9. Fethullahçı Terör Örgütünün (FETÖ) Hristiyan Örgütlerle Benzerlikleri: Karşılaştırmalı Sistematik Bir Değerlendirme.

Abstract

It is important for the strategy of struggle to analyze the historical and contemporary Christian organizations inspired by the Fetullah Terrorist Organization (FETÖ), which started out as a seemingly moderate religious movement and turned into a radical organization, and to identify their similarities. With the data obtained from multiple open sources by data mining method, the basic characteristics of FETÖ were determined under a total of forty-four titles, four of which are the history of the establishment and forty of which are descriptive. In the study, it was aimed to systematically analyze the similarities between the Christian religious organizations Jesuit, Opus Dei, Carbonari, Evangelist and Moon and FETÖ within the framework of these descriptive features. It is important that it is presented in an easy-to-understand manner in the form of a table with a comprehensive and systematic analysis, as well as being the first comprehensive analysis on this subject as well as being the strength of the study. While FETÖ, which has launched as an Islamic movement, shows similarities with Evangelism, one of the Protestant sects of Christianity, and Opus Dei, which is one of the Catholic sect organizations, in thirty-five of the forty-one sub-headings, it has been determined that it has similarities with Moonism in thirty-one sub-headings, and with Jesuits and Carbonaris in twenty-eight sub-headings. The main common feature of these six organizations, including FETÖ, is that they have transformed into violent organizational structures over time, regardless of their religious and sectarian affiliations. 18. Symptoms of rheumatoid arthritis and fibromyalgia and their relationship with anxiety

Abstract

Rheumatoid arthritis (RA) is an autoimmune disease that affects synovial attachments and negatively affects many organs and tissues. Non-inflammatory soft tissue rheumatism of the disorder and the disease. Pain is the most common and regular symptom in both diseases. The purpose of the study is to reveal the crises of pain with FMS and RA and psychiatric diseases such as anxiety or their association. The study included RA and FMS patients according to the 2010 ARA criteria. 50 patients with FMS and 50 patients with RA were included. If anxiety was present for both groups, the Hamilton Anxiety Scale (HAQ) was used for measurements, the Beck Depression Scale and Zung's vulnerability were used to determine the psychological dimensions of the copies, and the Visual Analogue Scale (VAS) was used to determine the pain intensity. The general care of the patients on a daily basis was examined. The patient and control group were compared to see if there was a significant difference in terms of these measurements.

21. Surgical Approaches to Iatrogenic Vascular Injuries in Adults.

Abstract

Surgical procedures can result in unforeseen vascular injuries. In cases of severe vascular injuries, the involvement of an experienced vascular surgeon in the operation can be life-saving. The aim of this study is to elucidate the surgical approaches and outcomes in vascular injuries that occurred during elective operations of various branches within our institution. Between October 2014 and June 2018, 18 patients required surgery due to unexpected severe vascular injuries. Gender, age, injury site, elective operation, intervention to vascular injury, hemodynamic status, type of anesthesia, mean operation time, and postoperative status of the patients were retrospectively obtained from hospital database. Diagnostic tests and imaging methods were recorded, and the utilization of blood products used during the operation has been investigated. The intensive care unit and service follow-ups of the patients as well as their post-discharge controls were collected from hospital records. Among the patients undergoing vascular intervention, 60% (n=11) experienced arterial injuries, while 40% (n=7) had venous injuries. When examining the type of intervention, all patients, except for one, underwent intervention by placing a side or cross-clamp on the vascular structure. Primary repair was performed in 11 patients, end -to-end repair in 2 patients, PTFE graft placement in 1 patient, saphenous vein interposition in 3 patients, and ligation in 1 patient. Two patients (11%) died during the surgery. As a result, the surgeon should accurately and quickly evaluate findings such as unexplained hypotension, tachycardia, loss of pulse or heat loss during the operation, and should suspect vascular injury. It is possible to reduce mortality and morbidity rates when iatrogenic vascular injuries are diagnosed early. [ABSTRACT FROM AUTHOR]

