In Europe, transjugular intrahepatic portosystemic shunt (TIPS) is considered the primary treatment for gastric varix (GV) bleeding to reduce portal hypertension. However, in Asian countries, balloon-occluded retrograde transvenous obliteration (BRTO) and its variants plug/coil assisted transvenous retrograde obliteration (PARTO/CARTO) are the preferred approaches. The purpose of this study is to report a European single-center experience in the use of PARTO/CARTO techniques for the treatment of GVs in patients with portal hypertension, focusing on the effectiveness and safety of the procedure. All the procedures involving the PARTO/CARTO techniques performed from 2019 to 2023 were retrospectively evaluated. Technical success was defined as the complete obliteration of both the GVs and the gastro-renal (GR) shunt on review of the Computed Tomography (CT) scan performed 3 days after the procedure. The obliteration rate was also evaluated through performing CT scans 1 and 12 months after the procedure, and through endoscopic follow-up at 3 and 6 months. Additionally, major and minor complications were reported. The study involved seven patients, with a technical success of 100%. During follow-up, there were no episodes of variceal rebleeding or upper gastrointestinal bleeding. Two patients developed ascites, which resolved with medical therapy. One patient exhibited focal portal thrombosis, which was successfully treated with heparin. PARTO/CARTO techniques demonstrated high technical and clinical success rates, offering advantages over traditional BRTO. The use of coils and plugs simplifies the procedure, eliminates sclerosing agents, and prevents complications associated with balloon guiding catheters.

Abstract Beta-thalassemia represents a heterogeneous group of haemoglobin inherited disorders, among the most common genetic diseases in the world, frequent in the Mediterranean basin. As beta-thalassemia patients’ survival has increased over time, previously unknown complications are observed with increasing frequency. Among them, an increased risk of hepatocellular carcinoma (HCC) has been registered. Our aim is to reduce inequalities in diagnosis and treatment and to offer patients univocal recommendations in any institution. The members of the panel - gastroenterologists, radiologists, surgeons and oncologists -were selected on the basis of their publication records and expertise. Thirteen clinical questions, derived from clinical needs, and an integration of all the committee members’ suggestions, were formulated. Modified Delphi approach involving a detailed literature review and the collective judgement of experts, was applied to this work. Thirteen statements were derived from expert opinions’ based on the current literature, on recently developed reviews and on technological advancements. Each statement is discussed in a short paragraph reporting the current key evidence. As this is an emerging issue, the number of papers on HCC in beta-thalassemia patients is limited and based on anecdotal cases rather than on randomized controlled studies. Therefore, the panel has discussed, step by step, the possible differences between beta-thalassemia and non beta-thalassemia patients. Despite the paucity of the literature, practical and concise statements were generated. This paper offers a practical guide organized by statements describing how to manage HCC in patients with beta-thalassemia.

ABSTRACT Objective The objective of our study is to evaluate the clinical outcomes and safety of radiofrequency thermal ablation (RFA) for benign thyroid nodules (BTNs) over a 1-year follow-up. Subjects and methods This is a monocentric retrospective study. Forty-eight patients with solid, non-functioning BTNs were treated by RFA using a 17G internally cooled electrode. We categorized thyroid nodules as small (≤ 12 mL), medium (12 to 30 mL), or large (over 30 mL). BTNs volume reduction, thyroid function, cosmetic and compressive score changes and side effect evaluation at 6 and 12 months were evaluated. Results BTN volume decreased significantly from baseline to 6 (mean percentage decrease of BTN volume was 66.8 ± 13.6%, p < 0.001). At 12 months, the mean percentage reduction of BTN volume compared to six months was 13.7 ± 17.1% (p < 0.001). At 6-month, symptom score had improved significantly (p < 0.001) while it does not change significantly between 6 and 12 months. In particular, symptom score improved significantly in the medium (p < 0.001) and large (p < 0.01) subgroups. Cosmetic score improved significantly between baseline and 6 months (p < 0.001) and between 6 and 12 months (p < 0.01). In all the subgroups, cosmetic score improved significantly between baseline and 6 months, while between 6 and 12 months it improved significantly only in the large group (p < 0.05). RFA was well tolerated. Only one patient experienced permanent right paramedian vocal cord palsy. Conclusions A single RFA treatment was effective in reducing BTNs volume, in particular small and medium nodules. Cosmetic score improved in all treated BTNs while symptom score only got better in the medium and large BTNs.

