Abstract

The Legouest military training hospital is one of the eight hospitals of the armed forces health service. Situated in the Grand-Est region, one of the regions most affected by the COVID-19 epidemic in spring, it had to reorganise itself within a few days with its regional and national partners. While continuing to support forces sent abroad, to overseas territories or located in the East of France, the armed forces hospital had three major missions: the support of other military hospital facilities, the continued care of non-COVID patients and the care of patients affected by COVID-19 requiring non-intensive hospital care.

Despite the recent emergence of COVID-19 and the absence of a specific cure, the hospital treatment of patients affected by this virus is well established. It is based on symptomatic treatments including oxygen therapy, physiotherapy and anticoagulation therapy. The treatment plan and potential therapeutic limitations must be well defined and adapted to the severity, predisposition and wishes of the patient.

Abstract

This worldwide health crisis forced hospitals to adapt their activities to this new confinement environment. Innovation was required to maintain a social bound between cohorting service's patients and their families, to guarantee a dedicated listening time and prevent as much as possible from the isolation due to this COVID-19 unusual situation. With that aim, the army training hospital Legouest created an information and support listening cell dedicated to COVID-19 hospitalized patients' families. It also provided several tablets for creating a patient and family digital gate.

Abstract

The SARS-Cov-2 pandemic took health care players across the world by surprise, including caregivers, managers and logisticians. In the absence of a vaccine or a specific therapy, detecting the disease at an early stage and isolating confirmed cases from the rest of the healthy population was soon considered to be essential. The RT-PCR technique was the gold standard method for testing for infection with the virus. It has high specificity but moderate sensitivity. It was difficult to access it during the first weeks of the pandemic. On the basis of the observations of the Chinese and Italian scientific community in particular, the recommendation was to combine use of the RT-PCR with a low-dose thoracic CT scan. In the absence of easy access to the RT-PCR, the use of the lung CT scan for a rapid triage of patients within emergency departments proved effective.

Abstract

Background Pulmonary arterial hypertension (PAH) is a rare complication of systemic lupus erythematosus (SLE). Methods We identified all patients with SLE and PAH (SLE-PAH) who were enrolled in the French Pulmonary Hypertension Registry with a diagnosis confirmed by right heart catheterization (RHC). A control group of 101 patients with SLE without known PAH was selected from SLE expert centers participating in the Pulmonary Hypertension Registry. Survival was estimated by the Kaplan-Meier method. Hazard ratios associated with potential predictors of death were estimated using Cox proportional hazard models. Results Of the 69 patients with SLE-PAH identified in the French Pulmonary Hypertension Registry, 51 were included in the study. They did not differ from the control group regarding age, sex, or duration of SLE at the time of the analysis but had a higher frequency of anti-SSA and anti-SSB antibodies. The delay between SLE diagnosis and PAH diagnosis was 4.9 years (range, 2.8-12.9) years. The 3- and 5-year overall survival rates were 89.4% (95% CI, 76.2%-96.5%) and 83.9% (95% CI, 68.8%-92.1%), respectively. The survival rate was significantly better in patients with anti-U1-RNP antibodies ( P = .04). Conclusions Patients with SLE-PAH have an overall 5-year survival rate of 83.9% after the PAH diagnosis. Anti-SSA/SSB antibodies may be a risk factor for PAH, and the presence of anti-U1-RNP antibodies appears to be a protective factor regarding survival.

Abstract

IntroductionIntra-articular (IA) and peri-articular glucocorticoid (GC) injections are common in sports medicine. However, from 1 January 2022, all injectable GC routes (including IA administration) will be prohibited in-competition by World Anti-Doping Agency (WADA). Owing to these rules, an IA GC treatment out-of-competition could result in an adverse analytical finding in-competition if the washout period is not clearly defined. The aim of this study is to determine the urinary excretion profile of triamcinolone acetonide following IA injection to strengthen the definition of the washout periods.Methods and analysisThis is a prospective multicentre trial to include 20 subjects who practice sports for at least 4 hours/week and present a knee disorder requiring IA injection of triamcinolone acetonide for therapeutic purposes. To determine the excretion profile of triamcinolone acetonide in both urine and blood following IA injection of the drug, We will perform 20 urinary tests and 20 dried blood spot tests, two prior to GC injection (baseline) and the last one at 35 days. Analyses will be performed by the French antidoping agency laboratory in accordance with WADA standards and regulations.Ethics and disseminationThe study protocol was approved by the French ethics committee (CPP Sud Est III—Lyon—2020-070B on 06 October 2020). All subjects will provide written informed consent. The results of this study will be accessible in peer-reviewed publication and be presented at academic conference.Trial registration numberNCT04574232.

