1. Evaluation of the anticonvulsant and neuroprotective effect of intracerebral administration of growth hormone in rats

Abstract

Introduction: The growth hormone (GH) has been reported as a crucial neuronal survival factor in the hippocampus against insults of diverse nature. Status epilepticus (SE) is a prolonged seizure that produces extensive neuronal cell death. The goal of this study was to evaluate the effect of intracerebroventricular administration of GH on seizure severity and SE-induced hippocampal neurodegeneration. Methodology: Adult male rats were implanted with a guide cannula in the left ventricle and different amounts of GH (70, 120 or 220 ng/3 μl) were microinjected for 5 days; artificial cerebrospinal fluid was used as the vehicle. Seizures were induced by the lithium–pilocarpine model (3 mEq/kg LiCl and 30 mg/kg pilocarpine hydrochloride) one day after the last GH administration. Neuronal injury was assessed by Fluoro-Jade B (F-JB) staining. Results: Rats injected with 120 ng of GH did not had SE after 30 mg/kg pilocarpine, they required a higher number of pilocarpine injections to develop SE than the rats pretreated with the vehicle, 70 ng or 220 ng GH. Prefrontal and parietal cortex EEG recordings confirmed that latency to generalized seizures and SE was also significantly higher in the 120 ng group when compared with all the experimental groups. FJ-B positive cells were detected in the hippocampus after SE in all rats, and no significant differences in the number of F-JB cells in the CA1 area and the hilus was observed between experimental groups. Conclusion: Our results indicate that, although GH has an anticonvulsive effect in the lithium–pilocarpine model of SE, it does not exert hippocampal neuroprotection after SE. Resumen: Introducción: La hormona de crecimiento (HC) es un factor que favorece la supervivencia neuronal en el hipocampo ante agresiones de diversa naturaleza. El status epilepticus (SE) es un tipo de crisis epiléptica de larga duración que produce muerte neuronal. El objetivo de este estudio fue evaluar el efecto de la administración intracerebroventricular de HC en la severidad de las convulsiones y la neurodegeneración hipocampal debida al SE. Metodología: A ratas macho adultas se les implantó una cánula guía en el ventrículo lateral izquierdo y se les microinyectaron diferentes cantidades de HC (70, 120 o 220 ng/3 μl) durante 5 días; como vehículo se inyectó líquido cefalorraquídeo artificial. Las convulsiones se generaron con el modelo de litio-pilocarpina (3 mEq/kg LiCl y 30 mg/kg clorhidrato pilocarpina) un día después de la última inyección de HC. La neurodegeneración se identificó con la tinción de Fluoro-Jade B (F-JB). Resultados: Las ratas a las que se les inyectaron 120 ng de HC requirieron 2 o 3 inyecciones de pilocarpina para desarrollar SE, en comparación con el resto de los grupos experimentales que requirieron solo una aplicación del convulsivante. Los registros EEG de la corteza prefrontal y parietal confirmaron que la latencia a las crisis generalizadas y al SE fue mayor en dicho grupo experimental. Todas las ratas con SE presentaron células positivas al FJ-B en el área CA1 e hilus del hipocampo, y no se identificaron diferencias entre los tratamientos. Conclusión: Nuestros resultados muestran que, aunque la HC tiene un efecto anticonvulsivante, una vez que se ha desarrollado el SE no promueve neuroprotección en el hipocampo.

Abstract

Watershed health refers to the maintenance of the normal status of a watershed. With the use of Geographic Information Systems (GIS), a watershed health assessment framework was developed and applied to the Manila Bay Watershed (MBW) based on the sub-indices used by the US EPA and the Minnesota DNR. These three sub-indices were used and modified: geomorphology, connectivity, and hydrology. Geomorphology sub-index accounted for the effects of soil erosion using the Unit Stream Power-based Erosion/Deposition model (USPED). Connectivity sub-index considered the connection between habitats within three environments: terrestrial, aquatic, and riparian zone. Hydrology sub-index accounted for the effects of impervious cover and urbanization on the movement of water using three factors, namely, natural cover, tree cover, and loss of hydrologic storage. Land cover map was the most used dataset in this scorecard, where the maintained natural land covers generally received high scores and built-up areas received the lowest. The overall watershed health score of MBW is 75.713 from the mean of the three sub-indices. Pampanga River Basin, which is the largest river basin within the MBW, got the highest score of 79.462 since it consists of huge portions of maintained natural land cover. Manila River Basin, known to have dense built-up areas, got the lowest average of 60.773. On the provincial level, the province of Nueva Ecija got the highest score, and the National Capital Region (NCR) got the lowest. The developed framework successfully quantified a relative health score which can be used to rank and prioritize subwatersheds, and to measure in totality the improvement or degradation of subwatershed/s over time.

