Abstract

Abstract A search for full energy depositions from bosonic keV-scale dark matter candidates of masses between 65 and 1021 keV has been performed with data collected during Phase II of the GERmanium Detector Array (Gerda) experiment. Our analysis includes direct dark matter absorption as well as dark Compton scattering. With a total exposure of 105.5 kg years, no evidence for a signal above the background has been observed. The resulting exclusion limits deduced with either Bayesian or Frequentist statistics are the most stringent direct constraints in the major part of the 140–1021 keV mass range. As an example, at a mass of 150 keV the dimensionless coupling of dark photons and axion-like particles to electrons has been constrained to $$\alpha '/\alpha 1.5 \times 10^{24}$$ τ n > 1.5 × 10 24 years and for a proton $$\tau _\textrm{p} > 1.3 \times 10^{24}$$ τ p > 1.3 × 10 24 years at 90% CI. For the electron decay $$e^\text {-} \rightarrow \nu _\textrm{e} \gamma $$ e - → ν e γ a lower limit of $$\tau _\textrm{e} > 5.4\times 10^{25}$$ τ e > 5.4 × 10 25 years at 90% CI has been determined.

Abstract

Objectives: To investigate the change in severity of mitral regurgitation (MR) after transcatheter aortic valve replacement (TAVR) and its effect on 5-year mortality. Background: There is inconsistency in literature on pre-existing MR influencing long-term survival in patients who undergo TAVR. Methods: Patients who underwent TAVR at the University Hospital Schleswig-Holstein (USKH) Campus Kiel between March 2009 and February 2018 have been enrolled. Echocardiography determined the degree of MR before and within 7 days after TAVR. Patients were divided into two groups according to their MR at baseline: MR-grade ≤ 2 (non-relevant MR, nr-MR) and baseline MR-grade > 2 (relevant MR, r-MR). Primary endpoint was a composite of MR baseline influence on mortality and MR reduction and its’ impact on mortality. Results: A total of 820 patients (642 nr-MR and 178 in r-MR) were included in this study. Of these, 167 patients showed an improvement in MR-grade. Abstract

Abstract The NEMO-3 results for the double- $$\beta $$ β decay of $$^{150}$$ 150 Nd to the 0 $$^+_1$$ 1 + and 2 $$^+_1$$ 1 + excited states of $$^{150}$$ 150 Sm are reported. The data recorded during 5.25 year with 36.6 g of the isotope $$^{150}$$ 150 Nd are used in the analysis. The signal of the $$2\nu \beta \beta $$ 2 ν β β transition to the 0 $$^+_1$$ 1 + excited state is detected with a statistical significance exceeding 5 $$\sigma $$ σ . The half-life is measured to be $$T_{1/2}^{2\nu \beta \beta }(0^+_1) = \left[ 1.11 ^{+0.19}_{-0.14} \,\left( \hbox {stat}\right) ^{+0.17}_{-0.15}\,\left( \hbox {syst}\right) \right] \times 10^{20}$$ T 1 / 2 2 ν β β ( 0 1 + ) = 1 . 11 - 0.14 + 0.19 stat - 0.15 + 0.17 syst × 10 20 year, which is the most precise value that has been measured to date. 90% confidence-level limits are set for the other decay modes. For the $$2\nu \beta \beta $$ 2 ν β β decay to the 2 $$^+_1$$ 1 + level the limit is $$T^{2\nu \beta \beta }_{1/2}(2^+_1) > 2.42 \times 10^{20}~\hbox {year}$$ T 1 / 2 2 ν β β ( 2 1 + ) > 2.42 × 10 20 year . The limits on the $$0\nu \beta \beta $$ 0 ν β β decay to the 0 $$^+_1$$ 1 + and 2 $$^+_1$$ 1 + levels of $$^{150}$$ 150 Sm are significantly improved to $$T_{1/2}^{0\nu \beta \beta }(0^+_1) > 1.36 \times 10^{22}~\hbox {year}$$ T 1 / 2 0 ν β β ( 0 1 + ) > 1.36 × 10 22 year and $$T_{1/2}^{0\nu \beta \beta }(2^+_1) > 1.26 \times 10^{22}~\hbox {year}$$ T 1 / 2 0 ν β β ( 2 1 + ) > 1.26 × 10 22 year .

Abstract

Abstract We search for tri-nucleon decays of $$^{76}$$ 76 Ge in the dataset from the GERmanium Detector Array (GERDA) experiment. Decays that populate excited levels of the daughter nucleus above the threshold for particle emission lead to disintegration and are not considered. The ppp-, ppn-, and pnn-decays lead to $$^{73}$$ 73 Cu, $$^{73}$$ 73 Zn, and $$^{73}$$ 73 Ga nuclei, respectively. These nuclei are unstable and eventually proceed by the beta decay of $$^{73}$$ 73 Ga to $$^{73}$$ 73 Ge (stable). We search for the $$^{73}$$ 73 Ga decay exploiting the fact that it dominantly populates the 66.7 keV $$^{73m}$$ 73 m Ga state with half-life of 0.5 s. The nnn-decays of $$^{76}$$ 76 Ge that proceed via $$^{73m}$$ 73 m Ge are also included in our analysis. We find no signal candidate and place a limit on the sum of the decay widths of the inclusive tri-nucleon decays that corresponds to a lower lifetime limit of 1.2 $$\times $$ × 10 $$^{26}$$ 26 yr (90% credible interval). This result improves previous limits for tri-nucleon decays by one to three orders of magnitude.

