1. Uterine and ovarian remnants in an incorrectly spayed bitch: a case report

Abstract

A spayed Samoyed bitch, 12 years old, was presented to the Veterinary Clinical Hospital of the University of Cordoba (Spain) with abundant vulvar sanguineous discharge over the previous three days. The clinical examination revealed a remarkable vulvar mass, which protruded through the vulvar lips. Abdominal ultrasonography revealed the presence of structures compatible with uterus and ovary, which had been presumably removed eight years previously. An exploratory laparotomy was carried out, which confirmed the presence of the right ovary and a remnant of the uterus. The histological evaluation confirmed a granulosa cell tumour in the ovary, and an enlarged portion of the right uterine horn with brownish contents. The vulvar mass was also surgically removed and fibroma with some fibrosarcoma areas was diagnosed. This case shows the evolution of ovary and uterus into the abdomen, which were incorrectly removed after ovariohysterectomy eight years previously.

Abstract

Solid biomass is often mixed with coal for cofiring in power plants to reduce greenhouse gases emissions. The resulting powder mixtures can give rise to handling and feeding problems due to the occurrence of intermittent flow or blockage. In this paper the flow properties of mixtures of a coal-petcoke powder and two types of biomass materials, dried olive husk (OH) and grape seed meal (GSM), were studied. The most significant effects on the cohesive properties of some powder mixtures were found for low biomass content mixtures due to their higher fraction of fine particles.

Abstract

This article focuses on the aerodynamic design of a morphing aerofoil at cruise conditions using computational fluid dynamics (CFD). The morphing aerofoil has been analysed at a Mach number of 0.8 and Reynolds number of $3 \times 10^{6}$ , which represents the transonic cruise speed of a commercial aircraft. In this research, the NACA0012 aerofoil has been identified as the baseline aerofoil where the analysis has been performed under steady conditions at a range of angles of attack between $0^{^{\kern1pt\circ}}$ and $3.86^{^{\kern1pt\circ}}$ . The performance of the baseline case has been compared to the morphing aerofoil for different morphing deflections ( $w_{te}/c = [0.005 - 0.1]$ ) and start of the morphing locations ( $x_{s}/c = [0.65 - 0.80]$ ). Further, the location of the shock wave on the upper surface has also been investigated due to concerns about the structural integrity of the morphing part of the aerofoil. Based upon this investigation, a most favourable morphed geometry has been presented that offers both, a significant increase in the lift-to-drag ratio against its un-morphed counterpart and has a shock location upstream of the start of the morphing part.

Abstract

This document presents the design of a fuzzy control system applying photoplethysmography techniques to generate input data and Mamdani-type fuzzy logic for its processing and generation of system output values, through which blood pressure levels can be indicated based on the parameters recommended by the medical area for an adult person. The integral design of the system includes the acquisition of the input signals, obtained through the reflection photoplethysmography technique, by means the optical coupling between IR light-emitting diodes and photo receptors, which allow the flow of current proportional to the amount of reflected light by blood, which is related to the flow level of the cardiac cavities, derived from the systolic and diastolic stages of the cardiac cycle. The fuzzy control was designed with two inputs and one output of 5 triangular membership functions each using the hardware description language (VHDL), the fuzzification, inference, aggregation and defuzzification stages were designed using the COSAA method. The response values ​​obtained using the Test Bench tool of ISE Design suite were compared with theoretical values ​​and with values ​​obtained with the Fuzzy Logic Designer tool of Matlab, the results showed an excellent approximation. &nbsp

