Abstract

Introduction: Early focal brain injuries lead to long-term disabilities with frequent cognitive impairments, suggesting global dysfunction beyond the lesion. While plasticity of the immature brain promotes better learning, outcome variability across individuals is multifactorial. Males are more vulnerable to early injuries and neurodevelopmental disorders than females, but long-term sex differences in brain growth after an early focal lesion have not been described yet. With this MRI longitudinal morphometry study of brain development after a Neonatal Arterial Ischemic Stroke (NAIS), we searched for differences between males and females in the trajectories of ipsi- and contralesional gray matter growth in childhood and adolescence, while accounting for lesion characteristics. Methods: We relied on a longitudinal cohort (AVCnn) of patients with unilateral NAIS who underwent clinical and MRI assessments at ages 7 and 16 were compared to age-matched controls. Non-lesioned volumes of gray matter (hemispheres, lobes, regions, deep structures, cerebellum) were extracted from segmented T1 MRI images at 7 (Patients: 23 M, 16 F; Controls: 17 M, 18 F) and 16 (Patients: 18 M, 11 F; Controls: 16 M, 15 F). These volumes were analyzed using a Linear Mixed Model accounting for age, sex, and lesion characteristics. Results: Whole hemisphere volumes were reduced at both ages in patients compared to controls (gray matter volume: -16% in males, -10% in females). In ipsilesional hemisphere, cortical gray matter and thalamic volume losses (average -13%) mostly depended on lesion severity, suggesting diaschisis, with minimal effect of patient sex. In the contralesional hemisphere however, we consistently found sex differences in gray matter volumes, as only male volumes were smaller than inmale controls (average-7.5%), mostly in territoriesmirroring the contralateral lesion. Females did not significantly deviate from the typical trajectories of female controls. Similar sex differences were found in both cerebellar hemispheres. Discussion: These results suggest sex-dependent growth trajectories after an early brain lesion with a contralesional growth deficit in males only. The similarity of patterns at ages 7 and 16 suggests that puberty has little effect on these trajectories, and that most of the deviation in males occurs in early childhood, in line with the well-described perinatal vulnerability of themale brain, and with no compensation thereafter. Abstract

Introduction: Fetal Alcohol Spectrum Disorders (FASD) are characterized by a variety of multiple cognitive and behavioral impairments, with intellectual, attentional, and executive impairments being the most commonly reported. In populations with multiple neurodevelopmental disorders, the Full Scale Intelligence Quotient (FSIQ) may not be a proper measure of intellectual abilities, rarely interpreted in FASD clinical practice because the heterogeneity of the cognitive profile is deemed too strong. We propose a quantitative characterization of this heterogeneity, of the strengths and weaknesses profile, and a differential analysis between global cognitive (FSIQ) and elementary reasoning abilities in a large retrospective monocentric FASD sample. Methods: Using clinical and cognitive data (Wechsler Intelligence Scale for Children) from 107 children with FASD, we characterized subject heterogeneity (variance and scatter of scaled/composite scores), searched for strengths and weaknesses, and specified intellectual functioning in terms of FSIQ and elementary reasoning (General Abilities Index, Highest Reasoning Scaled Score), in comparison with standardization norms and a Monte-Carlo-simulated sample from normalization data. Results: Performance of children with FASD was lower on all subtests, with a significant weakness in working memory and processing speed. We found no increase in the variance and scatter of the scores, but a discordance between the assessment of global cognitive functioning (28% borderline, 23% deficient) and that of global and elementary reasoning abilities (23–9% borderline, 15–14% deficient). Conclusion: Our results question the notion of WISC profile heterogeneity in FASD and point to working memory and processing speed over-impairment, with global repercussions but most often preserved elementary reasoning abilities. Abstract

OBJECTIVE:Motion-robust multi-slab imaging of hippocampal inner structure in vivo at 7T.MATERIALS AND METHODS:Motion is a crucial issue for ultra-high resolution imaging, such as can be achieved with 7T MRI. An acquisition protocol was designed for imaging hippocampal inner structure at 7T. It relies on a compromise between anatomical details visibility and robustness to motion. In order to reduce acquisition time and motion artifacts, the full slab covering the hippocampus was split into separate slabs with lower acquisition time. A robust registration approach was implemented to combine the acquired slabs within a final 3D-consistent high-resolution slab covering the whole hippocampus. Evaluation was performed on 50 subjects overall, made of three groups of subjects acquired using three acquisition settings; it focused on three issues: visibility of hippocampal inner structure, robustness to motion artifacts and registration procedure performance.RESULTS:Overall, T2-weighted acquisitions with interleaved slabs proved robust. Multi-slab registration yielded high quality datasets in 96 % of the subjects, thus compatible with further analyses of hippocampal inner structure.CONCLUSION:Multi-slab acquisition and registration setting is efficient for reducing acquisition time and consequently motion artifacts for ultra-high resolution imaging of the inner structure of the hippocampus.