Abstract

It is important to analyze the historical and contemporary religious movements inspired by the Fethullahist Terrorist Organization (FETÖ), which started out as a seemingly religious organization about fifty years ago and turned into a terrorist organization on July 15, and to determine its similar methods, in terms of the strategy of struggle and future projection. As a result of the scans made from the open sources that can be reached, no comprehensive study could be found that systematically addresses the similarities between organizations on this subject under multiple headings. With the data obtained from multiple open sources, the basic descriptive features of FETO were determined under forty-four headings, three of which were related to its history and 40 were related to its organizational structure. Within the framework of these determined features, it was aimed to systematically analyze and discuss the aspects of FETÖ, which are similar to the Shiite Assassins and Sunni Qadianis, in a tabular form. Being the first comprehensive and systematic evaluation on this subject constitutes the strength of the study. While the study showed similarities with FETO's Shiite-Nizari Assassins in thirty of the forty sub-headings, it showed similarity with Sunni-Hanefi Qadianis in twenty-eight sub-headings. This situation made us think that two completely different sectarian currents such as Shiite and Sunni did not differ in being the source of FETO, only some sub-titles were more dominant in an organization. Although they are in different geographical and socio-cultural environments, it is important to evaluate the current results of a fifty-year-old FETO organization showing such similarities with a thousand-year-old Shiite Assassins and one hundred- and fifty-year old Sunni Qadianis. Abstract

Amaç: Bu çalışmanın amacı, mülteci üniversite öğrencilerinde travmatik deneyimlerin oranını ve travmalar ile anksiyete, yalnızlık, depresyon, travma sonrası gelişim ve ağrı arasındaki ilişkiyi araştırmaktır. Gereç ve Yöntemler: Bu çalışma Eylül 2017 ve Eylül 2018 tarihleri arasında Gaziantep Üniversitesi'nde gerçekleştirilmiştir. Değerlendirme için UCLA (California Üniversitesi, Los Angeles) Yalnızlık Ölçeği, Travma Sonrası Büyüme Envanteri (TSBE), Beck Depresyon Envanteri (BDE), Beck Anksiyete Envanteri (BAE), Travmatik Yaşantilar Kontrol Listesi (TYKL) kullanılmıştır. Bulgular: Çalışmaya toplam 71 öğrenci dahil edildi. UCLA ve BDE, TYKL arasında pozitif, UCLA ve TSBE arasında negatif korelasyon vardı. BDI, BAE ve TYKL arasında pozitif korelasyon vardı. TSBE ve BDE ile TYKL arasında negatif bir korelasyon saptandı. Bir gün içinde hissedilen ortalama ağrı süresi ile TYKL, BDE arasında anlamlı ve pozitif bir korelasyon ve ağrı süresi ile TSBE arasında negatif korelasyon gözlendi. Sonuç: Mülteci öğrencilerde travmatik yaşantıların fazla olması depresyon ile pozitif, travmatik büyüme ile negatif ilişkiliydi. Gün içinde hissedilen ortalama ağrı süresi, depresif belirtilerin fazlalığı ve travmatik olayların fazlalılığı ile pozitif ilişkilidir, ancak ağrının varlığı / ağrının lokalizasyonu ve değerlendirme ölçekleri arasında bir ilişki saptanmamıştır., Aim: The purpose of this study was to investigate the rate of traumatic experiences and the relationship between traumas and anxiety, loneliness, depression, posttraumatic growth and pain, in refugee university students. Materials and Methods: This study was conducted at the Gaziantep University between September 2017 and September 2018. UCLA (University of California, Los Angeles) Loneliness Scale, Posttraumatic Growth Inventory (PTGI), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Traumatic Events Checklist (T.E.C) were used for assessment purposes. Results: A total of 71 students were included in our study. There was a positive correlation between UCLA and BDI, T.E.C and a negative correlation between UCLA and PTGE. There was a positive correlation between BDI and BAI, T.E.C. A negative correlation was determined between PTGE and BDI and also T.E.C. A significant and positive correlation was observed between duration of pain felt during a day and T.E.C, BDI, and a negative correlation between duration of pain and PTGE. Conclusion: Excessive traumatic experiences were positively related with depression and negatively related with traumatic growth. The duration of pain felt during a day was positively related with the excess of depressive symptoms and traumatic events, however there was no relationship between the presence of pain/localization of pain and assessment scales.

