In industrial environments, motors in Variable Speed Drive (VSD) systems act as primary conduits for the propagation of conducted electromagnetic disturbances, including Differential Mode (DM). The Split-Phase Induction Motor (SPIM) is among the most widely used motors in ASD systems, making the analysis of its impedances and High-Frequency (HF) properties crucial for ensuring Electromagnetic Compatibility (EMC) and mitigating Electromagnetic Interference (EMI). Poor EMC management can result in malfunctions of nearby electronic devices.This article examines the DM current propagation paths in two motor types. The investigation aims to: (i) Identify the HF characteristics of these motors within the critical frequency range for conducted EMI (150 kHz – 30 MHz), (ii) Assess key EMC performance indicators, focusing on the windings of the main and auxiliary phases, and (iii) Compare the EMC performance of different SPIM designs through experimental analysis to select the most suitable motors for critical EMC applications. This approach not only addresses EMC challenges but also optimizes motor performance and ensures regulatory compliance in electrical systems where these motors play a crucial role.

Abstract Background Gastrosplenic fistula is a rare and potentially fatal complication of various conditions. Lymphoma is the most common cause. It can occur spontaneously or after chemotherapy. Gastrosplenic fistula diagnosis can be confused with a splenic abscess because of the presence of air into the mass. The computed tomography identification of the fistulous tract is the key to a right diagnosis. Treatment modalities include surgical resection, chemotherapy, or a combination of both. Case presentation Here we report two patients with gastrosplenic fistula due to diffuse large B cell lymphoma. The first patient was a 54-year-old Caucasian woman with an enormous primary splenic diffuse large B cell lymphoma leading to the development of a spontaneous fistula in the stomach. The second patient was a 48-year-old Caucasian male patient with an enormous splenic diffuse large B cell lymphoma complicated by fistula after chemotherapy. Both patients died of septic shock several days after surgery. Conclusion Gastrosplenic fistula is a rare complication with a poor-prognosis, for which surgery is currently the preferred treatment.

The Single-Phase Induction Motor (SPIM) has gained widespread adoption in various power applications. This article introduces a new study and analysis of SPIM, focusing on its high-frequency (HF) modeling within the conducted electromagnetic interference (EMI) frequency range of interest (150 kHz– 30MHz). This research is crucial for ensuring electromagnetic compatibility (EMC), evaluating electromagnetic interference issues, optimizing machine design, and complying with EMC standards. To identify common-mode current propagation paths accurately, a precise impedance analyzer is required to determine the common-mode impedances models of the high-frequency motor. The article proposes an HF modeling method based on a genetic algorithm (GA), which offers the advantages of high accuracy, versatility, and fast simulation time. This approach effectively reproduces models, trends, and relationships observed in real data, indicating a high level of fidelity and reliability in its predictions. Case studies validate the efficiency of the proposed HF modeling method. These precise predictions of undesired HF phenomena provide valuable insights into identifying areas where parasitic currents may propagate in the motor and understanding the impact of each motor phase on the overall behavior of the motor’s common-mode impedance.

The present study offers a comprehensive content analysis of the Forward-Looking on Green Human Resources Management (GHRM) research, providing insights into significant findings, determinants, challenges, and future trends of GHRM. The role of GHRM in developing green behaviour and improving environmental performance is becoming increasingly important, with evidence of positive effects on competitive advantage, company sustainability, and employee engagement. In addition, the study underscores the relevance of GHRM for organisational sustainability, focusing on different HRM practices such as recruitment, the Rewards System, and employee involvement. Furthermore, it identifies vital determinants of GHRM, such as integration of technology, effective green HRM policies, employee environmental awareness, top management support, and the promotion of a sustainable organisational culture. In parallel, challenges such as lack of awareness, need for practice change, insufficient training, absence of standard metrics, and cultural boundaries, are recognised. The study delineates promising trends for the future of GHRM, such as technological integration, a stronger focus on training and development, strategic integration of GHRM, broader application across different sectors, increased regulatory compliance, and an emphasis on green employee behaviour. Importantly, the study concludes by acknowledging the critical role of GHRM in promoting environmental sustainability within organisations while advancing employee engagement and performance, underlining the need for further research to solidify the business case for GHRM and contribute to a more sustainable future.

