Tubulocystic renal cell carcinoma (RCC) is a rare renal cancer first recognized by the WHO in 2016 as independent disease category, characterized by its typically indolent features and low rates of metastasis. We present a 35-year-old male with tubulocystic RCC diagnosed incidentally on evaluation of flank pain. Magnetic resonance imaging showed Bosniak class 4 renal cyst, although initial computed topography showed a hypodense nonenhancing lesion classified as Bosniak 1 cyst. Patient underwent robotic assisted partial nephrectomy, histopathology confirmed as tubulocystic RCC. This case highlights the importance of considering tubulocystic RCC in the differential diagnoses of renal cysts and other solid renal masses to ensure timely and effective treatment plan.

Testicular seminomas are the most common type of testicular tumor; atypical presentations can make diagnosis more challenging, leading to delayed treatment. Here, we present a 40-year-old previously healthy male who presented to the emergency department with severe right-sided scrotal swelling and pain. He had a diagnosed but untreated inguinal hernia. Physical examination revealed a large irreducible right inguinoscrotal swelling and tenderness. Doppler ultrasound reported herniated content in the right inguinal area, extending to the scrotum. Upon surgical exploration, a large right testicular tumor was discovered, surrounded by hematoma. A radical orchiectomy was performed, and the tumor was identified as a stage pT2 testicular seminoma. This case illustrates the diagnostic challenges posed by atypical presentations of testicular seminomas. A high index of suspicion and thorough clinical and radiological assessments are crucial for accurate diagnosis and management. In conclusion, large testicular seminomas presenting with acute pain mimicking strangulated hernias are rare. Comprehensive clinical and radiological evaluations are essential to avoid misdiagnosis and ensure appropriate surgical planning and patient management.

Robotic assisted partial nephrectomy is the gold standard treatment for small renal masses. Ureteric stricture is a rare but serious complication that significantly increase the morbidity and worsens the quality of life for cancer patients. Definitive treatment such as surgical reconstruction or ureteroureterostomy is not always feasible as in patients with significant morbidity or high-risk patients. Other options include ureteric double J stent or nephrostomy tube placement with regular exchange. We present a case of iatrogenic upper ureteric stricture post robotic assisted partial nephrectomy for right renal mass that was discovered on postoperative follow up imaging treated with metallic ureteral stent (Memokath) as reconstructive surgery was difficult due to proximity to the tumor bed. We found that if reconstructive surgery is not feasible, metallic ureteral stents has good durability, better quality of life than ureteric double stents for the management of ureteric stricture.

IntroductionLower urinary tract dysfunction (LUTD) in cerebral palsy (CP) and other neuromuscular diseases can present with chronic retention that leads to hydronephrosis, recurrent urinary tract infections (UTI), and stone formation. Whenever the conservative treatment of LUTD fails for any reason, it is considered to be complicated LUTD, in which a surgical approach is warranted. Cutaneous vesicostomy (CV) is a simple, well-tolerated, and potentially reversible procedure that protects the upper tracts. We describe our experience using CV for this complex population.Materials and methodsChildren with CP and other neuromuscular diseases admitted to pediatric long-term care units for palliative care between 2015 and 2019 were included in the study. They present multi-system involvement, polypharmacy, and Gross Motor Function Classification System levels of 4 or 5. We retrospectively studied this population's indications and results of CV.ResultsOf the 52 admitted patients, 18 presented LUTD with UTI (n:18; 100%), stones (n:5; 28%), progressive hydroureteronephrosis (n:3; 17%), or stones (n:2; 11%). Conservative initial management (catheterizations, prophylaxis antibiotics) was effective in half the cases. The remaining nine were defined as complicated LUTD and underwent CV. After a mean follow-up of 11.3 months, the follow-up showed improved hydronephrosis in all nine (100%) patients. Recurrent UTIs were no longer seen in eight of nine patients, although three patients required bladder irrigations; bladder stones did not recur after CV; the kidney stones needed further intervention. Revision of the CV was required in two (11%) cases at 12 and 24 months postoperatively due to stoma stenosis.ConclusionCV is a relatively simple and effective procedure representing a pragmatic solution for managing complicated LUTD in complex long-term institutionalized pediatric palliative care patients with neuropathic bladders.

