Perilla frutescens (PF) is a plant with various biological activities, including antioxidant and antidepressant activities; however, the efficacy of its fermented product has not been evaluated. Herein, we found that the Bacillus subtilis-fermented PF (FPF) increased non-rapid eye movement sleep and slow wave activity. FPF improved the sleep onset latency and sleep duration in a pentobarbital-induced sleep model, and this efficacy was abolished by the adenosine A1 receptor (A1R) antagonist. In addition, FPF increased and decreased the neuronal activity in sleep-activating and inhibiting brain regions, respectively, and these effects were also blocked by A1R antagonist 8-cyclopentyl-1,3-dipropylxanthine. Furthermore, FPF exhibited direct binding and agonistic activities for A1R. HPLC/NMR analysis revealed uracil, protocatechuic acid, apigenin-7-O-diglucuronide, and luteolin-7-O-glucuronide to be the major active components responsible for the sleep-promoting efficacy of FPF. In conclusion, this study suggests that FPF has A1R agonistic activity and this novel activity may be critical for its sleep-promoting effect.

Abstract This study aimed to assess the impact of varying monopolar diathermy power settings on postoperative pain, hemorrhage, and wound healing following tonsillectomy. A single-center, prospective, randomized, double-blinded, controlled clinical study was conducted. During bilateral tonsillectomy procedures, one tonsil received low-power settings (15 W, cutting/blend) while the other tonsil received high-power settings (35 W, cutting/blend). Postoperative pain scores (0–10) and wound healing scores (0–3) were evaluated immediately after surgery and at 1, 2, and 4 weeks postoperatively using the visual analog scale. Additionally, histological analysis was performed on electrically resected tonsil tissues to assess tissue damage in the tonsil bed. The allocation of high and low power settings to each side was randomized. Results showed that 1 week after the surgery, the high-power group experienced significantly higher pain scores (mean ± standard deviation: 4.84 ± 2.21) compared to the low-power group (3.56 ± 2.24, p = 0.049). Moreover, the high-power side exhibited slower wound healing during the initial 1–2 weeks postoperatively, as indicated by lower wound scores at 2 weeks (high-power: 1.96 ± 0.64; low-power: 2.43 ± 0.59, p = 0.008). Obstructive sleep apnea (OSA) is a common disorder characterized by upper airway obstruction during sleep. To reduce the morbidity of OSA, sleep specialists have explored various methods of managing the condition, including manifold positive airway pressure (PAP) techniques and surgical procedures. Nasal obstruction can cause significant discomfort during sleep, and it is likely that improving nasal obstruction would enhance the quality of life and PAP compliance of OSA patients. Many reliable studies have offered evidence to support this assumption. However, few comprehensive guidelines for managing OSA through nasal surgery encompass all this evidence. In order to address this gap, the Korean Society of Otorhinolaryngology-Head and Neck Surgery (KORL-HNS) and the Korean Society of Sleep and Breathing designated a guideline development group (GDG) to develop recommendations for nasal surgery in OSA patients. Several databases, including OVID Medline, Embase, the Cochrane Library, and KoreaMed, were searched to identify all relevant papers using a predefined search strategy. The types of nasal surgery included septoplasty, turbinate surgery, nasal valve surgery, septorhinoplasty, and endoscopic sinus surgery. When insufficient evidence was found, the GDG sought expert opinions and attempted to fill the evidence gap. Evidence-based recommendations for practice were ranked according to the American College of Physicians’ grading system. The GDG developed 10 key action statements with supporting text to support them. Three statements are ranked as strong recommendations, three are only recommendations, and four can be considered options. The GDG hopes that this clinical practice guideline will help physicians make optimal decisions when caring for OSA patients. Conversely, the statements in this guideline are not intended to limit or restrict physicians’ care based on their experience and assessment of individual patients.

Obstructive sleep apnea (OSA) is characterized by cyclic normoxic and hypoxic conditions (intermittent hypoxia, IH) induced by the repeated closure of the upper-airway respiratory tract. As a pathomechanism of OSA, IH results in various comorbidities via chronic inflammation and related pathways. However, the role of other inflammatory cells, such as lymphocytes, has not been well-explored. This study aimed to examine the effects of IH on the distribution and balance of T cell subsets and other related cytokines, and mechanisms in the immune system. We modified OSA mouse model (male C57BL/6N male) using our customized chamber that controls specific sleep and oxygenic cycles. To induce hypoxia, the IH group was repeatedly exposed to 5% O2 and 21% O2 lasting for 120 s each for 7 h daily for 4 weeks. Mice were then subjected to a recovery period of 4 weeks, in which IH stimulation was ceased. T cells and related cytokines were analyzed using flow cytometry and immunohistochemistry. Compared with the control group, the IH group had significantly lower levels of CD4+CD25+Foxp3+ regulatory T cells but higher levels of Th 17, IL-4, HIF-1, and inflammatory cytokines. After the recovery period, these altered changes in the immune cells were recovered, and we found no significant difference in their levels between the control and recovery groups. This study revealed that the Th17/Treg ratio is increased by intermittent hypoxia, and this imbalance can explain immune-related diseases, including recently reported allergies, autoimmune, and even cancer diseases, arising from OSA.