34. The Effectiveness of Home-Based Pulmonary Rehabilitation in Elderly Patients with COVID-19 Infection

Abstract

While Coronavirus disease-2019 (COVID-19) affects the whole world, most affected group is geriatric individuals. There is a need for strategies for geriatric individuals returning to society and their social life. The aim of this study is to investigate the effect of an 8-week home-based Pulmonary Rehabilitation program on quality of life, depression, anxiety and functional capacity in geriatric individuals with COVID-19. Twenty-five patients were included in this prospective study. Patients over 65 years of age with COVID-19 were evaluated. Physical capacity with the 6-minute walking test (6MWT), dyspnea status with the Medical Research Council Dyspnea Scale, quality of life with the Short Form-36 (SF-36), depression and anxiety with the Hospital Anxiety and Depression Scale were evaluated. The mean age of the patients included in the study was 68.9 ± 6.4 years. 56% of the patients were men and 44% were women. When pre-treatment and post-treatment values were compared, a significant improvement was found in the 6MWT, dyspnea scores, SF-36 and all subgroups, anxiety and depression scores (p, Coronavirüs hastalığı-2019 (COVID-19) tüm dünyayı etkilerken en çok etkilenen grup geriatrik bireylerdir. Bu prospektif çalışmaya 25 hasta dahil edildi. COVID-19 geçirmiş 65 yaş üstü hastalar değerlendirildi. Fiziksel kapasite 6 dakikalık yürüme testi (6DYT) ile, dispne durumlar Medical Research Council Dispne Skalası ile, yaşam kalitesi Kısa Form-36 (KF-36) ile, depresyon ve anksiyete durumları ise Hastane Anksiyete ve Depresyon Ölçeği ile değerlendirildi. 44. The relationship between procalcitonin, D-dimer, ferritin, troponin and lactate levels with COVID-19

Abstract

Aim: Coronavirus disease-2019 patients may experience an increase in inflammation or clotting disorders. It is believed that some biomarkers can be decisive in decision to hospitalize. Our aim in this study was to investigate the relationship between serum procalcitonin, troponin, D-dimer, ferritin and lactate levels, and Coronavirus disease-2019. Methods: Patients suspected of suffering from the Coronavirus disease-2019 and whose serum biochemistry and blood gas tests were performed, were included in the study. The relationships between the blood parameters of the patients with negative or positive Polymerase Chain Reaction test results, and those who were hospitalized or non-hospitalized, were examined. Results: Out of a total of 452 patients, 244 (54%) were male and 208 (46%) were female. There was a statistically significant difference between polymerase chain reaction test positive and test negative groups in terms of procalcitonin, D-dimer, lactate and troponin levels (p˂0.005). There was no statistically significant difference between hospitalized and non hospitalized patient in terms of ferritin (p=0.224). However, there was a significant difference in terms of procalcitonin, D-dimer and troponin levels (p˂0.005). Conclusion: Our results revealed that high serum procalcitonin, D-dimer, lactate and troponin levels are associated with the decision to hospitalize Coronavirus disease-2019 patients, whereas ferritin levels played no such role.