Abstract

Abstract The ability to detect liquid argon scintillation light from within a densely packed high-purity germanium detector array allowed the Gerda experiment to reach an exceptionally low background rate in the search for neutrinoless double beta decay of $${}^{76}$$ 76 Ge. Proper modeling of the light propagation throughout the experimental setup, from any origin in the liquid argon volume to its eventual detection by the novel light read-out system, provides insight into the rejection capability and is a necessary ingredient to obtain robust background predictions. In this paper, we present a model of the Gerda liquid argon veto, as obtained by Monte Carlo simulations and constrained by calibration data, and highlight its application for background decomposition.

Abstract

Abstract The GERmanium Detector Array (Gerda) collaboration searched for neutrinoless double- $$\beta $$ β decay in $$^{76}$$ 76 Ge using isotopically enriched high purity germanium detectors at the Laboratori Nazionali del Gran Sasso of INFN. After Phase I (2011–2013), the experiment benefited from several upgrades, including an additional active veto based on LAr instrumentation and a significant increase of mass by point-contact germanium detectors that improved the half-life sensitivity of Phase II (2015–2019) by an order of magnitude. At the core of the background mitigation strategy, the analysis of the time profile of individual pulses provides a powerful topological discrimination of signal-like and background-like events. Data from regular $$^{228}$$ 228 Th calibrations and physics data were both considered in the evaluation of the pulse shape discrimination performance. In this work, we describe the various methods applied to the data collected in Gerda Phase II corresponding to an exposure of 103.7 kg year. These methods suppress the background by a factor of about 5 in the region of interest around $$Q_{\beta \beta }= 2039$$ Q β β = 2039 keV, while preserving $$(81\pm 3)$$ ( 81 ± 3 ) % of the signal. In addition, an exhaustive list of parameters is provided which were used in the final data analysis.

Abstract

Abstract A search for double-beta decay of $$^{190}$$ 190 Pt and $$^{198}$$ 198 Pt with emission of $$\gamma $$ γ -ray quanta was realized at the HADES underground laboratory with a 148 g platinum sample measured by two ultralow-background HPGe detectors over 8946 h. The isotopic composition of the platinum sample has been measured with high precision using inductively coupled plasma mass spectrometry. New lower limits for the half-lives of $$^{190}$$ 190 Pt relative to different channels and modes of the decays were set on the level of $$\lim T_{1/2}\sim 10^{14}$$ lim T 1 / 2 ∼ 10 14 – $$10^{16}$$ 10 16 year. A possible exact resonant $$0\nu KN$$ 0 ν K N transition to the 1,2 1326.9 keV level of $$^{190}$$ 190 Os is limited for the first time as $$T_{1/2} \ge 2.5 \times 10^{16}$$ T 1 / 2 ≥ 2.5 × 10 16 year. A new lower limit on the double-beta decay of $$^{198}$$ 198 Pt to the first excited level of $$^{198}$$ 198 Hg was set as $$T_{1/2} \ge 3.2\times 10^{19}$$ T 1 / 2 ≥ 3.2 × 10 19 year, one order of magnitude higher than the limit obtained in the previous experiment.

The experimental signature of the decay is a monoenergetic signal at $$Q_{\beta \beta }$$ Q β β $$=2039.061(7)$$ = 2039.061 ( 7 ) keV in the measured summed energy spectrum of the two emitted electrons. Both the energy reconstruction and resolution of the germanium detectors are crucial to separate a potential signal from various backgrounds, such as neutrino-accompanied double- $$\beta $$ β decays allowed by the Standard Model. The energy resolution and stability were determined and monitored as a function of time using data from regular $$^{228}$$ 228 Th calibrations. In this work, we describe the calibration process and associated data analysis of the full Gerda dataset, tailored to preserve the excellent resolution of the individual germanium detectors when combining data over several years.

Abstract

Abstract Neutrinoless double- $$\beta $$ β decay of $$^{76}$$ 76 Ge is searched for with germanium detectors where source and detector of the decay are identical. For the success of future experiments it is important to increase the mass of the detectors. We report here on the characterization and testing of five prototype detectors manufactured in inverted coaxial (IC) geometry from material enriched to 88% in $$^{76}$$ 76 Ge. IC detectors combine the large mass of the traditional semi-coaxial Ge detectors with the superior resolution and pulse shape discrimination power of point contact detectors which exhibited so far much lower mass. Their performance has been found to be satisfactory both when operated in vacuum cryostat and bare in liquid argon within the Gerda setup. The measured resolutions at the Q-value for double- $$\beta $$ β decay of $$^{76}$$ 76 Ge ( $$Q_{\beta \beta }$$ Q β β = 2039 keV) are about 2.1 keV full width at half maximum in vacuum cryostat. After 18 months of operation within the ultra-low background environment of the GERmanium Detector Array (Gerda) experiment and an accumulated exposure of 8.5 kg $$\cdot $$ · year, the background index after analysis cuts is measured to be $$4.9^{+7.3}_{-3.4}\times 10^{-4} \ \text {counts}/(\text {keV} \cdot \text {kg} \cdot \text {year})$$ 4 . 9 - 3.4 + 7.3 × 10 - 4 counts / ( keV · kg · year ) around $$Q_{\beta \beta }$$ Q β β . This work confirms the feasibility of IC detectors for the next-generation experiment Legend.

