Infection rounds in Intensive Care Units (ICU) can impact antimicrobial stewardship (AMS). The aim of this survey was to assess the availability of microbiology, infection, AMS services, and antimicrobial prescribing practices in the UK ICUs. An online questionnaire was sent to clinical leads for ICUs in each region listed in the Critical Care Network for the UK. Out of 217 ICUs, 87 deduplicated responses from England and Wales were analyzed. Three-quarters of those who responded had a dedicated microbiologist, and 50% had a dedicated infection control prevention nurse. Infection rounds varied in their frequency, with 10% providing phone advice only. Antibiotic guidance was available in 99% of the units; only 8% of those were ICU-specific. There were variations in the availability of biomarkers & the duration of antibiotics prescribed for pneumonia (community, hospital, or ventilator), urinary, intra-abdominal, and line infections/sepsis. Antibiotic consumption data were not routinely discussed in a multi-disciplinary meeting. The electronic prescription was available in ~60% and local antibiotic surveillance data in only 47% of ICUs. The survey highlights variations in practice and AMS services and may offer the opportunity to further collaborations and share learnings to support the safe use of antimicrobials in the ICU.

Summary: We identify amino acid variants within dominant SARS-CoV-2 T cell epitopes by interrogating global sequence data. Several variants within nucleocapsid and ORF3a epitopes have arisen independently in multiple lineages and result in loss of recognition by epitope-specific T cells assessed by IFN-γ and cytotoxic killing assays. Complete loss of T cell responsiveness was seen due to Q213K in the A∗01:01-restricted CD8+ ORF3a epitope FTSDYYQLY207-215; due to P13L, P13S, and P13T in the B∗27:05-restricted CD8+ nucleocapsid epitope QRNAPRITF9-17; and due to T362I and P365S in the A∗03:01/A∗11:01-restricted CD8+ nucleocapsid epitope KTFPPTEPK361-369. CD8+ T cell lines unable to recognize variant epitopes have diverse T cell receptor repertoires. These data demonstrate the potential for T cell evasion and highlight the need for ongoing surveillance for variants capable of escaping T cell as well as humoral immunity.

Background The first epidemic wave of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) in Scotland resulted in high case numbers and mortality in care homes. In Lothian, over one-third of care homes reported an outbreak, while there was limited testing of hospital patients discharged to care homes. Aim To investigate patients discharged from hospitals as a source of SARS-CoV-2 introduction into care homes during the first epidemic wave. Methods A clinical review was performed for all patients discharges from hospitals to care homes from 1st March 2020 to 31st May 2020. Episodes were ruled out based on coronavirus disease 2019 (COVID-19) test history, clinical assessment at discharge, whole-genome sequencing (WGS) data and an infectious period of 14 days. Clinical samples were processed for WGS, and consensus genomes generated were used for analysis using Cluster Investigation and Virus Epidemiological Tool software. Patient timelines were obtained using electronic hospital records. Findings In total, 787 patients discharged from hospitals to care homes were identified. Of these, 776 (99%) were ruled out for subsequent introduction of SARS-CoV-2 into care homes. However, for 10 episodes, the results were inconclusive as there was low genomic diversity in consensus genomes or no sequencing data were available. Only one discharge episode had a genomic, time and location link to positive cases during hospital admission, leading to 10 positive cases in their care home. Conclusion The majority of patients discharged from hospitals were ruled out for introduction of SARS-CoV-2 into care homes, highlighting the importance of screening all new admissions when faced with a novel emerging virus and no available vaccine. The COVID-19 pandemic has had a significant impact on surgical specialties. COVID-19 carries a significant risk to the surgical patient and the healthcare workers looking after them, with an increased incidence of pulmonary complications and mortality in patients who test positive perioperatively. Appropriate infection prevention and control measures are critical to ensure appropriate care is given and to reduce the risk of onward transmission. This article will discuss the measures that have been instigated and contributed to infection control in surgery, such as testing, patient isolation, personal protective equipment and ventilation. The COVID-19 pandemic has led to healthcare workers across many specialities working together to provide essential clinical care. This collaborative approach is critical to maintain excellent infection prevention and control practices required during this pandemic, which protect patients and preserve surgical services.

