In this paper, the fractional equations of the mass-spring-damper system with Caputo and Caputo–Fabrizio derivatives are presented. The physical units of the system are preserved by introducing an auxiliary parameter σ. The input of the resulting equations is a constant and periodic source; for the Caputo case, we obtain the analytical solution, and the resulting equations are given in terms of the Mittag–Leffler function; for the Caputo–Fabrizio approach, the numerical solutions are obtained by the numerical Laplace transform algorithm. Our results show that the mechanical components exhibit viscoelastic behaviors producing temporal fractality at different scales and demonstrate the existence of Entropy 2015, 17 6290 material heterogeneities in the mechanical components. The Markovian nature of the model is recovered when the order of the fractional derivatives is equal to one.

Photodynamic therapy (PDT) is a clinically approved therapeutic option for the treatment of various types of cancer. PDT calls for the application of photosensitizers (PSs) and photoactivation with a particular light wavelength while tissue oxygen is present. Anticancer efficacy depends on the combination of these three substrates leading to the generation of cytotoxic reactive oxygen species (ROS) that promote apoptosis, necrosis, and autophagy of cancer cells. However, one of the biggest problems with conventional PDT is the poor accumulation and targeting of PSs to tumor tissues, resulting in undesirable side effects and unfavorable therapeutic outcomes. To overcome this, new photosensitizers have been developed through bioconjugation and encapsulation with targeting molecules, such as peptides, allowing a better accumulation and targeting in tumor cells. Several studies have been conducted to test the efficacy of several peptide-conjugated photosensitizers and improve PDT efficacy. This review aims to present current insights into various types of peptide-conjugated photosensitizers, with the goal of enhancing cancer treatment efficacy, addressing the limitations of conventional PDT, and expanding potential applications in medicine.

Background/Objectives: To compare the lumbopelvic muscle mechanical properties (MMPs) of women with and without multiple sclerosis (MS) and explore relationships between these properties and sociodemographic/clinical characteristics. Methods: This cross-sectional observational study included 22 women with MS and 22 age- and BMI-matched women without MS. MMPs (frequency, stiffness, decrement, relaxation, and creep) of pelvic floor and lumbar paravertebral muscles were assessed using a MyotonPRO device. Sociodemographic and clinical data related to pelvic floor health were also collected. Results: Women with MS showed significant differences in pelvic floor MMPs, including higher frequency (3.26 Hz; 95% CI [2.12, 4.41]), stiffness (90 N/m; 95% CI [55.09, 124.91]), and decrement (0.2; 95% CI [0.09, 0.31]), and lower relaxation (6.15 ms; 95% CI [8.26, 4.05]) and creep (0.24; 95% CI [0.34, 0.13]) compared to women without MS. For lumbar paravertebral muscles, differences were observed only on the right side, with lower frequency (2.15 Hz; 95% CI [0.28, 4.02]) and stiffness (62.17 N/m; 95% CI [10.7, 113.65]) in women with MS. Correlation patterns between MMPs and clinical characteristics differed by group, with moderate correlations found only in the MS group (e.g., EDSS: r = 0.57; p = 0.006; PFDI-20: r = 0.47; p = 0.026). Conclusions: Women with MS exhibit altered pelvic floor MMPs, characterized by reduced tone and stiffness and increased elasticity and viscoelasticity, while lumbar paravertebral differences are minimal. These findings highlight the need for objective MMP assessments in women with MS to guide preventive and therapeutic interventions.

Background/Objectives: Vascular graft infections (VGIs) represent a life-threatening complication, occurring in 0.2–6% of patients following aortic prosthetic placements. Historically, the primary focus for reducing VGIs has been on prevention. Currently, antimicrobial grafts are not available on the market. This study aimed to evaluate the efficacy of combining two antibiotics, fosfomycin and oritavancin, impregnated into the commercially available GelweaveTM vascular graft as a prophylactic alternative against the most commonly implicated bacteria responsible for VGI. Methods: The antimicrobial activity of fosfomycin and oritavancin was assessed using the broth microdilution method, and a synergistic effect was demonstrated using the checkerboard assay against Staphylococcus epidermidis, methicillin-resistant Staphylococcus aureus, and vancomycin-resistant Enterococcus faecium. The antibiotics were impregnated into the commercial vascular graft through immersion, and the antimicrobial efficacy of the fosfomycin/oritavancin-impregnated graft was assessed over a period of 7 days. Results: Eradication of all microorganisms tested was achieved using impregnation solutions with concentrations of 40 mg/mL of fosfomycin and 256 µg/mL of oritavancin. Conclusions: Impregnation with the combination of fosfomycin/oritavancin proved to be a promising approach to prevent VGIs. Vascular grafts with impregnated antibiotics are not yet available on the market, and this work represents an important step toward the development of a new class of antimicrobial vascular grafts.

