The exact solution to the simple pendulum problem has long been known in terms of Jacobi elliptic functions, of which an efficient numerical evaluation is standard in most scientific computing software packages. Alternatively, and as done in V. Fairén, López and L. Conde (Power series approximation to solutions of nonlinear systems of differential equation, Am. J. Phys.   56 (1988) 57–61], the pendulum equation can be analytically solved exactly by the power series solution method. Although recursive formulae for the series coefficients were provided in V. Fairén et al. , the series itself—as well as the optimal location about which an expansion should be chosen to maximize the domain of convergence—has not yet been examined, and this is provided here. By virtue of its representation as an elliptic function, the pendulum function has singularities that lie off of the real axis in the complex time plane. This, in turn, imposes a radius of convergence on the power series solution in real time. By choosing the expansion point at the top of the trajectory, the power series converges all the way to the bottom of the trajectory without being affected by these singularities. We provide an exact resummation of the pendulum series that accelerates the series' convergence uniformly from the top to the bottom of the trajectory. We also provide the formulae needed to extend the pendulum series for all time via symmetry. The pendulum problem, in its relative simplicity, provides an explicit demonstration of the effect of singularity structure and initial condition location on convergence properties of power series solutions. Premature closure is often described as a significant contributor to diagnostic error. Therefore, developing strategies to mitigate premature closure could reduce diagnostic errors and improve patient care. Here we propose the novel concept of pursuit of an "endpoint diagnosis" as a cognitive forcing strategy (CFS) for avoiding premature diagnostic closure. We define an "endpoint diagnosis" as an underlying causative explanation for a patient's signs, symptoms, and laboratory and radiographic data that exhausts additional relevant diagnostic evaluation. We have observed four contexts in which the error of not pursuing an endpoint diagnosis most often occurs: (1) diagnoses that appear to result in the same treatment regardless of etiology, (2) cases that are particularly complex, (3) clinical scenarios that are vulnerable to systems errors, and (4) situations in which patients' problems are attributed to uncontrolled underlying risk factors or an exacerbation of a known condition. Additionally, we address why we believe endpoint diagnoses are not universally pursued, delineate when this approach might be particularly useful, attempt to reconcile the potential conflict between accepting diagnostic ambiguity in certain instances and pursuing endpoint diagnoses, and outline possible concerns that might arise with using this CFS, including the possibility of lengthy evaluations resulting in overdiagnosis and overtreatment. Objectives: Formal nighttime education is becoming increasingly necessary as more internal medicine (IM) residency programs adopt night-float rotations (NFRs); however, the efficacy of an NFR curriculum throughout an academic year and which topics in an NFR curriculum increase trainee confidence are unknown. We implemented a 12-module, self-paced NFR curriculum for 76 postgraduate year-1 residents at an academic IM residency program. We evaluated the impact of this curriculum on postgraduate year-1 residents' clinical confidence, as well as longitudinal efficacy of the curriculum.Methods: Night-float interns' (NFIs) clinical confidence regarding specific curricular topics was evaluated overall and during specific timeframes within the academic year. Pre- and post-NFR surveys using Likert scales for each topic were administered to NFIs from June 24, 2020 to March 2, 2021, representing 32 week-long NFR cycles.Results: NFIs' pre- and postrotation confidence in managing clinical scenarios significantly improved for all 12 topics in the NFR curriculum. The NFR curriculum resulted most significantly in improved confidence during the first 4 months of the academic year, with 11 of 12 curricular topics reaching the threshold for statistical significance. Modules on altered mental status, hypotension, narrow-complex tachyarrhythmias, new fever, and sepsis and antibiotic escalation maintained their efficacy for the longest periods of time.Conclusions: It may be especially important to emphasize an NFR curriculum at the beginning of the academic year. IM residency programs also may wish to carefully consider that certain topics may maintain their efficacy throughout the year, whereas other topics should potentially be replaced with more complex modules as the academic year progresses. We analyze two preindustrial experiments from the Community Earth System Model version 2 to characterize the impact of sea ice physics on differences in coastal sea ice production around Antarctica and the resulting impact on the ocean and atmosphere. The experiment in which sea ice is a more realistic "mushy" mixture of solid ice and brine has a substantial increase in coastal sea ice frazil and snow ice production that is accompanied by decreasing bottom ice growth and increasing bottom melt. The more realistic "mushy" physics leads to an increase in water mass formation at denser water classes due primarily to surface ice processes. As a result, the subsurface ocean is denser, saltier, and there is an increase in Antarctic Bottom Water formation of ∼0.5 Sv. For the atmosphere, "mushy" ice physics leads to decreased turbulent heat flux and low level cloud cover near the Antarctic coast. Plain Language Summary: We analyze experiments from the Community Earth System Model to better understand the impacts of representing sea ice as a mixture of salty water and solid ice rather than just solid ice. We focus on sea ice produced around the Antarctic coasts and find that the ways in which the sea ice grow and melt change with the two representations of sea ice, but the differences compensate so that the average sea ice state is minimally changed. However, the near surface ocean water is denser in the experiment with sea ice represented by a mix of solid ice and salty water, mainly because the ocean is saltier due to surface sea ice processes. This leads to increased formation of dense Antarctic Bottom Water. In addition, there is less energy input into the atmosphere and less low level cloud cover around the Antarctic coasts in the experiment with the sea ice represented as a mix of salty water and solid ice. Thus, there are important impacts on the Earth system based solely on the way sea ice is represented. • We studied how guilt, shame and fear predict intention to use a phone while driving. • We surveyed a sample of young drivers (18 to 25) as a high-risk driver group. • An extended TPB model that included emotions outperformed the standard TPB model. • We compared road signs with emotion-evoking messages with those without. • Increased emotions across signs correlated with decreased engagement intention. This study sought to evaluate the relationship between young drivers' intention to engage in cellphone distractions while driving and their emotions towards the associated risks. First, we assessed whether the emotions of guilt, shame, and fear are associated with young drivers' intention to engage in cellphone distractions, through an extended Theory of Planned Behavior (TPB) model. Second, we evaluated whether road signs that may evoke these negative emotions reduce cellphone use intentions among young drivers. An online survey was conducted with young drivers (18 to 25 years old) from Ontario, Canada. 