The optimal timing of anticoagulation for patients with acute ischaemic stoke with atrial fibrillation is uncertain. We investigated the efficacy and safety of early compared with delayed initiation of direct oral anticoagulants (DOACs) in patients with acute ischaemic stroke associated with atrial fibrillation.

13. Enrollment Management Professionals in Community Colleges: An Exploratory Study of Their Influence on Student Recruitment and Retention

Abstract

Community college leaders rely on enrollment management professionals (EMPs) to recruit and retain students, but research does not report the attributes these professionals should possess to contribute to student recruitment and retention. The purpose of this exploratory study was to determine if characteristics exist among EMPs that contribute to their influence on the matriculation and persistence of community college students. This study was designed to assist college leaders in the identification of EMPs who can transform institutional enrollment trends and contribute to college strategic planning. The goals were to describe EMPs from an eastern U.S. state and determine the salient factors that contribute to their effectiveness. Student development, leadership, and enrollment management theories were used as the theoretical framework. A quantitative, cross-sectional research design was applied in the self-administered, online survey e-mailed to EMPs. The significant findings included: (a) EMPs with strong institutional leadership skills were more effective in recruiting and retaining students; (b) these EMPs are actively engaged in the college strategic planning process; and (c) strong enrollment management skills, strong marketing skills, collaboration, and caring were attributes found to have the strongest correlation for EMPs and institutional effectiveness. 14. Student Characteristics Related to Persistence for First-Year Community College Students

Abstract

This study analyzed student characteristics that impact persistence among first-year students attending a large, metropolitan community college. The Cooperative Institutional Research Program (CIRP) Freshmen Survey was administered to first-time students during orientation. Factor analysis was used to classify students' personality and behavioral characteristics and discriminant function analysis was used to predict retention or attrition. The discriminant model accurately predicted retention in 78.8% of the cases. Findings suggested that student characteristics impacting persistence can be classified into three categories: barriers, motivations and aspirations, and expectations. Among the strongest predictors of attrition were the number of developmental classes required, the intention to transfer to a four-year institution, and the expectation that English as a second language could be a problem for college students. Among the strongest predictors of persistence were being African American or Latino, cumulative GPA, and the length of time students plan to spend at the college. Recommendations are shared for how community college administration, faculty, and staff can work with students and the community to raise student expectations, motivation, and preparation long before they become first-year college students. Abstract

The researchers examined the contracting efficiency metrics and leadership perceptions of telework within the United States Army’s Regional Health Contracting Office-Atlantic (RHCO-A) before and during the onset of the FY20 through FY21 COVID-19 pandemic. The research identifies the perceptions, challenges, and successes of telework in the federal government before and after the onset of the COVID-19 pandemic. First, the study explores previous research on perceptions of telework before and during the COVID-19 pandemic along with a past report on contracting efficiency metrics within the U.S. Army. This research, along with a study on telework by the RAND Corporation, provide a foundation for the analysis of contracting efficiency metrics and leadership perceptions of telework within RHCO-A during the COVID-19 pandemic. Next, the analysis outlines RHCO-A’s contracting efficiency metrics before FY20 and after a mass shift to full time telework among the RHCO-A workforce. Third, the analysis compares the telework perceptions of RHCO-A leaders before and during FY20. The results highlight the impact of the shift to near 100 percent full-time telework on contracting leadership perceptions of telework. Last, the recommendations focus on the objective measures contracting leaders should use to guide future telework policies within their organizations. Abstract

BACKGROUND: The success of coronary artery bypass grafting surgery (CABG) is dependent on long-term graft patency, which is negatively related to early wall thickening. Avoiding high-pressure distension testing for leaks and preserving the surrounding pedicle of fat and adventitia during vein harvesting may reduce wall thickening.METHODS: A single-centre, factorial randomized controlled trial was carried out to compare the impact of testing for leaks under high versus low pressure and harvesting the vein with versus without the pedicle in patients undergoing CABG. The primary outcomes were graft wall thickness, as indicator of medial-intimal hyperplasia, and lumen diameter assessed using intravascular ultrasound after 12 months.RESULTS: Ninety-six eligible participants were recruited. With conventional harvest, low-pressure testing tended to yield a thinner vessel wall compared with high-pressure (mean difference [MD; low minus high] -0.059 mm, 95% confidence interval (CI) -0.12, +0.0039, p = .066). With high pressure testing, veins harvested with the pedicle fat tended to have a thinner vessel wall than those harvested conventionally (MD [pedicle minus conventional] -0.057 mm, 95% CI: -0.12, +0.0037, p = .066, test for interaction p = .07). Lumen diameter was similar across groups (harvest comparison p = .81; pressure comparison p = .24). Low-pressure testing was associated with fewer hospital admissions in the 12 months following surgery (p = .0008). Harvesting the vein with the pedicle fat was associated with more complications during the index admission (p = .0041).CONCLUSIONS: Conventional saphenous vein graft preparation with low-pressure distension and harvesting the vein with a surrounding pedicle yielded similar graft wall thickness after 12 months, but low pressure was associated with fewer adverse events.

Abstract

Objective: Inflammatory Bowel Disease (IBD) consists of Ulcerative Colitis (UC) and Crohn's Disease (CD). The etiology of IBD has been studied and is still unknown, but several risk factors have been associated with the disease. Possible risk factors include genetic predisposition, oral contraceptive use and cigarette smoking. It is important to further research the link between cigarette smoking and IBD. Methods: Reviews and studies were obtained in order to evaluate the relationship between IBD and smoking. Discussion: Current cigarette smoking was found to increase the relative risk of developing Crohn's Disease and decrease the risk of developing Ulcerative Colitis. Conclusion: Although cigarette smoking seems to be protective against developing UC, the other risks associated with cigarettes far outweigh the possible benefits. In order to better understand the connection between IBD and cigarette smoking, it would be necessary to further research the causative mechanism of UC and CD.
