Introduction: Functional endonasal endoscopic surgery is a frequent surgical procedure among otorhinolaryngologists. In 2014, the European Society of Rhinology published the “European Position Paper on the Anatomical Terminology of the Internal Nose and Paranasal Sinuses”, aiming to unify the terms in the English language. We do not yet have a unified terminology in the Portuguese language. Objective: Transcultural adaptation of the anatomical terms of the nose and paranasal cavities of the “European Anatomical Terminology of the Internal Nose and Paranasal Sinuses” to Portuguese. Methods: A group of rhinologists from diverse parts of Brazil, all experienced in endoscopic endonasal surgery, was invited to participate in the creation of this position paper on the anatomical terms of the nose and paranasal sinuses in the Portuguese language according to the methodology adapted from that previously described by Rudmik and Smith. Results: The results of this document were generated based on the agreement of the majority of the participants according to the most popular suggestions among the rhinologists. A cross-cultural adaptation of the sinonasal anatomical terminology was consolidated. We suggest the terms “inferior turbinate”, “nasal septum”, “(bone/cartilaginous) part of the nasal septum”, “(middle/inferior) nasal meatus”, “frontal sinus drainage pathway”, “frontal recess” and “uncinate process” be standardized. Conclusion: We have consolidated a Portuguese version of the European Anatomical Terminology of the Internal Nose and Paranasal Sinuses, which will help in the publication of technical announcements, scientific publications and the teaching of the internal anatomical terms of the nose and paranasal sinuses in Brazil. Em 2014, a Sociedade Europeia de Rinologia publicou o “Documento Europeu para Posicionamento sobre a Terminologia Anatômica Interna do Nariz e das Cavidades Paranasais” com o objetivo de unificar os termos na língua inglesa. Sugerimos que se busque uniformizar termos como “concha inferior”, “septo nasal”, “porção (óssea/cartilagionasa) do septo nasal”, “meato (médio/ inferior) nasal”, “via da drenagem do seio frontal”,“recesso frontal” e “processo uncinado”. Conclusão: Consolidamos uma versão adaptada em português da “European Anatomical Terminology of the Internal Nose and Paranasal Sinuses” que auxiliará a publicação de comunicados técnicos, publicações científicas e o ensino dos termos anatômicos internos do nariz e das cavidades paranasais no Brasil. Architected cellular structures based on triply periodic minimal surfaces (TPMS) have attracted significant attention due to their lightweight, superior, and controllable mechanical properties. Such lattice structures can be potential candidates for high specific energy absorption (SEA) applications. In this study, five TPMS sheet-based structures (Gyroid, Primitive, IWP, Diamond and Fisher-Koch) were designed, fabricated, and tested under quasi­-static and dynamic loading conditions. Laser powder bed fusion (L-PBF) is employed to facilitate the fabrication of these complex structures using stainless steel (SS316L) at three different relative densities. Scanning electron microscopy (SEM) and micro­ Computed Tomography (micro­CT) were utilized to assess the quality of the printed structures. The dynamic compressive behavior is investigated by conducting direct impact compression tests utilizing a Direct Impact Hopkinson Bar (DIHB) at a strain-rate of 2057 s−1. Quasi-­static tests are also performed at a strain-rate of 0.005 s−1. The quasi-static and dynamic mechanical responses are then compared to explore the changes in plateau stress and specific energy absorption values of the five TPMS lattices in these two loading regimes. Furthermore, the effects of changing both architecture and relative density on the properties of lattice structures are investigated. The results show that all five topologies exhibit an enhanced mechanical performance under dynamic loading. In fact, Diamond structure demonstrates the highest SEA value of 35.57 J/g under the high strain-rate loading condition, in comparison to 30.85 J/g in the quasi-static loading. This study suggests that TPMS structures could be potential candidates not only for quasi­-static, but also for dynamic applications related to a high strain-rate loading.