403 responses were collected, of which, 99 responses were used to evaluate the first objective and all 403 responses were used to evaluate the second objective. Anticipating feelings of guilt, shame, and fear negatively predicted the intention to engage in cellphone distractions, above and beyond the standard TPB constructs (i.e., attitudes, subjective norms, and perceived behavioral control). When prompted with potentially emotion-evoking road signs (children crossing, my mom/dad works here), an increase in anticipated feelings of these emotions corresponded with lower intention to engage in cellphone distractions. Countermeasures that target young driver emotions toward distracted driving risks may be effective in reducing their distraction engagement. What has become known in bioethics as "the Nazi analogy" likens a change's potential to precipitate moral deterioration to Nazi atrocities of the mid-20th century. This analogy has been applied in physician aid-indying (PAD) deliberations by those fearful that a physician's role in enabling a patient's death is too similar to Nazi physicians' roles in systematic murders during the Holocaust. This article suggests the importance of carefully distinguishing between when the Nazi analogy is aptly applied and when its use is limited to urging great caution about abuse or inequity. The temperature dependence of the indirect bandgap in thallium bromide has been determined using variable temperature (5K to 300 K) cathodoluminescence. The spectra include transitions associated with both the indirect (2.66 eV at 5K) and the direct (3.0 eV at 5K) bandgaps. Leastsquares analysis has been used to obtain fitting parameters for three analytical models commonly applied to describe the dependence of the bandgap on temperature for the lowest energy indirect transition. The indirect bandgap emission shifts to an energy of 2.86 eV at 300 K. We find a significant difference in the behavior of the luminescence associated with the direct bandgap, which does not appear to shift to the extent predicted by earlier measurements of the exciton absorption edge. The admittance characteristics of a well understood oxide system of Gd0.25 Ga0.15 O0.60/Ga2 O3 on In0.53 Ga0.47As are studied at low temperature where the majority of the defect states are frozen. The capacitance-voltage (C-V) characteristics are broadened beyond those expected for an ideal sample. The broadening is ascribed to the presence of a non-uniform surface potential. We report a simple method for measuring the surface potential distribution width using the ac signal amplitude dependence of the C-V profile. A distribution of surface potentials can explain the observed behavior, but only if it includes the admittance of the semiconductor. This is in contrast to the usual approximation that the effect of a distribution of surface potentials on the admittance of the semiconductor is ignored. It is found that the charge state of defect states in the oxide and variation in the doping density of the substrate contribute to the broadening. The implications for inclusion of the semiconductor admittance for the interpretation of ideal characteristics and for the analysisof characteristics with interface states are explored. Both the conductance method and the quasi-equilibrium C-V methods are affected to some extent. An alternative method that can more accurately extract information from the admittance characteristics of the MOS samples is proposed. [ABSTRACT FROM AUTHOR]

The electrical characteristics of n+ GaAs and In0.53Ga0.47As MOS capacitors with a dielectric stack of Ga2O3/Gd0.25Ga0.15O0.6 have been examined in detail and compared to the interface state model. The deviations from the model are assessed and the limitations of different interface state density extraction techniques are highlighted. The results of a model which accounts for many of the electrical characteristics of the InGaAs material by including states within the oxide and at the interface are reported. A hypothesis that may explain the difference between the GaAs and InGaAs characteristics and the similarities between the properties of many different oxides on InGaAs is discussed, leading to suggestions on how the oxide quality may be improved. Open racetrack flumes represent the most common reactor for algae cultivation in the biodiesel industry, however, despite numerous experimental and numerical studies into reactor modifications, conditions remain suboptimal for algal growth. In response, a full-scale racetrack flume was constructed at the Ecohydraulics and Ecomorphodynamics Laboratory at the University of Illinois. Experiments on the racetrack flume were conducted for various depth and velocity conditions, using acoustic doppler velocimetry and surface particle velocimetry to characterize mean and turbulent velocity statistics, and dissolved oxygen measurements to investigate the effect of turbulent structures on gas transfer at the water-air interface. Longitudinal bed modifications were introduced to induce secondary flows in the straight portions of the flume. Semicircular and triangular bars of two different sizes were tested in an effort to increase the transfer velocity at the free surface. A range of flow structures were observed including secondary currents of Prandtl's first and second kinds, vortex shedding off of bend vanes, and periodic oscillations in surface lateral currents. Findings indicate that bend dynamics introduce the strongest and most resilient flow structures, and any attempt to induce vertical mixing or accelerate transfer velocities at the free surface will need to utilize or overwhelm these existing structures. Introduction Contemporary haemophilia care demands Patient-Reported Outcomes. SO- FIT is a UK multi-centre study, assessing self-reported function, health-related quality of life ( HRQoL) and joint health in boys with severe haemophilia. Methods Subjective physical function (PedHAL, HEP-Test-Q) and