In der Vergangenheit gab es in Deutschland unterschiedliche Empfehlungen zur Kariespravention durch Fluoridanwendung im Sauglings- und fruhen Kindesalter (0 bis 6 Jahre). Parallel existierende, unterschiedliche Empfehlungen fuhren jedoch zur Verunsicherung von Beratungskraften und Eltern/Betreuungspersonen, zu geringer Akzeptanz und zu unzureichender Umsetzung der Empfehlungen. Vertreterinnen und Vertreter der relevanten Fachgesellschaften und -gruppen bewerteten in einem vom Netzwerk Gesund ins Leben koordinierten Prozess die wissenschaftliche Evidenz, unter Berucksichtigung von Umsetzbarkeit und Erreichbarkeit auch von Familien mit erhohtem Kariesrisiko. In einem mehrstufigen Prozess wurden gemeinsame Empfehlungen formuliert. Von der Geburt bis zum Zahndurchbruch sollen Sauglinge ein Supplement mit 400–500 I.E. Vitamin D und 0,25 mg Fluorid erhalten. Nach dem Zahndurchbruch wird das Kind behutsam und allmahlich an das Zahneputzen herangefuhrt. Dabei soll entweder die Weiterfuhrung der systemischen Fluoridanwendung (0,25 mg Fluorid und 400–500 I.E. Vitamin D) oder die Fluoridanwendung durch Zahnpasta mit 1000 ppm Fluorid (bis zu 2‑mal taglich, jeweils bis zu 0,125 g, Reiskorngrose) gewahlt werden; das Vitamin-D-Supplement wird bis zum zweiten erlebten Fruhsommer weitergefuhrt. Ab 12 Monaten wird das 2‑mal tagliche Zahneputzen mit einer fluoridhaltigen Zahnpasta (1000 ppm Fluorid) fur alle Kinder empfohlen, zunachst mit einer geringen Zahnpastamenge (jeweils bis zu 0,125 g, Reiskorngrose), ab 24 Monaten mit einer groseren Menge (jeweils bis zu 0,25 g, Erbsengrose). Um eine zu hohe Fluoridaufnahme zu vermeiden, ist eine korrekte Dosierung der Zahnpasta unerlasslich. Die Aufklarung und Beratung zur Kariespravention und zur korrekten Umsetzung der Empfehlungen sollen im Rahmen der padiatrischen und zahnarztlichen Fruherkennungsuntersuchungen sowie im Rahmen der zahnmedizinischen Gruppenprophylaxe nach § 21 SGB V erfolgen.

2012 hatte das Netzwerk Gesund ins Leben erstmalig Empfehlungen herausgegeben, die sich mit der Ernährung und dem Lebensstil in der Schwangerschaft befassen. Behandelt werden unter anderem Themen wie Körpergewicht, Nährstoffbedarf, Supplemente und körperliche Aktivität. Diese Empfehlungen wurden nun aktualisiert und um das Thema präkonzeptioneller Lebensstil ergänzt. Eine Zusammenfassung von Katharina Kerlen-Petri.

Endorsement date 21 January 2021 Implementation date 27 March 2021 Following a request from the European Commission, EFSA was asked to provide scientific and technical guidance for the preparation and presentation of a dossierfor evaluation of an infant and/or follow‐on formula manufactured from protein hydrolysates. This guidance document addresses the information and data to be submitted to EFSA on infant and follow‐on formulae manufactured from protein hydrolysates with respect to the nutritional safety and suitability of the specific formula and/or the formula's efficacy in reducing the risk of developing allergy to milk proteins. The guidance will be further reviewed and updated with the experience gained from the evaluation of specificdossiers, and in the light of applicable Unionguidelines and legislation. The guidance was adopted by the Panel on Dietetic Products, Nutrition and Allergies on 5 April 2017.Upon request from the European Commission in 2020, it has been revised to inform food business operators of the new provisions in the pre‐submission phase and in the procedure set out in the General Food Law, as amended by the Transparency Regulation. This revised guidance applies to all dossiers submitted as of 27 March 2021 and shall be consulted for the preparation of dossiers intended to be submitted from that date onwards. For dossiers submitted prior to 27 March 2021, the previous guidance, published in May 2017 remains applicable.