Abstract

ObjectivesTo evaluate the efficacy and safety of rituximab in patients with primary Sjögren's syndrome (pSS).MethodsThe AutoImmune and Rituximab registry has included 86 patients with pSS treated with rituximab, prospectivey followed up every 6 months for 5 years.ResultsSeventy-eight patients with pSS (11 men, 67 women), who already had at least one follow-up visit, were analysed. Median age was 59.8 years (29–83), median duration of disease was 11.9 years (3–32). Indications for treatment were systemic involvement for 74 patients and only severe glandular involvement in four patients. The median European Sjögren's Syndrome disease activity index (ESSDAI) was 11 (2–31). 17 patients were concomitantly treated with another immunosuppressant agent. Median follow-up was 34.9 months (6–81.4) (226 patient-years). Overall efficacy according to the treating physician was observed in 47 patients (60%) after the first cycle of rituximab. Median ESSDAI decreased from 11 (2–31) to 7.5 (0–26) (pConclusionsIn common practice, the use of rituximab in pSS is mostly restricted to patients with systemic involvement. This prospective study shows good efficacy and tolerance of rituximab in patients with pSS and systemic involvement.

Abstract

International audience; Evidence before this study We searched PubMed on June 30, 2016, for English-language cohort studies published since Jan 1, 1980, of patients with invasive listeriosis worldwide with the keywords " listeria " , " listeriosis " , " maternal " , and " neurolisteriosis ". Studies had to include epidemiological or clinical data on listeriosis. All clinical forms of infection were included (bacteraemia, neurolisteriosis, and maternal–neonatal infection). Host risk factors for listeriosis have been well identified, but the clinical features and prognostic factors of the disease are based on retrospective studies compiling heterogeneous data or random collected cases. Furthermore, no clinical trial has ever been done and medical management is not evidence based. Added value of the study Our study is the first prospective clinical study focusing on all forms of invasive listeriosis. The study is based on a national mandatory system that allowed the nearly complete capture of microbiologically proven cases. The study shows a higher burden of listeriosis than reported before: more than 80% of infected mothers experienced major fetal or neonatal complications (fetal loss, very high prematurity, early or late onset disease); only 39% of patients with neurolisteriosis survived and fully recovered. Abstract

BACKGROUND Pleural and pulmonary manifestations of giant cell arteritis are rare and not well known. They can be associated to more typical signs of the disease and to an inflammatory biological syndrome which are comprised in the multisystemic manifestations of the disease. They can be inaugural, leading to a late management if unrecognized. METHODS Retrospective and descriptive study of 8 cases over a 10 year period was conducted. Five females and three males with a 67-year-old average age were included according to the American College of Rheumatology criteria. They illustrated the clinical and/or radiological respiratory manifestations of the disease. RESULTS Pulmonary manifestation was inaugural in six cases over eight. The time to diagnosis range was 15-60 days. Cough was the most frequent symptom (five cases over eight). Dyspnea with orthopnea was described in one case. Pleural and parenchymal radiological manifestations had no specific characteristics: pleurisy, pleural thickening, nodules of variable size, reticular lesions. Temporal artery biopsy was positive in five cases, atypical in one case and negative in two cases. Bronchial and transbronchial biopsies (in two and one cases respectively) did not find any specific lesion. Clinical and radiological signs disappeared quickly after the introduction of glucocorticoid therapy. CONCLUSION The knowledge of these different respiratory manifestations during giant cell arteritis (persistent cough, nodules, pleural effusion) is useful for the clinician. It helps him in prescribing non invasive investigations or even a presumptive glucocorticoid therapy, in an often old and weakened patient.

INTRODUCTION: Recent suicide bombings pose the novel problem for Trauma Centers of the massive simultaneous arrival of many gravely wounded patients. METHODS: We report the experience of the French-German Military Trauma Group, a Level 2 Trauma Center, in Afghanistan during the wave of suicide bombings in February 2007. RESULTS: Fourteen casualties were received. A first triage was carried out by the U S Army Level I group prior to evacuation. A second surgical triage was carried out with systematic ultrasound exam. Four cases (ISS>25) were re-categorized and underwent emergency surgical procedures. CONCLUSION: Suicide bombing in crowded locations near an evacuation hospital may overwhelm the medical resources of the receiving center. It has been referred to as "The Main Gate Syndrome." We introduced the novel concept of a semi-evacuation hospital or receiving center where a second surgical triage was carried out. These exceptional circumstances require open-minded flexibility, a tailored approach, and close cooperation between surgeons and anesthetists to share experience, opinions, and ideas. 19. Abcès hépatiques à pyogènes. Étude descriptive rétrospective de 31 patients

