Resume Objectif Evaluation clinique de la keratoprothese Alphacor™ chez les patients a haut risque de rejet d’allogreffe de cornee. Design Etude retrospective. Patients et methode L’implantation de l’Alphacor™ a ete realisee selon une technique chirurgicale en deux phases avec une insertion intrastromale puis une exposition secondaire de l’optique apres un intervalle de six mois. Les parametres evalues etaient l’acuite visuelle et la survenue de complications postoperatoires. Resultats Quatorze implantations ont ete realisees. Le suivi moyen etait de 15,6 ± 5,6 mois (de deux a 24 mois). Le gain d’acuite visuelle moyen etait de 2,5 ± 3,1 lignes (de 0 a +11 lignes). Vingt et un pour cent des patients presentaient une acuite superieure ou egale a 1/10. Sept patients (50 %) ont presente une necrose stromale dont un cas (7,1 %) d’extrusion definitive de la prothese, trois cas (21,4 %) ont necessite la realisation d’une keratoplastie transfixiante architectonique et chez trois patients, une greffe lamellaire a ete realisee. Le taux de retention a l’issue du suivi etait de 71,4 %. Trois patients (21,4 %) ont developpe une membrane retroprothetique. Un patient (7,1 %) a developpe une endophtalmie tardive. Conclusion L’Alphacor™ est une alternative therapeutique chez les patients presentant une cecite corneenne et avec une faible probabilite de survie d’une allogreffe de cornee. Il est capital de prendre en consideration au cours de l’evaluation preoperatoire, les principaux facteurs limitant de cette keratoprothese dans ses indications et des etudes complementaires sont necessaires afin de preciser et determiner la technique chirurgicale optimale d’implantation ainsi que l’efficacite des traitements postoperatoires.

7. Nouvelle approche thérapeutique du kératocône par segments d’anneaux intracornéens assistés par laser femtoseconde (Tecnolas Perfect Vision®) : indications, technique opératoire et résultats

Resume But Nous rapportons les premiers resultats de l’utilisation des segments d’anneaux intracorneens (Intacs™) assistes par laser femtoseconde Tecnolas 20/10 Perfect Vision® (Tecnolas Perfect Vision®, Heidelberg, Allemagne). Patients et methodes Il s’agit d’une etude retrospective. Tous les yeux ont ete operes a l’aide du laser femtoseconde Tecnolas Perfect Vision®. Nous avons evalue en pre- et postoperatoire : l’acuite visuelle non corrigee (AVNC), la meilleure acuite visuelle corrigee (MAVC), la topographie corneenne speculaire, l’analyse aberrometrique et la pachymetrie corneenne centrale. Les donnees ont ete analysees grâce au test de Wilcoxon. Resultats Dix-sept yeux de 15 patients ont ete inclus. Le suivi moyen a ete de 6,9 ± 0,71 mois. L’AVNC et la MAVC ont augmente significativement dans 82,35 % (p = 0,027) et 94,12 % (p = 0,003) des cas respectivement. La sphere et le cylindre subjectif ont diminue significativement de −10,29 ± 5,54 a −6,5 ± 5,03 D (p = 0,002) et de 5,43 ± 3,20 a 3,69 ± 2,30 D (p = 0,026) respectivement. L’indice d’asymetrie de surface (SAI) a diminue de facon significative (p = 0,011). Conclusion Le traitement des ectasies corneennes par implantation d’anneaux intracorneens assistee par laser femtoseconde est une technique efficace. La poursuite de l’optimisation des parametres chirurgicaux devrait permettre de poursuivre l’amelioration des resultats.

Resume Introduction L’imagerie par coherence optique est une technologie employee depuis de nombreuses annees dans le domaine de l’imagerie du segment anterieur. Jusqu’a present, il s’agissait d’une technologie « Time Domain » qui permettait d’atteindre une definition des structures oculaires proche de celle de l’histologie. Actuellement, avec l’apparition d’une nouvelle generation d’appareil utilisant le domaine spectral, il est possible de realiser des images en tres haute definition dont la precision est inegalee. Materiel et methode Nous avons utilise un 3D OCT-1000® (Topcon, Tokyo, Japon) parametre afin d’obtenir des images en haute definition et en trois dimensions des structures irido-corneennes. Resultats Nous presentons un recueil d’images pouvant etre obtenues par cette technique. Discussion A partir d’un outil initialement exclusivement dedie a l’imagerie retinienne et du glaucome, il est possible d’obtenir une analyse du segment anterieur d’excellente qualite aussi bien en haute definition qu’en trois dimensions, ceci augmentant par la meme l’interet d’acquerir un tel appareil du fait de sa grande polyvalence. Conclusion L’interet du 3D OCT-1000® pour l’analyse du segment anterieur ne doit pas etre neglige par les utilisateurs de cet appareil qui a deja fait ses preuves pour l’analyse du segment posterieur et qui, grâce a quelques parametrages, permet de realiser des images de qualite et en trois dimensions du segment anterieur, et ce en une seule acquisition.