Background: Microvascular dysfunction is an often overlooked feature of hypertrophic cardiomyopathy (HCM). Our aim was to assess the association between microvascular dysfunction, wall thickness, tissue characteristics and myocardial deformation in HCM patients, by analyzing individual myocardial segments. Methods: Prospective assessment including cardiac magnetic resonance to assess wall thickness, T1 and T2 mapping, extracellular volume, late gadolinium enhancement (LGE) and stress perfusion. Results were stratified according to the 16 American Heart Association segments. Results: Seventy-five patients were recruited (1200 segments), 63% male, mean age 54.6±14.8 years, maximal wall thickness of 20.22±4.6 mm. Among the 424 segments (35%) with perfusion defects, 24% had defects only in the endocardial layer and 12% in both endocardial and epicardial layers. Perfusion defects were more often detected in hypertrophied segments (64%). Among the 660 segments with normal wall thickness, 19% presented perfusion defects. This study explores the effectiveness of the antineoplastic agent 5-FU in cancer cells by leveraging the unique properties of cationic antimicrobial peptides (CAMPs) and cell-penetrating peptides (CPPs). Traditional anticancer therapies face substantial limitations, including unfavorable pharmacokinetic profiles and inadequate specificity for tumor sites. These drawbacks often necessitate higher therapeutic agent doses, leading to severe toxicity in normal cells and adverse side effects. Peptides have emerged as promising carriers for targeted drug delivery, with their ability to selectively deliver therapeutics to cells expressing specific receptors. This enhances intracellular drug delivery, minimizes drug resistance, and reduces toxicity. In this research, we comprehensively evaluate the ADMET (absorption, distribution, metabolism, excretion, and toxicity) properties of various AMPs and CPPs to gain insights into their potential as anticancer agents. The peptide synthesis involved a solid-phase synthesis using a Liberty Microwave Peptide Synthesizer. The peptide purity was confirmed via LC-MS and HPLC methods. For the ADMET screening, computational tools were employed, assessing parameters like absorption, distribution, metabolism, excretion, and toxicity. The cell lines A549 and UM-UC-5 were cultured and treated with 5-FU, CAMPs, and CPPs. The cell viability was measured using the MTT assay. The physicochemical properties analysis revealed favorable drug-likeness attributes. The peptides exhibited potential inhibitory activity against CYP3A4. The ADMET predictions indicated variable absorption and distribution characteristics. Furthermore, we assessed the effectiveness of these peptides alone and in combination with 5-FU, a widely used antineoplastic agent, in two distinct cancer cell lines, UM-UC-5 and A549. Our findings indicate that CAMPs can significantly reduce the cell viability in A549 cells, while CPPs exhibit promising results in UM-UC-5 cells. Understanding these multifaceted effects could open new avenues for antiviral and anticancer research. Further, experimental validation is necessary to confirm the mechanism of action of these peptides, especially in combination with 5-FU.

Background Two‐dimensional speckle tracking echocardiography has been shown to correlate with microvascular dysfunction, a hallmark of hypertrophic cardiomyopathy (HCM). We hypothesized that there is an association between myocardial work and left ventricular ischemia, with incremental value to global longitudinal strain, in patients with HCM. Methods and Results We performed a prospective assessment of patients with HCM, undergoing 2‐dimensional speckle tracking echocardiography and stress perfusion cardiac magnetic resonance. Results were stratified according to obstructive or nonobstructive HCM and the presence of significant replacement fibrosis (late gadolinium enhancement ≥15% of left ventricular mass). Seventy‐five patients with HCM (63% men, age 55±15 years) were evaluated, 28% with obstructive HCM (mean gradient 89±60 mm Hg). Perfusion defects were found in 90.7%, involving 22.5±16.9% of left ventricular mass, and 38.7% had late gadolinium enhancement ≥15%. In a multivariable analysis, a lower global work index (r=−0.519, β‐estimate −10.822; P=0.001), lower global work efficiency (r=−0.379, β‐estimate −0.123; P=0.041), and impaired global constructive work (r=−0.532, β‐estimate −13.788; P−15.5%) had a sensitivity of 64% and a specificity of 57%. The association between myocardial work and perfusion defects was significant independently of late gadolinium enhancement ≥15% and obstructive HCM. Conclusions Impaired myocardial work was significantly correlated with the extent of ischemia in cardiac magnetic resonance, independently of the degree of left ventricular hypertrophy or fibrosis, with a higher predictive power than global longitudinal strain.