Abstract

Background The risk of femoral neck fracture progressively increases with age. However, the reasons behind this consistent increase in the fracture risk can't be completely justified by the decrease in the bone mineral density. The objective of this study was to analyze the correlation between various bone structural features and age. Study Design & Methods A total of 29 consecutive patients who suffered an intracapsular hip fracture and underwent joint replacement surgery between May 2012 and March 2013 were included in this study. A 2 cm × 1 cm O cylindrical trabecular bone sample was collected from the femoral heads and preserved in formaldehyde. Bone mineral density (BMD), microarchitecture, organic content and crystallography were analyzed using a Dual-energy X-ray absorptiometry scan, micro-CT scan, and high resolution magic-angle-spinning-nuclear magnetic resonance (MAS-NMR), respectively. Statistical correlations were made using Spearman´s or Pearson´s correlation tests depending on the distribution of the continuous variables. Results The mean patient age was 79.83 ± 9.31 years. A moderate negative correlation was observed between age and the hydrogen content in bone (1H), which is an indirect estimate to quantify the organic matrix (r = −0.512, p = 0.005). No correlations were observed between BMD, trabecular number, trabecular thickness, phosphorous content, apatite crystal size, and age (r = 0.06, p = 0.755; r = −0.008, p = 0.967; r = −0.046, p = 0.812; r = −0.152, p = 0.430, respectively). A weak positive correlation was observed between Charlson´s comorbidity index (CCI) and c-axis of the hydroxiapatite (HA) crystals (r = −0.400, p = 0.035). Conclusion The femoral head relative protein content progressively decreases with age. BMD was not correlated with other structural bone parameters and age. Patients with higher comorbidity scores had larger HA crystals. The present results suggest that the progressive increase in the hip fracture risk in elderly patients could be partially explained by the lower bone protein content in this age group.

Abstract

ABSTRACT: The objective was to compare pregnancy per service event (P/S) in lactating dairy cows following timed artificial insemination (AI) or timed embryo transfer (ET) using either fresh or frozen in vitro–produced embryos. Oocytes were collected once per week for up to 9 wk using transvaginal ovum pick-up from elite dairy donors (ET-DAIRY; n = 40; Holstein-Friesian and Jersey) and elite beef donors (ET-ELITE-BEEF; n = 21; Angus). Both ET-DAIRY and ET-ELITE-BEEF donors consisted of heifers and cows. In addition, oocytes were collected from the ovaries of beef heifers of known pedigree following slaughter at a commercial abattoir (ET-COMM-BEEF; n = 119). Following in vitro maturation and fertilization, presumptive zygotes were cultured in vitro to the blastocyst stage. Grade 1 blastocysts were either transferred fresh or frozen for on-farm thawing and direct transfer. A total of 1,106 recipient cows (all lactating, predominantly Holstein-Friesian) located on 16 herdlets were blocked based on parity, calving date, and Economic Breeding Index, and randomly assigned to receive AI (n = 243) or ET (n = 863) after estrous synchronization with a 10-d Progesterone-synch protocol. Cows assigned to ET were further randomized to receive fresh (n = 187) or frozen (n = 178) ET-ELITE-BEEF embryos, fresh (n = 169) or frozen (n = 162) ET-DAIRY embryos, or fresh (n = 80) or frozen (n = 87) ET-COMM-BEEF embryos. Pregnancy was diagnosed using transrectal ultrasound on d 32 to 35 after synchronized ovulation and confirmed on d 62 to 65, at which time fetal sex was determined. Pregnancy per service event at d 32 was not different between AI (48.8%) and ET (48.9%) and did not differ between dairy and beef embryos (50.3% vs. 48.1%, respectively). However, P/S was less on d 32 following transfer of frozen embryos (41.6%) compared with fresh embryos (56.1%). Pregnancy loss between d 32 and 62 was greater for ET (15.1%) compared with AI (4.7%), with greater losses observed for frozen beef (18.5%), fresh beef (17.3%), and frozen dairy (19.2%) compared with fresh dairy (6.0%) embryos. Serum progesterone (P4) concentration on d 7 was associated with P/S at d 32 and 62. Cows in the quartile with the least serum P4 concentrations (quartile 1) had less probability of being pregnant on d 32 (33.4%) compared with cows in the 3 upper quartiles for serum P4 (45.7%, 55.6%, and 61.2% for quartile 2, quartile 3, and quartile 4, respectively). Sex ratio (male:female) at d 62 was skewed toward more male fetuses following ET (61.1:38.9) compared with AI (43.2:56.8) and was consistent with the sex ratio among in vitro blastocysts (61.2:38.8). In conclusion, P/S was similar for AI and ET, although pregnancy loss between d 32 and 62 was greater for ET than for AI.