9. Incisions arciformes au laser femtoseconde pour correction de l’astigmatisme après kératoplastie transfixiante

Resume But Evaluer l’efficacite du laser femtoseconde pour la realisation de keratotomies arciformes afin de corriger un astigmatisme apres greffe de cornee. Methode Onze yeux ont ete inclus dans cette etude retrospective. Tous les yeux ont beneficie de deux keratotomies arciformes realisees a l’aide d’un laser femtoseconde. L’acuite visuelle, la densite cellulaire endotheliale et l’astigmatisme refractif, keratometrique et topographique ont ete evalues en pre- et postoperatoire. La profondeur des incisions a ete mesuree par OCT-3. Resultats Le suivi postoperatoire moyen etait de 7,4 ± 6,7 mois. L’acuite visuelle non corrigee n’etait pas modifiee alors qu’un gain significatif ( p = 0,007 ) de 1,68 ± 1,59 ligne de meilleure acuite visuelle corrigee etait observe. L’astigmatisme subjectif moyen preoperatoire etait de 5,18 ± 1,15 D et de 3,41 ± 1,93 D ( p = 0,045 ) apres keratotomies arciformes. L’astigmatisme keratometrique (7,79 ± 3,69 D) et topographique (7,98 ± 2,41 D) preoperatoires atteignaient 4,81 ± 2,52 D ( p = 0,021 ) et 4,36 ± 2,59 D ( p = 0,005 ) respectivement apres chirurgie. La densite endotheliale n’etait pas modifiee. On constatait un decalage moyen entre la profondeur prevue et mesuree des incisions de 10,7 ± 63,5 μm vers la surface corneenne. Aucune complication n’etait observee. Discussion Les keratotomies arciformes realisees a l’aide d’un laser femtoseconde sont efficaces pour la correction des astigmatismes post-keratoplastie, ameliorent la securite et la precision chirurgicale. L’efficacite pourra etre amelioree par le developpement de nomogrammes de traitement adaptes. Conclusion Les keratotomies arciformes assistees par laser femtoseconde sont efficaces, sures et de realisation aisee pour la correction des astigmatismes post-keratoplastie. Cependant, il sera necessaire de completer notre serie afin d’ajuster les parametres de traitement et d’optimiser la reduction de l’astigmatisme.

Conrath Purpose: Optical coherence tomography has been used for many years, in particular in the field of the retinal imaging, and is a promising technique in the analysis of the anterior segment, providing a similar resolution to histology for ocular tissue. Material and methods: We present a collection of images obtained with the OCT3 (Carl Zeiss Meditec, Dublin, CA), with parameters set to obtain good-quality images of the iris and corneal structures. Results: The OCT provides detailed analysis of the cornea, precise assessment of the dimensions and locations of corneal lesions – in the anterior-posterior plane as well as transversally or obliquely – with 10-µm precision, and the study of the iridocorneal angle and its dynamics. Discussion: Using the highly precise OCT3 for retinal imaging, it is possible to obtain good-quality anterior segment imaging, which makes the device highly versatile and thus increases reasons for acquiring this type of apparatus. Conclusions: OCT3 users should not neglect this potential of the device, already in wide use for examining the retina.

11. Kératoplastie lamellaire antérieure assistée par laser femtoseconde

Introduction Les keratoplasties lamellaires anterieures sont de realisation delicate et l’utilisation du laser femtoseconde dans cette indication doit permettre d’en ameliorer les resultats anatomiques et fonctionnels. Nous rapportons le premier cas de keratoplastie lamellaire anterieure assistee par un laser femtoseconde. Observation Il s’agit d’une patiente âgee de 63 ans, presentant une dystrophie corneenne anterieure de Francois, traitee par keratoplastie lamellaire anterieure assistee par laser femtoseconde ( Femtec 20/10 Perfect Vision ® ). Une decoupe corneenne lamellaire par laser femtoseconde a ete realisee a 400 μm de profondeur sur un greffon corneen, positionne sur une chambre anterieure artificielle ( Moria ® , France ), puis sur la cornee receveuse du patient. Resultats Au cours de la periode postoperatoire, le lenticule corneen etait parfaitement adapte au lit receveur avec une interface claire. L’evaluation en OCT III (Carl Zeiss Meditech, Dublin) a permis de constater une pachymetrie centrale de 713 μm avec un lit receveur extremement regulier et une epaisseur centrale de 132 μm. Discussion Les decoupes corneennes assistees par laser femtoseconde doivent en comparaison avec les decoupes mecanisees ameliorer la securite operatoire, la reproductibilite, et la predictibilite des decoupes obtenues. Une meilleure congruence du greffon limite le risque de deplacement secondaire et la meilleure regularite de l’interface doit ameliorer le resultat fonctionnel. Conclusion Les keratoplasties lamellaires anterieures assistees par laser femtoseconde Femtec 20/10 Perfectvision ® presentent de nombreux avantages pour la prise en charge des pathologies corneennes necessitant une keratoplastie lamellaire anterieure. Des etudes comparatives, multicentriques, et randomisees sont necessaires afin d’evaluer les resultats de cette technique.

12. Formation à la phacosection en milieu tropical

But L’objectif de cette etude est l’evaluation prospective de la formation a la technique de phacoexerese par phacosection d’un ophtalmologiste malgache maitrisant deja l’extraction extracapsulaire. Materiel et methode Dans le cadre du programme Sight First de lutte contre la cecite a Madagascar, l’ophtalmologiste de Majunga a selectionne des patients atteints de cataracte blanche totale. Ces derniers ont ete operes par phacosection en utilisant le materiel habituel de l’hopital local, auquel etaient ajoutes d’une part, les deux instruments specifiques de la phacosection et d’autre part, du materiel jetable supplementaire (couteaux de 3,2 mm et produits visco-elastiques). L’anesthesie etait peribulbaire avec oculo-pression. Les controles post operatoires (acuite visuelle et etat corneen) etaient effectues a J1, J7 et J30. Resultats Quarante-quatre cataractes ont ete operees. Les sept premieres ont ete realisees par le formateur aide par le stagiaire et les vingt-six suivantes par le stagiaire aide par le formateur. Nous avons note cinq complications peroperatoires : deux ruptures capsulaires posterieures (dont une au cours de l’implantation) et trois cataractes ayant necessite une reprise operatoire avec lavage de masses residuelles. En postoperatoire, nous retrouvons peu d’astigmatisme induit, une bonne recuperation visuelle et a J30 toutes les cornees limpides. Discussion Les resultats sont tout a fait acceptables pour cette periode d’apprentissage, avec seulement deux complications notables (dont une non liee a la technique operatoire), et une bonne rehabilitation visuelle en moins de quinze jours. Cette technique par petite incision a permis d’une part, de reduire l’astigmatisme induit, et d’autre part, de traiter sans risque des cataractes blanches totales (quel que soit l’operateur). Conclusion La petite incision, grâce a la phacosection, est donc possible pour les cataractes difficiles en milieu tropical avec un cout additionnel faible et en toute securite. De plus, la formation d’un chirurgien maitrisant l’extraction extracapsulaire semble rapide et sans risque pour les patients.