Abstract Full-night polysomnography (PSG) is the gold standard for diagnosing obstructive sleep apnea (OSA). However, PSG requires several sensors to be attached to the patient’s body, which can interfere with sleep. Moreover, non-contact devices that utilize impulse radio ultra-wideband radar have limitations as they cannot directly measure respiratory airflow. This study aimed to detect respiratory events through infrared optical gas imaging and verify its feasibility for the diagnosis of OSA. Data collection through PSG and infrared optical gas imaging was simultaneously conducted on 50 volunteers. Respiratory airflow signal was extracted from the infrared optical gas images using an automated algorithm. We compared the respiratory parameters obtained from infrared optical gas imaging with those from PSG. All respiratory events scored from the infrared optical gas imaging were strongly correlated with those identified with standard PSG sensors. Based on a receiver operating characteristic curve, infrared optical gas imaging was deemed appropriate for the diagnosis of OSA. Infrared optical gas imaging accurately detected respiratory events during sleep; therefore, it may be employed as a screening tool for OSA.

Background and Objective From the past, several researchers have studied and measured the partial pressure of carbon dioxide during sleep in normal children to establish diagnostic criteria for hypoventilation in children. Here we tried to verify the existing definition of sleep-related hypoventilation in Asian by evaluating and analyzing the carbon dioxide partial pressure in pediatric subjects who underwent polysomnography. Methods We retrospectively analyzed clinical information of 196 children who underwent polysomnography at our hospital from Feb 2011 to Apr 2021. Among Asian pediatric subjects, those with serious chronic or genetic diseases, craniofacial deformities, and hypoventilation confirmed by polysomnography were excluded. We evaluated partial pressure of carbon dioxide in target group by end-tidal carbon dioxide (EtCO2) measured with a stream capnometer through nasal cannula. The target group was classified by apnea-hypopnea index and analyzed. Results The mean value of the time with EtCO2 ≥ 45 mm Hg in total sleep time was 47.00% which is higher than previous studies. The mean value of the time with EtCO2 ≥ 50 mm Hg in total sleep time was 2.77% which is similar to previous studies. Also, our subjects showed the highest mean EtCO2 value in non-rapid eye movement (NREM) stage and lowest mean EtCO2 value when wake. Conclusions In this study, the diagnostic criteria for sleep-related hypoventilation at the American Academy of Sleep Medicine (AASM) in 2017 established by measuring the EtCO2 in western children seems appropriate to apply to Asian children to define hypoventilation.