BackgroundEthnic-racial identity (ERI) development refers to how individuals’ experiences, beliefs, and attitudes influence understanding of ethnic-racial group membership. Messages about race, from multiple ecosystems, influence identity development and how individuals come to form their ERI. There has been a shift in ERI research to focus on Multiracial populations, however, most of the research focus is on Black/white biracial and general, non-specified Multiracial populations. The ERI development process and experience for persons of other Multiracial backgrounds (e.g., AfroLatinx or AsianBlack) is not as extensively studied. This systematic literature review aims to elucidate the existing conceptualization of Multiracial ERI development for non-Black/white biracial and general Multiracial populations in the United States.MethodsA comprehensive search strategy was employed across multiple academic databases to identify relevant studies based on explicit inclusion criteria. The initial search resulted in 1,846 articles, but when only Black/white biracial and non-specified general Multiracial studies were eliminated from this review, only 18 articles met the criteria for inclusion.ResultsCommon themes emerged from the reviewed literature, including the importance of spaces, conflicting social messages directed at Multiracial individuals, and coping responses used by Multiracial individuals when faced with challenges by family members and peers regarding their multiracial identity.DiscussionThe findings underscore the need for a more nuanced exploration of ERI development among diverse Multiracial populations. Understanding the unique strengths, experiences, and challenges of different Multiracial populations beyond the Black-white biracial paradigm is essential for understanding ERI development across and between different Multiracial populations in today’s world.

Abstract

La producción de zarzamora es un tema que ha tomado relevancia con el paso del tiempo debido al incremento en la popularidad de este frutillo, cada día su uso se extiende a más mercados. El hecho de que su producción requiera de condiciones geográficas y de cuidados específicos restringe la cantidad de oferta en el mercado internacional. No obstante, cabe resaltar que México presenta las características apropiadas para su siembra y cosecha. Debido a lo anterior, el objetivo de este trabajo es determinar la rentabilidad y ventaja comparativa de la producción de la zarzamora mexicana para el año 2021, a través de la Matriz de Análisis de Política (MAP). Una limitación del trabajo es el periodo de tiempo seleccionado que, en este caso, corresponde al año 2021. Dentro de las principales conclusiones se destaca que con un valor de 0.3 y un valor de 0.53 se puede deducir que la producción de estos frutillos es una actividad competitiva y con ventaja para los productores mexicanos.

Abstract

Economy, energy and environmental quality in APEC, 1990-2018 Humanity faces the challenge of assure the well-being of the population while preserving the environmental quality. Therefore, this research aims to identify the relationship between economic growth, renewable energy consumption, fossil energy consumption and carbon dioxide emissions in the APEC economies during 1990-2018. Applying tests of cross dependence, intercept heterogeneity and unit root, were estimated as a model of Common Correlated Effects, as a novel proposal in this type of studies. The results show an inverted U-shaped trajectory between economic development and environmental pollution. It is noteworthy that a negative relationship was found between CO2 and renewable energy consumption and a positive relationship between CO2 and fossil energy consumption as well as the expected relationship between GDP, GDP2 and CO2. One of the shortcomings is the fact that the results are displayed as a whole for the economies in APEC and not for each individual economy. However, the methodology is a novelty for these type of research. It is recommended to increase the participation of renewable energies, to improve their efficiency and to promote their implementation as an effective means of reducing emissions.

Abstract

The production of berries, one of which is the blueberry, is a topic that has become relevant over time, due to the increase in the popularity of this strawberry. Every day, its use extends to more markets; The fact that their production requires specific geographical conditions and care restricts their supply on the international market. It is noteworthy that Michoacán has the appropriate characteristics for planting and harvesting. Therefore, the objective of this work is to determine the profitability and comparative advantage of Mexican blueberry production in Michoacán for the year 2023 through the Policy Analysis Matrix (MAP). A limitation of the work is the selected period of time, which in this case corresponds to the year 2023. Among the main conclusions, it stands out that with a value of 0.73 and another of 0.41, it can be deduced that blueberry production is a competitive activity and with an advantage for Michoacan producers. Abstract

Reliable estimates of soil moisture and other field observations (e.g., precipitation, irrigation) are critical to quantify the seasonal variability of surface water and groundwater relationships. This is especially important in pasture-based agroecosystems that rely on surface water diversions and precipitation inputs for agricultural production. The objectives of this study were to (1) quantify soil water balance components in irrigated and non-irrigated pasture fields in western Oregon, USA and (2) evaluate soil moisture and shallow aquifer recharge relationships in irrigated vs. non-irrigated pasture fields. Four monitoring stations in each field were used to measure soil water content in the upper 0.8 m profile and shallow groundwater levels. A soil water balance (SWB) approach was used to determine deep percolation based on field measurements of several other hydrology variables (e.g., irrigation and soil moisture). The water table fluctuation method (WTFM) was used to estimate shallow aquifer response to irrigation and precipitation inputs. Results from this study add to the understanding of seasonal water transport through the vadose zone and into the shallow aquifer in agroecological systems with fine-textured soils in the Pacific Northwest region of the United States.