13. Décompensation endothéliale bilatérale après coma post septicémie à staphylocoque

Nous rapportons le cas d’un homme, âge de 71 ans, ayant presente une decompensation endotheliale bilaterale de cause inexpliquee au reveil d’un coma. Ce coma etait du a une septicemie a staphylocoque a point de depart cutanee suite a la pose d’un catheter en vue de la realisation d’un scanner. Le patient n’avait pas d’antecedents medicaux, ni de pathologie ophtalmologique preexistante. L’acuite visuelle au reveil etait de 1/20e P20 a l’œil gauche et 2/10e P8 a l’œil droit. A la lampe a fente, etaient notes un epaississement stromal et des plis de la membrane de Descemet responsables d’un flou corneen bilateral predominant a gauche. Le reste de l’examen ophtalmologique et notamment le fond d’œil etaient normaux. Une keratoplastie transfixiante a l’œil gauche combinee a une chirurgie de la cataracte fut effectuee. L’acuite visuelle a 6 mois postoperatoire etait de 9/10e P2. Cette decompensation endotheliale bilaterale reste pour l’instant inexpliquee avec certitude. Toutefois, en dehors d’une decompensation d’une pathologie infraclinique preexistante au coma et compte tenu de la toxicite connue chez l’animal de l’oxacilline (bristopen®), l’hypothese d’une toxicite corneenne iatrogene est la plus probable.

14. Interest of endosaver® using on endothelial graft

Purpose To describe characteristics of a delivery device of endothelial graft using in endothelial keratoplasty procedure (Descemet Stripping Endothelial Keraplasty, DSAEK) and to report clinical results. Methods 20 patients are including in our prospective non-randomised study. Patients were benefited of a DSAEK by an experienced surgeon. The dissection of the endothelial graft were realised after removal epithelium and cutting a cornea anterior hat of 350μm by a microkeratom (Moria France). Grafts are inserted by the Endosaver® (Ocular System Inc USA) by a 4 mm incision and applied by a buffering of a air bubble in the anterior segment after a peripheral iridotomy. Authors reports experiences with this device and the density of endothelial cells after 6 month. Results Any failure of the graft was reported. The diminution of endothelial cells was correlated to the thickness of the graft and situated between 26% and 31% depending on pachymetry (50-120 μm and 120-150μm).49% of patients had more than 20/40 at 6 month. Conclusion our study showed the interest of Endosaver® on endothelial grafts to reduce the incision and the diminution of endothelial cells on DSAEK.

Clinical evaluation of Alphacor keratoprosthesis in patients at high risk of corneal allograft rejection.Retrospective case series.Alphacor implantation was performed via a two-step procedure with intrastromal insertion followed by secondary exposure of the optic after 6 months. Visual acuity and occurrence of postoperative complications were evaluated.Fourteen eyes of 14 patients underwent Alphacor keratoprosthesis implantation. Mean follow-up was 15.6 ± 5.6 months (from 2 to 24 months). Postoperative mean visual acuity gain was 2.5 ± 3.1 lines (from 0 to +11 lines). Visual acuity was superior or equal to 20/200 in 21% of cases. Seven cases of stromal melt (50%) occurred, of which one case (7.1%) experienced spontaneous extrusion of the implant, three cases (21.4%) required tectonic penetrating keratoplasty, and three patients underwent lamellar keratoplasty on top of the implant. Three cases (21.4%) of retroprosthetic membrane were observed and successfully managed. One patient (7.1%) developed late endophthalmitis.Alphacor is an alternative to corneal allograft in cases of corneal blindness at high risk of allograft failure. Throughout the preoperative evaluation, it is imperative to take into account the limitations of this keratoprosthesis in terms of indications, and additional studies are necessary in order to determine and refine the optimal surgical technique for implantation as well as the efficacy of postoperative treatments.

Purpose To assess the use of a new polygonal trephination pattern for penetrating keratoplasty (PK) assisted by femtosecond laser. Design Prospective, nonrandomized clinical study. Methods Sixteen eyes underwent decagonal PK. Nine had Fuchs dystrophy, 4 had pseudophakic bullous keratopathy, 1 had experienced trauma, 1 had corneal amyloidosis, and 1 had keratoconus. A Femtec (Tecnolas PerfectVision) laser was used to create decagonal penetrating cuts on both donor and recipient corneas. All patients were evaluated for uncorrected visual acuity, best spectacle-corrected visual acuity, pachymetry, topography, and endothelial cell density. Scanning electron microscopy was performed on corneal tissue after surgery. Results All eyes were treated successfully without intraoperative complications. The mean follow-up ± standard deviation was 9.75 ± 3.5 months. Mean postoperative best spectacle-corrected visual acuity was 20/53, and there was a significant improvement in both uncorrected visual acuity ( P = .0019) and best spectacle-corrected visual acuity ( P = .001). At 6 months, mean ± standard deviation manifest astigmatism was 1.90 ± 1.20 diopters. Mean endothelial cell density was 1502 ± 458 cells/mm 2 . Scanning electron microscopy displayed straight decagonal cut margins and minor remaining tissue bridges. Conclusions Use of the decagonal trephination profile was effective and safe to perform PK. Short-term visual results and refractive results are encouraging compared with those of conventional PK studies. Longer-term follow-up and comparative studies are necessary to determine precisely advantages the and optimal surgical settings of this technique.