Following a request from the European Commission, the EFSA Panel on Dietetic Products, Nutrition and Allergies (NDA) derives dietary reference values (DRVs) for vitamin K. In this Opinion, the Panel considers vitamin K to comprise both phylloquinone and menaquinones. The Panel considers that none of the biomarkers of vitamin K intake or status is suitable by itself to derive DRVs for vitamin K. Several health outcomes possibly associated with vitamin K intake were also considered but data could not be used to establish DRVs. The Panel considers that average requirements and population reference intakes for vitamin K cannot be derived for adults, infants and children, and therefore sets adequate intakes (AIs). The Panel considers that available evidence on occurrence, absorption, function and content in the body or organs of menaquinones is insufficient, and, therefore, sets AIs for phylloquinone only. Having assessed additional evidence available since 1993 in particular related to biomarkers, intake data and the factorial approach, which all are associated with considerable uncertainties, the Panel maintains the reference value proposed by the Scientific Committee for Food (SCF) in 1993. An AI of 1 μg phylloquinone/kg body weight per day is set for all age and sex population groups. Die Entscheidung der Mutter uber die Ernahrung ihres Sauglings fallt in der Regel schon vor der Geburt. Deshalb sollte im Rahmen der Geburtsvorbereitung von erfahrener Seite auch uber Sauglingsernahrung aufgeklart werden. Wer dies tut (Frauenarzt oder Hebamme, Kinderarzt, Kinderkrankenschwester oder Laktationsberaterin) spielt keine Rolle, entscheidend sind einvernehmliche umfassende Sachkenntnisse und eine einfuhlsame Darstellung. Dazu ist auch eine systematische Weiterbildung des Klinikpersonals und eine entsprechende Betreuung der Mutter wahrend des Klinikaufenthalts unumganglich. Nur so lassen sich Angste und Nervositat bei der Mutter und damit Unruhe beim Kind sicher vermeiden, die ein erfolgreiches Stillen gefahrden. Eine intensive Stillforderung, wie sie u. a. von WHO und UNICEF mit der weltweiten „Baby-Friendly Hospital Initiative“ eingeleitet wurde, verdient jegliche Unterstutzung von Seiten der Kinderarzte. Dazu gehoren u. a. die Ermoglichung des initialen Hautkontaktes zwischen der Mutter und ihrem Neugeborenen, das ihr auf den Bauch gelegt wird, das Anlegen des Neugeborenen innerhalb der ersten 1 bis 2 Stunden post partum, die Beschrankung einer Zufutterung auf die wenigen wirklich indizierten Falle, gemeinsame Unterbringung von Mutter und Kind (Rooming-in) sowie kundige, widerspruchsfreie und warmherzige Beratung und Betreuung. Sauglinge, die nicht gestillt werden, erhalten nach den gesetzlichen Regelungen hergestellte Sauglingsanfangsnahrung oder nach Einfuhrung von Beikost eine Folgenahrung. Beikost wird fruhestens ab dem Beginn des 5. Lebensmonats und spatestens ab dem Beginn des 7. Lebensmonats eingefuhrt, je nach der individuellen sensomotorischen Entwicklung des Sauglings und in gleicher Weise fur Kinder mit und ohne familiar erhohtes Allergierisiko. Gegen Ende des 1. Lebensjahres geht die spezielle Sauglingsernahrung nach und nach in die Teilnahme an Mahlzeiten der Familienernahrung uber.

The European Commission asked EFSA to deliver an opinion on the nutritional safety and suitability of a specific protein hydrolysate. It is derived from whey protein concentrate and used in an infant and follow-on formula by Danone Trading ELN B.V, which submitted a dossier to the European Commission to request an amendment of Regulation (EU) 2016/127 with respect to the protein sources that may be used in the manufacture of infant and/or follow-on formula. This opinion does not cover the assessment of the safety of the food enzymes used in the manufacture of the protein hydrolysate. The protein hydrolysate under evaluation is sufficiently characterised with respect to the fraction of the hydrolysed protein. In the pertinent intervention study provided, an infant formula manufactured from the protein hydrolysate with a protein content of 2.3 g/100 kcal and consumed as the sole source of nutrition by infants for 3.5 months led to growth equivalent to a formula manufactured from intact cow's milk protein (2 g protein/100 kcal). No experimental data have been provided on the nutritional safety and suitability of this protein source in follow-on formula. However, given that it is consumed with complementary foods and the protein source is considered nutritionally safe and suitable in an infant formula that is the sole source of nutrition of infants, the Panel considers that the protein hydrolysate is also a nutritionally safe and suitable protein source for use in follow-on formula. The Panel concludes that the protein hydrolysate under evaluation is a nutritionally safe and suitable protein source for use in infant and follow-on formula, as long as the formula in which it is used contains a minimum of 2.3 g/100 kcal protein and complies with the compositional criteria of Commission Delegated Regulation (EU) 2016/127 and the amino acid pattern in its Annex IIIA. Fur Eltern ist es oft eine sensible Phase, in der sie alles richtig machen mochten. Hier liegt eine grose Chance fur die Beratung. Schlieslich werden bereits im fruhkindlichen Alter die Weichen fur das spatere Ernahrungs- und Bewegungsverhalten sowie die Gesundheit im Erwachsenenalter gestellt.