Abstract

International audience; The export of organic carbon export by the rivers to the oceans either as particulate organic carbon (POC) or dissolved organic carbon (DOC) is very sensitive to climate change especially in permafrost affected catchments where soils are very rich in organic carbon. With global warming, organic carbon export in both forms is expected to increase in Arctic regions. It should affect contemporary biogeochemical cycles in rivers and oceans and therefore modify the whole food web. This study tries to understand complex processes involved in sediment, POC and DOC riverine transport in the Yenisei River basin and to quantify their respective fluxes at the river outlet. The SWAT (Soil and Water Assessment Tool) hydrological model is used in this study to simulate water and suspended sediment transfers in the largest Arctic river. POC and DOC export have been quantified with empirical models, adapted from literature for the study case. First, the hydrological model has been calibrated and validated at a daily time step for the 2003–2008 and the 2009–2016 periods respectively, and its output has been compared with field data for water and sediment fluxes. Based on conceptualization of transfer processes, calibration on climate and soil properties has been performed in order to correctly represent hydrology and sediment transfer in permafrost basins. Second, calibration of empirical models for DOC/POC transport have been performed by comparing their output with field data, available from 2003 to 2016. Our study reveals that SWAT is capable of correctly representing hydrology, sediment transfer, POC and DOC fluxes and their spatial distribution at a daily timescale, and outlines the links between these fluxes and permafrost features. Our simulation effort results in specific sediment, POC and DOC fluxes of 2.97 t km−2 yr−1, 0.13 t km−2 yr−1 and 1.14 t km−2 yr−1 for the period 2003–2016 which are in the range of previous estimates. About 60% of the total fluxes of sediment, DOC and POC to the Arctic Ocean are exported during the two months of the freshet. Spatial analysis show that permafrost-free areas have returned higher daily organic carbon export than permafrost affected zones, highlighting the thawing permafrost effect on carbon cycle in climate change feedback.

Abstract

The primary cause of death worldwide is heart disease and the most common type of heart disease is coronary artery disease. While coronary artery disease is treated with medications, it responds to lifestyle interventions. A low-fat plant-based diet was designed for reversing coronary artery disease and it is effective in reversing the disease. It has not been tested, however, as far as we know, whether diets with customary levels of fat can also reverse coronary artery disease. Nevertheless, evidence is accumulating to show that atherosclerosis and coronary artery disease are reversed with diets containing customary levels of fat. It has been known that fats of plant origin decrease the risk factors of cardiovascular disease. It is also known that vegans who consume diets with customary levels of fat have the lowest risk of cardiovascular disease. But recent and more specific data show that atherosclerosis was decreased when nuts that are rich in fat were added to a Mediterranean diet while atherosclerosis was increased in the controls. Also, two clinical cases show that coronary artery disease was reversed by low-fat plant-based diets that were supplemented with fat-rich foods of plant origin. These data, then, provide evidence that coronary artery disease may be reversed with a diet containing customary levels of fat from plant sources. We hypothesize that coronary artery disease may be reversed by diets with customary levels of fat of plant origin that are low in saturated fat content. This hypothesis needs to be tested by comparing a traditional low-fat plant-based diet with a plant-based diet containing customary levels of fat of plant origin in their effectiveness to reverse coronary artery disease.

Abstract

C/N ratios in TiCN-TiC-WC-Cr3C2-Ni cermets can be intentionally modified by changing the composition of the presintering atmosphere. This affects not only the sinterability but also the final microstructure of these materials. Melting occurs at lower temperatures as the nitrogen content of the presintering atmosphere rises. However, the final densities are lower due to the precipitation of free carbon. Grain growth of the carbonitride phase is accelerated in samples presintered in N2 due to the activation of solution reprecipitation kinetics. Finally, the precipitation of the Cr-rich M7C3 carbide can also be controlled by the appropriate selection of the presintering atmosphere up to 7.8 wt.% Cr contents.

