1. Cardiometabolic health after first pregnancy: Associations with social determinants of health. A nuMoM2b-HHS study

Abstract

Study objective: This study sought to evaluate the associations between social determinants of health (SDOH) at the time of first pregnancy and subsequent cardiometabolic health, defined as the development of metabolic syndrome. Design: nuMoM2b-HHS (Nulliparous Pregnancy Outcomes Study- Monitoring Mothers-to-Be-Heart Health Study) is an ongoing prospective cohort study. Setting: Eight academic medical centers enrolled and continue to follow participants. Participants: 4484 participants followed a mean of 3.2 years from the time of their first pregnancy. Interventions: N/a. Main outcome measure: Unadjusted and adjusted Poisson regression models with robust standard errors were used to obtain relative risks and 95% confidence intervals estimating the risk of metabolic syndrome for each baseline SDOH. In secondary analyses we examined the associations between SDOH and incident hypertension, obesity, and diabetes mellitus. Results: Metabolic syndrome developed in 13.6% of participants. Higher socioeconomic position at the time of pregnancy was associated with lower rates of metabolic syndrome [income > 200% poverty level aRR 0.55 (95% CI, 0.42–0.71), attainment of a bachelor's degree aRR 0.62 (0.46–0.84) or higher aRR 0.50 (0.35–0.71)], while being single [aRR 1.45 (95% CI, 1.18–1.77)] and having low health literacy were associated with a greater risk of metabolic syndrome [aRR 1.98 (95% CI, 1.28–3.07)]. Conclusions: Over a short interval following first pregnancy, participants accumulated high proportions of cardiovascular risk factors and metabolic syndrome, with some risk associated with SDOH. The impact of interventions addressing SDOH in pregnant people on cardiometabolic health should be tested as a means of reducing health inequities at the population level.

2. Study for the Reconfiguration of Cooling Water Networks in Retrofit Scenarios

Abstract

Conventional cooling water systems are typically based on once-though usage of cooling water for each cooler. Cooling water between coolers can be re-used, which allows reducing overall cooling water flowrate at the increased cooling water return temperature (Kim and Smith, 2001). This reuse concept for cooling water can provide new debottlenecking options by effectively utilizing existing cooling tower. However, care must be taken for the implementation of cooling water reuse in retrofit scenarios, as temperature difference between process streams and reused cooling water is likely to be decreased and hence feasibility of heat transfer for coolers may not be sustained. This study investigates impacts resulted from the reconfiguration of cooling water network when cooling water reuse is introduced for debottlenecking of existing cooling water systems. Conceptual design guidelines are provided to regain driving force of heat transfer for coolers by increasing cooling water flowrate for the cooler. Case study is given to illustrate how cooling water network should be reconfigured for retrofit cases in which overall cooling water flowrate is considerably reduced without compensating performance of coolers.

Abstract

This study was conducted to evaluate the effect of dietary standardized ileal digestible lysine (SID Lys) and copper (Cu) density on growth performance, nutrient digestibility, blood profiles, fecal microbiota, backfat thickness and lean meat percentage in growing pigs. A total of 240 castrated male growing pigs [(Yorkshire × Landrace) × Duroc)] with an average BW of 22.7 ± 0.11 kg were used in this 42-day trial. Pigs were randomly allocated to 3 treatments plus control based on the BW. Basal diet contained 100% SID Lys of the estimated NRC (2012) requirement and 17 mg/kg Cu. Dietary treatments included different SID Lys of the estimated NRC (2012) requirement (100, 103 and 106%) with an extra 150 mg/kg supplemental Cu. Abstract

