Alpha-1 antitrypsin deficiency (AATD) is a genetic condition that predisposes a person to certain diseases over their lifetime, mainly including lung disease (in the form of emphysema) and liver disease (liver cirrhosis). Quality of life questionnaires are instruments designed to quantify the deterioration of a patient's health. Background/Objectives: This study aimed to assess whether certain quality of life tests that are routinely used in clinical practice can be useful for patients with AATD. Methods: A sample of AATD patients, with various genotypes, but with the common characteristic that they must have both altered alleles (Pi* ≠ M), participated in the study. Different quality of life tests were used, including the COPD Assessment Test (CAT), COPD and Asthma Sleep Impact Scale, the short form of the Short Form Health Survey, and EuroQol 5 dimensions, and were related to differing clinical and functional characteristics. Results: The sample was composed of 54 patients, and slightly more than half of the participants were women (57.4%), with a mean age of 51.5 ± 13.7. The main genotypes were Pi*SZ (43.4%) and Pi*ZZ (34%). In patients under 65 years of age (n = 47), those who were actively working could walk a greater distance in the walking test, namely, 573 m (511–629), compared to those who were not actively working, namely, 415.5 m (392–469; p < 0.001). Active non-workers had a worse CAT (13.6 ± 7.8 vs. 4.6 ± 4.3; p < 0.001). In total, 80% of non-working patients had exacerbations, but only 46. 9% of those who were active, although the association did not reach statistical significance (p = 0.068). Having a lower score in the physical component of SF-12 was related to suffering from lung disease (46.0 ± 11.4 vs. 38.4 ± 11.1 (p = 0.026)). Conclusions: Quality of life tests were able to detect differences and relate them to functional factors such as the distance covered in the walking test, being sensitive and specific in this regard. INTENSIVE care units ,QUALITY-adjusted life years ,COST effectiveness ,FOOD consumption ,NUTRITION - Abstract
Introduction: Hospital malnutrition is very common and worsens the clinical course of patients while increasing costs. Lacking clinical-economic studies on the implementation of nutrition screening encouraged the evaluation of the CIPA (Control of Food Intake, Protein, Anthropometry) tool.Material and Methods: An open, non-randomized, controlled clinical trial was conducted on patients admitted to internal medicine and general and digestive surgery wards, who were either assigned to a control (standard hospital clinical care) or to an intervention, CIPA-performing ward (412 and 411, respectively; n = 823). Length of stay, mortality, readmission, in-hospital complications, and quality of life were evaluated. Cost-effectiveness was analysed in terms of cost per quality-adjusted life years (QALYs).Results: The mean length of stay was higher in the CIPA group, though not significantly (+ 0.95 days; p = 0.230). On the surgical ward, more patients from the control group moved to critical care units (p = 0.014); the other clinical variables did not vary. Quality of life at discharge was similar (p = 0.53), although slightly higher in the CIPA group at 3 months (p = 0.089). Patients under CIPA screening had a higher mean cost of € 691.6 and a mean QALY gain over a 3-month period of 0.0042. While the cost per QALY for the internal medicine patients was € 642 282, the results for surgical patients suggest that the screening tool is both less costly and more effective. Conclusions: The CIPA nutrition screening tool is likely to be cost-effective in surgical but not in internal medicine patients. Information technology ,T58.5-58.64 ,Public aspects of medicine ,RA1-1270 - Abstract
BackgroundType 2 diabetes mellitus (T2DM) is a chronic disease in which health outcomes are related to decision making by patients and health care professionals. ObjectiveThis study aims to assess the effectiveness of internet-based multicomponent interventions to support decision making of all actors involved in the care of patients with T2DM in primary care. MethodsThe INDICA study is an open, community-based, multicenter trial with random allocation to usual care or the intervention for patients, the intervention for health care professionals in primary care, or the combined intervention for both. In the intervention for patients, participants received an educational group program and were monitored and supported by logs, a web-based platform, and automated SMS. Those in the intervention for professionals also received an educational program, a decision support tool embedded in the electronic clinical record, and periodic feedback about patients’ results. A total of 2334 people with T2DM, regardless of glycated hemoglobin (HbA1c) levels and without diabetes-related complications, were included. The primary end point was change in HbA1c level. The main analysis was performed using multilevel mixed models. ResultsFor the overall sample, the intervention for patients attained a significant mean reduction in HbA1c levels of ‒0.27 (95% CI ‒0.45 to ‒0.10) at month 3 and ‒0.26 (95% CI ‒0.44 to ‒0.08) at month 6 compared with usual care, which remained marginally significant at month 12. A clinically relevant reduction in HbA1c level was observed in 35.6% (191/537) of patients in the intervention for patients and 26.0% (152/586) of those in usual care at month 12 (P=.006). In the combined intervention, HbA1c reduction was significant until month 18 (181/557, 32.6% vs 140/586, 23.9%; P=.009). Considering the subgroup of patients uncontrolled at baseline, all interventions produced significant reductions in HbA1c levels across the entire study period: ‒0.49 (95% CI ‒0.70 to ‒0.27) for the intervention for