Local area networks (LANs) allow computer users to access information on both local and remote servers. Increasingly, these users need access to their company's network continuously for such tasks as reading e-mail or retrieving and logging information. Wireless data networks can allow workers continuous connectivity, even when they are away from their desks. In some instances, a wireless network already may be in place in the form of a personal wireless telecommunications (PWT) phone system that provides wireless voice access. Such wireless phone systems can be extended to serve also as low-speed data networks. In this paper, we present a system architecture that provides wireless data access using the same infrastructure as a PWT phone system. We describe the system's design and present results from an implementation using Lucent Technologies' DEFINITY® Wireless Business System (DWBS) with custom modifications.

Eine 27-jahrige Patientin wurde mit Fingerspitzennekrosen bei jeweils langerstreckigem Verschluss der A. subclavia und der A. brachialis links stationar aufgenommen. Um die Extremitat zu erhalten, wurde zunachst eine Embolektomie mittels Fogarty-Katheter durchgefuhrt. Bei der Ursachenabklarung zeigte sich eine Halsrippe mit Kompression der A. subclavia links. Der Verschluss der A. brachialis wurde als arterielle Embolie gedeutet. Die weitere Therapie bestand in der transaxillaren Resektion der Halsrippe und der 1. Rippe sowie Anlage eines subklavioaxillaren Venenbypasses. Auch rechts zeigte sich eine Halsrippe mit vollstandiger Kompression der A. subclavia bei Elevation und Abduktion, weshalb auf dieser Seite ebenfalls die Resektion der Halsrippe und der 1. Rippe im Intervall erfolgen soll. Vaskulare Komplikationen infolge von Anomalien des thorakozervikalen Ubergangs sind selten. Ursachen sind in der Regel angeborene Abnormitaten, meist Halsrippen oder Variationen der 1. Rippe sowie des Querfortsatzes der unteren Halswirbel. Bei jungeren Patienten mit bewegungsabhangiger Claudicatio des Armes sowie akuten und chronischen arteriellen und venosen Verschlussen sollte aber auch an diese Differentialdiagnose gedacht werden. Die kontralaterale Seite sollte auch bei fehlender Symptomatik immer mit abgeklart werden. Wichtigstes diagnostisches Mittel ist die klinische Untersuchung. Die Therapie besteht in einer Resektion der abnormen Strukturen und gegebenenfalls in einer Revaskularisation.

Hintergrund Steroideluierende Elektroden weisen im Vergleich zu herkommlichen Elektroden nur einen geringen initialen Reizschwellenanstieg auf. Eine Reduktion der chronischen Reizschwelle und damit eine Verlangerung der Batterielebensdauer durch eine Verminderung der programmierten Impulsamplitude konnte bisher nicht schlussig nachgewiesen werden. Studienziel Die vorliegende, prospektiv durchgefuhrte Multicenterstudie evaluiert die Akut- und Langzeitperformance einer neuen Dexamethason freisetzenden Vorhofelektrode mit Platin-Iridium-Spitze (Selute, CPI). Methoden 48 Patienten erhielten 2:1 randomisiert die Studienelektrode bzw. eine identische nichtsteroideluierende Elektrode implantiert. Unmittelbar nach Implantation, vor Entlassung, 1, 3, 6 und 12 Monate nach Implantation wurden die atriale Reizschwelle bei vorgegebener Impulsdauer von 0,4 ms, 0,2 ms, 0,1 ms und 0,05 ms sowie die P-Wellen-Amplitude und Impedanz bestimmt. Wir berichten uber den Fall einer 56jahrigen Patientin mit einem ICD-System Typ Ventak AV (CPI), implantiert nach mehrfacher Reanimation bei Kammerflimmern und vorbestehendem, intermittierendem AV-Block III°. Bei ventrikularer Extrasystolie kam es wiederholt zur asynchronen Stimulation, wobei der ventrikulare Stimulus zu unterschiedlichen Zeiten der Ventrikelerregung einfiel. Ursache des Undersensings der ventrikularen Extrasystolen war eine Blankingzeit von 66 ms nach atrialer Stimulation, wodurch die ventrikulare Extrasystole vom ICD nicht wahrgenommen werden konnte. Folge war eine ventrikulare Stimulation in die vulnerable Phase des Herzzyklus bei ventrikularer Extrasystolie. Die Induktion hohergradiger ventrikularer Arrhythmien wurde nicht dokumentiert. Bei fehlender Programmierbarkeit der ventrikularen Blankingzeit sowie einer Sicherheitsstimulation und bereits maximaler Empfindlichkeit des ICD konnte die Inzidenz dieser Ereignisse durch eine Anhebung der Grundfrequenz und durch eine medikamentose Suppression der VES weitgehend reduziert werden.