HRQoL (Haemo-QoL Short Form [SF]) were assessed alongside joint health using the objective Haemophilia Joint Health Score (HJHS v2.1). Demographic and clinical data were collected. Results Data from 127 boys mean age 12.38 ± 2.5 (range 8-17) treated at 16 sites were analysed. One-hundred-and-thirteen had haemophilia A, 25/9 past/current inhibitor, 124 were treated prophylactically (46.8% primary) and three on-demand. In the preceding 6 months, boys reported median 0 joint bleeds (range 0-8) with a median HJHS score of 1 (range 0-30). Boys reported good physical functioning; HEP-Test-Q (M = 80.32 ± 16.1) showed the highest impairments in the domain 'endurance' (72.53 ± 19.1), in Ped HAL (M = 85.44 ± 18.9) highest impairments were in the domains 'leisure activities & sports' (M = 82.43 ± 23.4) and 'lying/sitting/kneeling/standing' (M = 83.22 ± 20.3). Boys reported generally good HRQoL in Haemo-QoL SF SF (M = 22.81 ± 15.0) with highest impairments in the domains 'friends' (M = 28.81 ± 30.5) and 'sports & school' (M = 26.14 ± 25.1). HJHS revealed low correlations with the Haemo-QoL SF ( r = .251, P < .006), the PedHAL ( r = −.397, P < .0001) and the HEP-Test-Q ( r = −.323, P < .0001). A moderate correlation was seen between HEP-Test-Q and Haemo-QoL SF of r = −.575 ( P < .0001) and between Ped HAL and Haemo-QoL SF r = −.561 ( P < .0001) implying that good perceived physical function is related to good HRQoL. Conclusions The SO- FIT study has demonstrated that children with severe haemophilia in the UK report good HRQoL and have good joint health as reflected in low HJHS scores. [ABSTRACT FROM AUTHOR]

Road traffic collisions (RTCs) are a global public health concern; however, research on the impact of bereavement on families remains limited. A critical realist approach was adopted to explore experiences of families suffering bereavement following RTCs, using interviews with 14 participants in the United Kingdom (UK) who have lost a family member. Three key themes were identified: (1) worsening mental health following bereavement, (2) negative impact of an RTC-related bereavement upon family members, (3) limited support following an RTC. Findings highlighted the requirement for appropriate support for bereaved families, and outlined significant flaws within the UK legal system, sentencing, and treatment of families. Entamoeba histolytica is an enteric pathogen responsible for amoebic dysentery and liver abscess. It alternates between the host-restricted trophozoite form and the infective environmentally-stable cyst stage. Throughout its lifecycle E. histolytica experiences stress, in part, from host immune pressure. Conversion to cysts is presumed to be a stress-response. In other systems, stress induces phosphorylation of a serine residue on eukaryotic translation initiation factor-2α (eIF2α). This inhibits eIF2α activity resulting in a general decline in protein synthesis. Genomic data reveal that E. histolytica possesses eIF2α (EheIF2α) with a conserved phosphorylatable serine at position 59 (Ser59). Thus, this pathogen may have the machinery for stress-induced translational control. To test this, we exposed cells to different stress conditions and measured the level of total and phospho-EheIF2α. Long-term serum starvation, long-term heat shock, and oxidative stress induced an increase in the level of phospho-EheIF2α, while short-term serum starvation, short-term heat shock, or glucose deprivation did not. Long-term serum starvation also caused a decrease in polyribosome abundance, which is in accordance with the observation that this condition induces phosphorylation of EheIF2α. We generated transgenic cells that overexpress wildtype EheIF2α, a non-phosphorylatable variant of eIF2α in which Ser59 was mutated to alanine (EheIF2α-S59A), or a phosphomimetic variant of eIF2α in which Ser59 was mutated to aspartic acid (EheIF2α-S59D). Consistent with the known functions of eIF2α, cells expressing wildtype or EheIF2α-S59D exhibited increased or decreased translation, respectively. Surprisingly, cells expressing EheIF2α-S59A also exhibited reduced translation. Cells expressing EheIF2α-S59D were more resistant to long-term serum starvation underscoring the significance of EheIF2α phosphorylation in managing stress. Finally, phospho-eIF2α accumulated during encystation in E. invadens, a model encystation system. In this review we critically analyze pulmonary, gastrointestinal and cardiac manifestations of childhood-onset systemic lupus erythematosus (cSLE). Clinical manifestations of these organ systems may be the initial manifestation of cSLE; frequently occur with very active cSLE; and are potential life-threatening manifestations often presenting to the emergency department and requiring admission to the intensive care unit. Early recognition and treatment of the pulmonary, gastrointestinal and cardiac manifestations of cSLE will result in improved prognosis and better outcomes. [ABSTRACT FROM AUTHOR]

We investigate Ramsey spectroscopy performed on a synchronized ensemble of two-level atoms. The synchronization is induced by the collective coupling of the atoms to a heavily damped mode of an optical cavity. We show that, in principle, with this synchronized system it is possible to observe Ramsey fringes indefinitely, even in the presence of spontaneous emission and other sources of individual-atom dephasing. The assessment and prediction of mine water rebound has become increasingly important for the gold mining industry in South Africa. The cessation of mine dewatering has led to large volumes of contaminated discharges in the western parts of the Witwatersrand Basin. Towards the eastern extremity of the Basin, the detached Evander Goldfield has been mined since the early 1950s at depths of between 400 m and 2 000 m below ground level, while overlain by shallower coal mining operations. The hydrogeology of the Evander Basin can be categorised by a shallow weathered-fractured rock aquifer comprising of the glacial and deltaic sediments of the Karoo Supergroup, while the deeper historically confined fractured bedrock aquifer consists predominantly of quartzite with subordinate lava, shale and conglomerate of the Witwatersrand Supergroup. The deep Witwatersrand aquifer has been actively dewatered for the past 60 years at a peak rate of 60 ML/cl in the mid-to-late 1960s. Modelling the impacts of mine dewatering and flooding on a regional scale as for the Evander Goldfield presents many challenges including the appropriate discretisation of mine voids and the accurate modelling of layered aquifer systems. To predict the environmental impacts of both the historic and future deep-mining operations at Shaft 6, a detailed conceptual model of the aquifer systems and a 3-dimensional model of the mine voids were incorporated into a numerical groundwater model. This model was used to simulate the dewatering and post-closure rebound of the water tables in the vicinity of the mine. This model could serve as an example for the successful modelling of mine dewatering and flooding scenarios for other parts of the Witwatersrand Basin. [ABSTRACT FROM AUTHOR]