Abstract

Background & Aim Induced pluripotent stem cells (iPSC) offers a renewable source for reliably and robustly generating off-the-shelf cell products for regenerative medicine. A number of differentiation protocols have been developed and PSC-derived cells have been used in a variety of animal models to treat disease and progressed through to clinical trials in Asia and more recently the United States. However, the manufacturing processes for expansion and differentiation of iPSC cells remain reliant on poorly scalable technologies with limited in process monitoring despite typically > 14 days of culture and manipulation required; increasing the risks and costs of batch manufacturing failure. The industrialisation of pluripotent stem cell-derived cell therapies requires the development of process analytical technologies (PAT) for the monitoring and control of large-scale cell differentiation bioprocesses. The extracellular microenvironment provides fertile hunting ground to determine the impact of processing conditions on process outcome with the myriad of molecules consumed and produced by cells during differentiation alluding to cell state and potential cell fate. PAT that targets the spent media enables control of critical processing parameters to maintain product critical quality attributes without requiring cell-based analysis to be conducted. Methods, Results & Conclusion Using two exemplar differentiation processes, going from induced pluripotent stem cells to, either, definitive endoderm or cardiomyocytes, we have characterised the cell secretome with respect to metabolites and secreted proteins, and performed multi-parametric analysis to determine potential biomarkers for monitoring future large-scale differentiation processes using cost- and time-effective PAT alternatives.

Abstract

The production of a Z boson, decaying to two charged leptons, in association with jets in proton-proton collisions at a centre-of-mass energy of 13TeV is measured. Data recorded with the CMS detector at the LHC are used that correspond to an integrated luminosity of 2.19fb-1. The cross section is measured as a function of the jet multiplicity and its dependence on the transverse momentum of the Z boson, the jet kinematic variables (transverse momentum and rapidity), the scalar sum of the jet momenta, which quantifies the hadronic activity, and the balance in transverse momentum between the reconstructed jet recoil and the Z boson. The measurements are compared with predictions from four different calculations. The first two merge matrix elements with different parton multiplicities in the final state and parton showering, one of which includes one-loop corrections. The third is a fixed-order calculation with next-to-next-to-leading order accuracy for the process with a Z boson and one parton in the final state. The fourth combines the fully differential next-to-next-to-leading order calculation of the process with no parton in the final state with next-to-next-to-leading logarithm resummation and parton showering. © 2018, CERN for the benefit of the CMS collaboration.

Abstract

Background Derived neutrophils/(leukocytes-neutrophils) ratio (dNLR) and lactate dehydrogenase (LDH) level have been correlated with immune checkpoint inhibitors (ICI) outcomes. A lung immune prognostic index (LIPI) that showed association with ICI outcomes was developed by Mezquita L et al. based on these 2 systemic inflammation indicators (dNLR upper limit of normal (ULN)), characterizing 3 prognostic groups: good, 0 factors; intermediate, 1 factor; poor, 2 factors. This index hasn’t been specifically studied in first-line setting of NSCLC with pembrolizumab monotherapy. Methods This is a multicenter retrospective study to explore the prognostic value of LIPI score in treatment-naive advanced NSCLC with high PD-L1 expression ( > =50%) treated with pembrolizumab. Consecutive patients treated in 19 Spanish hospitals between March 2015 and April 2019 were included. Pretreatment LIPI score was calculated for all subjects and primary endpoint was OS. Results 223 patients were included. Mean age 67 years (SD 9.8). 77.6% were male and 75% PS Conclusions LIPI score was able to define a group of patients with poor benefit from pembrolizumab monotherapy in such a selected population (advanced NSCLC with high PD-L1 expression) with high probability to get benefit from it according to KN-024. Abstract

BACKGROUND: Data regarding response to treatment in lymphomatoid papulosis (LyP) are scarce. AIM: To assess the daily clinical practice approach to LyP and the response to first-line treatments. METHODS: This was a retrospective study enrolling 252 patients with LyP. RESULTS: Topical steroids, methotrexate and phototherapy were the most common first-line treatments, prescribed for 35%, 20% and 14% of the patients, respectively. Complete response (CR) was achieved in 48% of treated patients. Eczematous lesions significantly increased relative risk (RR) of not achieving CR (RR = 1.76; 95% CI 1.16-2.11). Overall median time to CR was 10 months (95% CI 6-13 months), and 78% of complete responders showed cutaneous relapse; both results were similar for all treatment groups (P > 0.05). Abstract

The term 'sclerosing diseases of the skin' comprises specific dermatological entities, which have fibrotic changes of the skin in common. These diseases mostly manifest in different clinical subtypes according to cutaneous and extracutaneous involvement and can sometimes be difficult to distinguish from each other. The present guideline focuses on characteristic clinical and histopathological features, diagnostic scores and the serum autoantibodies most useful for differential diagnosis. In addition, current strategies in the first-and advanced-line therapy of sclerosing skin diseases are addressed in detail. Part 1 of this guideline provides clinicians with an overview of the diagnosis and treatment of localized scleroderma (morphea), and systemic sclerosis including overlap syndromes of systemic sclerosis with diseases of the rheumatological spectrum.

