Spontaneous coronary artery dissection (SCAD) is a cause of acute coronary syndrome and myocardial infarction, more frequent among young women. Invasive coronary angiography (ICA) is the gold standard for the diagnosis of SCAD, although the risk of propagating dissection flap is considerable. Therefore, coronary computed tomography angiography (CCTA) is an emerging alternative modality to diagnose SCAD with the advantage of being a non-invasive technique. Clinicians should be aware of the predisposing conditions and pathophysiology to raise the pre-test probability of SCAD and select the most appropriate diagnostic tools. In recent times, improvements in spatial and temporal resolution and the use of semi-automated software providing quantitative assessment make CCTA a valid alternative to ICA also for the follow-up. Moreover, CCTA may be helpful to screen and evaluate extra-coronary arteriopathies closely related to SCAD. In this review, we illustrate the current and the potential role of CCTA in the diagnosis of SCAD, highlighting advantages and disadvantages of this imaging modality compared to ICA.

Cardiac magnetic resonance (CMR) imaging has progressively become part of the imaging methods recommended in patients with heart failure. CMR represents the gold standard for assessing volumes, function, biventricular kinetics and providing tissue characterization through scans with and without contrast medium. In patients with heart failure with reduced ejection fraction (HFrEF) and ischemic dilated cardiomyopathy, CMR allows to search for viability, accurately estimate volumes and ejection fraction. It can assess scar extent for predicting response to cardiac resynchronization therapy and for establishing an indication for implanting a defibrillator in borderline cases. In patients with HFrEF and non-ischemic dilated cardiomyopathy, CMR helps to identify specific etiological subgroups and to estimate the arrhythmic risk beyond ejection fraction. In patients with heart failure with preserved ejection fraction, CMR offers the possibility of diagnosing specific phenotypes, including sarcomeric hypertrophic cardiomyopathy, amyloidosis or Fabry disease, and adds prognostic information. Both clinical and scientific interest in this imaging method is constantly expanding; the clinicians dealing with heart failure cannot fail to know the technique, the indications and all the potential that CMR can offer.

Background: Contemporary guidelines advocate for early invasive strategy with coronary angiography in patients with non-ST-elevation acute coronary syndromes (NSTE-ACS). Still, the impact of an invasive strategy in older patients remains controversial and may be challenging in spoke hospitals with no catheterization laboratory (cath-lab) facility. Purpose: The purpose of this study was to analyse the characteristics and outcomes of patients ≥80 years old with NSTE-ACS admitted to spoke hospitals. Methods: Observational–retrospective study of all consecutive NSTE-ACS patients admitted to two spoke hospitals of our cardiology network, where a service strategy (same-day transfer between a spoke hospital and a hub centre with a cath-lab facility in order to perform coronary angiography) was available. Patients were followed up for 1 year after the admission date. Results: From 2013 to 2017, 639 patients were admitted for NSTE-ACS; of these, 181 (28%) were ≥80 years old (median 84, IQR 82–89) and represented the study cohort. When the invasive strategy was chosen (in 105 patients, or 58%), 98 patients (93%) were initially managed with a service strategy, whereas the remainder of the patients were transferred from the spoke hospital to the hub centre where they completed their hospital stay. Of the patients managed with the service strategy, a shift of strategy after the invasive procedure was necessary for 10 (10%). These patients remained in the hub centre, while the rest of the patients were sent back to the spoke hospitals, with no adverse events observed during the back transfer. The median time to access the cath-lab was 50 h (IQR 25–87), with 73 patients (70%) reaching the invasive procedure

Whether balloon aortic valvuloplasty (BAV) may provide an effective palliation in symptomatic high-risk patients is uncertain. Therefore, we aimed to evaluate outcomes in symptomatic high-risk patients with severe aortic stenosis (AS), who underwent BAV. All-cause mortality and length of hospitalization for heart failure (HF) up to death or to 1-year follow up were collected after BAV. In our commentary we elucidated the fact that functional mitral regurgitation (FMR) is divided into two morphological subtypes: annular dilation with normal leaflet motion (Carpentier type I) and restricted systolic motion with left ventricular remodeling or dysfunction (Carpentier type IIIb). However, these phenotypes show some degree of overlap and there are currently no distinctive diagnostic cut-off values, therefore they may represent a continuum of the same disease. Moreover, correct left chambers quantification is critical for differentiating between atrial FMR and ventricular FMR. Three-dimensional echocardiography can quantify better leaflet areas, lengths, and closing angles, as well as the coaptation index, by eliminating geometric assumptions and integrating tethering of all leaflet surface and annular area enlargement. Also, It is expected that the use of artificial intelligence solutions and the deployment of automated onboard software that requires no or limited human input will stimulate wider clinical use of three-dimensional echocardiography in left chambers quantification.

In una fase storica nella quale il Servizio sociale ha dovuto dedicarsi per lo più ai bisogni emersi a seguito dell’emergenza sanitaria da Covid-19 diventa necessario confrontarsi su modalità operative che tengano conto di metodologie collaborative e generative. La pandemia, o meglio sindemia come insieme di problemi di salute, ambientali, sociali ed economici prodotti dall’interazione sinergica di due o più malattie trasmissibili e caratterizzata da pesanti ripercussioni, in particolare sulle fasce di popolazione svantaggiata, ha richiamato il Servizio sociale ad occuparsi della vulnerabilità sociale, economica, relazionale e sanitaria. Come sottolineano Cottatellucci e Ongari (2020, p. 7): «Un evento dunque che, ben lungi dal configurarsi nei soli termini di un gravissimo vulnus circoscritto all’ambito sanitario, si propone sempre più evidentemente nei termini di un vero e proprio trauma collettivo con caratteristiche di complessità, stanti le ricadute a livello sociale e psicologico». Oggi più che mai chi lavora con le persone deve possedere una conoscenza teorica, culturale e metodologica che tenga conto della complessità intesa secondo l’accezione emersa lungo la metà del Novecento che la intende come problema o ancora come sfida (Manghi, 2020). Molti sono i cambiamenti nelle discontinuità biografiche delle persone, nelle costellazioni familiari, nelle modalità di vivere le comunità.