Abstract

Abstract The GERmanium Detector Array (Gerda) experiment at the Gran Sasso underground laboratory (LNGS) of INFN is searching for neutrinoless double-beta (0νββ) decay of 76Ge. The technological challenge of Gerda is to operate in a “background-free” regime in the region of interest (ROI) after analysis cuts for the full 100 kg·yr target exposure of the experiment. A careful modeling and decomposition of the full-range energy spectrum is essential to predict the shape and composition of events in the ROI around Q ββ for the 0νββ search, to extract a precise measurement of the half-life of the double-beta decay mode with neutrinos (2νββ) and in order to identify the location of residual impurities. The latter will permit future experiments to build strategies in order to further lower the background and achieve even better sensitivities. In this article the background decomposition prior to analysis cuts is presented for Gerda Phase II. The background model fit yields a flat spectrum in the ROI with a background index (BI) of 16.04 − 0.85 + 0.78 · 10 − 3 $$ {16.04}_{-0.85}^{+0.78}\cdotp {10}^{-3} $$ cts/(keV·kg·yr) for the enriched BEGe data set and 14.68 − 0.52 + 0.47 · 10 − 3 $$ {14.68}_{-0.52}^{+0.47}\cdotp {10}^{-3} $$ cts/(keV·kg·yr) for the enriched coaxial data set. These values are similar to the one of Phase I despite a much larger number of detectors and hence radioactive hardware components.

Abstract

Abstract The GERmanium Detector Array (Gerda) is a low background experiment located at the Laboratori Nazionali del Gran Sasso in Italy, which searches for neutrinoless double-beta decay of $$^{76}$$ 76 Ge into $$^{76}$$ 76 Se+2e$$^-$$ - . Gerda has been conceived in two phases. Phase II, which started in December 2015, features several novelties including 30 new 76Ge enriched detectors. These were manufactured according to the Broad Energy Germanium (BEGe) detector design that has a better background discrimination capability and energy resolution compared to formerly widely-used types. Prior to their installation, the new BEGe detectors were mounted in vacuum cryostats and characterized in detail in the Hades underground laboratory in Belgium. This paper describes the properties and the overall performance of these detectors during operation in vacuum. The characterization campaign provided not only direct input for Gerda Phase II data collection and analyses, but also allowed to study detector phenomena, detector correlations as well as to test the accuracy of pulse shape simulation codes.

Abstract The full data set of the NEMO-3 experiment has been used to measure the half-life of the two-neutrino double beta decay of $$^{100}$$ 100 Mo to the ground state of $$^{100}$$ 100 Ru, $$T_{1/2} = \left[ 6.81 \pm 0.01\,\left( \text{ stat }\right) ^{+0.38}_{-0.40}\,\left( \text{ syst }\right) \right] \times 10^{18}$$ T1/2=6.81±0.01stat-0.40+0.38syst×1018 year. The two-electron energy sum, single electron energy spectra and distribution of the angle between the electrons are presented with an unprecedented statistics of $$5\times 10^5$$ 5×105 events and a signal-to-background ratio of $$\sim $$ ∼ 80. Clear evidence for the Single State Dominance model is found for this nuclear transition. Limits on Majoron emitting neutrinoless double beta decay modes with spectral indices of $$\mathrm{n}=2,3,7$$ n=2,3,7 , as well as constraints on Lorentz invariance violation and on the bosonic neutrino contribution to the two-neutrino double beta decay mode are obtained.

Abstract Using data from the NEMO-3 experiment, we have measured the two-neutrino double beta decay ($$2\nu \beta \beta $$ 2νββ ) half-life of $$^{82}$$ 82 Se as $$T_{\smash {1/2}}^{2\nu } \!=\! \left[ 9.39 \pm 0.17\left( \text{ stat }\right) \pm 0.58\left( \text{ syst }\right) \right] \times 10^{19}$$ T1/22ν=9.39±0.17stat±0.58syst×1019 y under the single-state dominance hypothesis for this nuclear transition. The corresponding nuclear matrix element is $$\left| M^{2\nu }\right| = 0.0498 \pm 0.0016$$ M2ν=0.0498±0.0016 . In addition, a search for neutrinoless double beta decay ($$0\nu \beta \beta $$ 0νββ ) using 0.93 kg of $$^{82}$$ 82 Se observed for a total of 5.25 y has been conducted and no evidence for a signal has been found. The resulting half-life limit of $$T_{1/2}^{0\nu } > 2.5 \times 10^{23} \,\text{ y } \,(90\%\,\text{ C.L. })$$ T1/20ν>2.5×1023y(90%C.L.) for the light neutrino exchange mechanism leads to a constraint on the effective Majorana neutrino mass of $$\langle m_{\nu } \rangle < \left( 1.2{-}3.0\right) \,\text{ eV }$$ ⟨mν⟩