Abstract

A search is presented for an excess of events with heavy-flavor quark pairs (tt¯ and bb¯) and a large imbalance in transverse momentum in data from proton–proton collisions at a center-of-mass energy of 13TeV. The data correspond to an integrated luminosity of 2.2fb-1 collected with the CMS detector at the CERN LHC. No deviations are observed with respect to standard model predictions. The results are used in the first interpretation of dark matter production in tt¯ and bb¯ final states in a simplified model. This analysis is also the first to perform a statistical combination of searches for dark matter produced with different heavy-flavor final states. The combination provides exclusions that are stronger than those achieved with individual heavy-flavor final states. © 2017, CERN for the benefit of the CMS collaboration.

Abstract

The CMS tracker consists of 206 m2 of silicon strip sensors assembled on carbon fibre composite structures and is designed for operation in the temperature range from -25 to +25°C. The mechanical stability of tracker components during physics operation was monitored with a few μm resolution using a dedicated laser alignment system as well as particle tracks from cosmic rays and hadron-hadron collisions. During the LHC operational period of 2011-2013 at stable temperatures, the components of the tracker were observed to experience relative movements of less than 30μm. In addition, temperature variations were found to cause displacements of tracker structures of about 2μm°C, which largely revert to their initial positions when the temperature is restored to its original value. © CERN 2017 for the benefit of the CMS collaboration..

Abstract

The CMS apparatus was identified, a few years before the start of the LHC operation at CERN, to feature properties well suited to particle-flow (PF) reconstruction: a highly-segmented tracker, a fine-grained electromagnetic calorimeter, a hermetic hadron calorimeter, a strong magnetic field, and an excellent muon spectrometer. A fully-fledged PF reconstruction algorithm tuned to the CMS detector was therefore developed and has been consistently used in physics analyses for the first time at a hadron collider. For each collision, the comprehensive list of final-state particles identified and reconstructed by the algorithm provides a global event description that leads to unprecedented CMS performance for jet and hadronic τ decay reconstruction, missing transverse momentum determination, and electron and muon identification. This approach also allows particles from pileup interactions to be identified and enables efficient pileup mitigation methods. The data collected by CMS at a centre-of-mass energy of 8show excellent agreement with the simulation and confirm the superior PF performance at least up to an average of 20 pileup interactions. © 2017 CERN.

Abstract

Resumen Objetivo Investigar los mecanismos moleculares de respuesta tisular tras el tratamiento con la tecnica Electrolisis Percutanea Intratisular (EPI®) en la tendinosis inducida por colagenasa tipo i en ratas Sprague Dawley. Metodos En una muestra de 24 ratas Sprague Dawley de 7 meses de edad y 300 g se indujo tendinosis mediante la inyeccion en el tendon rotuliano de 50 μg de colagenasa tipo i . Se procedio a dividir la muestra en 4 grupos: un grupo control, un grupo colagenasa y 2 grupos de tratamiento con tecnica EPI® a 3 y 6 mA, respectivamente. Se aplico una sesion de tratamiento EPI® y tras 3 dias se procedio al analisis de los tendones mediante tecnicas de inmunodeteccion y electroforesis. Se analizaron las proteinas citocromo C, Smac/Diablo, factor de crecimiento endotelial vascular y su receptor 2. Tambien se analizo el factor de transcripcion nuclear peroxisoma proliferador activado del receptor gamma. Resultados Se observo un aumento estadisticamente significativo en la expresion del citocromo C, Smac/Diablo, factor de crecimiento endotelial vascular, su receptor 2 y peroxisoma proliferador activado del receptor gamma en los grupos a los que se les aplico la tecnica EPI® respecto al grupo control. Conclusiones La tecnica EPI® produce, en la lesion tendinosa inducida con colagenasa tipo i en ratas, un aumento de los mecanismos moleculares antiinflamatorios y angiogenicos.

