4. Barriers and Facilitators of Early Palliative Care in the Trajectory of People Living with Chronic Condition: A Mini Review Using Socio-ecological Framework to Inform Public Health Strategy.

Abstract

The increasing incidence of chronic conditions on a global scale requires a comprehensive approach to palliative care, which is recognized as an essential element of the continuum of care for people with life-threatening conditions. It has been argued that the integration of early palliative care for people with chronic conditions is beneficial. Nevertheless, barriers to integrating early palliative care have been extensively not reviewed. This review aimed to explore the barriers and facilitators of integrating early palliative care for people living with chronic conditions, employing a socio-ecological framework to provide a critical understanding of the influencing factors at multiple levels. The method was a mini review. This review emphasizes the complex and diverse factors that either hinder or facilitate progress in four areas: individual (such as gaps in knowledge and emotional reactions), interpersonal (such as support from family caregiver), healthcare professionals and policy (such as barriers in education, guidelines, and collaboration among professionals), and community (such as societal knowledge and attitudes). Each level presents unique challenges and opportunities for enhancing early palliative care integration, from addressing misconceptions and fears at the individual level, to fostering supportive policies and community awareness. In conclusion, the effective management of early palliative care requires a comprehensive strategy that spans various dimensions, including educational initiatives, policy adjustments, and active involvements of the community. Healthcare systems are supposed to enhance the integration of early palliative care into the trajectory of chronic conditions and enhance the quality of life of affected persons and their families by recognizing and addressing the socio-ecological elements involved. Abstract

To identify the latest evidence on the benefits of PV-I in wound infection prevention and healing in all areas. Methods: This review is a scoping review by Arskey and O'Malley and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) is used for reporting. The literature search used databases including PubMed, CINAHL, and search engines such as Scopus. This study was conducted using thematic analysis. Results: This review yielded 19 out of 2109 studies that were identified in the initial search. Four outcomes comprised 20 sub-outcomes have been identified. Although strong evidence is lacking, PV-I in the form of foam is effective in improving wound healing, shortening healing time, and producing fewer adverse events than hydrocelluers. Moreover, saline and PV-I are effective as skin disinfectants when compared to saline alone in reducing surgical site infection (SSI), and they are also more effective than hypochlorous acid in improving wound healing. Nevertheless, there was no difference between PV-I and hypochlorous acid in the prevention of SSI and bacterial growth. In terms of healing time, silver foam and hyaluronic acid were more favorable than PV-I. Moreover, the use of chlorhexidine to improve SSI and silver dressing to improve hospital stay is more favorable than that of PV-I. Conclusion: There is limited evidence regarding the effectiveness of wound care outcomes, and the efficacy of PV-I as a surface disinfectant for wound infection prevention remains controversial. Consequently, PV-I is not highly recommended for wound care, and further investigation into the effectiveness of PV-I as a surface disinfectant is required for various types of surgeries. The International Society for Peritoneal Dialysis (ISPD) recommends that every PD unit worldwide implement local PD training programs with the goal of improving self-care capabilities. This scoping review aims to give an overview of recent literature and recommendations on PD training programs aiming to improve the quality of care and outcomes for PD patients.Methods: The literature search was conducted using the PC (Population, Concept) approach. The population of interest in this study is PD patients, and the study concept is the PD training program. Several databases were used to conduct the literature search, including PubMed, Science Direct, and CINAHL. The search process began from July 2022 until January 2023. The inclusion criteria for the search included research articles and recommendations.Results: The search yielded 22 articles recommending training programs lasting from 5– 8 days, with 1– 3-hour sessions and a nurse-to-patient ratio of 1:1. A cumulative training time of 15 hours or more is recommended to enhance patient independence and reduce peritonitis rates. 29. Effectiveness of Telehealth Collaboration between Academic, Health Provider and Community towards People’s Participation for COVID-19 Self-reporting

Abstract

IntroductionNovel Corona Virus Disease (COVID-19) has been a major health issue in the world including Indonesia. This disease spread very fast in Indonesia with 1,298,608 positive cases and 35,405 people died. Self-assessment of COVID-19 and self-reporting of risks of infection are important to reduce infection in the community. However, there is also a need to perform physical distancing to reduce the spread of the disease. Thus, telehealth is considered a means to educate people about the importance of self-assessment and self-reporting of Covid-19 signs and symptoms. This study aimed to examine the effectiveness of telehealth collaboration between academic, health provider, and community towards people’s participation for COVID-19 self-assessment and self-reporting in West Java, Indonesia. Methods This study used a prospective cohort design with a time frame from 7th April to 13th July 2020. The samples were people in 26 sub-districts in West Java Province Indonesia. A Total of 6,578 people reported their condition through a website system. The telehealth was performed by 63 nursing students and 16 nursing lecturers in collaboration and partnership with health providers in community health centres and community leaders in 26-sub-districts. The 26 sub-districts comprised 3 groups with different levels of engagement: the first group consisted of 9 sub-district with a low level of telehealth engagement, second group 7 sub-district with a moderate level of telehealth engagement, and the third group 10 sub-districts with a high level of telehealth engagement. Data were analysed using one-way ANOVA test. Results The results showed that in the first group, 354 people were self-reported (mean=39.33). In the second group 1,111 people were self-reported (mean= 158.7). In the third group, 5,118 people were self-reported (mean= 511.8). The one-way ANOVA test showed that there is a significant difference between the three groups (F = 9.117; p=0.001).DiscussionCollaboration and partnership between academics, health providers, and community leaders have been proven can improve people’s participation in COVID-19 self-reporting. Telehealth is an effective way to facilitate collaboration and partnership. However, in some areas of this study, telehealth is not optimal due to limited phone signals and limited people who owned smartphones. ConclusionsTele-health is effective as one alternative to reach out and educate people about the importance of self-reporting of COVID-19 signs and symptoms. Nurses and health professionals in community health centres can use telehealth in addition to direct face-to-face interaction. Lessons learned Collaboration and partnership are important during pandemic COVID-19. Tele-health can become one of the means to reach out to educate people.Limitations Due to the circumstances during the early days of the pandemic, this study used a prospective cohort design, so that all activities were conducted without any efforts to control the samples. Suggestions for future researchFurther research can be conducted with a randomized controlled trial design and qualitative design to explore influencing factors for the people’s participation.

