4. Thermoregulatory influence on reliability of flow-mediated vasodilation (FMD) testing : FMD measurement at elevated skin temperature

Abstract

Die Endothelfunktion als wichtiger Teil der Gefäßgesundheit kann durch die Messung der flussvermittelten Dilatation (flow-mediated dilation (FMD)) getestet werden. Bei diesem Verfahren wird eine lokale Hypoxie erzeugt und der Prä- und Postdurchmesser der zugehörigen Leitungsarterie gemessen (Baselinedurchmesser versus Spitzendurchmesser). Wenngleich die Testung laufend durch Leitlinien verbessert wird, scheint ein Aspekt jedoch noch weitestgehend unberücksichtigt: Die vasomotorische Aktivität der stromabwärtsgelegenen Gefäße stellt einen Schlüsselfaktor dar. Allerdings wird vor allem die Hautdurchblutung und damit der Durchmesser der kutanen Gefäße von Umweltfaktoren wie der Umgebungstemperatur erheblich beeinflusst. In der vorliegenden Arbeit wird daher untersucht, ob eine moderat erhöhte Hauttemperatur die Ergebnisse einer FMD-Messung der Brachialarterie verändert (lokale Hypoxie im Unterarm). Zehn Freiwillige (40% weiblich, 26.1 ± 4 Jahre) nahmen an zwei reaktiven Hyperämie-FMD-Messungen teil. Der erste FMD-Test erfolgte ohne thermischen Einfluss (individuelle FMD), bei der zweiten Messung wurde die Haut des Unterarms auf 35°C erwärmt (Wärme-FMD). Weder die relativen noch die absoluten FMD-Ergebnisse zeigten signifikante Unterschiede (FMD%: Individuell, 6,86 ± 2,32 % gegenüber Wärme, 6,45 ± 2,77 %; Baselinedurchmesser: Individuell, 0,40 ± 0,05 cm gegenüber Wärme, 0,41 ± 0,06 cm; Spitzendurchmesser: Individuell, 0,43 ± 0,05 cm gegenüber Wärme, 0,44 ± 0,05 cm). Es kann somit geschlussfolgert werden, dass eine moderate Erhöhung der Hauttemperatur keinen signifikanten Einfluss auf die Ergebnisse eines reaktiven Hyperämie-FMD-Tests hat. Darüber hinaus deuten die vorliegenden Daten darauf hin, dass die Hautdurchblutung allgemein eine untergeordnete Rolle im FMD-Mechanismus einnimmt, was jedoch weiter untersucht werden sollte., The endothelial function, as one part of vascular health, is tested by the flow-mediated dilation (FMD) measurement. Here a local hypoxia is generated, and the diameter of the associated conduit artery is measured before and after (baseline diameter versus peak diameter). Efforts are made to constantly improve this testing. However, one part seems not yet to be investigated: The vasomotor activity of the downstream vascular bed is one key determinant for the test outcomes, but especially the skin perfusion and therefore its vessels diameter are influenced by environmental factors such as ambient temperature. The present work therefore examines whether a moderately elevated skin temperature alters the results of an FMD measurement of the brachial artery (BA) (local hypoxia in the forearm). Ten participants (40% female, 26.1 ± 4 years) took part in two reactive hyperaemia FMD measurements. The first FMD testing included no thermal influence (individual FMD), during the second one the skin of the forearm was heated to 35°C (heated FMD). Neither relative, nor absolute FMD results showed significant differences (FMD%: Individual, 6.86 ± 2.32 % versus Heated, 6.45 ± 2.77 %; Baseline diameter: Individual, 0.40 ± 0.05 cm versus Heated, 0.41 ± 0.06 cm; Peak diameter: Individual, 0.43 ± 0.05 cm versus Heated, 0.44 ± 0.05 cm). Therefore, it can be concluded that a moderate increase in skin temperature is not significantly influencing reactive hyperaemia FMD outcomes. Mittels Infrarotthermografie können Veränderungen der Oberflächentemperatur der Haut dargestellt werden. Kraftbeanspruchungen führen zur Wärmeproduktion in der belasteten Muskulatur. Diese erzeugte Wärme wird zur Hautoberfläche weitergeleitet und daraus resultierende Veränderungen der Hauttemperatur können durch den Einsatz der Infrarotthermografie visualisiert und quantifiziert werden. Die vorliegende kumulativ angefertigte Arbeit umfasst Beiträge, die wesentliche Einflussfaktoren auf belastungsinduzierte Hauttemperaturveränderungen nach Kraftbeanspruchungen aufzeigen. Dazu wurden in einer Studie der Einfluss des Belastungsfaktors Trainingsintensität und in einer weiteren Untersuchung wesentliche Faktoren der Körperkomposition überprüft. Eine dritte Untersuchung zeigt einen Ansatz für eine effiziente automatische Messmethode zur Bestimmung der Hauttemperaturen mittels Fusion einer Infrarotkamera mit einer Tiefenbildkamera. Die Arbeit leistet somit einen Beitrag für die Anwendung der Infrarotthermografie im leistungsphysiologischen Kontext.

Abstract

Highlights • Warming the feet improved vibration perception thresholds in healthy subjects. • The sensitivity improvement was more marked with active warming (walking) than with passive warming (heat). • Patients with postural disturbances could benefit from increasing skin temperature in their feet., Objective Skin temperatures are known to increase cutaneous sensitivity. However, it is unclear whether the amount of improved sensitivity differs depending on the protocol of heat application. Therefore, this study aimed to investigate the effects of active (treadmill walking) and passive (infrared radiator) warming of the foot sole on vibration perception thresholds. Methods Sixty healthy and injury-free subjects voluntarily participated in this study. Vibration perception thresholds (200 Hz) and plantar temperatures were measured at the hallux and 1st metatarsal head. In experiment 1, warming and mechanically stimulating the skin was achieved by walking on a treadmill for 30 min. In a follow-up study (experiment 2), external plantar heat was administered via an infrared radiator (30 min). Results In both experiments, increasing temperatures led to increased plantar sensitivity. However, the amount of improved sensitivity was greater in experiment 1, although plantar temperature increases were lower compared to experiment 2. Conclusions Warming in conjunction with mechanical stimulation seems to have a greater potential to enhance plantar sensitivity compared to external heat supply only. Significance The possible influence of mechanical stimulation and warming towards superior plantar afferent feedback highlights its importance regarding human posture and fall prevention.

Eine max. Hauttemperaturseitendifferenz \(>2^{o}C\) galt bisher als diagnostischer Beweis für ein komplexes regionales Schmerzsyndrom (CRPS), wurde jedoch durch einmalige Messung oder mit aufwändigen Verfahren erfasst. Mittels einer Langzeitmessung unter Alltagsbedingungen wurde die akrale Hauttemperatur bei 22 CRPS-Pat., bei 18 Pat. mit Extremitätenschmerzen anderer Genese und bei 23 gesunden Probanden untersucht. CRPS-Pat. unterschieden sich signifikant von den übrigen Pat. v.a. in Parametern, die die dynamische Veränderung der Hauttemperatur charakterisieren, dagegen z.B. nicht in der mittleren Temperaturseitendifferenz oder in dem Anteil der Messzeit mit einer Hauttemperaturseitendifferenz \(>2^{o}C\). Verminderte Anzahl von Temperaturschwankungen trat nur in der CRPS-Gruppe auf (27%). Die Berücksichtigung der Temperaturverlaufsanalyse zusätzlich zu der mittleren Hauttemperaturseitendifferenz, kann die autonome Dysfunktion in CRPS besser charakterisieren und erhöht die Spezifität der Methode.