Enquadramento: a família é o contexto privilegiado de cuidados de suporte à vida e saúde dos indivíduos. A abordagem sistémica da família tem, conceptualmente vindo a ganhar projeção. A Ontologia de Enfermagem, propõe uma estrutura onde os conceitos da disciplina e as suas relações são especificados, descrevendo uma representação formal do conhecimento de enfermagem. Objetivos: representar o conhecimento em enfermagem sobre a família como cliente dos cuidados, em três classes de itens de informação: dados, diagnósticos e intervenções. Metodologia: estudo qualitativo inferencial; realizada análise documental à parametrização do Sistema de Apoio à Prática de Enfermagem - SAPE®, revisão da literatura e focus group. Resultados: na parametrização nacional constatou-se pouca visibilidade da família como unidade de cuidados bem como a ausência de representação dos modelos concetuais que suportam o conhecimento disciplinar. Na Ontologia de Enfermagem, o processo familiar engloba: organização do funcionamento da casa e edifício residencial; preparação da família para integrar um familiar dependente no autocuidado; preparação da família para a chegada do recém-nascido; e, planeamento familiar. Para cada domínio foram especificadas três classes de informações de enfermagem. Conclusão: Este estudo apresenta-se como um contributo para a formalização do conhecimento de enfermagem no domínio do Processo Familiar.

Resumen: El cáncer de mama es la primera causa de muerte en el mundo entre las mujeres. El Sistema Nacional de Salud (SNS) introdujo el cribado poblacional de cáncer de mama en 1990. Como en la mayoría de los programas europeos, el riesgo se identifica con base en la edad y se ofrece una mamografía cada dos años a mujeres de 50 a 69 años. La evidencia científica está avanzando hacia un cribado personalizado, basado en el riesgo individual. En este artículo se presentan los ensayos clínicos que evaluarán la eficacia del cribado personalizado y algunos estudios realizados en nuestro entorno sobre el efecto de informar a las mujeres de los beneficios y efectos adversos del cribado o la aceptabilidad y viabilidad de ofrecer cribado personalizado, que incluya la toma de decisiones compartidas. El Programa de Actividades Preventivas y Promoción de la Salud puede ayudar a transformar el cribado en nuestro SNS. Abstract: Breast cancer is the leading cause of death in the world among women. The Spanish National Health System (SNHS) introduced population-based breast cancer screening in 1990. As in most European programs, risk is identified on the basis of age and a mammogram is offered every two years to women aged 50–69 years. Scientific evidence is moving toward personalized screening, based on individual risk. This article presents the clinical trials that will evaluate the efficacy of personalized screening and some studies carried out in our environment on the effect of informing women of the benefits and adverse effects of screening or the acceptability and feasibility of offering personalized screening, in the Shared Decision Making context. The Preventive Activities and Health Promotion Program can help transform screening in our SNHS.

Aims: The aim of the study is to investigate the association between the degree of ischemia due to coronary microvascular dysfunction (CMD) and the left ventricular (LV) tissue characteristics, systolic performance, and clinical manifestations in hypertrophic cardiomyopathy (HCM).Methods and Results: This prospective study enrolled 75 patients with HCM without obstructive epicardial coronary artery disease. Each patient underwent cardiovascular magnetic resonance (CMR) including parametric mapping, perfusion imaging during regadenoson-induced hyperemia, late gadolinium enhancement (LGE) and three-dimensional longitudinal, circumferential, and radial strains analysis. Electrocardiogram, 24-h Holter recording, and cardiopulmonary exercise testing (CPET) were performed to assess arrhythmias and functional capacity. In total, 47 (63%) patients were men with the mean age of 54.6 (14.8) years, 51 (68%) patients had non-obstructive HCM, maximum wall thickness (MWT) was 20.2 (4.6) mm, LV ejection fraction (LVEF) was 71.6 (8.3%), and ischemic burden was 22.5 (16.9%) of LV. Greater MWT was associated with the severity of ischemia (β-estimate:1.353, 95% CI:0.182; 2.523, p = 0.024). Ischemic burden was strongly associated with higher values of native T1 (β-estimate:9.018, 95% CI:4.721; 13.315, p < 0.001). The association between ischemia and LGE was significant in following subgroup analyses: MWT 15–20 mm (β-estimate:1.941, 95% CI:0.738; 3.143, p = 0.002), non-obstructive HCM (β-estimate:1.471, 95% CI:0.258; 2.683, p = 0.019), women (β-estimate:1.957, 95% CI:0.423; 3.492, p = 0.015) and age 15% (AUC 0.766, sensitivity 0.724, specificity 0.659). Ischemia was also associated with atrial fibrillation or flutter (AF/AFL) (OR-estimate:1.481, 95% CI:1.020; 2.152, p = 0.039), but no association was seen for non-sustained ventricular tachycardia. Ischemia was associated with shorter time to anaerobic threshold (β-estimate: −0.442, 95% CI: −0.860; −0.023, p = 0.039).Conclusion: In HCM, ischemia associates with morphological markers of severity of disease, fibrosis, arrhythmia, and functional capacity.