Abstract Background Uterine fibroids incarcerated in abdominal wall hernias during pregnancy are rare, with only six cases reported in umbilical hernias. This case report presents the first reported case of an incarcerated uterine fibroid in an epigastric hernia. Case presentation A 31-year-old primigravid Caucasian woman at 28 weeks gestational age presented with sudden onset abdominal pain and vomiting. Physical examination revealed an incarcerated epigastric hernia containing a non-reducible firm mass. Ultrasound showed a healthy fetus, and during surgery, a subserosal and sessile fibroid originating from the anterior uterine wall was found in the hernia sac. It was easily reduced, and the hernia was repaired with no complications. The patient proceeded to deliver a healthy baby boy by cesarean section at full term. Conclusion Uterine fibroids incarcerated in abdominal wall hernias during pregnancy are rare and affect mostly primigravid women in the third trimester. Abdominal ultrasound may facilitate the diagnosis, and pedunculated fibroids may be resected while sessile fibroids should be simply reduced. Clinicians should consider incarcerated fibroid as a differential diagnosis in pregnant women with irreducible ventral abdominal wall hernias. This case report aims to contribute to the literature and optimize the management of abdominal wall hernias in pregnant women.

Current research on photovoltaic generators is focused on improving operational efficiency and ensuring a long-life service, previous studies indicate that the degradation of photovoltaic generators generally results from the stress of various climatic factors in the natural environment; Sandstorms being among them and possibly the most influential where surface erosion is generated. In this paper, a novel simulator to test wind and sandstorms blowing in the southwestern Algeria Desert is presented. The developed simulator was able to generate winds at different speeds, with varying sand grain densities. as well as we rotated the samples using 360° to allow the wind to reach them from all directions. These properties adopted in the simulator will be used in subsequent research in accelerated tests to predict and estimate the degradation under the influence of sandstorms separately from any other climatic factor. We created an actual database of sand and wind density, taking measurements during the monsoon season. the obtained results confirming in consistency with those measured. We have succeeded to estimate the photovoltaic unit lifetime, and ensure better operating conditions.

In this study, we will shed light on the aggressive effect of a blowing sandstorm in the presence of high temperatures on the photovoltaic inter-module connectors in a solar station located in the desert environment of southwestern Algeria. After a short period of operation, it is observed that the MC4 connectors, which tighten the interconnection between the photovoltaic modules, are completely faulty because of sand grains carried away by a wind blowing at an average annual speed, which exceeds 6 m/s. Then, we analyzed and evaluated the connector failure to propose an appropriate solution. We checked the variations of the intrinsic temperatures of each connector employing the thermocouple for the three cases of operating temperatures, as well as the current measurement flowing through each connector, to calculate the powers dissipated in each branch of the circuit. Finally, this experimental work shows that a loss of power reaches almost 10% from the nominal power of the typical case. The proposed solution for this problem is to cover the connectors with an impervious plastic cover.

Global political and financial entities have witnessed many scandals linked to leaders’ unethical behavior. Previous research suggests that the susceptibility of leaders to act in an unethical way is rooted in various individual, organizational, and social factors. However, unethical leadership is scarcely examined in the context of African countries. This study explores the role of poor leadership skills and qualities in eliciting unethical leadership behavior in Algerian public organizations. Using an exploratory qualitative approach, 15 leaders and managers were included in series of in-depth interviews. The results of the data analysis revealed that poor leadership skills and competencies are critical antecedents of unethical leadership behavior within Algerian public organizations. This study contributes to the ongoing research efforts that seek to identify and minimize the causes of unethical behaviors of leaders across sectors and disciplines. This study provides insights into the construct of unethical leadership and how individual, organizational, and social factors predict leaders’ unethical behavior. While there are many studies on the issue of unethical leadership, research on the potential role of poor leadership skills in eliciting unethical behavior is scarce, especially in Algeria. This study found that there is a strong link between skills and competencies of leaders and their ethical behavior within organizational settings.