The advent of Powder Bed Fusion (PBF) techniques allows the additive manufacturing of complex structures, as Triply Periodic Minimal Surfaces (TPMS) lattices, which exhibit promising characteristics for impact applications, such as lightweight and high-energy absorption. Thus, this work aims to develop a numerical model of TPMS structures to investigate the mechanical response of such structures when subjected to impact loadings. To fulfill this task, stainless steel samples made by PBF technique were mechanically characterized at different strain rates using a universal testing machine and Split Hopkinson Pressure Bar. The testing campaign also explored the compressive and tensile material response, with the strain field being monitored by Digital Image Correlation technique. It was noted that the material exhibits a similar elasto-plastic response on both tension and compression and an evident strain rate hardening when the material is loaded from static (0.001 s-1) to dynamic strain rates (4000 s-1). Constitutive parameters were then obtained and implemented in an explicit finite element model developed through Abaqus CAE. Samples of TMPS lattices were manufactured and tested at different loading velocities, which showed that the FE model developed can be used to predict the impact response of TMPS lattices.

Selective laser melting (SLM) is employed to facilitate the fabrication of such complex structures out of stainless steel (SS316L). Scanning electron microscopy (SEM) is utilized to assess the quality of the printed structures. The dynamic compressive behaviour is investigated through performing a direct impact compression test via a Direct Impact Hopkinson Bar (DIHB) at a strain rate of 2000 s-1. Quasi-static tests are also performed at a strain rate of 0.005 s-1. The specific energy absorption (SEA) is compared under both loading conditions to investigate the performance of such structures under dynamic loading. Results show that both structures exhibit higher SEA values under high deformation rates. In fact, Neovius structures outperform FRD structures in terms of specific energy absorption as it exhibits a SEA value of 22.11 J/g and 24.8 J/g SEA in quasi-static and dynamic conditions, respectively.

Topical dexamethasone associated with hyaluronic acid in cochlear implant surgery demonstrated a statistically significant difference in the preservation of low-frequency thresholds when compared with topical dexamethasone alone and a control group. Topical dexamethasone alone was not superior in hearing preservation when compared to the control group.To compare the effects of topical dexamethasone alone and associated with hyaluronic acid intraoperatively in hearing preservation in cochlear implantation.Eighteen severely to profoundly hearing-impaired adult patients with measurable hearing were divided into three groups preoperatively: cochlear implantation as a control group (group 1), cochlear implantation using topical dexamethasone intraoperatively (group 2), and cochlear implantation using topical dexamethasone associated with hyaluronic acid intraoperatively (group 3). Preimplant and postimplant low-frequency pure-tone averages (PTAs) were calculated from unaided audiograms at 125, 250, and 500 Hz.The mean changes in the low-frequency PTA comparing postoperative against preoperative thresholds were 28.03 ± 6.77 dB in group 1, 30 ± 14.53 dB in group 2, and 7.23 ± 6.12 dB in group 3. There was statistical difference when comparing group 3 with groups 1 and 2 using one-way ANOVA (p = 0.002) followed by Scheffé post hoc test.

Objective. To evaluate Auditory Steady-State Responses (ASSR) at high intensities in pediatric cochlear implant candidates and to compare the results to behavioral tests responses.Methods. This prospective study evaluated 42 children with suspected severe-to-profound hearing loss, aged from 3 to 72 months. All had absent ABR and OAE responses. ASSR were evoked using binaural single frequency stimuli at 110 dB HL with a 10 dB down-seeking procedure. ASSR and behavioral test results were compared.Results. Forty-two subjects completed both ASSR and behavioral evaluation. Eleven children (26.2%) had bilateral responses. Four (9.5%) showed unilateral responses in at least two frequencies, all confirmed by behavioral results. Overall 61 ASSR responses were obtained, most (37.7%) in 500 Hz. Mean thresholds were between 101.3 and 104.2 dB HL. Among 27 subjects with absent ASSR, fifteen had no behavioral responses. Seven subjects showed behavioral responses with absent ASSR responses. No spurious ASSR responses were observed at 100 or 110 dB HL.Conclusion. ASSR is a valuable tool to detect residual hearing. No false-positive ASSR results were observed among 42 children, but in seven cases with absent ASSR, the test underestimated residual hearing as compared to the behavioral responses.