Abstract

Objective To investigate the molecular mechanisms of tissue response after treatment with the Intratissue Percutaneous Electrolysis (EPI®) technique in collagenase-induced tendinopathy in Sprague-Dawley rats. Methods Tendinopathy was induced by injecting 50 μg of type i collagenase into the patellar tendon of 24 Sprague Dawley rats of 7 months of age and weighting 300 g. The sample was divided into 4 groups: the control group, collagenase group, and two EPI® technique treatment groups of 3 and 6 mA, respectively. An EPI® treatment session was applied, and after 3 days, the tendons were analyzed using immunoblotting and electrophoresis techniques. An analysis was also made of cytochrome C protein, Smac/Diablo, vascular endothelial growth factor and its receptor 2, as well as the nuclear transcription factor peroxisome proliferator-activated receptor gamma. Results A statistically significant increase, compared to the control group, was observed in the expression of cytochrome C, Smac/Diablo, vascular endothelial growth factor, its receptor 2 and peroxisome proliferator-activated receptor gamma in the groups in which the EPI® technique was applied. Conclusions EPI® technique produces an increase in anti-inflammatory and angiogenic molecular mechanisms in collagenase-induced tendon injury in rats.

Abstract

Background Post-operative platinum-based chemotherapy is the standard of care for resected NSCLC with nodal involvement. Expression of genes involved in DNA repair may have a prognostic role for the outcome. BRCA1 plays an important role in DNA repair pathways and could have a prognostic impact in this setting. The SCAT trial results found that for low BRCA1 levels subgroup Cis-Gem was superior to Cis-Doc and in high BRCA1 subgroup docetaxel single agent without platinum achieved similar survival to Cis-Doc. Risk of recurrence analysis according BRCA-1 levels has been performed. Methods From Jun/2007 to May/2013 591 patients were screened and 500 were included, 108 in the control arm treated with Cis-Docetaxel and 392 in the experimental arms treated with Cis-Gem, Cis-Doc or docetaxel alone according to terciles of BRCA1 expression level. With a cut-off September 30th 2018 and a median follow-up of 60 months, recurrence patterns were analysed for the whole group according BRCA1 levels in tumor tissue and comparison were made for risk of recurrence, single/multiple recurrence, thoracic/extrathoracic and metastatic sites (liver, bone, brain). Results Cumulative recurrence was evaluable in 232/456 patients (50.8%), 182/354 patients treated in the experimental arm and in 50/102 patients treated in the control arm (RR 1.04; 0.83-1.30) (p = 0.672). The majority of recurrences 159/232 (68.5%) were single site intrathoracic recurrences in 121/232 (52%) while 111/232 were extrathoracic (47.8%). Overall recurrence was 56.5% (113/200 p) for low tercile BRCA1 vs 48.8% (63/129) for intermediate tercile vs 44% (56/127) for high tercile (p = 0.025). No differences were seen between tercile groups for single site (p = 0.35), multiple site (p = 0.26), intrathoracic (p = 0.36), or extrathoracic (p = 0.38). More frequent distant metastatic sites were: bone (42 patients), brain (38 patients) and liver (11 patients). RIsk reduction was seen for brain metastases in patients with higher tercile BRCA1 (p = 0.003). Conclusions For NSCLC resected patients with lymph node involvement risk of recurrence remains high with a cumulative rate > 50%. Relative risk of recurrence was lower for tumors with higher BRCA1 levels. Distant metastases were seen in 47.8% of patients. Brain metastases risk was significant lower for patients with low BRCA1 expression. BRCA1 levels acts as a prognostic factor in early stages NSCLC. Abstract