Abstract

Abstract The Gerda collaboration is performing a sensitive search for neutrinoless double beta decay of $$^{76}\hbox {Ge}$$ 76Ge at the INFN Laboratori Nazionali del Gran Sasso, Italy. The upgrade of the Gerda experiment from Phase I to Phase II has been concluded in December 2015. The first Phase II data release shows that the goal to suppress the background by one order of magnitude compared to Phase I has been achieved. Gerda is thus the first experiment that will remain “background-free” up to its design exposure ($$\hbox {100 kg}~\hbox {year}$$ 100 kgyear ). It will reach thereby a half-life sensitivity of more than $$10^{26}$$ 1026 year within 3 years of data collection. This paper describes in detail the modifications and improvements of the experimental setup for Phase II and discusses the performance of individual detector components.

Abstract

We present the first search for bosonic superweakly interacting massive particles (super-WIMPs) as keV-scale dark matter candidates performed with the GERDA experiment. GERDA is a neutrinoless double-β decay experiment which operates high-purity germanium detectors enriched in Ge76 in an ultralow background environment at the Laboratori Nazionali del Gran Sasso (LNGS) of INFN in Italy. Searches were performed for pseudoscalar and vector particles in the mass region from 60 keV/c2 to 1 MeV/c2. No evidence for a dark matter signal was observed, and the most stringent constraints on the couplings of super-WIMPs with masses above 120 keV/c2 have been set. As an example, at a mass of 150 keV/c2 the most stringent direct limits on the dimensionless couplings of axionlike particles and dark photons to electrons of gae<3×10−12 and α′/α<6.5×10−24 at 90% credible interval, respectively, were obtained.

Abstract

LEGEND searches for lepton number violation in neutrinoless double beta (0νββ) decay using $^{76}$Ge-enriched high-purity germanium (HPGe) detectors immersed in liquid argon. The first stage, LEGEND-200, will operate 200 kg of detectors, a significant fraction of which are the new large Inverted Coaxial Point Contact (ICPC) detectors currently under production. An extensive characterization campaign to determine the basic operational parameters of the newly produced detectors is performed in vacuum cryostats at two underground sites in order to reduce the internal creation of cosmogenic radionuclides. Other dedicated scanning setups are used to obtain a detailed understanding of alpha and beta surface events on the p+ and n+ contacts which are projected to be the dominant backgrounds for the 0νββ decay search. This poster will present the production experience of the new ICPC detectors, the ongoing characterization campaigns, and the projected detector performances based on recent measurements.

Abstract

Approximately 30 % of patients suffering from severe valvular heart disease, such as mitral valve regurgitation are non-compliant to the gold standard of minimally invasive surgery, reconstruction or valve replacement. The number of these mostly old patients with severe comorbidities is increasing; therefore, transcatheter interventions have been developed to address an unmet clinical need and may be an alternative therapeutic option to the reference standard. Apart from the successful MitraClip therapy, alternative transcatheter reconstruction technologies are being developed. As with transcatheter aortic valve implantation (TAVI) procedures, the off-pump implantation of a valved stent into the mitral position mainly via a transapical approach will be of great benefit. Recently, the feasibility of transcatheter mitral valved stent implantation in high-risk patients has already been reported.

Abstract

The BiPo-3 detector, running in the Canfranc Underground Laboratory (Laboratorio Subterr\'aneo de Canfranc, LSC, Spain) since 2013, is a low-radioactivity detector dedicated to measuring ultra low natural radionuclide contaminations of $^{208}$Tl ($^{232}$Th chain) and $^{214}$Bi ($^{238}$U chain) in thin materials. The total sensitive surface area of the detector is 3.6 m$^2$. The detector has been developed to measure radiopurity of the selenium double $\beta$-decay source foils of the SuperNEMO experiment. In this paper the design and performance of the detector, and results of the background measurements in $^{208}$Tl and $^{214}$Bi, are presented, and validation of the BiPo-3 measurement with a calibrated aluminium foil is discussed. Results of the $^{208}$Tl and $^{214}$Bi activity measurements of the first enriched $^{82}$Se foils of the double $\beta$-decay SuperNEMO experiment are reported. Abstract

We report the results of a first experimental search for lepton number violation by four units in the neutrinoless quadruple-β decay of ^{150}Nd using a total exposure of 0.19 kg yr recorded with the NEMO-3 detector at the Modane Underground Laboratory. We find no evidence of this decay and set lower limits on the half-life in the range T_{1/2}(1.1-3.2)×10^{21} yr at the 90% C.L., depending on the model used for the kinematic distributions of the emitted electrons.

