Abstract

BackgroundThere is little information on the incidence of AIDS-defining events which have been reported in the literature to be associated with immune reconstitution inflammatory syndrome (IRIS) after combined antiretroviral therapy (cART) initiation. These events include tuberculosis, mycobacterium avium complex (MAC), cytomegalovirus (CMV) retinitis, progressive multifocal leukoencephalopathy (PML), herpes simplex virus (HSV), Kaposi sarcoma, non-Hodgkin lymphoma (NHL), cryptococcosis and candidiasis.MethodsWe identified individuals in the HIV-CAUSAL Collaboration, which includes data from six European countries and the US, who were HIV-positive between 1996 and 2013, antiretroviral therapy naive, aged at least 18 years, had CD4 cell count and HIV-RNA measurements and had been AIDS-free for at least 1 month between those measurements and the start of follow-up. For each AIDS-defining event, we estimated the hazard ratio for no cART versus less than 3 and at least 3 months since cART initiation, adjusting for time-varying CD4 cell count and HIV-RNA via inverse probability weighting.ResultsOut of 96 562 eligible individuals (78% men) with median (interquantile range) follow-up of 31 [13,65] months, 55 144 initiated cART. The number of cases varied between 898 for tuberculosis and 113 for PML. Compared with non-cART initiation, the hazard ratio (95% confidence intervals) up to 3 months after cART initiation were 1.21 (0.90-1.63) for tuberculosis, 2.61 (1.05-6.49) for MAC, 1.17 (0.34-4.08) for CMV retinitis, 1.18 (0.62-2.26) for PML, 1.21 (0.83-1.75) for HSV, 1.18 (0.87-1.58) for Kaposi sarcoma, 1.56 (0.82-2.95) for NHL, 1.11 (0.56-2.18) for cryptococcosis and 0.77 (0.40-1.49) for candidiasis.ConclusionWith the potential exception of mycobacterial infections, unmasking IRIS does not appear to be a common complication of cART initiation in high-income countries.

Abstract

Background. Tenofovir disoproxil fumarate (TDF) has been linked to renal impairment, but the extent to which this impairment is reversible is unclear. We aimed to investigate the reversibility of renal decline during TDF therapy. Methods. Cox proportional hazards models assessed factors associated with discontinuing TDF in those with an exposure duration of >6 months. In those who discontinued TDF therapy, linear piecewise regression models estimated glomerular filtration rate (eGFR) slopes before initiation of, during, and after discontinuation of TDF therapy. Factors associated with not achieving eGFR recovery 6 months after discontinuing TDF were assessed using multivariable logistic regression. Results. We observed declines in the eGFR during TDF exposure (mean slopes, −15.7 mL/minute/1.73 m2/year [95% confidence interval {CI}, −20.5 to −10.9] during the first 3 months and −3.1 mL/minute/1.73 m2/year [95% CI, −4.6 to −1.7] thereafter) and evidence of eGFR increases following discontinuation of TDF therapy (mean slopes, 12.5 mL/minute/1.73 m2/year [95% CI, 8.9–16.1] during the first 3 months and 0.8 mL/minute/1.73 m2/year [95% CI, .1–1.5] thereafter). Following TDF discontinuation, 38.6% of patients with a decline in the eGFR did not experience recovery. A higher eGFR at baseline, a lower eGFR after discontinuation of TDF therapy, and more-prolonged exposure to TDF were associated with an increased risk of incomplete recovery 6 months after discontinuation of TDF therapy. Conclusions. This study shows that a decline in the eGFR during TDF therapy was not fully reversible in one third of patients and suggests that prolonged TDF exposure at a low eGFR should be avoided.