To assess the results of intracorneal ring segment implantation assisted by the Tecnolas Perfect Vision(®) femtosecond laser (Heidelberg, Germany).Retrospective observational study.The patients were operated on with the Tecnolas Perfect Vision(®) femtosecond laser. The following parameters were evaluated before and after surgery: uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), corneal topography, wavefront analysis, and central corneal thickness. The Wilcoxon test was used for statistical analysis.Seventeen eyes of 15 patients were included. The mean follow-up was 6.9±0.71 months. Significant improvement of UCVA and BCVA was observed in 82.35% (P=0.027) and 94.12% (P=0.003) of the eyes, respectively. The subjective refraction showed a significant decrease of the sphere from -10.29±5.54 to -6.5±5.03D (P=0.002). The subjective cylinder significantly decreased from 5.43±3.20 to 3.69±2.30D (P=0.026). The SAI also decreased significantly (P=0.011).Femtosecond-assisted intracorneal ring segment implantation is an effective treatment for corneal ectasia. Further studies are needed to improve surgical parameters and outcomes.

Anterior segment imaging using optical coherence tomography (OCT) time domain technology has been used for many years. When it appeared, it was a promising technique in the analysis of the anterior segment, making it possible to reach a definition of the ocular structures comparable with histology. Now with new-generation OCT, it is possible to perform high-definition and three-dimensional imaging.A 3D OCT-1000 (Topcon, Tokyo, Japan) parameterized to obtain high-definition and 3D imaging of the iridocorneal structures.We present a collection of images obtained using this technique.Based on a very sophisticated tool for retinal and glaucoma imagery, it is now possible to obtain very-high-quality imaging of the anterior segment, which, with its great versatility, increases the value of acquiring this type of device.The potential of 3D OCT-1000, already a very good exam for the posterior segment, and the only one to perform anterior segment three-dimensional imaging in a single acquisition, should not be neglected.

Evaluation of the effectiveness of arcuate keratotomy performed with femtosecond laser for correction of postkeratoplasty astigmatism.This retrospective clinical study included 11 eyes. All cases underwent arcuate keratotomy using femtosecond laser. Outcome measures included visual acuity and endothelial cell density as well as refractive, keratometric, and topographic astigmatism. The incision depth was also evaluated by OCT-3.With a mean follow-up of 7.4+/-6.7 months, uncorrected visual acuity was not modified and the mean best corrected visual acuity significantly improved from 1.68+/-1.59 lines (p=0.007). The mean preoperative refractive cylinder was 5.18+/-1.15D, decreasing to 3.41+/-1.93D (p=0.045) after laser-arcuate keratotomy. The reduction of preoperative keratometric (7.79+/-3.69D) and topographic (7.98+/-2.41D) astigmatism was higher, with a decrease to 4.81+/-2.52D (p=0.021) and 4.36+/-2.59D (p=0.005) postoperatively. Endothelial cell density was not modified after surgery. The difference between achieved and planned incision depth was -10.7+/-63.5mum. All cases were uncomplicated.Arcuate keratotomies performed with the femtosecond laser were effective in reducing postkeratoplasty astigmatism and has a number of advantages over conventional techniques. However, efficacy could be improved by a more accurate nomogram.Arcuate keratotomy performed with femtosecond laser is a relatively easy, safe, and effective means of treating postkeratoplasty astigmatism. Given our small sample, much larger series are needed to provide more confident estimates of astigmatism reduction proportions and to adjust correction parameters.

Optical coherence tomography has been used for many years, in particular in the field of the retinal imaging, and is a promising technique in the analysis of the anterior segment, providing a similar resolution to histology for ocular tissue.We present a collection of images obtained with the OCT3 (Carl Zeiss Meditec, Dublin, CA), with parameters set to obtain good-quality images of the iris and corneal structures.The OCT provides detailed analysis of the cornea, precise assessment of the dimensions and locations of corneal lesions - in the anterior-posterior plane as well as transversally or obliquely - with 10-microm precision, and the study of the iridocorneal angle and its dynamics.Using the highly precise OCT3 for retinal imaging, it is possible to obtain good-quality anterior segment imaging, which makes the device highly versatile and thus increases reasons for acquiring this type of apparatus.OCT3 users should not neglect this potential of the device, already in wide use for examining the retina.

Purpose To compare the effectiveness of arcuate keratotomy (AK) performed with a femtosecond laser (FSL) or Hanna keratome (Moria, Anthony, France) for correction of postkeratoplasty astigmatism. Design Prospective, randomized study. Methods This clinical study included 20 eyes. Two groups of 10 eyes underwent AK using an FLS or keratome. Refractive and keratometric astigmatism were evaluated before surgery and 6 months after surgery. The astigmatic changes in the 2 groups were measured through arithmetic and vector analysis (Alpins method). Results Six months after surgery, the mean uncorrected and corrected visual acuities did not change significantly. The mean preoperative refractive cylinder was 8.6 ± 3.0 diopters (D) and 6.7 ± 2.1 D, decreasing to 3.9 ± 2.4 D and 4.7 ± 2.4 D after laser AK and mechanized AK, respectively. The mean arithmetic change was significantly higher after laser AK, with a decrease of −55.4 ± 20.7% ( P = .011). Vector analysis showed a systematic undercorrection of astigmatism in both groups with a refractive correction index of 0.82 and 0.90 after laser AK and mechanized AK, respectively. Although no statistically significant differences were detected, a wider spread of angle of error and an almost significant difference of mean absolute angle of error ( P = .052) suggest a larger misalignment of treatment during mechanized AK. All cases were uncomplicated after laser AK, 1 microperforation occurred and 1 case with off-center incisions occurred after mechanized AK. Conclusions AK performed with the femtosecond laser was effective in reducing postkeratoplasty astigmatism and has some advantages over conventional techniques. However, efficacy could be improved by a more accurate nomogram and alignment of treatment.