Abstract

Laserassistierte Thrombolyse ist eine neue mogliche Therapieoption bei der Behandlung der tiefen Venen Thrombose. Bei der Therapie der tiefen Bein- und Beckenvenenthrombose (TVT) dominiert nach wie vor der konservative Behandlungspfad. Neben der selten durchgefuhrten offen-operativen Thrombektomie liegt der Fokus der klinischen Praxis auf der therapeutischen Antikoagulation und der Thrombolyse. Dabei kommt es haufig lediglich zu einer Limitation des Thrombuswachstums und nicht zu einer vollstandigen Auflosung thrombotischen Materials und damit einhergehender Pravention embolischer Ereignisse. Ziel der vorliegenden Arbeit ist es, in einem In-vitro-Experiment den Vorteil einer laserkatheterassistierten Thrombolyse gegenuber der alleinigen pharmakologischen Lyse zu demonstrieren. Wir verwendeten das Chandler-Loop-System als extrakorporales Zirkulationsmodell zur Erzeugung roter Thromben. Es erfolgte eine Laserexposition der Thromben und anschliesend eine Lyse mit „recombinant tissue-type plasinogen activator“ (rt-PA). Das Thrombusgewicht der mit Laser und rt-PA behandelten Proben war signifikant geringer als das Thrombusgewicht der mit rt-PA behandelten Proben Aufgrund eines signifikant verringerten Thrombusgewichts im In-vitro-Experiment konnte die laserassistierte Thrombolyse zu einer gefasschonenden und risikoarmen Alternative bei der Therapie der TVT werden.

Abstract

Background and objectivesThe myocardial performance index is a non-geometric, heart rate-independent echocardiography-derived index of left ventricular performance combining systolic and diastolic function. There is an ongoing debate whether the myocardial performance index is affected by preload or not. Moreover, a systematic evaluation of the effect of changing tidal volume ventilation on the myocardial performance index is still lacking. The aim of our study was to assess whether acute changes in preload and/or different depth of tidal volume ventilation affect the myocardial performance index.MethodsIn all, 14 anesthetized pigs (35 ± 2 kg) were studied during changing tidal volumes (VT 5, 10 and 15 mL kg−1) at baseline, after removal of 500 cm3 of blood (haemorrhage) and after retransfusion of shed blood plus additional 500 cm3 6% hydroxyethyl starch (fluid loading). Echocardiographic measurements at each experimental stage included myocardial performance index, left ventricular end-diastolic area and fractional area change. Central venous pressure, pulmonary capillary wedge pressure, cardiac output and stroke volume index were obtained by a pulmonary artery catheter. Global end-diastolic volume was obtained by transpulmonary thermodilution.ResultsComparing different loading conditions, we found significant changes in cardiac output, stroke volume index, central venous pressure, pulmonary capillary wedge pressure, global end diastolic volume and left ventricular end-diastolic area, indicating clinically relevant changes in preload. In the haemorrhage group, there was a significant reduction in the myocardial performance index (P < 0.05) independent of tidal volume applied and this was reversed after fluid loading. However, myocardial performance index was significantly impaired (P < 0.05) by high tidal volume ventilation (15 mL kg−1), while tidal volumes of 5 and 10 mL kg−1 had no effect.ConclusionsThe myocardial performance index is largely dependent on changes in preload. Moreover, high tidal volume ventilation significantly impaired the myocardial performance index.

Abstract

Transcatheter aortic valve implantation (TAVI) can result in paravalvular leakage and stent deformation in the presence of severe calcification. This study was undertaken to determine the efficacy of laser-assisted resection of calcific aortic valve leaflets as a method to minimize the effects of calcium on perivalvular leakage during TAVI.A Q-switched Tm:YAG laser emitting at a wavelength of 2.01 μm was used to evaluate the cutting efficiency on highly calcified human aortic leaflets in vitro (N = 10). A pulse energy of 4.3 mJ, a pulse duration of 0.8-1 μs, and a repetition rate of 1 kHz were used. The radiation was transmitted via a 200 µm core diameter quartz fiber. Resection was performed in a fiber-tissue contact mode on water-covered samples in a dish. The remnant particles were analyzed with respect to quantity and size by light microscopy.A resection rate of 40.4 ± 22.2 mg/min on highly calcified aortic leaflets was achieved. This corresponds to a cutting speed of approximately 1 cm/min; a laser dissection time of 3 min per leaflet is expected. The majority of the remnant particles (85.4%) were6 μm in diameter, with only 0.1% exceeding 300 μm.The Q-switched Tm:YAG laser system showed promising results in cutting calcified aortic valves, by transmitting sufficient energy through a small flexible fiber. Catheter-based removal of aortic valve calcification may help to improve TAVI technology.

Abstract

The OPERA neutrino experiment in the underground Gran Sasso Laboratory (LNGS) was designed to perform the first detection of neutrino oscillations in direct appearance mode in the vμ→ v τ channel, the v τ signature being the identification of the τ -lepton created in its charged current interaction. The hybrid apparatus consists of a large mass emulsion film/lead target complemented by electronic detectors. Placed in the LNGS, it is exposed to the high-energy long-baseline CERN Neutrino beam to Gran Sasso (CNGS) 730 km away from the neutrino source. The observation of a first v τ candidate event was reported in 2010. In this paper, we discuss the result of the analysis of the data taken during the first two years of operation (2008-2009) underlining the major improvements brought to the analysis chain and to the Monte Carlo simulations. The statistical significance of the one event observed so far is then evaluated to 95%. © IOP Publishing Ltd and Deutsche Physikalische Gesellschaft.