Nella formazione dell’assistente sociale appare sempre più importante fornire la capacità di comprendere i bisogni della persona, dei gruppi, della comunità. Il nuovo Codice Deontologico sottolinea l’importanza del ruolo politico e sociale della professione con l’obiettivo di costruire contesti sociopolitici e istituzionali sempre più in linea con i bisogni delle persone e delle comunità da un lato e in grado di costruire nuovi legami sociali dall’altro. La sfida del servizio sociale è rappresentata dal lavoro con le persone, nel loro contesto di vita, nel rapporto con la comunità, attraverso un approccio sistemico, che sia in grado di restituire loro la consapevolezza di sentirsi parte di quella comunità, accompagnandole a riallacciare legami significativi. Come sottolinea Quaresima nel suo capitolo citando Seppilli la salute è «una condizione di armonico equilibrio, fisico e psichico, dell’individuo dinamicamente integrato nel suo ambiente naturale e sociale» (Seppilli, 1966). Non si intende quindi semplicemente un benessere circoscrivibile al piano fisco, come assenza di malattia o infermità, ma il termine salute assume un’accezione più vasta, includendo oltre a quella fisica anche l’area psicologica e sociale. L’approccio ecologico dello sviluppo umano (Bronfenbrenner, 2002), pone la persona al centro di una vasta dimensione relazionale gettando le basi della prospettiva sistemica, per accompagnare l’operatore nella comprensione di una complessità di intrecci e di reciproche influenze. Come sottolineano Cheli e Ricciutello: «Non il singolo intervento, quindi, ma un sistema che si fa curante attraverso la costruzione di connessioni fra diverse discipline, risorse e livelli d’intervento» (Cheli, Ricciutello, 2013).

The combination of aortic stenosis (AS) and mitral regurgitation (MR) is common in patients with degenerative valvular disease. It is characterized by having complex pathophysiology, leading to potential diagnostic pitfalls. Evidence is scarce in the literature to direct the diagnostic framework and treatment of patients with this particular combination of multiple valvular diseases. In this complex scenario, the appropriate use of advanced echocardiography and multimodality imaging methods plays a central role. Transcatheter mitral valve replacement or repair and transcatheter aortic valve replacement widen the surgical options for valve diseases. Therefore, there is an increasing need to reconsider the function, timing, and mode intervention for patients with a combination of AS with MR towards more personalized treatment.

Aortic stenosis (AS) may present frequently combined with other valvular diseases or mixed with aortic regurgitation, with peculiar physio-pathological and clinical implications. The hemodynamic interactions between AS in mixed or combined valve disease depend on the specific combination of valve lesions and may result in diagnostic pitfalls at echocardiography; other imaging modalities may be helpful. Indeed, diagnosis is challenging because several echocardiographic methods commonly used to assess stenosis or regurgitation have been validated only in patients with the single-valve disease. Moreover, in the developed world, patients with multiple valve diseases tend to be older and more fragile over time; also, when more than one valvular lesion needs to address the surgical risk rises together with the long-term risk of morbidity and mortality associated with multiple valve prostheses, and the likelihood and risk of reoperation. Therefore, when AS presents mixed or combined valve disease, the heart valve team must integrate various parameters into the diagnosis and management strategy, including suitability for single or multiple transcatheter valve procedures. This review aims to summarize the most critical pathophysiological mechanisms underlying AS when associated with mitral regurgitation, mitral stenosis, aortic regurgitation, and tricuspid regurgitation. We will focus on echocardiography, clinical implications, and the most important treatment strategies.

Echocardiography is the most validated, non-invasive and used approach to assess left ventricular hypertrophy (LVH). Alternative methods, specifically magnetic resonance imaging, provide high cost and practical challenges in large scale clinical application. To include a wide range of physiological and pathological conditions, LVH should be considered in conjunction with the LV remodeling assessment. The universally known 2-group classification of LVH only considers the estimation of LV mass and relative wall thickness (RWT) to be classifying variables. However, knowledge of the 2-group patterns provides particularly limited incremental prognostic information beyond LVH. Conversely, LV enlargement conveys independent prognostic utility beyond LV mass for incident heart failure. Therefore, a 4-group LVH subdivision based on LV mass, LV volume, and RWT has been recently suggested. This novel LVH classification is characterized by distinct differences in cardiac function, allowing clinicians to distinguish between different LV hemodynamic stress adaptations in various cardiovascular diseases. The new 4-group LVH classification has the advantage of optimizing the LVH diagnostic approach and the potential to improve the identification of maladaptive responses that warrant targeted therapy. In this review, we summarize the current knowledge on clinical value of this refinement of the LVH classification, emphasizing the role of echocardiography in applying contemporary proposed indexation methods and partition values.

Background The dynamic consequences of mitral annular disjunction (MAD) on the mitral apparatus and the left ventricle remain unclear and are crucial in the context of mitral surgery. Thus, the aim of this study was to assess mitral valvular, annular, and ventricular dynamics in mitral valve prolapse (MVP) stratified by presence of MAD. Methods In 61 patients (mean age, 62 ± 11 years; 25% women) with MVP and severe mitral regurgitation undergoing mitral surgery between 2009 and 2016, valvular and annular dimensions and dynamics by two-dimensional transthoracic and three-dimensional transesophageal echocardiography and left ventricular dimensions and dynamics were analyzed stratified by presence of MAD before and after surgery. Results MAD (mean, 8 ± 3 mm) was diagnosed in 27 patients (44%; with a mean effective regurgitant orifice area of 0.55 ± 0.20 cm2 and similar to patients without MAD), more frequently in bileaflet prolapse (52% vs 18% in patients without MAD, P = .004), consistently involving P2 (P = .005). Patients with MAD displayed larger diastolic annular areas (mean, 1,646 ± 410 vs 1,380 ± 348 mm2), circumferences (mean, 150 ± 19 vs 137 ± 16 mm), and intercommissural diameters (mean, 48 ± 7 vs 43 ± 6 mm) compared with those without MAD (P ≤ .008 for all). Dynamically, mid- and late systolic excess intercommissural diameter, annular area, and circumference enlargement were associated with MAD (P ≤ .01 for all). MAD was also associated with dynamically annular slippage, larger prolapse volume and height (P ≤ .007), and larger leaflet area (mean, 2,053 ± 620 vs 1,692 ± 488 mm2, P = .01). Although patients with MAD compared with those without MAD showed similar ejection fractions (mean, 65 ± 5% vs 62 ± 8%, respectively, P = .10), systolic basal posterior thickness was increased in patients with MAD (mean, 19 ± 2 vs 15 ± 2 mm, P Conclusions MAD in patients with MVP involves a predominant phenotype of bileaflet MVP and causes profound annular dynamic alterations with considerable expansion and excess annular enlargement in systole, potentially affecting leaflet coaptation. MAD myocardial and annular slippage simulates vigorous left ventricular function without true benefit after surgical annular suture. Thus, although MAD does not hinder the feasibility and quality of valve repair, it requires careful suture of ring to ventricular myocardium, lest it persist postoperatively.