Purpose To evaluate the use of the Femtec femtosecond (fs) laser for penetrating keratoplasty (PK) in the treatment of corneal diseases. Design Prospective, nonrandomized clinical study. Methods Nine eyes of nine patients underwent surgery for PK. Five had pseudophakic bullous keratopathy, three had Fuchs dystrophy, and one presented in a keratoconus patient. A Femtec (20/10 PerfectVision; GmbH, Heidelberg, Germany) fs laser was used to create penetrating cuts on donor and recipient corneas. All patients were evaluated for uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), pachymetry, topography, and endothelial cell density (ECD). Scanning electron microscopy (SEM) was performed on corneal tissue after surgery. Results All eyes were treated successfully without intraoperative complications. The mean follow-up was 6 ± 3 months. At the last postoperative examination mean BSCVA was 20/69 and there was a significant improvement ( P = .08) in both UCVA and BSCVA. Mean astigmatism was 2.9 ± 1.2 diopters. Mean ECD was 1194 ± 465 cells/mm 2 with a mean cell loss after surgery of 49.8% ± 19.8%. SEM displayed smooth rectilinear cut margins and minor remaining tissue bridges. One patient presented a retinal detachment three months after surgery that was successfully treated and two subjects showed an allograft rejection. Conclusion Use of the Femtec fs laser was effective and safe to perform PK. Short-term visual results and refractive results are analogous to conventional PK or other fs laser–assisted PK studies. Longer-term follow-up of additional cases is necessary to precisely quantify the endothelial cell loss after fs surgery.

We report a case of unexplained bilateral corneal endothelial decompensation after a coma. A 71-year-old man with no medical history presented with bilateral endothelial decompensation that required penetrating keratoplasty of the left eye combined with cataract surgery. This coma was caused by septicemia originally due to staphylococcus infection following catheter placement in preparation for a CT scan. Visual acuity of the left eye was 20/400 and 20/100 in the right eye when the patient awoke from the coma. After examination, we noted only stromal thickening and Descemet membrane folds causing corneal edema predominating OS. The rest of the exam was normal. Six months after surgery, visual acuity improved to 20/25. The most probable physiopathological mechanism of this decompensation is an iatrogenic complication from drugs administered during the patient's stay in intensive care (oxacillin), but we cannot rule out direct aggression of a bacterium or its toxin or the decompensation of a preexisting pathology.

The aim of this study was to prospectively evaluate the training of a Malagasy ophthalmologist, already proficient in classic manual extracapsular cataract extraction (ECCE), in a small incision manual technique (phacosection).Within the activity of the Sight First program to fight blindness in Madagascar, the ophthalmologist of Majunga recruited patients presenting total white cataracts. These patients were operated by phacosection using the local hospital's usual instrumentation along with two specific phacosection instruments and extra single-use material (a precalibrated 3.2 mm knife and viscoelastic products). Peribulbar anesthesia with eye pressure was provided. Postoperative follow-up (visual acuity and corneal status) was conducted on days 1, 7 and 30.Forty-four eyes were operated, the first seven by the instructor assisted by the local ophthalmologist, the next 36 by the local ophthalmologist assisted by the instructor. Five postoperative complications were noted: two cases of posterior capsular rupture (one during implantation) and three cases needing reoperation with aspiration of residual cortical masses. Induced astigmatism was low and visual rehabilitation was good, with all corneas clear at day 30.The results are quite acceptable given the training period, with only two marked complications (one not related to the operative technique) and good visual rehabilitation in less than 2 weeks. This small incision technique allowed reduction of induced astigmatism and risk-free management of total white cataracts (independent of operator).Small incision manual ECCE by phacosection is a safe, possible management option for difficult cataracts in a tropical setting with only a small increase in cost. Moreover, the training of an ophthalmologist mastering classic manual ECCE appears rapid and risk-free for patients.

Objectif Determination des parametres d’incision et resultats cliniques des keratoplasties transfixiantes realisees au laser femtoseconde Femtec 20/10. Materiels et Methodes Onze yeux de 10 patients (8 hommes et 2 femmes), avec un âge moyen de 62 ± 25,9 ans (de 28 a 83 ans) ont beneficie d’une keratoplastie. Les indications chirurgicales concernaient des dystrophies du pseudophaque (45 %), des dystrophies endotheliales de Fuchs (27 %), des taies post-traumatiques (18 %) et un cas de keratocone (10 %). La trepanation corneenne a ete realisee a l’aide d’un laser femtoseconde de 40 kHz, Femtec 20/10. Le laser a ete programme pour obtenir un diametre pour le donneur de 8,2 a 8,4 mm et pour le receveur de 8 a 8,2 mm. L’angulation des bords etait de 90 degres. L’energie du faisceau laser etait fixee entre 3200 et 3400 nJ. Le greffon corneen etait dispose sur une chambre anterieure artificielle et la procedure de decoupe effectuee selon les parametres precedents puis suture chez le receveur par une combinaison de 8 points separes et d’un surjet antitorsion. L’ablation selective des sutures etait pratiquee si l’astigmatisme corneen etait superieur a 3 dioptries (3D). Resultats L’acuite visuelle moyenne post-operatoire etait de 4/10 ± 7.9 Lignes et l’astigmatisme subjectif moyen de 4.3 ± 1.7 D. La pachymetrie centrale moyenne etait de 559 μ et la perte endotheliale moyenne de 29 %. Pour analyser la qualite de la coupe, la microscopie electronique a balayage sur la cornee du receveur et sur l’anneau donneur a revele une decoupe rectiligne, lisse, sans anomalies. Discussion Dans notre serie de cas, l’utilisation du laser femtoseconde Femtec 20/10 pour les keratoplasties transfixiantes est efficace et sure. Conclusion Les premiers resultats visuels post-operatoires des keratoplasties transfixiantes au laser femtoseconde Femtec 20/10 et la perte en cellules endotheliales sont similaires a la trepanation mecanique. Des etudes complementaires sont necessaires pour optimiser les parametres chirurgicaux.