Abstract

After analysis of 5797 h of data from the detector NEMO3, new limits on neutrinoless double beta decay of Mo-100 (T_{1/2} > 3.1 10^{23} y, 90% CL) and Se-82 (T_{1/2} > 1.4 10^{23} y, 90% CL) have been obtained. The corresponding limits on the effective majorana neutrino mass are: m 1.4 10^{22} y (90% CL) for Mo-100 and T_{1/2}> 1.2 10^{22} y (90%CL) for Se-82. Corresponding bounds on the Majoron-neutrino coupling constant are g < (0.5-0.9) 10^{-4} and < (0.7-1.6) 10^{-4}. Two-neutrino 2b-decay half-lives have been measured with a high accuracy, T_{1/2} Mo-100 = [7.68 +- 0.02(stat) +- 0.54(syst) ] 10^{18} y and T_{1/2} Se-82 = [10.3 +- 0.3(stat) +- 0.7(syst) ] 10^{19} y.

Abstract

The OPERA neutrino oscillation experiment is based on the use of the Emulsion Cloud Chamber (ECC). In the OPERA ECC, nuclear emulsion films acting as very high precision tracking detectors are interleaved with lead plates providing a massive target for neutrino interactions. We report on studies related to the effects occurring from the contact between emulsion and lead. A low radioactivity lead is required in order to minimize the number of background tracks in emulsions and to achieve the required performance in the reconstruction of neutrino events. It was observed that adding other chemical elements to the lead, in order to improve the mechanical properties, may significantly increase the level of radioactivity on the emulsions. A detailed study was made in order to choose a lead alloy with good mechanical properties and an appropriate packing technique so as to have a low enough effective radioactivity., Comment: 19 pages, 11 figures

36. Transapical mitral valve implantation: the Lutter valve

Abstract

The development of transcatheter techniques for treatment of severe mitral valve regurgitation in the beating heart is focus of recent research. An off-pump treatment technique poses great benefits, particularly for multimorbid patients, often being non-compliant to the gold standard treatment, being open heart surgery with use of a cardiopulmonary bypass. Thereto, two approaches are being followed: transcatheter valve repair and transcatheterimplantation of a valved stent into the native mitral valve annulus. A valved stent has to provide safe and secure fixation within the high pressure system of the left heart. One of the main challenges in the development of such a valved stent is the complex anatomy of the mitral valve, with no clearly defined structures for device anchorage. Our group has developed a self-expanding nitinolvalved stent for transapical implantation in the beating heart. During the development process of thevalved stent, different design iterations were conducted to decrease the risk of paravalvular leakages, to enhance the reproducibility and to improve the overall stent performance. This article reviews the major milestones passedin the development process of our mitral valved stent and advances achieved withinthe last years. Multiple design iterations lead to a prototype providing secure stent deployment, hig h reproducibility, low paravalvular leakages and only mild stent deformation in the beating heart. In future, further long-term in vivo trials have to be conducted before attempting the step towards clinical application of this novel device.

Abstract

We report results from the NEMO-3 experiment based on an exposure of 1275 days with 661 g of (130)Te in the form of enriched and natural tellurium foils. The ββ decay rate of (130)Te is found to be greater than zero with a significance of 7.7 standard deviations and the half-life is measured to be T(½)(2ν) = [7.0 ± 0.9(stat) ± 1.1(syst)] × 10(20) yr. This represents the most precise measurement of this half-life yet published and the first real-time observation of this decay.

Abstract

Background: Previous studies have shown that stroke volume variation (SVV) may be used to assess preload and fluid responsiveness. It is currently under debate, if SVV reliably displays changes in preload during ventilation with clinically used tidal volumes. This study was designed to evaluate whether the predictive value of SVV depends on the tidal volume applied particularly during acute changes of preload. Methods: We studied 14 anesthetized pigs (35 ± 2 kg) during changing tidal volumes (5, 10 and 15 ml/kg) at normovolemia (BL), after removal of 500 cc of blood (Hypo) and after retransfusion plus additional 500 cc 6% hydroxyethyl starch (Hyper). SVV was recorded continuously, and global end-diastolic volume (GEDV) was obtained by transpulmonary thermodilution at each experimental stage. Results: GEDV changed significantly comparing the different experimental stages (P < 0.0001). During ventilation with 5 ml/kg, SVV did not change significantly at the different loading conditions. In contrast, during ventilation with both 10 and 15 ml/kg, SVV changed significantly comparing hemorrhage to fluid loading. However, at 15 ml/kg SVV was above the recommended value throughout the experiment. Conclusions: In this animal model, SVV was not sensitive to acute changes in preload during ventilation with a tidal volume of 5 ml/kg. Moreover, ventilation with high tidal volume may suggest volume loading even after sufficient volume resuscitation.

