Abstract

BACKGROUND: It is unclear whether patients age 65 years and over with a recent stroke or TIA benefit from statin treatment to a similar degree as younger patients. METHODS: The 4,731 patient cohort in the SPARCL study was divided into an elderly group (65 and over) and a younger group. The primary endpoint (fatal or nonfatal stroke) and secondary endpoints were analyzed, with calculation of the hazard ratio (HR) and p values from a Cox regression model. RESULTS: There were 2,249 patients in the elderly group and 2,482 in the younger group. The baseline LDL (133 mg/dL) and total cholesterol were comparable in the two groups. The elderly and younger groups had a 61.4 mg/dL and 58.7 mg/dL decrease in mean LDL during the trial. The primary endpoint was reduced by 26% in younger patients (HR 0.74, 0.57-0.96, p = 0.02) and by 10% in elderly subjects (HR 0.90, 0.73-1.11, p = 0.33). A test of heterogeneity for a treatment-age interaction was not significant (p = 0.52). The risk of stroke or TIA (HR 0.79, p = 0.01), major coronary events (HR 0.68, p = 0.035), any coronary heart disease event (HR 0.61, p = 0.0006), and revascularization procedures (HR 0.55, p = 0.0005) was reduced in the elderly group. CONCLUSIONS: There was no heterogeneity in the stroke reduction seen with atorvastatin in the elderly and younger groups. Cardiac events and revascularization procedures were also lower in both the elderly and younger subgroups treated with atorvastatin. These results support the use of atorvastatin in elderly patients with recent stroke or TIA.

Abstract

BACKGROUND: It is unclear whether patients age 65 years and over with a recent stroke or TIA benefit from statin treatment to a similar degree as younger patients. METHODS: The 4,731 patient cohort in the SPARCL study was divided into an elderly group (65 and over) and a younger group. The primary endpoint (fatal or nonfatal stroke) and secondary endpoints were analyzed, with calculation of the hazard ratio (HR) and p values from a Cox regression model. RESULTS: There were 2,249 patients in the elderly group and 2,482 in the younger group. The baseline LDL (133 mg/dL) and total cholesterol were comparable in the two groups. The elderly and younger groups had a 61.4 mg/dL and 58.7 mg/dL decrease in mean LDL during the trial. The primary endpoint was reduced by 26% in younger patients (HR 0.74, 0.57-0.96, p = 0.02) and by 10% in elderly subjects (HR 0.90, 0.73-1.11, p = 0.33). A test of heterogeneity for a treatment-age interaction was not significant (p = 0.52). The risk of stroke or TIA (HR 0.79, p = 0.01), major coronary events (HR 0.68, p = 0.035), any coronary heart disease event (HR 0.61, p = 0.0006), and revascularization procedures (HR 0.55, p = 0.0005) was reduced in the elderly group. CONCLUSIONS: There was no heterogeneity in the stroke reduction seen with atorvastatin in the elderly and younger groups. Cardiac events and revascularization procedures were also lower in both the elderly and younger subgroups treated with atorvastatin. These results support the use of atorvastatin in elderly patients with recent stroke or TIA.