The authors examined the clinicopathologic features of a 4-year-old boy with a diffuse choroidal hemangioma, yet without any features of the Sturge-Weber syndrome. The tumor occurred as a grayish intraocular mass with an overlying retinal detachment and was highly reflective in the ultrasonic examination. After contrast, the CT scan revealed diffuse thickening of the ocular wall associated with an enhanced irregular orbital mass. The pathologic examination revealed a cavernous hemangioma involving the entire choroid with extrascleral extension. Diffuse choroidal hemangioma occurring in childhood and unrelated to the Sturge-Weber syndrome is a rare condition that should be included in the differential diagnosis of retinoblastoma.

PURPOSE To assess corneal epithelial cell survival after keratoplasty. METHODS Corneal impression cytology (CIC) was performed on sex-mismatched corneal transplants. Fluorescent in situ hybridization (FISH) with sex chromosome-specific probes was performed to identify epithelial cell mosaicism and therefore allocate the donor or recipient origin of the cells. Twenty-four samples of corneal epithelial cells derived from 21 transplanted patients were analyzed. All patients received post-operative treatment using dexamethasone eye drops, with progressive tapering over 18 months, and nine patients also received 2% cyclosporine eye drops. RESULTS Out of the 24 samples reaching quality criteria, sex mosaicism was found in 13, demonstrating the presence of donor-derived cells at the center of the graft for at least 211 days post keratoplasty. Kaplan-Meier analysis established a median survival of donor corneal epithelial cells of 385 days. Although not statistically significant, the disappearance of donor cells seemed to be delayed and the average number of persistent cells appeared to be greater when 2% cyclosporine was used topically as an additional immunosuppressive therapy. CONCLUSIONS The combination of corneal impressions and FISH analysis is a valuable tool with negligible side effects to investigate the presence of epithelial cell mosaicism in sex-mismatched donor transplants. Epithelial cells survived at the center of the graft with a median survival of more than one year, suggesting slower epithelial turnover than previously described.

But Etudier la survie des cellules epitheliales corneennes humaines derivees du donneur apres keratoplastie (confirmer cette survie et preciser sa duree). Materiels et Methodes Vingt-quatre prelevements de cellules epitheliales sur 21 yeux de 21 patients greffes ont ete analysables. Tous les patients etaient traites par de la dexamethasone collyre en decroissance sur 18 mois et 9 patients avaient recu en plus de la ciclosporine 2 % collyre. Tous les greffons etaient clairs et provenaient de donneur dont le sexe etait different du receveur (greffe en sex-mismatched ). Les cellules epitheliales etaient recueillies par des empreintes corneennes par apposition d’une lamelle de verre sur la cornee. Les cellules prelevees etaient analysees par hybridation in-situ en fluorescence des chromosomes sexuels. Cette technique est hautement sensible pour definir l’origine des noyaux cellulaires issus soit du donneur soit du receveur. Resultats Sur 24 empreintes, 13 ont revele la presence de cellules du donneur au centre de la cornee de jusqu’a 211 jours apres la transplantation. La disparition des cellules du donneur parait etre retardee et le nombre moyen de cellules persistantes parait etre plus grand lors de l’utilisation de collyre immuno-suppresseur tel que la ciclosporine 2 %. Discussion La combinaison des empreintes corneennes et de l’analyse par hybridation in-situ en fluorescence constitue un outil precieux denue d’effets secondaires, pour s’enquerir de la presence des cellules epitheliales du donneur dans ces greffes en sex-mismatched. Il n’est pas rare que les cellules epitheliales derivees du donneur persistent jusqu’a 7 mois au centre du greffon. Des cellules souches pourraient etre presentes au centre de la cornee et participeraient a l’auto-renouvellement de l’epithelium. Mais cela souleve une question : pour quelles raisons ne vivent-elles pas aussi longtemps que des cellules souches ? Le rejet epithelial a bas bruit semble etre l’hypothese la plus probable. Cependant, l’utilisation des traitements immunosuppresseurs locaux n’est probablement pas suffisante pour prevenir cette perte cellulaire. Conclusion Nos resultats pourraient offrir des perspectives pharmacologiques et avoir une implication importante en physiologie, en pathologie et dans le traitement des pathologies de surface oculaire.

Resume But Evaluer l’impact de la chirurgie du strabisme sur l’endothelium corneen chez l’enfant. Materiel et methode Nous avons realise une etude prospective de septembre 2007 a novembre 2008, incluant 34 enfants, d’âge moyen 8,21 ans, ayant subi une chirurgie du strabisme. Le comptage cellulaire a ete realise par microscope speculaire non-contact de type Topons® SP2000 P. La densite cellulaire endotheliale (DCE), l’hexagonalite (H) (pleiomorphisme) et la surface cellulaire (polymegatisme) ont ete mesurees en pre- et post-operatoire dans une aire de 3 x 6 cm. Les enfants ont ete repartis en 4 groupes en fonction du geste chirurgical effectue : fixation posterieure isolee sur le muscle droit medial (groupe 1, n = 8), recul du muscle droit medial associe a une fixation posterieure (groupe 2, n = 18), recul du muscle droit lateral (groupe 3, n = 19), et resection du muscle oblique superieur (groupe 4, n = 5). Resultats La DCE etait de 3413,8 ± 318,51 cellules/mm 2 en pre-operatoire (DCE pre-op) versus 3391,9 ± 284,65 cellules/mm 2 en postoperatoire (DCE post-op). Une perte d’hexagonalite significative de 6,16 % ( p = 0,039) etait notee. Il existait une augmentation des cellules d’une surface de 200 et 300 μm 2 et une perte des cellules d’une surface de 100 et 400 μm 2 . Dans le groupe 1, la DCE pre-op moyenne etait de 3501,7 ± 399,7 cellules/mm 2 , la DCE post-op moyenne de 3499,7 ± 243,28 cellules/mm 2 , avec une variation +0,43 % de l’hexagonalite. Dans le groupe 2, la DCE pre-op moyenne etait de 3498,3 ± 182,8 cellules/mm 2 et la DCE post-op moyenne de 3422,8 ± 274,2 cellules/mm 2 , avec une diminution statistiquement significative de l’hexagonalite (-13,66 %, p = 0,0001). Dans le groupe 3, la DCE pre-op moyenne etait de 3238 ± 385,5 cellules/mm 2 , la DCE post-op moyenne de 3366 ± 334,4 cellules/mm 2 , avec -3,29 % d’hexagonalite. Dans le groupe 4, la DCE pre-op moyenne etait de 3264 ± 325,36 cellules/mm 2 , la DCE post-op moyenne de 3328 ± 124,42 cellules/mm 2 , l’hexagonalite diminuait de -6,2 %. Discussion-conclusion La chirurgie du strabisme n’entraine pas de diminution de la DCE, mais une perte significative d’hexagonalite pour le geste le plus agressif.

