Background: TBI is a leading cause of disability among veterans and active duty military personnel, and presents an obstacle to community reintegration. Prior studies examining adult survivors of TBI pursuing postsecondary education have methodological flaws that limit the understanding the scope and severity of sequelae experienced by persons with TBI who attend college. Objective: To describe (a) physical and cognitive functioning, and (b) postconcussion and mental health symptoms in veterans and military personnel (V/M) with traumatic brain injury (TBI) enrolled in postsecondary education programs after discharge from rehabilitation. Method: Cross-sectional study. Participants were recruited from five Veterans Affairs (VA) Polytrauma Rehabilitation Centers, enrolled in the VA TBI Model Systems parent study, and attending school during follow-up (N = 155). Outcome measures included the Functional Independence Measure (FIM), Neurobehavioral Symptom Inventory (NSI), Post-traumatic Stress Disorder (PTSD) Checklist-Civilian version (PCL-C), Patient Health QuestionnaireDepression (PHQ-9), and Generalized Anxiety Disorder Questionnaire (GAD-7). Findings: Participants were mostly male (92.9%) and White (81.4%), with mild (40.0%), moderate (11.5%), severe (34.5%), or very severe TBI (23.0%). Depression, anxiety, PTSD, and postconcussion symptoms were lowest in participants with very severe TBI and highest in those with mild TBI. There were no significant differences in FIM across TBI severity levels. Conclusion: This study supports the need for rehabilitation counselors, educators, and administrators to prepare future practitioners to deliver tailored services to student V/M with TBI. These services can facilitate successful community reintegration and transition into civilian school settings. Symptom profiling may inform personalized cognitive interventions to enhance these students' academic success.

Depressed youth frequently present with comorbid symptoms. Comorbidity is related to a poorer prognosis, including treatment resistance, academic problems, risk of suicide, and overall impairment. Studies examining the latent structure of depression support the notion of multiple presentations of depressed youth; however, it is unclear how these presentations are represented among acutely impaired youth. Participants (n = 457) in this naturalistic study were admitted to a psychiatric inpatient unit (Mean age = 14.33 years, SD = 1.94;76% female;46.6% Black/African-American). Selected subscales from the parent-report Behavior Assessment System for Children, Second Edition, were utilized as indicators in a latent profile analysis. Subgroups were validated based on their relationships with meaningful clinical correlates (e.g., family factors, discharge diagnosis) and further described by their associations with demographic variables. A five-class model provided the best balance of fit and parsimony. Subtypes of depressed youth included Predominantly Depressed (39.1%), Oppositional (28.2%), Severely Disruptive (12.3%), Anxious-Oppositional (11.6%), and Anxious-Withdrawn (8.8%). Comorbid symptoms were present in four of the five classes (60.9% of sample). High levels of externalizing symptoms were a prominent clinical feature associated with three classes (52.1% of the sample). Construct validity of the respective classes was demonstrated by differential association with clinical correlates, family characteristics, and demographics. Findings suggest that depressed youth presenting for acute inpatient psychiatric care displayed varied clinical presentations. The identified latent groups aligned with existing research reflecting comorbidity with anxiety, inattention, and externalizing disorders. Findings underscore the need for an increased clinical appreciation of comorbidity and encourage more targeted and effective prevention and treatment strategies. [ABSTRACT FROM AUTHOR]

Background: There is a limited evidence‐base describing clinical features of delirium in youth. What is known is largely extrapolated from studies of adults or samples with heterogeneous etiologies. It is unclear if the symptoms experienced by adolescents differ from those experienced by adults, or the degree to which delirium impacts the ability of adolescents to return to school or work. Objective: To describe delirium symptomatology among adolescents following a severe traumatic brain injury (TBI). Symptoms were compared by adolescent delirium status and across age groups. Delirium and its relationship with adolescent employability 1 year post‐injury was also examined. Design: Exploratory secondary analysis of prospectively collected data. Setting: Free‐standing rehabilitation hospital. Patients: Severely injured TBI Model Systems neurorehabilitation admissions (n = 243; median Glasgow Coma Scale = 7). The sample was divided into three age groups (adolescents, 16–21 years, n = 63; adults 22–49 years, n = 133; older adults ≥50 years, n = 47). Interventions: Not applicable. Measures: We assessed patients using Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM‐IV) diagnostic criteria and the Delirium Rating Scale‐Revised 98 (DRS‐R‐98). The employability item from the Disability Rating Scale was the primary 1‐year outcome. Results: Most items on the DRS‐R‐98 differentiated delirious from non‐delirious adolescents. Only "delusions" differed among age groups. Among adolescents, delirium status 1 month post‐TBI provided acceptable classification of employability prediction 1 year later (area under the curve [AUC]: 0.80, 95% confidence interval [CI]: 0.69–0.91, p <.001). Delirium symptom severity (AUC: 0.86, 95% CI: 0.68–1.03, SE: 0.09; p <.001) and days of post‐traumatic amnesia (AUC: 0.85, 95% CI: 0.68–1.01, SE: 0.08; p <.001) provided excellent prediction of outcomes for TBI patients in delirium. Conclusions: Delirium symptomatology was similar among age groups and useful in differentiating the delirium status within the adolescent TBI group. Delirium and symptom severity at 1 month post‐TBI were highly predictive of poor outcomes. Findings from this study support the utility of DRS‐R‐98 at 1 month post‐injury to inform treatment and planning. [ABSTRACT FROM AUTHOR]

Some of the most crucial components of concussion recovery are determining when a student incurs an injury, when to return the student to school, in what capacity the student returns, and what adjustments are needed in the process. It is important for school professionals to understand the signs and symptoms of a concussion so they may apply academic accommodations which align with the student's symptom profile. Effective concussion management seeks to enhance recovery while returning the student to full classroom activities as soon as they are able. Concussion symptoms can affect student learning, engagement, and academic achievement (Ransom et al., 2015). For example, cognitive symptoms like slowed processing can affect a student's ability to learn new information, engage in class activities, and perform well on assignments. Physical symptoms like headaches can affect a student's ability to concentrate in class. Students may experience emotional symptoms such as irritability, anxiety, and sadness. Concussed students may suffer from disturbed sleep, which may contribute to excessive daytime sleepiness and impact their ability to stay alert and engaged in the classroom. Furthermore, although most concussion symptoms resolve within 7-10 days, some students may have symptoms which persist for weeks to months (Eisenberg, Meehan, & Mannix, 2014). In summary, the symptoms of a concussion (e.g., headache, slowed processing, irritability, fatigue, sensitivity to light and noise) may negatively impact student participation and achievement in the learning environment (Ransom et al., 2015). The goal of concussion response and management is to mitigate symptom intensity and duration while facilitating recovery. This article discusses the importance of identifying when a student injury occurs and how to assemble an effective school-based concussion team.

We investigated 109 (79.8% female; 76% White, and 83.5% Heterosexual) mental health trainees’ explicit and implicit attitudes toward heterosexual, lesbian, and gay White couples adopting and raising Black children. To determine explicit attitudes, we used a vignette depicting a Black child ready for adoption and three types of equally qualified White families who were headed by a heterosexual couple, gay couple, or lesbian couple. The trainees were asked to indicate which type of family they preferred to adopt the child. To determine implicit attitudes, we used the computer programed latency-based multifactor implicit association test (IAT) protocol. The IAT data were collected from each participant individually. Explicit data showed that over 80% of the participants indicated no strong preference in terms of which type of family should adopted the child. However, IAT data showed that the trainees implicitly preferred lesbian couples. Overall, the degree of congruence between explicit and implicit was very low.