Abstract

Two neutrino double beta decay of $^{76}$Ge to excited states of $^{76}$Se has been studied using data from Phase I of the GERDA experiment. An array composed of up to 14 germanium detectors including detectors that have been isotopically enriched in $^{76}$Ge was deployed in liquid argon. The analysis of various possible transitions to excited final states is based on coincidence events between pairs of detectors where a de-excitation $\gamma$ ray is detected in one detector and the two electrons in the other. No signal has been observed and an event counting profile likelihood analysis has been used to determine Frequentist 90\,\% C.L. bounds for three transitions: ${0^+_{\rm g.s.}-2^+_1}$: $T^{2\nu}_{1/2}>$1.6$\cdot10^{23}$ yr, ${0^+_{\rm g.s.}-0^+_1}$: $T^{2\nu}_{1/2}>$3.7$\cdot10^{23}$ yr and ${0^+_{\rm g.s.}-2^+_2}$: $T^{2\nu}_{1/2}>$2.3$\cdot10^{23}$ yr. These bounds are more than two orders of magnitude larger than those reported previously. Bayesian 90\,\% credibility bounds were extracted and used to exclude several models for the ${0^+_{\rm g.s.}-0^+_1}$ transition.

Abstract

Surgical revascularization allows especially for the diabetic patient with coronary multi-vessel disease a superior long-termperspective compared to percutaneous coronary interventions. However, the specific advantages and also risks associated with the expanded use of arterial grafts as coronary conduits in diabetics are not clearly answered. A systemic analysis of the current literature underlines the following statements: The radial artery is, compared to the internal mammary artery, to a higher extent prone to atherosclerosis, especially in diabetic patients. Better long-term survival for patients with bilateral IMA-grafts has also been assessed for diabetic patients. The risk of thoracic wound infections is not obligatory increased for diabetics with double IMA-grafts. Revascularization with radial arteries can be performed with low perioperative risks. Long-term results or studies on radial artery bypass grafting especially in diabetics are not available. In our own experience (3,548 isolated coronary operations, 01/2001 until 05/2005), diabetic patients have no increased perioperative mortality, even when using more complex arterial grafting. Conclusively, diabetic patients should also significantly benefit from the use of the second mammary artery. Regarding conduit quality, a second IMA appears favorable over the radial artery.

Abstract

OBJECTIVE This experimental study was initiated to determine whether TMLR may prevent porcine myocardium from ischemia and necrosis after acute myocardial infarction. In addition, the influence of TMLR on healthy myocardium was analyzed. METHODS The short-term effectiveness of TMLR was evaluated in 38 open-chest anesthetized pigs with (n = 18) or without (n = 20) acute LAD occlusion (observation period 6 hours): Six pigs served as controls (thoracotomy only). An additional six pigs had LAD occlusion only (ischemic group). A subsequent 12 pigs were treated by TMLR (CO2) prior to LAD occlusion: Six pigs received one laser channel/cm2 (group 1) and in six pigs two channels/cm2 in the LAD territory (group 2) were performed. In addition, 14 pigs underwent TMLR without ischemia: Seven pigs received 1 channel/cm2 (group 3) and seven pigs 2 channels/cm2 (group 4). Pathomorphological assessment and histology were performed. RESULTS TMLR limits the expansion of the myocardial infarction zone: laser group 2 demonstrated a significantly smaller area of necrosis in the area at risk (ischemic group (32%) vs. laser group 1 (18%, p = ns) and 2 (8%, p = 0.0076); laser group 1 vs. 2, p = 0.0056). The amount of the area of necrosis of laser groups 3 (0.4%) and 4 (0.04%) compared to control (0%) did not differ significantly (p = ns). Furthermore, in the lased territories of laser groups 3 and 4 microscopic analysis revealed signs of ischemia in 10 +/- 30.9% of all examined histological samples (p = ns vs. control). During a short coronary occlusion the laser-induced tracks were partially filled with blue dye in 94.8 +/- 27.0/85.9 +/- 34.3/94.85 +/- 22.0%/70.21 +/- 47.0% (laser groups 1 - 4 respectively p = ns) The myocardial water content-measurements (MWC) of the ischemia and laser group 1 were not different at the end of the experiment (p = ns). In contrast, laser groups 2, 3 and 4 revealed significantly higher MWC values compared to control (p = 0.036, p < 0.001, p < 0.001; respectively). CONCLUSIONS This prolonged acute study demonstrates that preventive CO2-laser revascularization significantly reduces the amount of necrosis in the area at risk. Histological examination supported the idea that some pigment gained access to the ischemic tissue via patent channels. In healthy myocardium, TMLR significantly increases myocardial water content and induces non-significant small ischemic and very small necrotic areas surrounding open laser channels.