Context. Prescription opioid abuse is a major public health concern and an ongoing epidemic in the United States. Loperamide is a widely available and inexpensive over-the-counter antidiarrheal with peripheral mu-opioid receptor activity. Online resources discuss the use of loperamide for the amelioration of withdrawal symptoms or recreational abuse. We describe the clinical course of 5 patients abusing loperamide, 3 of whom had life-threatening cardiac arrhythmias. Methods. In this observational case series, patients with cardiac arrhythmias or history of loperamide abuse with cardiac arrhythmias were identified; 5 patients were identified and 4 of the 5 patients were seen directly at the bedside. Clinical profile and outcome of patients is reported. Results. We report 5 patients with history of loperamide abuse; 3 of the 5 patients had life-threatening cardiac arrhythmias. One of the patients experienced a second life-threatening arrhythmia after he resumed loperamide abuse. Loperamide levels were obtained in 4 of the 5 patients and were at least one order of magnitude greater than therapeutic concentrations. Discontinuation of loperamide resulted in complete resolution of cardiac conduction disturbances. Conclusion. This case series describes several patients with cardiac conduction abnormalities and life-threatening ventricular arrhythmias temporally related to loperamide abuse. With the recent efforts to restrict the diversion of prescription opioids, increasing abuse of loperamide as an opioid substitute may be seen. Photo-induced conductivity transient spectroscopy (PICTS) and cathodoluminescence (CL) measurements were performed on nominally undoped detector grade samples of TlBr. In PICTS measurements, nine traps were detected in the temperature range 80-250 K using four-gate analysis. Five of the traps are tentatively identified as electron traps, and four as hole traps. CL measurements yielded two broad peaks common to all samples and most likely associated with defects. Correlations between the optically and electrically detected deep levels are considered. Above 250 K, the photoconductivity transients measured in the PICTS experiments exhibited anomalous transient behavior, indicated by non-monotonic slope variations as a function of time. The origin of the transients is under further investigation, but their presence precludes the accurate determination of trap parameters in TlBr above 250 K with traditional PICTS analysis. Their discovery was made possible by the use of a PICTS system that records whole photoconductivity transients, as opposed to reduced and processed signals. [ABSTRACT FROM PUBLISHER]

Sea ice cover in the Arctic Ocean is a continued focus of attention. This study investigates the impact of the snow overlying the sea ice in the Arctic Ocean. The impact of snow depth biases in the Community Climate System Model (CCSM) is shown to impact not only the sea ice, but also the overall Arctic climate. Following the identification of seasonal biases produced in CCSM simulations, the thermodynamic transfer through the snow-ice column is perturbed to determine model sensitivity to these biases. This study concludes that perturbations on the order of the observed biases result in modification of the annual mean conductive flux through the snow-ice column of 0.5Wm2 relative to an unmodified simulation. The results suggest that the ice has a complex response to snow characteristics, with ice of different thicknesses producing distinct reactions. Our results indicate the importance of an accurate simulation of snow on the Arctic sea ice. Consequently, future work investigating the impact of current precipitation biases and missing snow processes, such as blowing snow, densification, and seasonal changes, is warranted. Purpose: To investigate the impact of a new class of anti-Ig autoantibodies reactive with variable heavy (V) chain framework sequences (human anti-V autoantibodies) on the pharmacology and safety of an anti-TNFR1 V domain antibody (GSK1995057) in healthy human subjects. Methods: Single-blind, randomised, placebo-controlled dose escalation study in which healthy males ( n = 28) received a single GSK1995057 intravenous infusion of 0.0004, 0.002 and 0.01 mg/kg. All enrolled subjects were pre-screened for human anti-V (HAVH) autoantibody status and prospectively stratified accordingly. Serum samples from drug-naïve, HAVH-positive volunteers were used to investigate the effect of HAVH/GSK1995057 complexes on the activation of TNFR1 and cytokine release in vitro. Results: Human anti-V autoantibodies were detected in approximately 50 % of drug-naïve healthy human subjects and clinical and in vitro studies were performed to evaluate their impact on the pharmacology and safety of GSK1995057. We demonstrated that formation of HAVH autoantibody/GSK1995057 complexes activated TNFR1 and caused cytokine release in vitro in some, but not all, of the human cell types tested. When GSK1995057 was administered to healthy subjects, clinical and physiological signs of cytokine release were observed in two HAVH autoantibody-positive subjects following GSK1995057 infusion. In vitro, HAVH autoantibody levels correlated with TNFR1-dependent cytokine release and propensity for cytokine release in humans following GSK1995057 dosing. Conclusions: Our data support a greater focus on the impact of pre-existing, drug-reactive autoantibodies on the development of antibody fragments and biotherapeutics targeting cell surface receptors. The Community Earth System Model (CESM) is a flexible and extensible community tool used to investigate a diverse set of Earth system interactions across multiple time and space scales. This global coupled model significantly extends its predecessor, the Community Climate System Model, by incorporating new Earth system simulation capabilities. These comprise the ability to simulate biogeochemical cycles, including those of carbon and nitrogen, a variety of atmospheric chemistry options, the Greenland Ice Sheet, and an atmosphere that extends to the lower thermosphere. These and other new model capabilities are enabling investigations into a wide range of pressing scientific questions, providing new foresight into possible future climates and increasing our collective knowledge about the behavior and interactions of the Earth system. Simulations with numerous configurations of the CESM have been provided to phase 5 of the Coupled Model Intercomparison Project (CMIP5) and are being analyzed by the broad community of scientists. Additionally, the model source code and associated documentation are freely available to