Percutaneous ethanol injection (PEI) is a technique used only for benign thyroid nodules, cystic or mixed cystic-solid with a large fluid component. It is a quite low-cost, safe, and outpatient method of treatment. Rare and severe complications have been described after PEI: jugular vein thrombosis and severe ethanol toxic necrosis of the larynx combined with necrotic dermatitis. Moreover, only four thyrotoxicosis cases due to Graves’ disease have been reported. We report a case of 58-year-old female with a voluminous thyroid cystic nodule, occupying almost the entire left thyroid lobe. Our patient had already performed surgical visit and intervention of thyroidectomy had been proposed to her, which she refused. At baseline, our patient has a normal thyroid function with negative autoantibodies. According to the nodular structure, intervention of PEI has been performed with a significant improvement of compressive symptoms and cosmetic disorders. About 30 days after treatment, there was a significant volume reduction, but patient developed an acclaimed symptomatic thyrotoxicosis. After ruling out several causes of hyperthyroidism and according to the thyroid scintigraphy findings, we made the diagnosis of Plummer adenoma. To our knowledge, our patient is the first case of Plummer adenoma following PEI treatment of nontoxic thyroid nodule.

Intraductal papillary mucinous neoplasms (IPMNs) are a very common incidental finding during patient radiological assessment. These lesions may progress from low-grade dysplasia (LGD) to high-grade dysplasia (HGD) and even pancreatic cancer. The IPMN progression risk grows with time, so discontinuation of surveillance is not recommended. It is very important to identify imaging features that suggest LGD of IPMNs, and thus, distinguish lesions that only require careful surveillance from those that need surgical resection. It is important to know the management guidelines and especially the indications for surgery, to be able to point out in the report the findings that suggest malignant degeneration. The imaging tools employed for diagnosis and risk assessment are Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) with contrast medium. According to the latest European guidelines, MRI is the method of choice for the diagnosis and follow-up of patients with IPMN since this tool has a highest sensitivity in detecting mural nodules and intra-cystic septa. It plays a key role in the diagnosis of worrisome features and high-risk stigmata, which are associated with IPMNs malignant degeneration. Nowadays, the main limit of diagnostic tools is the ability to identify the precursor of pancreatic cancer. In this context, increasing attention is being given to artificial intelligence (AI) and radiomics analysis. However, these tools remain in an exploratory phase, considering the limitations of currently published studies. Key limits include noncompliance with AI best practices, radiomics workflow standardization, and clear reporting of study methodology, including segmentation and data balancing. In the radiological report it is useful to note the type of IPMN so as the morphological features, size, rate growth, wall, septa and mural nodules, on which the indications for surveillance and surgery are based. These features should be reported so as the surveillance time should be suggested according to guidelines.

Abstract The aim of the article is to introduce a new term in post-procedural events related to the procedure itself. All the Societies and Councils report these events as complications and they are divided in mild, moderate and severe or immediate and delayed. On the other hand the term error is known as the application of a wrong plan, or strategy to achieve a goal. For the first time, we are trying to introduce the term “consequence”; assuming that the procedure is the only available and the best fit to clinical indication, a consequence should be seen as an expected and unavoidable occurrence of an “adverse event” despite correct technical execution.

In this contribution, new data concerning the distribution of native vascular flora in Italy are presented. It includes new records, confirmations, and exclusions to the Italian administrative regions. Nomenclatural and distribution updates, published elsewhere, and corrigenda are provided as supplementary material.