Abstract Objective Cone‐beam computed tomography (CBCT) is a promising imaging modality for sinonasal evaluation, with advantages of relatively low radiation dose, low cost, and quick outpatient imaging. Our study aimed to compare the diagnostic performance and image quality of CBCT with those of multi‐detector computed tomography (MDCT) with different slice thickness. Methods We retrospectively reviewed 60 consecutive patients who had undergone both CBCT and MDCT. MDCT images was reconstructed with 1 and 3 mm slice thickness. The quantitative image quality parameters (image noise, signal‐to‐noise ratio [SNR], and contrast‐to noise ratio [CNR] were calculated and compared between the two imaging modalities. Two observers (ENT surgeon and neuroradiologist) evaluated the presence of seven sinonasal anatomic variations in each patient and interobserver agreements were analyzed. The diagnostic performance of CBCT (0.3 mm) and MDCT (3 mm) was assessed and compared with that of high resolution MDCT (1 mm), which is considered as the gold standard. Results The image noise was significantly higher and SNR and CNR values were lower in the CBCT (0.3 mm) group than in the MDCT groups (1 and 3 mm). The diagnostic performance of CBCT (0.3 mm) was similar to that of MDCT (1 mm) and superior to that of MDCT (3 mm). The highest interobserver agreement was for high resolution MDCT (1 mm), followed by CBCT (0.3 mm), and MDCT (3 mm). Conclusion Considering its low radiation dose, low cost, and ease of clinical access, CBCT may be a useful imaging modality for as first line sinonasal evaluation and repeated follow up. The distribution shift of forest species due to the fact of climate change may negatively affect ecosystem services including the production of medicinal resources. Climate change impact assessments of habitat range changes are essential to sustainably manage forest resources. A change in the habitat suitability due to the fact of climate change was predicted for Angelica gigas, which has high economic value among forest medicinal resources in South Korea. The habitat suitability was predicted by an ensemble species distribution model that combined the results of nine single algorithm models using the committee averaging method. A total 168 occurrence data and 10 environmental variables were used for the single algorithm models. The area under the receiver operation characteristic curve (AUC) and true skill statistic (TSS) were applied to evaluate the models’ performance, and the contribution of the environmental variables was calculated as an important value for each single algorithm model. Climate change scenarios were projected to predict future habitat suitability. The future suitable habitat for A. gigas was gradually reduced to the high mountain regions of the eastern part of South Korea regardless of the climate change scenarios. The main environmental variable was the annual mean temperature, and the rise in temperature due to the fact of climate change was found to have a negative effect on the habitat suitability for A. gigas. The decline in the habitat suitability for A. gigas, a major forest medicinal resource, is expected to result in the reduction in its production. Therefore, it is required to establish adaptation measures to mitigate the negative impact of this decrease, such as protecting the natural habitats of A. gigas.

Cement-augmented pedicle screw instrumentation (CAPSI) is a widely used surgical technique in posterior spondylodesis of osteoporotic patients. Complications related to cement leakage after CAPSI was rarely reported compare to that of vertebroplasty, kyphoplasty and most of them are asymptomatic. Herein the authors report the case of a 67-year-old female who experienced cardiac perforation by string shape cement embolus after CAPSI. She underwent the foreign body removal operation through the 5th intercostal thoracotomy and she was well recovered consequently. Surgeons should be aware of this complication entity when encounter postoperative chest pain to avoid fatal results. The authors also reviewed the case reports for cardiac perforation by cement embolus and preventive modalities for the cement leakage.

BackgroundCirculating apolipoprotein J (ApoJ) is closely associated with insulin resistance; however, the effect of exercise on circulating ApoJ levels and the association of ApoJ with metabolic indices remain unknown. Here, we investigated whether a combined exercise can alter the circulating ApoJ level, and whether these changes are associated with metabolic indices in patients with type 2 diabetes mellitus.MethodsPostmenopausal women with type 2 diabetes mellitus were randomly assigned into either an exercise (EXE, n=30) or control (CON, n=15) group. Participants in the EXE group were enrolled in a 12-week program consisting of a combination of aerobic and resistance exercises. At baseline, 4, 8, and 12 weeks, body composition and metabolic parameters including homeostatic model assessment of insulin resistance (HOMA-IR) and serum ApoJ levels were assessed.ResultsIn the EXE group, ApoJ levels decreased 26.3% and 19.4%, relative to baseline, at 8 and 12 weeks, respectively. The aim of the present study was to reveal the associations between skeletal and soft tissue features of the nasomaxillary complex and development and severity of obstructive sleep apnea (OSA) in adolescents. A total of 100 adolescents (mean age, 14.9 ± 1.4 years; age range, 13–17 years) were enrolled. All participants underwent full-night polysomnography and had an assessment of size and position of the tongue, tonsillar size, body mass index (BMI), and circumference of the waist, neck, and hip. The skeletal features of the nasomaxillary complex, including the zygomatic arch width, nasal cavity width, nasal base width, intercanine width, intermolar width, maxillary dental arch length, palatal vault angle, palatal depth, and SNA were measured on the three-dimensional images constructed with computed tomography data. Participants with an apnea and hypopnea index (AHI) of lower than 5 (AHI ≤ 5) were classified as control and participants while those with an AHI of greater than 5 were classified as OSA group. Each variable with a significant outcome in the independent T-test and age and sex factors were integrated into the multivariate linear regression and the dependent variable was AHI. There were significant differences in the BMI and hip circumference between two groups. The width of nasal base, palatal vault angle and SNA also showed significant differences between groups. The results from multivariate linear regression demonstrated that the BMI, width of the nasal base, and SNA showed significant contributions to the severity of OSA in adolescents. The features of the nasomaxillary complex seemed to have significant influences on development and severity of OSA.