PURPOSE/OBJECTIVE(S) Definitive radiation therapy (RT) is a curative treatment modality for gastric MALT. Reducing radiation dose to organs at risk (OAR) is imperative for patients with curative disease and excellent long-term prognosis. Advanced RT planning with DIBH and/or IMRT is available to improve the therapeutic ratio of RT by minimizing dose to normal tissues while maintaining adequate target coverage. We aimed to compare dosimetric parameters when using different radiation planning and delivery techniques in patients with gastric MALT. MATERIALS/METHODS After institutional review board approval, we identified adult patients (age ≥ 18 years) with biopsy-proven gastric MALT who were treated at our institution from 2010 to 2020. Each patient underwent two simulation CT scans: free breathing (FB) and DIBH. Four plans were generated for each patient including DIBH-IMRT, DIBH-3DCRT, FB-IMRT, and FB-3DCRT with a prescribed dose of 30 Gy in 20 fractions. The CTV was defined as the entire stomach including the gastroesophageal junction to the pylorus. The PTV included a 1 to 1.5 cm expansion from the CTV. Paired t-tests were used to compare mean calculated dose values for each OAR based on treatment technique. RESULTS Our cohort consisted of 8 patients (6 male, 2 female) with a median age 62.5 years (39 to 83 years). Compared to 3D-CRT, IMRT was associated with significantly decreased heart dose for both DIBH (Dmean 354.7 vs 440.5 cGY, P = 0.029) and FB (Dmean 521.2 vs 699.6 cGY, P = 0.006). IMRT was also associated with decreased dose to the left ventricle (LV), left anterior descending artery (LAD), liver, and lungs compared to 3D in both FB and DIBH. For IMRT plans, DIBH was associated with significantly lower heart V30 and V20 and a trend towards significance for lower heart Dmean (354.7 vs 521.2 cGY, P = 0.059) in comparison to FB. For 3D plans, DIBH was associated with a lower heart V30 and V20, and a higher lung V20, V10, and V5 in comparison to FB. CONCLUSION Both IMRT and DIBH represent modalities for reducing heart dose in gastric MALT patients receiving definitive RT. IMRT also reduces LV, LAD, liver and lung dose regardless of technique used to account for respiratory motion whereas DIBH was not associated with reduced doses to the LAD, kidney, or liver.

24. Isolated Tumor Cells and Micrometastatic Nodal Disease in Breast Cancer Patients After Neoadjuvant Chemotherapy: Is Post Mastectomy Radiation Therapy Indicated?

Abstract

Purpose/Objective(s) The prognostic and treatment implications of isolated tumor cells (ypN0i+) and micrometastatic (ypN1mi) nodal disease after neoadjuvant chemotherapy (NACT) is not well-studied. These patients are excluded from the ongoing NSABP B-51 trial which only includes patients with macroscopic nodal disease after NACT. We evaluate the long-term outcomes and the role of post-mastectomy radiation therapy (PMRT) in breast cancer patients with ypN0i+ and ypN1mi disease after NACT and mastectomy (MX). Materials/Methods Using the National Cancer Database (NCDB), we identified patients (≥ 18 year old) with breast cancer who were treated with NACT and MX diagnosed between 2004 and 2016. Patients with cT4 or metastatic disease were excluded. Chi-square testing was used to evaluate clinicopathological parameters of those treated with and without PMRT. Kaplan-Meier analysis was used to evaluate the effect of PMRT on overall survival (OS). Univariate and multivariate Cox proportional hazard models with and without inverse probability treatment weighting (IPTW) were used to identify prognostic factors associated with OS. Results Our final cohort consisted of 3,253 patients (34.0% ypN0i+, 66.0% ypN1mi) with a median age at diagnosis of 49 years (29 – 74 years). PMRT was utilized in 60.4% of the entire cohort, 51.8% of ypN0i+ patients and in 64.9% of ypN1mi patients. There was an increase in use of PMRT throughout the study period (56.2% in 2004-2011 vs. 61.9% in 2012-2016). With a median follow up of 36.8 months, the 5-year OS was 84.3% and 79.9% for ypN0i+ and ypN1mi patients, respectively (HR 1.23 [0.97-1.55], P = 0.092). Without accounting for covariates, there was no significant effect of PMRT on OS in ypN0i+ patients, ypN1mi patients, or in the entire cohort. In IPTW-MVA, PMRT had no significant effect on OS (HR 0.93 [0.74-1.18], P = 0.565]. Factors that were significant for worse OS on IPTW-MVA were higher comorbidity score, higher grade, hormone-receptor negative subtype, higher ypT stage, presence of lymphovascular invasion, Asian/Pacific Islander race, and more recent year of diagnosis. HER-2 positive subtype, receipt of hormone therapy, and private insurance were associated with improved OS. Conclusion Patients with ypN1mi disease had a trend towards worse OS in comparison to patients with ypN0i+ disease. The majority of patients with ypN0i+ and ypN1mi are receiving PMRT with higher rates of PMRT in ypN1mi vs. ypN0i+. PMRT rates are increasing with time. There is no association of PMRT with improved OS. Prospective randomized controlled data is needed to further evaluate the role of PMRT in these patients who were excluded from the ongoing NSABP B-51.