13. The restricted SAR protocol: A method to assess MRI coil prototypes in an unconditionally safe manner.

Abstract

Purpose: Testing an RF coil prototype on subjects involves laborious verifications to ensure its safety. In particular, it requires preliminary electromagnetic simulations and their validations on phantoms to accurately predict the specific absorption rate (SAR). For coil design validation with a simpler safety procedure, the restricted SAR (rS) mode is proposed, enabling representative first experiments in vivo. The goal of the developed approach is to accelerate the transition of a custom coil system from prototype to clinical use. Methods: The restricted specific absorption rate (SAR) (rS) mode imposes a radical limitation on the transmitted RF power based on a worst‐case scenario of local RF power absorption. The limitations used are independent of the SAR spatial distribution, making this approach unconditionally safe. The developed rS protocol contains the sequences required for coil evaluation and satisfies the imposed rS conditions. It provides a quantitative characterization of the coil transmission and reception profiles and a qualitative evaluation of the anatomical images. Protocol validation was performed on commercial and pre‐industrial prototype coils on a small cohort of healthy volunteers. Results: The proposed rS protocol enables coil evaluation within an acquisition time compatible with common clinical protocol duration. The total time of all evaluation steps does not exceed 17 min. At the same time, the global SAR remains 100 times less than the International Electrotechnical Commission safety limit for played sequences. Conclusion: The rS protocol allows characterizing and comparing coil prototypes on volunteers without extensive electromagnetic calculations and phantom validations in an unconditionally safe way. Abstract

International audience; As a long-term consequence of neonatal arterial ischaemic stroke (NAIS), the presence of cerebral palsy (CP) depends on the structural integrity of brain areas, especially of basal ganglia. Yet, it remains challenging to establish an early diagnosis of CP from a conventional structural MRI. In this study, we introduce a graph neural network-based classification for the recognition of NAIS children and mainly for the detection of children with CP among the NAIS ones. From the structural MRI of 68 children aged 7 years old and their corresponding segmentation of basal ganglia, we construct graphs where nodes represent structures, carrying on node and edge attributes structural information (volumes, distances). The classification accuracy achieved by the proposed method is of 86% for the detection of NAIS and of 89% for the detection of CP among neonatal stroke children.

Abstract

International audience; INTRODUCTION: Working memory (WM) disorders often persist long time after an acquired brain injury (ABI), and involve difficulties in daily life (Azouvi et al. 2017). For several decades, methods of cognitive rehabilitation and training in healthy subjects have been developed. But contrary to the literature on healthy subjects (Papa et al, 2020), to date there are still very few studies evaluating the cerebral correlates of rehabilitation in patients with ABI. The objective of this study is to study the effects of cognitive rehabilitation on brain plasticity. METHODS: PLASTIM-MDTC is a study with a repeated single case experimental design on individuals with moderate to severe TBI. After two baseline cognitive assessments, patients underwent WM rehabilitation during 3 months (3/week). The effectiveness of rehabilitation was evaluated post-rehabilitation and after 3 months. A multimodal MRI was performed before and after rehabilitation. PRELIMINARY RESULTS: We will present the results obtained for 3 patients. The results showed an effectiveness of rehabilitation on the behavioral level and an evolution of the functional activation profile on MRI during an n-back task. Primary outcome measures showed an average improvement in the Brown-Peterson task and a mean decrease in complaints in daily life related to WM. However, the evolution profile differed from one patient to another, with higher improvement in patients with more severe baseline impairments, variability between outcome measure in post and 3 months follow-up assessment, and different fMRI n-back evolution profile (decrease or increase). Interestingly, a far transfer in long-term memory appears in some patients, especially during the 3-months follow-up. DISCUSSION-CONCLUSION: The preliminary results of the PLASTIM study are in agreement with those observed in the repeated case studies of Hubacher et al (2015) on MS patients. The continuation of this research should make it possible to study the different evolution profiles and contribute to the search for early markers of the effectiveness of rehabilitation.