Introduction We aimed to assess mitral valvular/annular/ventricular dynamics in mitral valve prolapse (MVP) stratified by mitral annular disjunction (MAD) presence, in the context of mitral surgery. Results MAD (8 ± 3 mm) was diagnosed in 27 (44%) patients [ERO of 0.50(0.36–65)cm2], more frequently in bileaflet MVP (52 vs.19%, P = 0.007), involving consistently P2 (P = 0.03). MAD displayed larger diastolic annular area (1646 ± 410 vs. 1380 ± 348 mm2, P = 0.008), circumference (150 ± 19 vs. 137 ± 16 mm, P = 0.005) and intercommissural diameter (48 ± 7 vs. 43 ± 6 mm, P = 0.03) vs. no-MAD. Dynamically, early-systolic annular contraction and saddle shape accentuation were unaffected but mid- and late-systolic excess inter-commissural diameter, annular area and circumference enlargement were associated with MAD (all P 0.07). However, despite similar ejection fraction (65 ± 5 vs. 62 ± 8%, P = 0.1), systolic basal posterior thickness was increased in MAD (19 ± 2 vs. 15 ± 2 mm, P 0.50) ( Fig. 1 ). Conclusion MAD is frequent in MVP, mostly associated with bileaflet/deep MVP and compounds profound annular alterations. MAD myocardial/annular slippage masquerades stronger LV function without benefit post-surgery. Hence, MAD valvular/ventricular alterations warrant careful detection and interpretation.

Gli ultimi decenni hanno registrato l’arrivo – nelle nostre comunità – dei migranti, fenomeno connesso al collasso geopolitico generato soprattutto dalle profonde ingiustizie sociali ed economiche. Tale presenza ha prodotto nella società due reazioni contrapposte: una di rifiuto – usato ed enfatizzato da alcuni partiti politici – e una d’accoglienza. Una società si misura sulla base dello sviluppo economico-sociale conseguito e delle innovazioni prodotte, ma soprattutto del suo livello d’umanità, della sua capacità di convertire l’hostis in un hospes, il nemico in un ospite. Il lavoro sociale, che si pone l’obiettivo d’attivare la società per risolvere specifici problemi della comunità, ha il compito di favorire questo passaggio, nella consapevolezza che l’integrazione delle diversità favorisce la coesione sociale e ne promuove le dimensioni valoriali. Il fenomeno migratorio è certamente articolato, ha richiesto e richiede un arricchimento sia dell’apparato teorico sia della metodologia del servizio sociale che consenta d’attrezzare adeguatamente la formazione universitaria dei futuri assistenti sociali (ma non solo). Nel presente volume si è ampliato lo sguardo per capire come un servizio, composto da operatori dell’area sociale, educativa e sanitaria, possa attrezzarsi per rispondere a un’utenza straniera diversificata. La famiglia immigrata è infatti rappresentata da una pluralità di modelli connessi alla diversa provenienza geografica, linguistica, culturale, religiosa e dal particolare vissuto dei migranti. Un percorso formativo adeguato aiuta i servizi a non leggere il fenomeno dell’immigrazione come un tutt’uno omogeneo, ma può e deve aiutare gli operatori che ne fanno parte a promuovere processi d’inclusione e d’integrazione che tengano conto delle specificità di tale fenomeno.

Il contributo riporta i risultati di una ricerca realizzata dalle Università di Bologna e di Parma con gli Uffici di esecuzione penale esterna (UEPE) di Bologna, Modena e Reggio-Emilia. La ricerca ha inteso indagare le finalità della c.d. probation, nonché la conoscenza e l’opinione degli operatori giuridici e sociali sulla ‘messa alla prova’, dispositivo penale esteso di recente ai maggiorenni (L. 67/2014), dopo essere stato appannaggio, per trent’anni, del settore minorile. Lo studio ha preso avvio dalla constatazione del mutamento normativo, per cui il reato non viene più considerato (solo) come lesione dell’ordine giuridico astratto, ma piuttosto dei beni e degli interessi concreti di una persona e della comunità in cui vive. Si introduce così la formulazione di dispositivi che appartengono al modello della cosiddetta Giustizia riparativa (Restorative Justice), tramite cui l’esecuzione penale può tendere (anche) al coinvolgimento diretto dell’autore del reato, per considerare (finalmente) i vissuti e le aspettative di un soggetto finora rimasto “dietro le quinte” della scena processuale: la vittima. Il sistema della probation, com’è noto, prevede la compresenza di tre elementi: la sospensione dell’azione penale, l’imposizione di oneri e prescrizioni all’imputato e soprattutto l’affiancamento di questo da parte dei Servizi sociali, durante la “prova” (l’etimo probation fa riferimento appunto alla prova, da affrontare e valutare). La probation - e la messa alla prova, nello specifico - si presta quindi ad essere indagata per osservare le dinamiche che interessano l’intreccio tra la metodologia del servizio sociale e le procedure giudiziarie. La ricerca ha permesso l’analisi di 192 fascicoli di imputati adulti ‘messi alla prova’, realizzata tramite il Software SPSS; la somministrazione di un questionario standardizzato rivolto ai funzionari del Servizio Sociale degli UEPE dell’Emilia-Romagna diretto a rilevare prassi e finalità dell’ operato dei servizi; e la conduzione di 13 interviste semi-strutturate con testimoni privilegiati (operatori giuridici e operatori sociali) focalizzate sulla riparazione, sulla mediazione e sull’utilizzo del lavoro di pubblica utilità.