Introduction La keratite bacterienne a Nocardia Transvalensis est une infection corneenne extremement rare. Nous exposons ici le deuxieme cas rapporte dans la litterature. Objectifs et Methodes Au cours d’un voyage en Asie du sud-est, l’atteinte corneenne initiale chez notre patient correspondait a une atteinte corneenne superficielle faisant suite a la projection oculaire droite d’un corps etranger. Le patient a presente secondairement une keratite epitheliale et stromale anterieure atteignant 3 quadrants, s’etendant jusqu’en zone paracentrale. Resultats Le sequencage de l’ARN 16S sur le prelevement epithelial a permis le diagnostic bacteriologique. La Nocardia Transvalensis est un bacille gram + ramifie. La mise en culture avec realisation d’un antibiogramme est une procedure longue et delicate mais necessaire en raison de l’existence de resistances aux antibiotiques. L’evolution fut favorable sous un traitement local par collyres renforces reposant sur l’association amikacine et trimethoprime-sulfamethoxazole. Discussion Le polymorphisme clinique et la difficulte d’identification bacteriologique des Nocardia retardent frequemment son diagnostic. Il faut savoir evoquer une mycobacterie atypique en presence d’infiltrats superficiels multifocaux indolents chez un patient avec un antecedent de traumatisme corneen, notamment apres un sejour en zone d’endemie. Le sequencage de l’ARN 16S est un moyen efficace d’identification des agents infectieux atypiques. Le traitement des keratites a Nocardia repose sur l’utilisation des collyres renforces aminoglycosides et sur l’association trimethoprime-sulfamethoxazole, mais on note une emergence de souches bacteriennes resistantes a ces molecules. Conclusion Il s’agit a notre connaissance du deuxieme cas recense de keratite a Nocardia transvalensis . Un diagnostic bacteriologique rapide et performant par sequencage de l’ARN 16S permet de traiter precocement ces keratites infectieuses atypiques.

Objectif Evaluer les resultats cliniques a 6 mois d’un nouveau concept de decoupe au laser femtoseconde, le profil decagonal et son impact sur la recuperation visuelle et l’astigmatisme corneen. Materiels et Methodes 16 patients (d’âge moyen 72 ans) ont beneficie d’une keratoplastie transfixiante avec un profil de decoupe decagonal par le laser femtec 20/10. Les indications operatoires etaient : 4 decompensations endotheliales du pseudophake, 9 dystrophies de Fuchs, 1 keratocone, 1 dystrophie amyloide et 1 traumatisme. Le laser a ete programme avec une decoupe decagonale (10 cotes) afin d’obtenir un diametre de 8 a 8,2 pour le donneur et de 7,8 a 8 pour le receveur. L’angulation des bords etait de 90°. Toutes les decoupes ont ete realisees en ≪double pass≫. Le greffon etait suture par 10 points separes associes a un surjet. Resultats En post-operatoire l’etancheite du greffon a toujours ete excellente. A 3 mois, l’acuite visuelle moyenne (hormis 4 patients avec atteinte retinienne) etait de 0,31 +/− 1,4 sans correction, 0,5 +/− 1 ,7 avec correction et un astigmatisme moyen subjectif de 2,18 dioptrie. A 6 mois, l’acuite visuelle moyenne etait de 0,4 sans correction, 0,67 avec correction et un astigmatisme moyen subjectif de 1,75 dioptrie. La perte endotheliale etait de 29 %. Aucune complication n’a ete observee. Discussion Tous les patients avaient en post-operatoire une etancheite parfaite du greffon. La recuperation visuelle non corrigee ou corrigee avec un cylindre modere a ete rapidement satisfaisante. Conclusion La decoupe decagonale permet un emboitement horizontal parfait entre le donneur et le receveur, diminuant de facon significative l’astigmatisme et l’irregularite corneenne.