Abstract Background: Infective endocarditis (IE) is associated with severe complications and high mortality. The assessment of mortality rates and predictors for fatal events is important to identify modifiable factors related to the pattern of treatment, in order to improve outcomes. Objectives: We sought to evaluate clinical outcomes of patients with IE and to determine predictors of in-hospital mortality. Methods: Retrospective single-center study including patients with IE admitted during a 10-year period (2006-2015). Data on comorbidities, clinical presentation, microbiology and clinical outcomes during hospitalization were evaluated. Risk factors of in-hospital death were analyzed. A p-value < 0.05 was considered significant. Results: A total of 134 cases were included (73% males, mean age of 61 ± 16 years-old). Half of them had previous valvular heart disease. Healthcare-associated IE and negative blood-cultures occurred in 22% and prosthetic IE in 25%. The aortic valve was the one most often affected by infection. Staphylococcus aureus was the most commonly isolated microorganism. Forty-four (32.8%) patients underwent cardiac surgery. The in-hospital mortality rate was 31.3% (42 patients). The identified risk factors for in-hospital mortality were Staphylococcus aureus etiology (OR 6.47; 95% CI: 1.07-39.01; p = 0.042), negative blood-cultures (OR 9.14; 95% CI: 1.42-58.77; p = 0.02), evidence of valve obstruction in echocardiography (OR 8.57; 95% CI: 1.11-66.25; p = 0.039), clinical evolution with heart failure (OR 4.98; 95%CI: 1.31-18.92; p = 0.018) or septic shock (OR 20.26; 95% CI: 4.04-101.74; p < 0.001). Cardiac surgery was a protective factor of mortality (OR 0.14; 95% CI 0.03-0.65; p = 0.012). Conclusion: The risk factors for in-hospital mortality were clinical (heart failure, septic shock), evidence of valve obstruction in echocardiography, Staphylococcus aureus etiology or negative blood cultures. Invasive treatment by surgery significantly decreased the mortality risk.

Introduction Personalised cancer screening aims to improve benefits, reduce harms and being more cost-effective than age-based screening. The objective of the DECIDO study is to assess the acceptability and feasibility of offering risk-based personalised breast cancer screening and its integration in regular clinical practice in a National Health System setting.Methods and analysis The study is designed as a single-arm proof-of-concept trial. The study sample will include 385 women aged 40–50 years resident in a primary care health area in Spain. The study intervention consists of (1) a baseline visit; (2) breast cancer risk estimation; (3) a second visit for risk communication and screening recommendations based on breast cancer risk and (4) a follow-up to obtain the study outcomes.A polygenic risk score (PRS) will be constructed as a composite likelihood ratio of 83 single nucleotide polymorphisms. The Breast Cancer Surveillance Consortium risk model, including age, race/ethnicity, family history of breast cancer, benign breast disease and breast density will be used to estimate a preliminary 5-year absolute risk of breast cancer. A Bayesian approach will be used to update this risk with the PRS value.The primary outcome measures will be attitude towards, intention to participate in and satisfaction with personalised breast cancer screening. Secondary outcomes will include the proportions of women who accept to participate and who complete the different phases of the study. The exact binomial and the Student’s t-test will be used to obtain 95% CIs.Ethics and dissemination The study protocol was approved by the Drug Research Ethics Committee of the University Hospital Arnau de Vilanova. The trial will be conducted in compliance with this study protocol, the Declaration of Helsinki and Good Clinical Practice.The results will be published in peer-reviewed scientific journals and disseminated in scientific conferences and media.Trial registration number NCT03791008.

