Resume Introduction Les manifestations digestives du purpura rhumatoide de l’adulte sont parfois severes. Leur traitement est mal codifie et leur pronostic peu decrit. Methodes A partir d’une serie retrospective de 23 patients ayant un purpura rhumatoide (criteres de l’ACR) comportant des manifestations digestives, nous avons etabli un score de severite clinique et tomodensitometrique et evalue l’interet d’un traitement par corticoides. Resultats L’âge median au diagnostic etait de 39 ans (16 a 80). Les manifestations digestives comprenaient des douleurs abdominales (96 %), des vomissements (52 %), des hemorragies digestives (39 %), de la diarrhee (13 %) et un syndrome occlusif (9 %). Parmi les 21 patients chez qui le score de severite clinique pouvait etre calcule, ces manifestations etaient considerees comme severes, moyennement severes et peu severes chez 14, trois et quatre patients respectivement. Dix des 14 patients ayant une atteinte digestive clinique severe avaient une atteinte radiologique severe (epaississement parietal digestif interessant plusieurs anses d’un meme segment ou plusieurs segments avec atteinte de l’ileon). Une corticotherapie a ete debutee chez 11 patients severes sur 14, deux patients moyennement severes sur trois et trois patients peu severes sur quatre. En cas d’atteinte severe, les signes digestifs ont regresse en moyenne en deux jours contre 12,3 jours sans traitement (p Conclusion Nos resultats plaident en faveur de la corticotherapie pour le traitement des atteintes digestives severes du purpura rhumatoide de l’adulte. La tomodensitometrie abdominale en revelant des anomalies dont la severite est bien correlee a la clinique, conforte la mise en route de la corticotherapie en ecartant une complication chirurgicale.

Resume Propos. – Le purpura rhumatoide est une vascularite rare chez l’adulte. L’atteinte digestive peut parfois etre a l’origine de complications et justifier un traitement adapte. Les donnees tomodensitometriques de l’atteinte digestive sont peu connues et pourraient etre utiles dans la prise en charge. Patients et methodes. – Six adultes atteints de purpura rhumatoide (criteres de l’American College of Rheumatology) avec des symptomes digestifs ont beneficie d’une tomodensitometrie abdominopelvienne avant tout traitement. Il s’agissait de quatre hommes et deux femmes âges de 19 a 74 ans. Les symptomes abdominaux ont ete concomitants du purpura dans deux cas, ou lui ont fait suite. Resultats. – Des lesions de l’intestin grele etaient constantes avec un epaississement parietal segmentaire prenant l’aspect d’image en double halo chez trois patients. L’atteinte etait duodenale chez deux patients, jejunale chez deux et ileale chez cinq. Deux segments etaient atteints chez un patient et trois segments chez un autre. Un epanchement intraperitoneal etait toujours observe. Il n’existait pas d’atteinte colique radiologique. Une fois les resultats connus, tous les patients ont recu une corticotherapie generale debutee par voie intraveineuse cinq fois a des posologies de 80 a 1 000 mg/j de methylprednisolone. L’evolution clinique a toujours ete favorable et confirmee par echographie ou tomodensitometrie trois fois. Conclusion. – La tomodensitometrie abdominopelvienne a montre des lesions evocatrices de vascularite intestinale par leur aspect, leur topographie et leur caractere segmentaire et focal. Elle a aussi contribue a la recherche d’une eventuelle cause neoplasique sous-jacente, a permis d’ecarter une autre cause de douleur abdominale et surtout une complication abdominale du purpura rhumatoide contre-indiquant la corticotherapie. Ces elements ont conforte l’indication d’une corticotherapie generale et aucun malade n’a eu de complication severe requerant une chirurgie. Ces images ne sont cependant pas specifiques du purpura rhumatoide et doivent etre replacees dans le contexte clinique et biologique.

Gastrointestinal manifestations of Henoch-Schönlein purpura (HSP) in adults may be severe. Data about treatment are controversial and the outcome is seldom described.Twenty-three patients with gastrointestinal manifestations of HSP (ACR criteria) were retrospectively studied. We proposed to use clinical and radiological (CT scan) severity scores to assess the usefulness and the efficacy of corticosteroid therapy.Age at onset ranged from 16 to 80 years (median 39). Gastrointestinal manifestations included abdominal pain (96%), vomiting (52%), gastrointestinal bleeding (39%), diarrhoea (13%) and intestinal obstruction (9%). Scores of disease clinical severity were calculated in 21 patients. Fourteen, three and four had severe, intermediate and mild disease, respectively. Ten patients in the group with severe clinical involvement underwent CT scan that showed severe radiological involvement (parietal thickening of several bowel loops of the same segment or several segments with ileal involvement). Eleven patients out of 14 with clinically severe disease were given corticosteroids. Two patients out of three with intermediate and three patients out of four with mild scores also received corticosteroids. The others received supportive care. In-patients with severe clinical scores, gastrointestinal symptoms improved within 2 days when they were given corticosteroids and within 12.3 days without corticosteroids (p0.0002). No side effect was observed with steroid therapy.These results suggest that corticosteroids may reduce abdominal symptoms of HSP in adults with clinically severe disease. They are safe when CT scan is performed.

Henoch-Schonlein purpura is a rare eventuality in adulthood. Abdominal involvement can worsen the short-term prognosis and justify medical treatment to avoid surgery.Abdominopelvic computed tomography (CT) was performed in six adult patients (four men and two women; age range, 19-74 years) with Henoch-Schonlein purpura before any treatment. Abdominal symptoms were concomitant with the purpuric rash in two cases and followed it in the other cases.The small bowel was always involved with a parietal thickening, and the target sign was observed in three patients. The lesions were located in the duodenum in two patients, the jejunum in two and the ileum in five. Two segments were involved in one patient and three segments in another patient. A peritoneal effusion was always present. No colonic lesion was discovered on CT. Once the results were known, all patients received steroids with initially intravenous methylprednisolone in five patients at doses ranging from 80 to 1,000 mg/day. On follow-up, none of the patients underwent surgery.Abdominopelvic CT scan demonstrated signs suggestive of intestinal vasculitis. An underlying neoplasia or an eventual other cause of abdominal pain were explored and a steroid therapy was then began with a good outcome. Abdominal CT can be helpful in the diagnosis of abdominal involvement in Henoch-Schonlein purpura.