Accoglienza e integrazione: da un lato sono fasi inevitabili e per certi aspetti inseparabili nel percorso di migrazione, dall’altro chiamano in causa problematiche e bisogni diversi dei migranti stessi, nonché varie competenze e organizzazioni che vengono richieste ai servizi. Il volume affronta il dibattito su queste tematiche, approfondendo non solo la fatica dei migranti che arrivano e provano a integrarsi sul nostro territorio, ma anche quella di dirigenti e operatori dei servizi ad affrontare situazioni tanto complesse. Nella prima parte si propone una riflessione sociologica sull’integrazione tra individuo e società, declinando il concetto sul tema delle migrazioni per poi passare a una ricerca condotta su alcuni rifugiati nel territorio di Parma. Nella seconda parte si approfondisce il caso di Bologna, trattando sia i suoi servizi di accoglienza (legati al sistema SPRAR, ora SIPROIMI) sia il sistema di integrazione, alla luce del recente riordino in un’ottica di servizio sociale di comunità (primo comune italiano a farlo) e di valorizzazione della dimensione di quartiere. Attraverso interviste in profondità, effettuate sia a migranti che a operatori, la ricerca evidenzia le tante fragilità di chi vive questa condizione, le difficoltà dei servizi nell’affrontarle ma anche numerose buone pratiche e il lavoro di rete che sembra accomunarle.

The service strategy (same-day transfer between the spoke hospital and the hub center with catheterization laboratory facility to perform invasive procedures) has been shown to improve the management of patients with non-ST-elevation acute coronary syndrome admitted to spoke hospitals. However, few data exist about the safety of this strategy and, in particular, the safety of retransferring patients to spoke centers immediately after successful percutaneous coronary intervention.We used data from a prospective registry to retrospectively describe the application, performance and outcome of the service strategy in the daily clinical practice in our province, organized in 5 spoke hospitals and a hub center in Reggio Emilia, Italy.From January 2013 to December 2017, 1183 consecutive patients were admitted to the cath-lab in the hub center from spoke hospitals with a diagnosis of non-ST-elevation acute coronary syndrome. Mean age was 68 ± 12 years, with a mean GRACE risk score of 137 ± 3. Overall, 1063 patients (90%) were managed with a service strategy. The mean time between hospital admission and access to the cath-lab for coronary artery angiography ± percutaneous coronary intervention was 46.6 h (27.5-71.2). No major adverse events (all-cause mortality, arrhythmias, or acute myocardial infarction) were observed during the back transfer from the invasive center to the referring non-invasive center. At 30 days, acute myocardial infarction was observed in 1.3% of patients and all-cause mortality was 0.5%. At 1-year follow-up, acute myocardial infarction was reported in 10% of patients and all-cause mortality was 2.8%.A wide adoption of the service strategy in our provincial network in patients with non-ST-elevation acute coronary syndrome admitted to spoke centers allowed an early access to the cath-lab as recommended by international guidelines. The safety of the service strategy is confirmed in our experience, with no major adverse events occurring during the back transfer.

Despite advancement in therapy and management, left ventricular thrombus (LVT) after anterior myocardial infarction (MI) is sporadically encountered and remains associated with a very high risk of major cardiovascular events and mortality. Cardiac magnetic resonance (CMR) is considered the gold standard technique for LVT detection, but it is a time-consuming and expensive test not available in all centers, especially when repeated examinations are necessary. Transthoracic echocardiography represents a useful tool to screen for LVT and to identify predictors of high risk of developing LVT. The advances in ultrasound technology and the use of contrast agents may potentially help clinicians to identify LVT and the use of sequential echocardiography for each patient with acute MI complicated by LVT may provide an opportunity to quantify regression and its correlation with outcomes to tailor the management of these patients. Hence, this narrative review focuses on the added value of echocardiographic-guided LVT management in patients with recent anterior MI to reduce mortality and morbidity excess related to LVT based on current evidence.

The need for multiple transducer positions, especially from right parasternal windows, is consistently mentioned in the recommendations for the accurate measurement of peak velocities across a stenotic aortic valve, but yet poorly adopted.We performed a subanalysis of the largest prospective series on the right parasternal acoustic windows in patients with aortic stenosis (330 consecutive) to calculate the degree of misalignment and estimate the potential outcome implication of this often-forgotten approach.The right parasternal view was highly feasible with an average estimated misalignment from the apical view of 14 ± 16 degree; in 10 cases, an estimated misalignment >40 degree. Right parasternal assessment (vs. apical alone) provided a significant reclassification from moderate to severe or even very-severe aortic valve stenosis. Considering a wellestablished survival benefit provided by either percutaneous or surgical valve replacement in patients with severe aortic stenosis the reclassification would result in approximately 1 life-year saved for every 30-35 patients in whom parasternal view were effectively utilized.

Il capitolo analizza il fenomeno del policonsumo, cioè del consumo di più sostanze psicoattive, legali ed illegali. Tale analisi è stata svolta nell'ambito delle indagini, condotte dal Dipartimento di Sociologia e Diritto dell’Economia presso l’Alma Mater Studiorum – Università di Bologna, sul consumo di sostanze stupefacenti nella popolazione generale adulta (GPS).