Sustainable energy projects can help societies reduce climate change’s negative impacts. In the present paper, published studies regarding European community energy schemes (CESs) were analysed to investigate CESs’ barriers and drivers towards including community members and, more specifically, vulnerable groups. This review followed the PRISMA guidelines to ensure transparency. Exclusion and inclusion criteria were defined to select the proper articles and books that were aligned with the scope of this review. A total of 143 publications from 2019 to February 2024 were considered and selected. Content analysis was used to outline the various definitions of energy poverty, vulnerable groups, and CESs as well as the different strategies and approaches followed by European countries to tackle energy poverty and increase public engagement in CESs. This research suggests that environmental citizenship is a concept that can bring individuals and society together to promote changes in energy-related behaviours. The outcome reveals factors that enhance community energy acceptance and social cohesion such as providing energy education or education for sustainability to community members, financial support, increased prosocial culture, and the participation of vulnerable groups in CESs. This can be achieved through various interventions to help community members, including vulnerable groups, understand sustainable energy behaviour and thus reduce energy poverty.

This article aims to assess novel trends in science communication relating to how policymakers in the field of innovation and digitalisation policy consume and use scientific findings. We investigate the mutual influence that science communication and policy have on each other and answer the question how emerging science communication trends in the science-policy nexus might influence the use of scientific findings in the policymaking process. By using Google and the largest scholarly repositories, Google Scholar, ResearchGate, and Academia.edu, from 1st March to 31st May 2020, we reviewed policy documents and academic literature containing relevant information on the evolution of characteristics of global, European, and national science communication activities and the interrelated policy responses to identify the most relevant current trends in the evidence-to-policy process alongside three key challenges; trust, translation, and timing. The three identified main trends are (1) a stronger engagement between science and policy, (2) more open, reliable, and accountable science communication practices with policymakers, and (3) the increasing digitalisation and visualisation of science communication. We deepened our investigation by conducting online semi-structured interviews with relevant policy stakeholders at the international and national level between 1st May and 31st July 2020. With the support of the European Commission and building on the existing network of partners, we identified decision-makers and advisors with relevant experience in fields related to innovation and digitalisation policy working in four countries that represent different levels of generalized social trust: Austria, Italy, Hungary, and the Netherlands, and at the international/European Union level. After an online consultation process with a global set of policymakers, these theoretical findings were translated to policy recommendations showcasing possible solutions in science communication that may be initiated, strengthened, or continued by policy stakeholders in order to reach a more effective and efficient uptake of scientific findings in evidence-informed policymaking.

Background: This paper presents part of a wider research project called TRESCA[1] which aims to develop trust in science through the innovation of communication practices. Connected with the topic of trust in sciences, in terms of the credibility and reliability of scientific information, a part of the project was dedicated to the assessment of the explanatory power of two main elements of communication: audio and video. Particular attention was given to how these two elements relate to the perception of citizens, mediating with the latent imaginaries, emotional charges and value judgements that are the basis of the framing of relevant news, and thus with the ability of people to distinguish between correct and false communication. Methods: To investigate these aspects, an ad hoc workshop was designed and implemented in three European countries with the aim of understanding how people interpret the content of a scientific communication video with particular attention to the role of images and audio. Some probe questions were carefully selected to explore content and latent imaginaries, emotional and critical aspects also related with the trust of the communication. Results: By involving citizens in discussions and innovation efforts, many suggestions and recommendations have been collected. In today’s everyday life, where the visual is very widespread, thanks to the ever-growing presence of social media, the power of images can exceed that of audio. Conclusions: Since watching a video without sound can mislead the real content of the message, especially when the source or the speaker aren’t recognisable. This can have many repercussions on people’s ability to evaluate the truthfulness of a news and, consequently, on the choice to grant trust - or distrust.

Breast cancer (BC) is the most frequently diagnosed malignancy among women. In the past two decades, new technologies and BC screening have led to the diagnosis of smaller and earlier-stage BC (ESBC). Therefore, percutaneous minimally invasive techniques (PMIT) were adopted to treat patients unfit for surgery, women who refuse it, or elderly patients with comorbidities that could make surgery a difficult and life-threatening treatment. The target of PMIT is small-size ESBC with the scope of obtaining similar efficacy as surgery. Minimally invasive treatments are convenient alternatives with promising effectiveness, lower morbidity, less cost, less scarring and pain, and more satisfying cosmetic results. Ablative techniques used in BC are cryoablation, radiofrequency ablation, microwave ablation, high-intensity focused ultrasound (US), and laser ablation. The aim of our study is to discuss the current status of percutaneous management of BC, evaluate the clinical outcomes of PMIT in BC, and analyze future perspectives regarding ablation therapy in BC.