Background Lung cancer is the leading cause of cancer mortality worldwide with more than 2 million new cases and 1.8 million deaths in 2018. The TTR could be of use to objectively understand the clinical reality of lung cancer in Spain. Methods Observational cohort multicenter study performed in Spain. Enrollment took place between Aug’16 and Dec’18. Evaluation included a review of demographic data. Results 6600 patients in 56 Spanish hospitals were included. Median age at diagnosis was 64 in advanced disease and 65 in early-stage. The majority of patients presented with advanced, stage III (n = 1874, 28.4%) or IV (n = 3446, 52.2%) at diagnosis, independently of gender (63.1% in males; 70.6% in females). There was a higher number of patients with no comorbidities at diagnosis in advanced disease than in early-stage (63.4% vs. 36.6%, p Conclusions The RTT provides information on comorbidities, risk associations, personal and family history of cancer necessary for the knowledge of lung cancer in real life, which could even be useful when planning the analysis of national health policies. Abstract

Background Immunotherapy is a new and effective form of therapy in several types of cancer, although molecular predictors of response are limited. Recently, we have described that immune-related adverse-events (irAEs) are significantly associated with immunotherapy efficacy, and it has been suggested that body mass index (BMI) might be a favourable factor. We have studied whether excess weight influences the efficacy outcomes of immunotherapy and whether it is associated with immune-related adverse events. We have also evaluated the combined effect of overweight and irAEs. Methods We calculated BMI in patients treated with single-agent anti-PD-1 antibodies for advanced cancer. Efficacy of anti-PD-1 treatment was evaluated with both objective response (OR) rate, and progression-free survival (PFS), and toxicity with irAEs. We established the association between overweight and OR, PFS and irAEs. Results One hundred and thirty-two patients were included in the study. Primary malignancies were lung cancer (n = 93), melanoma (n = 12), head and neck carcinoma (n = 9), renal carcinoma (n = 6), urothelial carcinoma (n = 4), Hodgkin’s lymphoma (n = 3), and other cancers (n = 5). Median BMI was 24.9 kg/m2, and 64 patients (48.4%) were excess weight (BMI≥25 kg/m2). An OR was achieved in 50 patients (38.0%), and median PFS was 6 months. IrAEs occurred in 44 patients (33.3%). OR was significantly higher in excess weight patients that in patients with BMI Conclusions Excess weight patients with advanced cancer that receive single-agent anti-PD-1 antibody therapy exhibit a significantly improved clinical outcome compared with patients with normal BMI. Abstract

Background CT-RT is the standard treatment for unresectable LA-NSCLC. P plus vinorelbine is widely used. Metronomic CT is a frequent administration of low doses of CT. mOV has shown good efficacy and improved safety, and could improve the RT effect. Our goal is to evaluate the efficacy and safety of P-mOV with radical RT in patients (pts) with LA-NSCLC. Methods Pts aged 18-75 years with histologically proven untreated and unresectable LA-NSCLC, adequate bone marrow, hepatic & renal function, ECOG PS0-1, received P 80mg/m2 D1 every 3 weeks combined with mOV 50mg/day on days D1, 3 & 5/weekly, 2 cycles (cy) as induction; patients without progression received 2 more cy of P at the same dose with mOV 30mg/day on D1, 3 & 5/weekly, concurrently with RT (66Gy in 6.5weeks). Primary endpoint was progression-free survival (PFS) by RECIST v1.1; secondary endpoints were: overall response rate (ORR), disease control rate (DCR), overall survival and safety profile. To guarantee an overall type-1 α error no greater than 0.05 and a type II (β) error 0.1 for PFS, a sample size of 67 pts was planned. Results Sixty-seven pts were recruited in 17 Spanish sites from 04/2016 to 06/2017. One of them didn’t meet all the inclusion criteria. We analyzed 66 pts included. Pt characteristics: Male 77.3%; median age 62 (range 33-75); PS 0/1 52/49%; smokers 53%; adenocarcinoma/squamous 43.9/42.4%; stage IIIA/B 42.4/57.6%. Only 32.3% of pts presented any grade 3-4 adverse event, including: neutropenia 20.0%; anemia 4.6%; febrile neutropenia 6.2%; esophagitis 3.1%; pneumonitis 1.5%. There were two deaths non-related to the treatment, during this period. Fifty-one pts have completed the treatment. ORR: 67.7%. DCR: 84.6%. With a median follow-up of 22.3 months (range 1.1-34.9), the median PFS is 11.5 months (CI95%; 9.9-15,4). Conclusions mOV-P with RT is as effective as the standard administration of vinorelbine, improving its safety profile. Clinical trial identification EudraCT 2015-003312-21. Abstract