Purpose: Isolated trapezoid fractures are rare injuries, particularly among adolescents, constituting only 0.4% of all carpal bone fractures. This study aims to present two cases of isolated trapezoid fracture in adolescent goalkeepers and a scoping review of the literature to provide guidelines for the management of this injury. Methods: Following PRISMA-ScR guidelines, a scoping review of reported cases was conducted. Two hundred and twenty articles were found using PubMed and Google Scholar. After full-text review, a total of 30 cases from 22 articles along with our 2 cases were analyzed based on demographics, injury mechanism, method/timing of diagnosis, prognosis, and time to recovery. Results: Thirty-two reported cases of trapezoid fractures with a mean age of 26.7 years (75% male) were found, with pain as the most common presenting symptom. A majority (78%) had initial negative findings on radiography, and the diagnosis was primarily established through computed tomography (59%; n = 19) or magnetic resonance imaging (50%; n = 16). There was a substantial delay in diagnosis (mean 26 days), primarily because computed tomography/magnetic resonance imaging was frequently ordered late. The majority of cases (78%) were managed conservatively, with immobilization periods ranging from 4 to 12 weeks. The average duration for full recovery was 4.5 months, with operative management taking 7.3 months and conservative management taking 3.5 months. Conclusion: Trapezoid fractures, though rare, are often not promptly diagnosed on initial plain radiographs, leading to a potential underreporting of cases. Because of the risk of complications associated with this type of injury, clinicians should maintain a high level of vigilance and consider trapezoid fracture as a possible differential diagnosis when presented with carpal pain, swelling, or limited movement, particularly after axial load incidents. Further research and guidelines are needed to enhance our understanding and management of this uncommon injury in the future. Ureteral intussusception is a rare condition that historically occurs as a complication of ureteral neoplasms or iatrogenic endoscopic procedures. Although the exact mechanism of ureteral intussusception is unclear, most reported cases are due to leading points as malignant or benign masses. Urolithiasis related is rarely reported and can be challenging in stone management as it might decrease the spontaneous stone passage rate. In addition, it will increase the complexity of the endoscopic stone management. We present the second reported case of urolithiasis-related ureteric intussusception presented with urosepsis due to obstructive uropathy, successfully managed by an endourological approach.

The use of cryoablation in the management of small renal masses is widely acceptable. Although rare but ureteral injury during the procedure with subsequent stricture formation can result in devastating effects on renal function. On the other hand, the management of such strictures requires reconstructive surgery as gold standard. Unfortunately, in some cases the reconstructive surgery might not be feasible, and the treatment usually is ureteral stent insertion that need to be changed regularly. Here we present a case of a 53-year-old gentleman who developed an upper ureteric iatrogenic stricture post cryoablation in which the reconstructive surgery was not feasible due to high procedural risk. We used metallic ureteral stent (Memokath) instead of regular ureteral double J stent. We found that if the reconstructive surgery is not possible the usage of Memokath in treating iatrogenic ureteral strictures is associated with better quality of life, lower costs and a similar functional outcome when compared to ureteral double J stent that needs regular frequent changes.

Male pelvic cyst rarely causes symptoms; here, we are presenting a case of a 48-year-old gentleman who presented with acute urinary retention stemming from a pelvic cyst. This presentation has been recurrent despite undergoing repeated TRUS aspiration of the cyst to relieve the symptoms. We performed a robotic pelvic cyst excision with peritoneal window in an attempt to cure the patient. Based on the MRI and histopathology, it was likely a seminal vesicle cyst that is causing these recurrent episodes. On 3 months follow-up, the patient was symptom free without any complaints.

Testicular Large cell calcifying Sertoli cell tumours (LCCSTs) are extremely rare. The primary challenge in benign LCCSTs, which are typically multifocal and bilateral tumours affecting young males, is to confirm the diagnosis to avoid radical intervention and preserve fertility potential. Patient clinical presentation, laboratory results, diagnostic radiological tests along with confirmatory histopathological studies, are the cornerstones in such cases, nevertheless genetic testing is warranted, as LCCSTs can be part of genetic syndrome such Carney complex. We present a case of bilateral benign LCCSTs in young male managed with testicular preservation approach with characteristic clinical, radiological and histopathological features.

The bladder is the most common site of foreign bodies in the urinary tract. Most foreign bodies are self-inserted via the urethra due to exotic impulses, psychometric problems, or sexual curiosity. Here we present a rare case of bladder stones due to the migration of the Heme-o-lok clip. We present a case of a 76-year-old male with hematuria for 4 days. An abdominal computed tomography (CT) scan showed a 15 mm calculus noted in the urinary bladder. The patient underwent cystolitholapaxy which was successful. Foreign bodies inserted in the bladder pose a significant challenge and require timely intervention.

