2. Risk factors for prolonged hospitalization as a marker for difficult-to-manage exacerbations of chronic obstructive lung disease (COPD): the DiMECO Study

Abstract Background Exacerbation is an independent risk factor for chronic obstructive pulmonary disease (COPD)-related morbidity and mortality. Despite optimal care, there may be risk factors that lead to difficulties in managing exacerbations that may be associated with prolongation of length of hospital stay (LOS). Methods This is a multicenter prospective observational study of COPD patients hospitalized with exacerbations. Prolonged LOS was calculated according to the 50th percentile and defined as ≥ 9 days. Potentially predicting factors of LOS were stratified into 4 pillars as patient-related, disease and exacerbation-related, treatment-related and, hospital utility-related. These categories were systematically documented throughout the duration of the hospitalization. Results A total of 434 patients, 361 males and 73 females, with a mean age of 69.2 ± 9.3 years, were included in the study. Variables of each pillar were tested with univariate analysis to identify potential risk factors for prolonged LOS. Subsequently significant factors excluding factors associated with hospital utility were tested with multivariate logistic regression analysis for detecting potential associated factors for difficult-to-manage COPD exacerbation. Biomass exposure, past history of non-invasive mechanical ventilation (NIMV), low bicarbonate levels at admission, antibiotic switching, need for theophylline, increasing oxygen requirement, need for in-hospital non-invasive mechanical ventilation, nutritional support and physiotherapy were found as defining factors. Conclusions The DiMECO study can help to identify COPD exacerbators who are at risk for prolonged hospitalizations that may associate with difficult-to-manage COPD exacerbations. Difficult to manage COPD exacerbation may serve as a provocative framework, underscoring the necessity for a better understanding of the multifaceted approaches to the management of COPD exacerbations. This conceptualization warrants further investigation across diverse clinical settings to validate its applicability and efficacy.

OBJECTIVE: Chronic obstructive pulmonary disease (COPD) that is the third leading cause of death in the world is one of the main economic burden. The cost is primarily due to COPD exacerbations and hospitalizations. We aimed to determine the factors associated with increasing costs in severe COPD exacerbation. MATERIAL AND METHODS: It was a multicenter and prospective observational recording study. 294 patients who severe COPD exacerbation were included in the study. An amount of more than 429.58 euros was accepted as increasing costs (IC). Factors associated with IC were determined by regression analysis. RESULTS: Mean age was 69.90 ± 9.79/years (minimum: 40 maximum: 95), mean costs were 594.9 ± 70.9 euros. About 83.7% of the patients were male, 24.1% (71) were active smokers, and 81% (238) had at least 1 comorbidity. Factors associated with IC in the regression analysis were delay of discharge (due to prolonged consultation), antibiotic use longer than 7 days, need to enteral/parenteral feeding, application of pulmonary rehabilitation (physiotherapy) at hospitalization, and refusal to be discharged. CONCLUSION: The increasing costs in severe COPD exacerbation depends not only treatment but also the patient’s social status and hospital-related factors. We think that the cost of severe COPD exacerbation can be reduced by interventions on interchangeable factors such as patient's social status and hospital-related factors.

Abstract Aims Left ventricular non‐compaction cardiomyopathy (LVNC) is a poorly understood entity resulting in heart failure. Whether it is a distinct form of cardiomyopathy or an anatomical phenotype is a subject of discussion. The current diagnosis is based on morphologic findings by comparing the compacted to non‐compacted myocardium. The study aimed to compare demographic and prognostic variables of patients with dilated cardiomyopathy (DCM) and LVNC. Emphasis was given to cardiac magnetic resonance (CMR) imaging analysis. Data on survival were also assessed. Methods and results We retrospectively evaluated the characteristics and outcomes of 262 non‐ischaemic cardiomyopathy patients with LVNC and DCM phenotypes. Petersen's CMR criteria of non‐compacted to the compacted myocardial ratio 2.3 were used to diagnose LVNC. The primary endpoint was a composite endpoint of major adverse cardiovascular events comprising cardiovascular‐related death, left ventricular assisted device implantation, or heart transplantation. A total of 262 patients with CMR data were included in the study. One hundred fifty‐five patients who fulfilled CMR criteria were diagnosed as LVNC. CMR findings revealed that LVNC patients had higher left ventricular end‐diastolic (137.2 ± 51.6, 116.8 ± 44.6, P = 0.002) and systolic volume index (98.4 ± 49.5, 85.9 ± 42.7, P = 0.049). 8. An Instructional Design with iTunes U to Develop One-to-One Teaching Strategies in K12 Schools