Il libro Si scrive lavoro, si legge uomo. Umanesimo e impresa cerca di dare conto di un percorso che trova il suo compimento nell’ambito lavorativo, ma che in realtà parte da più lontano. È un volume, in altre parole, che arriva al rapporto tra umanesimo ed impresa dopo una lunga riflessione sulle condizioni socio-culturali dei nostri giorni, sulla quotidianità e sulle istituzioni dei nostri territori, sui segni di disagio che caratterizzano una parte importante della nostra popolazione. La formazione prevista del progetto non si limita ad affrontare il tema della sicurezza sul lavoro, per quanto fondamentale; ambisce infatti alla salute dei lavoratori, nel senso suggerito dalla Dichiarazione di Alma-Ata nel 1978, cioè come «stato di completo benessere fisico, mentale e sociale, e non soltanto come assenza di malattia o di infermità». Qualità del lavoro e qualità della vita sembrano quindi incontrarsi in questo progetto. Per fare questo, però, è necessario individuare e comprendere i segni del disagio tra i lavoratori; un lavoro decisamente difficile tanto più che questo è spesso presente in modo asintomatico. Formare in tal senso, evidentemente, significa “irrobustire” contro i tanti rischi che l’attività lavorativa può comportare: parliamo infatti di soggetti potenzialmente “vulnerabili”. Questo percorso è stato possibile grazie alla collaborazione tra tre attori: il gruppo industriale Marcegaglia (cha ha promosso il progetto), Arca Formazione (che l’ha coordinato) ed il Ce.P.Ci.T. (che ha offerto una consulenza scientifica). Il gruppo Marcegaglia, come è noto, è una delle realtà industriali più importanti nella siderurgia italiana, ed ha nello stabilimento di Contino di Volta Mantovana uno dei suoi principali punti di produzione. Il volume si apre con il contributo di Giovanni Pieretti, Uscire dalla cultura del surplus. Partendo da alcuni dati statistici, l’autore sottolinea il “mutamento antropologico” (riprendendo l’efficace espressione di P.P. Pasolini) che ha caratterizzato la società italiana negli ultimi decenni, abbandonando progressivamente una cultura dell’essenzialità di matrice popolare e contadina per “abbracciarne” una del surplus, del superfluo. Con queste premesse, lo scivolare nel senso di insuccesso, nel disagio e nelle dipendenze diventa purtroppo facile, come dimostrano gli stessi dati statistici. Già in questo capitolo, però, si suggerisce un possibile percorso di uscita da questo circolo vizioso, un percorso che passa dalla relazione con gli altri senza gerarchie, senza strumentalizzazioni, seguendo i propri tempi interni; si delineano quindi gli elementi di una terapia ambientale che caratterizza tante comunità terapeutiche italiane e che affonda il segreto del proprio successo in alcune peculiarità della nostra cultura: la solidarietà, l’assenza di competizione, l’attenzione agli aspetti essenziali della vita. Tutti questi elementi possono (e devono) diventare sempre più parte integrante dei servizi socio-sanitari del territorio. Nel secondo capitolo, Un viaggio nelle dipendenze, Sergio Bovi presenta le principali dipendenze che affliggono la nostra società, delineando un quadro talmente complesso da rendere anche poco utile la distinzione tra “droghe leggere” e “droghe pesanti”, tanto più che le dipendenze non da sostanze si sono ormai diffuse in una quota rilevante della popolazione italiana: da quella da gioco d’azzardo a quella da Internet passando per lo shopping compulsivo. Anche in questo capitolo troviamo molti dati recenti ma anche la segnalazione dell’esigenza di prevenzione, esigenza che non vede però un singolo attore protagonista ma tutta una comunità e le sue istituzioni chiave: la famiglia, la scuola, i servizi, le imprese. Dal cap. 3 al cap. 7, il volume riprende i suddetti aspetti dedicando un approfondimento al territorio mantovano, quello in cui è stato implementato il progetto Si scrive lavoro, si legge uomo. Questi capitoli, scritti tutti da Natale Bottura, sono altrettante tappe di un percorso che parte dalla cultura e dalle usanze di quel territorio, evidenzia gli elementi di crisi della contemporaneità e prova a suggerire alcune “vie d’uscita” dal disagio che passano dal lavoro, dagli affetti e dalla riscoperta delle proprie radici. Nel capitolo "Cancelli", l’autore parte dai suoi ricordi e dalla sua esperienza di insegnante e dirigente scolastico per sottolineare la difficoltà di relazionarsi davvero con i giovani e la necessità di andare loro incontro, sia nella formazione a scuola sia in quella in azienda. Nel capitolo "Il tempo di adesso", viene presa in considerazione la “società della conoscenza” in cui viviamo attualmente, una società caratterizzata da un cambiamento sempre più rapido ma anche da un costante calo di affetti veri; ciò che cresce, invece, è l’incertezza in tutti gli aspetti della nostra quotidianità. Il bisogno di formazione diventa ancora più importante in un presente come questo, una formazione che può e deve accompagnare l’individuo in tutti i momenti della sua vita. Nel capitolo "La famiglia nella modernità" si considerano i recenti cambiamenti di questa istituzione nel nostro Paese, e le tante criticità emerse: difficoltà di relazioni, precarietà imperante, fatica a conciliare tempi di cura e tempi di lavoro, ma forse anche mancanza di coraggio, dimenticando il tanto che abbiamo oggi rispetto al poco o niente che avevano i nostri genitori e i nostri nonni. Nonostante ciò, la famiglia resta un’esigenza molto sentita dai giovani, una delle poche risorse ancora spendibili per apprendere “competenze di vita”, e forse una dimostrazione che l’indipendenza e la ricerca del nuovo come vie alla felicità sono due miti da sfatare. Nel capitolo "Strade di futuro", l’autore riscopre l’importanza dei sentimenti “classici” e lo fa “con l’aiuto dei classici”, dai testi sacri a quelli degli autori greci e latini. Parte quindi dall’amicizia passando dalla fratellanza e, più in generale richiama ad un’attenzione al prossimo che non può mai mancare, così come non possono mai mancare la scuola e la famiglia nell’insegnarla. L’autore parla anche di bisogno di “sobrietà”, uscendo da quella cultura del surplus già considerata da Pieretti nel primo capitolo, e conclude con “tre parole per vivere”: fiducia, speranza e futuro. In "Il possibile nostro", infine, si invita a “fermare la corsa e pensare”, a riscoprire quel “senso del noi” attraverso le piccole cose quotidiane, suggerendo che sono proprio quelle a dare o ridare senso e spessore alla vita. Nel fare questo, l’autore ritorna alle origini della propria terra e alle sue radici rurali, tant’è che parla di “parole del grano”: sembra essere lì una possibile risposta a tante domande annegate nelle dipendenze, e soprattutto la riscoperta che “l’altro esiste ed è uno di noi”, sia nel lavoro sia fuori. L’ultimo capitolo, "Uomini o caporali?" di Paolo Parma, è dedicato più specificamente al progetto Si scrive lavoro, si legge uomo. L’autore parte da alcuni dati e alcuni riferimenti legislativi su sicurezza e infortuni sul lavoro; al tempo stesso, sottolinea come, dietro questi infortuni, possano non di rado esserci segni di disagio del lavoratore. Un’impresa che creda nell’umanesimo non può non dedicare attenzione a questi segni, nell’interesse del lavoratore e naturalmente anche nel proprio. È proprio da questa consapevolezza che ha preso vita il progetto.

