Several important papers discussing phoretic dispersal in animals have appeared in recent years, but the nature and evolutionary significance of the phenomenon are still not fully understood. This article reviews the current knowledge of this behaviour in Uropodina mites, which disperse at the deutonymphal stage. We first examine the morphological and behavioural attributes of Uropodina deutonymphs involved in phoretic dispersal. We then provide a systematic review of the animals that carry Uropodina, and examine the carrier-specificity of 30 well-known species. Most species are associated with a single family of carriers, or at most, two families. The available information shows that phoretic deutonymphs of most species do not show any preference for male or female carriers. The spatial pattern of attachment of phoretic deutonymphs varies according to the identity of the carrier insect. The nature of deutonymph infestation depends on the size and morphology of both the mite and its carrier. Successful phoresy also depends on synchronisation of the life cycles of the mite and its carrier, but the temporal dynamics of phoresy remain particularly poorly studied. Phoresy has ecological and evolutionary consequences for both the Uropodina and their carriers. Future research on phoresy will benefit from studies of its evolutionary development, the reasons why beetles appear to be specially favoured as carriers, how phoretic mites affect the insect that carries them, and how the apparent global decline in insect populations is affecting their symbiotic mites. The subcutaneous parasitic mite Hemimyialges macdonaldi was found in a specimen of the Orange‐bellied Parrot Neophema chrysogaster that died in captivity. A review of the host range and geographic distribution of this mite shows it is cosmopolitan and occurs in a very wide range of birds. Remote telehealth practices were forced to advance 10 years in a few short weeks in March 2020 due to the onset of a global pandemic. In the sphere of non-clinical medicine, a dramatic element of uncertainty entered the psyche of doctors and lawyers in relation to the validity of remote or virtual independent medical examination (vIME). This paper considers the key issues surrounding the virtual assessment of clients for medicolegal purposes. Our main hypothesis was that, within certain defined parameters, the vIME technique can deliver reliable and accurate assessments. To explore this, a systematic literature search focusing on advanced device-based range of motion measurement was conducted, along with an historical snapshot of observation-based range of motion measurement considering application to remotely performed IME. While some specialists are of the view that observational measurement may be applied reliably to some joints when conducted by experienced orthopaedic surgeons, evidence for this is scant. The results, instead, support the notion of using task substitution, that is specialists appropriately assisted in conducting vIMEs by musculoskeletal trained allied health practitioners, regardless of the measurement tool, for permanent impairment assessments. Moreover, self-performed examinations by injured individuals using advanced technology are not reliable in this setting. Our final contention is that remote examinations with limited clinical assessment have utility for legal matters, such as the assessment of causation of injury, treatment advice or approvals and fitness for pre-employment tasks or safe variations, with objective clinical adjunct support such as Picture Archiving and Communication System-based modern radiology systems.

BACKGROUND: The presence of late-gadolinium enhancement (LGE) predicts life threatening ventricular arrhythmias in non-ischemic dilated cardiomyopathy (NIDCM); however, risk stratification remains imprecise. LGE shape and simulations of electrical activity may be able to provide additional prognostic information. OBJECTIVE: This study sought to investigate whether shape-based LGE metrics and simulations of reentrant electrical activity are associated with arrhythmic events in NIDCM patients. METHODS: CMR-LGE shape metrics were computed for a cohort of 156 NIDCM patients with visible LGE and tested retrospectively for an association with an arrhythmic composite end-point of sudden cardiac death and ventricular tachycardia. Computational models were created from images and used in conjunction with simulated stimulation protocols to assess the potential for reentry induction in each patient’s scar morphology. A mechanistic analysis of the simulations was carried out to explain the associations. RESULTS: During a median follow-up of 1611 [IQR 881-2341] days, 16 patients (10.3%) met the primary endpoint. - Abstract
We describe a new species of Ophiomegistus Banks, 1914 from Australia—O. rex sp. nov. parasitic on the lizard Ctenotus regius Storr, 1971. We review the state of knowledge of the genus Ophiomegistus in Australia, and conclude that much more extensive collecting will be needed to determine the degree of host specificity in this genus of parasites.