Introducao: As respostas auditivas de estado estavel permitem avaliar de forma objetiva limiares auditivos frequencia-especifica. A audiometria tonal e o exame padrao-ouro; no entanto, nem sempre pode ser conclusiva, principalmente em criancas e adultos nao colaborativos. Objetivo: Comparar os limiares auditivos da RAEE aos da audiometria tonal em individuos com audicao normal. Materiais e metodos: Foram incluidos neste estudo prospectivo de corte transversal 26 adultos (52 orelhas), de ambos os generos, com audiometria normal e sem queixas otologicas. Os pacientes foram submetidos a anamnese, otomicroscopia, audiometria e imitanciometria. A seguir, realizou-se avaliacao de respostas auditivas de estado estavel. Os resultados obtidos foram analisados estatisticamente e comparados entre si. Resultados: A diferenca entre os limiares (em dB NA) obtidos em ambos os exames para cada frequencia testada foi de 7,12 dB para 500 Hz; 7,6 dB para 1000 Hz; 8,27 dB para 2000 Hz e 9,71 dB para 4000 Hz, com limiares mais elevados na RAEE, em todas as frequencias. Nao houve diferenca estatistica entre as medias para cada frequencia testada. Conclusao: Os limiares obtidos na RAEE foram comparaveis aos da audiometria tonal em individuos normouvintes; entretanto, nao deve ser usado como unico metodo de avaliacao auditiva.

Objectives/Hypothesis: Measure the dimensions of the nasoseptal (NS) flap and the anterior skull base (ASB) defect. Verify whether the flap is sufficient to cover the defect. Study the anatomy of the septal artery (SA). Study Design: Anatomical and radiological study. Methods: After endoscopic craniofacial resection, sufficiency of the flap to cover the ASB defect was assessed. The SA was dissected. The number of branches in the pedicle and the distance between the artery and the sphenoid ostium were noted. Radiologic study analyzing CT scans of 30 patients for comparison among measurements of the NS flap and the ASB defect was performed. Results: In all cases the flap was sufficient to cover the ASB. Two branches of the SA were found in the pedicle in 71.4%. The distance between the SA and the sphenoid ostium was 9.3 mm. The reconstruction area of the flap (17.12 cm2) was larger than the defect area (8.64 cm2) (P < .001). The difference between the superior length of the flap and the anterior-posterior distance of the defect was ≤5 mm in 26.7%. Comparison between the anterior flap width and the anterior defect width revealed that in 33% the difference was ≤5 mm. Conclusions: The dimensions of NS flap are sufficient to cover completely the ASB defect. The anterior edge of the defect presents increased risk for failure in coverage. Additional width adding the nasal floor mucosa to the flap is important to decrease the risk of gap in the anterior orbit-orbit defect. The productive work between otolaryngologists and neurosurgeons has resulted in the emergence of endoscopic endonasal skull base surgery. The goal of the present study is to describe the endoscopic anatomy of the endonasal approach to the sellar region and planum sphenoidale, highlighting the key points of the surgical approach and the neurovascular landmarks. METHOD: Descriptive study of the endoscopic endonasal dissection of 9 fresh cadavers with exposure of the anatomic structures. RESULTS: The endoscopic endonasal ethmoidectomy and sphenoidotomy allows an expanded access to the sellar area and planum sphenoidale. The surface anatomy of the sphenoid sinus is easily identifiable and provides safe landmarks, guiding the intracranial dissection. CONCLUSION: The endoscopic endonasal approach to the skull base by the ENT and neurosurgeon is feasible, but it requires adequate anatomical knowledge and endoscopic skills for its realization, which can be obtained by practicing in cadavers. O trabalho cooperativo entre otorrinolaringologistas e neurocirurgiões resultou no aprimoramento das técnicas cirúrgicas e no surgimento da cirurgia endoscópica endonasal da base do crânio. O estudo tem como objetivo descrever a anatomia endoscópica do acesso endonasal da região selar e plano esfenoidal, destacando os pontos fundamentais do acesso cirúrgico e referências neuro-vasculares. MÉTODO: Estudo descritivo da dissecção endoscópica endonasal de 9 cadáveres frescos com exposição das estruturas anatômicas. RESULTADOS: A etmoidectomia e esfenoidotomia endonasal endoscópica permite um acesso estendido à região selar e plano esfenoidal. A anatomia de superfície do seio esfenoidal é facilmente identificada e fornece pontos de referência seguros, guiando a dissecção intracraniana. CONCLUSÃO: O acesso endoscópico endonasal para a base do crânio pelo otorrinolaringologista e neurocirurgião é perfeitamente viável, mas para isso o conhecimento anatômico da região e de seus pontos de referência é essencial, o que pode ser obtido pelo treinamento em cadáveres.