patients, ‒0.35 (95% CI ‒0.59 to ‒0.14) for the intervention for professionals, and ‒0.35 (95% CI ‒0.57 to ‒0.13) for the combined intervention. Differences in HbA1c for the area under the curve considering the entire period were significant for the intervention for patients and the combined intervention compared with usual care (P=.03 for both). Compared with usual care, the intervention for professionals and the combined intervention had significant longer-term reductions in systolic and diastolic blood pressure. ConclusionsIn uncontrolled patients, the intervention for patients at baseline provided clinically relevant and significant longer-term reductions of HbA1c levels. The intervention for professionals and combined intervention also improved the cardiovascular risk profile of patients. Acute respiratory failure ,Neurally adjusted ventilatory assist ,Ventilator-free days ,Lung-protective ventilation ,Assist ventilation ,Liberation from mechanical ventilation - Abstract
Background: Patient-ventilator asynchrony is a common problem in mechanically ventilated patients with acute respiratory failure. It is assumed that asynchronies worsen lung function and prolong the duration of mechanical ventilation (MV). Neurally Adjusted Ventilatory Assist (NAVA) is a novel approach to MV based on neural respiratory center output that is able to trigger, cycle, and regulate the ventilatory cycle. We hypothesized that the use of NAVA compared to conventional lung-protective MV will result in a reduction of the duration of MV. It is further hypothesized that NAVA compared to conventional lung-protective MV will result in a decrease in the length of ICU and hospital stay, and mortality. Methods/design This is a prospective, multicenter, randomized controlled trial in 306 mechanically ventilated patients with acute respiratory failure from several etiologies. Only patients ventilated for less than 5 days, and who are expected to require prolonged MV for an additional 72 h or more and are able to breathe spontaneously, will be considered for enrollment. Eligible patients will be randomly allocated to two ventilatory arms: (1) conventional lung-protective MV (n = 153) and conventional lung-protective MV with NAVA (n = 153). Primary outcome is the number of ventilator-free days, defined as days alive and free from MV at day 28 after endotracheal intubation. Secondary outcomes are total length of MV, and ICU and hospital mortality. Discussion This is the first randomized clinical trial examining, on a multicenter scale, the beneficial effects of NAVA in reducing the dependency on MV of patients with acute respiratory failure. Trial registration ClinicalTrials.gov website (NCT01730794). Registered on 15 November 2012. Acute respiratory distress syndrome ,Dexamethasone ,Corticoids ,Positive end-expiratory pressure ,Lung-protective ventilation - Abstract
Background: Although much has evolved in our understanding of the pathogenesis and factors affecting outcome of patients with acute respiratory distress syndrome (ARDS), still there is no specific pharmacologic treatment for ARDS. Several clinical trials have evaluated the utility of corticoids but none of them has demonstrated a definitive benefit due to small sample sizes, selection bias, patient heterogeneity, and time of initiation of treatment or duration of therapy. We postulated that adjunctive treatment of persistent ARDS with intravenous dexamethasone might change the pulmonary and systemic inflammatory response and thereby reduce morbidity, leading to a decrease in duration of mechanical ventilation and a decrease in mortality. Methods/design This is a prospective, multicenter, randomized, controlled trial in 314 patients with persistent moderate/severe ARDS. Persistent ARDS is defined as maintaining a PaO2/FiO2 ≤ 200 mmHg on PEEP ≥ 10 cmH2O and FiO2 ≥ 0.5 after 24 hours of routine intensive care. Eligible patients will be randomly allocated to two arms: (i) conventional treatment without dexamethasone, (ii) conventional treatment plus dexamethasone. Patients in the dexamethasone group will be treated with a daily dose of 20 mg iv from day 1 to day 5, and 10 mg iv from day 6 to day 10. Primary outcome is the number of ventilator-free days, defined as days alive and free from mechanical ventilation at day 28 after intubation. Secondary outcome is all-cause mortality at day 60 after enrollment. Discussion This study will be the largest randomized controlled clinical trial to assess the role of dexamethasone in patients with persistent ARDS. Trial registration Registered on 21 November 2012 as DEXA-ARDS at ClinicalTrials.gov website (NCT01731795). sexting ,autoestima ,género - Abstract
In this paper we intend to study the relationship between sexting and different variables such as gender, self-esteem, age and having or not having a partner. In this way, the relationship between sexting, gender and self-esteem is investigated, on the one hand, and, on the other hand, between sexting, gender and having or not having a partner. In the same way, the relevance of age for this behavior is explored and, lastly, the relationship that gender has with different sexting behaviors such as active disposition, real participation and emotional expression. In the present investigation, 302 people aged between 18 and 58 participated. An analysis of variance has been carried out, finding that men show greater disposition and emotionality for this behavior and that they perform it more when they have high or medium self-esteem. In terms of frequency, it was shown that women and men participate in the same way, increasing the frequency when they have a partner. Lastly, age has a significant influence, producing an increase up to 24 years of age and a decrease after that age, both in men and women.