Following a request from the European Commission, the Panel on Nutrition, Novel Foods and Food Allergens (NDA) revised its 2009 Opinion on the appropriate age for introduction of complementary feeding of infants. This age has been evaluated considering the effects on health outcomes, nutritional aspects and infant development, and depends on the individual's characteristics and development. „Gesund ins Leben – Netzwerk Junge Familie“ ist ein Bestandteil von „IN FORM – Deutschlands Initiative fur gesunde Ernahrung und mehr Bewegung“ der Bundesregierung und wird durch das Bundesministerium fur Ernahrung und Landwirtschaft gefordert. In diesem bundesweiten Netzwerk haben sich medizinische und wissenschaftliche Fachgesellschaften, Berufsverbande sowie fachlich ausgerichtete Institutionen zusammengeschlossen, um Eltern rund um die Geburt mit Informationen zur Ernahrung, zur Bewegung und zur Allergiepravention zu begleiten und zu unterstutzen. Die 2010 erstmals erschienenen Handlungsempfehlungen zu Sauglingsernahrung und Ernahrung der stillenden Mutter wurden auf Basis aktueller Empfehlungen relevanter Fachorganisationen und Institutionen sowie einschlagiger wissenschaftlicher Publikationen von den Mitgliedern des wissenschaftlichen Beirats des Netzwerks diskutiert, uberarbeitet und im Konsens formuliert. Diese Empfehlungen werden zum einen als anwendungsorientierte, alltagsbezogene Botschaften an junge Familien weitergegeben, zum anderen in der Fortbildung der Multiplikatoren, erganzt durch weiterfuhrende Informationen, eingesetzt. Die Empfehlungen umfassen die Themen Stillen, Sauglingsnahrung, Beikost, Getranke (erganzende Flussigkeitszufuhr), Essenlernen, Ernahrung der stillenden Frau, Nahrstoffsupplemente fur den Saugling und die stillende Mutter sowie Alkohol, Rauchen und Medikamente in der Stillzeit. Des Weiteren gehoren allgemeine Empfehlungen zur Allergiepravention beim Kind sowie Empfehlungen zur Bewegung im Sauglingsalter und zur Bewegung der stillenden Frau dazu.

Hintergrund: Ernahrung, Bewegung und die Lebensweise vor und wahrend der Schwangerschaft beeinflussen die mutterliche und kindliche Gesundheit. Werdende Eltern werden dabei haufig mit unterschiedlichen Aussagen konfrontiert. Deshalb hat das Netzwerk „Gesund ins Leben – Netzwerk Junge Familie“ – Teil des Nationalen Aktionsplans IN FORM der Bundesregierung – harmonisierte Handlungsempfehlungen zur Ernahrung und Bewegung in der Schwangerschaft als Beratungsstandard entwickelt. Material und Methodik: Die Autoren haben sich bei der Entwicklung der Handlungsempfehlungen vor allem auf vorhandene Leitlinien, Metaanalysen und systematische Ubersichten gestutzt. Ergebnisse: Bis zum Ende der Schwangerschaft steigt der Energiebedarf nur um etwa 10 %, wahrend der Bedarf an einzelnen Mikronahrstoffen viel starker zunimmt. Die Ernahrungsempfehlungen orientieren sich an den Empfehlungen fur die Allgemeinbevolkerung. Zusatzlich sollen schon bei Kinderwunsch und mindestens im ersten Schwangerschaftsdrittel 400 µg Folsaure/Tag als Supplement eingenommen werden. Neben jodreichen Lebensmitteln und Jodsalz wird ein tagliches Supplement mit 100 – 150 µg Jod empfohlen. Eine Vitamin-D-Supplementierung ist ratsam, falls keine regelmasige Sonnenlichtexposition erfolgt. Eisenpraparate sollen nur individuell nach medizinischer Anamnese und Blutuntersuchung verwendet werden. Bei gezielter Lebensmittelauswahl ist eine gute Nahrstoffversorgung auch mit einer ovo-lakto-vegetarischen Ernahrung plus den genannten Supplementen moglich. Bei einer rein pflanzlichen (veganen) Ernahrung ist eine ausreichende Versorgung ohne weitere Nahrstoffsupplemente nicht zu gewahrleisten. Schwangere sollten korperlich aktiv sein, sportliches Training aber nur mit masiger Intensitat betreiben. Schlussfolgerungen: Multiplikatoren sollten werdende Eltern zu einem gesundheitsfordernden Lebensstil motivieren. Dafur bieten die Handlungsempfehlungen die fachliche Beratungsgrundlage. Um den Bekanntheitsgrad der Handlungsempfehlungen zu steigern, bietet das Netzwerk Medien fur Fachkrafte und junge Familien sowie Multiplikatorenfortbildungen an.