Purpose: Doing a bibliometric analysis of the papers on Total Quality Management published in Algerian Scientific Journals Platform (ASJP). Methodology/Approach: Following Bibliometric analysis approach that reaches the export of scientific papers from ASJP during 2005-2022 and using Bibiloshiny based on the R language (RStudio). Findings: The study highlighted the prominent authors, universities, and journals, identifying both opportunities and research advancements in TQM research and urging scientific collaboration. TQM practices in different sectors remain a current and relevant research topic, however, the analysis outlined some conceptual structure gaps within particular areas. The outputs provide insights and suggestions for scholars and practitioners in the field of TQM. Research Limitation/Implication: The study examines only papers published in ASJP until 2022. Findings could be useful for TQM stakeholders in emerging countries. Practitioners benefit from experiments to deal with the TQM method. Originality/Value of paper: In Algerian context, the study’s substantive contribution denotes that Industrial and Health care Areas constitute knowledge and empirical gaps in the TQM field; for future, researchers should exploit key words used by prominent authors; the TQM performance and higher education are the most explored topics as Motor themes; organisational culture as emerging theme had grown interest need exploration; total quality or continuous improvement themes are important for the TQM research require further works.

In order to provide new information on hybrid photovoltaic systems, this paper presents a technical, economic, and ecological study of a hybrid PV system connected to a storage system and utilized to power a remote site. We came up with a plan of action based on the HOMER software in order to accomplish our objective. The results indicate that, in comparison to PV alone and diesel alone, the hybrid system is the optimum choice given the location selected using local resources and the characteristics of the load to be provided. In addition, the hybrid system offers various advantages over diesel alone in terms of technology, economy, and even the environment. All gas emissions are significantly reduced, and the results highlight the importance of renewable energy in reducing gas emissions. These results might assist manufacturers by providing the answers to various queries regarding this kind of installation.

In this study, a three-dimensional (CFD) model is employed to simulate and optimize the CCS (Channel Cross-Section) shape of the single straight channel PEMFC. Four CCS shapes, namely trapeze, inverted trapeze, half of ellipse and inverted half of ellipse, are investigated using ANSYS-FLUENT software and compared to the rectangular and triangular CCS shapes. The results obtained from the simulation are compared to the experimental results of the literature. A good agreement is observed between the numerical and experimental results. From the obtained results, it appears that the best delivered power density is reported by the trapeze CCS configuration, whereas, the worst delivered power density is obtained by the inverted half of ellipse CCS configuration. The highest pressure-drop and pumping power are obtained with the triangular CCS configuration and the smallest are resulted by the rectangular CCS configuration. Finally, the highest net power output is reported by the trapeze channel cross-section configuration, while, the lowest one is yielded by the inverted half of ellipse CCS configuration.

Purpose: Feedback conversation is the process of conveying information in the performance appraisal process. It helps employees to develop the right and appropriate behavior in order to achieve the targeted outcome. This study investigates the influence of feedback conversation (i.e., feedback frequency, the credibility of the feedback provider, receptive capability, organizational culture, and national culture) on employees’ performance in Malaysian-based telecommunication companies. Design/Methodology/Approach: The study utilized a descriptive quantitative approach, in which a 5-point Likert scale questionnaire has been used during data collection. A total of 341 respondents have participated out of 3019 employees in the selected company. The collected data were then be analyzed using SPSS and the inferential analysis of Multilinear Regression (MLR) was applied to test the hypotheses. Findings: MLR analysis revealed that the credibility of the feedback provider, the receptive capability of employees, and organizational culture and national culture have a significant positive impact on employees' performance. Meanwhile, feedback frequency has no significant impact on the employees’ performance. Implications/Originality/Value: This study has added values to management literature and offered practical managerial implications related to selected aspects of feedback conversations on the employees’ performance. Other similar organizational settings may also consider these findings to design and develop a more effective feedback conversation to boost employees’ performance.

Pancreatic metastasis (PM) of renal cancer is a rare condition. It is characterized by a long period after initial nephrectomy and a favorable prognosis compared to other pancreatic malignancies. Its diagnosis may confuse clinicians if the medical history is not known. In the era of targeted therapies for metastatic renal carcinoma, surgery stands as the best treatment option for PM of renal cancer. We report the case of a woman who underwent successfully left splenopancreatectomy for corporeal PM of renal cancer treated seven years ago. This case underlines the necessity of long-term follow-up of patients treated for kidney cancer.