Background Pembrolizumab monotherapy improved overall survival (OS), progression free survival (PFS) and response rate compared to chemotherapy in patients with treatment-naive advanced NSCLC with high PD-L1 expression ( > =50%), but we see in daily clinical practice that not all patients in this subgroup benefit equally. Prognostic and predictive factors and tools are needed. Methods Multicentric retrospective review of advanced NSCLC patients with high PD-L1 treated with frontline pembrolizumab from March 2015 to April 2019 was performed in 19 Spanish hospitals. We analyzed the prognostic value of different clinical variables and Lung Immune Prognostic Index (LIPI), and selected those who were prognostic in the univariate analysis. Multivariate Cox regression models were adjusted to evaluate the adequacy of models, C-index was used (values over 0.7 indicate a good model, and values over 0.8 indicate a strong model. Results 223 patients were included. Mean age 67 years (SD 9.8). 77.6% were male and 75% PS =2 metastatic locations (HR 2,74;p Conclusions LIPI-FAMACE model has a good capability for predicting survival in patients with advanced NSCLC and high PD-L1 expression treated with frontline pembrolizumab. (2010) European Journal of Cancer Care Management of chylous fistula as a complication of neck dissection: a 10-year retrospective review Chylous fistula is a serious complication of neck surgery. The aim of this study was to analyse the incidence, treatment and evolution of chylous fistula in neck dissection. We conducted a retrospective study of 304 patients, 295 (97.03%) men and nine (2.97%) women. Ages ranged from 24 to 80 years (mean = 59.28 years, SD = 6.02) and they had all undergone neck dissection. Chylous fistula occurred in four cases (1.31%). Incidence was 1.83% in laryngeal cancer and 2.7% in oral cavity and oropharyngeal cancer. No statistically significant correlation was found between tumoral stage and fistula occurrence. Radiotherapy prior to surgery was a risk factor although the association was not statistically significant. The incidence rates for radical and functional neck dissection were 3.3% and 0.46%, respectively, statistically significant (P = 0.042). The fistulas were located on the left side in all cases. One of the four patients required surgical intervention and another one died. The occurrence of chylous fistula increased significantly the length of hospital stay (P = 0.01). Chylous fistulas appear on the left side, radiotherapy prior to surgery is a risk factor and there is not correlation with tumoral stage. Chylous fistulas are significantly more common in radical than in functional dissections and increase significantly the length of hospital stay.

Abstract

Sentinel lymph node biopsy (SLNB) is a technique that facilitates the study of metastasis in oral and oropharyngeal cancer and reduces morbidity in patients affected by this pathology.Oral and oropharyngeal cancers are common and are frequently associated with spread to cervical lymph nodes. We attempted to confirm the usefulness of SLNB in patients suffering from squamous cell carcinoma of the oral cavity as a prognostic technique for lymph node prediction of micrometastatic invasion. Using surgical parameters, we compared its relevance against a retrospective group without SLNB.We studied a prospective group of 22 patients, 21 men and 1 woman, aged 40-83 years (mean=57.77, SD=13) in which SLNB was performed and a retrospective group of 22 patients, 19 men and 3 women, aged 34-76 years (mean=52.68, SD=11) in which SLNB was not performed. Both groups presented oral cavity tumors at clinical stages T1-T3 and radiological stage N0. SLNB consisted of preoperative lymphoscintigraphy (LG), intraoperative detection of the sentinel lymph node (SLN), and a histopathological examination following the recommendations of the Association of Directors of Anatomic and Surgical Pathology (ADASP) and the Canniesburn protocol.We observed SLNs in 21 of 22 patients, with a total of 32 SLNs detected at cervical level II (65.63%), I (21.87%), and III (12.5%). SLNB displayed a sensitivity of 73% (confidence interval (CI)=0.51-0.99) and 100% specificity (CI=1). Negative and positive predictive values were 80% and 100%, respectively. SLNB significantly reduced the incidence of radical neck dissection (p=0.003), the need for recovery/resuscitation beds after surgery (p=0.002), surgical complications (p=0.034), and length of hospital stay (p=0.01).