Key Points Question What is the best way to ensure the validity of clinical trials of breast milk substitutes while protecting trial participants? Findings Through a Delphi consensus project, guidance was developed to address issues specific to trials of breast milk substitutes assessing growth and tolerance, as well as trials of breast milk substitutes with other objectives. This consensus guidance summarizes best practice for the design, conduct, analysis, and reporting of trials of breast milk substitutes. Meaning Use of this guidance, in conjunction with existing clinical trial regulations, should enhance the quality and validity of trials of breast milk substitutes, protect trial participants, and support the evidence base for infant nutrition recommendations., Importance Breast milk substitutes (BMS) are important nutritional products evaluated in clinical trials. Concerns have been raised about the risk of bias in BMS trials, the reliability of claims that arise from such trials, and the potential for BMS trials to undermine breastfeeding in trial participants. Existing clinical trial guidance does not fully address issues specific to BMS trials. Objectives To establish new methodological criteria to guide the design, conduct, analysis, and reporting of BMS trials and to support clinical trialists designing and undertaking BMS trials, editors and peer reviewers assessing trial reports for publication, and regulators evaluating the safety, nutritional adequacy, and efficacy of BMS products. Design, Setting, and Participants A modified Delphi method was conducted, involving 3 rounds of anonymous questionnaires and a face-to-face consensus meeting between January 1 and October 24, 2018. Participants were 23 experts in BMS trials, BMS regulation, trial methods, breastfeeding support, infant feeding research, and medical publishing, and were affiliated with institutions across Europe, North America, and Australasia. Guidance development was supported by an industry consultation, analysis of methodological issues in a sample of published BMS trials, and consultations with BMS trial participants and a research ethics committee. Results An initial 73 criteria, derived from the literature, were sent to the experts. The final consensus guidance contains 54 essential criteria and 4 recommended criteria. An 18-point checklist summarizes the criteria that are specific to BMS trials. Key themes emphasized in the guidance are research integrity and transparency of reporting, supporting breastfeeding in trial participants, accurate description of trial interventions, and use of valid and meaningful outcome measures. Conclusions and Relevance Implementation of this guidance should enhance the quality and validity of BMS trials, protect BMS trial participants, and better inform the infant nutrition community about BMS products., This consensus guidance summarizes best practice for the design, conduct, analysis, and reporting of trials of breast milk substitutes.

Diet and exercise before and during pregnancy affect the course of the pregnancy, the child's development and the short- and long-term health of mother and child. The Healthy Start - Young Family Network has updated the recommendations on nutrition in pregnancy that first appeared in 2012 and supplemented them with recommendations on a preconception lifestyle. The recommendations address body weight before conception, weight gain in pregnancy, energy and nutritional requirements and diet (including a vegetarian/vegan diet), the supplements folic acid/folate, iodine, iron and docosahexaenoic acid (DHA), protection against food-borne illnesses, physical activity before and during pregnancy, alcohol, smoking, caffeinated drinks, oral and dental hygiene and the use of medicinal products. Preparation for breast-feeding is recommended already during pregnancy. Vaccination recommendations for women planning a pregnancy are also included. These practical recommendations of the Germany-wide Healthy Start - Young Family Network are intended to assist all professional groups that counsel women and couples wishing to have children and during pregnancy with uniform, scientifically-based and practical information.Ernährung und Bewegung vor und während der Schwangerschaft wirken sich auf den Schwangerschaftsverlauf, die Entwicklung des Kindes und die kurz- und langfristige Gesundheit von Mutter und Kind aus. Das Netzwerk Gesund ins Leben hat die 2012 erstmals erschienenen Empfehlungen zur Ernährung in der Schwangerschaft aktualisiert und um Empfehlungen zum präkonzeptionellen Lebensstil ergänzt. Die Empfehlungen adressieren das Körpergewicht vor der Konzeption, die Gewichtsentwicklung in der Schwangerschaft, Energie- und Nährstoffbedarf sowie Ernährungsweise (inklusive vegetarische/vegane Ernährung), die Supplemente Folsäure/Folat, Jod, Eisen und Docosahexaensäure (DHA), den Schutz vor Lebensmittelinfektionen, körperliche Aktivität vor und in der Schwangerschaft, Alkohol, Rauchen, koffeinhaltige Getränke, Mund- und Zahngesundheit und den Umgang mit Arzneimitteln. Die Vorbereitung auf das Stillen wird bereits in der Schwangerschaft empfohlen. Für Frauen, die eine Schwangerschaft planen, sind zudem Impfempfehlungen enthalten. Diese Handlungsempfehlungen des bundesweiten Netzwerks Gesund ins Leben sollen alle Berufsgruppen, die Frauen und Paare mit Kinderwunsch und in der Schwangerschaft beraten, mit harmonisierten, wissenschaftsbasierten und anwendungsorientierten Informationen unterstützen.

