Abstract

La deficiencia de ácido fólico es la causa más frecuente de anemia en nuestro medio, después del síndrome anémico de origen ferropénico. Los folatos son componentes esenciales de la dieta humana y animal. En los alimentos el ácido fólico se encuentran principalmente en forma de poliglutamatos, formas que luego son hidrolizadas en el intestino delgado a nivel de yeyuno proximal. Es importante definir con exactitud el defecto vitamínico causante de la anemia megaloblástica, puesto que, la administración de vitamina B12 a pacientes con deficiencia de folatos puede corregir parcialmente las alteraciones megaloblásticas, sin embargo, la administración de ácido fólico a pacientes con deficiencia de cobalamina induce mejoría hematológica, pero empeora el cuadro neurológico. Las principales causas de anemia por deficiencia de folatos son un aporte dietético insuficiente, un aumento de los requerimientos, defectos de su absorción o interacción con fármacos. Los folatos, pueden verse perjudicados por la sensibilidad a la luz y a las altas temperaturas así como por su alta afinidad por el agua, lo que facilita su eliminación por lavado o cocción. Folic acid deficiency is the second most common cause of anemia in our environment, after anemia secondary to iron deficiency. Folates are essential components of human and animal diet. Folic acid is mainly in poliglutamate form, and it is hydrolyzed in the proximal jejunum. It is important to identify adequately the exact vitamin deficiency that causes megaloblastic anemia, because vitamin B12 administration in folate deficiency may correct partially megaloblasticalterations, but administration of folic acid in cobalamin deficient patients improves haematological parameters but deteriorates the neurological syndrome. Main causes of anemia secondary to folate deficiency are inadequate dietetic administration, increased requirements, impaired absorption and pharmacologic interactions. Folates are altered by light, high temperature and by water affinity, which facilitates its elimination by washing or cooking.

Abstract

The polyethylene glycol antiglobulin test has been found to enhance the reactivity of most alloantibodies.To investigate the utility of polyethylene glycol antiglobulin test for detection and identification of red blood cell antibodies, a comparison study of polyethylene glycol (PEG), a low-ionic-strength additive solution (LISS) and bovine serum albumin (BSA) was conducted. The sera of 47 different patients with positive antibody screening test by the LISS method, were tested in parallel with reagent antibody-detection cells using PEG, LISS and BSA.In the sera of 47 patients, 57 antibodies were detected. We identified 39 antibodies by the three methods. Twelve antibodies reacted by the BSA method and the LISS method but did not react with the PEG method (8 anti-I, 1 anti-P1, 1 anti-Lea(a), and two antibodies missed by the PEG method because they did not react with anti-IgG: 1 anti-M and 1 anti-K). Three antibodies reacted only with the LISS method (3 anti-I). Four clinically significant antibodies were detected only by the PEG method (2 anti-Jka, 1 anti-Jkb, 1 anti-c). The serum from a patient with delayed hemolytic transfusion reaction and no antibody detectable by the LISS and the BSA methods was tested by the PEG method. We were able to detect an anti-Jka by PEG in the pretransfusion sample. In 24 (60%) of 40 samples with clinically significant antibodies, PEG antiglobulin reactions were stronger (total score 221) than LISS antiglobulin reactions (total score 170) and BSA antiglobulin reaction (total score 184); in 14 (35%) of 40 samples, they were identical, and in 2 (5%) agglutination in the PEG method was weaker.In our experience, the polyethylene glycol antiglobulin test is more sensitive than LISS and BSA in detecting clinically significant antibodies and is an acceptable technique for routine compatibility test.

Abstract

Sweet's syndrome is a neutrophilic dermatosis characterized clinically by raised, erythematous, tender lesions on the face, neck, upper thorax and extremities. Several diseases have been associated with this entity. We report a case of Sweet's syndrome associated with chronic myelogenous leukemia: a 48-year-old woman who developed recurrent skin lesions 3 years after the diagnosis of chronic myelogenous leukemia. Progression to an accelerated phase of the disease was detected 3 months after the beginning of the skin lesions. This case shows the convenience of evaluation and closer follow-up of patients with chronic myelogenous leukemia who develop skin lesions, especially if these lesions are recurrent.