Background The emergence of specific therapies for transthyretin cardiac amyloidosis (CA) warrants the need for a systematic review of the literature. Methods and Results A systematic review of the literature was conducted according to Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) guidelines. A systematic search was performed on MEDLINE, PubMed, and Embase databases on November 29, 2019. Studies were selected based on the following predefined eligibility criteria: English‐language randomized controlled trials (RCTs), non‐RCTs, or observational studies, which included adult patients with variant/wild‐type transthyretin‐CA, assessed specific therapies for transthyretin‐CA, and reported cardiovascular outcomes. Relevant data were extracted to a predefined template. Quality assessment was based on National Institute for Health and Care Excellence recommendations (RCTs) or a checklist by Downs and Black (non‐RCTs). From 1203 records, 24 publications were selected, describing 4 RCTs (6 publications) and 16 non‐RCTs (18 publications). Tafamidis was shown to significantly improve all‐cause mortality and cardiovascular hospitalizations and reduce worsening in 6‐minute walk test, Kansas City Cardiomyopathy Questionnaire—Overall Summary score, and NT‐proBNP (N‐terminal pro‐B‐type natriuretic peptide) in variant/wild‐type transthyretin‐CA. Patisiran showed promising results in a subgroup analysis of patients with variant transthyretin‐CA, which have to be confirmed in RCTs. Inotersen showed conflicting results on cardiac imaging parameters. The one study on AG10 had only a 1‐month duration and cardiovascular end points were exploratory and limited to cardiac biomarkers. Limited evidence from noncomparative single‐arm small non‐RCTs existed for diflunisal, epigallocatechin‐3‐gallate (green tea extract), and doxycycline+tauroursodeoxycholic acid/ursodeoxycholic acid. Conclusions This systematic review of the literature supports the use of tafamidis in wild‐type and variant transthyretin‐CA. Novel therapeutic targets including transthyretin gene silencers are currently under investigation.

The authors report the clinical and genetic investigation of a family with hypertrophic cardiomyopathy (HCM). The individuals described are three affected first-degree relatives (father, daughter and son), one affected niece and unaffected nephew and niece. Those affected all share a very similar phenotype consisting of asymmetric HCM, with hypertrophy particularly affecting the septum and the anterior wall, and similar electrocardiographic features, including a short PR interval. Case 1 (proband) presented with obstructive HCM and had undergone myectomy and mitral valve replacement. Case 2 (oldest offspring of Case 1) had non-obstructive HCM with exertional angina and NYHA II heart failure (HF) symptoms; she developed non-sustained ventricular tachycardia during follow-up and received a single-chamber ICD for primary prevention of sudden cardiac death. Case 3 (son of case 1) presented with asymptomatic non-obstructive HCM and developed NYHA II HF symptoms during follow-up. Case 4 had non-obstructive HCM, mainly with NYHA II HF symptoms. Testing of the proband for sarcomeric mutations and phenocopies was initially negative. After eight years of clinical follow-up, the suspicion of an undiscovered pathogenic gene mutation shared among the members of this family led us to enroll the proband in a whole-genome sequencing research project, which revealed a heterozygous pathogenic intronic MYBPC3 variant (c.1227-13G>A [rs397515893]), cosegregating with the phenotype. Resumo: Os autores descrevem a investigação clínica e genética de uma família com miocardiopatia hipertrófica (MCH). Os casos descritos incluem três parentes de primeiro grau (pai e dois filhos) e uma sobrinha com MCH e dois sobrinhos sem MCH. Todos os casos afetados apresentavam um fenótipo semelhante, que consistia em MCH assimétrica, com hipertrofia envolvendo o septo e a parede anterior e alterações eletrocardiográficas idênticas, incluindo um intervalo PR curto. O caso 1 (índice) apresentava uma MCH obstrutiva e tinha sido previamente submetido a miectomia e substituição valvular mitral. O caso 2 (filha mais velha) apresentou-se com uma MCH não obstrutiva, sintomas de angina e insuficiência cardíaca (IC) classe II de NYHA; desenvolveu taquicardia ventricular não mantida (TVNM) no seguimento e foi submetida a implantação de CDI, em prevenção primária de morte súbita cardíaca. O caso 3 (filho) apresentou-se com MCH não obstrutiva assintomática, tendo desenvolvido queixas de IC classe II de NYHA durante o seguimento. O caso 4 (sobrinha) apresentou-se com MCH não obstrutiva, predominantemente com sintomas de IC clase II NYHA. A pesquisa de mutações sarcoméricas e fenocópias no caso-índice foi, inicialmente, «negativa». Após oito anos em seguimento clínico e perante a suspeita de um novo gene patogénico/nova mutação partilhada entre todos os parentes, incluímos o caso-índice num projecto whole-genome sequencing. Foi diagnosticada uma variante intrónica patogénica, em heterozigotia, do gene MYBPC3 (c.1227-13G>A (rs397515893)), com cossegregação familiar.

