 nachuntersuchten Patienten erreichten durchschnittlich 35 Punkte im DASH-Score und 56 Punkte im Constant-Score. Patienten mit einer primar prothetischen Versorgung zeigten ein schlechteres klinisches Ergebnis als Patienten mit einer Osteosynthese. Ipsilaterale Begleitverletzungen in Kombination mit proximalen Humerusfrakturen sind haufig und spiegeln ihren Stellenwert als osteoporotische Fraktur wider. Der Behandlungsalgorithmus sieht bei der uberwiegenden Anzahl eine kombinierte operative Versorgung vor, um die schnellstmogliche Mobilisation zu gewahrleisten. Die zu erwartenden Ergebnisse sind von der Versorgungsart der proximalen Humerusfraktur abhangig.

Hintergrund: Die Behandlung vollschichtiger Knorpeldefekte stellt trotz intensiver Forschungsbemuhungen unverandert eine klinische Herausforderung dar. Die Transplantation autologer osteochondraler Zylinder stellt eine mogliche Therapie dieser Defekte dar. Bekannte Probleme dieser Technik sind unter anderem degenerative Veranderungen der Zylinder und des Umgebungsknorpels sowie eine fehlende Integration zum umgebenden Knorpelgewebe. Langzeitergebnisse sind daher zur qualitativen Methodenbewertung wegweisend, wobei dem subjektiven Ergebnis und der vorhandenen Lebensqualitat eine hohe Bedeutung zukommt. Patienten/Material und Methoden: Wir fuhrten eine longitudinale Untersuchung an 22 Patienten 88 ± 14,5 Monate nach einer autologen osteochondralen Transplantation durch. Bei der Nachuntersuchung erfolgte neben der Bestimmung von klinischen Scores eine Messung der gesundheitsbezogenen Lebensqualitat mit dem SF-36 und dem EQ-5D. Zur radiologischen Beurteilung wurden Rontgenaufnahmen beider Kniegelenke angefertigt und in Bezug auf arthrotische Veranderungen ausgewertet. 21 Patienten erhielten zeitnah zur Nachuntersuchung ein aktuelles MRT des operierten Kniegelenks. Als klinische Kontrollgruppe diente ein nach BMI, Geschlecht und Alter gematchtes Kollektiv von 19 Patienten mit einem fokalen Defekt an der medialen Femurkondyle, welche mit einer Mikrofrakturierung behandelt wurden. Ausschlusskriterien waren eine tibiale Kissing-Lasion, Bandinstabilitat, generalisierte Arthrose sowie Achsfehlstellung. Ergebnisse: Fur die OAT-Patienten betrug der mittlere Punktwert des IKDC-Scores 76,1 ± 18,2. Im Lysholm-Score wurden im Mittel 85,7 ± 16,3 Punkte erreicht. Verglichen mit einem 13,5 Monate nach der Operation erfassten Lysholm-Score ergab sich kein statistischer Unterschied zur jetzigen Nachuntersuchung. In den konventionellen Rontgenaufnahmen zeigte sich eine hohergradige Arthrose im betroffenen medialen Kompartiment, verglichen zur gesunden medialen Gegenseite. Bei der MRT-Auswertung zeigte sich ein mittlerer modifizierter MOCART-Score von 41,2 ± 7,7 fur die OAT-Gruppe und von 39,4 ± 16,1 fur die MF-Gruppe ohne statistisch signifikante Differenz. In der OAT-Gruppe fand sich weiterhin bei allen Patienten eine vollstandige ossare Integration der transplantierten Zylinder. Ein Odem im transplantierten Zylinder zeigte sich zentral gelegen bei 7 Patienten und generalisiert bei 2 weiteren Patienten. Ein Zylinderodem ging mit einer tendenziell hoheren Angabe der Schmerzintensitat auf einer VAS einher. In Bezug auf die Lebensqualitat zeigten OAT-Patienten im korperlichen Summenscore des SF-36 bessere Ergebnisse als die Kontrollgruppe. Im psychischen Summenscore fanden sich keine signifikanten Unterschiede. Schlussfolgerung: Die autologe osteochondrale Transplantation hat unverandert ihren Stellenwert in der Therapie vollschichtiger Knorpeldefekte und bietet zufriedenstellende mittelfristige Ergebnisse, auch wenn eine fortschreitende unikompartimentelle Arthrose nicht zu verhindern scheint. Ein Odem des transplantierten Zylinders geht mit einer verstarkten Schmerzintensitat einher und kann ein indirektes Zeichen einer daruberliegenden Knorpeldegeneration darstellen.