Purpose: To report the case of a 25-year-old male who sustained significant ocular trauma from a confirmed foam round. To review the scientific literature on kinetic impact projectiles and legislation currently proposed to regulate their use. Observations: A 25-year-old male presented to the emergency department with acute pain and vision loss in his left eye after being struck by a foam round. Initial exam showed significant periorbital ecchymosis, multiple eyelid lacerations, microhyphema, and vitreous hemorrhage. Computed tomography revealed fractures of the inferior and medial orbital walls. Optical coherence tomography also demonstrated full-thickness macular hole. Microhyphema resolved after 15 days with steroid and mydriatic drops. Vision at 60 days after injury stabilized at 20/60. Repeat OCT at this time revealed closure of the macular hole. Care for the patient is ongoing. Conclusions and Importance: Foam rounds can cause a pattern of vision-threatening ocular trauma similar to that of rubber bullets with the additional risk of chemical injury. During the George Floyd protests, isolated reports of significant foam round-related ocular injuries have been reported in the press, but systematic epidemiologic and clinical data on the subject remains lacking. While accurate identification of the causative weapon is not required in the management of ocular trauma, it may impact advocacy efforts undertaken by physicians and the subsequent legislative efforts they inspire. Current state and federal law does not adequately protect individuals from these munitions. Multiple proposals in Congress aptly recognize the significant risk of blinding injury and mortality posed by all kinetic impact projectiles.

Abstract Recently, HTTP-based video streaming traffic has continued to increase. Therefore, video service providers have been using HTTP-based adaptive streaming (HAS) technology to reduce the traffic load of the HTTP server. Accordingly, many adaptive bit rate (ABR) schemes have been proposed to provide a high quality of experience (QoE) to video service clients. In this paper, we propose a new ABR scheme using an adaptive network-based fuzzy inference system (ANFIS), which is one of the neuro-fuzzy structures. The proposed scheme learns optimal fuzzy parameters by using (1) the learning ability of ANFIS and (2) the video streaming data providing high QoE to clients. Then, the bit rate of the next segment is determined according to these trained parameters. In the simulation using NS-3, we show that the proposed scheme selects the appropriate bit rate under various wireless network conditions and provides better QoE to clients than the existing schemes.

Obstructive sleep apnea is a highly prevalent cyclic repetitive hypoxia-normoxia respiratory sleep disorder characterized by intermittent upper-airway collapse. It is mainly diagnosed using in-laboratory polysomnography. However, the time-spatial constraints of this procedure limit its application. To overcome these limitations, there have been studies aiming to develop clinical prediction formulas for screening of obstructive sleep apnea using the risk factors for this disorder. However, the applicability of the formula is restricted by the group specific factors included in it. Therefore, we aimed to assess the risk factors for obstructive sleep apnea and develop clinical prediction formulas, which can be used in different situations, for screening and assessing this disorder. We enrolled 3,432 Asian adult participants with suspected obstructive sleep apnea who had successfully undergone in-laboratory polysomnography. All parameters were evaluated using correlation analysis and logistic regression. Among them, age, sex, hypertension, diabetes mellitus, anthropometric factors, Berlin questionnaire and Epworth Sleepiness Scale scores, and anatomical tonsil and tongue position were significantly associated with obstructive sleep apnea. To develop the clinical formulas for obstructive sleep apnea, the participants were divided into the development (n = 2,516) and validation cohorts (n = 916) based on the sleep laboratory visiting date. We developed and selected 13 formulas and divided them into those with and without physical examination based on the ease of application; subsequently, we selected suitable formulas based on the statistical analysis and clinical applicability (formula including physical exam: sensitivity, 0.776; specificity, 0.757; and AUC, 0.835; formula without physical exam: sensitivity, 0.749; specificity, 0.770; and AUC, 0.839). Analysis of the validation cohort with developed formulas showed that these models and formula had sufficient performance and goodness of fit of model. These tools can effectively utilize medical resources for obstructive sleep apnea screening in various situations.

In this study, we investigated the immune-enhancing activity of water extracts from Hydrangea macrophylla subsp. serrata (WE-HML). WE-HML increased cell viability and production of immunomodulators, which contributed to activating phagocytic activity in RAW264.7 cells. Inhibition of JNK and NF-κB reduced the production of immunomodulators by WE-HML. ROS inhibition suppressed the production of immunomodulators, and the activation of JNK and NF-κB signaling by WE-HML. TLR4 inhibition attenuated the production of immunomodulators, and activation of JNK and NF-κB signaling by WE-HML. In the immunosuppressed mouse model, WE-HML increased the spleen index, the levels of the cytokines, the numbers of white blood cells, lymphocytes, and neutrophils. However, WE-HML inhibited LPS-mediated overproduction of pro-inflammatory mediators in RAW264.7 cells, which indicated that WE-HML may have anti-inflammatory activity under excessive inflammatory conditions. Taken together, WE-HML may be considered to have immune-enhancing activity and expected to be used as a potential immune-enhancing agent.