The article also shows how the BBC managed to make its Arabic Service an integral part of modern Arab culture and how with the help of skilful broadcasters recruited among the Arab diaspora the Service’s programming testifies to a remarkable history of cultural interchange.

Les tumeurs pseudopapillaires et solides du pancréas (TPPS) sont des tumeurs épithéliales rares. Dans la plupart des cas, il s'agit de tumeurs survenant chez la femme jeune dans la deuxième ou la troisième décennie de la vie. La survie après résection primaire approche 90% à 5 ans. Nous rapportons le cas d'une jeune patiente de la vingtaine qui présente une tumeur pseudopapillaire et solide du pancréas découverte devant des douleurs abdominales sans perturbations des bilans biologiques. La tomodensitométrie (TDM), l'imagerie par résonance magnétique (IRM) et l'échographie endoscopique ont révélé une masse bien limitée se développant au dépend de l'isthme pancréatique. L'exérèse complète de la tumeur a été réalisée. L'examen anatomopathologique confirmait le diagnostic de tumeur pseudopapillaire et solide du pancréas. En conclusion, les tumeurs pseudopapillaires et solides du pancréas doivent être évoquées comme un des diagnostics différentiels de toute masse pancréatique en particulier chez les jeunes femmes. L'exérèse chirurgicale procure un bon pronostic.

Le lymphangiome kystique est une tumeur bégnine malformative rare des vaisseaux lymphatiques à localisations diverses. La localisation intra-abdominale du lymphangiome kystique est moins fréquente comparée à la localisation cervico-axillaire. Sa présentation clinique est polymorphe. Le diagnostic est évoqué par l'imagerie mais il nécessite une confirmation histologique. Le traitement de choix est chirurgical. Nous rapportons ici un cas rare de lymphangiome kystique intrapéritonéale acquis suite à une colectomie subtotale chez une patiente porteuse de maladie de Crohn à géni évolutif sévère et ayant nécessité l'introduction des anti TNF alpha. La patiente s'est présentée pour une masse para médiane droite douloureuse irréductible et non impulsive à la toux faisant retenir le diagnostic d'une éventration étranglée sur cicatrice de laparotomie médiane et amenant à opérer la patiente en urgence. L'exploration chirurgicale a montré une masse multi-kystique intra péritonéale extériorisée en partie à travers l'éventration paramédiane droite. L'examen anatomo-pathologique a permis de confirmer le diagnostic de lymphangiome kystique. Devant une exérèse chirurgicale incomplète, la patiente a eu deux séances de ponction évacuatrice en post-opératoire. C'est le premier cas de lymphangiome kystique rapporté chez un patient sous Anti-TNF alpha. Ceci pourrait être expliqué par la perturbation du système immunitaire et plus spécifiquement la population lymphocytaire. Cette association n'a pas été jusque-là prouvée et des études expérimentales sont nécessaires pour affirmer ou infirmer cette hypothèse.

Desmoid tumours are rare soft tissue tumours developed from myofibroblasts. They usually infiltrate into adjacent muscle bundles and are characterised by the absence of metastatic potential. However, a high local aggressiveness is seen with a high rate of recurrence after surgical excision. We hereby attempt to present a series of patients with desmoid tumours and review treatment options and management strategies. Ten tumours occurred in the abdominal wall, five in the mesentery, and one was presacral tumour. The mean diameter of the tumour was 6 cm (range, 2.3-40 cm). Associated Familial Adenomatous Polyposis (FAP) was found in five patients. The margin of surgical resection was healthy in five patients. During follow up period, two patients returned with recurrence of tumours.

