Cervical necrotizing fasciitis (CNF) is a life-threatening bacterial infection that runs aggressively in the subcutaneous tissue and fascia in the head and neck area with clinical presentation of extensive necrotic tissue that develops from infection of the teeth or gingiva and other supporting tissues accompanied by an endosteal or periosteal infection of the jaw. This research aims to obtain information about characteristic patients with cervical necrotizing fasciitis (CNF). This study is an observational study with a retrospective descriptive approach. Based on medical record data, the research was conducted at Hasan Sadikin General Hospital (RSHS) Bandung from March 2019 – March 2021. The data collected include age, gender, onset, nutritional status, region, clinical diagnosis, microbiology, antibiotic sensitivity resistance, length of stay, and source focal infection. In this study, 23 medical records were obtained. The most frequent characteristics of the subjects were elderly aged more than 50 years (47.8%) and males (61%). The most frequent locations were the submandible area (100%), and the most frequent source of focal infection was from mandibular posterior teeth (60.46%). From the onset to the symptoms frequently 7-14 days with the nutritional status, most patients were malnourished (58%). Regarding microbiology gram negative dominantly (66,6%), with the most bacterial that worsening the spread of CNF being Acinobacter Baumanii (26.0%), Tigecycline becomes antibiotic most sensitive (43,47%), Cefazoline is the most resistance (73.91%). Most patients were treated by surgery necrotomy debridement (95, 6 %), and most improved (76 %). The ability to diagnose quickly and take aggressive action is needed to treat patients with Cervical Necrotizing Fasciitis (CNF) because of its high progression.

Subcutaneous emphysema is an uncommon complication of dental procedures. This complication arises when air forced beneath the tissue. In dentistry it may appear with the use of high-speed bur and could cause a serious complication including airway obstruction. This case report highlights a complication during dental procedure to give information for the clinician the factors, diagnosis, and management of subcutaneous emphysema.A 33 years old male patient with difficulty in breathing and swelling at right lower jaw, neck and chest region. The swelling occurred about 2 hours before admission during tooth extraction of second right lower molar using a high-speed bur about 30 minutes, then he started to feel difficulty in breathing with swelling at right lower jaw, neck and chest region occurred and pain. Then he was transferred to Hasan Sadikin Emergency Department for further treatment. The patient was diagnosed with subcutaneous emphysema at right lower jaw, neck and chest region due to suspect iatrogenic. The management of this patient was conservative treatment with oxygenation 3 lpm, close observation vital sign and wide emphysema, IVFD ringer lactate 1500 cc/ 24 hours, medication with Ceftriaxone 1 gr, Ketorolac 30 mg, Omeprazole 20 mg intravenous and being hospitalized for 4 days. After general condition stable he was performed tooth extraction.Iatrogenic subcutaneous emphysema could be a serious and potentially life-threatening, so dentist has to be more careful while using high-speed bur for tooth extraction.

Pendahuluan: Pencabutan gigi molar ketiga merupakan salah satu prosedur kedokteran gigi yang sering dilakukan, tetapi bukan berarti resiko dan komplikasinya jarang terjadi. Salah satu komplikasi pencabutan gigi molar ketiga adalah fraktur mandibula. Penelitian ini bertujuan untuk mengetahui insidensi fraktur mandibula sebagai komplikasi pencabutan gigi molar ketiga rahang bawah. Metode: Rapid review ini dilakukan pada Desember 2020-Februari 2021 terhadap artikel berbahasa inggris yang melaporkan insidensi fraktur mandibula akibat pencabutan gigi molar ketiga rahang bawah. Pencarian artikel mengacu pada bagan PRISMA yang dilakukan melalui PubMed, ScienceDirect, dan Google Scholar tanpa membatasi tahun pencarian. Artikel terpilih memenuhi kriteria sesuai dengan kerangka PICO. Penapisan artikel dilakukan dengan memeriksa duplikasi, membaca judul dan abstrak, serta membaca isi keseluruhan teks. Hasil: Sebanyak delapan artikel dengan desain studi observasional dicakup dalam penelitian ini. Insidensi fraktur mandibula sebagai komplikasi pencabutan gigi molar ketiga berkisar antara 0,0046%-3,03%. Fraktur lebih banyak ditemukan pada laki-laki (n=42) daripada perempuan (n=20), pada usia rata-rata di atas 39 tahun, pada gigi-geligi lengkap (n=31) daripada tidak lengkap (n=3), pada gigi impaksi tulang penuh (n=47) daripada impaksi tulang sebagian (n=14), dan pada angulasi gigi horizontal (n=22) atau vertikal (n=20) daripada mesioangular (n=13) atau distoangular (n=6). Simpulan: Fraktur mandibula akibat pencabutan gigi molar ketiga rahang bawah jarang terjadi. Peningkatan kasus ditemukan pada pasien laki-laki, berusia di atas 39 tahun, memiliki gigi-geligi lengkap, gigi impaksi tulang penuh, dan angulasi gigi horizontal atau vertikal. Infeksi pre operatif dan kista merupakan keadaan patologis yang lebih sering ditemukan pada pasien fraktur.