Contains fulltext : 80963.pdf (Publisher’s version ) (Open Access) BACKGROUND: The extent to which mortality differs following individual acquired immunodeficiency syndrome (AIDS)-defining events (ADEs) has not been assessed among patients initiating combination antiretroviral therapy. METHODS: We analyzed data from 31,620 patients with no prior ADEs who started combination antiretroviral therapy. Cox proportional hazards models were used to estimate mortality hazard ratios for each ADE that occurred in >50 patients, after stratification by cohort and adjustment for sex, HIV transmission group, number of antiretroviral drugs initiated, regimen, age, date of starting combination antiretroviral therapy, and CD4+ cell count and HIV RNA load at initiation of combination antiretroviral therapy. Abstract

Background: There is little information on the incidence of AIDS-defining events which have been reported in the literature to be associated with immune reconstitution inflammatory syndrome (IRIS) after combined antiretroviral therapy (cART) initiation. These events include tuberculosis, mycobacterium avium complex (MAC), cytomegalovirus (CMV) retinitis, progressive multifocal leukoencephalopathy (PML), herpes simplex virus (HSV), Kaposi sarcoma, non-Hodgkin lymphoma (NHL), cryptococcosis and candidiasis.Methods: We identified individuals in the HIV-CAUSAL Collaboration, which includes data from six European countries and the US, who were HIV-positive between 1996 and 2013, antiretroviral therapy naive, aged at least 18 years, hadCD4+ cell count and HIV-RNA measurements and had been AIDS-free for at least 1 month between those measurements and the start of follow-up. For each AIDS-defining event, we estimated the hazard ratio for no cART versus less than 3 and at least 3 months since cART initiation, adjusting for time-varying CD4+ cell count and HIV-RNA via inverse probability weighting.Results: Out of 96 562 eligible individuals (78% men) with median (interquantile range) follow-up of 31 [13,65] months, 55 144 initiated cART. The number of cases varied between 898 for tuberculosis and 113 for PML. Compared with non-cART initiation, the hazard ratio (95% confidence intervals) up to 3 months after cART initiation were 1.21 (0.90-1.63) for tuberculosis, 2.61 (1.05-6.49) for MAC, 1.17 (0.34-4.08) for CMV retinitis, 1.18 (0.62-2.26) for PML, 1.21 (0.83-1.75) for HSV, 1.18 (0.87-1.58) for Kaposi sarcoma, 1.56 (0.82-2.95) for NHL, 1.11 (0.56-2.18) for cryptococcosis and 0.77 (0.40-1.49) for candidiasis.Conclusion: With the potential exception of mycobacterial infections, unmasking IRIS does not appear to be a common complication of cART initiation in high-income countries.

Abstract

Background: There is little information on the incidence of AIDS-defining events which have been reported in the literature to be associated with immune reconstitution inflammatory syndrome (IRIS) after combined antiretroviral therapy (cART) initiation. These events include tuberculosis, mycobacterium avium complex (MAC), cytomegalovirus (CMV) retinitis, progressive multifocal leukoencephalopathy (PML), herpes simplex virus (HSV), Kaposi sarcoma, non-Hodgkin lymphoma (NHL), cryptococcosis and candidiasis. Methods: We identified individuals in the HIV-CAUSAL Collaboration, which includes data from six European countries and the US, who were HIV-positive between 1996 and 2013, antiretroviral therapy naive, aged at least 18 years, hadCD4+ cell count and HIV-RNA measurements and had been AIDS-free for at least 1 month between those measurements and the start of follow-up. For each AIDS-defining event, we estimated the hazard ratio for no cART versus less than 3 and at least 3 months since cART initiation, adjusting for time-varying CD4+ cell count and HIV-RNA via inverse probability weighting. Results: Out of 96 562 eligible individuals (78% men) with median (interquantile range) follow-up of 31 [13,65] months, 55 144 initiated cART. The number of cases varied between 898 for tuberculosis and 113 for PML. Compared with non-cART initiation, the hazard ratio (95% confidence intervals) up to 3 months after cART initiation were 1.21 (0.90-1.63) for tuberculosis, 2.61 (1.05-6.49) for MAC, 1.17 (0.34-4.08) for CMV retinitis, 1.18 (0.62-2.26) for PML, 1.21 (0.83-1.75) for HSV, 1.18 (0.87-1.58) for Kaposi sarcoma, 1.56 (0.82-2.95) for NHL, 1.11 (0.56-2.18) for cryptococcosis and 0.77 (0.40-1.49) for candidiasis. Conclusion: With the potential exception of mycobacterial infections, unmasking IRIS does not appear to be a common complication of cART initiation in high-income countries. © 2014 Wolters Kluwer Health | Lippincott Williams & Wilkins.