Ticks identified as Rhipicephalus sanguineus (Latreille, 1806) have been recorded in Australia since 1896. It is now recognized that ticks identified as R. sanguineus belong to different lineages. Recently, the so‐called temperate lineage has been redescribed as R. sanguineus sensu stricto with a designated neotype. In Australia, the evidence suggests that only R. sanguineus sensu lato tropical lineage exists. We present a genetic evaluation of R. sanguineus sensu Roberts (1965) from museum material that was evaluated or identified by Roberts. We evaluated 12 museum specimens collected between 1913 and 1964, from various locations around Australia at the molecular level. DNA isolation was successful for 4/12 specimens. All four museum specimens were 100% identical to each other and to the reference R. sanguineus s.l. tropical lineage. Measurements of morphologically significant structures for male and female museum and contemporary tick specimens were obtained. The morphologically significant structures revealed there was no significant difference (ANOVA, P > 0.05) between these groups of male and female ticks. This study updates and supplement the comprehensive descriptions provided by Roberts (1965), which genetically belong to R. sanguineus s.l. tropical lineage. [ABSTRACT FROM AUTHOR]

ESC 2005 and AHA guidelines 2015 in sports cardiology restricted individuals with hypertrophic cardiomyopathy (HCM) from moderate and high intensity exercise and limited them to low intensity sport.1,2 Perceived increased risk of sudden cardiac death (SCD) with vigorous exercise in individuals with HCM was based on early registry data from the United States.3 This enforced sedentary lifestyle has led to an increasing trend towards obesity, atrial fibrillation and atherosclerotic coronary artery disease in patients with HCM.4 Recent ESC 2020 guidelines on exercise5 has enabled clinicians to recommend safe levels of exercise for these patients.We audited how well our outpatient practice reflected the guidance set out in 2020 ESC guidelines on exercise for patients with cardiovascular disease.Electronic records of 731 patients with a clinical diagnosis of HCM seen in our outpatient service between 2008-2023 were audited to determine whether written advice on exercise was given and if this was in line with current ESC guidelines at any encounter.731 patients with a mean age of 62±14.5 (range 17-91 years) were seen in our specialist outpatient service. 68% were male. 51% of patients received exercise advice either at their initial visit ± a follow-up visit. A significant proportional increase (16%,P<0.001) was seen in the number of patients receiving exercise advice at their initial visit after publication of the ESC 2020 guidelines (Table 1). 43% (270/634) of those who had their initial visit pre-guidelines received updated exercise advice as per 2020 ESC guidelines during follow-up. There was an increase in the proportion of patients receiving exercise advice each year after publication of ESC guidelines (Graph 1). Immediately following the publication, a high proportion received specific advice pertaining to intensity and duration of exercise (68% in 2020) with a slight decline subsequently (43%, 36% and 47% in 2021, 2022 and 2023 respectively). 27 patients had an implantable cardioverter defibrillator (ICD) in situ at the time of initial review with a further 127 undergoing ICD insertion under our care. SCD risk was documented for 66% (468/704) of patients according to the 2014 ESC Risk-SCD Model. 63% (214/338) of low risk patients received exercise advice, whilst 77% (40/52) and 54% (42/78) of intermediate and high risk patients received exercise advice respectively. 22 deaths (3%) were recorded during the follow-up period with none occurring during exercise. All cases except 2 were non-cardiac related.An overall improvement was seen in our practice with a significant increase in the number of patients receiving exercise advice after publication of the new guidelines. Despite this, only 51% of our patients received any written guidance on exercise demonstrating a need for further improvement in our outpatient practice to reflect the paradigm shift in the approach to exercise in individuals with HCM as set out by ESC 2020 guidance.