47. Relationship between metabolic syndrome and vitamin D level in patients with ankylosing spondylitis

Abstract

Amaç: Bu çalışmanın amacı, Ankilozan Spondilit (AS) hastalarında D vitamin seviyesi ile Metabolik Sendrom (MetS) gelişimi arasındaki ilişkiyi ortaya koymaktır.Gereç ve Yöntem: Kesitsel tanımlayıcı tipteki bu çalışmaya 67 AS hastası dahil edildi. Sosyo-demografik veriler, bel çevresi ve sistolik kan basıncı değerlendirildi. Açlık kan şekeri, HDL kolesterol, Trigliserid ve Vitamin D değerleri kaydedildi. MetS teşhisi, Ulusal Kolesterol Eğitim Programının Yetişkin Tedavi Paneli'ne (NCEP / ATP III) gore yapılmıştır. Hastalık aktivitesi ve yaşam kalitesi değerlendirildi.Bulgular: Bu çalışmada AS hastalarında Vitamin D seviyeleri ile MetS arasında anlamlı negatif bir ilişki tespi tedildi. HDL seviyeleri ve Vitamin D seviyeleri MetS'li hastalarda anlamlı olarak daha düşüktü. Çoklu regresyon analizi, MetS'nin BMI, Trigliserit seviyesi, açlık glikozu ile pozitif olarak ilişkili olduğunu ve HDL kolesterol ve Vitamin D ile negatif olarak ilişkili olduğunu gösterdi.Sonuç: Bu çalışma, AS'li hastalarda MetS gelişiminde D vitamininin rolü olabileceğini göstermektedir. Bununla birlikte, ankilozan spondilitli hastalarda D vitamin ve metabolik sendrom arasındaki ilişkiyi açıklamak için daha fazla çalışmaya ihtiyaç vardır., Purpose: The purpose of this study is to reveal the relationship between the level of Vitamin D and the development of Metabolic Syndrome (MetS) in patients with Ankylosing Spondylitis (AS).Materials and Methods: 67 AS patients were included in this cross-sectional descriptive study. Socio-demographic data, waist circumference and systolic blood pressure were evaluated. Fasting blood glucose, HDL cholesterol, Triglyceride and Vitamin D values were recorded. Diagnosis of MetS was made according to National Cholesterol Education Program’s Adult Treatment Panel (NCEP/ATP III). Disease activity and quality of life were assessed.Results: In the present study, a significantly negative correlation was detected between Vitamin D levels and MetS in AS patients. HDL levels and Vitamin D levels were significantly lower in patients with MetS. Multiregression analysis showed that MetS was positively associated with BMI, Triglyceride level, fasting glucose and negatively associated with HDL cholesterol and Vitamin D. Conclusion: This study shows that vitamin D may have a role in the development of MetS in patients with AS. However, more studies are needed to explain the relationship between vitamin D and metabolic syndrome in patients with ankylosing spondylitis.