Il capitolo tocca il consumo di sostanze psicotrope nell'ambito delle indagini, condotte dal Dipartimento di Sociologia e Diritto dell’Economia presso l’Alma Mater Studiorum – Università di Bologna, nella popolazione scolastica secondaria di secondo grado (SPS). Al fine di indagare quanto tra gli studenti viene percepita la pericolosità di assunzione delle varie sostanze stupefacenti, è stato chiesto loro di esprimere un’opinione su alcuni comportamenti giudicati a rischio per la salute, tra i quali consumare hashish o marijuana, assumere cocaina e/o crack, far uso di ecstasy, allucinogeni ed eroina, bere alcolici e fumare.

Il capitolo tocca il consumo di sostanze psicotrope nell'ambito delle indagini, condotte dal Dipartimento di Sociologia e Diritto dell’Economia presso l’Alma Mater Studiorum – Università di Bologna, sul consumo di sostanze stupefacenti nella popolazione generale adulta (GPS) e nella popolazione scolastica secondaria di secondo grado (SPS), sono l’oggetto del volume, insieme all’indagine sul gioco d’azzardo (GAP).

La circolarità tra teoria sociologica e fondamenti del servizio sociale da sempre ha portato risultati fecondi prima alla metodologia del servizio sociale e poi ai modelli operativi di intervento. La complessificazione crescente della società e i cambiamenti profondi all’interno del welfare hanno richiesto e richiedono un adeguamento sia dell’apparato teorico sia della metodologia del servizio sociale che consenta di attrezzare la formazione universitaria dei futuri assistenti sociali in modo adeguato. Il volume è il frutto della preziosa collaborazione tra il Corso di laurea in Servizio Sociale dell’Università di Bologna, l’Ordine degli assistenti sociali dell’Emilia Romagna e la comunità professionale degli assistenti sociali. La capacità e la volontà, da parte degli attori coinvolti, di saper connettere esperienze, saperi e competenze diverse, offrono allo studente un’esperienza didattica innovativa. L’analisi degli atteggiamenti professionali, degli aspetti e dilemmi etici, deontologici, attraverso la presentazione di alcuni casi e di simulate, che investono differenti ambiti dell’intervento sociale, offrono allo studente l’opportunità di interrogarsi e di sperimentarsi nell’esercizio della professione dell’assistente sociale. Il libro presenta inoltre lo strumento del colloquio professionale a partire da simulate e dall’illustrazione di situazioni tipo. Fondamentale è riprendere il tema del lavoro di comunità e dell’accompagnamento sociale come possibili modalità operative che possono sostenere il tirocinante prima, e il professionista successivamente, in una fase storica in cui i servizi sono chiamati a gestire situazioni multiproblematiche e ognuno deve necessariamente trovare il senso del proprio agire professionale.

Introduction: Despite current guidelines provide recommendations for the optimal management of degenerative mitral regurgitation (MR), this condition remains often undertreated with delay in surgical referral and dismal effect on outcomes. Areas covered: This review focuses on the role of echocardiography in guiding mitral valve (MV) surgical repair in degenerative MR due to leaflet prolapse. Expert opinion: A stepwise protocol-driven echocardiography shared by referring physician and surgeon may help to guide referral to surgical repair in degenerative MR. This protocol particularly is useful to identify the ideal patho-anatomy for a successful and durable repair especially when early surgery is proposed and to refer the patient to centers of excellence in case of complex anatomy. Nearly 100% repair rate can be achieved when the surgical technique is adapted to the lesions seen in each valve. Three-dimensional echocardiography predicts repair complexity may be useful and should therefore be implemented. However, the current literature is far from comprehensive deriving from small, single-center studies. Therefore, reproducibility and external validation, especially with newly developed quantitative automated software, are still needed.

Valvular heart disease is common, and any of the four valves of the heart can be affected, although left-sided valvular disease is far commoner than right-sided disease. Treatment depends on the valvular disease type and severity but, when severe and symptomatic, usually involves mechanical intervention. Asymptomatic patients without objective parameters suggesting high morbid or lethal risk are closely observed clinically, until surgical indications develop. Although often prescribed, no rigorous evidence supports pharmacological therapy in most chronic valve diseases, although drugs may be useful in acute valvular diseases or as a bridge to surgery in severely decompensated patients. Moreover, given the rising proportion of frail older people with symptomatic degenerative valve disease, in whom the risks of mechanical intervention are prohibitive, medical treatment could be the only option.

A recently developed algorithm for 3D analysis based on machine learning (ML) principles detects left ventricular (LV) mass without any human interaction. We retrospectively studied the correlation between 2D-derived linear dimensions using the ASE/EACVI-recommended formula and 3D automated, ML-based methods (Philips HeartModel) regarding LV mass quantification in unselected patients undergoing echocardiography. We included 130 patients (mean age 60 ± 18 years, 45% women). There was only discrete agreement between 2D and 3D measurements of LV mass (r = 0.662, r2 = 0.348, p &lt, 0.001). The automated algorithm yielded an overestimation of LV mass compared to the linear method (Bland–Altman positive bias of 13.1 g with 95% limits of the agreement at 4.5 to 21.6 g, p = 0.003, ICC 0.78 (95%CI 0.68−8.4). There was a significant proportional bias (Beta −0.22, t = −2.9) p = 0.005, the variance of the difference varied across the range of LV mass. When the published cut-offs for LV mass abnormality were used, the observed proportion of overall agreement was 77% (kappa = 0.32, p &lt, 0.001). In consecutive patients undergoing echocardiography for any indications, LV mass assessment by 3D analysis using a novel ML-based algorithm showed systematic differences and wide limits of agreements compared with quantification by ASE/EACVI- recommended formula when the current cut-offs and partition values were applied.