The theoretical analytical tools draw from pragmatism, symbolic interactionism and the negotiated order perspective.The analytical outcomes point to differing philosophical positions around palliative care as shaped by traditional medicine and the broader social, political, and economic contexts, all of which constantly challenge the positioning of palliative care in the health care system.

34. Factors Associated With Physical Activity and COVID-19 Preventive Behaviors: A Cross-Sectional Study in Indonesian Adults.

Abstract

Studies have shown that coronavirus disease (COVID-19) preventive measures and regular physical activity are essential for reducing disease risk. As such, utilizing the health belief model (HBM) framework, this cross-sectional study aimed to assess the association between sociodemographic and social-cognitive explored factors with physical activity and COVID-19 preventive behaviors in a nationally represented sample of 871 Indonesian adults. The Global Physical Activity Questionnaire was used to assess physical activity levels. The COVID-19 preventive behaviors and the social-cognitive factors were measured using instruments adapted from the WHO-COVID-19 Snapshot Monitoring and based on the HBM framework. Logistic regression was used to evaluate the association between the outcomes and each potential correlate, adjusted for potential confounders. The results showed that women were more likely to engage in sufficient COVID-19 preventive behaviors, while older adults were less likely to be physically active. Participants with high social-cognitive processes were also more likely to engage in sufficient COVID-19 preventive behaviors and be physically active. The perceived barrier, however, was not associated with COVID-19 preventive practices. The findings support using the HBM in understanding COVID-19 preventive and physical activity behaviors. Interventions to improve COVID-19 preventive practices in this target population should focus more on men, while physical activity promotion should focus more on older adults. [ABSTRACT FROM AUTHOR]

Abstract

COVID 19 pandemic has brought disruption to the normal health care provision in health facilities, including health care for HIV patients. Unfortunately, studies regarding this issue are still scarce. This study aims to analyse health care provision for people living with HIV during COVID-19 pandemic based on previous studies on this issue. This study used the scoping review method by searching two databases: CINAHL (EBSCO) and MEDLINE (PubMed). The article search was limited to articles written in English that were published during the period of 2019-2021, resulting in fourteen articles that met the inclusion criteria. It is demonstrated that various measures have been implemented to provide HIV care during the COVID 19 pandemic, including 1) modification of health facilities layout, 2) reduction of hospitals visits for check-ups, 3) limited visit time, 4) ensuring the availability of PrEP and PEP services, 5) SARSCOV-2 and HIV testing, 6) decentralisation, 7) telemedicine; and 8) reward provision. Abstract

This study sought to assess actions which Indonesia, Sudan, and Tanzania took to implement the health workforce commitments they made at the Third Global Forum on Human Resources for Health (HRH) in November 2013. The study was conducted through a survey of published and gray literature in English and field research consisting of direct contacts with relevant ministries and agencies. Results show that the three countries implemented interventions to translate their commitments into actions. The three countries focused their commitments on improving the availability, geographical accessibility, quality of education, and performance of health workers. The implementation of the Recife commitments primarily entailed initiatives at the central level, such as the adoption of new legislation or the development of accreditation mechanisms. This study shows that action is more likely to take place when policy documents explicitly recognize and document HRH problems, when stakeholders are involved in the formulation and the implementation of policy changes, and when external support is available. Abstract

Breast cancer-diagnosed women frequently use their spirituality as a way of coping to overcome difficulties. This study was conducted to determine the spiritual wellbeing among women diagnosed with breast cancer. A scoping review was conducted including the publication of various methodologies. Searches were conducted in PubMed, CINAHL, and Google Scholar databases published between 2016-2021, written in English, and peer-reviewed. This was acquired via Boolean searching for "breast cancer OR breast neoplasm OR breast carcinoma or breast tumo* OR breast malignanc* AND spiritual wellbeing OR spiritual wellness OR spirituality". Fourteen publications met the inclusion criteria. It is shown that the spiritual concepts among women with breast cancer influence how they deal with difficulties and provide meaning, as well as giving them a sense of purpose and self-efficacy by generating a good mental environment. The awareness and contemplation of the role of health professionals, particularly nurses, and their dedication to caring for and being near to the patient in a polite and dignified manner, establishing an environment of empathy, are necessary. Abstract

Neurological dysfunction caused depression, stress, anxiety, and a decrease in quality of life. The purpose of the study was to identify depression, stress, anxiety, and the quality of life which experienced by Stroke survivors during the COVID-19 pandemic. This scoping review was using advanced search techniques on several databases: EBSCO, PubMed, Scopus, and Google Scholar. Keywords included cerebrovascular accident, stroke, CVA, stroke survivor, quality of life, Depression, Stress, Anxiety, psychological disorder, Covid-19, Coronavirus, and sars-cov-2. Depression found 36% in Saudi Arabia, 38.7% got some complains, and 2.1% got extreme complain in Germany, 62% were depression in India, 32.2% have depressive symptoms in Ethiopia, 74.5% for anxiety/depression in Jordan, 23% in China, and 85% got mild and moderate, 15% got extreme in Spain. The quality of life of stroke survivors is low, as well as psychological disorders experienced by stroke survivors are anxiety, stress, and depression in moderate and severe. [ABSTRACT FROM AUTHOR]