Introduction Notre but est de demontrer experimentalement l’interet d’un traitement local par un facteur anti-angiogenique dans la prevention de l’apparition d’une neo vascularisation corneenne apres brulure chimique. Materiels et Methodes Le modele choisi etait des rats de type (rattus norvegicus) Long Evans. Les animaux etaient repartis en 2 groupes de 10 individus. Le premier groupe etait constitue d’individus dont nous avions cauterise une seule cornee (gauche) et qui recevait immediatement 1 injection sous conjonctivale de 0,1 ml de bevacizumab. Le 2 e groupe etait constitue d’individus dont nous avions cauterise une seule cornee (gauche), et qui recevait une injection sous conjonctivale de 0,1 ml de BSS ® . La neo-vascularisation corneenne etait induite par cauterisation au nitrate d’argent. Afin d’accroitre la reproductibilite de la cauterisation, celle-ci etait realisee par le meme investigateur chez tous les animaux. Des photographies numeriques en biomicroscopie etaient realisees a J0, J7, J14 et J28. La quantification de la surface de neo vascularisation est realisee par un traitement d’image effectuee par le logiciel ImageJ. Resultats Les premiers resultats demontrent une efficacite statistiquement significative sur l’inhibition de l’apparition d’une neo vascularisation corneenne. Cependant, les resultats definitifs ne sont pas disponibles a l’heure actuelle. Discussion Notre modele permet d’etudier l’effet de nouvelles therapeutiques telles que les anti-VEGF sur l’inhibition de l’apparition ou de la progression des neo vaisseaux corneens induits par une brulure chimique. Conclusion L’injection sous conjonctivale de bevacizumab semble efficace dans l’inhibition de la neo vascularisation corneenne apres brulure chimique.

- Abstract
MicroRNAs are key regulators in the development processes or stress responses in plants. In the last decade, several conserved or non-conserved microRNAs have been identified in Medicago truncatula. Different strategies leading to the inactivation of microRNAs in plants have been described. Here, we propose a protocol for an effective inactivation of microRNAs using a STTM strategy in M. truncatula transgenic roots.

- Abstract
This decade introduced "omics" approaches, such as genomics, transcriptomics, proteomics, and metabolomics in association with reverse and forward genetic approaches, developed earlier, to try to identify molecular pathways involved in the development or in the response to environmental conditions as well as in animals and plants. This review summarizes studies that utilized "omics" strategies to unravel the root development in the model legume Medicago truncatula and how external factors such as soil mineral status or the presence of bacteria and fungi affect root system architecture in this species. We also compare these "omics" data to the knowledges concerning the Arabidopsis thaliana root development, nowadays considered as the model of allorhiz root systems. However, unlike legumes, this species is unable to interact with soil nitrogen-fixing rhizobia and arbuscular-mycorrhizal (AM) fungi to develop novel root-derived symbiotic structures. Differences in root organization, development, and regulatory pathways between these two model species have been highlighted.

Two inorganic phosphate (Pi) uptake mechanisms operate in streptophytes and chlorophytes, the two lineages of green plants. PHOSPHATE TRANSPORTER B (PTB) proteins are hypothesized to be the Na
+ /Pi symporters catalysing Pi uptake in chlorophytes, whereas PHOSPHATE TRANSPORTER 1 (PHT1) proteins are the H+ /Pi symporters that carry out Pi uptake in angiosperms. PHT1 proteins are present in all streptophyte lineages. However, Pi uptake in streptophyte algae and marine angiosperms requires Na+ influx, suggesting that Na+ /Pi symporters also function in some streptophytes. We tested the hypothesis that Na+ /Pi symporters exist in streptophytes. We identified PTB sequences in streptophyte genomes. Core PTB proteins are present at the plasma membrane of the liverwort Marchantia polymorpha. The expression of M. polymorpha core PTB proteins in the Saccharomyces cerevisiae pho2 mutant defective in high-affinity Pi transport rescues growth in low-Pi environments. Moreover, levels of core PTB mRNAs of M. polymorpha and the streptophyte alga Coleochaete nitellarum are higher in low-Pi than in Pi-replete conditions, consistent with a role in Pi uptake from the environment. We conclude that land plants inherited two Pi uptake mechanisms - mediated by the PTB and PHT1 proteins, respectively - from their streptophyte algal ancestor. - Abstract
To discover mechanisms that controlled the growth of the rooting system in the earliest land plants, we identified genes that control the development of rhizoids in the liverwort Marchantia polymorpha. 336,000 T-DNA transformed lines were screened for mutants with defects in rhizoid growth, and a de novo genome assembly was generated to identify the mutant genes. We report the identification of 33 genes required for rhizoid growth, of which 6 had not previously been functionally characterized in green plants. We demonstrate that members of the same orthogroup are active in cell wall synthesis, cell wall integrity sensing, and vesicle trafficking during M. polymorpha rhizoid and Arabidopsis thaliana root hair growth. This indicates that the mechanism for constructing the cell surface of tip-growing rooting cells is conserved among land plants and was active in the earliest land plants that existed sometime more than 470 million years ago [1, 2]., (Copyright © 2016 The Authors. - Abstract
The colonization of the land by plants, sometime before 470 million years ago, was accompanied by the evolution tissue systems [1-3]. Specialized structures with diverse functions-from nutrient acquisition to reproduction-derived from single cells in the outermost layer (epidermis) were important sources of morphological innovation at this time [2, 4, 5]. In extant plants, these structures may be unicellular extensions, such as root hairs or rhizoids [6-9], or multicellular structures, such as asexual propagules or secretory hairs (papillae) [10-12]. Here, we show that a ROOTHAIR DEFECTIVE SIX-LIKE (RSL) class I basic helix-loop-helix transcription factor positively regulates the development of the unicellular and multicellular structures that develop from individual cells that expand out of the epidermal plane of the liverwort Marchantia polymorpha; mutants that lack MpRSL1 function do not develop rhizoids, slime papillae, mucilage papillae, or gemmae. Furthermore, we discovered that RSL class I genes are also required for the development of multicellular axillary hairs on the gametophyte of the moss Physcomitrella patens. Because class I RSL proteins also control the development of rhizoids in mosses and root hairs in angiosperms [13, 14], these data demonstrate that the function of RSL class I genes was to control the development of structures derived from single epidermal cells in the common ancestor of the land plants. Class I RSL genes therefore controlled the generation of adaptive morphological diversity as plants colonized the land from the water., (Copyright © 2016 The Authors. Published by Elsevier Ltd..