Abstract

Does transmyocardial laser revascularization (TMLR), a new surgical technique for treating patients with otherwise intractable angina pectoris, improve myocardial perfusion, metabolism, and, consequently, function?Patients referred for TMLR, alone or with coronary artery bypass grafting (CABG), were preoperatively evaluated clinically and by treadmill stress testing, echocardiography, ventriculography, radionuclide assessment of perfusion and metabolism, and hemodynamic assessment. Intraoperatively it was decided that some patients only required CABG. Follow-up evaluations were repeated after 6 (n = 40) and 12 months (n = 23) and compared with preoperative values.CABG only was performed in 35 cases, TMLR + CABG in 17, TMLR only in 45. 1-year mortality was 11% in the TMLR, zero in the TMLR + CABG, and 11% in the CABG groups. In all groups a significantly improved CCS angina- and NYHA class was observed immediately after operation and after 6 and 12 months. In all study groups treadmill tolerance (p0.05) improved, but regional and global function, perfusion at rest, and metabolism were not significantly changed at 6 and 12-months follow-ups. Perfusion studies under stress demonstrated an improvement only in the CABG group after 12 months (p0.05), whereas in both TMLR groups the lasered ischemic segments remained unchanged.TMLR significantly improves long-term clinical status and treadmill stress tolerance, but appears to have little if any effect upon regional and global function, perfusion, and metabolism.

Abstract

Across centers, transfusion can vary eightfold for the same mediastinal drainage after adult cardiac operations. Excessive blood drainage resulting in increased transfusions occurs in 29% of patients. Various strategies have been proposed to decrease bleeding and allogenic transfusion requirements in the perioperative period of heart surgery. Blood conservation methods were reviewed critically. Avoidance of preoperative anemia, tolerance of low hemoglobin concentrations, the use of autologous blood, and adherence to a strict transfusion protocol will reduce the use of allogenic transfusions. Perioperatively, maintenance of normothermia contributes to improved hemostasis.

Does transmyocardial laser revascularization (TMLR) as a new surgical technique for treating patients with otherwise intractable angina pectoris improve myocardial perfusion or contractility?Sixty-seven patients transferred for TMLR were evaluated by clinical evaluation, treadmill stress testing, echocardiography, ventriculography, and hybrid positron emission tomography preoperatively and in patients treated with TMLR at 6 and 12 month follow up. Hemodynamic assessment and clinical evaluation were performed perioperatively.In 28/67 cases (42%) CABG, in 9/67 patients (13%) CABG in combination with TMLR (combined group), and in 30/67 patients (45%) only TMLR (sole group) were performed. Perioperative mortality in the sole group was 13%, in the combined group zero, and in the CABG group 11%. In all groups a significantly improved clinical status (por = 0.01) 1 week postoperatively and in TMLR groups also at 6 and 12 months was observed. In the TMLR groups treadmill tolerance (p0.05) improved, although function, perfusion, and metabolism did not change significantly at the 6 and 12 month follow up.TMLR significantly improves clinical status and treadmill stress tolerance, but does not change function, perfusion, and metabolism.

48. Transmyocardial laser-revascularization: experimental studies on prolonged acute regional ischemia

Abstract

This experimental study in pigs was undertaken to answer the question whether TMLR after acute myocardial infarction may improve regional myocardial perfusion, left ventricular function and diminish myocardial necrosis in the area at risk.Thirty open-chest anesthetized pigs were observed for 6 h, six pigs served as controls. In 24 pigs, occlusion of the left anterior descending artery (LAD) beyond the first diagonal branch was performed: seven pigs had LAD occlusion only (ischemia group), and 17 pigs were treated by TMLR (using a CO2-laser, energy: 40 J) prior to coronary occlusion; nine pigs received one laser channel (1 mm diameter) per cm2 (laser group 1) and eight pigs two channels per cm2 in the LAD territory (laser group 2). Regional myocardial blood flow by microspheres, function (franc starling curves), histochemical assessment (triphenyl tetrazolium chloride, TTC and histology), were performed.The lased pigs were less prone to ventricular fibrillation (laser group 2, 38%; laser group 1, 56%; ischemic group, 100%; P0.05), and showed a significant smaller area of necrosis (TTC) in the area at risk (laser group 1, 23%; laser group 2, 14%; vs. ischemia group, 31%; P0.01). There was no significant difference between laser-treated and ischemia hearts regarding the amount of blood flow into the infarcted LAD region and the maximal left ventricular stroke work index after 6 h (P = n.s). Regional myocardial blood flow: ischemia group, 4 +/- 5 ml/100 g/min; laser group 1, 3 +/- 10 ml/100 g/min, and laser group 2, 2 +/- 10 ml/100 g/min; maximal left ventricular stroke work index: ischemia group, 1.8 mJ/g; laser group 1, 2.1 mJ/g and laser group 2, 2.1 mJ/g.This model of acute regional ischemia demonstrates that CO2-laser revascularization diminish significantly the incidence of ventricular fibrillation and necrosis in the area at risk, and does not change regional myocardial perfusion and global left ventricular function. This experiment indicates that TMLR may be an alternative in treating advanced ischemic heart disease.

Approximately three quarter of sarcoma arise from soft-tissue, about 15% are gastrointestinal stromal tumours (GISTs) and bone sarcoma represent the remaining 10%. The current guidelines will focus on soft-tissue and GIST, excluding Kaposi sarcoma and non-pleomorphic rhabdomyosarcoma