Abstract Neospora spp. are intracellular protozoa with worldwide distribution and closely related to Toxoplasma gondii, which can infect a variety of mammals including horses. From September 2013 to June 2014, 185 horses from northern, central and southern parts of mainland Portugal were randomly sampled and tested for detection of immunoglobulin (Ig) G antibodies to Neospora spp. using an indirect multi-species enzyme-linked immunosorbent assay (ELISA) commercial test (ID Screen® Neospora caninum Indirect Multi-species; ID.vet Innovative Diagnostics, Grabels, France). Two horses (1.1%; CI: 0.1-3.8%), one male and one female, were found to be seropositive for Neospora spp. Both seropositive animals were horses housed indoors but with access to outdoors, used for leisure activities and were apparently healthy, with good body condition and with no alterations at physical examination. This was the first serologic survey of antibodies to Neospora spp. carried out in horses from Portugal.

An analysis of the muscle mechanical properties (MMPs) of the pelvic floor muscles (PFMs) is relevant for understanding the physiopathology of urinary incontinence (UI). However, there is no objective and reliable methodology currently available for quantifying the MMPs of PFMs. Thus, the objective was to determine the intra-rater and inter-rater reliability of the MMPs of PFM assessment with a hand-held tonometer device, called the MyotonPRO, in young women with and without UI. Sociodemographic and pelvic floor questionnaires, plus MMPs of PFMs were assessed in 38 nulliparous women with UI and 40 matched healthy women by two trained physiotherapists on two different occasions, 48–72 h apart. Good to excellent absolute reliability was found for tone, stiffness, and decrement of both intra- and inter-rater analyses in both study groups (Intraclass Correlation Coefficient ranged from 0.75 to 0.92), with a trend of lower values for relaxation and creep. The standard error of measurement (SEM) did not achieve 10% of the mean values for any MMPs. The minimum detectable change (MDC) values were also provided for clinical applications. In conclusion, the relative reliability of tone, stiffness, and the assessment of the decrement of PFMs with MyotonPRO is good to excellent for UI and healthy women. The SEM and MDC values were acceptable for their application in clinical settings.

Objetivo: Racionalizar la prescripción en la población anciana es una prioridad de los sistemas sanitarios. Los criterios STOPP/START permiten detectar la prescripción potencialmente inadecuada (PPI) a nivel individual. Su aplicación a nivel poblacional permite determinar la prevalencia de PPI y su relación con diversas variables del paciente y del sistema sanitario. El objetivo de este estudio es analizar la prevalencia de PPI en población anciana a partir de bases de datos clínicos y de farmacia. Diseño: Estudio descriptivo transversal. Emplazamiento: Ámbito de atención primaria de la Región Sanitaria de Lleida, España. Participantes: Se incluyen 45.408 pacientes de 70 años o más atendidos en los centros de salud como mínimo una vez en el último año. Mediciones principales: Se calcula la frecuencia de incumplimiento de 43 indicadores STOPP y 12 indicadores START, a partir de los fármacos prescritos durante un año y se ajustan modelos de regresión logística para evaluar su asociación con diversas variables. Resultados: El 58% son mujeres. La edad media es de 79,7 años. El 58,1% incumplen como mínimo un criterio STOPP y/o START, relacionados principalmente con el uso de benzodiacepinas, antiinflamatorios no esteroideos e inhibidores de la bomba de protones por exceso, y los tratamientos para la osteoporosis, antiagregantes, estatinas, metformina y betabloqueantes por omisión. La PPI aumenta con la edad y la polifarmacia y es superior en pacientes ingresados en centros geriátricos y domiciliarios que en los ambulatorios. Conclusiones: Los criterios STOPP/START identifican PPI en más de la mitad de pacientes ancianos de una región sanitaria española.