The aim of this study is to investigate the volumetric measurements of the maxillary sinus among Indonesian population through computed tomography (CT) scan semi-automated segmentation. This project collected 802 retrospective head CT scan archives from Department of Radiology, Hasan Sadikin Hospital, Bandung, Indonesia between 2019-2020. Patients with craniofacial anomalies/pathology fracture in proximity of the maxillary sinuses, and mediocre image quality were excluded from this study resulting only 97 CT scan archives (194 maxillary sinuses; 52 males; 45 females; age range 0-25 years old). Three-dimensional craniofacial structures were reconstructed and volumetric measurements of the maxillary sinus were computed through semi-automated segmentation using ITK-SNAP. This study recorded the initial phase of maxillary sinus pneumatization during infancy. The maxillary sinus developed until reaching the maximum of average maxillary sinus volume at 13,278.73 mm3 in 16 to 20 years old group in which afterwards fell to 12,325.21 mm3. There was no difference found between right and left maxillary sinus volume. This study revealed that the pneumatization of maxillary sinus begin during infancy and climb until reaching the second decade of life, in which after that slowly decrease. Moreover, no difference between right and left maxillary sinus volume was detected. The volumetric dimension of maxillary sinus presented in this study may serve as the basis knowledge surgical intervention of maxillary sinus and its related structures.

Nyeri dan inflamasi merupakan respon tubuh pada kondisi patologis yang merupakan sinyal adanya kerusakan. Nyeri oromaksilofasial khususnya yang bersifat akut akibat luka inflamasi berpotensi menyebabkan gangguan fungsi rahang serta gangguan kualitas hidup penderita. Penelitian dengan menggunakan bahan alam seperti kurkumin yang dapat memodulasi respons inflamasi dapat membantu mengurangi rasa nyeri akibat luka inflamasi pada daerah oromaksilofasial. Pada kegiatan Program Pengabdian Masyarakat (PPM) yang dilakukan di Bandung Raya, penyuluhan dilakukan kepada 50 orang ibu-ibu PKK dari RW 08, Gunung Batu, Kelurahan Sukaraja, Kecamatan Cicendo secara daring dari kediaman masing-masing. Untuk efektivitas penelitian, dilakukan evaluasi melalui pemberian kuesioner pre-test dan post-test. Program edukasi yang dilakukan adalah edukasi mengenai efektivitas penggunaan kurkumin sebagai peningkatan sistem imun. Dari hasil evaluasi melalui pemberian pre-test dan post-test kepada sasaran, hasil dari kedua test tersebut dapat menunjukkan tingkat pengetahuan sasaran akan topik KKN-PPM kami meningkat sebesar 95,2%. Kegiatan penyuluhan yang telah dilakukan menunjukkan pengetahuan sasaran akan topik KKN-PPM meningkat setelah dilakukan penyuluhan.