Micro-EDM is an unconventional technology used to machine every type of electrically conductive material regardless of its mechanical properties. Material removal occurs through electrical discharges between the workpiece and the electrode immersed in a dielectric fluid. In drilling operations, the technology is able to realise microholes with excellent quality in terms of precision, quality surface, roundness, and taper to the detriment of the machining time, which is less than other technologies. Several efforts are being made to improve different features related to the process performance that are severely affected by both the operative conditions, such as the electrode material or the type of dielectric, and process parameters. The typical indexes used to characterise the performance are the machining time, the material removal rate, and the geometric indexes. These indexes are very effective and are easily measurable, but they do not give information about the evolution of the drilling process, which could be irregular due to the different phenomena occurring during machining. The aim of this paper is the development of a method able to elaborate the motion law of the electrode during the micro-EDM drilling operation. In order to do this, a single hole was manufactured in several steps, recording both the machining time and electrode wear for each step. In this way, the actual position of the electrode during the drilling can be measured without the use of a predictive model for electrode wear. It was tested to confirm that the multistep procedure did not introduce new phenomena, in contrast to the traditional drilling operation. This method was used to study the effects of the electrode diameter, the type of electrode, the length of the electrode out of the spindle, and the entity of the run-out on the process performance. The tests were executed on titanium alloy sheets using a tungsten carbide electrode and hydrocarbon oil as the dielectric. It was found that the descent of the electrode into the workpiece was not regular, but it depended on the level of debris concentration in the machining zone. The debris concentration was influenced by the type and diameter of the electrode, its length out of the spindle, and, to a lesser extent, the run-out. This method was found to be a useful method for an in-depth analysis of the micro-EDM drilling process, contributing to a better understanding of the physical aspects of the process.

There is increasing interest in studying science communication from an institutional point of view. With much of the empirical research focusing on views of institutional actors on communication and their roles in the organisation, less attention has been paid to practices and dispositions of universities to communicate their research with publics. Universities have professionalised communication structures for external relations, and science communication has been absorbed in this. Yet, the evidence on what those practices represent for the university-at different levels of the organisation-is insufficient to understand the role of science communication within the university landscape. This study investigates science communication at central offices of research universities. Sampling whole populations of universities in four European countries (Germany, Italy, Portugal, and the United Kingdom; 44% response rate), we disentangle practices of communication as a centralised function. To the best of our knowledge, this is the first cross-national study on this topic based on all universities within the surveyed countries. We compare general trends in science communication of universities across countries. The evidence shows that science communication is a secondary function at central offices of universities, strongly medialised, and points to a supporting role for central structures in facilitating science communication at other levels while moving away from doing it themselves. Universities might need to consider their long-term positioning in enhancing national science culture by fostering science communication through models of dialogue and public debate.

Abstract In recent years, there has been an exponential growth in the use of digital media for data storage and the creation of museums, libraries and digital archives. In particular, Compact Discs Recordable (CD-R) have become one of the most used tools for archiving documents related to cultural heritage. Many authors have investigated only the CD longevity, but very few studies have focused on their biological deterioration. The aim of this work was to examine the CD-R biodeterioration following two ways: (1) search badly preserved CD-R, showing clear degradation spots and verify that it is fungal deterioration; (2) fungal inoculation tests on blank CD-R to verify their bioreceptivity. The analysis of a badly preserved CD-R showed clear degradation spots, highlighting, after cultural and molecular analysis, the presence on the surface of three different fungi: Chaetomium globosum, Trichoderma atroviride e Coniochaeta sp. For the in vitro inoculation tests we used conidia suspensions of Cladosporium cladosporioides and Penicillium chrysogenum, and we observed the growth for 6 months. At the end of experiment, both fungi colonized the CD-R surface with hyphae and mycelia clearly visible, causing also loss of material and fractal structure presence. For the CD-R use as cultural heritage archiving, the correct storage is an essential phase and, in light of the results obtained, it will be necessary not only to consider the environmental factors (temperature, relative humidity) but also the biological deterioration caused by airborne fungi. Therefore, effort needs to be taken to regularly monitor temperature and relative humidity and it is essential to periodically conduct cleaning of optical media.