- Abstract
The tablet is one of the most important developments of the educational environments today. It is used at schools especially at private schools. The number of teacher and student using the tablet is rising day by day due to the development of digital instructional objects. Many approaches are being used in the schools and companies where tablets are used for education. LMS's which are introduced to the market and applications which can be reached from application stores are being used more effectively classrooms via web software. The most important points that schools and teachers have to emphasize are: (1) How to run the strategies of education with the help of BYOD or tablets given to students. (2) To form 1:1 instructions for teaching with the help of tablets. Modelling of education to be studied on is applied with functions like: (1) Getting students involved before the lesson; (2) Getting them taking part in the lessons in an efficient way; and (3) Make them a part of the process even when the lesson is over. This instructional model has started at a private school with 310 students and 160 teachers during 2014/15 in Istanbul, Turkey and continuing with closely 1000 students now. In this study, the focus is on the model that has been in progress.

Background: Asthma controller medications can be delivered via pressurized metered dose inhaler (pMDI) or dry powder inhaler (DPI) devices. Objective: This study aimed to evaluate the frequency of exacerbations and satisfaction rate with device use in asthmatics using pMDIs or DPIs. Methods: A multicenter, cross-sectional study was conducted in adults who used pMDIs or DPIs with correct inhaler technique and good adherence for asthma treatment. Demographic and asthma-related characteristics of the subjects and data regarding device satisfaction were collected through a face-to-face interview in the outpatient clinic. Rates of pMDI and DPI users and the data were compared between the two groups. Results: The study included 338 patients (mean age: 48.6 ± 14.5 years, 253 [74.9%] women). Among participants, 96 (28.4%) were using pMDI and 242 (71.6%) were using DPI. The age of patients using pMDI were significantly lower compared with DPI users. No significant difference was observed in terms of device satisfaction and clinical outcomes of asthma between pMDI and DPI users with good inhaler technique and good adherence. Conclusion: More asthmatics use DPIs, however, pMDIs are used in younger asthmatic patients. No significant difference in terms of device satisfaction and clinical outcomes of asthma was observed between pMDI and DPI users. [ABSTRACT FROM AUTHOR]

Aim: To present the characteristics of drug hypersensitivity reactions (DHRs) among taxane recipients with non-small cell lung carcinoma (NSCLC), and to describe the results of rapid drug desensitization (RDD). Methods: A retrospective cross-sectional study included 45 patients who were treated with taxane for NSCLC and were found to be hypersensitive to taxane. All patients were administered the standard 3-bag, 12-step RDD protocol following the development of DHR. RDD success was evaluated separately for each cycle, and successful RDD was defined as the completion of the cycle with application of 12 steps of the desensitization protocol and the absence of early and/or late reactions afterwards. Results: Among 45 patients hypersensitive to taxane 43 (95.6%) successfully received taxane cycles with desensitization. Failed RDD occurred in only 2 (4.4%) patients. The total number of desensitization cycles was 183, of which 181 (98.9%) were successful. The mean age of patients with successful desensitization was 59.42 ± 10.48 years and 37 (86.0%) of them were male. Conclusion: RDD is a reliable procedure that enables effective administration and completion of first-line taxane treatments in taxane-sensitive patients. [ABSTRACT FROM AUTHOR]

In particular, we show that if $ab$ is strongly clean, then so is $ba$. Consequently, we see that if $1-ab$ is strongly clean, then so is $1-ba$. Also, some characterizations are obtained for some certain elements in a corner ring. It is shown that for an idempotent $e$ and any arbitrary element $a$ in a ring, $ea+1-e$ is Drazin invertible if and only if $eae$ is Drazin invertible., Comment: 9 pages