Abstract

Quantifying soil water budget components, and characterizing groundwater recharge from irrigation seepage, is important for effective water resources management. This is particularly true in agricultural fields overlying shallow aquifers, like those found in the Willamette Valley in western Oregon, USA. The objectives of this two-year study were to (1) determine deep percolation in an irrigated pasture field with clay soils, and (2) assess shallow aquifer recharge during the irrigation season. Soil water and groundwater levels were measured at four monitoring stations distributed across the experimental field. A water balance approach was used to quantify the portioning of different water budget components, including deep percolation. On average for the four monitoring stations, total irrigation applied was 249 mm in 2020 and 381 mm in 2021. Mean crop-evapotranspiration accounted for 18% of the total irrigation applied in 2020, and 26% in 2021. The fraction of deep percolation to irrigation was 28% in 2020 and 29% in 2021. The Water Table Fluctuation Method (WTFM) was used to calculate shallow aquifer recharge in response to deep percolation inputs. Mean aquifer recharge was 132 mm in 2020 and 290 mm in 2021. Antecedent soil water content was an important factor influencing deep percolation. Study results provided essential information to better understand the mechanisms of water transport through the vadose zone and into shallow aquifers in agricultural fields with fine-textured soils in the Pacific Northwest region in the USA.

Abstract

Rituximab hypersensitivity reactions are rare but are one of the main causes of rituximab elimination from antilymphoma immunochemotherapy treatments. While the clinical picture may be indistinguishable from other infusion-related reactions, hypersensitivity reactions (HSR) do not disappear and instead become more intense with subsequent administrations. Objective. To describe the use of the 12-step protocol for desensitization to intravenous rituximab in clinical practice and the complementary study of a possible IgE-mediated HSR in the context of B-cell lymphoma treatment. Methods. A 12-step rituximab desensitization protocol was performed prospectively within clinical practice in 10 patients with a history of severe infusion reactions or in patients who had a repeated reaction at subsequent doses despite taking more intense preventive measures. Skin prick tests were performed at the time of reaction and at a later time to eliminate false negatives due to possible drug interference. Results. Overall, with the desensitization protocol, 70% of patients were able to complete the scheduled immunochemotherapy. Two patients had to discontinue the therapy due to clinical persistence and the third due to lymphoma progression. Intradermal tests with 0.1% rituximab were positive in only 20% of cases, demonstrating a mechanism of hypersensitivity. Conclusions. The 12-step desensitization protocol is very effective and assumable within healthcare practice. There is a need to determine the mechanism underlying the infusion reaction in a large proportion of cases due to the risk of future drug exposure.

Introduction: Multiple studies of attention deficit hyperactivity disorder (ADHD) have recognised a heritability factor in that a higher frequency of the disorder is observed in children with an affected relative. Our aim was to determine the association between ADHD symptoms in young children enrolled in five schools in Bogota and a history of ADHD symptoms in their parents using the Wender-Utah Rating Scale. Methodology: Case–control study of participants selected according to DSM-IV criteria for ADHD and the Behavioral Assessment System for Children (BASC) completed by parents and teachers; the WISC-IV scale was used to exclude children with cognitive deficit. Parents completed the Wender-Utah Rating Scale to retrospectively identify any ADHD symptoms in childhood. A score of 36 was used as a cut-off point. Results: The study included 202 children: 117 cases and 85 controls. A positive history of ADHD symptoms in childhood was identified for 16% of 175 mothers and 20.6% of 141 fathers. The presence of symptoms in either parent, especially the mother, constitutes a significant risk factor for ADHD in children and this relationship persists after controlling for different variables. If both parents have the disorder, the risk tends to increase. Conclusion: Although ADHD has been linked to a genetic component, other environmental factors may be involved in the disorder. The Zamora Valley is located in Michoacan state, Mexico, it is worldwide recognized locality because of its high performance agricultural production, specialy concerning strawberry export. The current study analyzed the agricultural productivity impact, productive resources, government programs and the company relationship on the Zamora´s international competitiveness. Taking in account that the productive factors, considered in the Marxist theory and the intensity of the Heckscher-Ohlin´s productive factors, which refer to land, capital and work, whereas they are the base from which productivity is generated, it is highlighting that in the current economic system, they need complementary intangible factors.

In order to demonstrate the capabilities of this system we will also deploy a SIEM tool and a Windows computer to simulate a possible setup that we will attack. Abstract

The maximum power point tracking (MPPT) problem has attracted the attention of many researchers, because it is convenient to obtain the maximum power of a photovoltaic module regardless of the weather conditions and the load. In this paper, a novel control for a boost DC/DC converter has been introduced. It is based on a sliding mode controller (SMC) that takes a current signal as reference instead of a voltage, which is generated by a neuronal reference current generator. That reference current indicates the current (IMPP) at the maximum power point (MPP) for given weather conditions. In order to test the designed control system, a photovoltaic module model based on a second artificial neuronal network (ANN) has been obtained from experimental data gathered during 18 months in the Faculty of Engineering Vitoria-Gasteiz (Spain). We have analyzed the performance of such model and we found that it is very accurate (MSE = 0.062 A and R = 0.991 with test dataset). We also have tested the performance of the overall SMC design with both simulated and real tests, concluding that it guarantees that the power in the output of the converter is very close to the power of the photovoltaic module output.