Abstract

Background and objectives: Guidelines indicate a plasma HIV-1 RNA load of 500-1000 copies/mL as the minimal threshold for antiretroviral drug resistance testing. Resistance testing at lower viral load levels may be useful to guide timely treatment switches, although data on the clinical utility of this remain limited. We report here the influence of viral load levels on the probability of detecting drug resistance mutations (DRMs) and other mutations by routine genotypic testing in a large multicentre European cohort, with a focus on tests performed at a viral load

Abstract

Background: The extent to which the prognosis for AIDS and death of patients initiating highly active antiretroviral therapy (HAART) continues to be affected by their characteristics at the time of initiation (baseline) is unclear. Methods: We analyzed data on 20,379 treatment-naive HIV-1- infected adults who started HAART in 1 of 12 cohort studies in Europe and North America (61,798 person-years of follow-up, 1844 AIDS events, and 1005 deaths). Results: Although baseline CD4 cell count became less prognostic with time, individuals with a baseline CD4 count 350 cells/μL (hazard ratio for AIDS = 2.3, 95% confidence interval [CI]: 1.0 to 2.3; mortality hazard ratio = 2.5, 95% CI: 1.2 to 5.5, 4 to 6 years after starting HAART). Rates of AIDS were persistently higher in individuals who had experienced an AIDS event before starting HAART. Individuals with presumed transmission by means of injection drug use experienced substantially higher rates of AIDS and death than other individuals throughout follow-up (AIDS hazard ratio = 1.6, 95% CI: 0.8 to 3.0; mortality hazard ratio = 3.5, 95% CI: 2.2 to 5.5, 4 to 6 years after starting HAART). Conclusions: Compared with other patient groups, injection drug users and patients with advanced immunodeficiency at baseline experience substantially increased rates of AIDS and death up to 6 years after starting HAART.

Abstract

Introduction: The development of a nursing practice, improvements in nurses’ autonomy, and increased professional and personal responsibility for the medical services provided all require professional documentation with records of health status assessments, decisions undertaken, actions and their outcomes for each patient. The International Classification for Nursing Practice is a tool that meets all of these needs, and although it requires continuous evaluation, it offers professional documentation and communication in the practitioner and researcher community. Aim: The aim of this paper is to present a theoretical critique of an issue related to policy and experience of the current situation in Polish nursing – especially of the efforts to standardize nursing practices through the introduction and development of the Classification in Poland. Discussion: Despite extensive promotion and training by International Council of Nurses members worldwide, there are still many countries where the Classification has not been implemented as a standard tool in healthcare facilities. Recently, a number of initiatives were undertaken in cooperation with the local and state authorities to disseminate the Classification in healthcare facilities. Conclusions: Thanks to intense efforts by the Polish Nurses Association and the International Council of Nurses Accredited Center for ICNP® Research & Development at the Medical University of Łódź, the Classification is known in Poland and has been tested at several centres. Nevertheless, an actual implementation that would allow for national and international interoperability requires strategic governmental decisions and close cooperation with information technology companies operating in the country. Implications for nursing & health policy: Discussing the barriers to the implementation of the Classification can improve understanding of it and its use. Abstract

Introduction: Excessive alcohol consumption and smoking increase the risk of hypertension and the incidence of cardiovascular disease. Nurses have a unique opportunity to help patients who smoke and consume alcohol at a risky level, not requiring a specialist, which may contribute to a reduction in blood pressure (BP), as well as providing more health benefits. The aim of the study was to evaluate the knowledge of nurses on how to assess the consumption of alcohol and smoking and their effect on BP. Material and methods: The study included 1,108 participants (W-1,089, M-19, aged 21-60, 0-37 years of work experience). The study was conducted in 2007-2009 using the diagnostic survey method and achievement tests. Results: Alcohol assessment methods were correctly indicated by 13.4% of respondents, while the correct interpretation of a standard unit of alcohol was made by 35.8% of respondents. The Fagerström Test was known only to 13.2% of respondents. The effects of alcohol and smoking on BP values were known to most subjects. The youngest participants, nursing graduates, were significantly more likely to have made the correct indication. Conclusions: Overall, knowledge of how to assess alcohol consumption and smoking allowing minimal intervention to take place in the group of nurses studied was relatively low. Significantly more correct indications were made by the youngest respondents, those who were not married and nursing graduates. It seems reasonable to popularize methods for identifying the risk of hypertension associated with alcohol consumption and cigarette smoking in postgraduate nursing education. [ABSTRACT FROM AUTHOR]