We obtain some new characterizations of quasipolar and strongly $\pi$-regular elements by using quasipolar general rings. We see that quasipolar general rings lies between strongly $\pi$-regular and strongly clean general rings. Objective: Peripartum cardiomyopathy (PPCM) diagnosis made by excluding identifiable causes of heart failure (HF) and occurs end of the pregnancy or during the postpartum period of five months. It presents a clinical HF spectrum with left ventricular systolic dysfunction. Background: The purpose of this study is to retrospectively evaluate the clinical characteristics, cardiac magnetic resonance (CMR) imaging features, and end-points consisting of left ventricle recovery, left ventricular assist device implantation, heart transplantation, and all-cause mortality. Method: Outpatient HF records between 2008 to 2021 were screened. Thirty-seven patients were defined as PPCM. Twenty-five patients had CMR evaluation at the time of diagnosis, and six patients were re-evaluated with CMR. Results: The mean age was 30.5±5.6 years, and the mean LVEF was 28.2±6.7%. In thirteen(35.7%) patients, LVEF recovered during the follow-up course. The median recovery time was 281(IQR [78-358]) days. LVEF on CMR was 35.3±10.5, and three patients exhibited late gadolinium enhancement(LGE) patterns. Sub-endocardial and mid-wall uptake pattern types were detected. 18(75%) patients met the Petersen left ventricle non-compaction cardiomyopathy(LVNC) criteria. Patients with NC/C ratio lower than 2.3 had lower LVEDVi and LVESVi (124.9±35.4,86.4±7.5, p=0.003;86.8±34.6,52.6±7.6, p=0.006), respectively. The median follow-up time was 2129 (IQR [911-2634]) days. The primary endpoint-free one-year survival was 88.9%(event rate 11.1%), and five-year survival was 75.7%(event rate 24.3%). Conclusion: In a retrospective cohort of PPCM patients, 35.7% of patients’ LVEF recovered, and the primary end-point of free-five-year survival was 75%. Twenty-five patients were assessed with CMR; three of four met the Petersen CMR-derived LVNC at initial evaluation.