Abstract

Approximately 48 medical institutions in South Korea own and use blood irradiators; however, the quality control of blood irradiators is insufficient. This study was conducted to establish quality control procedures for blood irradiators using an alanine dosimeter and to ensure reliability of dosimetry. For dosimetry using alanine dosimeters, measurements were performed using an alanine dosimeter jig developed in this study to ensure dose delivery system reliability. Furthermore, the dose and dose distribution were verified, and the uncertainty was evaluated.

Abstract

A methodology for rapid process window characterization using families of voltage contrast (VC) test structures is described. While VC test structures have been in common use in the semiconductor industry for targeted applications for twenty years, we propose comprehensive application of VC test structures as a powerful tool for greatly accelerating semiconductor technology development. Test chips may be developed covering essentially all possible failure mechanisms. These chips include families of structures for monitoring both the critical process windows and the nominal failure rate. These structures are inspected in-line thereby providing the earliest possible feedback for split experiment evaluation, yield projection, and excursion detection. This paper reports on the application of this methodology to an advanced FinFET technology. Best methods for test structure layout, inspection setup, and binning strategy and reporting are described. Selected test structure designs and results are presented to illustrate these principles.

Abstract

Glioblastomas, which contain stem cell-like glioblastoma stem cells (GSCs), are universally lethal cancers. While neural stem cells (NSCs) are usually quiescent, single-cell studies suggest that proliferating glioblastoma cells reside in the GSC population. Interrogating in silico glioma databases for epigenetic regulators that correlate with cell cycle regulation, we identified the chromatin remodeler HELLS as a potential target in glioblastoma. GSCs preferentially expressed HELLS compared with their differentiated tumor progeny and nonmalignant brain cells. Targeting HELLS disrupted GSC proliferation, survival, and self-renewal with induction of replication stress and DNA damage. Investigating potential molecular mechanisms downstream of HELLS revealed that HELLS interacted with the core oncogenic transcription factors, E2F3 and MYC, to regulate gene expression critical to GSC proliferation and maintenance. Supporting the interaction, HELLS expression strongly correlated with targets of E2F3 and MYC transcriptional activity in glioblastoma patients. The potential clinical significance of HELLS was reinforced by improved survival of tumor-bearing mice upon targeting HELLS and poor prognosis of glioma patients with elevated HELLS expression. Collectively, targeting HELLS may permit the functional disruption of the relatively undruggable MYC and E2F3 transcription factors and serve as a novel therapeutic paradigm for glioblastoma.