Inclusive education is necessary in every educational stage, from elemental to postsecondary. Throughout the years, education for deaf and hard-of-hearing people has evolved to including adaptation programs inside college settings as is the case of the Universidad de La Laguna, that has the PAED program (Attention Program for Students with Educational Aid Needs). The objective of this investigation was to know deaf and hard-of-hearing students’ perceptions of the adaptation program through the CCES (Classroom Communication Ease Scale) questionnaire, questions relative to the program, their perception of their integration in their families and social groups and their perception of the usefulness and dependence on Sign Language Interpreters. The sample was comprised of four students of the Universidad de La Laguna with hearing impairments that also benefited from the adaptation program. The results indicated that the program was seen as necessary for students with hearing impairment, even though the program should improve in keeping up with the students and in the access to material.

The Covid-19 pandemic has caused a major change in people's mental health and social relationships; more specifically, the confinement had major negative consequences for the population, such as increased cases of depression and anxiety (among other disorders). There have been multiple investigations on how the stress and anxiety generated by the coronavirus may have influenced people's behavior, relationships and psychological well-being. However, much of this research has occurred at times when many of the social behaviors were limited by confinement or restraints. The aim of this research is to study the relationship between these variables once all restrictive measures have been relaxed. The study variables were assessed by means of different questionnaires, more specifically: Covid-19 specific anxiety, assessed by the CAS and the C-19-A and interpersonal relationships in the Spanish population, for which the INQ-15 subscale and an ad-hoc developed questionnaire were used. In a sample of 104 participants obtained in May 2022 with a mean age of 29 years, no relationship was found between coronavirus-specific anxiety and satisfaction with social relationships or with the number of social interactions. The levels found at the time the study was conducted indicate that low levels of anxiety were already present in relation to Covid-19