Electron beam tomography (EBT, ultrafast computed tomography [CT], cine CT) combines unique temporal and high spatial resolution and is especially well suited for cardiac imaging. We established and evaluated a protocol for the noninvasive visualization and assessment of aortocoronary artery bypass grafts. Twenty-five patients with 56 bypass grafts were studied by EBT. Forty contiguous cross-sectional images were acquired triggered to the electrocardiogram during breathhold and intravenous injection of contrast agent. Three-dimensional reconstructions of the heart and bypass grafts were performed and compared with selective angiography of the bypass grafts. In 1 patient with 2 bypass grafts, a technically inadequate EBT examination was obtained. In the remaining patients (54 grafts), all 13 bypass occlusions were diagnosed with a sensitivity and specificity of 100%. Evaluation for hemodynamically relevant stenosis was possible in 84% of cases (36 of 43 patent grafts) and yielded a sensitivity of 100% (5 of 5 high-grade stenoses correctly detected) and specificity of 97% (1 false-positive diagnosis of high-grade graft stenosis). The main reasons for impaired ability to evaluate the scans were breathing artifacts and misplacement of the imaging volume, causing parts of the bypass grafts to be cut off. EBT permits noninvasive determination of bypass graft occlusion and relevant stenosis with high accuracy.

Die Elektronenstrahltomographie (EBT) ist ein der Computertomographie verwandtes Schnittbildverfahren mit extrem hoher raumlicher und zeitlicher Auflosung. Es wurde die Anwendbarkeit zur nichtinvasiven Darstellung der Koronararterien sowie zur Erkennung von Stenosen uberpruft. Nach Phantomstudien wurden 103 Patienten mittels EBT untersucht. Hierunter wurden 25 Patienten nach Koronarangioplastie (PTCA) in eine angiographisch blind kontrollierte Studie aufgenommen, um die Sensitivitat und Spezifitat des EBT im Vergleich zur Koronarangiographie zu evaluieren. Mittels EBT wurden nach periphervenoser Kontrastmittelgabe EKG‐getriggert 40 axiale Schnittbilder des Herzens angefertigt (Schichtdicke 3 mm, Tischvorschub 2 mm/Schicht, Pixelgrose 0,29 × 0,29 mm). Es wurden 3‐D‐Rekonstruktionen des Herzens und der Koronargefase vorgenommen und mit den Koronarangiogrammen verglichen. Nach 3‐D‐Rekonstruktion gelang die Darstellung des RIVA bei 94 % (prox. Drittel) bzw. 85 % (mittleres Drittel) der Patienten, fur die RCA (89 %/64 %) und RCX (74 %/54 %) war die Bildqualitat aufgrund von Bewegungsartefakten etwas eingeschrankt. Fur die Erkennung von Rezidivstenosen und ‐verschlussen nach Koronarangioplastie (PTCA) betrug die Sensitivitat des EBT 100 % (9/9), bei einem falsch‐positiven Befund betrug die Spezifitat 92 % (12/13), 3 Untersuchungen waren nicht auswertbar. Die Elektronenstrahltomographie als nichtinvasives Schnittbildverfahren erlaubt die Darstellung der Koronararterien sowie Diagnose von Verschlussen und hochgradigen Stenosen mit hoher Sensitivitat und Spezifitat.

A Computational Fluid Dynamics (CFD) based front tracking algorithm is applied to investigate the rising behaviour of a single bubble in a vertical pipe with a stagnant or flowing viscous liquid. The ratio of the pipe diameter to the bubble equivalent diameter ( b D D / ) is varied within the range of 1.0∼10.0. The wall effects on the terminal bubble rising speed (U ) and shape are investigated under various flow conditions, which are characterised by the parameters Archimedes number ( Ar ), Bond number ( Bo ), and bulk liquid flow speed ( l U ). It is found that the terminal bubble rising speed (U ) relates to the bubble rising speed in an infinite domain ( ∞ U ) and the pipe diameter by the formula α ) / ( / ) ( D D U U U b ∝ − ∞ ∞ , whereα is an exponent relating to the bubble deformability, and it is found to be in the range of 1.0∼0.7 in this study. In addition, the effects of flowing liquid on the terminal bubble rising speed and shape are also investigated for different sized pipes. It is demonstrated that the bubble rising behaviour is significantly affected by the flowing liquid in the pipe with a small diameter. Moreover, the detailed flow field around the bubble is presented to understand the physics of bubble rising behaviour in a vertical pipe under various flow conditions.