- Abstract
Introduction: Renal dysfunction in heart failure (HF) patients is associated with poor outcomes. Reduced cardiac index (CI) and right atrial pressure (RAP) are postulated to be a contributor the renal dysfunction. This study aimed to investigate the relationship between the estimated glomerular filtration rate (eGFR) and the pulmonary artery catheterization (PAC) results. Patients and Method: Hospitalized advanced HF patients, between 2016-2020 PAC performed included in the study. Renal dysfunction was defined as eGFRResults: 181 patients were included in the study, and the mean left ventricular ejection fraction (LVEF) was 20.9±3.7%, the mean eGFR was 79.8±25.4 ml/min/1.73 m , and 22.7% of patients had eGFR lower than 60 ml/min/1.73 m . CI (1.85±0.72; 1.84±0.64; p=0.47, respectively) and RAP (13.1±6.6; 13.7±6.8; p=0.61,respectively) was not significantly associated with renal dysfunction in HF patients. In the multivariable model, smoking history, AF, body mass index (BMI) revealed negative relation with eGFR, continuing ACEi or ARB therapy, and pulmonary artery capacitance index(PAC-i) were positively related variables with eGFR (pConclusion: These results indicate that CI or RAP is not the primary driver for eGFR. OBJECTIVE: Chronic obstructive pulmonary disease is currently the fourth leading cause of death in the world. Pulmonary rehabilitation is recommended for chronic obstructive pulmonary disease. MATERIAL AND METHODS: This study aimed to evaluate the effects of non-invasive ventilation, supplemental oxygen, and exercise training and supplemental oxygen during exercise training during pulmonary rehabilitation practice in comparison with only exercise training on lung functions, blood gases, lactate levels, respiratory muscle pressures, dyspnea, walking distances, quality of life, and depression in patients with severe chronic obstructive pulmonary disease. The main outcome measure is exercise capacity (6-minute walk test), and the secondary end-point included quality of life. RESULTS: Thirty-five patients (mean ± SD age, 65.4 ± 6.5 years) with a mean bronchodilator forced expiratory volume in the first second of expiration of 39.4 ± 7%, undergoing an 8-week outpatient pulmonary rehabilitation, were randomized to either non-invasive ventilation, supplemental oxygen, and exercise training, supplemental oxygen during exercise training, or exercise training groups. The improvements in respiratory muscle strength were higher in non-invasive ventilation, supplemental oxygen, and exercise training patients than the moderate improvements in the exercise training group. Both non-invasive ventilation, supplemental oxygen, and exercise training and supplemental oxygen during exercise training groups showed significant increases in the 6-minute walk test and incremental shuttle walk test. However, the increase in walking distance was better in non-invasive ventilation, supplemental oxygen, and exercise training group (69.8 ± 53.2 m in 6-minute walk test and 66.6 ± 65.2 m in incremental shuttle walk test, P = .001 and P = .005, respectively) compared to supplemental oxygen during exercise training group (42.5 + 55.5 m in 6-minute walk test and 53.5 + 70.2 m in incremental shuttle walk test, P = .01 each, respectively). The total St. George's Respiratory Questionnaire score was similar in all study groups after the intervention. Symptoms of depression significantly improved only in non-invasive ventilation, supplemental oxygen, and exercise training group (-2.8 + 2.8, P = .006). Tüm kararlar, Fakülte ve Dönem Temsilcileri, Ege Tıp Öğrenci Kurulu ile yapılan toplantılar, öğrenci ve öğretim üyelerinin geri bildirimleri doğrultusunda “öğrenen merkezli” olacak şekilde alınmış ve yürütülmüştür. Uzaktan öğretim kapsamında, öğrenme materyalleri Fakültemizin Öğrenme Kaynakları Merkezi (ÖKM) sunucusuna yüklenmiş; senkron dersler ise Üniversitemizin sağlamış olduğu çevrimiçi alt yapı ile gerçekleştirilmiştir. Eğitim programımızda yatay ve dikey entegrasyonu güçlendiren multidisipliner entegre oturum/panellerin tamamı ve bazı teorik dersler blok yürütme kurullarının organizasyonuyla senkron olarak yapılmıştır. Çevrimiçi Ölçme Değerlendirme Usul ve Esasları oluşturulmuş, sınavlar ödev şeklinde planlanarak ÖKM üzerinden uygulanmıştır. Mezuniyet sonrası uzmanlık eğitiminin sürekliliği çevrimiçi ortamda yapılan seminer, olgu tartışmaları, konseyler ve devam eden sağlık hizmeti ile sağlanmıştır. The decisions taken in the sub-commissions and boards, after being discussed and accepted by the Education Commission, entered into force with the