the scientific community to use for Earth system studies, making it a true community tool. Haemophilia carriers and women with inherited bleeding disorders ( IBD) experience menorrhagia, bleed following dentistry, surgery, injury or childbirth. Symptoms are easily treated leading to full and active lives. Nevertheless, some girls and women suffer with abnormal bleeding for many years before diagnosis. We explored the experiences of girls and young women (aged 9-34 years) with IBD by means of focus groups which consisted of moderated discussion addressing specific aspects of bleeding, management and coping strategies. Subsequently, these issues were explored further though a paper-based questionnaire distributed via five specialist haemophilia centres in the UK. The study suggested that young women with IBD who are managed at haemophilia centres receive appropriate care and feel well supported. Although the clinic-based literature available to these women is 'fit for purpose', it does not fully address the perceived needs specifically regarding sex, menorrhagia, conception and childbirth, the Pill, tattoos/piercings and so on, leading many to turn to other information sources. Most of those who responded to our survey are confident in their lives, able to manage their IBD and take pragmatic views towards the inherited nature of their condition. But there is a substantial subgroup of women who experience stigmatization, isolation and bullying and express concerns relating to fertility and conception. Overall, this cohort would benefit from opportunities for mutual support. This could be via Internet-based social networking and may be of particular value to those who are unable to seek help from traditional medical services due to religious or other cultural barriers. We apply state-of-art first principle calculations to study the polymorphism and electronic structure of three previously reported phases of TlBr. The calculated band structures of NaCl-structure phase and orthorhombic-structure phase have different features than that of commonly observed CsCl-structure phase. We further interpret photoluminescence spectra based on our calculations. . Clinical research should form a core component of the role of haemophilia nurse specialists. The UK Haemophilia Nurses Association sought to determine the barriers that prevent nurse specialists from engaging in research and to seek ways to promote clinical research by haemophilia nurses in the UK. Web-based survey with subsequent workshop discussion was conducted. Responses were received from 32 nurses (a 50% response rate), all of whom agreed that haemophilia nurses should be actively involved in nursing research although only 21 had actually participated in research specifically related to haemophilia practice. Of these, most research had been related to educational programmes or (less commonly) was limited to data collection as part of multidisciplinary studies. Involvement in research rarely resulted in publication. Some barriers to involvement in nursing research and subsequent publication were suggested by survey respondents. They also indentified key practice areas that warranted nurse-based research including carriership and antenatal decision-making, along with the role and impact on care of the specialist haemophilia nurse, education and empowerment. To overcome the barriers to engaging in research and publishing, nurses require dedicated research time, mentorship and collaboration with more experienced haemophilia nurse researchers. Deep levels in Se- and Pb-doped bulk TlBr detectors were characterized with photo-induced conductivity transient spectroscopy (PICTS) and cathodoluminescence (CL). Se-doped TlBr revealed two traps with energies of 0.35 and 0.45 eV in PICTS spectra. The Pb-doped material revealed three levels with energies of 0.11, 0.45, and 0.75 eV. CL measurements in both materials correlate with optical transitions involving some of the identified levels. The ambipolar carrier lifetimes of Se-doped and Pb-doped TlBr were measured with microwave reflectivity transients and found to be significantly lower than the lifetime of undoped TlBr. Access to modern treatments allows adolescents with haemophilia to manage their haemophilia at home, with improved treatment outcomes and quality of life, but has reduced peer support and the potential for experiential learning from older peers. Social networking, aided by modern communication technologies, may offer health benefits through peer support. We sought to assess whether or not disease-specific social networking could benefit adolescents with severe haemophilia. A total of 150 adolescents (aged 10-18) with severe haemophilia A or B from 11 UK treatment centres or those who had attended focus groups to explore the potential for a social network designed specifically for their use were surveyed. Teenage boys with severe haemophilia in the UK who responded to an online and paper questionnaire ( n = 47; 31% response rate) rarely knew of or socialized with others with haemophilia outside their families. Two-thirds of respondents said they would like to meet others. For 70% of boys, parents were the major source of information about haemophilia, yet more than half said they often had trouble finding answers to their questions. These boys frequently used online social networks to chat with friends. Adolescents with severe haemophilia frequently have limited contact with others and many wish to have greater contact. They may benefit from peer support and experiential learning gained through online social networking. The SixVibe restricted access social network is to be launched in 2011. Many polymer components are susceptible to catastrophic failure or are critical to the performance of the products they comprise. Because of this, the capability to monitor structural failures or performance reduction in these components is beneficial. It is difficult to fabricate sensors for polymer components because they often have complex shapes or are assembled in isolated locations. To solve this problem, micro-scale electronic sensors, embedded within polymer components, were developed at Purdue University. Conductive polymer materials were used as the primary sensing element in the sensors. Testing results reveal that embedded sensors in polymer components can successfully indicate significant signal changes more than 100 loading cycles prior to catastrophic failure. Multiple sensing methods and applications have been tested and more are being researched. The objective of this study was to assess the applicability and usefulness of the Laboratory Risk Indicator in Necrotising Fasciitis (LRINEC) score in patients from our institution. A retrospective analysis was undertaken of the case notes of all patients admitted to our facility between January 2002 and December 2005 with the admission diagnosis