3. Does Prior Percutaneous Coronary Intervention Influence the Outcomes of Coronary Artery Bypass Surgery?

Abstract

Abstract Introduction: Percutaneous coronary intervention (PCI) has been increasingly performed to treat coronary artery disease. The performance of multiple PCI has also been increasing. Consequently, the percentage of patients presenting for coronary artery bypass graft (CABG) surgery is reported to vary from 13 to 40%. The influence of previous PCI on CABG outcomes has been studied in single center, regional studies, registries and meta-analyses. Some reports showed a negative effect on mortality and morbidity in early or long-term follow-up, but others did not find this influence. Methods and Results: A cohort of 3007 patients consecutively operated for CABG, 261 of them with previous PCI, were included in this analysis. Comparison of the groups "previous PCI" and "primary CABG" was made in the original cohort and in a propensity score matched cohort of 261 patients. There were some differences in preoperative clinical characteristics in both types of cohort, even in the matched one. Outcomes were compared at 30 days, 1 year and 5 years of follow-up. There were no statistically significant differences in mortality in any period or cohort. There were some differences in other outcomes as readmission and composite events, including cardiovascular death at 1 and 5 years of follow-up. These differences, neverthless, were not confirmed in comparison with the matched cohort. Conclusion: Although there are some limitations in this study, it was not found consistent negative influence of previous PCI on CABG.

An integrable classical two-spin system is considered with characteristics which are particularly suited for elucidating the properties of integrable systems with two degrees of freedom. The foliation of phase space by the rational and irrational tori is shown with projections onto a particular phase space plane, an expression for the winding number is obtained, and the dense coverage of the irrational tori by the quasiperiodic trajectories is easily demonstrated for this system.

Abstract

Changes in cellular lipid metabolism are a common feature in most solid tumors, which occur already in early stages of the tumor progression. However, it remains unclear if the tumor-specific lipid changes can be detected at the level of systemic lipid metabolism. The objective of this study was to perform comprehensive analysis of lipids in breast cancer patient serum samples. Lipidomic profiling using an established analytical platform was performed in two cohorts of breast cancer patients receiving neoadjuvant chemotherapy. The analyses were performed for 142 patients before and after neoadjuvant chemotherapy, and the results before chemotherapy were validated in an independent cohort of 194 patients. The analyses revealed that in general the tumor characteristics are not reflected in the serum samples. However, there was an association of specific triacylglycerols (TGs) in patients' response to chemotherapy. These TGs containing mainly oleic acid (C18:1) were found in lower levels in those patients showing pathologic complete response before receiving chemotherapy. Some of these TGs were also associated with estrogen receptor status and overall or disease-free survival of the patients. Abstract

Intra-tumor heterogeneity concerns the existence of genetically different subclones within the same tumor. Single sample quantification of heterogeneity relies on precise determination of chromosomal copy numbers throughout the genome, and an assessment of whether identified mutation variant allele fractions match clonal or subclonal copy numbers. We discuss these issues using data from SNP arrays, whole exome sequencing and pathologist purity estimates on several breast cancers characterized by ERBB2 amplification. We show that chromosomal copy numbers can only be estimated from SNP array signals or sequencing depths for subclonal tumor samples with simple subclonal architectures under certain assumptions.

Abstract

Introduction: Peptic Ulcer Disease (PUD) is the most common diagnosis for upper abdomen pain, and it includes ulcerations and erosion in the stomach and duodenum. Complication such as Perforated PUD (PPUD) is only second to bleeding and remains a life-threatening emergency. Aim: To compare the American Association for the Surgery of Trauma-Emergency General Surgery (AAST EGS) scoring system with a widely accepted Peptic Ulcer Perforation (PULP) scoring system for PULP to determine the discriminative capacity and pairwise comparison of both scoring systems. Materials and Methods: This was a retrospective study conducted from September 2018 to August 2020 at Department of General Surgery, PES institute of Medical Science, Kuppam, Andhra Pradesh, India. All the adults diagnosed with perforated PUD were included. Preoperative, Intraoperative, and postoperative data were collected. The scores were generated for PULP and AAST EGS grades and analysed using Statistical Package for Social Sciences (SPSS) version 17.0. Spearman’s rho test evaluated a comparison of each variable with the AAST EGS grade. The pairwise comparison was performed for complication development, patient duration of stay, mortality and described using the Area Under the Receiver Operating Characteristic (AUROC) with 95% Confidence Intervals (CI). Results: This study included 165 patients in this 87% male with a mean age of 56.89±16.79 years. All the total patients were divided into those ≥50 years (n=56, mean age: 36.8±9.04 years) and >50 years (n=109, mean age: 67.2±8.4 years) and comparative analysis was performed accordingly. Overall, the patients were categorised into the following AAST EGS grade I (9, 5.5%), grade II (99, 60%), grade III (42, 25.45%), grade IV (15, 9%), there were no patients with grade 5 AAST EGS. The AAST EGS grade was comparatively better them PULP score for postoperative complications, but there is not much difference between the PULP score and AAST EGS score for the patient’s duration of stay. AAST EGS grade and the PULP discriminated, patient 30-day mortality similarly, but if Area Under Curve (AUC) >0.8, it is a good predictor. Conclusion: The AAST EGS scoring system and the PULP scoring system do similarly predict mortality and complication. But the presence of many variables with points and tabulation which requires laboratory investigation makes PULP scoring system inconvieninent at bedside.

Abstract

World artillery in the present scenario is changing its role from defensive to aggressive nature and is attempting to achieve higher penetration into enemy targets. Even for an autofrettaged gun barrel, higher ranges requirement leads to higher barrel weight and its associated demerits. The design of gun barrel is based on the choice of yield criteria. Tresca yield criterion provides conservative design for a ductile barrel material. On the other hand, more accurate von Mises criterion presents complexity. The two criteria to evaluate various parameters required for design of an autofrettaged gun barrel are compared. The methodology for evaluation of maximum safe pressure, based on von Mises criterion, for an autofrettaged gun barrel is also included in the paper. Based on case study included in the article, for an autofrettaged gun barrel or pressure vessel with uniform thickness, a theoretical weight reduction of approximately 16 per cent is feasible with von Mises criterion as compared to Tresca criterion. Abstract

The article describes an iterative algorithm for sparcity enforcing near-field acoustical holography over a wide frequency range based on the application of an optimized pseudo-random array geometry. Topics discussed include the method called wideband holography (WBH) as an example of remote sensing. Also mentioned are WBH being tested by real measurements on a pair of two-way loudspeakers and a car engine as well as typical application areas, such as engines and gearboxes.

In industrial products, vibration and noise are often major parameters that need to be considered. It may be necessary to measure material properties, such as stiffness and damping, to determine how vibrations are generated, transmitted, and damped. This article describes a procedure that will allow the standard method for the determination of complex modulus to be used at higher frequencies than otherwise possible.