approval of the relevant boards of the faculty and the university. All educational changes have been dynamically shared with internal stakeholders. All decisions were taken and carried out in a “learner-centered” manner in line with the meetings held with the Faculty and Term Representatives, Medical Student Council, and the feedback of the students and faculty members. Within the scope of distance education, learning materials were uploaded to the server of our Faculty's Learning Resources Center (OKM); Synchronous lessons were carried out with the online infrastructure provided by our university. In our education program, all of the multidisciplinary integrated sessions/panels that strengthen horizontal and vertical integration and some theoretical courses were held synchronously with the organization of the block executive committees. Online Measurement and Evaluation Procedures and Principles were established; exams were planned as homework and applied over the OKM. Continuity of postgraduate residency training was provided by online seminars, case discussions, councils, and ongoing healthcare services. Hypertension is an important public health problem due to its high prevalence and common complications. The aim of this study was to evaluate the quality of videos on YouTube related to reducing blood pressure and hypertension treatment. Using the six search terms "hypertension treatment", "cure hypertension", "hypertension medication", "control high blood pressure", "lower high blood pressure", and "reduce high blood pressure", a total of 360 relevant videos on YouTube were evaluated. Some parameters, i.e., the length of the video, number of days on YouTube, number of view counts, and number of likes, dislikes, and comments, were noted for all videos. The Global Quality Scale (GQS) was used to evaluate the quality of the videos. We categorized the video content as useful or misleading using the evidence-based medical literature. After exclusions, 104 videos were evaluated by two independent reviewers. Out of all the videos, 51% were useful, and 49% were misleading. Videos mentioned lifestyle changes (LCs) more (65 videos, 62.5%), and only 39.4% (41 videos) of all the videos contained information about pharmacological treatment (PT). Videos about alternative treatment (AT) had high numbers of views, and videos about PT had low numbers of views, and this difference was statistically significant. Videos that did not include PT but did include LCs and AT had more likes, similar to the number of views. YouTube, which is an important source of information, can guide individuals to in reducing high blood pressure with nonpharmacological and pharmacological methods. Uploaders; doctors (31.7%), herbalists/nutritionists (18.3%), independent users (10.6%), chiropractors (6.7%), yoga teachers (4.8%), and others (27.9%). - Abstract
Rus biçimciliği; bir eserin edebîliğini, eserden hareket ederek kendine özgü kavramlarla ortaya koymaya çalışan nesnel bir eleştiri yöntemidir. Bu eleştiri yöntemine göre edebîlik; şiir ve şiire yakın türlerde “alışılmışın kırılması”, roman ve romana yakın türlerde ise “süje”nin kurgulanışı ile sağlanır. Alışılmışın kırılması; bir metinde, günlük dilde kanıksanan yapılardan farklı dilsel formların oluşturulmasıdır. Süje ise olay ve durumların gerçekte olması beklenenden farklı şekilde organize edilmesidir. Alışılmış bir yapı okur tarafından otomatik bir şekilde algılanır, bu yüzden de yapı fark edilmez hâle gelir ve beklenen edebî etki ortaya çıkmaz. Otomatik algılamanın zararlı etkisi ancak bu olumsuz durumun ortadan kaldırılmasıyla yani alışılmış yapının farklılaştırılmasıyla aşılabilir. Temel olarak bu iki düzenleme, metnin otomatik algılanmasının önüne geçip onu algılanır hâle getirir ve edebîliği sağlar. Haldun TANER, Türk hikâyeciliğinin öncü ve önemli bir yazarıdır. Modern hikâye tekniklerini ustaca kullanan yazar, edebîliği eserlerinde etkili ve özgün şekilde yaratmıştır. Kahramanının bir köpek olduğu “Sancho’nun Sabah Yürüyüşü”, yazarın edebîliği sağlamada dikkat çekici ve özgün kullanımlar sergilediği bir hikâyesidir. Bu çalışmanın amacı, “Sancho’nun Sabah Yürüyüşü”nün Rus biçimciliği açısından eleştirisidir. Hikâyede, edebîliğin nasıl oluşturulduğu alışılmışın kırılması ve süje kavramlarından hareketle örnekler üzerinden ortaya konmaya çalışılmıştır. Hikâyedeki yapı, bu kavramlar açısından incelenmiş ve edebîliğin nasıl yaratıldığı metinden hareketle aydınlatılmak istenmiştir. Eleştirinin sonunda ise hikâyenin edebî boyutu ile ilgili Rus biçimciliği açısından bir sonuca ulaşılması hedeflenmiştir.

