Abstract Background Legionnaires’ Disease (LD) rarely evolves into pulmonary abscesses. The current systematic review has been designed to explore therapeutical strategies in pulmonary cavitary LD. Methods A research strategy was developed and applied to the databases Embase, Pubmed, and Web of Science from the 1st of January 2000 to the 1st of November 2022. Original articles, case series, case reports, and guidelines written in English, French, German, Italian, and Dutch were considered. Furthermore, medical records of patients treated at the University Hospital UZ Brussel for LD cavitary pneumonia, between the 1st of January 2016 to the 1st of January 2022, were reviewed. Results Two patients were found by the UZ Brussel’s medical records investigation. Through the literature review, 23 reports describing 29 patients, and seven guidelines were identified. The overall evidence level was low. Result of synthesis (case reports) The median age was 48 years and 65% were male. A polymicrobial infection was detected in 11 patients (44%) with other aerobic bacteria being the most commonly found. At diagnosis, 52% of patients received combination therapy, and fluoroquinolones were the preferred antimicrobial class. Anaerobic coverage was neglected in 33% of patients. Result of synthesis (guidelines) Three guidelines favor monotherapy with fluoroquinolones or macrolides, while one suggested an antimicrobial combination in case of severe LD. Four guidelines recommended anaerobic coverage in case of lung abscesses. Conclusion To date, the evidence supporting cavitary LD treatment is low. Monotherapy lowers toxicity and might be as effective as combination therapy. Finally, anaerobes should not be neglected. Graphical Abstract

Introducción: La sarcopenia aumenta el riesgo de caídas y fracturas, además de relacionarse con diversas enfermedades y aumento de la mortalidad. Distintos grupos de investigación proponen diferentes definiciones para el diagnóstico de sarcopenia, que incluyen la medición de la masa muscular por DEXA y la valoración de la fuerza de prensión palmar a través de un dinamómetro de mano. Sin embargo, estos 2 instrumentos no están habitualmente disponibles en los consultorios médicos. Objetivos: Determinar la utilidad diagnóstica de 2 pruebas de rendimiento físico: velocidad de marcha e incorporarse de una silla, como herramientas para la detección de pacientes con probable sarcopenia. Metodología: Se incluyeron 706 mujeres, entre 50 y 85 años, a las que se realizó las pruebas funcionales y DXA (n=376) para medir la masa muscular. Resultados: El 26,1% registró una velocidad de marcha disminuida mientras que el 55,3% presentó alteración en la realización de la prueba de incorporarse de una silla. La prevalencia de sarcopenia varió desde un 4,5% hasta un 35% según los criterios utilizados. Ambas pruebas presentaron un alto valor predictivo negativo (>70%) para el diagnóstico de sarcopenia, mientras que la prueba de incorporarse de una silla mostró además, una alta sensibilidad (> 66%) para dicho diagnóstico. Conclusión: La implementación en el consultorio de estas pruebas sencillas y sin costo, simplificaría la detección de pacientes con probable sarcopenia.

Se presenta el uso de una cámara anestésica de bajo costo, construida en vidrio, de fácil manejo y limpieza, como recurso didáctico, para explicar, en ratones de laboratorio, los fundamentos de la anestesia general inhalatoria a estudiantes de medicina veterinaria. La cámara permite adecuada dosificación del anestésico, garantiza la vida de los animales y disminuye la exposición de profesores y estudiantes a los gases anestésicos. Así, se pueden abordar didácticamente los aspectos básicos de la anestesia general inhalada, como el efecto de la concentración del anestésico en el aire inspirado y solubilidad.

This article focuses on the notion of terrain applied to the field of studies dedicated to Roma and Voyageurs (Travellers) in history and anthropology. Starting from an observation on one of the many living places of Voyageurs in France, in this case known as “private terrain”, the author draws up a series of comparisons between works on various populations belonging to the Romani sphere and questions the various uses of the notion of terrain.

In patients coming from countries with a high prevalence of tuberculosis and presenting with chronic infectious disease, tuberculosis with pulmonary and/or extrapulmonary involvement should be included in the differential diagnosis.

En la Universidad de Ciencias Informáticas (UCI) de La Habana los resultados académicos en la disciplina Matemática, en particular en la asignatura Matemática III, perteneciente al programa de la carrera Ingeniería en Ciencias Informáticas, son bajos. Teniendo en cuenta que en esta asignatura el Tema de Series Numéricas y de Funciones tiene un peso curricular notable, se hace una indagación de los conocimientos previos que tienen los estudiantes respecto a las sucesiones numéricas, tema que en la asignatura Matemática Discreta, que pertenece al primer año, se introducen los elementos básicos. La indagación se hace tomando como base el papel relevante que tienen estos contenidos para un aprendizaje eficiente en el Tema de Sucesiones Numéricas y de Funciones en Matemática III. Por tanto, los autores se proponen como objetivo fundamental indagar el nivel de aprendizaje de los saberes matemáticos previos, sobre sucesiones numéricas, de los estudiantes de segundo año y que son básicos para la comprensión eficiente de los contenidos del tema de sucesiones numéricas y de funciones que aparecen en la Matemática III.