The authors report a rare clinical case of myocardial bridging of the three major coronary arteries, which manifested in an unusual way with severe biventricular dysfunction in the context of tachycardia. For the diagnosis, the authors relied on non-invasive multimodality cardiac imaging, including cardiac magnetic resonance, computed tomography angiography and myocardial perfusion scintigraphy. The implementation of targeted medical and neurohormonal therapy resulted in the recovery of ventricular function and clinical improvement. Resumo: Os autores descrevem um caso clínico raro de bridging miocárdico dos três territórios coronários, que se apresentou de forma invulgar com disfunção biventricular grave em contexto de taquicardia. Para o diagnóstico os autores basearam-se em exames de imagem cardíaca não invasivos, como a ressonância magnética cardíaca, a angiografia tomográfica computorizada e a cintigrafia de perfusão miocárdica. A instituição de terapêutica médica dirigida e neuro-hormonal permitiu a recuperação da função ventricular e a melhoria clínica. Alzheimer’s disease (AD) is a multifactorial neurodegenerative disorder that is associated with the elderly. The current therapy that is used to treat AD is based mainly on the administration of acetylcholinesterase (AChE) inhibitors. Due to their low efficacy there is a considerable need for other therapeutic strategies. Considering that the malfunctions of different, but interconnected, biochemical complex pathways play an important role in the pathogenesis of this disease, a promising therapy may consist in administration of drugs that act on more than a target on biochemical scenery of AD. In this work, the synthesis and evaluation of xanthone and flavone derivatives as antioxidants with AChE inhibitory activity were accomplished. Among the obtained compounds, Mannich bases 3 and 14 showed capacity to inhibit AChE and antioxidant property, exerting dual activity. Moreover, for the most promising AChE inhibitors, docking studies on the target have been performed aiming to predict the binding mechanism. The results presented here may help to identify new xanthone and flavone derivatives as dual anti-Alzheimer agents with AChE inhibitory and antioxidant activities.

Introducción: Se sabe que hay diferencias entre los grupos de inmigrantes en lo que respecta a la prevalencia de diabetes, pero no disponemos de suficiente evidencia para afirmar que, una vez diagnosticada, sea distinto el grado de control. El objetivo de este estudio es analizar si hay diferencias en la detección y el grado de control de los pacientes diabéticos entre inmigrantes y autóctonos en nuestra región sanitaria. Método: Estudio poblacional observacional transversal que incluye a todos los pacientes diagnosticados de diabetes mellitus de tipo 2 asignados y atendidos durante 2010. Se analizaron los indicadores de calidad de la diabetes y se utilizaron modelos de regresión logística multivariada que se ajustaron por la edad, el sexo y el número de visitas. El método de ajuste fue forzado y se comprobó la no presencia de colinealidad mediante la curva ROC y el test de Hosmer & Lemeshow. Resultados: 77.999 pacientes autóctonos (6846 diabéticos) y 30.748 pacientes inmigrantes (415 diabéticos). El 8,78% de los pacientes autóctonos eran diabéticos (1,35% de los inmigrantes, p 0,05). Conclusiones: Hay un peor control y un peor registro de los indicadores de calidad en la atención de la diabetes mellitus en algunos grupos de inmigrantes.Introduction: Ethnic differences are known to exist in the prevalence of diabetes, but little is known about possible differences in the degree of diabetes control among ethnic groups. The aim of this study was to determine whether there are differences in diabetes detection and control between immigrants and the autochthonous population in our health region. Methods: We performed a cross-sectional, observational, population-based study of all patients diagnosed with diabetes mellitus type 2 registered and treated in 2010. We analyzed diabetes quality indicators and used multivariate logistic regression models adjusted for age, sex and number of visits. The adjustment method was forced and the absence of collinearity was identified through the ROC curve and Hosmer and Lemeshow's test. Results: There were 77,999 autochthonous patients (6,846 diabetics) and 30,748 immigrant patients (415 diabetics). A total of 8.78% of the autochthonous patients were diabetic versus 1.35% of immigrants (p 0.05). Conclusions: Diabetes control and quality indicators are poorer in some immigrant groups.