National audience; INTRODUCTION: Des troubles de la mémoire de travail (MDT) persistent souvent à long terme après une lésion cérébrale acquise, et engendrent des difficultés en vie quotidienne (Azouvi et al., 2017). Contrairement à la littérature sur le sujet sain (Papa et al., 2020), il n’existe encore à ce jour que très peu d’études évaluant les corrélats cérébraux de la rééducation chez le patient cérébrolésé. L’objectif de cette étude est donc d’étudier les effets de la rééducation cognitive sur la plasticité cérébrale.METHODE: L’étude PLASTIM-MDTC est une étude en cas unique répétés sur des patients TC modéré à sévères. Après deux évaluations cognitives en ligne de base, les patients ont bénéficié d’une rééducation de la MDT pendant 3 mois (3/sem). L’efficacité de la rééducation à été évalué en post rééducation et après 3 mois. Une IRM multimodale est effectuée avant et après rééducation.RESULTATS PRELIMINAIRES : Les résultats montrent une efficacité de la rééducation sur le plan comportemental et une évolution du profil d’activation fonctionnel à l’IRM lors d’une tâche de n-back. Mais le profil d’évolution diffère d’un patient à l’autre, tant sur le plan cognitif qu’à l’IRMf. Nous présenterons ici quelques études de cas.DISCUSSION : Les résultats préliminaires de l’étude PLASTIM sont en accord avec ceux observés dans les études de cas répétées de Hubacher et al. (2015) sur des patients SEP et constituent une occasion d’étudier les interactions et complémentarités entre données cognitives et IRM, notamment en terme de perception de l’efficacité de la rééducation selon le type de donnée.

Abstract

Aim: To identify easily accessible neuroanatomical abnormalities useful for diagnosing fetal alcohol spectrum disorders (FASD) in fetal alcohol syndrome (FAS) but more importantly for the probabilistic diagnosis of non‐syndromic forms (NS‐FASD). Method: We retrospectively collected monocentric data from 52 individuals with FAS, 37 with NS‐FASD, and 94 paired typically developing individuals (6–20 years, 99 males, 84 females). On brain T1‐weighted magnetic resonance imaging, we measured brain size, corpus callosum length and thicknesses, vermis height, then evaluated vermis foliation (Likert scale). For each parameter, we established variations with age and brain size in comparison individuals (growth and scaling charts), then identified participants with abnormal measurements (<10th centile). Results: According to growth charts, there was an excess of FAS with abnormally small brain, isthmus, splenium, and vermis. According to scaling charts, this excess remained only for isthmus thickness and vermis height. The vermis foliation was pathological in 18% of those with FASD but in no comparison individual. Overall, 39% of those with FAS, 27% with NS‐FASD, but only 2% of comparison individuals presented with two FAS‐recurrent abnormalities, and 19% of those with FAS had all three. Considering the number of anomalies, there was a higher likelihood of a causal link with alcohol in 14% of those with NS‐FASD. Interpretation: Our results suggest that adding an explicit composite neuroanatomical–radiological criterion for FASD diagnosis may improve its specificity, especially in NS‐FASD. What this paper adds: Neuroanatomical anomalies independent of microcephaly can be measured with clinical‐imaging tools.Small‐for‐age brain, small‐for‐brain‐size callosal isthmus or vermian height, and disrupted vermis foliation are fetal alcohol syndrome (FAS)‐recurrent anomalies.Associations of these anomalies are frequent in fetal alcohol spectrum disorder (FASD) even without FAS, while exceptional in typically developing individuals.These associations support higher likelihood of causal link with alcohol in some individuals with non‐syndromic FASD.A new explicit and composite neuroanatomical–radiological criterion can improve the specificity of FASD diagnosis. Abstract

In recent years, exploration of the developing brain has become a major focus for researchers and clinicians in an attempt to understand what allows children to acquire amazing and unique abilities, as well as the impact of early disruptions (eg, prematurity, neonatal insults) that can lead to a wide range of neurodevelopmental disorders. Noninvasive neuroimaging methods such as MRI are essential to establish links between the brain and behavioral changes in newborns and infants. In this review article, we aim to highlight recent and representative studies using the various techniques available: anatomical MRI, quantitative MRI (relaxometry, diffusion MRI), multiparametric approaches, and functional MRI. Today, protocols use 1.5 or 3T MRI scanners, and specialized methodologies have been put in place for data acquisition and processing to address the methodological challenges specific to this population, such as sensitivity to motion. MR sequences must be adapted to the brains of newborns and infants to obtain relevant good soft-tissue contrast, given the small size of the cerebral structures and the incomplete maturation of tissues. The use of age-specific image postprocessing tools is also essential, as signal and contrast differ from the adult brain. Appropriate methodologies then make it possible to explore multiple neurodevelopmental mechanisms in a precise way, and assess changes with age or differences between groups of subjects, particularly through large-scale projects. Although MRI measurements only indirectly reflect the complex series of dynamic processes observed throughout development at the molecular and cellular levels, this technique can provide information on brain morphology, structural connectivity, microstructural properties of gray and white matter, and on the functional architecture. Finally, MRI measures related to clinical, behavioral, and electrophysiological markers have a key role to play from a diagnostic and prognostic perspective in the implementation of early interventions to avoid long-term disabilities in children.