Abstract

Introduction Les abces hepatiques sont rares, mais responsables d’une importante morbi-mortalite. Leur prise en charge n’est pas consensuelle. Patients et methodes C’est pourquoi nous avons realise une etude descriptive retrospective multicentrique en recuperant les dossiers des patients a partir du codage PMSI, du 1er janvier 2008 au 31 decembre 2012, dans 2 centres hospitaliers generaux. Resultats Nous avons recense 37 dossiers dont 31 abces a pyogenes (83,8 %) et 6 amoeboses hepatiques (16,2 %). Parmi les abces a pyogenes, on notait une predominance d’hommes (sex-ratio = 1,58). L’âge moyen etait de 63 ans. La fievre etait quasiment constante (96,77 %), la douleur de l’hypochondre droit moins frequente (61,29 %). Seuls 4 patients (12,9 %) presentaient une hepatomegalie. On notait une hyperleucocytose a PNN en moyenne a 12 275/mm3, avec un syndrome inflammatoire marque (moyenne de la CRP a 261 mg/L). On retrouvait une cytolyse hepatique dans 51,61 % des cas. Les GGT etaient plus souvent augmentes (74,19 %). La natremie etait abaissee dans 48,39 %. Les hemocultures etaient positives dans 36,26 % des cas. Onze patients sur 31 (35,48 %) avaient beneficie d’une ponction de l’abces avec 15 ponctions au total dont 14 positives (93,33 %). Au total, 17 patients sur 31 (54,84 %) avaient une documentation microbiologique : 28 germes dont 13 BGN (46,43 %), 8 CG+ (28,57 %), 4 anaerobies (14,29 %), 2 champignons (7,14 %) et 1 mycobacterie (3,57 %). Les infections etaient communautaires dans 77,42 % des cas. L’origine de l’infection etait hepatobiliaire dans 45,16 % des cas, digestive dans 7 cas (22,58 %), cryptogenetique chez 19,35 % des patients. Onze patients (35,48 %) ont subi un drainage d’abces pendant 12 jours en moyenne. Les patients ont ete traites en moyenne avec 3,42 ATB differents pendant 83 jours en moyenne : beta-lactamines (47,16 %), metronidazole (19,81 %), fluoroquinolones (13,21 %) essentiellement. Onze patients (35,48 %) ont presente un sepsis severe et/ou un choc septique. Apres 8 mois de suivi en moyenne, 5 patients (16,13 %) sont decedes, et 26 patients gueris (83,87 %) dont 1 recidive. Conclusion La morbi-mortalite des abces hepatiques a pyogenes n’est pas negligeable. La ponction de(s) l’abces est indispensable pour adapter l’antibiotherapie, qui doit couvrir les germes hepatobiliaires et digestifs. Une penicilline associee a un inhibiteur des beta-lactamases semble appropriee en 1re intention. La duree du traitement est longue (minimum de 4 a 6 semaines).

Abstract

Introduction Un service de tele-expertise en medecine interne par Internet a ete concu et teste pendant un an a destination des medecins generalistes ayant une activite liberale en Moselle ou exercant dans les unites militaires de la region Nord-Est. Materiels et methodes Le formulaire de soumission a ete cree sur Google drive permettant aux medecins generalistes interesses de transmettre leurs demandes d’avis. Un identifiant leur etait fourni afin de garantir l’anonymat ainsi qu’un questionnaire de satisfaction. Les demandes etaient discutees en reunions bihebdomadaires par les medecins seniors et les internes du service de medecine interne de l’hopital d’instruction des armees (HIA) Legouest de Metz. Une reponse ecrite etait renvoyee par courriel au medecin demandeur. Resultats Cinq cent quarante-quatre medecins ont ete contactes, 26 (4,8 %) ont demande des identifiants, 15 d’entre eux ayant transmis 21 demandes d’avis entre le premier janvier 2014 et le premier janvier 2015. Une hospitalisation a ete proposee dans 43 % des cas, une redirection vers une autre specialite dans 29 % des cas, une poursuite de la prise en charge en ville dans 19 % des cas et une consultation en medecine interne dans 9 % des cas. Trois demandes concernaient des problemes d’aptitude au metier militaire emanant de medecins des unites. Les douze questionnaires renvoyes faisaient etat d’une satisfaction unanime. Discussion L’experience montre qu’un service de tele-expertise en medecine interne par Internet est facile a mettre en place, mais peu de medecins generalistes non militaires ont ete interesses. Les obstacles a l’utilisation de ce service semblaient etre : le manque de recours a Internet, la preference d’un contact immediat de type telephonique et l’insuffisance d’integration de l’HIA dans le reseau de consultants des medecins civils. Les avantages etaient : l’anonymat de la demande, la demande par courriel plus complete et parfois moins intimidante que la presentation orale, la possibilite de poser une question hors heures ouvrables, la reponse collegiale avec une trace ecrite medico-legale. Conclusion Ce type de service devrait se developper a l’avenir, par l’integration a des plateformes d’e-sante reconnues et securisees.