Abstract

In North Florida, increasing nitrogen loads and water quality declines have become a major concern, in part as result of anthropogenic non-point source activities such as agriculture. The main objective of this study was to investigate the effect of irrigation strategies and nitrogen (N) fertility rates on maize biomass, yield and water productivity in sandy soils. The field experiment was conducted 2015–2017 in Live Oak, Florida using a randomized complete block with a split plot design and four replicates. Treatments evaluated five irrigation strategies: (i) GROW, mimicking grower irrigation practices in the region, (ii) SWB, using a soil water balance to schedule irrigation; (iii) SMS, using soil moisture sensors to schedule irrigation; (iv) RED, applying 60% of the GROW treatment; and (v) NON, non-irrigated, and three N fertility rates: (i) low (157 kg N/ha), (ii) medium (247 kg N/ha), and (iii) high (336 kg N/ha). In comparison to GROW, the SWB, SMS and RED irrigation treatments showed no differences in final biomass, N uptake nor grain yield; however, these treatments achieved on average 41, 47, and 36% irrigation reduction, respectively, without impacts on yield during the three maize seasons evaluated. For most of the variables, statistical differences were found between the low and the high N rates, but no differences compared to the medium N rate. A 26% reduction of N fertilizer was achieved using the medium N rate without negative impact on N uptake, biomass nor yield in comparison to the high N fertilization rate. During this experiment, maize N uptake reached a plateau; thus, potential N losses resulted from applications exceeding recommended rates. Furthermore, the implementation of these more efficient irrigation and N fertilizer management strategies reduced irrigation and N fertilizer applications without negative impacts in yield. Thus, these practices may prevent potential N leaching to waterbodies while improving profits.

Abstract

A motion continuous control scheme for fully damped mechanical systems with constrained inputs is proposed. It gives the freedom to choose among finite-time and (local) exponential convergence through a simple design parameter. The control objective is achieved from any initial conditions, for desired trajectories that can be physically tracked avoiding actuator saturation and loss of motion error dissipation, globally induced through the aid of the natural damping terms explicitly considered in the open-loop dynamics. The stability analysis is based on a strict Lyapunov function and is formally developed within an appropriate analytical framework that takes into account the time-varying character naturally adopted by the closed loop. Simulation tests are further included.

HighlightsDSSAT simulations of final N uptake, biomass, and yield for a maize-peanut rotational field experiment with three irrigation treatments and three N fertilizer rates had good performance for the irrigated treatments (average nRMSE of 9%) but greater error for the rainfed treatments (average nRMSE of 15%).Experiments and DSSAT simulations demonstrated that N fertilizer and irrigation applications were reduced by 26% and 60%, respectively, when using a 247 kg N ha-1 fertilizer rate and a sensor-based irrigation schedule rather than conventional practices of 336 kg N ha-1 and a calendar-based irrigation method, with no impact on yield.Simulations demonstrated that N leaching during the crop rotation was reduced by 37% when an N fertilizer rate of 247 kg N ha-1 and sensor-based irrigation scheduling were used versus conventional practices.Soil N increased (=15 mg kg-1) when maize and peanut residues decayed and then leached during the fallow season. Cover or cash crops planted immediately after the maize and peanut harvests have potential to take up this N and reduce leaching.Abstract. Nitrogen (N) is an essential element for crop growth and yield; however, excessive N applications not taken up by crops can result in N leaching from the root zone, increasing N loads to waterbodies and leading to a host of environmental problems. The main objective of this study was to simulate water and N balances for a maize-peanut (Zea mays L. and Arachis hypogaea L.) rotational field experiment with three irrigation treatments and three N fertilizer rates. The irrigation treatments consisted of mimicking grower irrigation practices in the region (GROW), using soil moisture sensors to schedule irrigation (SMS), and non-irrigated (NON). The N fertilizer rates were low, medium, and high (157, 247, and 336 kg N ha-1, respectively) for maize with a constant 17 kg ha-1 for all peanut treatments. DSSAT maize genetic coefficients were calibrated using the SMS-high treatment combination under the assumption of no water or N stress. The other eight treatment combinations were used as independent data for model validation of the crop coefficients. All soil hydrologic parameters were specified based on measured values, and default DSSAT peanut genetic coefficients were used with no calibration. For the irrigated treatments, DSSAT models had good performance for N uptake, biomass, and yield (average nRMSE of 8%) and moderate performance for soil water content (average nRMSE of 18%). Soil nitrate RMSE was 21% lower than the standard deviation of the observed data (5.8 vs. 7.2 mg kg-1). For the rainfed treatments, DSSAT had greater error (average nRMSE of 15% for N uptake, biomass, and yield, and average nRMSE of 31% for soil water). Soil nitrate RMSE was 11% greater than the standard deviation of the observed data (8.0 vs. 7.2 mg kg-1), and nRMSE was >30% during the crop rotation. Simulations estimated that N leaching over the crop rotation was reduced by 24% on average when using the 247 kg N ha-1 fertilizer rate compared to 336 kg N ha-1 across the irrigation treatments. Furthermore, N leaching was reduced by 37% when using SMS to schedule irrigation and the 247 kg N ha-1 fertilizer rate for maize and 17 kg N ha-1 for peanut compared to conventional practices (GROW and 336 kg N ha-1 for maize and 17 kg N ha-1 for peanut). Moreover, this management practice reduced N fertilizer use by 26% and irrigation water use by up to 60% without negative impacts on yield. Observed and simulated soil N increased during maize and peanut residue decay, with simulations estimating that this soil N would leach below the root zone during the fallow season. This leaching could potentially be reduced if a cover crop or cash crop were planted between the maize and peanut crops to take up the mineralized N. Se estudió el efecto del coeficiente de estrés hídrico (Kc) sobre los rendimientos del cultivo de cebolla, a través de un gradiente de evapotranspiración (Etc), que incluyó condiciones estándar, (100% de la Etc, determinada experimentalmente) y cuatro niveles de reducciones simuladas de la Etc (Etc-aj) para las condiciones no estándar. Las simulaciones se realizaron con auxilio del programa CROPWAT 8.0, que incorpora el modelo Penman Montheith, modificado por FAO. Para cada variante, se calcularon los coeficientes de cultivo (Kc), siendo los coeficientes de cultivos globales de de la cebolla, 0,64; 0,57; 0,53, 0,51 y 0,45 además se determinaron los coeficientes de estrés hídrico (Ks) (1, 0,55; 0,45; 0,37 y 0,33) respectivamente; y se evaluó la influencia de los mismos sobre el rendimiento fresco de la cebolla los cuales oscilaron entre 18,7 y 13,1 kg/m2 en función de la variación relativa de la evapotranspiración, con reducciones hasta el 68% de la producción para la variante más extrema. El conocimiento sobre las potencialidades del uso del agua en diferentes escenarios de suministro, constituyen las bases científicas para proyectar medidas de adaptación y mitigación para la producción de cebolla, en condiciones de sequía.