We describe a practical technique of superior turbinectomy followed by posterior ethmoidectomy as a less invasive procedure for two-surgeon technique on endoscopic endonasal transsphenoidal surgery. After identification of the superior turbinate and the sphenoid ostium, the inferior third portion of the superior turbinate was coagulated and resected. This partial superior turbinectomy procedure exposed the posterior ethmoidal sinus. Resection of the bony walls between the sphenoid and posterior ethmoid sinuses provided more lateral and superior exposure of the sphenoid sinus. This technique was performed in 56 patients with midline skull base lesions, including 49 pituitary adenomas and 7 other lesions. Meticulous manipulation of instruments was performed in all cases without surgical complications such as permanent hyposmia/anosmia or nasal bleeding. Our findings suggested that the partial superior turbinectomy followed by retrograde posterior ethmoidectomy is a simple and safe technique providing a sufficient surgical corridor for two-surgeon technique to approaching midline skull base regions, mainly involving pituitary adenomas.

The correlations between behavioral and auditory steady-state response (ASSR) thresholds were significant at 500, 1000, 2000, and 4000 Hz. ASSR presented high sensitivity and specificity in the detection of residual hearing in cochlear implant candidates when compared with warble-tone audiometry.To assess residual hearing in cochlear implant candidates by comparing the electrophysiological thresholds obtained in dichotic single-frequency ASSR with behavioral thresholds at 500, 1000, 2000, and 4000 Hz.This was a comparative study between ASSR and warble-tone audiometry thresholds in 40 cochlear implant candidates (80 ears) before cochlear implantation with bilateral severe-to-profound sensorineural hearing loss.Thresholds were obtained in 62.5% of all frequencies evaluated in warble-tone audiometry and in 63.1% in the ASSR. ASSR sensitivity was 96% and specificity was 91.6%. Mean differences between behavioral and ASSR thresholds did not reach significance at any frequencies. Strong correlations between behavioral and ASSR thresholds were observed in 500, 1000, and 2000 Hz and moderate in 4000 Hz, with correlation coefficients varying from 0.65 to 0.81. On 90% of occasions, ASSR thresholds were acquired within 10 dB of behavioral thresholds.

Auditory steady-state responses (ASSR) are an important tool to detect objectively frequency-specific hearing thresholds. Pure-tone audiometry is the gold-standard for hearing evaluation, although sometimes it may be inconclusive, especially in children and uncooperative adults.Compare pure tone thresholds (PT) with ASSR thresholds in normal hearing subjects.In this prospective cross-sectional study we included 26 adults (n = 52 ears) of both genders, without any hearing complaints or otologic diseases and normal puretone thresholds. All subjects had clinical history, otomicroscopy, audiometry and immitance measurements. This evaluation was followed by the ASSR test. The mean pure-tone and ASSR thresholds for each frequency were calculated.The mean difference between PTand ASSR thresholdswas 7,12 for 500 Hz, 7,6 for 1000 Hz, 8,27 for 2000 Hz and 9,71 dB for 4000 Hz. There were no difference between PT and ASSR means at either frequency.ASSR thresholds were comparable to pure-tone thresholds in normal hearing adults. Nevertheless it should not be used as the only method of hearing evaluation.

