Objective: The objective of this study was to review the impact of an automatic email notification to infectious disease consultants. Design: Cases were identified from a community hospital system microbiology database by at least one positive blood culture. Records were reviewed both before (2013 and 2014) and after (2015 and 2016) the implementation of the automated email system (intervention). Prior to this policy, consultation with the infectious disease (ID) specialist was at the discretion of the primary team. Results: There were no significant differences in 30-day mortality between the two groups (18 vs 20%, p = 0.10). However, a trend of shorter hospital stays (12.2 vs 9.5 days, p = 0.03) and reduced 30 day readmissions (40% vs 19%, p = 0.03) was observed and antibiotics prescribed for complicated cases was more appropriate (57% vs 85%, p = 0.004). Conclusions: In this study population, the implementation of an automatic email generator to ID specialists was associated with a shorter hospital length of stay, less 30-day readmissions and more appropriate length of antibiotics prescribed in complicated cases of SAB. The authors recommend future studies replicating the methodology employed here with larger sample sizes before consideration of employing a similar automatic email ID consult generation at other health systems.

Candiduria are common findings in clinic especially in hospitalized patients, while its significance remains undetermined. Since there are few criteria to follow, physicians tended to make decisions by personal experience in many cases in clinical practice. The present study was designed to unveil the present situation of candiduria management in hospitalized patients in clinical practice. A total of 251 hospitalized candiduria patients were retrospectively enrolled in the study. Clinical data on patient demographics, basic conditions, catheter using, urinary symptoms, laboratory data, and antifungal therapies were obtained from electronic medical records. The high rate of the candiduria cases were managed inappropriately after the introduction of the Infectious Diseases Association of America (IDSA) evidence-based recommendations, both in the management of urinary catheter and antifungal agents. Overtreatment was common in asymptomatic candiduria patients. For symptomatic patients, improper drug selections were not rare. In addition, a part of candiduria patients did not receive antifungal therapies although the IDSA recommends. A statistically significant difference was only found in hospital charges of symptomatic candiduria patients managed following IDSA or not. The recurrence rate, mortality, and hospital stay length were similar in candiduria patients regardless of the clinical management. Physicians tend to start empiric antifungal therapy for candiduria patients with pneumonia, multisite of Candida colonization, higher urine Candida CFUs, and long hospital stay. Candiduria has not received special attention today, and empirical antifungal treatment is common. IDSA guidelines are important to standardize the management of candiduria in clinic; however, the significance of the guidelines needs to be further clarified in future multicenter investigations. Background: The Infectious Diseases Society of America (IDSA) has published guidelines indicating the criteria for brain imaging before lumbar puncture (LP) among patients with community-acquired meningitis (CAM). However, data on adherence to the guidelines and associated outcomes are currently limited. Methods: We conducted a prospective observational study among patients with CAM from January 2018 to March 2019 in the emergency department (ED) of a tertiary-care hospital in Thailand. Physicians' IDSA guidelines adherence rate for brain imaging before LP was determined. Clinical outcomes were compared between patients undergoing the procedures according and not according to the guidelines. Results: Of the 101 patients screened, 69 were included. The physicians' guidelines non-adherence rate for brain imaging before LP was 38%. The most common non-adherent practice was performing brain imaging despite no indication (96%). By multivariable logistic regression analysis, the only independent factor associated with non-adherence to the guidelines was caring patients with no indications for brain imaging before LP (P<0.001). The patients in the guidelines-adherent group were more-likely than those in the guidelines-non-adherent group to have underlying AIDS and present with seizure, while the 30-day survival rates were not different between the two groups (88% vs. 85%). Conclusions: Our study suggests a significant non-adherence to the guidelines due to the over- investigation of the physicians in patients with no indications for brain imaging before LP. Interventions to improve physicians' knowledge about these indications and practice are needed for better care of patients with CAM. [ABSTRACT FROM AUTHOR]