23. Un déficit immunitaire traité par immunomodulateur

Abstract

Introduction Nous rapportons le cas d’une patiente atteinte d’une maladie infectieuse rare, revelant un deficit immunitaire tout aussi rare, traite par antibiotiques et rituximab. Observation Il s’agit d’une femme de 52 ans, sans antecedent personnel ni familial, originaire du Laos, vivant en France depuis plusieurs annees. La patiente etait symptomatique depuis avril 2010, 15 jours apres son retour d’un voyage au Laos. Elle consultait dans notre hopital en juin 2011 pour une alteration febrile de l’etat general, des adenopathies supracentimetriques douloureuses diffuses, des polyarthralgies, une pustulose du tronc et 3 episodes d’hemorragie digestive. A la biologie on notait une polynucleose neutrophile et un syndrome inflammatoire marques avec 36 g/L de PNN et une CRP a 250 mg/L. Il existait egalement une hypergammaglobulinemie polyclonale a 20 g/L. Le bilan auto-immun retrouvait des anticorps antinucleaires et des anti-SSA positifs mais a des taux peu significatifs. Il n’y avait pas d’autre argument pour un lupus ou un syndrome de Gougerot. La serologie VIH se revelait negative a 2 reprises. L’IDR et le quantiferon s’averaient negatifs. Le myelogramme et la BOM mettaient en evidence une moelle reactionnelle sans cellule maligne. La FOGD retrouvait des varices cardiotuberositaires. Le scanner TAP retrouvait de nombreuses ADP sus- et sous-diaphragmatiques, ainsi qu’une hepatomegalie avec des signes d’hypertension portale. La biopsie ganglionnaire ne retrouvait pas de lymphome mais un aspect de lymphadenite reactionnelle sans granulome. Finalement, c’est la culture bacteriologique du ganglion qui permit le diagnostic d’infection chronique disseminee a Mycobacterium fortuitum. La patiente etait rapidement amelioree par une triple antibiotherapie (clarythromycine, moxifloxacine, doxycycline). Apres 6 mois de traitement efficace, les antibiotiques etaient suspendus. Deux jours apres la fin du traitement la patiente recidivait avec les memes symptomes initiaux. Une nouvelle biopsie ganglionnaire avec culture et antibiogramme etait realisee et un nouveau traitement antibiotique reintroduit, secondairement adapte a l’antibiogramme : ciprofloxacine, minocycline, amikacine. Devant cette evolution inhabituelle nous avons recherche une cause d’immunodepression. Les dosages de l’interferon gamma (IFNgamma) et de l’interleukine 12 (IL12) sont revenus indosables, et la recherche des auto-anticorps anti-interferon gamma tres positifs a 12 000 par la technique BioPlex. Etant donne la rechute precoce de l’infection, et des taux indosables d’IFNgamma et d’IL12 nous avons introduit un traitement par rituximab (4 cures hebdomadaires) afin d’enrayer la production d’anticorps anti-IFNgamma. Apres 9 mois de recul, on voit reapparaitre une activite pour l’IFNgamma et l’IL12, mais a des taux encore insuffisants pour permettre l’arret des antibiotiques. Discussion M. fortuitum fait partie des mycobacteries atypiques de l’environnement. Il s’agit d’une bacterie peu voire non pathogene chez les immunocompetents. Une telle infection severe doit faire rechercher un deficit de l’immunite cellulaire (VIH) et/ou de l’axe IL12-IFNgamma. La presence d’auto-anticorps anti-IFNgamma represente une entite rare (50 cas dans la litterature). Elle survient essentiellement chez des personnes originaires d’Asie du Su-Est. Elle favorise la survenue d’infections severes a mycobacteries atypiques, salmonelles et la reactivation de certains virus (CMV, VZV). Le 1er traitement est le traitement anti-infectieux de l’infection, mais on note souvent des rechutes tant que l’immunite n’est pas restauree. Plusieurs traitements corticoides et/ou immunosuppresseurs ont ete essayes. Celui qui semble le plus prometteur est l’anticorps anti-CD20 (rituximab), rapporte efficace dans 5 cas de la litterature. En effet, il permet de diminuer la production de ces anticorps et de restaurer l’activite de l’axe IL12-IFNgamma, permettant l’arret des anti-infectieux. Conclusion Une infection severe avec une mycobacterie atypique doit faire suspecter un deficit immunitaire soit des CD4 (VIH), soit de l’axe IL12-IFNgamma, avec notamment cette nouvelle entite d’auto-anticorps anti-IFNgamma. Le rituximab semble un traitement prometteur dans cette pathologie pour corriger ce deficit immunitaire.