Introduction Osimertinib is an approved therapy for patients with a Thr790met (T790M) mutation diagnosed with non-small cell lung cancer (NSCLC) that progresses during epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI) therapy. However, in 7–13% of patients, drug-related side effects lead to discontinuation of osimertinib treatment. In such cases, osimertinib desensitization is a treatment option that can be considered. Case report A 59-year-old female patient, who was followed up with the diagnosis of stage 4 NSCLC, was consulted to the allergy clinic because of urticaria. The patient developed urticaria plaques 20 h after the third dose of osimertinib tablet. Management & outcome With the diagnosis of osimertinib-induced urticaria, desensitization was planned for the patient. Treatment was started with a dose of 0.1 mg/day osimertinib. The procedure was completed in approximately 50 days, and a dose of 80 mg/day was reached with antihistamine suppression. Discussion Here, a successful osimertinib desensitization in a patient with a history of osimertinib-related type 1 allergic reaction is reported. Osimertinib desensitization is a treatment option that should be considered in cases where treatment has to be ceased due to drug-related side effects.

Background/aim: Our aim is to determine the caregiver burden of chronic obstructive lung disease (COPD) patient's caregivers, and to determine whether there is a workday loss. Materials and methods: 252 COPD patients and their caregivers were included. Disease information of the patients were recorded and a questionnaire was applied. Socio-demographic characteristics of the caregivers were recorded and a questionnaire consisting of 24 questions including COPD disease, treatment and loss of working days, and the Zarit Scale were used. Results: 128(50.8%) of the patients according to GOLD were group-D, 97(38.5%) of the patient's relatives were working, 62(24.7%) were not able to go to work for 1-14 days, and 125(57.1%) spent outside the home from 1-14 nights, because those accompanied to patients. In univariate analysis were detected modified medical research council (mMRC) (p < 0.001), CAT (p < 0.001), the number of comorbidities of patients (p = 0.027), forced expiratory volume in 1 FEV1cc (p = 0.009), FEV1% (p < 0.001), the presence of long term oxygen therapy (LTOT), and the number of comorbidities of the patient's relatives (p = 0.06) increased the care load. Aim: Osteoporosis is an important complication in patients with chronic obstructive pulmonary disease (COPD). The aim of this study is to determine the bone mineral density (BMD) and assess the relationship among bone mineral density, bone metabolism and the clinical data in patients with COPD. Patients and Method: 25 male COPD patients (mean age: 66±7.23 years) and 29 healthy male controls (mean age: 63.68±8.04 years) were enrolled into the study. COPD and control groups were assessed for hip (femoral neck and Ward’s triangle) and lumbar BMD, biochemical (blood calcium, blood phosphate, 24-hour urine calcium, creatinin clearance, osteocalcin, deoxypyridinoline) and hormonal [follicle-stimulating hormone (FSH), luteinising hormone (LH), free testosterone (sT), and parathyroid hormone (PTH)] markers. Results: No significant difference was found in BMD and T scores of hip and lumbar areas between COPD and control groups (p>0.05). When compared to the control group, COPD patients had significantly lower free testosterone (p=0.008) and significantly higher osteocalcin (p=0.0016) and PTH (p=0.00) values. Pulmonary function test and duration of disease were not correlate with lumbar and hip BMD in COPD group (p>0.05). Conclusion: Although BMD did not differ in any groups, some hormonal and biochemical markers were different in patients with COPD. Because osteoporosis is considered as an important complication to cause fractures, it is important to identify the risk groups for osteoporosis and take preventive and therapeutical measures. Purpose This study was performed to assess symptom variability and its impact on morning activities in stable patients with severe COPD in the Middle East and Africa (MEA) countries. Patients and methods Non-interventional, cross-sectional study (NCT03425760) in patients with severe COPD (GOLD 2015, C, or D categories). Symptom variability was assessed directly by interviewing the patient and using the Global Chest Symptoms Questionnaire (GCSQ). The impact on morning activities was assessed using the Capacity of Daily Living during the Morning (CDLM) and the Morning Activities and Symptoms Questionnaire (MASQ). Results A total of 3253 patients (mean±SD age: 64.1±9.5 years, 90.3% males) were enrolled. Overall, 81.6% and 83.4% of patients reported weekly and daily symptom variability, respectively. Bu çalışmada, Türkiye için temel bileşen analizi yöntemi aracılığıyla,üç ana finansal piyasadan ve Belirsizlik Anketi’nden belirsizliği temsil edenbir dizi değişken kullanılarak bir belirsizlik ölçütü oluşturulmuştur.Ardından, bu ölçütü kullanılarak, belirsizlik şoklarının Türkiye ekonomisiüzerindeki etkilerini tahmin etmek için bir vektör otoregresyon modelikurulmuştur. Sonuçlar, belirsizlik şoklarının sanayi üretimdeki düşüş, tüketicigüveninde bozulma ve işsizlik, enflasyon ve kredi faiz oranlarındaki artış ileilişkili olduğuna dair kanıtlar sunmaktadır. Belirsizlik şoklarının etkileri,ekonomik aktivitede düşüşe enflasyonda ise yükselişe neden olan arz şoklarınabenzer görünmektedir., In this paper, an uncertainty measure for Turkey is developed throughprincipal component analysis based on a number of uncertainty proxies fromthree main financial markets and the Expectations Survey. Then, using thismeasure, a vector autoregression model is constructed to estimate the effectsof uncertainty shocks on the Turkish economy. The results present evidence thatuncertainty shocks are associated with a fall in industrial production,worsening consumer confidence, and rises in unemployment, inflation and creditinterest rates. Uncertainty shocks seem similar to supply shocks, which causeeconomic activity to fall and inflation to rise.