Abstract In this paper, we propose a new resource allocation scheme for symbiosis of DASH clients and non-DASH clients (RASS) to provide seamless video streaming service to DASH clients and to guarantee fairness among non-DASH clients. In RASS, all the LTE downlink resource blocks are divided into two chunks by a KKT-conditions-based Resource-Division Controller (KCRDC). With the two divided chunks, the proposed scheme leads PFRA and KKT-conditions-based resource allocation scheme (KCRA) to allocate each chunk to non-DASH clients and DASH clients, respectively. PFRA allocates resource blocks with maintaining good trade-off between fairness and spectrum efficiency. On the other hand, KCRA allocates more resource blocks to DASH clients in the risk of re-buffering events to preferentially reduce the re-buffering-time ratios of the DASH clients. Finally, simulations are completed using the NS-3 (network simulator). The obtained results show that the proposed scheme shows a better synthetic performance than the existing resource allocation schemes in consideration of the re-buffering-time ratios and QoE value of DASH clients and the fairness among non-DASH clients.

Background: Although many studies indicate a positive correlation between GHRL gene Leu72Met polymorphism and an increased susceptibility to type 2 diabetes mellitus (T2DM), inconsistencies between independent studies still remain.Objective: Considering the inconsistencies between them, we have performed the current meta-analysis study. The objective of this study is to better examine the correlation of the GHRL gene Leu72Met polymorphism and T2DM.Methods: The current meta-analysis, involving 8,194 participants from 11 independent studies, was performed. A fixed effect model was used to evaluate the pooled odds ratios (ORs) and the corresponding 95% confidence intervals (95% CIs).Results: A significant association was found between T2DM and GHRL gene Leu72Met polymorphism under recessive (OR: 1.33, 95% CI: 1.01–1.76, P = 0.04), and homozygous genetic models (OR: 1.34, 95% CI: 1.01–1.78, P = 0.04) in the whole population. The correlation was more distinct in our subgroup analysis of the Chinese population under recessive (OR: 1.52, 95% CI: 1.07–2.15, P = 0.02), dominant (OR: 1.70, 95% CI: 1.38–2.10, P < 0.00001), additive (OR: 1.16, 95% CI: 1.02–1.33, P = 0.02), and homozygous genetic models (OR: 1.54, 95% CI: 1.07–2.20, P = 0.02).Conclusions: In short, GHRL gene Leu72Met polymorphism was significantly correlated with increased T2DM risk, particularly in the Chinese population. Individuals carrying the Met72 allele of GHRL Leu72Met gene polymorphism, particularly those of Chinese ancestry, may be more susceptible to developing T2DM disease.

Soil properties are one of the major factors determining the growth of vegetation. These properties drive the selection of the dominant bacterial community profiles, which eventually determines the soil quality and fertility. The abundance of preferential bacterial community assists in better productivity of a particular type of vegetation. The increasing focus on the health and well-being of the human population has resulted in a shift in paradigm to concentrate on the cultivation of medicinal plants such as Wild-simulated ginseng (WSG). These plant species take a long time for their growth and are generally cultivated in the mountainous forest trenches of Far East countries like South Korea. This study was conducted to decipher the bacterial community profiles and their correlation with soil chemical properties, which would give a broader idea about the optimum growing conditions of such an important medicinal plant. The important edaphic factor determined in this study was the soil pH, which was recorded to be acidic in all the studied cultivation sites. In agreement with the edaphic factor, the relative abundance of Acidobacteria was found to be highest as this phylum prefers to grow in acidic soils. Moreover, the total organic matter, total nitrogen and cation exchange capacity were found to be significantly correlated with the bacterial community. Hence, these results will help to identify the suitable cultivation sites for WSG and increase the productivity of these medicinal plants.

Building energy codes are key policy tools for improving building energy efficiency by defining the minimum requirement for the energy performance of new buildings. In Korea, the building energy code was focused on prescriptive criteria for a long time but is now gradually introducing performance criteria. However, switching to performance criteria is not straightforward because of the resistance of the market to abandoning the well-consolidated prescriptive criteria. The objective of this study is to derive appropriate measures to strengthen the prescriptive criteria and the performance criteria, considering both the energy-saving effect and the economic efficiency for new office buildings and educational buildings to increase the market acceptance of the building energy codes. To this end, the energy-saving effects of reference buildings resulting from the reinforcement of the prescriptive criteria in the past have been first analyzed. Then, based on the collected energy performance parameters and cost data, the economic efficiency relative to the energy saving deriving from the application of passive and active energy conservation measures (ECMs) were analyzed, and future building energy code’s reinforcement measures were derived.