Objectives The purpose of this study was to compare women and men with organic mitral regurgitation (MR) using pre- and post-operative comprehensive imaging and to analyze the effect on outcome. Background Management of organic MR has no sex-specific guideline recommendation, and sex differences on the basis of comprehensive imaging and links to outcome remain largely unknown. Methods Comprehensive imaging (MR cause, quantitation, ventricular and atrial measures, and post-operative reverse cardiac remodeling) was analyzed in 217 women and 447 men who underwent operations for organic MR from 1990 to 2000 with long-term follow-up analysis. Results Pre-operatively, women and men had similar age and ejection fraction. In women, a smaller left ventricle (LV) more often labeled as normal size (23% vs. 13%), left atrium size, and regurgitant volume (all p 0.06). During follow-up (10.4 ± 3.7 years) women had similar survival as men (p = 0.5) but experienced more heart failure (at 15 years: 36 ± 7% vs. 19 ± 3%; p = 0.03; adjusted hazard ratio 1.63 [95% confidence interval: 1.08 to 2.43]; p = 0.02) linked to more frequent pre-operative heart failure symptoms (p Conclusions Women who undergo mitral surgery for organic MR receive similar repair for similar degenerative lesions defined by echocardiography and enjoy similar survival and reverse cardiac remodeling, but they incur excess post-operative heart failure linked to worse pre-operative presentation. Imaging that does not account for body size shows smaller absolute cardiac dimensions and regurgitant volumes, which tends to underestimate MR severity in women.

L’articolo ha l’obbiettivo di illustrare le nuove pratiche dell’abitare e lavorare insieme, cohousing e coworking, modelli che con sempre maggiore evidenza stanno caratterizzando le modalita di sviluppo sociale ed economica dei centri urbani nazionali. Partendo da una rassegna delle due modalita citate, saranno delineate le caratteristiche generali per un successivo confronto rispetto ai tratti di similitudine e differenza, al fine di comprendere quali siano i possibili tratti di uniformita in prospettiva di strategie ed azioni indirizzate a contribuire allo sviluppo e riqualificazione socio-economico delle aree urbane.

In women with low to intermediate risk of coronary artery disease (CAD), prognostic detection strategies have been controversial. We present the follow-up data of the SMART trial in peri/postmenopausal women at low to intermediate risk of CAD.To determine the value of contrast stress echocardiography (CSE), stress electrocardiogram (sECG), and serum biomarkers for prediction of cardiovascular events (CE) in peri/postmenopausal women at low to intermediate risk of CAD.From January 2004 to August 2007, 400 peri/postmenopausal women were prospectively enrolled. All women had detailed risk factor assessment, and underwent simultaneous CSE (DefinityA total of 366 women (54.4 ± 5.5 years, Framingham risk 6.5% ± 4.4%) completed simultaneous CSE and sECG. Forty-two (11.5%) had abnormal CSE, while sECG was abnormal in 22 (6%) women. Follow-up (4.4 ± 1.2 years) was available in 315/366 (86%) women (78% exercise-CSE, 22% dobutamine-CSE). In those who completed follow-up, CSE was abnormal in 33 women (10.5%) and sECG was abnormal in 21 (6.7%). In 33 women with abnormal CSE, sECG was abnormal in 7 (21.2%) and normal in 26 (79%), p = 0.0004. CE occurred in 27 (8.6%) women: 8 all-cause mortality, 2 nonfatal MI, 13 CP, and 4 REVASC. CE occurred in 21% versus 7% of women with abnormal versus normal CSE, p = 0.014 and 38% versus 6% of women with abnormal versus normal sECG, p 0.0001. Rest BNP was higher in women with CE versus those without (p = 0.018). Abnormal sECG and abnormal CSE were associated with CE, while only abnormal sECG was an independent predictor of CE (adjusted HR 10.3 [1.9-61.4], p = 0.007). Of the laboratory results, only BNP was associated with CE (adjusted HR 2.9 [1.1-7.3], p = 0.028).sECG and rest BNP were independent predictors of subsequent CE within 5 years in peri/postmenopausal women at low to intermediate risk of CAD.

Background: Data on stress echocardiography (SE) time-related changes in referral patterns and diagnostic yield for detection of inducible ischemia could enhance Echo Lab quality benchmarks and performance measures. Aim: This study aims to evaluate temporal trends in SE test results among ambulatory patients with suspected or known coronary artery disease (CAD) in a tertiary care referral center with moderate (>100/year) volume SE activities and Cath-Lab facility. Methods: From January 2004 to December 2015, 1954 patients (mean age 62 ± 12 years, 42% women, 27% with known CAD) underwent SE (1673 exercise SE, 86%, 246 pharmacological SE, 12%, 35 pacing SE, 2%). Time was grouped into three 4 year periods, where clinical data and test results were evaluated. Results: Our series comprised low-to-intermediate pretest probability of CAD throughout the observation period (overall pretest probability of CAD 19% ± 15%). A progressive decline over time in the rate of pharmacological SE instead of a dramatic increment of exercise SE (79%–96%, P < 0.0001) was noted. The use of beta-blockers increased (from 43% to 66%, P < 0.0001), while the use of nitrates decreased (from 11% to 4%, P < 0.0001) over time. We noted a very uncommon occurrence of abnormal test results with a further decrease in the last period (from 11% to 3%, P < 0.0001). Conclusions: We observed, over a 12-year period, a progressive decrease in the frequency of inducible myocardial ischemia among patients with known or suspected CADe referred to our Echo Lab for SE with Cath-Lab facility, and this trend was parallel to changes in SE referral practice. These findings are particularly relevant if we consider the practical implications on diagnostic SE accuracy and risk assessment.

Despite its high prevalence, little is known about mechanisms of mitral regurgitation in degenerative mitral valve disease apart from the leaflet prolapse itself. Mitral valve is a complex structure, including mitral annulus, mitral leaflets, papillary muscles, chords, and left ventricular walls. All these structures are involved in physiological and pathological functioning of this valvuloventricular complex but up to now were difficult to analyze because of inherent limitations of 2-dimensional imaging. The advent of 3-dimensional echocardiography, computed tomography, and cardiac magnetic resonance imaging overcoming these limitations provides new insights into mechanistic analysis of degenerative mitral regurgitation. This review will detail the contribution of quantitative and qualitative dynamic analysis of mitral annulus and mitral leaflets by new imaging methods in the understanding of degenerative mitral regurgitation pathophysiology.