48. The relationship between AB0 blood groups and COVID-19

Abstract

Aim: Although COVID-19 infection spreads very quickly, not every individual is infected at the same rate. It should be investigated whether blood groups affect this difference in susceptibility to COVID-19. This study was conducted to investigate whether there was a relationship between AB0 and Rh blood groups and COVID-19 patients.Methods: A total of 535 patients suspected of COVID-19 who were admitted to the emergency departments and pandemic polyclinics of 3 different hospitals were included in this prospective cohort study. The patients were divided into two groups as those with positive and negative results according to the swab test. According to the ABO and Rh subgroups of the patients whose blood groups were known, each patient was evaluated for the presence of COVID-19. The data on patients' age, gender, complaints during the admission, blood groups, and swab test results were recorded in the study forms, and statistical analyses were performed.Results: The mean age of the patients was 45.5 (22.2) years. There were 291 (54.40%) males and 244 (45.60%) females. It was observed that most patients had A Rh + blood group (n=225, 42.1%). When age groups were compared in terms of gender, swab test results, and blood groups, no significant differences were found (P=0.307, P=0.316 and P=0.694, respectively). Conclusion: We found that no specific blood group increased the risk of getting infected with COVID-19. However, according to our results, those with A Rh – and A Rh +, 0 Rh - and AB Rh - blood groups had a higher risk of catching COVID-19, while those with 0 Rh + blood group had a lower risk., Amaç: COVID-19 enfeksiyonu toplumda çok hızlı yayılmasına rağmen her birey aynı hızla enfeksiyona yakalanmamaktadır. Bireylerin COVID-19’a yakalanmasındaki bu farklılıkların temelinde genetik olarak kan grupların bir etkisinin olup olmadığı araştırılmalıdır. Bu çalışma ABO ve Rh kan grupları ile COVID-19 hastaları arasında bir ilişki olup olmadığını araştırmak amaçlı yapılmıştır.Yöntemler: 3 farklı hastanenin acil servis ve pandemi polikliniklerine başvuran toplam 535 Covid-19 şüpheli hasta bu prospektif kohort çalışmasına dahil edildi. Hastalar, alınan sürüntü testine göre pozitif ve negatif sonuç olmak üzere iki gruba ayrıldı. Kan grupları belli olan hastalardan ABO ve Rh alt gruplarına göre her hasta Covid-19 varlığı açısından değerlendirildi. Hastaların yaş, cinsiyet, başvuru anındaki şikayetleri, kan grupları ve sürüntü test sonuçları verileri çalışma formlarına kayıt edilip istatistiksel analizleri yapıldı.Bulgular: Hastaların yaş ortalaması (standart sapması) 45,5 (22,2) yıl idi. Toplam 291 (%54,40) erkek ve 244 (%45,60) kadın hasta vardı. Kan gruplarına bakıldığında en fazla 225 (%42,1) ile A Rh + kan grubuna sahip hastalar olduğu görüldü. Yaş gruplarıyla; cinsiyet ile ve sürüntü test sonuçları ile kan grupları arasında karşılaştırıldığında anlamlı bir farklılık bulunmadı (sırasıyla P=0,307, P=0,316, P=0,694). Sonuç: Spesifik bir kan grubunda COVID-19’a yakalanma riskinde artış olmadığı görüldü. Ancak A Rh – ve A Rh +, 0 Rh - ve AB Rh - kan gruplarının COVID-19’a yakalanma riskinin yüksek olduğunu, 0 Rh + kan grubunda ise hastalığa yakalanma riskinin daha düşük olduğunu tespit ettik.