of necrotising fasciitis and the application of the LRINEC score upon the initial blood tests. The sensitivity, specificity and likelihood ratios were then calculated for patients with a LRINEC score of > or = 6 compared with the findings of a surgical biopsy. Twenty-eight patients were identified as having the admission diagnosis of necrotising fasciitis on the hospital database and were eligible for the study. Ten of these had biopsy-proven necrotising fasciitis. With a cut-off score of > or = 6, the LRINEC score had a sensitivity of 80%, specificity of 67%, a positive predictive value of 57% and a negative predictive value of 86% in distinguishing the patients with proven necrotising fasciitis from those with severe soft tissue infections. The likelihood ratio of a positive biopsy was 2.4 and the likelihood ratio of a negative biopsy was 0.3. A cut-off value of > or = 6 of the LRINEC score was not overly sensitive or specific, and the likelihood ratios suggest that, at this cut-off level, the LRINEC score would have only a very small effect on the post-test probability of the patients in the studied population having necrotising fasciitis. Interferometry with Heisenberg-limited phase resolution may play an important role in the next generation of atomic clocks, gravitational wave detectors and in quantum information science. For experimental implementations the robustness of the phase resolution is crucial since any experimental realization will be subject to imperfections. In this paper, we study the robustness of phase reconstruction with two number states as input subject to fluctuations in the state preparation. We find that the phase resolution is insensitive to fluctuations in the total number of particles and robust against noise in the number difference at the input. The phase resolution depends on the uncertainty in the number difference in a universal way that has a clear physical interpretation: fundamental noise due to the Heisenberg limit and noise due to state preparation imperfection contribute essentially independently to the total uncertainty in the phase. For number difference uncertainties less than one the first noise source is dominant and the phase resolution is essentially Heisenberg limited. For number difference uncertainties greater than one the noise due to state preparation imperfection is dominant and the phase resolution deteriorates linearly with the number difference uncertainty. Background and purpose:Cannabinoids are used therapeutically for the palliation of the adverse side effects associated with cancer chemotherapy. However, cannabinoids also inhibit both the activity and expression of the multidrug transporter, P-glycoprotein in vitro. Here we address the interaction of cannabinol (CBN), cannabidiol (CBD) and Δ9-tetrahydrocannabinol (THC) with the related multidrug transporter, ABCG2.Experimental approach:Cannabinoid inhibition of Abcg2/ABCG2 was assessed using flow cytometric analysis of substrate accumulation and ATPase activity assays. The cytotoxicity and chemosensitization by cannabinoids was determined with cell viability assays. Expression of cannabinoid and vanilloid receptors was assessed using reverse transcriptase polymerase chain reaction, and cannabinoid modulation of ABCG2 expression was examined using immunoblotting.Key results:CBN, CBD and THC increased the intracellular accumulation of the Abcg2/ABCG2 substrate, mitoxantrone, in an over-expressing cell line. The THC metabolite, (−)-11-nor-9-carboxy-Δ9-THC was much less potent. The plant cannabinoids inhibited both basal and substrate stimulated ATPase activity of human ABCG2. Cannabinoid cytotoxicity occurred in the absence of known cannabinoid cell surface receptors, and only at concentrations higher than those required for Abcg2/ABCG2 inhibition. Sub-toxic concentrations of the cannabinoids resensitized the overexpressing cell line to the cytotoxic effect of Abcg2/ABCG2 substrates, mitoxantrone and topotecan. This occurred in the absence of any effect on ABCG2 expression.Conclusions and implications:Cannabinoids are novel Abcg2/ABCG2 inhibitors, reversing the Abcg2-mediated multidrug-resistant phenotype in vitro. The increasing threat of emerging infectious diseases in both wildlife and humans has spurred interest in the causes of disease emergence, including the role of anthropogenic change. A prior field study of infection patterns in amphibians suggests that echinostome infection may be an emerging disease of green frogs Rana clamitans living in urbanized environments. We examined the impact of echinostome infection on green frog tadpoles at a wide range of developmental stages (Gosner stage 25–39). Echinostome infection was associated with green frog mortality rates of up to 40% in an early developmental stage, and none in later developmental stages. Tadpoles exposed to higher echinostome doses exhibited higher edema rates, a potential sign of compromised renal function. Histopathological analysis further supported the hypothesis that echinostome-induced tadpole mortality resulted from compromised renal function. Given that the timing of highest cercarial shedding can coincide with the most vulnerable stages of green frog tadpole development, echinostomes could significantly impact green frog survival in nature. Abstract: Compstatin is a peptidic complement inhibitor that prevents the cleavage of complement factor 3 (C3) by C3 convertase. Compstatin differs from other C3-regulatory proteins, such as complement receptor (CR) 1 and decay-accelerating factor (DAF), in that it binds native as well as activated C3 fragments and acts through mechanisms that do not involve the destabilization of the C3 convertase or the accelerated degradation of C3b. Compstatin''s activity most likely relies on its affinity for native C3 and the conformational change that results upon binding with C3. Although the intermolecular interactions between compstatin and C3 have been studied, the identity of the targeted region on C3 is still elusive. To address this issue, we synthesized a photo-crosslinking compstatin analog and used it to probe C3 for sites of interaction. We identified a 40-kDa region at the C-terminus of the β-chain of C3 that included the binding site of the compstatin analog. The specificity of the binding was confirmed by inhibition studies, which showed reduced crosslinking signal after pre-incubation of C3 with compstatin but not with various inactive analogs. Binding studies performed with a recombinant homolog of the 40-kDa region confirmed these findings. Five smaller recombinant proteins corresponding to various overlapping regions of the 40-kDa fragment did not bind compstatin, suggesting that a proper protein conformation, only found in larger fragments, is required for compstatin binding. The identified region on the β-chain has, thus far, not been implicated in C3 cleavage or interactions with other proteins. Therefore, further research on this part of the C3 molecule may have implications for studies on the regulation of C3 cleavage, as well as for complement-based drug design. [Copyright &y& Elsevier]