La hernie de l'aine de l'adulte reste une affection fréquente en chirurgie digestive. De nombreuses techniques de réparation ont été décrites à ce jour dont les procédés laparoscopiques. Deux méthodes furent rapidement adoptées par les différents praticiens pour le traitement chirurgical des hernies de l'aine par laparoscopie: la méthode laparoscopique totalement extra péritonéale (TEP) et la méthode laparoscopique transpéritonéale (TAPP). Le but était d'étudier la faisabilité de la cure de hernie de l'aine par celioscopie et de décrire ses résultats du point de vue récidive herniaire et douleur post opératoire. Ce travail était une étude rétrospective, uni centrique, et transversale, portant sur des patients opérés par des chirurgiens du service de chirurgie A La Rabta pour hernie de l'aine par voie laparoscopique, sur une période de 8 ans allant de janvier 2006 à décembre 2013. Le principal critère de jugement était la récidive herniaire. La douleur post opératoire et les complications étaient les critères de jugement secondaires. Nous avons colligés 104 hernies chez 92 patients respectant les critères d'inclusion de notre étude. La moyenne d'âge de nos patients était de 48 Ans (19-83). L'approche TAPP était la plus utilisée: 94 cas (90%) TAPP contre 10 cas TEP. Aucune complication per opératoires n'a été signalée. Le taux de conversion de notre série était nul. La mortalité opératoire était aussi nulle. La morbidité postopératoire était de 5% (5 patients). Elle était à type d'hématome dans 3 cas et de sérum dans 2 cas. La durée moyenne d'hospitalisation était de 1.2 jours (1-4jours). Le séjour post opératoire n'avait pas dépassé 2 jours chez 94% des patients. Seulement 2 patients avaient présenté une récidive. Les douleurs chroniques postopératoires étaient notées chez seulement 3 patients. Notre étude a montré que la cure de hernie de l'aine par laparoscopie a apporté un confort considérable à nos patients en ce qui concerne les phénomènes douloureux, les durées d'hospitalisation et d'arrêt de travail. Les résultats obtenus dans cette série sont bons et conformes aux résultats déjà publiés dans la littérature. Ceci nous encourage à poursuivre l'utilisation de ces techniques et à contrôler nos résultats à plus long terme.

INTRODUCTION: l’appendicite aigue représente l’urgence chirurgicale la plus fréquente aux urgences. Son diagnostic est avant tout clinique. Cependant, sa présentation clinique parfois trompeuse ainsi que le large éventail de diagnostics différentiels sont fréquemment sources d’erreurs diagnostiques et de retard de prise en charge. Afin de pallier à ces difficultés diagnostiques, de réduire le nombre d’examens complémentaires et d’actes chirurgicaux abusifs, plusieurs scores cliniques ont été ainsi développés, dont le score d’Alvarado. L’objectif de cette étude était d’appliquer ce score à une population de patients adultes venant consulter pour douleurs de la fosse iliaque droite afin d’évaluer ses performances ainsi que ses limites.Méthodes: il s’agissait d’une étude prospective qui a inclus tous les malades âgés de plus de 15 ans se présentant pour douleur de la fosse iliaque droite. Le diagnostic final d'appendicite aigue a été confirmé par examen anatomo-pathologique pour les patients opérés et infirmé lorsque la symptomatologie des patients avait totalement régressé en l'absence de tout traitement.Résultats: notre étude a inclus 106 patients. Chez les patients dont le score d'Alvarado était inférieur à 4, le diagnostic d'appendicite aigüe n'a jamais été retenu. Les meilleures sensibilités et spécificités ont été retrouvées pour une valeur seuil de 8 pour le score d'Alvarado. Ainsi, avec une bonne sensibilité (81,25%) et une valeur prédictive positive correcte (74,28%), notre étude a démontré que le score d’Alvarado pouvait apporter un bénéfice dans le diagnostic d'appendicite aigüe. Le groupe de patient avec un score strictement inférieur à 4 est considéré comme à faible risque. Les patients dont le score d'Alvarado était strictement supérieur à 6 nécessiteraient une hospitalisation afin, soit d'être opérer d'emblée, soit de compléter par un examen d'imagerie et une surveillance. Le groupe de patient ayant un score compris entre 4 et 6 (limites incluses), reste un groupe où le doute diagnostic est présent et où les examens complémentaires d'imagerie ont un apport certain.Conclusion: l’utilisation du score d'Alvarado dans nos urgences permet de rationaliser la prise en charge et d’orienter le diagnostic en limitant la prescription d’explorations radiologiques, le coût de la prise en charge et les actes chirurgicaux abusifs.