Resumo: Os autores apresentam o caso de uma doente de 76 anos, com o diagnóstico de tumor neuroendócrino do cego desde 2004 com metastização hepática, e com síndrome carcinóide desde setembro de 2010. A doente realizou ciclos de quimioterapia de forma intermitente, pelo que manteve sintomas e agravamento das lesões secundárias. Desde junho de 2011 apresenta sinais e sintomas compatíveis com insuficiência cardíaca direita, ficando internada em setembro de 2011. Nesta sequência realizou ecocardiograma transtorácico bi e tridimensional que revelou cavidades direitas dilatadas e folhetos das válvulas tricúspide e pulmonar espessados, fixos, condicionando insuficiência tricúspide grave e estenose e insuficiência pulmonar ligeiras, sugestivas de doença carcinóide cardíaca. Os autores discutem a relevância clínica da ecocardiografia transtorácica, e, atualmente, da ecocardiografia tridimensional, como o método de investigação de escolha nestes casos, ao permitir avaliar de forma única as válvulas e o aparelho subvalvular. Abstract: We describe the case of a 76-year-old woman, diagnosed with a neuroendocrine tumor of the cecum in 2004, with liver metastases and carcinoid syndrome since September 2010. The patient had been treated intermittently with chemotherapy cycles, and remained symptomatic, with worsening secondary lesions. In June 2011 she began to present signs and symptoms of right heart failure and was hospitalized in September 2011. Transthoracic two- and three-dimensional echocardiography revealed enlarged right atrium and ventricle and thickened and fixed tricuspid and pulmonary valve leaflets, causing severe tricuspid regurgitation and mild pulmonary regurgitation and stenosis, suggestive of carcinoid heart disease. The authors discuss the clinical importance of transthoracic echocardiography, and the more recent three-dimensional echocardiography, as the diagnostic exam of choice in these cases, as it is especially suitable for assessing the valves and subvalvular apparatus. This article aims to give the main achievements of the 1st part of phase 1 of the research project “CNO: A Double Opportunity: Promoting Family Literacy and Children’s School Success.” It is presented a summary of the obtained data of an exploratory study on the impact that the RVCC frequency had on family and professional life of the students, monitoring of their children school life, as well as promoting family literacy through new representations and practices of reading and writing. This study has an empirical scope and sought to understand how the RVCC process influences the adults perceptions in what concerns to the educational process, enhancing the development of family literacy and its implications for the children schooling path at the beginning of the 1st cycle of basic education.

Intravenous leiomyomatosis is an unusual clinical condition characterized by histologically benign smooth muscle lesions extending from the uterus into pelvic and systemic veins and, more rarely, into the right cardiac chambers. We report the case of a 45-year-old woman who presented with a three-week history of dyspnea on exertion, shortness of breath and fatigue. Echocardiography showed a large mobile mass in the right atrium prolapsing into the right ventricle and extending to the inferior vena cava. A computed tomography scan revealed a large mass extending from the right atrium to the inferior vena cava and through the systemic veins as far as the popliteal veins. A presumptive diagnosis of large thrombus was made; the correct diagnosis of intravenous leiomyomatosis with intracardiac involvement was obtained only after surgical resection and histologic examination. Resumo: A Leiomiomatose intravascular é uma situação clínica invulgar caracterizada histologicamente por lesões benignas de tecido muscular liso que se estendem a partir do útero para as veias pélvicas e suas aferentes, raramente atingindo as cavidades cardíacas direitas. Descrevemos o caso de uma mulher de 45 anos que se apresentou com dispneia de esforço, polipneia e fadiga, com três semanas de evolução. O ecocardiograma mostrou uma massa grande e móvel no interior da aurícula direita, com prolapso para o ventrículo direito e extensão para a veia cava inferior. A tomografia computorizada revelou uma massa que se estendia da aurícula direita para a veia cava inferior e ao longo das veias sistémicas até às veias popliteias. Foi feito o diagnóstico presuntivo de trombo extenso; o diagnóstico correto de leiomiomatose intravascular com envolvimento intra-cardíaco só foi possível após ressecção cirúrgica e exame histológico. Resumen Hoy día, la endocarditis infecciosa continúa siendo una causa importante de mor-bimortalidad en todo el mundo y es preocupante el aumento del número de infecciones asociadas con agentes virulentos y procedimientos médicos. Presentamos el caso de un indigente con antecedentes médicos desconocidos, ingresado por dolor lumbar, que comenzó a sentirse confuso, hipotenso y taquicárdico. Poco después de su ingreso su estado evolucionó a septicemia grave. Los resultados de las pruebas iniciales también eran indicativos de infarto agudo de miocardio, pero el ecocardiograma transtorácico reveló una vegetación masiva en la válvula aórtica con absceso perivalvular condicionando regurgitación aórtica grave. El paciente murió, con el diagnóstico definitivo de endocarditis infecciosa. Más tarde, en los hemocultivos se identificó Aggregatibacter aphrophilus, un microorganismo del grupo HACEK. Este caso confirma que, a pesar de los resultados favorables en general, hay casos de infecciones graves, sobre todo si el diagnóstico y el tratamiento fueron tardíos.