Scrotal pain and swelling are common presentation, the prompt evaluation and diagnosis is needed due to wide range of causes, uncommon causes of orchialgia should be kept in mind whenever there is no clear diagnosis. Spermatic vein thrombosis usually presents with orchialgia along with episodes of acute exacerbation of pain. The diagnosis is challenging and need high index of suspension and detailed radiological evaluation. We present a case of metachronous bilateral unprovoked spermatic vein thrombosis treated conservatively with anticoagulation with good response and resolution of symptoms.

Bladder noninvasive squamous lesions are usually rare; here, we are presenting a case of 39 years old male patient with a benign squamous cell papilloma. The tumor grossly presented as cauliflower mass mimicking squamous cell carcinoma, while histologically, the tumor showed extensive keratinization at its surface and showed no nuclear atypia or stromal invasion. This tumor is benign and extremely rare.In this manuscript, we summarized a case of Squamous cell papilloma of the bladder with the first operative video for the transurethral resection of squamous cell papilloma with percutaneous management of associated urinary bladder stones.

Abstract Background Oral rehydration solution (ORS) is a simple intervention that can prevent childhood deaths from severe diarrhea and dehydration. In a previous study, we mapped the use of ORS treatment subnationally and found that ORS coverage increased over time, while the use of home-made alternatives or recommended home fluids (RHF) decreased, in many countries. These patterns were particularly striking within Senegal, Mali, and Sierra Leone. It was unclear, however, whether ORS replaced RHF in these locations or if children were left untreated, and if these patterns were associated with health policy changes. Methods We used a Bayesian geostatistical model and data from household surveys to map the percentage of children with diarrhea that received (1) any ORS, (2) only RHF, or (3) no oral rehydration treatment between 2000 and 2018. This approach allowed examination of whether RHF was replaced with ORS before and after interventions, policies, and external events that may have impacted healthcare access. Results We found that RHF was replaced with ORS in most Sierra Leone districts, except those most impacted by the Ebola outbreak. In addition, RHF was replaced in northern but not in southern Mali, and RHF was not replaced anywhere in Senegal. In Senegal, there was no statistical evidence that a national policy promoting ORS use was associated with increases in coverage. In Sierra Leone, ORS coverage increased following a national policy change that abolished health costs for children. Conclusions Children in parts of Mali and Senegal have been left behind during ORS scale-up. Improved messaging on effective diarrhea treatment and/or increased ORS access such as through reducing treatment costs may be needed to prevent child deaths in these areas.

Mathematical creative thinking skill often becomes the orientation of mathematics learning, aiming to enhance students' creativity in mathematics. Recognizing that creativity encompasses the capacity for thinking creatively and creativity disposition is essential. Building on this conceptual foundation, the primary objective of this study is to develop a comprehensive model illustrating the relationship between students' aptitude for mathematical creative thinking and their creative disposition. The research methodology employed in this study aligned with the framework of cause-and-effect analysis. The study cohort consisted of 36 students, carefully selected by a cluster random sampling technique. The research instruments included a mathematical creative thinking ability assessment and a creative disposition scale. The data was analyzed using the Non-Recursive Structural Equation Modeling. The results showed the reciprocal cause-and-effect dynamic between mathematical creative thinking ability and creative disposition, exhibiting a mutually influential relationship with determination coefficients of 21.83% and 21.05%. This shows that mathematical creative thinking ability is better at explaining mathematical creative disposition than mathematical creative disposition explaining mathematical creative thinking ability, with a relatively small difference (0.78%). This study also concluded that an optimal approach to mathematics pedagogy entails a balanced and simultaneous focus on nurturing mathematical creative thinking ability and disposition.

Abdominal compartment syndrome is a life-threatening complication of conventional percutaneous nephrolithotomy (PCNL), with few cases reported in different positions. We present the first case of abdominal compartment syndrome as a complication of supermini percutaneous nephrolithotomy (SMP) in The Galdakao-modified supine Valdivia position, possibly predisposing factors, diagnosis, and management.Although it is a challenging diagnosis and life-threatening condition, morbidity and mortality can be decreased with early detection and drainage of the intra-peritoneal fluid, causing increased abdominal pressure, which is the most important prognostic factor.