Les cystadénomes mucineux sont des tumeurs bénignes à potentiel malin. Ils sont souvent révélés par des douleurs abdominales non spécifiques, un ictère, ou un épisode de pancréatite aiguë. Nous rapportons l'observation exceptionnelle d'un cystadénome mucineux doublement compliqué de pancréatite aigue et de rupture dans le rétro-péritoine. Il s'agit d'une patiente âgée de 30 ans, non tarée, qui nous a consulté pour des douleurs épigastriques et de l'hypochondre gauche évoluant depuis trois mois et qui se sont accentuées depuis 3 jours, sans fièvre ni ictère. L'examen clinique avait noté un empâtement à la palpation de l'épigastre et de l'hypochondre gauche. Il n'y avait pas de masse palpable. Le bilan biologique était sans anomalie en dehors d'une lipasémie à 8 fois la normale. Une TDM abdominale a été pratiquée montrant une masse kystique de la queue du pancréas, bi-loculée de 111 mm x 73 mm, à paroi fine et à contenu liquidien, associé à une infiltration du fascia para rénal gauche. Un complément d'IRM (A) a été réalisé montrant un aspect de cystadénome mucineux rompu dans rétro-péritoine. La portion caudale du canal pancréatique principal était légèrement dilatée et communiquait avec le kyste pancréatique. La malade a été opérée par voie bi sous costale. Il s'agissait d'une formation kystique de la queue du pancréas rompue dans le rétro-péritoine associée à une pancréatite aigue (coulée de nécrose de l'espace pré rénal antérieur gauche). Il a alors été réalisé une splénopancréatectomie caudale (B). Les suites opératoires ont été simples. L'examen anatomo-pathologique de la pièce opératoire avait conclu à un cystadénome mucineux pancréatique siège de lésions de dysplasie de bas grade.

L'ingestion d'un corps étranger (CE) est une urgence médicale courante. Chez l'adulte, cet accident est beaucoup plus rare que chez l'enfant et survient le plus souvent sur des terrains prédisposés. Nous rapportons le cas d'un patient âgé de 36 ans, suivi en psychiatrie, admis en urgence pour l'ingestion d'un fil de fer faisant 15 cm de long et qui a été spontanément expulsé à travers la paroi abdominale au niveau de l'épigastre.L'examen clinique était sans anomalie notamment pas de signe péritonéaux (A). La tomodensitométrie abdominale (B) avait mis en évidence un corps étranger linéaire extériorisé depuis l'antre gastrique à travers un trajet fistuleux gastro cutané. Le fil de fer a été retiré sous anesthésie générale par une traction douce extériorisant le bout restant dans l'antre laissant une fistule gastro cutané qui n'a rien ramené.Les suites opératoires étaient simples. L''ingestion de CE est un problème fréquent en pratique courante. Près de 80% des CE ingérés traversent spontanément la filière digestive sans aucune complication. Dix à 20% nécessitent la réalisation d'une extraction endoscopique et moins de 1% une intervention chirurgicale. L''expulsion spontanée à traves la paroi abdominale reste un événement exceptionnel.

La pancréatite aiguë médicamenteuse représente environ 2% des pancréatites aiguës. Son incidence est actuellement en augmentation avec plus de 260 médicaments incriminés. Cependant, cette pathologie reste encore peu rapportée dans la littérature, car se pose le problème de l'imputabilité. Nous rapportons notre expérience à travers une série de 10 patients colligés sur une période de 7 ans. La présentation clinique de la PA était souvent non univoque. Le score de Ranson variait de 0 à 5. Nous avons recensé 5 cas de pancréatites édémateuses et 5 cas de pancréatites nécrotico-hémorragiques. Ces pancréatites étaient souvent résolutives et sans récidive après arrêt définitif du médicament incriminé.

Intraductal Tubulopapillary Neoplasms (ITPN) is a rare and new entity defined as an intraductal, grossly visible, tubule-forming epithelial neoplasm with high- grade dysplasia and ductal differentiation without overt production of mucin. Its clinical presentation can be varied, which makes the diagnosis quite challenging. In this report, we present a case of pancreatic ITPN and review the published work to learn clinicopathological, radiological features and treatment strategies of this recently proposed pancreatic neoplasm.