Abstract: Post-translational modifications (PTMs) of proteins produced in vivo may be tissue, developmentally and/or disease specific. PTMs impact on the stability and function of proteins and offer a challenge to the commercial production of protein biotherapeutics. We have previously reported a marked deficit in galactosylation of oligosaccharides released from polyclonal IgG isolated from sera of patients with the anti-neutrophil cytoplasmic antibodies (ANCA) associated vasculitides; Wegener''s granulomatosis (WG) and microscopic polyangiitis (MPA). Whilst normal polyclonal IgG molecules are glycosylated within the IgG-Fc region, ∼20% of molecules also bear oligosaccharides attached to the variable regions of the light or heavy chain IgG-Fab. It is of interest, therefore to compare profiles of oligosaccharides released from the IgG-Fc and IgG-Fab of normal IgG with that isolated from the sera of patients with WG or MPA. This study shows that whilst the oligosaccharides released from ANCA IgG-Fc are hypogalactosylated those released from IgG-Fab are galactosylated and sialylated. These results show that hypogalactosylation of IgG-Fc is not due to a defect in the glycosylation or processing machinery. It rather suggests a subtle change in IgG-Fc conformation that influences the addition of galactose. Remarkably, this influence is exerted on all plasma cells. Interestingly, a licensed monoclonal antibody therapeutic, produced in Sp2/0 cells, is also shown to be hypogalactosylated in its IgG-Fc but fully galactosylated in its IgG-Fab. A coupled global climate model is used to evaluate processes that determine the equilibrium location of the sea-ice edge and its climatological annual cycle. The extent to which the wintertime ice edge departs from a symmetric ring around either pole depends primarily on coastlines, ice motion, and the melt rate at the ice–ocean interface. At any location the principal drivers of the oceanic heat flux that melts sea ice are absorbed solar radiation and the convergence of heat transported by ocean currents. The distance between the ice edge and the pole and the magnitude of the ocean heat flux convergence at the ice edge are inversely related. The chief exception to this rule is in the East Greenland Current, where the ocean heat flux convergence just east of the ice edge is relatively high but ice survives due to its swift southward motion and the protection of the cold southward-flowing surface water. In regions where the ice edge extends relatively far equatorward, absorbed solar radiation is the largest component of the ocean energy budget, and the large seasonal range of insolation causes the ice edge to traverse a large distance. In contrast, at relatively high latitudes, the ocean heat flux convergence is the largest component and it has a relatively small annual range, so the ice edge traverses a much smaller distance there. When the model is subject to increased CO2 forcing up to twice preindustrial levels, the ocean heat flux convergence weakens near the ice edge in most places. This weakening reduces the heat flux from the ocean to the base of the ice and tends to offset the effects of increased radiative forcing at the ice surface, so the ice edge retreats less than it would otherwise. A characteristic feature of Wegener's granulomatosis is the presence of antineutrophil cytoplasm antibodies (ANCA) to proteinase 3 (PR3).In vitro, ANCA activate neutrophils by co-ligating PR3 and FcγRIIa/IIIb receptors. ANCA are predominantly of the IgG isotype, and IgG1, IgG3 and IgG4 subclasses are particularly represented. To address the pathogenic role of individual ANCA-IgG subclass antibodies, patients’ sera were screened using indirect immunofluorescence, enzyme-linked immunosorbent assay (ELISA) and subclass PR3-ELISA to identify patients with high titres of PR3-ANCA within the IgG1, IgG3 or IgG4 subclasses. Unfractionated ANCA-IgG and subclass fractions were isolated by affinity chromatography and compared for their capacities to stimulate superoxide production by primed human neutrophils. Donor neutrophils were analysed for constitutive and induced FcγRI expression by flow cytometry. The IgG1, IgG3 and IgG4 subclass fractions, isolated from three different ANCA sera, each stimulated superoxide production from neutrophils derived from multiple donors. Subsequently, IgG4 subclass fractions isolated from a further four ANCA positive sera demonstrated varying abilities to stimulate release of superoxide; unrelated to PR3-ANCA titre, neutrophil donor, or neutrophil FcγRI expression. The stimulation of superoxide release by IgG1- and IgG3-ANCA subclass fractions is consistent with the proposed mechanism of co-ligation of PR3 antigen and FcγRIIa/IIIb receptors. However, the demonstration of similar activity for the IgG4-ANCA subclass fractions isolated from some sera was unexpected. This activity was independent of neutrophil donor and expression of FcγRI, suggesting it was capable of activating neutrophils via constitutively expressed FcγRIIa/IIIb or co-ligation of other, unidentified, cell surface molecules. The Northern Hemisphere polar amplification of climate change is documented in models taking part in the Coupled Model Intercomparison Project and in the new version of the Community Climate System Model. In particular, the magnitude, spatial distribution, and seasonality of the surface warming in the Arctic is examined and compared among the models. The range of simulated polar warming in the Arctic is from 1.5 to 4.5 times the global mean warming. While ice-albedo feedback is likely to account for much of the polar amplification, the strength of the feedback depends on numerous physical processes and parametrizations which differ considerably among the models. Nonetheless, the mean sea-ice state in the control (or present) climate is found to influence both the magnitude and spatial distribution of the high-latitude warming in the models. In particular, the latitude of the maximum warming is correlated inversely and significantly with sea-ice extent in the control climate. Additionally, models with relatively thin Arctic ice cover in the control climate tend to have higher polar amplification. An intercomparison of model results also shows that increases in poleward ocean heat transport at high latitudes and increases in polar cloud cover are significantly correlated to amplified Arctic warming. This suggests that these changes in the climate state may modify polar amplification. The effects of different heat treatment schemes (i.e. successively or directly heated to particular temperatures) and atmospheres (air or nitrogen) on