Persistent pulmonary hypertension (PPHN) of the newborn is a lung parenchymal disorder that causes a wide range of hemodynamic changes in the newborn's systemic circulation. Arising from a multifactorial web of etiology, PPHN is one of the most common reasons for neonatal intensive care unit hospitalization and is associated with increased morbidity and mortality. Historically, multiple treatment modalities have been explored, ranging from oxygen and surfactant therapy to newer upcoming medications like magnesium sulfate and adenosine. This review article has discussed the pathogenesis of PPHN and its relationship with the clinical implications of PPHN, such as heart failure and so on. This article has also explored the diagnostic guidelines and analyzed the existing and the upcoming modalities for treating PPHN.

Abstract

The most frequent type of ligament injury is an anterior cruciate ligament (ACL). The mechanisms of an ACL injury are classified as direct contact, indirect contact, and non-contact. Physical examination for the assessment of the ACL is commonly used in routine care in the evaluation of the knee and is part of the diagnostic process. Due to the high degree of variability in their presentation and outcomes, treatment must be tailored according to factors such as patient demographics, the severity of the damage, and long-term improvement profile. When it comes to ACL injuries, low-quality data have been produced that reveals no difference in patient-reported knee function results between surgical ACL restoration and conservative therapy. However, these results must be evaluated in the perspective of the fact that many individuals with an ACL rupture remained symptomatic after rehabilitation and eventually underwent ACL reconstruction surgery. This article has reviewed the risk factors and the mechanisms that commonly lead to ACL injuries. This article has also discussed the clinical significance of conservative and surgical management and has highlighted the implications of both approaches.

Abstract

Myelodysplastic syndromes (MDS) represent a large group of rare and diverse clonal stem cell disorders. These are classified into several different phenotypes and typically arise following a multistep genetic process, whereby genetic mutations alter the DNA damage and cellular stress responses, impacting transcription, RNA splicing, epigenetics, and cytokine signaling. However, despite the advances made regarding molecular pathophysiology and prognostic criteria and the influx of new treatment modalities, management is primarily based on prognostic scores, such as the Revised International Prognostic Scoring System. This poses a significant challenge to current healthcare professionals due to poor comprehension of the underlying pathophysiology. Hence, this review integrates the latest research and treatment modalities for MDS and discusses the different genetic mutations outlined in the revised World Health Organization 2016 MDS classification system and the associated treatment modalities. Additionally, future directions of research and clinical management of MDS are discussed.

Coronary artery disease (CAD) is caused by atheromatous blockage of coronary vessels leading to acute coronary events that usually occur when a plaque ruptures and a thrombus forms. CAD is a known cause of significant cardiovascular events, accounting for more than 50% of the deaths in western countries, and most of the patients with CAD remain asymptomatic. The coronary artery calcium (CAC) score has been created as a measure of coronary atherosclerosis. This article has compiled various studies that conclude the clinical relationship between coronary artery calcium and the development of cardiovascular (CV) events by using the CAC score as a reliable indicator of CAD. This article has reviewed the pathophysiology and risk factors of CAD, along with various methods of CAC scoring. It also underlined the reliability of CAC scoring for early detection of CAD in asymptomatic individuals. We emphasized the importance of age-dependent risk factor analysis combined with practical screening tools like CAC scoring for early diagnosis of CAD can help direct the treatment and prevent deaths in asymptomatic individuals.