A duodenal diverticulum is a pouch attached to the duodenum which may be present in 20% of the population. Although they are common entities, symptoms caused by duodenal diverticula are relatively rare and complications such as diverticulitis remain a difficult clinical problem. Nonoperative management has emerged as a safe, practical alternative to surgery in selected patient. We present a rare case of duodenal diverticulitis and its successful conservative management. A 57-year-old man was admitted to the Emergency Department with a 4-day history of epigastric and right upper quadrant pain. The patient claimed a six month history of abdominal pain and weight loss. Physical examination shows fever and tenderness of the epigastric and right upper quadrant. Laboratory tests revealed an elevated leukocyte count with normal liver tests, lipase level. Abdominal X-ray showed no intra-peritoneal free air. Computer tomography of the abdomen reveals an infected duodenal diverticulum with infiltration of neighboring fat. The patient was admitted to the acute care surgical service for a conservative management: nasogastric suction, bowel rest, intravenous antibiotic therapy, parenteral nutrition with a close clinical observation. The patient improved and was discharged on hospital day 10 without complications.

This article provides an overview of British trade unions’ activism against the United Kingdom staying first in the EEC and then in the European Union during the referendum campaigns held in 1975 and 2016. It assesses the evolution of the unions’ agenda and rhetoric from one referendum campaign to the other. The argument put forward consists in regarding British trade unions’ commitment to tripartism as the main motivation for their anti-Marketeer stand in 1975, whereas their cautious commitment over Brexit has developed into a left-wing alliance with Jeremy Corbyn, the new leader of the Labour Party. Hence their common aim to take part in a left-led pan-European dynamic eager to fight against the neoliberal policies of the European Union.

Transposition of the gallbladder to the left side without situs inversus viscerum is rare. These gallbladders are situated under the left lobe of the liver between Segment III and IV or on Segment III to the left of the falciform ligament. This is a report of a 50-year-old woman who was admitted to our department with a history of pain in her right upper abdomen. The physical examination showed tenderness in the right upper quadrant of the abdomen without a Murphy's sign. Abdominal ultrasonography showed gall bladder stones without dilatation of the bile ducts. The patient underwent a laparoscopic cholecystectomy using the French position and four ports positioned as usual. We discovered a left-sided gallbladder located on the left of the round ligament. The gallbladder was excised as usual. Intraoperative cholangiogram showed neither dilatation of the bile ducts nor associated congenital anomalies of the biliary tree. The patient was discharged on the first postoperative day. Because routine preoperative examinations may not detect the anomaly, the latter may take surgeons by surprise during laparoscopy. Awareness of the unpredictable confluence of the cystic duct into the common bile duct and selective use of intraoperative cholangiography both contributed to the safe laparoscopic management of this unusual problem.

La torsion de frange épiploïque (ou appendagite) est une pathologie rare qui survient principalement chez les adultes entre 20 et 50 ans. L'incidence de cette pathologie n'est pas réellement connue et elle varie de 2 à 7% chez les patients hospitalisés pour suspicion d'appendicite ou de sigmoïdite. Nous rapportons cinq cas d'appendagites dont nous précisons les particularités cliniques, radiologiques et thérapeutiques. Il s'agit de 5 patients dont l'âge moyen est de 34.6 ans (24-55). Le sexe ratio est de 1.5. Le principal motif de consultation était un syndrome douloureux de l'abdomen principalement au niveau de la fosse iliaque droite. L'examen abdominal montrait toujours une sensibilité localisée. La fièvre était présente chez 3 patients. Le bilan biologique révèle un syndrome inflammatoire biologique chez trois patients. Les examens complémentaires radiologiques en particulier échographie abdominale et TDM abdominale ont éliminé formellement une urgence chirurgicale et ont évoqué le diagnostic d'appendagite dans trois cas. Trois patients ont bénéficié d'une célioscopie diagnostique confirmant le diagnostic d'appendagite. L'évolution était favorable chez tous les patients. Les appendagites épiploïques primitives sont des étiologies rares et sous-estimées de syndrome abdominal aigu. Le diagnostic peut être affirmé par imagerie notamment avec le scanner hélicoïdal injecté, permettant d'instaurer ainsi un traitement médical premier et d'éviter un traitement chirurgical et des hospitalisations excessives.