Purpose: Mild traumatic brain injury (TBI) is common but accurate diagnosis and its clinical consequences have been a problem. Maxillofacial trauma does have an association with TBI. Neuron-specific enolase (NSE) has been developed to evaluate neuronal damage. The objective of this study was to investigate the accuracy of NSE serum levels to detect mild brain injury of patients with sustained maxillofacial fractures during motor vehicle accidents. Methods: Blood samples were drawn from 40 healthy people (control group) and 48 trauma patients who had sustained isolated maxillofacial fractures and mild brain injury in motor vehicle accidents. Brain injuries were graded by Glasgow Coma Scale. In the trauma group, correlations between the NSE serum value and different facial fracture sites were also assessed. Background: Ameloblastoma is the most common odontogenic tumor. Ameloblastoma is a borderline tumor because it is a benign but locally aggressive tumor with a high recurrence rate if the excision is not complete. The principle of treatment for ameloblastoma is excision all the tumor. This article aimed to conduct a retrospective study to analyze the characteristics of ameloblastoma in patients at Oral and Maxillofacial Surgery Hasan Sadikin Hospital Oral Surgery between the period of January 2018-December 2019Method: This is a retrospective study with 37 patients diagnosed with ameloblastoma during 2018-2019. We took data from each patient such as gender, age, radiological features, histopathological diagnosis of the location of ameloblastoma, management, defects, reconstruction. Result: A total of 22 patients were diagnosed with plexiform ameloblastoma, 12 cases of follicular ameloblastoma, 3 cases of mixed plexiform and follicular ameloblastoma. 31 patients were treated radically, while 6 patients were treated conservatively. Radical treatment is more often used to reduce recurrence rates, whereas conservative measures are indicated in children and adolescents, as well as adult patients with unilocular ameloblastoma types. Conclusion: The most characteristic of ameloblastoma is plexiform ameloblastoma in the mandible. Ameloblastoma is usually performed radically and reconstructed with an AO plate.

Objectives Oral carcinoma including oral squamous cell carcinoma (OSCC) is known for its detrimental and lethal effect. Therefore, a study regarding its epidemiology and related factors is of importance. The current study aimed on revealing the incidence of OSCC and its correlation with orofacial pain in Indonesia's patient sample. Methods Nine hospitals that are located at the province of West Java, Indonesia were randomly selected. From these hospitals, we collected a number of patients who visited the Oral Surgery outpatient clinic from July 2014 to June 2015 and were diagnosed with OSCC. The location of OSCC, age, sex, and the presence of orofacial pain were collected and cross tabulated. Significant correlation was analyzed by using Pearson correlation. Results Based on its location, the most commonly found OSCC in an Indonesian sample is located in the buccal area (26 cases) with a percentage of 27.4%. Eighty-four patients out of ninety-five patients with OSCC experienced pain due to the development of OSCC. There was a significant association between the anatomic site of OSCC and orofacial pain ( p = 0.00) whilst a significant and positive correlation was found between orofacial pain experienced by OSCC patient ( p = 0.03) and age. As for sex and the location of the OSCC, no significant correlations were found. Conclusion The high number of OSCC cases accompanied with orofacial pain in West Java Province, Indonesia indicated an immediate yet effective treatment requirement for OSCC in Indonesia.

Background: Curcumin, a natural herb that can be isolated from turmeric has been known for its therapeutic potential, including its chemopreventive potential, while heme oxygenase-1 (HO-1) is an antioxidant enzyme that can act as the biomarker for the progression of oral squamous cell carcinoma (OSCC). The current study investigated the efficacy of curcumin as a chemopreventive agent for OSCC by evaluating the immunoexpression of HO-1 at epithelial dysplasia stage. Methods: The current experimental study was conducted at the Veterinary Medicine Faculty, Bogor Agricultural Institute, Indonesia, using 40 Sprague-Dawley rats. All rats were randomly divided into 1 of 2 groups: the experimental group that was fed with standard rat food and curcumin and the control group that was fed with standard rat food without curcumin. At the beginning of the fifth week, every animal was wounded on the right buccal mucosa and was exposed to 0.5% 7,12-dymethylbenz(a)anthracene 3 times a week for 4 weeks in order to induce epithelial dysplasia of OSCC. On the ninth week, the right buccal mucosa was biopsied and the immunoexpression of HO-1 in both groups was compared and analyzed. Results: Sample examination revealed that all rats showed moderate to severe dysplastic epithelial dysplasia stage of OSCC, while a Mann-Whitney test showed that there was no significant ( P = .09) difference found between the 2 groups for the immunoexpression of HO-1. Conclusion: With regard to its chemopreventive potential, the curcumin dose used in the study was not proven to be effective. Further study is of importance.