Background Accuracy evaluation of navigated image free placement of double cortical fixation buttons for coracoclavicular tunnel position in comparison to conventional drill guide based placement. Methods Twenty-six patients with acute acromioclavicular joint instability were included in this non-randomized cohort study. All patients were treated with a Double- TightRope technique. In 13 cases the conventional drill guide based placement was used (group 1). In 13 patients surgery was performed as a navigated procedure with a fluoro-free optoelectronic system (group 2). The number of coracoclavicular drillings per patient (First pass accuracy; FPA (%)) was documented, the subcoracoidal position of the fixation buttons has been evaluated and graded as “intended position achieved (IPA)” or “intended position not achieved (IPnA)”. Results In group 1 drilling had to be repeated in four patients (30.8 %) to achieve proper placement of the subcoracoidal fixation buttons. 100 % first pass accuracy was observed in group 2 (p = 0.03). In group 1, the intended position of the subcoracoid buttons was not achieved (IPnA) in six patients (46.2 %). In group 2 all intended positions were achieved (p = 0.005). Conclusion Arthroscopic controlled fluoro-free navigated coracoclavicular drilling for the repair of acromioclavicular joint dislocation has higher first pass accuracy in comparison to conventional drill guide based placement. Therefore the navigation enables a precise position of the drill holes, may reduce the risk of an iatrogenic coracoid fracture and migration of fixation devices. Durch die Erfassung von „diagnosis related groups“ (DRG) und Diagnosen lasst sich deren Verteilung im Jahresverlauf ableiten. Ein gehauftes Auftreten von Erkrankungen zu bestimmten Zeiten kann so identifiziert werden. Fur die Zeit von 2004–2010 erfassten wir 22.293 Hauptdiagnosen und DRG an der unfallchirurgischen Klinik. Verletzungen mit gleicher Lokalisation und Behandlung wurden zusammengefasst. Die haufigsten Verletzungen waren das Schadel-Hirn-Trauma, Wirbelkorper- und Unterschenkelfrakturen. Fur diese zeigte sich keine Haufung im Jahresverlauf. Proximale Humerusfrakturen traten im Winter etwa 25% haufiger auf als im Sommer, Frakturen des oberen Sprunggelenks etwa 33% haufiger. Die Gonarthrose ist im Winter doppelt so oft Behandlungsdiagnose wie im Sommer. Es gilt zu prufen, inwiefern logisitische Folgerungen aus diesen Ergebnissen eine Effizienzsteigerung bewirken konnen. Das zugrunde liegende Analyseverfahren ist generell fur jedes Klinikum anwendbar und stellt ein valides Controlling-Instrument dar.

To analyse the radiogrammetric parameter “cortical index” (CI) and its predictive value for proximal humerus fractures. Furthermore, to investigate the reoperations and the cause of reoperations after locking plate osteosynthesis of displaced proximal humerus fractures. 113 consecutive patients (73 women and 40 men) with a median age of 66 years (range 18–100 years) were included in this study. The median follow-up time in our database was 4.7 years (range 45–72 months). For a comparative matched-group analysis of the CI, patients with a fall on the shoulder without fracture were selected. Demographic data and all reoperations were recorded after median 58 months postoperatively. The CI was measured at the proximal humeral diaphysis. The CI showed to be significant lower in the fracture group (mean 0.28) when compared to the matched group (mean 0.47, p