Ligadura da artéria etmoidal anterior (AEA) pode ser necessária em casos de epistaxe grave refratária ao tratamento tradicional. O uso da ligadura endoscópica endonasal da AEA ainda é bastante limitado. Existem poucos estudos na literatura sobre a técnica de abordagem endoscópica desta artéria. OBJETIVOS: Demonstrar a aplicabilidade técnica da ligadura periorbitária da AEA por via endoscópica transetmoidal. MATERIAL E MÉTODOS: Estudo prospectivo. 50 fossas nasais de cadáveres foram dissecadas. Após a realização de uma etmoidectomia anterior e remoção parcial da lâmina papirácea, a periórbita foi cuidadosamente dissecada até a identificação da AEA. Após sua identificação, a artéria foi exposta e ligada dentro da órbita. RESULTADOS: Todas as dificuldades inerentes ao procedimento, as complicações associadas, a curva de aprendizado e variações anatômicas foram coletados. CONCLUSÕES: A abordagem endoscópica da AEA na órbita de cadáveres mostrou-se factível. A identificação da artéria é fácil e a técnica evita incisões externas. Este acesso parece ser uma excelente alternativa para a abordagem da AEA. Estudos clínicos futuros são necessários para comprovar os benefícios desta técnica. AIM: To demonstrate the feasibility of periorbital AEA ligation in a transethmoidal endoscopic approach. METHODS: A prospective study where 50 nasal cavities were dissected. After anterior ethmoidectomy and partial removal of lamina papyracea, the periorbital area was carefully dissected along a subperiosteal plane to identify the AEA. The vessel was exposed within the orbit and dissected. RESULTS: Data on technical difficulties, complications, the learning curve and anatomical variations were gathered. CONCLUSION: An endonasal endoscopic approach to the AEA within the orbit was shown to be feasible. Identifying the artery is not difficult, and this technique avoids external incisions. This approach appears to be an excellent alternative for approaching the AEA. Further clinical studies are needed to demonstarte the benefits of this technique.

Wound closure after posterior cranial fossa surgery through the clivus remains a challenge. A multilayer closure was suggested by some authors1,2. On the other hand, a multilayer reconstruction may not be achieved in patients with clival defects; in this region, gravity may displace the inlay grafts downwards. Introdução: A paralisia facial periférica caracteriza-se pela interrupção, definitiva ou temporária, do funcionamento da musculatura facial. Decorre de lesão ou mau funcionamento das fibras do nervo facial. É uma enfermidade que causa enorme impacto estético e funcional. O topodiagnóstico e o tratamento da paralisia facial periférica pode requer abordagem médica e fonoaudiológica; ser exclusivamente medicamentoso ou associado à terapia de reabilitação, ou ainda, medicamentoso e cirúrgico, seguido da reabilitação da mímica facial. A via FCM tem sido usada para a descompressão do NF quando a audição precisa ser preservada. Esse acesso pode ser realizado de forma isolada, ou combinado à via transmastóidea. Objetivo: Descrever uma técnica inovadora para a descompressão do nervo facial via fossa craniana média que permite a exposição direta dos segmentos labiríntico e timpânico do nervo facial, com a preservação da função da orelha interna. Métodos: Vinte cabeças extraídas de cadáveres adultos de ambos os gêneros, sem sinais de malformação, traumatismo, doença ou manipulação cirúrgica prévia foram usados neste estudo. Os pontos de referência utilizados foram a artéria meníngea média, o nevo petroso superficial maior, a eminência arqueada, o seio petroso superior, e o plano meatal seguido no ápice petroso a partir da sua porção mais anterior e medial. Foi feita a dissecação do plano meatal, com visualização do meato acústico interno, seguido no ápice petroso a partir da sua porção mais anterior e medial até a região do gânglio geniculado. Foi aberto o tégmen timpânico e identificada a porção timpânica do nervo facial. A dissecação seguiu no sentido retrógrado da porção timpânica do nervo facial em direção ao gânglio geniculado, até