the solid-state NMR spectra obtained from (TiO2)0.15(ZrO2)0.05(SiO2)0.80 sol–gel materials are investigated. A combination of 1H, 13C, 17O and 29Si NMR is used. 29Si MAS NMR indicates that the extent of condensation of the silica-based network strongly depends on the maximum temperature the sample has experienced, but the condensation is largely independent of the details of the heat treatment scheme and atmosphere used. For sol–gel produced silicate-based materials the results show that the equilibrium structure at each temperature is reached rapidly compared to the time (2 h) spent at that temperature. The 17O NMR results confirm that a nitrogen atmosphere does significantly reduce loss of 17O from the structure but care must be taken since there could be differential loss of 17O from the regions having different local structural characteristics. The present study was conducted to test the responsiveness of the juvenile male reproductive axis to hormonal stimulation and to compare it to that of early maturing males. Long-term treatments with various combinations of T, GnRHa and pimozide did not result in an increased incidence of early maturing males, but did stimulate spermatogenesis slightly in juvenile fish. In early maturing males, the treatments appeared to be inhibitory since they resulted in a reduction of the GSI and a lower incidence of spermiating males. In early maturing males, pituitary LH content was elevated by GnRHa treatments alone while in juvenile males a combination of T and GnRHa was needed to increase the levels of LH in the pituitary. Thus, T may play an important role during puberty by potentiating the effects of GnRH on LH synthesis. In both juvenile and early maturing males, plasma LH levels could be increased only by high doses of GnRHa (in combination with T). Therefore, LH synthesis and release probably require different levels of GnRH stimulation. A GnRH challenge (single injection of 50 μg GnRHa/kg) at the end of the experiment resulted in a dramatic elevation of plasma LH levels in almost all animals. This finding demonstrates that pituitaries from juvenile and early maturing males were responsive to GnRHa stimulation, even after long-term hormonal treatments. The addition of pimozide did not affect the T- and GnRHa-induced increase in pituitary LH content but inhibited the release of LH in response to a GnRHa challenge. In conclusion, high doses of GnRHa in combination with T can increase plasma LH levels in juvenile males but do not induce complete testicular maturation. SUMMARY The triad of small vessel vasculitides (SVV) comprise Wegener's granulomatosis (WG), microscopic polyangiitis (MPA) and Churg–Strauss syndrome (CS). All three are associated with presence of circulating IgG antineutrophil cytoplasm antibodies (ANCA) which target autoantigens contained, primarily, within neutrophil azurophilic granules. The widely accepted model of pathogenesis suggests that ANCA activate cytokine-primed neutrophils within the microvasculature, leading to by-stander damage to endothelial cells, and rapid escalation of inflammation with recruitment of mononuclear cells. Activation may be initiated, in vitro , by the coligation of the PR3 or MPO antigen, translocated to the cell surface, and FcγRIIa/FcγRIIIb receptors. This suggests that the IgG subclass profile of ANCA and, possibly, its glycosylation status could influence the inflammatory mechanisms activated. The glycosylation status of total IgG isolated from the sera of patients with WG (13), MPA (6) and CSS (1) was determined by analysis of the released oligosaccharides. A deficit in IgG galactosylation is demonstrated for all patient samples, compared to controls. The mean percentage values for the agalactosylated (G0) oligosaccharides were 57% (SD ± 9·71), 47% (SD ± 4·25) and 28% (SD ± 4·09) for WG, MPO and control samples, respectively. The G0 levels for polyclonal IgG isolated from the sera of both WG and MPA patients were significantly increased compared to controls (P < 0·0001). The major glycoform present therefore is agalactosylated (G0) IgG. In previous studies the G0 glycoform of IgG has been shown to bind and activate mannan binding lectin, and hence to activate the complement cascade, and to facilitate mannose receptor binding and the uptake of IgG complexes by macrophages and dendritic cells. Complement, an important component of the innate immune system, is comprised of about 35 individual proteins. In mammals, activation of complement results in the generation of activated protein fragments that play a role in microbial killing, phagocytosis, inflammatory reactions, immune complex clearance, and antibody production. Fish appear to possess activation pathways similar to those in mammals, and the fish complement proteins identified thus far show many homologies to their mammalian counterparts. Because information about complement proteins, regulatory proteins, and complement receptors in fish is far from complete, it is unclear whether all the complement functions that have been identified in mammals also occur in fish. However, it has been clearly demonstrated that fish complement can lyse foreign cells and opsonise foreign organisms for destruction by phagocytes. There are also indications that complement fragments participate in inflammatory reactions. Fish possess multiple isoforms of several complement proteins, such as C3 and factor B. It has been hypothesised that the function of this diversity in complement proteins serves to expand their innate immune recognition capacity and response. Understanding the functions of complement in fish and the roles the individual proteins, including the various isoforms, play in host defence, is important not only for understanding the evolution of this system but also for the development of new strategies in fish health management. The most salient features of the Bose-Einstein condensation of a magnetically confined alkali vapor is the diluteness of the gas and the extremely weak effective interactions. From a theoretical point of view, the interesting aspect is the potential formulation of the many-body quantum theory for a non-uniform and potentially non-equilibrium system founded entirely on microscopic physics. The crucial postulate is the rapid attenuation of many particle quantum correlations in the dilute system which can be motivated from universal considerations. In principle, it will be possible to provide direct comparison between theory and experiment over all temperature scales with no phenomenological parameters—a challenge facing the theoretical community in the near future. The dilute gas experiments provide an exciting stage on which to build bridges linking the theory of complex and collective phenomena in superconducting and superfluid systems, with the single particle microscopic physics described in quantum optics and laser physics.