Background and objective: multi-drug resistance is an increasing problem and is associated with increased morbidity and mortality. High levels of antimicrobial consumption and broad-spectrum antimicrobial use are associated with antimicrobial resistance (AMR). Studies have shown substantial antimicrobial use across intensive care units (ICUs) worldwide. International guidelines advise antibiotic de-escalation (ADE) as part of an antimicrobial stewardship programme (ASP). ADE is a strategy to decrease the spectrum of the empirical antimicrobial therapy, with the aim to reduce the ecological impact on the patient’s microbiome and reduce the emergence of AMR. Our aim was to provide an insight into the latest developments on ADE in the intensive care setting.Methods: PubMed was searched using the terms ‘antibiotic de-escalation’ and ‘antimicrobial stewardship’ up to and including November 2021.Key content and findings: evidence to date is limited, ADE appears to be a safe intervention. The evidence is inconclusive regarding resistance development. Concerns regarding increased duration and superinfections with an ADE approach are unproven with studies finding mixed results. ADE should not be used as a sole quality indicator as this could encourage empiric broad-spectrum antimicrobial overuse.Conclusions: ADE appears safe. Evidence is inconclusive regarding resistance development but data to date is limited. ADE should be used alongside other antimicrobial stewardship (AMS) measures. These include appropriate empirical therapy, guided by local guidelines considering local epidemiology and host factors, optimal dosing, regular review of antimicrobial therapy with clinical progress and microbiology results, with de-escalation as soon as feasible and early cessation where appropriate, and infection control measures.

The novel SARS-CoV-2 virus responsible for COVID-19 has resulted in a worldwide pandemic. To curtail the ongoing pandemic, emphasis has been placed on enhanced testing. Consequently, there has been an exponential rise in the number of diagnostic platforms that are available. This review gives an overview of the diagnostic methods for COVID-19, highlighting key considerations needed when interpreting the test results. The aims of SARS-CoV-2 testing are to (1) diagnose acute infection in symptomatic patients, (2) screen for and diagnose asymptomatic infection and (3) identify past infection. The detection assays used include nucleic acid amplification tests (NAATs), antigen-based tests and antibody (serological) tests. NAATs are aimed at identifying the presence of SARS-CoV-2 ribonucleic acid (RNA) in a sample. Reverse transcription polymerase chain reaction (RT-PCR) is the gold standard NAAT used for the diagnosis of COVID-19. In RT-PCR, repeated, automated cycles of heating and cooling (thermocycling) are used to amplify specific targets, that is, segments of the virus’ genome. The targets for SARS-CoV-2 include open reading frame 1ab/RdRp, envelope, nucleocapsid (N) and spike (S) genes (figure 1). The amount of virus in a sample is then quantified in real time using fluorescently labelled probes. A test is positive if the fluorescence goes above a certain threshold level. The number of thermocycles required to get over this threshold is termed the cycle threshold (Ct) value. The lower the Ct value, the higher the quantity of viral genetic material in the sample; this is used as a proxy for viral load and infectivity. 11. A novel collaborative approach in improving management outcomes in patients with motor neurone disease (MND)

MND is a neurodegenerative disease leading to progressive limb, bulbar and respiratory muscle weakness and ultimately death. NICE guidance recommends a multi-disciplinary approach with regular respiratory function monitoring, consideration of non-invasive ventilation (NIV) and advance planning. The Medway Clinical MND team works with the Wisdom Hospice and cares for 25-30 patients at any one time. There is a joint palliative care and respiratory clinic to review patients9 NIV requirements. Case notes from 23 deceased patients were reviewed. MND presentation, lung function testing, NIV and advance care planning were audited. Cohort was 56% male, average age 66 years. 18 presented with limb symptoms, 3 speech issues and 2 with respiratory impairment. All patients had Sniff Nasal Inspiratory Pressure and oximetry recorded. 16 patients (70%) underwent lung function testing, 6 being commenced and continued on NIV. All patients were started at home with Specialist Nurse input. All patients commenced on NIV had end of life discussions. 65% patients died at home, 13% in hospital and 22% in the hospice. 87% died in their preferred place of death. This compares favourably to a UK-wide survey by O9Neill CL et al. J Neurol Neurosurg Psychiatry 2012;83:371-6 in which 73% NIV referrals were established on treatment and 75% had palliative care involvement (100% at Medway). A MDT approach involving respiratory medicine, palliative care and community physiotherapy ensures MND sufferers can discuss NIV and end of life care. NICE MND guidance is being followed at this centre with regular assessment for respiratory failure. End of life issues and possible withdrawal were appropriately discussed.