24. Intérêt de la PCR paludisme en pratique quotidienne. Étude rétrospective dans un service de maladies infectieuses

Abstract

Introduction La France est, parmi les pays industrialises, celui qui compte le plus de cas de paludisme d’importation (4370 cas en 2014). Parmi ces cas, on note 311 formes graves (13,5 %) dont 11 deces. La prise en charge des patients atteints de paludisme doit etre rapide, et les moyens diagnostiques fiables. Nous avons decide d’analyser l’apport de la PCR paludisme dans notre service de maladies infectieuses. Materiels et methodes Le laboratoire de notre hopital general possede une PCR pan-paludisme et 5 PCR specifiques pour chaque espece plasmodiale. Il s’agit de techniques PCR en temps reel, realisees sur thermocylceur LightCycler 2.0, rendant un resultat en 2H. Les amorces ciblent differentes regions de l’ARN 18S du Plasmodium. Nous avons analyse tous les cas ayant beneficie de cette technique depuis qu’elle est disponible, de janvier 2014 a juillet 2015, et nous avons compare les resultats avec les techniques usuelles (frottis sanguin, goutte epaisse, QBC, tests de detection antigenique). Resultats De janvier 2014 a juillet 2015, le laboratoire a recense 24 patients pour lesquels une recherche de paludisme a ete effectuee (22 positifs, 2 negatifs). Parmi ces patients, 9 (37,5 %) ont beneficie d’une PCR paludisme, dont 2 fois chez un meme patient (10 PCR au total). La PCR etait realisee a la demande du clinicien et/ou du laboratoire dans les cas suivants : 4 fois (40 %) chez des patients ayant recu un traitement antipaludeen preventif ou curatif, 2 fois (20 %) pour definir l’espece en raison d’une faible parasitemie, 2 fois (20 %) pour controle des tests microscopiques, 1 fois (10 %) a la recherche d’un multi-parasitisme, 1 fois (10 %) en raison de tests usuels negatifs. Lorsque les tests usuels etaient positifs, la PCR etait toujours positive, de meme pour les tests negatifs. Une fois la PCR a permis de faire le diagnostic de paludisme alors que les tests usuels etaient negatifs. Dans 4 cas sur 5, la PCR et les tests usuels etaient concordants pour l’espece plasmodiale. Dans 1 cas, la PCR a permis de diagnostiquer un multi-parasitisme avec la mise en evidence de Plasmodium falciparum, qui n’avait pas ete retrouve par les test usuels. La PCR a apporte le diagnostic de l’espece plasmodiale dans 3 cas. Au final, cette technique s’est revelee utile dans 5 situations (50 %) : 4 fois pour le diagnostic d’espece, 1 fois pour faire le diagnostic de paludisme. Discussion La PCR paludisme ne represente pas un examen de 1re ligne dans cette maladie, dans les pays non impaludes. Neanmoins, elle peut s’averer utile dans certaines situations precises : determination de l’espece en cas de faible parasitemie, diagnostic de la maladie chez un patient deja sous antipaludeens, doute sur un multi-parasitisme. Elle peut egalement servir dans les pays impaludes en cas de parasitemie faible, ou pour eviter des traitements antipalustres injustifies. Conclusion La PCR paludisme represente un nouvel examen diagnostique sensible et fiable, utile en cas de faible parasitemie, et chez les patients sous traitement antipaludeen.

Abstract

Anthrax is an acute bacterial infection caused by Bacillus anthracis. The infection is cutaneous in about 95% of human cases and respiratory in about 5%. Approximately 2000 cases of cutaneous anthrax are reported annually worldwide. This disease became exceptional in Europe thanks to strict veterinarian monitoring. The last human cases of anthrax indicated in France were in 1997. We report 3 new related cases of naturally acquired cutaneous anthrax that occurred in France in 2008. The unique features of these cases include the atypical clinical presentation and the contribution of the rapid and specific diagnosis techniques by polymerase chain reaction.In cutaneous forms of anthrax, although the local course is not influenced by the treatment, antibiotic therapy is necessary to control any bacterial distribution. A case of exposure similar to that of a confirmed human case or an exposure identified by epidemiologic inquiry should usually result in chemoprophylaxis. Chemoprophylaxis for the close relations of a patient or for health workers is unnecessary since person-to-person transmission has not been reported.