Abstract

Acromegaly is a complex endocrinological disorder commonly caused by hypersecretion of growth hormone (GH) typically due to pituitary gland tumors. Patients with acromegaly who are successfully treated and biochemically managed have a reasonably average life expectancy. However, it causes a cascade of multi-systemic involvement throughout the patient’s life, including cardiovascular, neuropsychiatric, respiratory, metabolic, neurological, neoplastic, and gastrointestinal involvement, resulting in a higher rate of hospitalization, lower quality of life, and a shorter life expectancy. Although cardiovascular complications are the primary cause of death in patients with acromegaly, malignancy is now emerging as a major killer in these individuals. Colorectal carcinoma has been reported to be prevalent in acromegaly individuals. This review article has compiled studies to demonstrate a link between acromegaly and colorectal neoplasia, intending to provide a strong foundation for their clinical relationship. This article has summarised a potential pathogenic mechanism and provided insights into the clinical presentation of such patients. Furthermore, this article has provided a brief overview of current screening recommendations for colorectal neoplasia in acromegaly patients.

This review article outlined various studies in order to understand the pathogenesis pathways that lead to the development of stroke-associated pneumonia, as well as therapeutic and preventive options to reduce pneumonia. The article looked for risk factors that increase the risk of developing pneumonia among stroke patients. In addition, it has reviewed various therapeutic modalities, such as postural modifications, pharmacological treatment, and other unique treatments, in an attempt to find which of them are efficient to decrease the occurrence of pneumonia and which of them are not. The article also attempts to emphasize the importance of early screening for dysphagia among stroke patients and demonstrates the importance of preventive strategies that can be easily implemented, such as routine oral care and behavioral modifications.

Abstract

Hereditary hemochromatosis (HH) is a common genetic metabolic disorder characterized by excessive iron absorption and elevated serum iron levels, which accumulate in various organs, such as the heart, pancreas, gonads, and damage these organs. There are only a few articles and clinical studies describing the characteristics of cardiac involvement in HH along with the significance of early diagnosis and management in preventing complications. In this review article, we have reviewed multiple pieces of literature and gathered available information regarding the subject. We compiled the data to investigate the importance of early detection of symptoms, regular monitoring, and prompt management with strict adherence to reverse or prevent complications. This article has reviewed different aspects of cardiac hemochromatosis, such as pathogenesis, clinical presentation, diagnosis, and management. Recognition of early symptoms, diagnosis of cardiac involvement with various modalities, and implementation of early treatment are essentially the foundation of better outcomes in HH.

Abstract

El turismo ha cobrado una mayor importancia para la economía de los países al aportarles importantes ingresos por el concepto de divisas recibidas, activar la generación de empleo y contribuir a aminorar la pobreza en los países emergentes, siendo un motor de desarrollo. La investigación parte de la hipótesis de que los catorce factores de la competitividad turística evaluados por el Foro Económico determinaron la competitividad turística de México en la región de Asia-Pacífico. El objetivo del presente trabajo es establecer el grado de influencia de los catorce factores mencionados en la competitividad turística de veinte países que conformaron la APEC para el periodo 2013. Para ello se utilizó el Análisis de Escalamiento Multidimensional. Los recursos culturales, la sustentabilidad ambiental y la infraestructura del transporte aéreo son los factores de la competitividad que se distinguen de manera efectiva.

Abstract

Saturating-proportional-derivative-type global continuous control for the finite-time or (local) exponential stabilisation of mechanical systems with bounded inputs is achieved involving the desired conservative-force compensation. Far from what one could expect, the proposed controller is not a simple extension of the on-line compensation case but it rather proves to entail a closed-loop analysis with a considerably higher degree of complexity. This gives rise to more involved requirements to guarantee its successful performance and implementability. Interesting enough, the proposal even shows that actuators with higher power-supply capabilities than in the on-line compensation case are required. Other important analytical limitations are further overcome through the developed algorithm. Experimental tests on a two-degree-of-freedom robotic arm corroborate the efficiency of the proposed scheme.

Abstract

Introduction The growth hormone (GH) has been reported as a crucial neuronal survival factor in the hippocampus against insults of diverse nature. Status epilepticus (SE) is a prolonged seizure that produces extensive neuronal cell death. The goal of this study was to evaluate the effect of intracerebroventricular administration of GH on seizure severity and SE-induced hippocampal neurodegeneration. Methodology Adult male rats were implanted with a guide cannula in the left ventricle and different amounts of GH (70, 120 or 220 ng/3 μl) were microinjected for 5 days; artificial cerebrospinal fluid was used as the vehicle. Seizures were induced by the lithium–pilocarpine model (3 mEq/kg LiCl and 30 mg/kg pilocarpine hydrochloride) one day after the last GH administration. Neuronal injury was assessed by Fluoro-Jade B (F-JB) staining. Results Rats injected with 120 ng of GH did not had SE after 30 mg/kg pilocarpine, they required a higher number of pilocarpine injections to develop SE than the rats pretreated with the vehicle, 70 ng or 220 ng GH. Prefrontal and parietal cortex EEG recordings confirmed that latency to generalized seizures and SE was also significantly higher in the 120 ng group when compared with all the experimental groups. FJ-B positive cells were detected in the hippocampus after SE in all rats, and no significant differences in the number of F-JB cells in the CA1 area and the hilus was observed between experimental groups. Conclusion Our results indicate that, although GH has an anticonvulsive effect in the lithium–pilocarpine model of SE, it does not exert hippocampal neuroprotection after SE.