Following a request from the European Commission, the Panel on Dietetic Products, Nutrition and Allergies (NDA) was asked to revise the tolerable upper intake level (UL) for vitamin D for infants (≤ 1 year) set in 2012. From its literature review, the Panel concluded that the available evidence on daily vitamin D intake and the risk of adverse health outcomes (hypercalciuria, hypercalcaemia, nephrocalcinosis and abnormal growth patterns) cannot be used alone for deriving the UL for infants. The Panel conducted a meta‐regression analysis of collected data, to derive a dose–response relationship between daily supplemental intake of vitamin D and mean achieved serum 25(OH)D concentrations. Considering that a serum 25(OH)D concentration of 200 nmol/L or below is unlikely to pose a risk of adverse health outcomes in infants, the Panel estimated the percentage of infants reaching a concentration above this value at different intakes of vitamin D. Based on the overall evidence, the Panel kept the UL of 25 μg/day for infants aged up to 6 months and set a UL of 35 μg/day for infants 6–12 months. The Panel was also asked to advise on the safety of the consumption of infant formulae with an increased maximum vitamin D content of 3 μg/100 kcal (Commission Delegated Regulation (EU) 2016/127 repealing Directive 2006/141/EC in 2020). For infants aged up to 4 months, the intake assessment showed that the use of infant formulae containing vitamin D at 3 μg/100 kcal may lead some infants to receive an intake above the UL of 25 μg/day from formulae alone without considering vitamin D supplemental intake. Die Ernahrungskommission der Deutschen Gesellschaft fur Kinder- und Jugendmedizin hat ihre Empfehlungen zur Ernahrung gesunder Sauglinge aktualisiert und als Expertenempfehlung in einer Ubersicht zusammengestellt. Zu 5 Hauptthemen (Stillen, Muttermilchersatz, Beikost, Familienkost, ubergreifende Aspekte) werden insgesamt 35 Kernaussagen formuliert und mit Hinweisen fur die praktische Anwendung kommentiert. Empfohlen wird Stillen ohne Zufutterung in den ersten 4 bis 6 Lebensmonaten, und weitergefuhrtes Stillen neben der Beikost. Vor allem postnatal ist eine Unterstutzung beim Stillen wichtig, einschlieslich Verzicht auf routinemasige Zufutterung. Nicht gestillte Sauglinge sollen Sauglingsanfangsnahrung erhalten, ebenfalls zusatzlich zur Beikost. Bei erhohtem Atopierisiko wird Hydrolysatnahrung (HA) bis zur Einfuhrung der Beikost empfohlen. Die Verwendung von Folgenahrung ist nach Einfuhrung der Beikost moglich. Flaschennahrung muss frisch zubereitet und unmittelbar verfuttert werden. Beikost sollte nicht vor dem Beginn des 5. Lebensmonats und nicht spater als mit Beginn des 7. Lebensmonats eingefuhrt werden. Empfohlen wird gleichzeitig die Einfuhrung kleiner Mengen Gluten und eine Variation der Lebensmittel. Als erste Beikostmahlzeit eignet sich ein Gemuse-Kartoffel-Fleisch- oder -Fisch-Brei (gut bioverfugbares Eisen, Omega-3-Fettsauren), gefolgt von einem (Kuh-)Milch-Getreide-Brei und einem Getreide-Obst-Brei, selbst hergestellt oder als industriell hergestelltes Produkt. Etwa ab dem Alter von 10 Monaten kann ausgewogene Familienkost eingefuhrt werden. Eine laktovegetarische Ernahrung ist moglich, eine vegane Ernahrung ohne Supplementierung ist fur Sauglinge abzulehnen. Die Supplementierung von 2 mg Vitamin K oral (am 1. Lebenstag, zwischen dem 3. und 10. Lebenstag und zwischen der 4. und 6. Lebenswoche) sowie von Vitamin D (400–500 IU/d) ab der 2. Woche kombiniert mit Fluorid (0,25 mg) als Tablette wird empfohlen.

Hintergrund Ernahrung und Bewegung im fruhen Kindesalter beeinflussen die gesunde Entwicklung und das Wohlbefinden des Kindes. Das Netzwerk „Gesund ins Leben – Netzwerk Junge Familie“, ein Projekt von „IN FORM – Deutschlands Initiative fur gesunde Ernahrung und mehr Bewegung“, entwickelte Empfehlungen fur die Ernahrung und Bewegung von Kindern von 1–3 Jahren.