This study analyzed data on the sources and the level of Italians’ awareness on the risk of infection by SARS-CoV-2 at the human–animal interface. Data were collected through a survey-type investigation on a representative sample of the Italian population. Forty-five percent of the interviewees were aware that companion animals could be infected by SARS-CoV-2. However, 29.8% were familiar with preventive measures to adopt to avoid viral transmission between infected humans and companion animals, and only 20.7% knew which companion animals could be at risk of infection. Higher awareness regarding the risk of SARS-CoV-2 transmission between animals and humans (51.7%) and the measures to prevent it (33.3%) was detected among companion animals’ owners. Notably, 40.4% of interviewees were not informed at all. Television broadcasts (26.4%) represented the main source of information, while only 3.5% of the interviewees relied on veterinarians, of which 31.9% considered this source of information as the most trustworthy. Overall, 72.4% of Italians recognized that the communication campaign on COVID-19 and companion animals was inadequate. This survey highlights the need for increasing the public awareness of the risk of companion animals being infected with SARS-CoV-2 and the involvement of professionals in the public communication on zoonoses.

Breast cancer is the most common forms of cancer and a leading cause of mortality in women. Early and correct diagnosis is, therefore, essential to save lives. The development of diagnostic imaging applied to the breast has been impressive in recent years and the most used diagnostic test in the world is mammography, a low-dose X-ray technique used for imaging the breast. In the first half of the 20th century, the diagnosis was in practice only clinical, with consequent diagnostic delay and an unfavorable prognosis in the short term. The rise of organized mammography screening has led to a remarkable reduction in mortality through the early detection of breast malignancies. This historical review aims to offer a complete panorama of the development of mammography and breast imaging during the last century. Through this study, we want to understand the foundations of the pillar of radiology applied to the breast through to the most modern applications such as contrast-enhanced mammography (CEM), artificial intelligence, and radiomics. Understanding the history of the development of diagnostic imaging applied to the breast can help us understand how to better direct our efforts toward an increasingly personalized and effective diagnostic approach. The ultimate goal of imaging applied to the detection of breast malignancies should be to reduce mortality from this type of disease as much as possible. With this paper, we want to provide detailed documentation of the main steps in the evolution of breast imaging for the diagnosis of breast neoplasms; we also want to open up new scenarios where the possible current and future applications of imaging are aimed at being more precise and personalized.

The idea of biological evolution is not accepted by many people around the world, with a large disparity amongst countries. Some factors may act as obstacles to the acceptance of evolution, such as religion, a lack of openness to experience, and not understanding the nature of science. Although the strength of the association between evolution acceptance and non-scientific factors varies among studies, it is often assumed that resistance to evolution is the byproduct of a religious background. Some studies are even more specific and try to associate the acceptance of evolution with precise religious affiliations. We aimed to explore the strength of associations among nationality, religion, and the acceptance of evolution by students using multiple correspondence analysis (MCA) and statistical tools, with nationwide samples from two different countries. Here, we show that wider sociocultural factors predict the acceptance of evolution to a higher degree than a religious background. We carried out two nationwide data collections that allowed us to compare differences in the acceptance of evolution in Italy and Brazil by high school students who declare to belong to the same religion in the two countries. Roman Catholic students showed significant differences between the two countries, and the gap between them was wider than between Catholics and non-Catholic Christians within Brazil. Our conclusions support those who argue that religious affiliation is not the main factor in predicting the level of evolution acceptance. The sociocultural environment and the level of evolutionary knowledge seem to be more important in this regard. These results open up new interpretative perspectives and provide a better understanding of attitudes towards evolution.