Abstract

Background Coronavirus disease 2019 (COVID-19) was declared a pandemic by the WHO on 11 March 2020 and global surgical practice was compromised. This Commission aimed to document and reflect on the changes seen in the surgical environment during the pandemic, by reviewing colleagues’ experiences and published evidence. Methods In late 2020, BJS contacted colleagues across the global surgical community and asked them to describe how severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) had affected their practice. In addition to this, the Commission undertook a literature review on the impact of COVID-19 on surgery and perioperative care. A thematic analysis was performed to identify the issues most frequently encountered by the correspondents, as well as the solutions and ideas suggested to address them. Results BJS received communications for this Commission from leading clinicians and academics across a variety of surgical specialties in every inhabited continent. The responses from all over the world provided insights into multiple facets of surgical practice from a governmental level to individual clinical practice and training. Conclusion The COVID-19 pandemic has uncovered a variety of problems in healthcare systems, including negative impacts on surgical practice. Global surgical multidisciplinary teams are working collaboratively to address research questions about the future of surgery in the post-COVID-19 era. The COVID-19 pandemic is severely damaging surgical training. The establishment of a multidisciplinary ethics committee should be encouraged at all surgical oncology centres. Innovative leadership and collaboration is vital in the post-COVID-19 era.

Abstract

Actualmente casi todos los envíos internacionales necesitan usar más de un tipo de transporte, desde el punto de origen al destino final. Cada uno de los tipos de transporte mundiales de carga y pasajeros ha desempeñado un papel esencial para facilitar la diversificación geográfica. En este trabajo se evalúa a través del Análisis de Componentes Principales la competitividad del transporte internacional, considerando a su vez las variables más importantes que inciden en este sector y el desempeño de las economías respecto de dichas variables, y además se analiza la estructura del sistema de transporte internacional para veintinueve países entre ellos siete de América Latina. Los resultados mostrados en el índice de competitividad del transporte internacional señalan que los países más competitivos en materia de transporte internacional acorde a las variables analizadas son en orden descendente: Hong Kong, Estados Unidos, Singapur, China, Suecia, España, Japón, Bélgica, Dinamarca y Canadá. Cabe señalar que los países de América Latina con mayores puntuaciones en este rubro son Brasil, seguido de Panamá, Chile y Costa Rica.

Abstract

Rituximab hypersensitivity reactions are rare but are one of the main causes of rituximab elimination from antilymphoma immunochemotherapy treatments. While the clinical picture may be indistinguishable from other infusion-related reactions, hypersensitivity reactions (HSR) do not disappear and instead become more intense with subsequent administrations. Objective. To describe the use of the 12-step protocol for desensitization to intravenous rituximab in clinical practice and the complementary study of a possible IgE-mediated HSR in the context of B-cell lymphoma treatment. Methods. A 12-step rituximab desensitization protocol was performed prospectively within clinical practice in 10 patients with a history of severe infusion reactions or in patients who had a repeated reaction at subsequent doses despite taking more intense preventive measures. Skin prick tests were performed at the time of reaction and at a later time to eliminate false negatives due to possible drug interference. Results. Overall, with the desensitization protocol, 70% of patients were able to complete the scheduled immunochemotherapy. Two patients had to discontinue the therapy due to clinical persistence and the third due to lymphoma progression. Intradermal tests with 0.1% rituximab were positive in only 20% of cases, demonstrating a mechanism of hypersensitivity. Conclusions. The 12-step desensitization protocol is very effective and assumable within healthcare practice. There is a need to determine the mechanism underlying the infusion reaction in a large proportion of cases due to the risk of future drug exposure.

Abstract

A Saturating-Proportional Saturating-Derivative type global continuous control scheme with desired gravity compensation for the finite-time or (local) exponential stabilization of robotic systems with constrained inputs, avoiding velocity variables in the feedback, is presented. The proposed output-feedback controller proves to need a closed-loop analysis with considerably higher degree of complexity, and entail more involved consequent requirements, than in the on-line compensation case. Other analytical limitations are further overcome through the developed algorithm. Simulation tests corroborate the efficiency of the proposed approach.

Abstract

The present work describes and supports the methodology for the improvement of the wall model developed in AINA and its specific application to the Japanese DEMO Water Cooled Pebbled Bed. The set-up and application of this approach aims to obtain robust models by estimating the behavior of the studied systems as accurately as possible. These systems are represented in a simplified way. This requires the computation of a 3D radiation transport which has been carried out by means of MCNP6.1, ADVANTG and thermal-hydraulic calculations using ANSYS ® Fluent ® . Several CFD mesh typologies and discretizations have also been employed to test the Richardson theorem. In addition, 1D simplified models have also been created and optimized for their usage in AINA code. The temperature distribution also shows good agreement (within 7%). In some cases the simplified models have not behaved in a conservative manner compared with the outcomes obtained for the 3D models. This observed absence of conservatism is intrinsic to the 1D approach. To cope with these effects, scaling functions have been determined as a ratio between the most conservative radial temperature distribution – computed by fully detailed 3D CFD – and the 1D simplified model. The scaling functions will be applied to the AINA computed wall temperature distribution. To conclude, the determination and coherence of the result obtained using independent tools and approaches, ANSYS ® Fluent ® vs AINA thermal-hydraulic routines, lead us to recommend the proposed methodology.