INTRODUCTION: Surgery remains the basic treatment of hepatic hydatid cyst (HHC). However, it is associated with significant morbidity. The aim of our study was to evaluate mortality and morbidity of surgery of the HHC and to highlight the risk factors. METHODS: A retrospective study was conducted from January 1, 1996 to December 31, 2006. 391 patients hospitalized for HHC and operated in the Department of General Surgery ,A, of the Rabta Hospital in Tunis, Tunisia. RESULTS: The overall mortality rate was 0.7% while the overall morbidity rate was 20.4%. About 16.6% suffered from specific complications, while 3.8% suffered from non-specific complications. Predictors of morbidity in a univariate analysis included cysts larger than 9 cm, dome cysts, cysts with bilious contents, type II, III, IV or V on ultrasound classification, fissured cysts and intrabiliary rupture of hepatic hydatid cyst. The multivariate study consisted of independent predictors of disease at the site of the liver dome, the cysto-biliary fistula and intrabiliary rupture of hepatic hydatid cyst. CONCLUSION: The hepatic hydatid cyst of the dome and the existence of preoperative complications in particular intrabiliary rupture of hepatic hydatid cyst are the main factors of morbidity. A better understanding of these factors allows the surgeon to choose the appropiate surgical technique that is associated with less morbidity.

This article’s rationale is that the Arabic Service of the BBC holds a special position in the development of British public diplomacy and that such a position has its roots in the 1937 negotiations between the BBC and the Foreign Office that led to its creation in 1938. These discussions resulted in the BBC’s willingness to handle the task of British projection responsibly, that is to say resorting to the omission of news which in the view of the Foreign Office would be inadvisable in times of crisis. The article seeks to show that this non-written agreement prevailed in the following six decades and is still valid up to the present day. The paper investigates the coverage of three moments of crisis in the history of British foreign policies in the Middle East. The first moment concerns the controversial news bulletin of 4 January 1938, the very first BBC broadcast in a foreign language. The other two moments relate to the Suez crisis and the first Gulf war. The article’s main line of inquiry therefore is to offer specific insights into the notion of soft power mediated by diasporic Arab broadcasters in circumstances when the stakes were high for Britain in the Middle East. The aim is to assess both the resilience and the ambiguities of John Reith’s vision of British projection through a model of broadcasting of his own making in a sensitive and complex region for the country’s formal diplomacy.

Introduction and Importance: Schwannomas are rare benign tumors that develop from Schwann cells that represent 0.3 to 0.4 cases per 100,000 persons per year. We report a case of pre-sacral schwannoma, a rare tumor, especially in the pelvic area. This case comes to help further teams in their management as its scarcity made any attempt to make proper recommendations obsolete., Case Presentation: a 53-year-old otherwise healthy woman who presented with a 2-year history of right-sided sciatica. The radiologic characteristics of the mass suggested presacral schwannoma type III of the Klimo classification as the most possible diagnosis.Therefore, surgical resection was decided, and an anterior approach was chosen. By laparotomy a 6cm retroperitoneal encapsulated mass with no invasion of the adjacent organs. We performed a digital enucleation of the tumor through a capsulotomy.Pathology confirmed the diagnosis of schwannoma. There were no features of malignancy. The post-operative period was uneventful. A follow-up examination at 6 months showed no signs of numbness or weakness in the right leg. The previously described pain totally regressed., Discussion: Although schwannoma is a benign lesion, it may become malignant, especially when associated with neurofibromatosis making its surgical removal primordial. Its pelvic location may make its diagnosis delayed due to non-specific symptoms mainly through compression of local organs. Its surgical management can be challenging due to large size tumors with adherence to peritoneal and retroperitoneal organs. Quality of the resection is important in the recurrence and necessity for reoperation. Pellagra is a nutritional disease caused by the deficiency of niacin. It is a clinical syndrome characterized by four "D's": diarrhea, dermatitis, dementia and ultimately death.