BackgroundAlthough solute carrier family 30 (zinc transporter) member 8 (SLC30A8) gene 807C/T polymorphism is associated with an increased risk of type 2 diabetes mellitus (T2DM) risk, there remains some inconsistency between individual studies.ObjectiveThe aim of the study is to explore the relationship between SLC30A8 gene 807C/T polymorphism and T2DM in the Chinese population.MethodsThe current meta-analysis compiles and analyzes the data of 6,942 participants from 10 independent studies. Either a fixed or random-effects model was adopted to evaluate the pooled odds ratio (ORs) and the corresponding 95% confidence interval (95% CI).ResultsA significant association between SLC30A8 gene 807C/T polymorphism and T2DM was found in the Chinese population under allelic (OR: 0.85, 95% CI: 0.80–0.91, P = 7.42 × 10−7), recessive (OR: 0.52, 95% CI: 0.38–0.72, P = 8.49 × 10−5), dominant (OR: 2.40, 95% CI: 1.68–3.41, P = 1.30 × 10−6), homozygous (OR: 0.52, 95% CI: 0.40–0.67, P = 2.90 × 10−7), heterozygous (OR: 0.79, 95% CI: 0.71–0.88, P = 1.63 × 10−5), and additive genetic models (OR: 0.73, 95% CI: 0.64–0.83, P = 7.05 × 10−7).ConclusionSLC30A8 gene 807C/T polymorphism was significantly associated with an increased T2DM risk in the Chinese population. Therefore, individuals of Chinese descent with the C allele of SLC30A8 gene 807C/T polymorphism may be more susceptible to developing T2DM, while individuals with the T allele may be protected against T2DM.

Background: Presence of the β3-Adrenergic receptor (ADRB3) gene Trp64Arg (T64A) polymorphism may be associated with an increased susceptibility for essential hypertension (EH). A clear consensus, however, has yet to be reached.Objective and methods: To further elucidate the relationship between the ADRB3 gene Trp64Arg polymorphism and EH, a meta-analysis of 9,555 subjects aggregated from 16 individual studies was performed. The combined odds ratios (ORs) and their corresponding 95% confidence intervals (CI) were evaluated using either a random or fixed effect model.Results: We found a marginally significant association between ADRB3 gene Trp64Arg polymorphism and EH in the whole population under the additive genetic model (OR: 1.200, 95% CI: 1.00–1.43, P = 0.049). Association within the Chinese subgroup, however, was significant under allelic (OR: 1.150, 95% CI: 1.002–1.320, P = 0.046), dominant (OR: 1.213, 95% CI: 1.005–1.464, P = 0.044), heterozygous (OR: 1.430, 95% CI:1.040–1.970, P = 0.03), and additive genetic models (OR: 1.280, 95% CI: 1.030–1.580, P = 0.02). A significant association was also found in the Caucasian subgroup under allelic (OR: 1.850, 95% CI: 1. 260–2.720, P = 0.002), dominant (OR: 2.004, 95% CI: 1.316–3.052, P = 0.001), heterozygous (OR: 2.220, 95% CI: 1.450–3.400, P = 0.0002), and additive genetic models (OR: 2.000, 95% CI: 1. 330–3.010, P = 0.0009).Conclusions: The presence of the ADRB3 gene Trp64Arg polymorphism is positively associated with EH, especially in the Chinese and Caucasian population. The Arg allele carriers of ADRB3 gene Trp64Arg polymorphism may be at an increased risk for developing EH.

