Abstract

The lack of electronic health record data in general and especially at initial phases of medical research projects is common and is one of the main reasons for delay or failure of such projects. One of the health areas with little attention is the home care area, where patients are being supported by their families or informal caregiver at home. In this paper we present related work on medical data formats and synthetical data generation of medical health records. Furthermore, it presents an approach to generate synthetic electronic health records (HER) that are readily available; suited to research; and free of legal, privacy, security and intellectual property restrictions to be used in home care research projects. We adapted and used SyntheaTM, an open-source software framework that simulates the lifespans of synthetic patients to generate synthetic EHRs. This paper presents the use case of home care from the capturing of user requirements of home care patients, translating the requirements into a data model, feeding the data model into Synthea (TM) framework, which produces synthetical health data records mainly as QuestionnaireResponse instance of the Fast Healthcare Interoperability Resources (FHIR) to using these EHRs to build an initial machine learning data model for home care.

Abstract

INGE steht für digitale Integrierte Gesundheits- und Pflegeversorgung mit IT-gestütztem Pflegeberatungsbesuch nach § 37 Abs. 3 SGB XI. Dahinter steckt die Idee ein Onlineportal und eine App zu entwickeln, Beobachtungen der häuslichen Pflegesituation und Erfassung von Problembereichen beinhalten und Pflegeberater*innen bei der Dokumentation der häuslichen Pflegesituation unterstützt. Gleichzeitig werden Unterstützungsangebote für den*die pflegende Angehörige aufgezeigt. Derzeit werden die Informationen jedoch meist nur partiell und in Papierform dokumentiert. Dies kann zu Informationsverlusten führen und die Weitergabe an andere Mitversorger*innen ist erschwert. An diesem Punkt setzt auch das Projekt INGE integrate4care an. Hier steht die Verbesserung der sektorübergreifenden Begleitung, also die Versorgung mit ambulanten und stationären Leistungen, von Pflegebedürftigen, die zu Hause von Angehörigen gepflegt werden, im Zentrum. INGE hat das Ziel, mittels einer App (INGE-App) die Dokumentation des gesetzlich verankerten Pflegeberatungsbesuches zu digitalisieren. Konkret bedeutet dies, dass die Pflegeberater*innen die INGE-App im Beratungsbesuch nutzen und alle Informationen digital erfassen.

Abstract

Hintergrund: Die sektorübergreifende Koordination von Behandlungsplänen und ein effizientes Management von Patienten (Pat.) mit Komorbiditäten sind von großer Bedeutung. Dies betrifft auch Pat. mit rheumatoider Arthritis (RA). Eine Orchestrierung der an verschiedenen Stellen über einen Pat. verfügbaren Informationen ist von großer Bedeutung, um eine effektive Datennutzung, sinnvolle Managementprozesse, Vermeidung redundanter Diagnostik und folglich Kosteneffizienz zu gewährleisten. Die im Rahmen des Horizon2020- geförderten PICASO-Projekts (www.picaso-project.eu) entwickelte Informations- und Kommunikationsplattform unterstützt die Orchestrierung von Pat.-Daten entlang des Versorgungskontinuums bestehend aus Krankenhäusern, Ambulanzen, Praxen, nicht-ärztlichen Gesundheitsdienstleistern bis zum häuslichen Pat.-Monitoring. Methodik: PICASO verfolgt einen nutzerzentrierten Ansatz, d.h. durch Workshops und Interviews mit Ärzten verschiedener Disziplinen, Pat. und weiteren Stakeholdern des Gesundheitswesens (z.B. Datenschutzbeauftragte) wurden Anforderungen an die Plattform ermittelt. Ethische Fragestellungen bearbeitet ein Begleitgremium, dem auch externe Mitglieder und Pat. angehören. Ein umfangreiches Framework dient der Evaluation (u.a. zu Nutzerzufriedenheit, klinischer Relevanz der Entwicklungen) der 6-monatigen Proof-of-concept Studie mit RA-Pat. und Ärzten. Ein positives Ethikvotum der Ethikkommission der Medizinischen Fakultät der Heinrich-Heine-Universität Düsseldorf liegt vor. Ergebnisse: Zur Entwicklung der Plattform wurden 111 identifizierte Nutzeranforderungen herangezogen. Zudem wurde auf Konformität mit der europäischen Datenschutzgrundverordnung sowie nationalen Regelungen in Deutschland geachtet. Um den Datenaustausch mit anderen Softwaresystemen im Gesundheitswesen zu ermöglichen, basieren alle Entwicklungen auf dem neuen HL7 Standard 'Fast Healthcare Interoperability Resources' (FHIR). Dieser bietet viele Vorteile wie ein semantisches Modell zur Beschreibung kleinster Einheiten (z.B. Medikationseinnahmezeitpunkte, diagnostische Verfahren), sodass Informationen hierüber miteinander verknüpft und bereitgestellt werden können. Dabei verbleiben die Daten beim Dateneigner. Rollenspezifische Datenzugriffe sind möglich. Bislang wurden 27 RA-Pat. (81,5% weiblich) rekrutiert, das mittlere Alter betrug 59,2±10,9 Jahre, die Krankheitsdauer 13,2±8,7 Jahre. Neben dem positiven Feedback bei Rekrutierung können während des DGIM-Kongresses Ergebnisse der begonnenen Nutzerevaluationen präsentiert werden. Zusammenfassung: Die PICASO-Plattform bietet eine sektorübergreifende Orchestrierung von Pat.-Daten und damit moderne Möglichkeiten für sinnvolle Managementprozesse (z.B. Treat-to-Target, Telemonitoring). Sie soll für RA-Pat. aber auch für eine Anwendbarkeit bei anderen chronischen Krankheiten verfügbar sein und hat das Potenzial, das Gesundheitssystem zu verändern.