Intima-media thickness (IMT) of carotid arteries, measured by vascular ultrasound, has widely been used as a surrogate marker for coronary atherosclerosis. As recent studies showed significant but only weak correlation between IMT and coronary artery stenoses, this study evaluated calcified plaques in the extracranial carotid arteries as a reasonable marker for coronary artery disease.139 patients underwent cardiac catheterization with selective coronary angiography and ultrasound examination (B-mode, standard Doppler, color Doppler) of the carotid arteries. In case of calcified plaques number and distribution among the extracranial cerebral arteries were determined (Figure 1). Coronary angiograms were analyzed for disease severity and extent (number of main vessels with50% stenosis). Besides the traditional vascular risk factors hypertension, diabetes, smoking and hypercholesterolemia as well as body mass index, age and sex were analyzed.Calcified plaques of carotid arteries were significantly correlated (r = 0.502, p0.001) with coronary artery stenoses as well as hypercholesterolemia (r = 0.410, p0.001), increasing age (r = 0.406, p0.001) and diabetes (r = 0.290, p0.001) in contrast to hypertension (r = 0.125, p = 0.075), smoking status (r = -0.043, p = 0.311), body mass index (r = -0.122, p = 0.083) and male sex (r = -0.103, p = 0.114) (Table 1). The number of calcified plaques was significantly correlated (r = 0.568, p0.001) with severity and extent of coronary artery disease, too (Figure 2). For two and more calcified plaques sensitivity concerning coronary artery stenoses was 80%, specificity 75%, positive predictive value 83%, negative predictive value 73% (Table 2). Multiple stepwise regression analysis was performed for all eight variables (calcified plaques, hypercholestrolemia, diabetes, hypertension, smoking status, age, sex, body mass index). Including calcified plaques the predictive value of all factors was higher (r = 0.644 to r = 0.607).Determination of calcified carotid plaques by ultrasound is useful to improve the predictive value of risk factor-based multivariate models based on traditional risk factors.

Reliable noninvasive assessment of coronary-artery stenoses and occlusions would constitute an advantage in the care of patients with known or suspected coronary artery disease. We investigated the accuracy of contrast-enhanced electron-beam computed tomography (CT) for the detection of high-grade coronary-artery stenoses and occlusions.Electron-beam CT was performed in 125 patients. After intravenous injection of a contrast agent, 40 cross-sectional images of the heart were acquired during inspiration, triggered by the electrocardiogram in diastole. Three-dimensional reconstructions of the heart and coronary arteries were rendered to facilitate evaluation of the images. The proximal and middle segments of the major coronary arteries were evaluated for the presence or absence of high-grade stenoses and occlusions. The results were compared with those of invasive coronary angiography in a blinded fashion.Because of technical problems that impaired the quality of the images, 124 (25 percent) of the 500 coronary arteries studied (left main, left anterior descending, left circumflex, and right coronary) in a total of 125 patients were excluded from evaluation. No vessels could be evaluated in 19 patients (15 percent), and another 28 patients (22 percent) had one, two, or three vessels that could not be evaluated. In the remaining coronary arteries with adequate image quality, electron-beam CT permitted visualization of 69 of 75 high-grade stenoses and occlusions (sensitivity, 92 percent), whereas in 282 of 301 arteries, the absence of high-grade stenoses and occlusions was correctly detected (specificity, 94 percent).When image quality is adequate, electron-beam CT may be useful to detect or rule out high-grade coronary-artery stenoses and occlusions.

We report a case of a 56-year-old white female with an ICD system (Ventak AV, CPI) implanted because of repeated episodes of ventricular fibrillation and preexisting intermittent total AV-block. Several times asynchronic stimulation could be observed in the case of ventricular extrasystole stimulating the ventricle during different times of depolarization. The reason for undersensing of the premature ventricular complexes was a blanking period of 66 ms after atrial stimulation. Therefore, the ICD was not able to sense the ventricular extrasytole and the stimulation of the ventricle occurred during the vulnerable period of the heart cycle. Induction of severe ventricular arrhythmia was not documented. Because of missing programmability of the ventricular blanking period, no possibility of a safety stimulation, and already maximal sensitivity of the ICD, the incidence of these events could only be reduced by increasing the lower rate limit of the pacemaker and suppression of the premature ventricular complexes by medication.

Coronary artery desease is one of the most important causes of death in western countries. This explains the high demand for noninvasive imaging techniques for the heart. X-ray based techniques, in addition to magnetic resonance and ultrasound imaging, remain important as only they are able to image and, potentially, to quantify coronary calcification. Fluoroscopy, conventional computed tomography (CT), spiral CT, and electron-beam CT have not yet become fully established; fluoroscopy fails to detect small amounts of calcium, conventional CT is not able to scan the volume of interest adequately during a single breathhold, and electron-beam CT systems are seldom available due to their high cost.