Abstract

Background: The addition of a taxane to anthracycline-based chemotherapy provided an improved outcome in neoadjuant setting. Two neoadjuvant chemotherapy with 4 cycles of AC followed by 4 cycles of docetaxel (AC4-D4) and 3 cycles of FEC followed by 3 cycles of docetaxel (FEC3-D3) are widely used. Short duration of chemotherapy, 6 cycles rather than 8 cycles might be an attractive approach. Methods: This is a randomized, single-center, prospective, parallel group, comparative phase III trial (NCT02001506). Patients (pts) with breast cancer of clinically stage II or III, or sized  1.5 cm with histologically proven lymph-node involvement were included. Pts were stratified according to hormone receptor and HER2 expression status and randomized to AC4-D4 and 3 cycles of FEC3-D3 treatment. The primary endpoint was pathological complete response, defined as the absence of invasive disease in the breast and axillary lymph nodes, analyzed by intention to treat. Results: At the time of submission, a total of 207 pts were enrolled; 1 pt failed screening; 25 pts dropped out (5 pts in AC4-D4 arm and 2 pts in FEC3-D3 arm discontinue treatment due to progressive disease); 39 pts are still receiving neoadjuvant chemotherapy; 142 pts, who received surgery, were included for this analysis. In AC4-D4 arm, among 64 pts, 57 pts achieved clinical response (6 complete response [CR] and 51 partial response [PR]) and among them 9 pts achieved pathologic complete response [pCR]. In FEC3-D3 arm, among 78 pts, 66 pts achieved clinical response (7 CR and 59 PR) and among them 11 pts achieved pCR. Addition of docetaxel increased clinical response in both arms. The most common adverse event was febrile neutropenia. Without prophylactic G-CSF, grade ≥3 febrile neutropenia (FN) occurred 23/661 cycles (3.5%) in AC4-D4 arm and 23/552 cycles (4.2%) in FEC3-D3 arm, respectively. Grade 3 and 4 toxicities other than FN were reported at expected levels in both groups. Sixty-one severe adverse events were reported; 33 (including 23 FN) in AC4-D4 arm and 28 (including 23 FN) in FEC3-D3 arm. Conclusion: Compared to AC4-D4, shorter duration of FEC3-D3 neoadjuvant chemotherapy showed similar efficacy of pCR rate of 14.0% (versus 14.1% in AC4-D4 arm). The most common and important adverse event was febrile neutropenia in both arms. Updated study findings will be provided. Abstract

Binary coded decimal (BCD) to 7-segment decoder with oxide thin film transistors (TFTs) was implemented on a glass to display 7-segment display from BCD. The developed BCD to the 7-segment decoder with indium–gallium–zinc oxide thin film transistors was verified up to 1 kHz operation frequency under the power supply voltage of 5 V, and converted successfully 4-bit binary input code to 7-segment inputs. The binary output voltages were 0.4 V for the low and 4.1 V for the high at an operation frequency of 500 Hz, respectively. The output signals of the developed 7-segment decoder matched well the BCD to the decimal values. The result showed that oxide TFT can be used for the integrated BCD to 7-segment decoder for the various sensing applications, particularly for wearable devices and the integrated bio sensing platform.

High-quality Schottky junctions are fabricated on n-type 4H SiC epitaxial layers grown by chemical-vapor deposition (CVD) on C- and Si-face substrates to understand the effect of growth direction on the growth mechanism and formation of defects. The Schottky junction fabricated on C-face SiC epilayers have displayed a lower leakage current, as compared with that of Si-face epilayers.

The flux distributions on the device in the solar reactor with the solar furnace were studied. This study aims to understand of characters of concentrated sun rays through KIER solar furnace and design of device shape for uniform heat distribution on the device surface. For the calculation of heat flux on the device with the KIER solar furnace, the optical modeling program Soltrace was used. At first, the KIER 45 kWth solar furnace and flat disk type device shape was simulated for understand of past experimental results. And then 3 cylinder shape device model and 1 conical shape device model was suggested and the heat flux intensity on the device was calculated. Finally, 5 models which is including flat disk type device shape, 3 cylinder shape, and 1 conical shape device models was calculated and compared. The results show that the concentrated sun rays from dish and heat flux intensity are has a directional characteristic concentrated to normal direction than perpendicular direction. The results will be applied to next solar demonstrations which are design of new solar reactor and new device shape.