Abstract

The present work exposes the 3D steady state thermal hydraulic (TH) analysis of the Irregular Sector number 3 (IrS#3) of the ITER Vacuum Vessel (VV), including the Irregular Field Joints (IrFJ) by means of a commercial CFD (Computational Fluid Dynamics) code. The geometry of the IrS#3 has been simplified and healed in order to be suitable for CFD analysis. The simplified geometry has been meshed and converted to a polyhedral cell based mesh so as to enhance accuracy and calculation stability. Nuclear heat deposition ( H ) from the latest C Lite model [1] has been implemented through several User Defined Functions (UDF). Water coolant and stainless steel shell are solved together as a steady state conjugate heat transfer problem in order to assess the impact of the nuclear heat deposition on the IrS#3 cooling scheme. Hence, the IrS#3 is simulated as a whole, without splitting the domain. Results show the total IrS#3 pressure drop as well as the flow and temperature distribution all over the IrS#3. Moreover, the heat transfer coefficient ( h ) has been calculated at the fluid-solid interface. Velocity magnitude in the water coolant has an average value of 0.10 m/s and mass flow rate distribution is 11% and 89% respectively for Inboard and Outboard. The pressure drop, mainly concentrated at inlet and outlet ducts, is of 77.4 kPa. The average h is of 5 690 W/(m 2 K), way above the design limit of 500 W/(m 2 K). The fluid temperature stays at an average value of 101.7 °C. The results obtained have a significant importance regarding design and safety and give a valuable insight on the current cooling scheme and system behavior for the IrS#3 of the ITER VV. This follows a previous work presenting the methodology and the results of a 3D TH analysis of the Irregular Sector number 2 (IrS#2) of the ITER Vacuum Vessel (VV) by means of CFD [2].

Abstract

An observer-less output-feedback global continuous control scheme for the finite-time or (local) exponential stabilization of mechanical systems with constrained inputs is proposed. The approach is formally developed within the theoretical framework of local homogeneity. The closed-loop analysis incorporates a complementary insight on the control-induced motion dissipation through an ad hoc feedback-system passivity theorem. The work includes a simulation implementation section where the performance difference of the proposed scheme with previous observer-based and differentiation algorithms is brought to the fore.

Resumen: Introducción: En el trastorno por déficit de atención e hiperactividad (TDAH), la heredabilidad ha sido reconocida en múltiples estudios. En este sentido, se ha observado una mayor frecuencia del trastorno en niños con familiares afectados. El objetivo fue determinar la asociación entre la presencia de síntomas de TDAH en niños escolares de 5 instituciones educativas de Bogotá y el antecedente de síntomas de TDAH en sus padres utilizando el cuestionario de Wender-Utah. Metodología: Estudio de casos y controles seleccionados de acuerdo con los criterios DSM-IV para TDAH, la escala multifuncional de la conducta (BASC) para padres y maestros y WISC-IV para descartar déficit cognitivo. A los padres se les aplicó un cuestionario de Wender-Utah para identificar de manera retrospectiva síntomas de TDAH en su infancia. Un score de 36 fue tomado como punto de corte. Resultados: De 202 niños disponibles, 117 fueron casos y 85 controles. Un 16% de 175 madres y un 20,6% de 141 padres tuvieron antecedentes positivos para síntomas de TDAH en la infancia. La presencia de síntomas en alguno de los progenitores, principalmente en la madre, es un factor de riesgo significativo para la presencia de TDAH en los niños y esta relación persiste si se controla por diferentes variables. Si los 2 progenitores tienen el cuadro, existe una tendencia a que el riesgo aumente. Conclusión: A pesar de que el TDAH se ha asociado a un componente genético, otros factores del entorno pueden estar involucrados en la génesis del trastorno. Abstract: Introduction: Multiple studies of attention deficit hyperactivity disorder (ADHD) have recognised a heritability factor in that a higher frequency of the disorder is observed in children with an affected relative. Our aim was to determine the association between ADHD symptoms in young children enrolled in five schools in Bogota and a history of ADHD symptoms in their parents using the Wender-Utah Rating Scale. Methodology: Case-control study of participants selected according to DSM-IV criteria for ADHD and the Behavioral Assessment System for Children (BASC) completed by parents and teachers; the WISC-IV scale was used to exclude children with cognitive deficit. Parents completed the Wender-Utah Rating Scale to retrospectively identify any ADHD symptoms in childhood. A score of 36 was used as a cutoff point. Results: The study included 202 children: 117 cases and 85 controls. A positive history of ADHD symptoms in childhood was identified for 16% of 175 mothers and 20.6% of 141 fathers. The presence of symptoms in either parent, especially the mother, constitutes a significant risk factor for ADHD in children and this relationship persists after controlling for different variables. If both parents have the disorder, the risk tends to increase. Conclusion: Although ADHD has been linked to a genetic component, other environmental factors may be involved in the disorder.