Ernahrung, Bewegung und die Lebensweise vor und wahrend der Schwangerschaft beeinflussen die mutterliche und kindliche Gesundheit. Das Netzwerk „Gesund ins Leben – Netzwerk Junge Familie“ – Teil des Nationalen Aktionsplans IN FORM der Bundesregierung – entwickelte gemeinsam mit Fachgesellschaften einheitliche Handlungsempfehlungen zur Ernahrung in der Schwangerschaft als Grundlage der Beratung werdender Eltern. Bis zum Ende der Schwangerschaft steigt der Energiebedarf nur um etwa 10 %, wahrend der Bedarf an einzelnen Mikronahrstoffen viel starker zunimmt. Normalgewicht sollte moglichst schon vor der Schwangerschaft angestrebt werden. Die Ernahrungsempfehlungen orientieren sich an den Empfehlungen fur die Allgemeinbevolkerung. Zusatzlich sollen schon bei Kinderwunsch und mindestens im ersten Schwangerschaftsdrittel 400 µg Folsaure/Tag als Supplement eingenommen werden. Neben jodreichen Lebensmitteln und Jodsalz wird ein tagliches Supplement mit 100–150 µg Jod empfohlen. Zur Bedarfsdeckung an langkettigen Omega-3-Fettsauren wird wochentlich mindestens eine Portion fettreicher Meeresfisch empfohlen, bei Verzicht auf Fischverzehr die Einnahme eines Praparates mit der Omega-3-Fettsaure Docosahexaensaure. Eine Vitamin-D-Supplementierung ist ratsam, falls keine regelmasige Sonnenlichtexposition erfolgt. Arzte, Hebammen und weitere Multiplikatoren sollten Paare mit Kinderwunsch zu einer gesundheitsfordernden Lebensweise motivieren. Teil 2 des Beitrags stellt die weiteren Kernaussagen und Handlungsempfehlungen zur Ernahrung der Mutter, auch zu vegetarischer und veganer Ernahrung, Empfehlungen zum Schutz vor Lebensmittelinfektionen, Empfehlungen zu Supplementen, zu Bewegung und zu Genussmitteln in der Schwangerschaft dar.

Bei einer rein pflanzlichen (veganen) Ernahrung ist eine ausreichende Versorgung ohne weitere Nahrstoffsupplemente nicht zu gewahrleisten. Zur Vorbeugung von Listeriose und Toxoplasmose sollen rohe tierische Lebensmittel, Weichkase und abgepackte frische Salate gemieden sowie frisches Obst, Gemuse und Salate immer grundlich gewaschen und bald verzehrt werden. Schwangere sollten korperlich aktiv sein, sportliches Training aber nur mit masiger Intensitat betreiben. Sie sollen Alkohol meiden, nicht rauchen und sich nicht in Raumen aufhalten, in denen geraucht wird und wurde. Bis zu drei Tassen Kaffee am Tag gelten als unbedenklich, Energydrinks sollten jedoch nicht konsumiert werden. Das Meiden bestimmter Lebensmittel in der Schwangerschaft hat keinen Nutzen fur eine kindliche Allergievorbeugung. Der regelmasige Verzehr fettreicher Meeresfische wird auch fur die Allergiepravention empfohlen. Arzte, Hebammen und weitere Multiplikatoren sollten Paare mit Kinderwunsch zu einer gesundheitsfordernden Lebensweise motivieren. Complementary food is needed when breast milk (or infant formula) alone is no longer sufficient for both nutritional and developmental reasons. The timing of its introduction, therefore, is an individual decision, although 6 months of exclusive breastfeeding can be recommended for most healthy term infants. The new foods are intended to ‘complement’ ongoing breastfeeding with those dietary items whose intake has become marginal or insufficient. Both breastfeeding and complementary feeding can have direct or later consequences on health. The evaluation of consequences of both early and late introduction of complementary food can neither disregard the effect of breastfeeding compared to formula feeding nor the composition or quality of the complementary food. Possible short-term health effects concern growth velocity and infections, and possible long-term effects may relate to atopic diseases, type 1 and 2 diabetes, obesity and neuromuscular development. On the basis of the currently available evidence, it is impossible to exactly determine the age when risks related to the start of complementary feeding are lowest or highest for most of these effects, with the possible exception of infections and early growth velocity. The present knowledge on undesirable health effects, however, is mainly based on observational studies, and although some mechanisms have been proposed, further prospective studies have to clarify these unsolved issues. Even less evidence on the consequences of the timing of complementary food introduction is available for formula-fed infants.