Introduction: Head and neck infections commonly caused by spreading odontogenic infection, which able to cause a local and systemic manifestation, from mild to severe complications according to its severity. Assessment of the severity of odontogenic infections is essential to determine the prognosis, assessed through local and systemic parameters. Laboratory examination used to examine the biological marker that indicates the ongoing infection. C-reactive protein (CRP) is a protein synthesised by hepatocytes in response to the body's inflammatory process, including odontogenic infection. This study was aimed to analyse the severity score of odontogenic infections using the CRP levels in patients with odontogenic infections. Methods: An analytic study with a cross-sectional approach was conducted on 30 patients of Dr Hasan Sadikin General Hospital Bandung diagnosed with odontogenic infections, determined by calculations according to Hosmer and Lemeshow. Assessment of the severity score in odontogenic infections and quantitative CRP levels with the Dimension tool was conducted afterwards. Correlation between odontogenic infection severity score and CRP levels was analysed using regression and correlation tests. Results: From 30 subjects, there was found 33.3% with mild severity score, 63.3% moderate score and 3.3% severe score. The mean of the CRP level was significantly increased according to the odontogenic severity scoring category. Introduction: Infratemporal abscess due to odontogenic infection is a rare case, because most dental infections spread through the submandibular space or to the deep neck spaces. Abscess formation in this area may lead to dangerous complications, which infection from this area might spread through the pterygoid plexus to the cavernous sinus or through the valveless ophthalmic veins into the orbit, and may result in intracranial and orbital complications. Case report: We reported 3 cases of infratemporal abscess which were handled at the Oral and Maxillofacial Surgery department of Dr. Hasan Sadikin Hospital Bandung. Patients were two males and one female. They all had a history of toothache in the left lower jaw region. All of the patients underwent surgical incision drainage. Conclusion: Diagnosis of an infratemporal abscess can be confirmed by careful history taking and clinical examination. Early detection and aggressive treatment of infratemporal abscess due to odontogenic infection can avoid mortality and minimized complication caused by delay in treatment.

Introduction: Patients with a facial fracture with open depressed frontal fracture and head trauma required prompt management. Delayed management of head trauma caused massive bleeding, infections, damage to brain function, or even caused by death. This case report aimed to explain the emergency treatment of facial trauma accompanied by an open depressed frontal bone. Case report: A 34-year-old female patient came with bleeding from the mouth to Hasan Sadikin Emergency Department. ± 1 day before admission, she had a history of the traffic accident. Extraoral examination there was edema at the facial region, post situational suturing at frontonasal, left palpebra and lower lip region, and lacerated wound at left palpebra region. Intraoral examination there was an open bite, hematoma, and edema at the upper lip, gingiva, and vestibule region. The patient was diagnosed with a facial fracture with open depressed frontal fracture and head trauma. This article will be reported a case about the management of open depressed fracture frontal with facial fractures by controlled bleeding to prevented aspiration. After the emergency management, the patients are treated at the ward for 4 days and the management of facial fractures by elective open reduction with internal fixation (ORIF). Conclusion: Immediate and appropriate treatment of facial trauma accompanied by head injury at the frontal region can prevent complications and morbidity of the patient

Depending on its stage on diagnosis, oral squamous cell carcinoma (OSCC) might cause excruciating pain and decreased quality of life. As for treatment, the treatment of OSCC might vary from chemotherapy to surgery. The objective of the current study was to assess the preoperative and postoperative oral cancer pain, anxiety, and quality of life of OSCC patients with invasive treatment procedure. The current study was conducted by interviewing 21 (10 males; 11 females) patients who had been diagnosed with stage 3 and stage 4 OSCC and about to go through surgery at the inpatient ward of Surgical Oncology Department, Hasan Sadikin Hospital, Bandung, Indonesia. A preoperative and interview was conducted by using the Hospital Anxiety and Depression Scale (HADS), the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QOL)-C30, the shortened EORTC QOL Questionnaire for Oesophageal Cancer (OES)18, the visual analog scale (VAS), and the University of California San Francisco (UCSF) Oral Cancer Pain Questionnaire. Haemangiopericytoma is an uncommon tumour that is particularly rare in children. The tumour has a wide anatomical distribution, including the oral cavity. This report is of a 2-year-old child with haemangiopericytoma in the tongue.

Ankylosis of the temporomandibular joint (TMJ) involves fusion of the mandibular condyle to the base of the skull. Trauma and infection are the leading causes of ankylosis. A case of true bilateral ankylosis of the temporomandibular joint is presented. A 21-year-old male patient had a multiple bone fractures history at the age of 13 due to a sports injury. A TMJ injury might not be detected at that time resulting in a progressive restriction of his mouth opening. He presented with almost complete lack of mobility of the mandible. Surgical treatment was a resection of the ankylotic bone mass, interposition temporalis superficial fascia flaps, and early mobilization and aggressive physiotherapy. The functional results showed good remarks.