T’ikahina es un drama escrito en 1920 (fecha estimada) por el sacerdote de origen quechua Nemesio Zúñiga Cazorla, y está ambientado en la época colonial peruana. En él se relata la historia de una familia noble del Cusco que, a lo largo de la obra, cuestiona sus tradiciones ancestrales puestas en conflicto con la cultura evangelizadora. Zúñiga utiliza el teatro como una herramienta para evangelizar, tal como se hiciera en distintos lugares de América desde la conquista, pero con la particularidad de incluir diversos elementos de la cultura prehispánica. Así, el autor hace alusión tanto a situaciones cotidianas como a ceremonias rituales, destacando además el uso de un lenguaje lleno de poesía, saludos protocolares y vocabulario de raíz indígena.

Los servicios sanitarios presentan disímiles retos, uno de los problemas que ocurren diariamente en los hospitales son los cuellos de botella. Estos provocan anomalías o eventualidades en el flujo de los procesos, afectando directamente la calidad de la atención a los pacientes. Las organizaciones de salud procuran aplicar un enfoque de gestión por procesos y el uso de diferentes tecnologías con el fin de mejorar su rendimiento. La minería de procesos es una tecnología para el análisis de procesos, la cual posibilita identificar características y problemas asociados a la ejecución de los procesos desde sistemas de información. El sistema XAVIA HIS de la UCI tiene las características necesarias para contribuir a la identificación de cuellos de botella aplicando minería de procesos. La presente investigación tuvo como objetivo desarrollar e incorporar una vista de análisis para identificar cuellos de botella desde el sistema XAVIA HIS. Para ello se seleccionó y personalizó la técnica Replay a log on Petri Net for Performance/Conformance sobre dicho sistema. La vista de análisis desarrollada e incorporada al sistema, permitirá realizar análisis de la ejecución de los procesos de las instituciones sanitarias. Se espera que a partir de la propuesta de solución se beneficien los profesionales y los pacientes de la institución hospitalaria, y así incrementar la calidad de los servicios que se prestan.

This article presents the general framework of Romani-Gypsy presence in France at the beginning of the 20th century. Based on a synthesis of printed sources and regional or general descriptions, this study proposes to define the modalities of an ancient and yet unknown territorial implantation. Starting from a critique of the diasporic model applied to these populations, the analysis proposes to combine the historiography of migrations and that of itinerant mobility to rethink a presence inscribed at the crossroads of various geographical frames. The ladder game, both regional and cross-border, thus seems to define a specific morphology, based on local rooted communities and on the exercise of a current or intermittent circulation.

The Amsterdam-Zaandam cohort has been studied intermittently since 1987. The cohort was selected for optimal pregnancy and birth, in whom prenatal, lactational and more recently current dioxin exposures were measured. In the perinatal period and during the years thereafter, effects on various organ systems have been documented: thyroid, metabolism, immunity, haematology, motor development, brain development, lung function and puberty. We present a review of the endpoints studied, from the perinatal period into adolescence.

Brown tumor is a non-neoplastic lesion which resulting from abnormal bone metabolism caused by hyperparathyroidism. Many diseases affect the pleural space in both adults and children, including common diseases such as pneumonia, cancer and heart failure. Pleural effusion is the most common manifestation of pleural disease, and it is often a secondary effect of another disease process. Imaging plays a crucial role in the management of pleural disease. Chest radiography often remains the first examination in the assessment of these patients. Depending on the clinical context, the optimal imaging technique for further evaluation might be computed tomography (CT), ultrasound (US), or magnetic resonance (MR).

Erdheim-Chester disease (ECD) is a rare non-Langerhans’ cell histiocytosis. Mild but permanent juxta-articular bone pain in mainly knees and ankles is the most frequent associated symptom. Despite the pathognomonic radiographic findings, most cases are still diagnosed by the pathologist. The lesions consist of lipid-storing CD 68 +/ CD 1a – non- Langerhans’ cell histiocytes, most frequently localized in bone but also involving multiple organ systems in the body. We present a case report in which the diagnosis of ECD was established with 99mTc MDP bone SPECT/CT.