49. Retrospective analysis of possible drug interactions in prescriptions written by branch and emergency physicians

Abstract

Objectives: Drug-drug interactions may occur when more than one drug is taken by the same patient. These interactions can result in increasing, decreasing or preventing the effectiveness of drugs. In this study, prescriptions given by branch and emergency physicians were examined in terms of possible drug interactions.Material and Methods: Patients over 65 years of age who received a prescription from both the emergency department and the internal medicine or cardiology outpatient clinic were screened over a period of six months from July to December 2019. For the selected patients, information on the name and number of drugs prescribed, age, and gender were recorded. Then, the interactions between the drugs included in the prescriptions were investigated using Drug Interaction Checker-Medscape software.Results: The study included 93 patients (57% females), with a mean age of 73.5 years. A total of eleven serious interactions, with the highest number belonging to the interaction of aspirin-ramipril were observed in the prescriptions given by branch physicians. Comparing the prescriptions of the branch and emergency physicians, 33 serious interactions, mostly that of ibuprofen-aspirin, were detected. Among the prescriptions of branch physicians, the majority of interactions were observed between aspirin and B blockers. For the emergency physicians, the highest number of interactions were seen in NSAID-NSAIDs. In the comparison of the prescriptions of the branch and emergency physicians, the highest number of interactions was 28, observed between NSAID and beta-blockers.Conclusion: Emergency physicians should take a detailed history of elderly patients, especially concerning their regularly renewed prescriptions when prescribing NSAID-group drugs., Amaç: Aynı hasta tarafından birden fazla ilaç yenildiğinde ilaç-ilaç etkileşimleri oluşabilir. Bu etkileşimler ilaçların etkinliğini arttırma, azaltma veya engelleme şeklinde olabilir. Bu çalışmada, branş ve acil hekimlerinin yazdığı reçeteler incelenerek olası ilaç etkileşimleri incelendi.Gereç ve Yöntem: Temmuz- Aralık 2019 arasını kapsayan 6 aylık sürede hem acil servisten hem de dahiliye veya kardiyoloji poliklinikten reçete düzenlenen 65 yaş üstü hastalar belirlendi. Belirlenen hastalara, yazılan ilaçların isimleri, ilaç sayısı, hastalara ait yaş, cinsiyet gibi bilgiler kaydedildi. Sonrasında Drug Interaction Checker- Medscape programı kullanarak yazılan reçetelerdeki ilaç etkileşimleri araştırıldı.Bulgular: Çalışmaya yaş ortalaması 73.5 ve 57%’si kadın olan 93 hasta dahil edildi. Branş hekimlerinin yazdığı reçetelerde en yüksek sayıda aspirin-ramipril arasında olmak üzere toplam 11 ciddi etkileşim görüldü. İki hekimin reçeteleri arasında en fazla ibuprofen-aspirin olmak üzere 33 ciddi düzeyde etkileşim tespit edildi. Branş hekimlerinin reçetelerinde en fazla aspirin-B bloker arasında etkileşim görüldü. Acil hekimlerinin reçetelerinde en fazla NSAID-NSAID arasında görüldü. İki hekimin reçeteleri arasında ise 28 etkileşim ile ne yüksek NSAID-B bloker arasında görüldü.Sonuç: Acil hekimleri, ileri yaş hastalara özellikle NSAID grubu ilaçları yazarken hastanın kullandığı raporlu ilaçlarla ilgili ayrıntılı öykü almalıdır.

50. Effect of preoperative HbA1C levels on postoperative acute renal failure in diabetic patients undergoing coronary bypass surgery

Abstract

Introduction: Open heart surgery in patients with diabetes mellitus (DM) is associated with a higher mortality and morbidity than other patients. Diabetes mellitus (DM) is present in 30 to 40% of patients undergoing coronary bypass surgery (CABG). In this study, we aimed to clarify the relationship between preoperative glycohemoglobin (HbA1C) levels and postoperative acute renal failure (ARF) in patients with DM undergoing isolated coronary bypass surgery. Methods: We retrospectively enrolled a total of 295 patients who underwent elective, isolated CABG between January 2014 and February 2017 in our clinic and whose information was recorded. DM was detected in 118 of 295 patients. These patients were divided into two groups as Group 1 (HbA1c levels 7%, n = 46). All patients were treated with standard insulin therapy after consulting the internal medicine department before the operation. Results: Of the 118 patients included in the study, 82 were males and 36 were females. There were 72 patients (51 M, 21 F) in Group 1 and 46 patients (31 M, 15 F) in Group 2. The mean age was 62.4 +/- 3.2 years in Group 1 and 61.5 +/- 4.5 years in Group 2. The mean duration of DM diagnosis was 10.2 +/- 3.3 years in Group 1 and 11.7 +/- 2.6 years in Group 2. The mean duration of hospitalization in intensive care unit was 6.10 +/- 2.3 days in Group 1 and 9.1 +/- 2.5 days in Group 2, which was found to be statistically significant (p=0.008). Discussion and conclusion: Hemodialysis (HD) may be required after coronary bypass surgery in diabetic patients. Although there is no direct correlation between high HbA1c levels and postoperative HD, we believe that these patients should be more closely monitored with more frequent measurements of urea, creatinine, blood gas and electrolyte levels.