This study aimed to identify the means and reality of treating learning loss in mathematics at the intermediate stage. The study adopted a mixed methods approach and used the following tools: questionnaire, individual interviews, and collective interviews. There were 40 male and female teacher participants in the first stage and 25 teachers in the second. Among the many findings, the most salient were that the concept of learning loss differs among mathematics teachers and they devote part of their teaching to the treatment of learning loss using different treatment mechanisms. In particular, they reported "always" using teaching practices supporting the affective aspect of treating learning loss and their satisfaction with their teaching practices concerning learning loss was "average." The study also identified several challenges that limit the treatment of learning loss related to the teacher, the student, and the family. Taking these findings into consideration, the research developed some recommendations and proposals, the most important being enhancing teachers' awareness of learning loss, showing successful experiences in treating learning loss, studying the reality of treatment in different educational stages, and adopting teaching models for treating learning loss in mathematics.

Introduction: The parathyroid gland is a small endocrine gland that appears as small oval-shaped bodies with a brown color. It is called the parathyroid gland due to its location closely with the thyroid gland. Materials and Methods: The study used six adult males of Felis catus from the local markets in Baghdad. Formalin (10%) was used to fix the samples for preparing all specimens to conduct histological study. They were passed in an alcohol ascending series 70, 80, 90 and 100% and cleared by xylene. Masson's Trichrome stain (MTc), Haris hematoxlin and Eosin stain (H&E), Periodic Acid Schiff stain (PAS), were used to stain the gained sections. Results: The current study observed the presence of two pairs of parathyroid glands in Felis catus animal, which are in contact with the thyroid gland, occupying an apical peripheral site and a lateral peripheral site, and located within the thyroid gland tissue embedded in its substance. Moreover, the gland appears as lobes consist of 2-4 with oval or irregular in shape and surrounded with a thin capsule. A capsule of connective tissue surrounds the parathyroid gland in F. catus is an extension of the capsule of the thyroid gland consisting of collagen, elastic and reticular fibers, nuclei of smooth muscle fibers, blood vessels, and nerves. The septa are extended from the capsule which divides the gland into lobules, the parathyroid gland tissue has three types of cells represented by chief cells, which are the most common cells, while the other two types are Oxiphyl cells and water-clear cells. Conclusion: The current study Felis catus, observed the presence of two pairs of parathyroid glands in Felis catus animal, which are in contact with the thyroid gland. Moreover, the gland appears as lobes consist of 2-4 with oval or irregular in shape and surrounded with a thin capsule, and their internal histological structure consists of three cell kinds: chief, Oxphill, and water clear cells. Introduction: The parathyroid glands have great importance in the human physiology and anatomy due to that produce hormones necessary for growth and body functions. Materials and Methods: the parathyroid gland samples were collected from S. anomalus from 5 adult animals (males) from Baghdad Governorate's local markets, The specimens were fixed using formalin (10%), and the samples were washed well using ethyl alcohol (70%) for the removal of the fixative solution, and then specimens were dehydrated in ascending grades of ethyl alcohol (70%, 80%, 90%, 100%). Xylene was used to clear the samples which were placed in paraffin wax in an oven at 58-60° C . The prepared sections were stained using Haris Haematoxlin and Eosin (H&E) stains & Masson trichrome (MTC). Results: This study has proven the presence of two pairs of parathyroid glands in S. anomalus, which is located in contact with the thyroid gland and occupies two sites within its tissue, an apical site as well as embedded within the thyroid tissue. The gland appears as an oval or irregularly shaped lobe which thin connective tissue capsule surrounds and is an extension of the thyroid capsule, and septa extend from it to the internal tissue, dividing it into incomplete lobules. In addition, the capsule consists of collagen fibers, elastic fibers, a few reticular fibers, and nuclei of smooth muscle fibers. Histologically, the parathyroid gland consists of three types of cells: chief cells, which are the most common, oxphill cells, and water clear cells. These two types of cells are arranged in the form of columns or cell cords In this study, While the third type of cells are water clear cells, which is the few cell in comparison to the other types. Conclusion: The parathyroid glands are located on both sides of the thyroid glands in adult males of the S. anomalus. A connective tissue capsule surrounds parathyroid glands, and their internal histological structure consists of three cell kinds: chief, Oxphill, and water clear cells. [ABSTRACT FROM AUTHOR]