Abstract

BACKGROUND AND PURPOSE: In SPARCL, treatment with atorvastatin 80 mg daily reduced stroke risk in patients with recent stroke or TIA and no known coronary heart disease by 16% versus placebo over 4.9 years of follow-up. The purpose of this secondary analysis was to determine whether men and women similarly benefited from randomization to statin treatment. METHODS: The effect of sex on treatment-related reductions in stroke and other cardiovascular outcomes were analyzed with Cox regression modeling testing for sex by treatment interactions. RESULTS: Women (n=1908) constituted 40% of the SPARCL study population. At baseline, men (n=2823) were younger (62.0+/-0.21 versus 63.9+/-0.27 years), had lower systolic BPs (138.1+/-0.35 versus 139.5+/-0.47 mm Hg), higher diastolic BPs (82.2+/-0.20 versus 81.0+/-0.25 mm Hg), more frequently had a history of smoking (73% versus 38%), and had lower total cholesterol (207.0+/-0.54 versus 218.9+/-0.67 mg/dL) and LDL-C levels (132+/-0.45 versus 134+/-0.57 mg/dL) than women. Use of antithrombotics and antihypertensives were similar. After prespecified adjustment for region, entry event, time since event, and age, there were no sex by treatment interactions for the combined risk of nonfatal and fatal stroke (treatment Hazard Ratio, HR=0.84, 95% CI 0.68, 1.02 in men versus HR=0.84, 95% CI 0.63, 1.11 in women; treatment x sex interaction P=0.99), major cardiac events (HR=0.61, 95% CI 0.42, 0.87 in men versus HR=0.76, 95% CI 0.48, 1.21 in women; P=0.45), major cardiovascular events (HR=0.78, 95% CI 0.65, 0.93 in men versus HR=0.84, 95% CI 0.65, 1.07 in women; P=0.63), revascularization procedures (HR=0.50, 95% CI 0.37, 0.67 in men versus HR=0.76, 95% CI 0.46, 1.24 in women; P=0.17), or any CHD event (HR=0.54, 95% CI 0.41, 0.72 in men versus 0.67 95% CI 0.46, 0.98 in women; P=0.40). CONCLUSIONS: Stroke and other cardiovascular events are similarly reduced with atorvastatin 80 mg/d in men and women with recent stroke or TIA.

Abstract

Background and Purpose— In SPARCL, treatment with atorvastatin 80 mg daily reduced stroke risk in patients with recent stroke or TIA and no known coronary heart disease by 16% versus placebo over 4.9 years of follow-up. The purpose of this secondary analysis was to determine whether men and women similarly benefited from randomization to statin treatment. Methods— The effect of sex on treatment-related reductions in stroke and other cardiovascular outcomes were analyzed with Cox regression modeling testing for sex by treatment interactions. Results— Women (n=1908) constituted 40% of the SPARCL study population. At baseline, men (n=2823) were younger (62.0±0.21versus 63.9±0.27 years), had lower systolic BPs (138.1±0.35 versus 139.5±0.47 mm Hg), higher diastolic BPs (82.2±0.20 versus 81.0±0.25 mm Hg), more frequently had a history of smoking (73% versus 38%), and had lower total cholesterol (207.0±0.54 versus 218.9±0.67 mg/dL) and LDL-C levels (132±0.45 versus 134±0.57 mg/dL) than women. Use of antithrombotics and antihypertensives were similar. After prespecified adjustment for region, entry event, time since event, and age, there were no sex by treatment interactions for the combined risk of nonfatal and fatal stroke (treatment Hazard Ratio, HR=0.84, 95% CI 0.68, 1.02 in men versus HR=0.84, 95% CI 0.63, 1.11 in women; treatment×sex interaction P =0.99), major cardiac events (HR=0.61, 95% CI 0.42, 0.87 in men versus HR=0.76, 95% CI 0.48, 1.21 in women; P =0.45), major cardiovascular events (HR=0.78, 95% CI 0.65, 0.93 in men versus HR=0.84, 95% CI 0.65, 1.07 in women; P =0.63), revascularization procedures (HR=0.50, 95% CI 0.37, 0.67 in men versus HR=0.76, 95% CI 0.46, 1.24 in women; P =0.17), or any CHD event (HR=0.54, 95% CI 0.41, 0.72 in men versus 0.67 95% CI 0.46, 0.98 in women; P =0.40). Conclusion— Stroke and other cardiovascular events are similarly reduced with atorvastatin 80 mg/d in men and women with recent stroke or TIA.

Abstract

Using ultrasonographic measurements based on Doppler's phenomenon the author determined in 86 subjects the velocity of blood flow in the common carotid arteries before and after breathing 6% CO2. Acceleration of blood flow resulting from dilation of cerebral blood vessels was observed in all healthy subjects up to the age of 45 years. In older subjects the vasomotor reaction was sometimes absent, significantly more frequently in cases of past cerebrovascular accidents. These results indicate absence of vasomotor reserve in most patients with disturbances of cerebral circulation. The used method of investigation makes possible, in view of its simplicity and non-invasiveness, a more widespread determination of cerebral vascular reactions in clinical studies.