In den letzten Jahren brachten verschiedene Hersteller Milchgetranke fur Kleinkinder unter Bezeichnungen wie Kindermilch, Kleinkindmilch, Juniormilch oder im englischen Sprachraum „milk for kids“, „growing-up milk“ (GUM) oder „toddler milk“ auf den Markt, die aufgrund ihrer Zusammensetzung als vorteilhaft gegenuber Kuhmilch beworben werden. Die Ernahrungskommission der Deutschen Gesellschaft fur Kinder- und Jugendmedizin (DGKJ) stellt, wie bereits im Jahr 2001, fest, dass spezielle Milchgetranke einschlieslich Folgemilch fur die Ernahrung von Kleinkindern grundsatzlich nicht notwendig sind. Wenn jedoch Trinkmilch durch Kleinkindermilch ersetzt wird, sollten sich die Produkte in Bezug auf Kalzium-, Vitamin-A- und -B2-Gehalt an Vollmilch und in Bezug auf den Energiegehalt an teilentrahmter Kuhmilch orientieren. Wunschenswert ist, dass sie sich in Bezug auf den Jod- und Vitamin-D-Gehalt an den Vorgaben der Folgenahrung orientieren. Aromastoffe und Susungsmittel sollten moglichst vermieden werden. Die Produkte sollen aus Tasse oder Becher und nicht aus einer Saugflasche getrunken werden.

Netzwerk Das Netzwerk Gesund ins Leben – Netzwerk Junge Familie ist ein Projekt von IN FORM – Deutschlands Initiative fur gesunde Ernahrung und mehr Bewegung. In diesem bundesweiten Netzwerk haben sich medizinische und wissenschaftliche Fachgesellschaften, Berufsverbande sowie fachlich ausgerichtete Institutionen zusammengeschlossen, um Eltern rund um die Geburt mit Informationen zur Ernahrung und zur Allergiepravention zu begleiten und zu unterstutzen.

Food authorities should base their decisions on both risk assessment and benefit assessment in relation to foods, food constituents or nutrients when risk–benefit assessment can be carried out. Benefit assessment should mirror the well-established steps of risk assessment. However, methods for their comparison, weighing risks against benefits, have as yet to be developed. The general framework currently available in risk–benefit health assessment of food is described and illustrated with two examples: food fortification with vitamins and minerals, and the consumption of vegetables in relation to their nitrate content.

Die Vitamin-K-Prophylaxe bei Sauglingen dient der Pravention von Vitamin-K-Mangel-Blutungen. In jungerer Zeit wird in Deutschland eine niedrigdosierte tagliche oder eine wochentliche Gabe von Vitamin K propagiert. Die Ernahrungskommission hat die Datenlage gepruft. Eine postnatale Vitamin-K-Gabe von 1 mg oral mit nachfolgender taglicher Gabe von 25 μg oral wird wegen unzureichender Schutzwirkung nicht empfohlen. Angeraten wird weiterhin eine 3-malige orale Gabe von 2 mg Vitamin K jeweils am 1. Lebenstag (U1), zwischen dem 3. und dem 10. Lebenstag (U2) und erneut zwischen der 4. und der 6. Lebenswoche (U3). Diese Form der Prophylaxe ist effektiv, kann aber nicht alle Falle von spaten Vitamin-K-Mangel-Blutungen verhindern. Die wirksamste Form der Vitamin-K-Prophylaxe ist die einmalige intramuskulare Gabe von 1 mg Vitamin K1 rasch nach der Geburt. Der Verdacht einer Risikoerhohung fur Leukamien oder andere Tumoren konnte nicht bewiesen werden. Eine parenterale Prophylaxe wird bei Reifgeborenen (1 mg Vitamin K1 i.m.) mit schlechtem Allgemeinzustand, bei Verdacht auf Resorptionsstorungen oder bei Zweifeln an der Durchfuhrbarkeit der 3-maligen oralen Vitamin-K-Gabe sowie bei Fruhgeborenen mit einem Geburtsgewicht unter 1500 g (0,2 mg/kg i.m. oder s.c.) empfohlen.

Data from Berlin (Germany) show a significant correlation between the increase in the stork population around the city and the increase in deliveries outside city hospitals (out-of-hospital deliveries). However, there is no correlation between deliveries in hospital buildings (clinical deliveries) and the stork population. The decline in the number of pairs of storks in the German state of Lower Saxony between 1970 and 1985 correlated with the decrease of deliveries in that area. The nearly constant number of deliveries from 1985 to 1995 was associated with an unchanged stork population (no statistical significance). However, the relevance of the stork for the birth rate in that part of Germany remains unclear, because the number of out-of-hospital deliveries in this area is not well documented. A lack of statistical information on out-of-hospital deliveries in general is a severe handicap for further proof for the Theory of the Stork.