Bu çalışmada, yükselen piyasa ve gelişmekte olan ekonomilerde finansal sektör gelişimi, ticari açıklık ve ekonomik büyüme arasındaki nedensellik ilişkisi incelenmiştir. Abrigo ve Love (2015)’in Panel Vektör Otoregresif yaklaşımı 86 ülkeden oluşan bir örnekleme, 1998-2016 dönemi için uygulanmıştır. Çalışmaya dahil edilen ülkeler gelir seviyelerine göre düşük ve alt-orta gelir ile üst-orta ve yüksek gelirli olmak üzere iki gruba ayrılmıştır. Böylelikle gelir seviyesindeki farklılaşmanın değişkenler arasındaki nedensellik ilişkilerini nasıl etkilediği araştırılmıştır. Çalışmanın ampirik bulguları, düşük ve alt-orta gelir ülke grubunda ekonomik büyümeden ticari açıklığa ve ticari açıklıktan finansal sektör gelişimine doğru nedensellik ilişkileri olduğunu göstermektedir. Ayrıca, düşük ve alt-orta gelir ülke grubu için finansal sektör gelişimi ekonomik büyümenin önemli bir belirleyicisi olarak bulunmuştur. Sonuçlar, üst-orta ve yüksek ülke grubu için de ekonomik büyümeden ticari açıklığa ve ticari açıklıktan finansal sektör gelişimine doğru Granger-nedensellik ilişkisinin olduğuna dair kanıt sunmaktadır

Amaç – Bu çalışmanın amacı Borsa İstanbul’da likidite düzeyi ile getiri oynaklığı arasındaki ilişkinin varlığı ve yönünün farklı likidite ölçütleri kullanılarak Ocak 2008 - Ağustos 2019 dönemi için ampirik olarak analiz edilmesidir. Yöntem – Likidite düzeyi ile getiri oynaklığı arasındaki ilişki VAR modeli kullanılarak Granger (1969) nedensellik testi ve genelleştirilmiş etki tepki analizleri ile incelenmiştir. Bu kapsamda likiditeyi temsil etmek üzere en yüksek-en düşük farkı, Amihud likidite yetersizliği ve oynaklık hacim oranı olmak üzere üç farklı likidite ölçütü hesaplanmıştır. Getiri oynaklığı ise GARCH (1,1) modeliyle tahmin edilmiştir. Bulgular – Bulgular, Borsa İstanbul için tüm likidite ölçütleri ile getiri oynaklığı arasında çift yönlü Granger nedensellik ilişkisi ortaya koymaktadır. Etki tepki analizleri ise likidite düzeyindeki bir şokun getiri oynaklığını hızla artırdığını ve söz konusu artışın etkisinin uzun sürdüğünü göstermektedir. Diğer taraftan, bulgular getiri oynaklığında meydana gelen bir şokun likidite düzeyini aşağı çektiğini, ancak söz konusu tepkinin getiri oynaklığının likidite şokuna verdiğine kıyasla daha az olduğunu ileri sürmektedir. Tartışma – Çalışma, firmaların ve politika yapıcıların likiditeyi artırmaya yönelik önlemler almasını desteklemektedir. Bu bağlamda, gelecekteki çalışmalarda likiditenin belirleyicilerine odaklanmak yararlı olacaktır.