Abstract

In this study, the natural convection phenomenon of the air side and the forced convection phenomenon of the oil side were simulated in the radiator through a 3-D numerical analysis, and the total heat released by the oil side into the radiator heating plate and then to the air side was evaluated. Also, a quantitative analysis was carried out on the effect of each thermal resistance on the overall heat transfer coefficient through a 1-D thermal circuit analysis on the heat transfer mechanisms of the radiators considered in this study. In addition, for the diverse shapes of the heating plates considered in this study, the pressure drops of the oil side were quantitatively compared and evaluated. The temperatures at the air side and the oil side outlets of the radiators with 8 different fin shapes considered in this study had almost similar values showing a difference of +/-3% and, accordingly, the total heat transfer also showed similar heat dissipation performance in all the models. As a result of the 1-D thermal circuit analysis, in all the models considered in this study, while the thermal resistance of the air side accounted for 92% to 96% of the total, that of the oil side was 5 to 7%, and that of the heating plate showed a very small value of 0.02%.

Abstract

To evaluate recurrence rate and associated risk factors for recurrence after ethanol ablation (EA) in patients with predominantly cystic thyroid nodules.This observational study was approved by the Ethics Committee of the Institutional Review Board and informed consent for procedures was obtained. From April 2009 to April 2013, 107 consecutive patients with predominantly cystic nodules were treated using EA. Recurrence was defined as nodules showing a residual solid portion with internal vascularity, cosmetic problems remaining, or persistent symptoms, and patients who requested additional therapy to resolve their symptomatic or cosmetic problems. Delayed recurrence was defined as treated nodules that showed no recurrent features at 1 month, but showed newly developed recurrent features during the longer follow-up period. Multivariate analysis was used for variables to demonstrate the independent factors related to volume reduction.One month after EA, 18.7% of patients (20/107) showed recurrence. Among 87 patients with non-recurrence, 24.1% (21/87) showed delayed recurrence. The total recurrence rate was 38.3% (41/107). Patients with recurrence (n = 41) were treated using radiofrequency ablation (n = 28), second EA (n = 4), and refused further treatment (n = 9). These patients responded well to repeat EA and radiofrequency ablation. Multivariate analysis demonstrated that the initial nodule volume (20 ml; p 0.036) and vascularity (grade1; p 0.049) were independent predictors of volume reduction at last follow-up.The results revealed that although EA seemed to be effective during the initial period, delayed recurrence should be considered during longer-term follow-up. The independent predictors of recurrence were initial volume (20 ml) and vascularity.

Abstract

A major reason for poor long-term kidney transplant outcomes is the development of chronic allograft injury, characterized by interstitial fibrosis and tubular atrophy. Currently, an invasive biopsy that samples only 1% of the kidney is the gold standard for detecting kidney allograft fibrosis. We report the use of magnetic resonance elastography (MRE) to quantify tissue stiffness as a noninvasive and whole-kidney measurement tool of allograft fibrosis in a kidney transplant patient at 2 time points. The MRE whole-kidney stiffness values reflected the changes in fibrosis of the kidney allograft as assessed by histologic examination. To our knowledge, this technique is the first observation of change over time in MRE-derived whole-kidney stiffness in an allograft that is consistent with changes in histology-derived fibrosis scores in a single patient.

Abstract

The absolute muon flux between 20 and 3000 GeV is measured with the L3 magnetic muon spectrometer for zenith angles ranging from 0°to 58°. Due to the large exposure of about 150 m2sr d, and the excellent momentum resolution of the L3 muon chambers, a precision of 2.3% at 150 GeV in the vertical direction is achieved.The ratio of positive to negative muons is studied between 20 and 500 GeV, and the average vertical muon charge ratio is found to be 1.285±0.003(stat.)±0.019(syst.)., Physics Letters B, 598 (1-2), ISSN:0370-2693, ISSN:0031-9163, ISSN:1873-2445

Abstract

Events from the e+e− → Zγ process with hard initial-state radiation collected with the L3 detector at centre-of-mass energies between 183 and 209 GeV are used to measure the mass of the Z boson. Decays of the Z boson into hadrons or muon pairs are considered and the Z mass is determined to be 91.272 ± 0.032(stat) ± 0.033(syst) GeV, in agreement with the value measured at the Z resonance. Alternatively, assuming this measured value of the Z mass, the method determines the LEP centre-of-mass energy, found to be 175 ± 68(stat) ± 68(syst) MeV lower than the nominal value., Facultad de Ciencias Exactas