Within the group of botanical products there is a large range of variation with regard to their properties. Some products are identical to foods while others come close to or are medicines. Botanical products are regulated differently within the different member states of the European Union (EU) and globally. They are regulated either as food or as medicinal products, and in the latter case often with simplified registration procedures. These differences are caused by differences in traditional use, in cultural and historical background, in scientific substantiation and in enforcement of current legislation. One may expect that in the future differences will remain, unless EU legislation is enacted with sufficient room for different approaches. The strengths and weaknesses of the different regulatory procedures have been reviewed and evaluated as well as the current methods for quality, efficacy and safety evaluation. Criteria to categorize botanical products have been defined, such that botanical products can be regulated under the current food and medicinal regulations. Furthermore, a decision tree has been developed as a tool to distinguish herbal medicinal products from botanical health products and vice versa, and to provide a stepwise framework for the assessment of safety and efficacy.

Legislation on a particular food or on a particular claim to be used in connection with a food require a definition of the food and unequivocal requirements for the use of the claim. The definitions of prebiotics and probiotics presently place these terms between the categories for conventional foods and foods for special dietary uses. Because probiotics and prebiotics, as a group, do not fulfill the criteria for special dietary uses, they have to comply with the rules and laws for conventional foods even if the requirements for the use of the terms prebiotic and probiotic include effects on body functions. These effects on the health and wellness of the consumer and to stimulatory activity, eg, body defense mechanisms, can be used in claims that should underline the importance of the total dietary pattern. It is suggested that setting up rules for the use of the terms prebiotic and probiotic is preferable to creating new food standards.

Dietary treatment of phenylketonuria is well established to be safe and to prevent developmental and mental impairment in patients with low or absent phenylalanine hydroxylase activity. The use of semi-synthetic diets necessitates careful and longitudinal control not only of physical and intellectual development, which are both near normal in well treated patients, but also of potential diet inherent insufficiencies of essential nutrients. Concern has been raised by some reports on growth retardation in young patients on strict diets and on decreased bone density in older phenylketonuric children. The clinical significance of these findings is not known.Changes have been found, although inconsistently, in connection with selenium, zinc, iron, retinol and polyunsaturated fatty acid status in dietetically treated patients with phenylketonuria. Both the mechanism and significance of these changes is doubtful at present.

The identification and characterization of benefits as a consequence of consumption of food, food constituents or nutrients used to be neglected in comparison to the assessment of risks because the safety of food had priority. Interest in benefit assessment is the consequence of the realisation that both adverse and positive effects on health can follow the consumption of the same food or food constituent and that a balance between the two should be the aim. Moreover, proven benefits in connection with food are the basis of health related claims on food labels. Benefit assessment should follow a procedure which is parallel to structured risk assessment and apply the same stringent criteria with respect to substantiation. Benefits will consist of either the reduction of the probability of adverse health effects or the increase of the probability of positive health effects.

Diet and physical activity before and during pregnancy affect short- and long-term health of mother and child. The energy needs at the end of pregnancy increase only by about 10% compared to nonpregnant women. An excessive energy intake is undesirable since maternal overweight and excessive weight gain can increase the risks for a high birth weight and later child overweight and diabetes. Maternal weight at the beginning of pregnancy is especially important for pregnancy outcome and child health. Women should strive to achieve normal weight already before pregnancy. Regular physical activity can contribute to a healthy weight and to the health of pregnant women. The need for certain nutrients increases more than energy requirements. Before and during pregnancy, foods with a high content of essential nutrients should be preferentially selected. Supplements should include folic acid and iodine, iron (in case of suboptimal iron stores), the ω-3 fatty acid docosahexaenoic acid (in case of infrequent consumption of ocean fish) and vitamin D (in case of decreased sun exposure and decreased endogenous vitamin D synthesis). Pregnant women should not smoke and not stay in rooms where others smoke or have smoked before (passive smoking). Alcohol consumption should be avoided, since alcohol can harm unborn children.

Possible short-term health effects concern growth velocity and infections while possible long-term effects may relate to obesity, cardiovascular disease, autoimmunity (celiac disease and type 1 diabetes) and atopic disorders. For most of these it is impossible on the basis of the available evidence to conclude on the age when risks related to the start of complementary feeding are lowest or highest, with the possible exception of infections and early growth velocity. For undesirable health consequences, whilst potential mechanisms are recognized, the evidence from mostly observational studies is insufficient and requires more and prospective research. While the 6-month goal is desirable, introduction of suitable complementary food after 4 completed months with ongoing breastfeeding can be considered without adverse health consequences for infants living in affluent countries. Even less evidence on the consequences of the timing of complementary food introduction is available for formula-fed infants.