Abstract

A range of accessibility evaluation tools are available to support Web developers in producing accessible Web resources. It is important that these tools support web developers very well, so that the developers can concentrate on the tasks of Web development. In addition, the tools should enhance Web developers understanding of accessibility issues. This paper presents a usability evaluation of five entry level accessibility evaluations tools. A group heuristic evaluation was conducted, with 5 experts in usability and accessibility working through each tool together, but rating usability problems separately. The results showed that the usability of these tools is surprisingly poor and that they do not support web developers adequately in checking the accessibility of their web resources. Abstract

Internet portals are gateways to the World Wide Web, which offer an amalgamation of services, like search engines, online shopping information, email, news, weather reports, stock quotes, community forums, maps, travel information, etc. Furthermore, with the arrival of the Mobile Web, they are also frequently used in Ambient Intelligence scenarios. This paper will discuss basic design considerations inspired by systems theory fundamental principles, where the portal as a whole and its components (known as portlets) are analyzed. Abstract

Background: The in-home care consultation visits according to § 37.3 of the Social Security Statute Book XI have been carried out inconsistently so far and with a lack of structured care documentation. In the INGE-integrate4care project, an app was developed to support the quality assurance of visits., Method: Based on a user-centered design approach, 10 semistructured expert interviews were conducted and analyzed using qualitative content analysis according to Mayring. In group-based expert walkthroughs with 7 participants, the design, contents and functions of the app were discussed, requirements were collected in feedback lists and digitally implemented by developers., Results: Interviewees reported that visits are heterogeneous, that there are individual approaches and that this should be reflected in the design of the app. Important topics for the app were, e.g. contents of the NBA and the burden on caregivers. Functions such as the recommendation of measures on the basis of documented information are desirable. On the other hand, too stringent guidelines of the app would be an obstacle, as these would limit the conversational character of the visit. The NBA and BIZA‑D were chosen by the experts in the project as suitable basic assessments and adapted to the visits. Flexible access to items, visualization of care status per category, and the ability to select and track measures were specified to support workflow., Discussion: The user-centered design approach enabled the development of an app that is characterized by both flexibility and structure. Abstract

CAREPATH project is focusing on providing an integrated solution for sustainable care for multimorbid elderly patients with dementia or mild cognitive impairment. The project has a digitally enhanced integrated patient-centered care approach clinical decision and associated intelligent tools with the aim to increase patients' independence, quality of life and intrinsic capacity. In this paper, the conceptual aspects of the CAREPATH project, in terms of technical and clinical requirements and considerations, are presented.