Pleural and parenchymal radiological manifestations had no specific characteristics: pleurisy, pleural thickening, nodules of variable size, reticular lesions. Temporal artery biopsy was positive in five cases, atypical in one case and negative in two cases. Bronchial and transbronchial biopsies (in two and one cases respectively) did not find any specific lesion. Clinical and radiological signs disappeared quickly after the introduction of glucocorticoid therapy.The knowledge of these different respiratory manifestations during giant cell arteritis (persistent cough, nodules, pleural effusion) is useful for the clinician. It helps him in prescribing non invasive investigations or even a presumptive glucocorticoid therapy, in an often old and weakened patient.

28. Méningite à méningocoque Y par déficit en fraction C6 du complément

Abstract

Introduction La survenue d’une meningite a Neisseria meningitidis doit faire evoquer un deficit immunitaire en composants terminaux du complement, et d’autant plus que des facteurs de risques peuvent etre identifies. Nous rapportons ici un cas de meningite a meningocoque de serogroupe Y compliquant un deficit en fraction C6 du complement. Observation Nous rapportons le cas d’un patient de 22 ans sans antecedent notable, originaire du Cameroun et vivant en France depuis 2010. Ce patient avait presente des cephalees de survenue brutale, sans fievre initialement, et sans nausee ni vomissement. Une consultation au service d’accueil des urgences objectivait un syndrome meninge. Le patient rapportait une prise d’anti-inflammatoires la semaine precedente. L’analyse du liquide cerebro-spinal retrouvait 7000 leucocytes/mm3 dont 80 % de polynucleaires neutrophiles. La culture se revelait sterile en raison d’une antibiotherapie prealable. La recherche moleculaire par polymerase chain reaction (PCR) revenait positive a N. meningitidis de genogroupe Y. Le scanner cerebral etait strictement normal. Initialement, on notait une leucocytose a 12,2 G/L, a predominance de polynucleaires neutrophiles (10,9 G/L) et une CRP a 252 mg/L. Le reste du bilan biologique etait sans particularite. Apres 24 heures d’administration d’une antibiotherapie parenterale par cephalosporine de 3e generation (cefotaxime 12 g/j) et d’une corticotherapie par dexamethasone (40 g/j), le syndrome meninge regressait totalement sans phono-photophobie, ni nausee ou trouble de la vigilance. Le dosage des fractions C3–C4 du complement etait normal mais l’activite CH50 etait legerement abaissee a 64 % (normale : 70–100 %), confirme sur un nouveau prelevement a 53 %. L’exploration des fractions terminales du complement montrait une diminution de l’activite fonctionnelle du complement terminal inferieure a 10 %, et surtout un deficit quantitatif du composant C6 inferieur a 10 %. Discussion Le depistage des deficits des fractions terminales du complement permet le diagnostic de 500 a 600 cas par an, representant seulement 1 a 3 % des sujets ayant presente une infection a N. meningitidis dans la population caucasienne. La prevalence des deficits en complement augmente chez des patients ayant eu plus d’un episode infectieux a N. meningitidis (41 %) et d’autant plus, en cas de serogroupe rare (19 %) ou d’antecedent familial d’infection a meningocoque (14 %). Le depistage d’un deficit en proteine du complement impose le dosage du CH50, C3, C4, AP50 et Properdine. Les deficits homozygotes en composants terminaux se compliquent principalement d’infections invasives a meningocoque. Un complement C3, C4 dans des valeurs normales avec diminution du CH50 et de l’AP50 doit faire evoquer un deficit des fractions terminales. Dans cette population deficitaire en C5, C6, C7 ou C8, on estime le risque de meningite a 6 × 103 fois superieure aux individus non deficitaires, mais avec une mortalite 10 fois moindre. Ces meningites surviennent a un âge plus tardif. Le serotype Y est le plus frequent. On retrouve une frequence elevee des alleles nuls C6D dans la population africaine (environ 1 sur 400). L’etude familiale, notamment celle de la fratrie, est essentielle dans un but diagnostique. Cela est d’autant plus important que nous disposons actuellement d’une prophylaxie vaccinale anti-meningococcique conjuguee A, C, Y, W135 et inerte contre le serogroupe B. Conclusion Devant une meningite a N. meningitidis : – des antecedents familiaux d’infections a N. meningitidis ; – des infections recidivantes ; – une infection par une souche de serogroupe rare ; – des formes fulminantes de meningites ou ; – un âge inferieur 6 mois ou superieur a 5 ans doit imposer la recherche d’un deficit en fraction terminale du complement. Une origine africaine doit faire evoquer en premier lieu un deficit en C6 en raison d’une prevalence elevee dans cette population. Le risque d’infection a meningocoque peut etre diminue efficacement par la prophylaxie vaccinale.