The country appearsto have adequate legislation, however, greaterconsistency of laws and decrees are required. Updatethe technical rule is necessary and increasesthe capacity to monitoring, identification andelimination of organochlorine pesticides.

A low-cost glass-assembled and easy to handleanesthetic chamber is presented, easy handlingand cleaning, as a didactic manner to explain,by using mice, the basis of the inhaled generalanesthesia to veterinary medicine students. Thechamber, allows adequate dose administration ofanesthetic, preserves life of animals and decreasesexposure of teachers and students to anestheticsgases. Thus, it’s possible to explain in a didacticfashion the basic concepts of inhaled anesthesia,such as anesthetic concentration and solubility.

Leiomyomatosis peritonealis disseminata (LPD) – or diffuse abdominal leiomyomatosis – is a very rare benign abdominal entity. Only a little more than 100 cases have been reported in the English literature since its first description in 1965. Middle aged female are typically affected and the clinical presentation is rather aspecific. The differential diagnosis between benign LPD and diffuse peritoneal carcinomatosis or abdominal disseminated malignancy represents the crucial diagnostic challenge that can only definitively be made through biopsy and histologic analysis. Multimodal imaging features (ultrasound, CT, MR and PET) of a case of LPD diagnosed in a 50-year old female are presented with review the literature.

We report the CT findings in a case of partial anomalous pulmonary venous return (PAPVR) from the left upper lobe in an adult. PAPVR is an anatomic variant in which one to three pulmonary veins drain into the right atrium or its tributaries, rather than into the left atrium. This results in a left-to-right shunt with varying clinical presentation. These can range from asymptomatic patients to advanced cardiac failure.

A 39-year old man, a smoker without significant medical history, presented with vague chest pain, cough, and hemoptysis. His symptoms had started 2 weeks earlier. Radiography showed an area of decreased lucency (ground glass appearance) in the right lower lobe. CT scan confirms the presence of a triangular area of inhomogeneous parenchymatous increased attenuation mainly of ground glass with inlying bronchocoeles and tree-in-bud appearance compatible with an intrapulmonary sequestration. The arterial supply is derived from the lower thoracic aorta. The venous drainage is to the left atrium. The hemoptysis is a result of a supratherapeutic International Normalized Ratio (INR) on Sintrom intake for atrial fibrillation (AF). Treatment of sequestration consisted of a thoracoscopic lobectomy.

A 62-year old female with a history of appendix carcinoma came to our department for an 18 F FDG PET/CT to further characterize a subcutaneous fixed small mass in the right fosse detected at clinical examination. Fourteen months earlier, she had undergone a right hemicolectomy because of an appendiceal mucinous cystadenocarcinoma. Since the diagnosis of appendicitis was presumed before she went into surgery, an explorative laparoscopy was performed. An inflammatory mass with loss of all anatomical references was found; hence the laparoscopy was reverted to a laparotomy.

In order to exclude acute pulmonary embolism, a lung perfusion scintigraphy was performed in a 53-year-old male, with a history of Fallot’s tetralogy. He had been immobilized because of a tibial fracture and complained of worsening chest pain and dyspnea.

During virtual colonoscopy, performed for severe constipation in a 77-year-old male, a mass (22 x 51 mm) was detected in the tail of the pancreas. The CT features of the lesion were compatible with a mucous cystadenoma / cystadenocarcinoma: low density (HU: 40), smooth margins and presence of micro-calcifications (Fig. A).

A routine follow-up FDG-PET/CT was performed in a 67-year-old female with a history of head and neck carcinoma. In 2006, a T1 N0 M0 squamous cell carcinoma of the mouth floor had been resected. Subsequent clinical and radiological follow-up on regular base showed no evidence of locoregional recurrence or metastatic disease. There was no actual symptomatic disease.

An 88-year-old woman was seen at the emergency department with sudden onset of abdominal pain and nausea. She had been constipated for a week. Medical history was positive for arterial hypertension, diabetes mellutis type II, and heart failure. Laboratory findings showed an elevation of cholestatic liver enzymes. CRP was within normal limits. An upright radiograph of the abdomen showed multiple air fluid levels (F) in the small bowel consistent with obstruction. Pneumobilia (short arrow) was also seen as well as an obstructing gallstone (long arrow). A CT of the abdomen was performed. This showed pneumobilia (long arrow), a small bowell faeces sign (white asterisk) in the jejenum consistent with obstruction, and a non radioopaque gallstone in the ileum (dark asterisk). A CT scan from a few years earlier shows signs of cholecystitis and the non radioopaque gall stone (dark asterisk).The patient was treated with laparoscopic enterotomy with removal of the gallstone and cholecystectomy. The cholecystoduodenal fistula was demonstrated.