Abstract

Patients with brain strokes are particularly prone to development of venous thrombosis in the paretic extremities, and the diagnosis of this thrombosis is very difficult. This raises the risk of pulmonary embolism. The authors studied venous circulation in 50 patients with brain strokes using the method of ultrasonography based on Doppler's phenomenon. Signs of venous thrombosis were demonstrated in 11 cases. Venous thrombosis was diagnosed when the following signs were simultaneously present: slowing down of venous flow of considerable degree (by above 50%), abolition of cyclic fluctuations in flow velocity depending in respiration, and absence of blood flow acceleration after pressure exerted on muscles. Thrombosis can be diagnosed, frequently, before the appearance of local signs or in case of doubtful clinical manifestations. Doppler ultrasonography is a simple diagnostic method which can be repeated without restrictions and makes possible conduction of motor rehabilitation of patients under steady control of venous circulation. Owing to this venous thrombosis can be diagnoses early and treated accordingly reducing the risk of pulmonary embolism.

Abstract

The author investigated the blood flow in the common carotid arteries by means of Doppler ultrasonography in 98 patients after stroke. An UDP 10 ultrasound flow detector was used and the results obtained in patients dying during the treatment were compared with those who were discharged improved. In the cases with lethal outcome considerable (over 30%) asymmetry of blood flow was found, usually but not always with slowing of this flow on the side of the lesion. Diastolic blood flow was in this group significantly lower on the side of the cerebral lesion and compared with the group with favourable outcome. Other differences were not characteristic. The results of these investigations showed that in patients with a high-grade asymmetry of blood flow in the common carotid arteries and impairment of brain perfusion evidenced by slowing down of the diastolic blood flow the course of the stroke is often more severe. This may be of prognostic significance.

Abstract

The authors carried out quantitative analysis of the spectrum of the Doppler blood flow signals in the common carotid artery in 48 healthy subjects in two age groups (16-28 and 51-83 years) and 29 patients after ischaemic stroke. In the young subjects the greatest range of the spectrum was covered by the 1-2 kHz band (40.2% of the area of spectrum diagram). In later age this value decreased to 32.9% and in the patients it was 22.3%. At the same time the area covered by the 0-1 kHz band increased, from 35.9% in young subjects to 50.4% in the older ones and 66.9% in the patients. The spectrum comprised, most frequently, the frequency range 0-5 kHz and had a tendency for narrowing in the patients after stroke. In cases of common carotid artery stenosis it was found to increase to 10 kHz at the site of the stenosis, while above and below the stenosis the spectrum was reduced to 2 kHz, with 90% of its area occupied by the 0-1 kHz band. these results indicate the possibility of quantitative determination of haemodynamic disturbances by means of frequency analysis of Doppler signals.

Abstract

The author carried out ultrasonography of the common carotid arteries by the method based on Doppler's phenomenon in 100 subjects aged 16--86 years. The maximal, minimal and mean flow velocity, vascular resistance, and symmetry and shape of the recorded curve were evaluated. Early and progressing with age changes were found in the curve indicating decreasing vascular wall elasticity. The maximal flow velocity decreased also rapidly. On the other hand, the mean and the minimal flow velocity and the index of vascular resistance remained without substantial changes up to the age of 70 years. In later age the minimal and mean flow velocity decreased rapidly and the index of vascular resistance increased greatly. This suggested accelerated rate of atherosclerosis development and/or exhaustion of compensatory mechanisms. In 3 cases of considerable asymmetry of the curve was observed caused by asymptomatic stenosis of one artery. Doppler ultrasonography could be useful at the time of intensification and evolution of atherosclerotic changes in the carotid arteries, and their effect on the condition of cerebral circulation.

Abstract

In a group of 34 patients with lumbosacral pain acupuncture was applied in 18 cases and suggestion in 16 cases and the results of both methods were compared. The therapeutic effects were evaluated immediately after the procedure and after a series of 4 procedures. The therapeutic results were better both immediately and after a series of acupuncture. The difference in the results of treatment was statistically significant in the patients with longest duration of pains. Presence of degenerative spinal changes was without influence on the results of treatment by both methods.