Russian formalism is an objective method of criticism that tries toreveal the literariness of a work with its own unique concepts based onthe work. According to this criticism method, literature is provided by theconstruction of “defamiliarization” in poetry and genres close to poetry,and “syuzhet” in genres close to novels and novels. Defamiliarization is thecreation of linguistic forms in a text that are different from the structuresthat are taken for granted in daily language. Syuzhet is the term connotingthat events and situations are organized differently from what is actuallyexpected to happen. A familiar structure is automatically perceived bythe reader, as a result of which the structure becomes invisible and theexpected literary effect does not occur. This harmful effect of automaticdetection can only be overcome by differentiation of the usual structure.These two techniques prevent the automatic perception of the text, makeit perceptible and provide the literarily.Haldun Taner is a pioneer and important writer of Turkish narrative.The author, who skillfully uses modern story techniques, has created theliterary style in his works in an effective and original way. “Sancho’sMorning Walk”, in which the protagonist is a dog, is a story in which theauthor exhibits remarkable and original uses in providing literature.The aim of this study is to criticize “Sancho’s Morning Walk” in termsof Russian formalism. It has been tried to reveal how the literature iscreated in the story, based on the defamiliarization and syuzhet techniques.In the story, how syuzhet was formed and how the defamiliarizationprinciple was applied are tried to be revealed through examples. Thestructure in the story has been examined in terms of these concepts and ithas been tried to explain how the literature was created. It has been triedto reach a conclusion in terms of Russian formalism regarding the literarydimension of the story., Rus biçimciliği; bir eserin edebîliğini, eserden hareket ederek kendine özgü kavramlarla ortaya koymaya çalışan nesnel bir eleştiri yöntemidir. Bu eleştiri yöntemine göre edebîlik; şiir ve şiire yakın türlerde“alışılmışın kırılması”, roman ve romana yakın türlerde ise “süje”ninkurgulanışı ile sağlanır. Alışılmışın kırılması; bir metinde, günlük dildekanıksanan yapılardan farklı dilsel formların oluşturulmasıdır. Süje iseolay ve durumların gerçekte olması beklenenden farklı şekilde organizeedilmesidir. Alışılmış bir yapı okur tarafından otomatik bir şekilde algılanır, bu yüzden de yapı fark edilmez hâle gelir ve beklenen edebî etkiortaya çıkmaz. Otomatik algılamanın zararlı etkisi ancak bu olumsuz durumun ortadan kaldırılmasıyla yani alışılmış yapının farklılaştırılmasıylaaşılabilir. Temel olarak bu iki düzenleme, metnin otomatik algılanmasınınönüne geçip onu algılanır hâle getirir ve edebîliği sağlar.Haldun TANER, Türk hikâyeciliğinin öncü ve önemli bir yazarıdır.Modern hikâye tekniklerini ustaca kullanan yazar, edebîliği eserlerindeetkili ve özgün şekilde yaratmıştır. Kahramanının bir köpek olduğu “Sancho’nun Sabah Yürüyüşü”, yazarın edebîliği sağlamada dikkat çekici veözgün kullanımlar sergilediği bir hikâyesidir.Bu çalışmanın amacı, “Sancho’nun Sabah Yürüyüşü”nün Rus biçimciliği açısından eleştirisidir. Hikâyede, edebîliğin nasıl oluşturulduğu alışılmışın kırılması ve süje kavramlarından hareketle örnekler üzerinden ortaya konmaya çalışılmıştır. Hikâyedeki yapı, bu kavramlar açısındanincelenmiş ve edebîliğin nasıl yaratıldığı metinden hareketle aydınlatılmak istenmiştir. Eleştirinin sonunda ise hikâyenin edebî boyutu ile ilgiliRus biçimciliği açısından bir sonuca ulaşılması hedeflenmiştir.

Chronic obstructive pulmonary disease (COPD) is an important public health problem that manifests with exacerbations and causes serious mortality and morbidity in both developed and developing countries. COPD exacerbations usually present to emergency departments, where these patients are diagnosed and treated. Therefore, the Emergency Medicine Association of Turkey and the Turkish Thoracic Society jointly wanted to implement a guideline that evaluates the management of COPD exacerbations according to the current literature and provides evidence-based recommendations. In the management of COPD exacerbations, we aim to support the decision-making process of clinicians dealing with these patients in the emergency setting.