Olfactory dysfunction and tinnitus are age-related otorhinolaryngological disorders with a high prevalence in the elderly population and share several common clinical features. However, there is no study investigating the relationship between these two diseases. We studied the prevalence of olfactory dysfunction and tinnitus among Koreans and studied the relationship between these two diseases based on the Korean National Health and Nutrition Examination Survey. The subjects of this study were enrolled from the Fifth Korean National Health and Nutrition Examination Survey (2010-2012, n = 25,534). Data of subjects aged 40 years and older who underwent physical examination and completed a self-reported questionnaire and other anthropometric variables were statistically analyzed. Odds ratios were calculated to identify the relationship between olfactory dysfunction and tinnitus, using multiple logistic regression models. Older males, non-smokers, non/lower alcohol drinker groups exhibited the relationship between olfactory dysfunction and tinnitus. Metabolic syndrome and mental health problems were associated with both olfactory dysfunction and tinnitus. After adjusting for confounding factors, olfactory dysfunction was significantly associated with tinnitus (OR 1.318). There was a dose-response relationship between tinnitus severity and the odds of olfactory dysfunction (ORs for mild, moderate and severe tinnitus were, respectively, 1.134, 1.569 and 2.044). Additional molecular genetics and animal studies are needed to determine the shared pathophysiology of the two diseases.

BackgroundMany studies suggest that the small ubiquitin-like modifier 4 (SUMO4) M55V gene polymorphism (rs237025) may be associated with an increased risk of type 2 diabetes mellitus (T2DM). However, due to other conflicting results, a clear consensus is lacking in the matter.Objective and methodsA meta-analysis consisting of 6,823 subjects from 10 studies was conducted to elucidate relationship between the SUMO4 M55V gene polymorphism and T2DM. Depending on the heterogeneity of the data, either a fixed or random-effects model would be used to assess the combined odds ratio (ORs) and their corresponding 95% confidence interval (CI).ResultsSUMO4 gene M55V polymorphism was significantly associated with T2DM in the whole population under allelic (OR: 1.18, 95% CI: 1.10–1.28, P = 1.63 × 10−5), recessive (OR: 1.59, 95% CI: 1.14–2.23, P = 0.006), dominant (OR: 0.815, 95% CI: 0.737–0.901, P = 6.89 × 10−5), homozygous (OR: 1.415, 95% CI: 1.170–1.710, P = 0.0003), heterozygous (OR: 1.191, 95% CI: 1.072–1.323, P = 0.001), and additive genetic models (OR: 1.184, 95% CI: 1.097–1.279, P = 1.63 × 10−5). In our subgroup analysis, a significant association was found again in the Chinese population, but not in Japanese or Iranian population.ConclusionSUMO4 gene M55V polymorphism may correlate with increased T2DM risk. Chinese carriers of the V allele of the SUMO4 gene M55V polymorphism may be predisposed to developing T2DM.

Background: Although endothelial cell protein C receptor (EPCR) gene Ser219Gly polymorphism has been associated with venous thromboembolism (VTE) susceptibility, no clear consensus has yet been reached.Objective and methods: A meta-analysis of 9,494 subjects from 13 individual studies was conducted to better elucidate the potential relationship between the EPCR gene Ser219Gly polymorphism and VTE. Pooled odds ratios (ORs) and their corresponding 95% confidence intervals (CIs) were evaluated by using fixed or random effect models.Results: The current meta-analysis suggested that there was a significant association between EPCR gene Ser219Gly polymorphism and VTE under allelic (OR: 1.42, 95% CI: 1.21–1.66, P = 1.30 × 10−5), recessive (OR: 2.02, 95% CI: 1.44–2.85, P = 5.35 × 10−5), homozygous (OR: 2.24, 95% CI: 1.59–3.16, P = 3.66 × 10−6), and additive genetic models (OR: 1.63, 95% CI: 1.30–2.04, P = 2.24 × 10−5).Conclusions:EPCR gene Ser219Gly polymorphism was associated with an elevated risk of VTE and the Gly residue carriers of the EPCR gene might be predisposed to VTE.

We synthesized polyethylene (PE), poly(ethylene-co-1-decene), poly(ethylene-co-p-methylstyrene) and poly(ethylene-ter-1-decene-ter-p-methystyrene) using a rac-Et(Ind)2ZrCl2 metallocene catalyst and a methylaluminoxane cocatalyst system. The materials were characterized using nuclear magnetic resonance spectroscopy and Fourier transform infrared spectroscopy. We compared and studied the kinetics, thermal properties and mechanical ones of these polymers. Tg was related to the amorphous phase of the polymers, whereas the tensile strength and storage modulus (E’) were related to the crystalline phase. We also found that PE has the highest crystallinity through differential scanning calorimetry and wide-angle X-ray scattering analysis. The polymerization rates of poly(ethylene-co-1-decene) and poly(ethylene-ter-1-decene-ter-p-methystyrene) were always higher than that of polyethylene.