Emergency Radiology is a unique branch of imaging, as rapidity in the diagnosis and management of different pathologies is essential to saving patients’ lives. Artificial Intelligence (AI) has many potential applications in emergency radiology: firstly, image acquisition can be facilitated by reducing acquisition times through automatic positioning and minimizing artifacts with AI-based reconstruction systems to optimize image quality, even in critical patients; secondly, it enables an efficient workflow (AI algorithms integrated with RIS–PACS workflow), by analyzing the characteristics and images of patients, detecting high-priority examinations and patients with emergent critical findings. Different machine and deep learning algorithms have been trained for the automated detection of different types of emergency disorders (e.g., intracranial hemorrhage, bone fractures, pneumonia), to help radiologists to detect relevant findings. AI-based smart reporting, summarizing patients’ clinical data, and analyzing the grading of the imaging abnormalities, can provide an objective indicator of the disease’s severity, resulting in quick and optimized treatment planning. In this review, we provide an overview of the different AI tools available in emergency radiology, to keep radiologists up to date on the current technological evolution in this field.

Energy Justice (EJ) and particularly Energy equality (EE), arguably a radical conceptualization of energy justice, advocated for distributional justice and policies addressing distributional inequalities. Distributional policies are known to be contentious and often raise debates on the opportunity to interfere with the free-market allocation of goods in capitalistic economies. Whether EE inspired policies might be considered implementable or not depends on their social acceptability. Therefore, holding on to previous research findings pointing to the higher acceptability of equitable climate policies and the relationship between economic inequality and environmental degradation, we analyse EU data regarding income and income and wealth inequality and data from the H2020 ECHOES project, which consists of an extensive European survey of household energy consumption attitudes. We found that economic equality accounts for 41% of the variance explained at the country level of our sustainable energy care index (SECI), accounting for sustainable energy attitudes. We conclude that the interplay between economic equality and sustainable energy attitudes deserves further attention and might warrant a broader discussion about distributive policies within and beyond the energy sector.

Sporophores of a new Psathyrella species have been reported for the first time as growing at the base of Cladium mariscus culms in the Botanical Garden of the University of Calabria, Rende, Cosenza, southern Italy. The fungus was initially identified as P. thujina (= P. almerensis) by means of both ecology and macro- and microscopic characteristics of the basidiomes, then referred to P. cladii-marisci sp. nov. after extraction, amplification, purification and analysis of the rDNA ITS region. We came to this conclusion after comparing our specimen with the descriptions of the taxa available in the literature for the genus Psathyrella.

The social acceptability of wind farms has been researched for several decades now, with the first research tracing back to the 1980s. This paper aims to deliver a literature review within the structural framework proposed by the paper of variables influencing the acceptability of wind farms. The large amount of research published on the social acceptability of wind farms requires an effort to identify and categorise variables to deliver a holistic understanding of opposition and support to wind energy. We classify the variables into three main categories: first, ‘psychological variables’, including perceived benefits and costs, emotions, and attitudes; second, ‘contextual variables’, including community energy schemes and media influence; and finally, third, ‘personal resources’, including income or wealth, place of residence, and relevant knowledge in relation to the wind farm. In agreement with other scholars, we argue that NIMBYism (not in my backyard) is an outdated and simplistic explanation for opposition to wind farms. Instead, we provide a theoretical framework to explain the acceptability of wind energy and show how these variables might influence both acceptance and opposition.

Orchids are fascinating for many reasons: their reproductive strategies, their pollination systems and the various morphological adaptations they have evolved, including the presence of pollen grains agglomerated into two masses, called pollinia, which form a structure known as a pollinarium. After withdrawal from a flower, the pollinarium undergoes a bending movement such that the pollen masses become correctly orientated to strike the stigma. We evaluated the duration of pollinator visits to inflorescences and the effects of temperature on pollinaria reconfiguration in eight orchid species in order to analyze the effects of increasing air temperature on the changes in bending time, and thus on geitonogamy levels. The impact of temperature on insect behavior was not assessed because our priority was to understand the effects of temperature on the process of pollinaria reconfiguration. All the examined species showed natural reconfiguration times that were 1.7–3.0 times longer than the pollinator residency times. A higher temperature showed a reduction in bending time regardless of the species tested. However, the bending time was never shorter than the residence time of the insects on the flower. Our data showed that high temperatures had a limited effect on the pollinarium reconfiguration time, thus indicating that high temperatures had a limited effect on folding compared to the effect that it had on the viability of the pollen.