a sua porção labiríntica. A aracnoide do meato acústico interno era aberta, e depois de identificado o nervo facial, a bainha deste nervo era aberta em extensão exposta. Resultados: As distâncias médias, entre o canal semicircular lateral e a porção média do cabo do martelo eram similares em ambos os lados (4,0mm±0,5mm). As distâncias médias, da porção timpânica do nervo facial até metade do cabo do martelo foram determinadas e resultaram em 2,0mm ± 0,44mm, e em 2,2mm ± 0,48mm para os lados direito e esquerdo, respectivamente. O segmento timpânico do nervo facial apresentou, em média, comprimento total de 11mm ± 0,67mm para o lado direito, e 11,5mm±0,60mm para o esquerdo. Os comprimentos longitudinais da janela óssea confeccionada no tégmen timpânico foram, em média, 16,8mm±1,67mm para o lado direito, e 16,8mm ± 1,20mm para o lado esquerdo. Já os comprimentos transversais da janela óssea confeccionada no tégmen timpânico foram, em média, 5,5mm ± 1,20mm e 5,0mm ± 1,75mm para os lados direito e esquerdo, respectivamente. O valor médio da área elíptica formada pelos comprimentos longitudinal e transversal da janela óssea confeccionada no tégmen timpânico foram 72,5mm2 ± 22,5mm2 à direita, e 65,9mm2 ± 30,3mm2 à esquerda. Conclusão: A técnica proposta pode ser utilizada para a cirurgia de descompressão do nervo facial via fossa craniana média, pois permite acesso aos segmentos timpânico, labiríntico e metal desse nervo, sem impor risco à audição. Pela possibilidade de acessar a porção timpânica do nervo, sem a necessidade da abertura da mastoide, o procedimento permite que se reduza o tempo cirúrgico e os riscos aos pacientes Background: Peripheral facial palsy is characterized by the permanent or temporary interruption of the functioning of the facial muscles. The middle cranial fossa (MCF) approach has been used for the decompression of the facial nerve (FN) when hearing needs to be preserved. In this work, we describe an innovative technique for the decompression of the FN through the MCF approach that allows the direct exposure of the labyrinthine and entire tympanic segment of the FN, with the preservation of inner ear function. Methods: Twenty cadavers heads were used in this study. The reference landmarks used were the middle meningeal artery, the greater superficial petrosal nerve, the arcuate eminence, the inferior petrosal sinus and the meatal plane following the petrous apex from its most anterior and medial portion. Results: The tympanic segment of the FN presented, on average, a total length of 11 ± 0.67mm to the right, and 11.5 ± 0.60mm to the left. The longitudinal lengths of bone window in the tegmen tympani were 16.8±1.67mm to the right, and 16.8 ± 1.20mm to the left. The cross-sectional lengths of the bone window in the tegmen tympani were 5.5 ± 1.20mm and 5.0±1.75mm to the right and left sides, respectively. The average value of elliptical area formed by the longitudinal and transversal lengths of the bone window made in the tegmen tympani were 72.5 ± 22.5mm2 to the right, and 65.9 ± 30.3mm2 to the left. Conclusion: The proposed technique can be used for the surgical decompression of the tympanic, labyrinthine and meatal segments of the FN through the MCF, without imposing a risk to hearing, in addition to reducing the surgical time and the risk to patients

INTRODUÇÃO: As Respostas Auditivas de Estado Estável permitem avaliação frequência específica em intensidades de até 120 dB NA e a detecção de audição residual em pacientes com perda auditiva severo-profunda. O objetivo deste estudo é comparar os limiares à RAEE e os resultados da avaliação comportamental em crianças com suspeita de surdez severo-profunda. MÉTODO: Estudo transversal para comparar respostas à RAEE e por audiometria com reforço visual (VRA) em 63 crianças candidatas ao implante coclear (126 orelhas) com idade entre 6 e 72 meses. Foram incluídas crianças com otomicroscopia normal, ausência de respostas ao PEATE clique a 90 dB NA e às emissões otoacústicas. Foram excluídas crianças com malformações de orelha interna, doenças do espectro da neuropatia