ObjectivesThe mucociliary transport system is a major defense mechanism of the respiratory tract. The performance of mucous transportation in the nasal cavity can be represented by a ciliary beating frequency (CBF). This study proposes a novel method to measure CBF by using optical flow.MethodsTo obtain objective estimates of CBF from video images, an automated computer-based image processing technique is developed. This study proposes a new method based on optical flow for image processing and peak detection for signal processing. We compare the measuring accuracy of the method in various combinations of image processing (optical flow versus difference image) and signal processing (fast Fourier transform [FFT] vs. peak detection [PD]). The digital high-speed video method with a manual count of CBF in slow motion video play, is the gold-standard in CBF measurement. We obtained a total of fifty recorded ciliated sinonasal epithelium images to measure CBF from the Department of Otolaryngology. The ciliated sinonasal epithelium images were recorded at 50-100 frames per second using a charge coupled device camera with an inverted microscope at a magnification of ×1,000.ResultsThe mean square errors and variance for each method were 1.24, 0.84 Hz; 11.8, 2.63 Hz; 3.22, 1.46 Hz; and 3.82, 1.53 Hz for optical flow (OF) + PD, OF + FFT, difference image [DI] + PD, and DI + FFT, respectively. Of the four methods, PD using optical flow showed the best performance for measuring the CBF of nasal mucosa.ConclusionsThe proposed method was able to measure CBF more objectively and efficiently than what is currently possible.

Purpose: To report a case of unilateral cytomegalovirus (CMV) panuveitis with occlusive vasculitis after injection of intravitreal dexamethasone implant in a patient with type 2 diabetes mellitus. Methods: Observational case report. Results: A 60-year-old immunocompetent man with well-controlled type 2 diabetes mellitus was treated with intravitreal dexamethasone implant for recurrent uveitis that was responsive to steroids. Three months after implantation, the patient develops panuveitis with occlusive vasculitis. Anterior chamber tap confirms diagnosis of cytomegalovirus retinitis. Intravitreal foscarnet and oral valganciclovir led to quiescent disease. Conclusions: Patients treated with local immunosuppressants should be monitored carefully to assess treatment response and complications, even in the absence of frank immunodeficiency. Background: Pre-diabetes can develop into type 2 diabetes mellitus, but can prevented by regular exercise. However, the outcomes when combining unsupervised Kinect-based mixed-reality (KMR) exercise with continuous glucose monitoring (CGM) remain unclear. Therefore, this single-arm pilot trial examined changes in blood glucose (BG) concentrations over 672 hours (4 weeks), including a 2-week period of KMR exercise and CGM in individuals with pre-diabetes. Methods: This was a pre-and post-treatment case-control study with nine participants. General questionnaires were administered and body composition, fasting BG concentrations, and 2-hour oral glucose tolerance test (2- OGTT) results were measured pre-and post-treatment. Weekly average glucose concentrations, hyperglycemia rate, hypoglycemia rate, average glucose concentration over time, amount of physical activity, amount of food intake, and pre- and postprandial BG (immediately and 30, 60, 90, and 120 minutes after lunch) were measured over 4 weeks (pre-test, exercise, and post-test weeks). Glucose concentrations were measured before exercising, between sets, and 30 and 60 minutes after exercise during the 2 weeks of unsupervised exercise (3 days/week). Results: In all participants, body mass index (27.16±2.92 kg/m²), fasting BG (108.00±7.19 mg/dL), 2-OGTT (162.56±18.12 mg/dL), hyperglycemia rate (P=0.040), and 90-minute postprandial BG (P=0.035) were significantly reduced during the 2 exercise weeks, and the 2-OGTT result (P=0.044) and diastolic blood pressure (DBP) (P=0.046) were significantly reduced at the post-test as compared with the pre-test. Conclusion: This study found that 2 weeks of unsupervised KMR exercise reduced 2-OGTT, DBP, hyperglycemia rate, and 90-minute postprandial BG concentration. In various industries, especially manufacturing and chemical industries, it is often observed that the distribution of a specific process, initially having followed a normal distribution, becomes skewed as a result of unexpected causes. That is, a process deviates from a normal distribution and becomes a skewed distribution. The skew-normal (SN) distribution is one of the most employed models to characterize such processes. The shape of this distribution is determined by the asymmetry parameter. When this parameter is set to zero, the distribution is equal to the normal distribution. Moreover, when there is a shift in the asymmetry parameter, the mean and variance of a SN distribution shift accordingly. In this paper, we propose procedures for monitoring the asymmetry parameter, based on the statistic derived from the noncentral t-distribution. After applying the statistic to Shewhart and the exponentially weighted moving average (EWMA) charts, we evaluate the performance of the proposed procedures and compare it with previously studied procedures based on other skewness statistics.