Abstract

The effect of heat-treatment on 10 wt% CuO-ZnAl[sub 2]O[sub 4] catalytic activity in methylation of phenol and the degree of interaction of CuO active phase with support spinel phase were investigated. The CuO-ZnAl[sub 2]O[sub 4] sample was subjected to heat-treatment up to 1000°C. The thermal products were characterized by X-ray diffraction (XRD) analysis, nitrogen adsorption-desorption at -196°C and temperature-programmed desorption (TPD-MS) of CO[sub 2]. Additionally, the reducibility of copper phases was investigated by temperature-programmed reduction (TPR). XRD patterns of the fresh catalyst sample (calcined at 600°C) indicated the presence of a mixture of poorly crystallized CuO and ZnAl[sub 2]O[sub 4] spinel phase. The presence of two reducible copper species has been found on fresh CuO-ZnAl[sub 2]O[sub 4] catalyst by TPR analysis. After subsequent calcinations in air at elevated temperatures some CuO disappeared with appearance of CuAl[sub 2]O[sub 4] phase. The catalytic results revealed that the CuO addition to ZnAl[sub 2]O[sub 4] increases the activity in ortho-methylation of phenol. Subsequent heat-treatment up to 900°C causes partial deactivation of copper centers, which is the result of transformation of CuO to the inactive CuAl[sub 2]O[sub 4] phase. [ABSTRACT FROM AUTHOR]

Abstract

Aldehydes and their derivatives - primary alcohols and esters undergo ketonization in the gas phase in the range of 350-450 °C over multicomponental oxide catalyst - Zr-Mg-Y-O. This method is appropriate for processing mixtures of components. An experimental study was performed on ketonization of methyl esters fractions of waste fats and residuary alcohol fractions originating from industrial installations. In both cases mainly ketone fractions were obtained. During transformation of methyl esters and alcohols total yields of ketones amounted to over 60% and about 75%, respectively. Also, other catalytic systems (i.e. Fe-Si-Cr-K-O and Sn-Ce-Rh-O) can be used for ketonization of both raw materials. [ABSTRACT FROM AUTHOR]

Abstract

The vapour phase alkylation of 2-naphthol with C1−C3 alcohols in the presence of an iron catalyst that contains Cr-, Si- and K-oxides was investigated. 2-Naphthol was methylated in a temperature range 520–645 K giving first 1-methyl-2-naphthol and then 1,3-dimethyl-2-naphthol with total selectivity of ortho-alkylation over 98%. In the cases of ethanol and n-propanol, derivatives of 2-naphthol alkylated in the 1-position were obtained with high yield and selectivity. Abstract

The methyl ester mixture obtained from rape oil, and the mixture of butyl esters produced from animal fatty acids, were converted into mixtures of ketones with a total yield above 65%. The experiments were performed at atmospheric pressure in a temperature range of 300-400 °C using a standard flow system, with an iron catalyst modified with silicon, chromium, and potassium oxides, in the molar ratio of 100 : 2 : 1 : 0.1. The described method may extend the prospects of using waste technical fats.

Abstract

The susceptibility to ketonization of oxidized derivatives of n-decane (n-decanol, n-decanal, their equimolar mixtures, n-decyl n-decylate, and 3-hydroxy-2-octyl-dodecanal) in the gas phase at atmospheric pressure over Fe3O4catalyst was studied. Using those reactants, 10-nonyldecanone was the main product. Condensing n-decanal, n-decyl n-decylate, and 3-hydroxy-2-octyl-dodecanal, ketone is obtained with higher yields than from n-decanol.

50. Alkylation of phenol with C2and C3alcohols over iron catalyst

Abstract

The results of research on catalytic alkylation of phenol with ethyl, n- and iso-propyl alcohols conducted in gas phase in presence of iron catalyst are presented. We found that C2and C3alcohols exhibit high selectivity of ortho-alkylation, for ethanol — 99%, for n-propanol and for iso-propanol — up to 92% in the investigated conditions of reaction.