33. F-05: Étude de la mortalité à 1 an après bactériémie chez la personne âgée versus le sujet jeune : cohorte de 77 patients

Abstract

Introduction – objectifs Beaucoup d’etudes retrouvent une surmortalite a court et moyen terme (jusqu’a 3 mois) apres la survenue d’une bacteriemie chez le sujet âge par rapport au sujet jeune. Mais aucune etude n’a evalue la mortalite a 1 an. Materiels et methodes Nous avons realise une etude monocentrique retrospective de 78 patients ayant presente une bacteriemie en 2011 dans notre hopital. Le but principal etait d’etudier la mortalite a 1 an. Les buts secondaires etaient d’analyser la mortalite a J2, S1, M1, M3, M6 et de realiser une etude descriptive des 2 groupes. Les patients etaient repartis en 2 groupes : sujet âge > 65 ans (divisee en jeune-vieux de 65 a 74 ans, vieux de 75 a 84 ans, tres vieux > 85 ans) et sujet jeune Resultats Sur l’annee 2011 nous avons recense 77 bacteriemies vraies : 48 sujets âges et 29 jeunes. Il y avait une predominance d’hommes (31/17) dans le groupe « sujet âge » par rapport aux sujets jeunes (13/16). Le groupe « sujet âge » presentait plus de syndrome confusionnel que le groupe « sujet jeune » (60 % vs 28 %, p = 0,005), et d’apyrexie a l’admission (61 % vs 15 %, p = 0,004). Les infections urinaires hautes etaient plus frequentes chez le sujet âge (31 % vs 10 %, p = 0,021). L’etude microbiologique ne revelait pas de difference entre les 2 groupes pour le type de bacteries (68 % de BGN vs 66 %). Les patients âges presentaient plus d’infections a BMR (49 % vs 41 %, p = 0,43) et d’infections nosocomiales (31 % vs 14 %, p = 0,07). L’antibiotherapie etait plus souvent inappropriee chez le sujet âge (28 % vs 10 %, p = 0,049). Pour la mortalite a 1 an, on notait une surmortalite chez le sujet âge mais sans significativite (44 % vs 24 %, p = 0,075). La mortalite chez les sujets jeunes n’evoluait plus apres 1 mois suivant la bacteriemie (stable a 24 %), alors qu’elle continuait de croitre jusqu’a 1 an pour le groupe « sujet âge » (25 % a M1, 35 % a M3, 40 % a M6). Conclusion La mortalite a 1 an est plus elevee chez la personne âgee parmi les patients ayant presente une bacteriemie.

Abstract

Introduction.– La presence d’un anticoagulant circulant chez une patiente lupique represente un facteur de risque de thrombose veineuse et/ou arterielle, entrant dans le cadre d’un syndrome des anticorps anti-phospholipide (SAPL). Cependant, dans de tres rares cas, ces anticorpspeuvent etre responsablesd’un syndromehemorragique pouvant menacer le pronostic vital. Nous rapportons le cas d’une patiente chez qui la presence de menorragies a conduit au diagnostic de lupus erythemateux aigudissemine (LEAD) avec SAPL et syndrome anticoagulant circulant lupique hypoprothrombinemie (LA-H pour lupus anticoagulant-hypoprothrombinemia). Patients et methodes.–Une patiente de 33ans, avec ATCD de thrombose veineuse superficielle en 2011et de syndrome depressif, consultait au printemps 2013pour des menorragies abondantes associees a une douleur thoracique, des arthralgies d’horaire inflammatoire et une urticaire. L’examen clinique retrouvait une pâleur cutaneomuqueuse marquee et une urticaire. Il n’y avait ni arthrite ni frottement pericardique. L’examen gynecologique et l’echographie pelvienne etaient normaux. Sur la NFS les plaquettes etaient normales mais il existait une anemie microcytaire avec une Hb a 8,4 g/dl. Le TCA etait allonge a 2 fois la normale et le TP abaisse a 35%. Les tests d’anticoagulation mettaient en evidence un effondrement isole du facteur II a 16%. La recherche d’anticorps anti-facteur II au CHU deMarseille revenait positive avec un taux a 8,5UI/L pour une normale