Students face stressors that place them in a situation of psychological vulnerability, although the symptoms associated with this situation are of low clinical intensity. Evidence shows that the use of new media and communication technologies offer adequate resources to maintain and improve mental health. In order to know the users' opinion about these resources, the aim of this study is to observe the willingness of Spanish students to use a psychological assistance service offered by an Artificial Intelligence trained chatbot compared to when this service is offered in the same way but by a human professional, and the expectations of the service presented. The sample of 101 students was randomly divided into two experimental groups, one group was presented with an example of an AI-trained chat service (n= 55%), and the other group with a human professional (n= 46%). Both completed an online form consisting of a dichotomous response question about willingness to use the service and an adhoc Expectations Questionnaire. The results show a high willingness to use a chat counselling service (n= 69.3%), and the level of acceptance was not related to the type of chat offered (human vs artificial intelligence) (χ²(1) = 0.84; p = .359). Nor were significant differences found between the experimental groups in expectations about the service presented (t (1, 99) = 0.17, p = .87; d = 0.03), although a higher level of expectations (t (99) = -6.53, p < .001, d = -1.408) can be observed in those willing to participate (M = 81.1; SD = 15.2) compared to those not (M = 60.3; SD = 13.7). These data show favorable conditions for implementing chat support services for students, in addition to proposing new resources from the hand of Artificial Intelligence, taking advantage of its multiple advantages.

Negative beliefs about old age are relevant not only because they influence the physical and mental health of older people but also the treatment they receive as a result of such stereotypes. Moreover, the perception of old age itself is changing in a society that is moving towards an ageing population. The relationship between the level of stereotyping and the self-perception of ageing in middle age (range 40-60 years) was studied with a sample of 111 participants with an average age of 53 years, using four questionnaires: ERA-12 and CENVE for stereotypes about old age; B-APQ and ATOA for selfperception of ageing. Those who had lower levels of negative stereotypes of old age had a more positive view of their own ageing and considered those over 75 to be capable of making decisions without prior assessment. This study supports the idea that decreasing stereotypes towards the elderly could have an impact on a better perception of one's own ageing, which in turn influences better physical and mental health.

Contacto ,Dependencia ,Edadismo - Abstract
In the following work, we intend to know if negative stereotype beliefs towards old age (ageism) are lower in people with regular contact with people over 65 years of age. To do this, a form was given to people between 18 and 64 years old, with a scale to measure the level of dependency of the older contact person (Lawton and Brody) and another to know the level of ageism of each participant (CENVE). . From the results found, it can be concluded that people with regular contact with independent older people have lower levels of ageism, and that these increase according to the level of dependency of the older people with whom they have contact.

In the following study it is explored the relation between the following variables: the conflict that the children perceived in their parents relation after a divorce or separation process. The contact they had right after the breakup with each parent and the communication they actually have.It was applied in online format a small adaptation of the Family Communication Scale (Barnes and Olson, 1982) and the Interparental Conflict Scale from the children perspective (Grych et al., 1992) to a sample of 69 young adults whom parents went through a divorce or separation process during the participants childhood or adolescence. It was found significant differences in the communication they currently have with their male parents with respect to the contact frequency they previously to the divorce or separation had. This difference was marginally significant only when it was analyzed by the multivariable regression analysis. At 3 the same time, there exists a relation between the perceived conflict variables and the grade of contact: when there was perceived more conflict in the process, there was less contact with the male parent. From this study it could be concluded that the frequency that children have contact with their parents after a divorce or separation seems to be related with the future quality of their communication with them. So, this must be considered whenever we have to come through these situations

Immigration from Africa to the Canary Islands is a common phenomenon, although it occurs intermittently. In 2020, the migratory process was more particular than in previous years due to the massive arrival in such a short space of time, and this situation was compounded by the pandemic caused by COVID19. The aim of this study was to analyse the Canarian population's perception of this irregular immigration, by means of a scale of perception of threat from the exgroup and a questionnaire that evaluated the willingness to allocate public resources to immigrants. A total of 249 people living in the Canary Islands took part. The results indicated that there was not a high level of threat perception in the Canarian population, although it was higher among those with lower levels of education. Those participants who reported higher levels of perceived threat tended to recognise a greater sense of insecurity and to say that immigrants should be resettled on the mainland or on other islands.

The current study tested the relationship between watching streams of videogames and sensitivity to violence. Participants were randomly assigned to watch a non violent or a violent videogames stream. After watching the video, measures of identification with the player & the game character and perception of violence from the player & game were taken. The analyses didn’t detect any differences between the violent game stream and the non violent game stream. The hypothesis that watching violent videogames stream would desensitize to violence was not supported. Subsequent analysis found a relationship between not playing videogames more sensitive to violence.