- Abstract
Background: Femoral neck fractures are associated with high rates of revision surgery after management with internal fixation. Using data from the Fixation using Alternative Implants for the Treatment of Hip fractures (FAITH) trial evaluating methods of internal fixation in patients with femoral neck fractures, we investigated associations between baseline and surgical factors and the need for revision surgery to promote healing, relieve pain, treat infection or improve function over 24 months postsurgery. Additionally, we investigated factors associated with (1) hardware removal and (2) implant exchange from cancellous screws (CS) or sliding hip screw (SHS) to total hip arthroplasty, hemiarthroplasty, or another internal fixation device. Methods: We identified 15 potential factors a priori that may be associated with revision surgery, 7 with hardware removal, and 14 with implant exchange. We used multivariable Cox proportional hazards analyses in our investigation. Background Improved understanding of dilated cardiomyopathy (DCM) due to titin truncation (TTNtv) may help guide patient stratification. Objectives The purpose of this study was to establish relationships among TTNtv genotype, cardiac phenotype, and outcomes in DCM. Methods In this prospective, observational cohort study, DCM patients underwent clinical evaluation, late gadolinium enhancement cardiovascular magnetic resonance, TTN sequencing, and adjudicated follow-up blinded to genotype for the primary composite endpoint of cardiovascular death, and major arrhythmic and major heart failure events. Results Of 716 subjects recruited (mean age 53.5 ± 14.3 years; 469 men [65.5%]; 577 [80.6%] New York Heart Association function class I/II), 83 (11.6%) had TTNtv. Patients with TTNtv were younger at enrollment (49.0 years vs. 54.1 years; p = 0.002) and had lower indexed left ventricular mass (5.1 g/m2 reduction; padjusted = 0.03) compared with patients without TTNtv. There was no difference in biventricular ejection fraction between TTNtv+/− groups. Overall, 78 of 604 patients (12.9%) met the primary endpoint (median follow-up 3.9 years; interquartile range: 2.0 to 5.8 years), including 9 of 71 patients with TTNtv (12.7%) and 69 of 533 (12.9%) without. There was no difference in the composite primary outcome of cardiovascular death, heart failure, or arrhythmic events, for patients with or without TTNtv (hazard ratio adjusted for primary endpoint: 0.92 [95% confidence interval: 0.45 to 1.87]; p = 0.82). Conclusions In this large, prospective, genotype-phenotype study of ambulatory DCM patients, we show that prognostic factors for all-cause DCM also predict outcome in TTNtv DCM, and that TTNtv DCM does not appear to be associated with worse medium-term prognosis.

- Abstract
Background Reoperation rates are high after surgery for hip fractures. We investigated the effect of a sliding hip screw versus cancellous screws on the risk of reoperation and other key outcomes. Methods For this international, multicentre, allocation concealed randomised controlled trial, we enrolled patients aged 50 years or older with a low-energy hip fracture requiring fracture fixation from 81 clinical centres in eight countries. Patients were assigned by minimisation with a centralised computer system to receive a single large-diameter screw with a side-plate (sliding hip screw) or the present standard of care, multiple small-diameter cancellous screws. Surgeons and patients were not blinded but the data analyst, while doing the analyses, remained blinded to treatment groups. The primary outcome was hip reoperation within 24 months after initial surgery to promote fracture healing, relieve pain, treat infection, or improve function. Abstract: Drought significantly shifts the mix of functional groups present in river ecosystems and abundances of many aquatic fauna decline. The consequences of drought for fish often include the decline of ecological specialists and the predominance of generalist species. However, factors influencing the restructuring of littoral fish assemblages following prolonged drought in heavily regulated rivers are poorly understood. We compared the restructuring of littoral fish assemblages in 2 large connected lakes, which differ in hydrology and habitat availability, after 15 years of drought in their catchment. The once‐abundant and diverse fish assemblages showed limited signs of postdrought recovery after 5 years. There were differences between the 2 lakes related to the species present, their abundances, and the functional groups within fish assemblages. The functional groups present also shifted through time from alien‐dominated to freshwater generalist‐dominated. Only 2 of 5 native fish functional groups (freshwater generalist and diadromous) had increasing abundances (used as a proxy for recovery) following the drought, and these increases only occurred in the lake with the greater connectivity and diversity in habitat and hydrology. In contrast to patterns observed for natives, abundances of alien fishes in the littoral zone of the lakes declined after an initial spike in numbers associated with substantial river flows immediately following drought. This study shows that the recovery of native fish populations following prolonged drought in a heavily regulated semiarid river occurs over a time scale of years and recovery of some extant ecological specialists was incomplete after as many as 5 years. Lectrides varians (Mosely) is a large, ecologically-important, caddisfly found in perennial and intermittent streams throughout much of eastern Australia. We conducted a population genetic analysis to investigate the dispersal potential of L. varians, building on previous works that have assessed life-history traits associated with drought resistance. Genetic analyses of L. varians from the Grampians region of Victoria, based on mitochondrial DNA sequence data, revealed extensive gene flow and a lack of genetic structure across the sample range (ΦST = 0.04). This suggests that the species is a strong disperser and is likely to be resilient to increased drying and habitat fragmentation under climate change considering other known resistance traits. However, during this study, two divergent genotypes were identified, indicating a potential species complex. A comprehensive phylogenetic analysis of L. varians across its current range was subsequently performed, confirming the species is indeed paraphyletic, consisting of one lineage that is restricted to the Grampians National Park and the other being widespread throughout south-eastern Australia. Further analyses revealed consistent morphological differences between these lineages supporting the notion that L. varians is a species complex. We discuss the implications of these findings with regard to conservation and taxonomy of this important invertebrate group.