Electron beam tomography (EBT) is a computed tomography technique with unique spatial and temporal resolution. The aim of the study was to establish and evaluate a protocol for the non-invasive visualization and detection of stenoses of the coronary arteries. Following phantom studies, 103 patients were investigated. Among these, 25 patients after coronary angioplasty (PTCA) were included in a prospective, blinded comparison to coronary angiography. After injection of contrast agent in a peripheral vein, 40 axial cross sections of the heart were acquired, triggered to the ECG (slice thickness 3 mm, 1 mm overlap, pixel size 0.29 x 0.29 mm). Three-dimensional reconstructions of the heart and coronary arteries were performed in the form of "shaded surface display," the results were compared to coronary angiography. In the reconstructions, the proximal and mid section of the LAD was visualized in 94% and 85%, image quality was reduced for the right coronary artery (89%/64%) and left circumflex (74%/54%), mainly due to movement artifacts. EBT displayed a sensitivity of 100% (9/9) for the detection of occlusions and high-grade restenoses in patients after PTCA, due to one false-positive result, specificity was 92% (12/13). Three PTCA patients could not be evaluated due to respiration artifacts. Contrast-enhanced electron beam tomography (EBT) permits the visualization of coronary arteries and the detection of stenoses and occlusions with high sensitivity and specificity.

1. In den elektrokardiographischen Eigentumlichkeiten beim Ulkuskranken (“Ulkus-Ekg”) sehen wir den Ausdruck der konstitutionsbedingten Eigenarten einer Ulkuspersonlichkeit. 2. Zwei “Ulkustypen” werden gegenubergestellt: das uberwiegend in der Konstitution begrundete Ulkus (Ulkus der Personlichkeit, “Konstitutionsulkus”) und das vorwiegend situationsgebundene Ulkus (Ulkus der Situation, “Krisenulkus”). Sinngemas ergibt sich aus unseren Untersuchungen an Kranken mit Magenresektion eine Unterscheidung im elektrokardiographischen Kurvenbild in Kranke mit einem “Ulkus-Ekg” (Gruppe I) und in Kranke ohne “Ulkus-Ekg” (Gruppe II). 3. Als elektrokardiographische Eigentumlichkeiten eines Ulkuskranken gelten: flaches, breites, angedeutet doppelgipfligesP (Vagus-P); verlangertesP-Q, mitunter auch auffallend kurzePQ-Zeit; geringgradige Auspragung oder Fehlen derQ-, wie auch derS-Zacke; ein bezeichnender Kurvenzug der Endschwankung (der abfallende Schenkel derR-Zacke geht unmittelbar in einer leicht angehobenen flach muldenformig verlaufenden Zwischenstrecke uber in die angedeutet asymmetrisch geformte hoch positiveT-Zacke); Vagus-T; “Kurvenruhe”; Bradykardie; geringe Neigung zur Extrasystolie. —Die einzelnen Merkmale konnen in verschiedener Auspragung vorliegen, doch das Typische eines “Ulkus-Ekg” liegt in dem Zusammentreffen, im elektrokardiographischen Gesamtbild. 4. Das “Ulkus-Ekg” vermag auch nach einer Magenresektion jahrelang, selbst bis ins hohe Alter, die Ulkuspersonlichkeit zum Ausdruck zu bringen. 5. Kranke der Gruppe I (Magenresezierte mit “Ulkus-Ekg”), ganz uberwiegend Manner, hatten fast ausschlieslich ein Ulcus duodeni, wurden haufig wegen rezidivierender Ulcera operiert, klagen meist uber fortbestehende oder wiederauftretende Oberbauchbeschwerden nach der Operation, oft in ahnlicher Art wie vor dem Eingriff und weisen vielfach eine familiare Ulkusneigung auf. 6. Kranke der Gruppe II (Magenresezierte ohne Ulkus-Ekg) hatten ofters ein Ulcus ventriculi oder sonstige Magenwandveranderungen mit entsprechender absoluter Indikation zur Operation, lassen vielfach schwerwiegende Lebenssituationen als auslosende Ursachen der Ulkuskrisen erkennen, sind und bleiben nach der Magenresektion haufiger beschwerdefrei als Kranke der Gruppe I. 7. Dem vorwiegend konstitutionsbedingten Ulkusleiden mit typischen “elektrokardiographischen Eigentumlichkeiten eines Ulkus-Ekg ”steht das vorwiegend situationsbedingte Ulkusleiden ohne” Ulkus-Ekg” gegenuber.

Segontin-Digoxin and digoxin were tested in a cross-over double blind trial in 30 patients with coronary heart disease and concomitant organic insufficiency in a cardiological specialist practice. As far as the coronary symptoms were concerned, treatment with Segontin-Digoxin was clearly superior to digoxin therapy alone. With regard to the elimination or improvement of signs of myocardial insufficiency, no difference could be established between the two preparations. A distinct lowering of heart rate and blood pressure, especially in hypertensives, was observed with Segontin-Digoxin therapy compared with digoxin alone.