Abstract

Introduction et objectifs L’infarctus splenique est une complication rare decrite lors des acces palustres a Plasmodium falciparum ou vivax . Nous rapportons un cas d’infarctus splenique lors d’un acces palustre a Plasmodium ovale , le premier cas decrit dans la litterature a notre connaissance. Materiels et methodes Patient de 34 ans, militaire, sans ATCD particulier. 2 sejours en zone d’endemie palustre : Senegal en 2002, Cote d’Ivoire en 2004, de 4 mois chacun avec chimioprophylaxie par doxycycline (mais oublis ponctuels). Consulte pour fievre a 39,5 °C, cephalees, asthenie, myalgies et polyarthralgies, douleur abdominales diffuses avec nausees et vomissements. L’examen clinique retrouve une splenomegalie a 14 cm, non douloureuse. Sur le plan biologique, il existe une anemie a 11,8 g/dl, une leucopenie a 3 250/mm 3 , une lymphopenie a 839/mm 3 et une thrombopenie a 117 000/mm 3 . La CRP est a 37 mg/l. Le frottis sanguin met en evidence un Plasmodium ovale , avec une parasitemie a 0,001 %. Le diagnostic d’espece est confirme par biologie moleculaire, sans biparasitisme. Traitement par quinine IV car vomissements. Apres 6 jours de traitement, apparition d’une douleur intense de l’hypochondre gauche, accentuee a la palpation avec irradiation vers l’omoplate homolaterale. Un scanner abdominal met en evidence une splenomegalie avec quatre infarctus spleniques. La prise encharge a ete limitee a la surveillance clinique etroite et a l’analgesie. L’evolution clinique et biologique a ete favorable et le controle scannographique montrait une regression des infarctus spleniques, partielle a J15 et totale a 4 mois. Resultats Le paludisme d’importation est frequent en France (7 000 cas rapportes en 1999). L’infarctus splenique est une complication rare (8 cas rapportes dans medline dont 7 lies a Plasmodium falciparum et un a Plasmodium vivax ). Nous decrivons le 1 er cas d’infarctus splenique apres infection par P. ovale. Cette complication rare peut se produire malgre le traitement antipaludique approprie, et donc quelque soit l’espece plasmodiale. On ne connait pas de signes predictifs. Conclusion Les cliniciens doivent etre conscients qu’une douleur de l’hypochondre gauche pendant un acces palustre doit faire rechercher un infarctus splenique quelque soit l’espece plasmodiale.

Abstract

Objectives To evaluate the efficacy and safety of rituximab (RTX) in patients with primary Sjogren’s syndrome (pSS). Methods The AutoImmune and Rituximab registry (AIR), an independent registry of the French Society of Rheumatology, includes 3662 off-trial patients with refractory autoimmune diseases treated with RTX and prospectivey followed up for 5 years. Efficacy was defined according to the opinion of the physician. Each treating physician was asked to validate the data of his patient(s). ESSDAI scores were retrospectively scored. Results In the AIR registry 86 patients (pts) with pSS were treated with RTX. The data have been validated by clinicians for 79 pts. 78 pts who had at least one follow-up visit (median follow-up of 34.9 months [6-81.4], 226 patient/years) were analyzed. Median age (11 males, 67 females) was 59.8 years [29-83], the median duration of pSS was 11.9 years [3-32]. Indications of the treatment were: systemic involvement for 74 pts (articular (n=27), peripheral (PNS)/central nervous system (CNS) (n=12/6), pulmonary (n=9), renal (n=6), muscular (n=3), haematological (n=2) involvements, vasculitis (n=8, including 5 cryoglobulinemias), autoimmune pancreatitis (=1), and only glandular involvement in 4 pts. Median ESSDAI assessed in 78 pts was 11 [2-31]. 67 pts were initially treated by 2 infusions of 1g, 11 pts by 4 infusions of 375mg/m 2 . 17 pts were concomitantly treated with: methotrexate (7 pts), mycophenolate mofetil (1 pt), hydroxychloroquine (7pts), azathioprin (1 pt), cyclophosphamide (1 pt). 29 pts were also treated with corticosteroids (median dosage 18 mg [3-60]). Efficacy: Overall efficacy according to the clinician was observed in 47 pts (60%) after the 1st cycle of RTX. Efficacy was observed in 45 pts (61%) with systemic involvement: articular (63% of pts), PNS (50%), CNS (33%), pulmonary (78%), renal (83%), muscular (0%), haematological (100%) involvement, autoimmune pancreatitis (100%), and vasculitis (62%). No data was available regarding the evolution of dryness. Efficacy was observed in 2 pts (50%) with only glandular involvement (2 parotid swellings). Median ESSDAI after RTX decreased from 11 [2-31] to 7.5 [0-26] (p Tolerance: 4 serious immediate infusion reactions and 1 delayed serum-sickness like disease occurred. 3 serious infections occurred (1.3/100 patient/years) and 2 cancers (0.9/100 patient/years). The 2 pts with cancer died. RTX was discontinued in 41 pts (inefficacy: 35, serious adverse events:6). The median number of RTX cycles was 2.3 [1-12]. Median duration between 1st and 2nd cycles was 11 months (5.4-29.4). Conclusions This study shows good efficacy and short term safety of RTX for systemic features. This study demonstrates that in real life, clinicians mostly use RTX off-label in pSS patients with systemic involvement. This large prospective study suggests the efficacy of RTX in systemic complications of pSS with a corticosteroid sparing effect. These results have to be confirmed by controlled studies.