The timing of surgical correction of mitral regurgitation remains controversial. A major source of dispute regards the potential short- and long-term postoperative outcome penalty associated with the type of guideline-based indication for surgery.Between 1990 and 2000, 1512 patients (aged 64 ± 14 years, mitral prolapse in 89%, valve repair in 88%) underwent surgical correction of pure organic mitral regurgitation. Patients were stratified according to surgical indication into class I triggers (ClassI-T: heart failure symptoms, ejection fraction60%, end-systolic diameter ≥40 mm, n = 794), class II triggers based on clinical complications (ClassII-CompT: atrial fibrillation or pulmonary hypertension, n = 195), or early class II triggers based on a combination of severe mitral regurgitation and high probability of valve repair (ClassII-EarlyT: n = 523).Operative mortality was highest with ClassI-T (1.1% vs 0% and 0%, P = .016). Long-term survival was lower with ClassI-T (15-year 42% ± 2%; adjusted hazard ratio [HR], 1.89; 95% confidence interval [CI], 1.53-2.34; P.0001) and ClassII-CompT (15-year 53% ± 4%, adjusted HR, 1.39; 95% CI, 1.04-1.84; P = .027) versus ClassII-EarlyT (15-year 70% ± 3%, P.0001). Postoperative excess mortality with ClassI-T and ClassII-CompT was confirmed by age stratification, inverse probability weighting, and expected survival adjustment. Excess postoperative heart failure was high with ClassI-T (adjusted HR, 2.49; 95% CI, 1.82-3.47; P.0001) and ClassII-CompT (adjusted HR, 1.98; 95% CI, 1.30-3.00; P = .002).The type of guideline-based indication for surgical correction of organic mitral regurgitation is associated with profound outcome consequences on long-term postoperative mortality and heart failure, despite low operative risk and high repair rates. Conversely, surgical correction of severe mitral regurgitation based on high probability of repair (ClassII-EarlyT) is associated with improved survival and low heart failure risk and should be the preferred strategy in valve centers offering low operative risk and high repair rates.

Background: We evaluated the impact of contrast stress echocardiography (CSE) testing results on cardiovascular (CV) risk behaviors in postmenopausal women presenting with chest pain symptoms. This was a substudy of the Stress Echocardiography in Menopausal Women at Risk for Coronary Artery Disease (SMART) trial. Methods: From 2004 to 2007, 366 women (mean age 54.4±5.5 years, range 40–65; body mass index (BMI) 31.4±6.68; Caucasian in 95%) completed CSE and were invited to participate in the Women's Heart Clinic Risk Assessment Questionnaire (WHCRAQ survey) at the time of CSE and after 2 years. Of the 366, 203 (55%) postmenopausal women completed both the baseline and 2-year follow-up surveys (age 61±5 years; Framingham risk score 6±4%, 81% treadmill CSE, 19% dobutamine CSE). WHCRAQ assessed medical history, hormone therapy (HT), and CV risk behaviors (cigarette smoking history, including current smoking and mean cigarettes smoked per day; physical activity or exercise, including mean exercise minutes per week; and dietary fat intake, alcohol intake, and mean alcohol drinks per week). Abnormal CSE was defined as a new or worsening stress wall motion abnormality. Post-CSE changes in CV risk behaviors were determined by comparing baseline versus 2-year data. Results: Of the 203 women who completed the survey at baseline and 2-year CSE, 29 were excluded to avoid confounding effect (coronary angiography [CA] was performed during follow-up). Of 174 women (55% hypertensive, 10% diabetic, 76% hyperlipidemic, and 25% on HT), CSE was abnormal in 10%. Baseline characteristics were not significantly different in normal versus abnormal CSE, apart from diabetes (8% vs. 24%, p=0.04). Slightly more women with normal CSE were taking HT than not (27% vs. 12%, p=0.19). CV risk behaviors that were different in normal versus abnormal CSE included current smoking, alcohol drinks per week, and higher fat diet (8% vs. 24%, p=0.03; 2.5±3.5 vs. 1.94±5.2, p=0.031; and 92% vs. 76%, p=0.03, respectively). Cigarette smoking decreased (mean difference of −1.5 cigarettes per day, p=0.014) in the abnormal-CSE group, whereas number of alcohol drinks per week increased (mean difference +0.38, p=0.009) in the normal-CSE group. Conclusion: We observed an association of lifestyle changes with CSE test results in postmenopausal women

Il contributo prende avvio dalla presentazione del tirocinio all'interno del Corso di laurea in Servizio Social dell'Università di Bologna. Nella prima parte vengono introdotti il tirocinio e l'immagine che ne hanno i docenti delle materie professionali del corso. Vengono toccati: l'interdipendenza tra dimensione teorica e pratica, la complessità e la multidimensionalità del tirocini, il ruolo del supervisore. nella seconda parte viene presentata l'esperienza bolognese: gestione dei tirocini e dei rapporti con gli enti convenzionati, svolgimento del corso di laboratorio di guida al tirocinio, criteri e modalità di valutazione dell'esperienza svolta.

We aim to describe changes in exercise habits and barriers to physical activity over 5 years in menopausal women at low-intermediate risk for cardiovascular disease.Women in the Stress Echocardiography in Menopausal Women at Risk for Coronary Artery Disease trial were prospectively enrolled in a multisite study from 2004 to 2007. Inclusion criteria were as follows: peri- and postmenopausal women with symptoms and/or risk factors for cardiovascular disease resulting in referral for stress echocardiography. A questionnaire, which assessed details of medical history, physical activity, and body mass index (BMI), was administered at baseline and 5 years.216 menopausal women (62.5% hypertensive, 15.3% diabetic, 52.3% prior or current smokers) were studied. At baseline, age was 54.9 ± 4.8 years, BMI was 30.7 ± 6.4 kg/mThese data suggest that women can increase their level of physical activity regardless of BMI and that overweight or obese status is not a barrier to initiating an aerobic, nonaerobic, or strength training exercise routine.