Summary: Cryptococcosis is an invasive fungal infection caused primarily by Cryptococcus neoformans and Cryptococcus gattii species, presenting predominantly as meningoencephalitis. The aim of this study is to assess all cryptococcal infections managed at our facility from 2001 to 2015 to determine incidence, risk factors, and comparison of outcomes prior to and following introduction of the 2010 Infectious Disease Society of America (IDSA) guidelines. Retrospective analysis of all patients diagnosed and treated for cryptococcal infection occurring between January 2001 and December 2015. Of 102 patients diagnosed with cryptococcal infection, 97 were eligible for study inclusion. There appears to be an overall increased incidence of cryptococcosis in both transplant and non‐transplant cohorts with a peak in 2015 of 6 transplant and 13 non‐transplant cases. In the meningitis cohort, 38/52 (73%) of identified isolates were C. neoformans, and 14/52 (27%) were C. gattii. Notably, 14/14 (100%) of C. gattii isolates were associated with meningitis, as compared to only 38/64 (59%) C. neoformans associated with meningitis (P: .003). It appears that patients presenting with cough are less likely to have meningitis, 17/27 (63%), (P: .005). When stratifying for culture positive meningitis lumbar puncture opening pressure, the median in the culture positive cohort was 31.5 cm H2O compared with 15.5 cm H2O (P: .036).Multiple admissions were required prior to diagnosis in the majority of cases with only 18/72 (25%) diagnosed on 1st presentation. Postguideline mortality has improved from 15% to 6.1% (P: .046). Cryptococcal infection remains relatively uncommon, but there appears to be an increasing trend in incidence. Purpose. A protocol to optimize the duration of antimicrobial therapy (DAT) for uncomplicated pneumonia at hospital discharge was evaluated. Methods. This retrospective quasiexperimental study was conducted at Boise Veterans Affairs Medical Center from March 2013 through June 2015. Patients were included in the study if they were diagnosed with pneumonia, were hospitalized for more than 24 hours, received antimicrobial treatment within 48 hours of admission, and survived until hospital discharge. The intervention included development of a pneumonia DAT triage algorithm, a process for assessment of the appropriate DAT by pharmacists, and recommendations to providers to limit excessive discharge DATs prescribed. Interrupted time-series analysis was performed to determine the mean monthly DAT per patient and the 30-day readmission rate. Results. Of the 707 patients discharged with a diagnosis of pneumonia, 560 met the criteria for study inclusion (366 in the preimplementation group and 194 in the postimplementation group). Change in slope of monthly mean DAT per patient postimplementation was significantly reduced (p = 0.03) from the preimplementation slope (p = 0.95), indicating an association between the intervention and mean DAT per patient. The intervention was not associated with the 30-day readmission rate. The mean ± S.D. DAT decreased from 9.5 ± 2.4 days preimplementation to 8.2 ± 2.9 days postimplementation, primarily due to the reduction of outpatient DAT from 5.2 ± 3.0 days preimplementation to 4.2 ± 3.0 days postimplementation. Conclusion. A pharmacy-based triage algorithm helped to reduce excessive DATs for patients with pneumonia at hospital discharge without negatively affecting 30-day readmission rates. Abstract: Treatment of candidemia is more complicated because of the changing epidemiology of Candida and introduction of newer antifungal agents. Utilization and benefit of practice guidelines and infectious disease consultation for the management of candidemia has not been previously described in the routine clinical setting. We prospectively studied the impact of the Infectious Disease Society of America (IDSA) guidelines for the management of candidemia and infectious disease consultation on clinical outcomes in 119 patients with candidemia at a tertiary care hospital. Medical records were reviewed to capture data concerning use of antifungal agents, management of central venous catheters, and infectious disease consultation. Initial antifungal therapy was consistent with the IDSA guidelines in 76% of patients. Variation from the guidelines was independently associated with higher mortality (24% versus 57%, P = 0.003). Infectious disease consultation was independently associated with lower mortality (18% versus 39%, P &#60; 0.01). Use of the IDSA guidelines and infectious disease consultation service was found to improve patient outcomes in patients with candidemia at our institution. Further studies should be performed to validate newer guidelines in a clinical setting at other institutions. [Copyright &y& Elsevier]

Aim: Recent guidelines recommended removing metronidazole as a therapeutic option for Clostridioides difficile infections (CDI). However, superiority of vancomycin over metronidazole in mild CDI is not established and use of vancomycin might lead to emergence of vancomycin-resistant enterococci (VRE). Patients & methods: A retrospective cohort study and efficacy analyses were conducted at Shamir Medical Center, Israel (2010-2015), among adults with acute CDI. Results:  A total of 409 patients were enrolled. In multivariable analyses, metronidazole was noninferior to vancomycin for mild CDI, but vancomycin was an independent predictor for post-CDI VRE acquisition. Conclusion: A significant independent association was evident between treatment with vancomycin and, later, acquisition of VRE. In first episodes of mild acute CDI, metronidazole should be considered a valid therapeutic option.

Candiduria is common in hospitalized patients, and asymptomatic candiduria contributes to antifungal overuse. The guidelines for management of asymptomatic candiduria do not recommend antifungal use, but rather the elimination of predisposing factors. It is unknown whether these recommendations are being followed. The primary objective of this study was to characterize candiduria management among hospitalized patients. This was a retrospective cohort study of a random sample of 305 hospitalized patients with candiduria at four U.S. medical centers from January 2010 to December 2013. Patients were classified as asymptomatic or symptomatic based on established criteria, and data were collected by chart review. Infectious Diseases Society of America (IDSA) treatment guideline adherence and its association with clinical outcomes, including candiduria recurrence (short- and long-term) and 30-day readmission, were assessed. Eighty percent of patients were classified as having asymptomatic candiduria. Overall, 143 (47%) patients were not managed according to recommended guidelines, including 105/243 (43%) in the asymptomatic candiduria group and 38/62 (61%) in the symptomatic group ( P = 0.01). Discordance among asymptomatic patients was driven by overtreatment with an antifungal (98/105 [93%]). Thirty-three percent of patients with asymptomatic candiduria not managed according to the guidelines were treated for over 7 days, and 5% received over 14 days of therapy. Fluconazole was the most commonly used empirical antifungal among asymptomatic candiduria patients (96%), followed by micafungin (4%). Asymptomatic candiduria patients not managed according to the guidelines had a trend toward higher 30-day readmission (35% versus 26%, P = 0.27).