auditiva, ou que não completaram a avaliação comportamental ou não atingiram ruído eletroencefalográfico < 30 nV durante a RAEE. Foram utilizados estímulos com tons contínuos sinusoidais (100% AM e 20% FM) nas frequências de 500, 1000, 2000 e 4000 Hz em intensidade máxima de 110 dB NA. Os limiares à VRA foram obtidos por tom warble nas frequências de 500, 1000, 2000 e 4000 Hz em cada orelha através de fones de inserção (ER-5A) ou tipo casco (TDH-39). A intensidade máxima de estimulação foi de 120 dB NA em cada frequência. RESULTADOS: Limiares comportamentais foram obtidos em 36,7% (185/504) de todas as frequências em todas as crianças, 9% em intensidade maior que 110 dB NA. Entre as 504 medidas da RAEE em 63 indivíduos, 53 limiares foram obtidos (10,5%). Ao todo, 89,5% das frequências testadas não apresentaram nenhuma resposta em 110 dB NA. A distribuição dos limiares à RAEE foi semelhante à da avaliação comportamental. A maioria das respostas foram em 500 Hz, diminuindo nas frequências agudas. A diferença média entre os limiares à VRA e à RAEE variou entre 0,09 e 8,94 dB. Foram realizadas 27 comparações entre RAEE e VRA: 12 em 500 Hz, 9 em 1000 Hz, 5 em 2000 Hz e 1 em 4000 Hz. Respostas ausentes foram observadas em ambos os testes em 38,1% em 0,5 KHz, 52,45% em 1 KHz, 74,6% em 2 KHz e 81,0% em 4 KHZ. A especificidade foi > 90% em 1, 2 e 4 KHz. Nas orelhas sem resposta comportamental em 120 dB NA, todos os limiares à RAEE estavam na faixa de perda profunda, 90% deles > 110 dB NA. CONCLUSÃO: A ausência de respostas nas altas intensidades na RAEE foi o principal achado (especificidade > 90%) o que prediz limiares comportamentais na faixa de surdez profunda Introduction and Objective: ASSR allows frequency-specific evaluation in intensities up to 120 dB HL and detection of residual hearing in patients with severe-toprofound hearing loss. The aim of this study was to compare ASSR thresholds and behavioral test results in children with suspected severe-to-profound hearing loss. Methods: A cross sectional study was carried out to compare ASSR and Visual Reinforcement Audiometry (VRA) responses in 63 pediatric cochlear implant candidates (126 ears) aged between 6 to 72 months. We included children with normal otomicroscopy findings, absent responses to click-ABR at 90 dB HL and otoaccoustic emissions. We excluded children with inner ear malformations, auditory neuropathy spectrum disorder or who did not complete VRA or achieve EEG noise < 30 nV during the ASSR test. Air-conduction ASSR stimuli were continuous sinusoidal tones (100% AM and 20% FM) presented at 0.5, 1, 2 and 4 kHz starting at the maximum presentation level of 110 dB HL. VRA thresholds were acquired with warble tones presented at 0.5, 1, 2 and 4 KHz in each ear through ER-tone 5A or TDH-39 phones. Maximum presentation level was 120 dB HL for each frequency. Results: Behavioral thresholds were obtained in 36.7% (185/504) of all frequencies in all subjects, 9% were in intensities > 110 dB HL. Among 504 ASSR measurements from 63 subjects, 53 thresholds were obtained (10.5%). Overall 89.5% of the tested frequencies did not show any response at 110 dB HL. The distribution of ASSR responses was similar to the behavioral test results. Most responses were at 500 Hz, decreasing among the higher frequencies. Mean differences between behavioral and ASSR thresholds varied from 0.09 to 8.94 dB. Overall, 27 comparisons of behavioral and ASSR thresholds were obtained: 12 at 0.5 KHz, 9 at 1 KHz, 5 at 2 KHz and 1 at 4 KHz. Absent responses were observed in both tests in 38.1% at 0.5 KHz, 52.4% at 1 KHz, 74.6% at 2 KHz and 81.0% at 4 KHz. The specificity was > 90% at 1, 2 and 4 KHz. In ears with no behavioral response at 120 dB HL all ASSR thresholds were in the profound hearing loss range, 90% of them were equal or > than 110 dB HL. Conclusion: Among 63 pediatric CI candidates, absent responses to high-intensity ASSR was the major finding (specificity > 90%) predicting behavioral thresholds in the profound hearing loss range. These findings can be helpful to confirm the decision for cochlear implantation
