Abstract

Abstract Background Intermittent hemodialysis (IHD) and continuous renal replacement therapy (CRRT) are the two main RRT modalities in patients with severe acute kidney injury (AKI). Meta-analyses conducted more than 10 years ago did not show survival difference between these two modalities. As the quality of RRT delivery has improved since then, we aimed to reassess whether the choice of IHD or CRRT as first modality affects survival of patients with severe AKI. Methods This is a secondary analysis of two multicenter randomized controlled trials (AKIKI and IDEAL-ICU) that compared an early RRT initiation strategy with a delayed one. We included patients allocated to the early strategy in order to emulate a trial where patients would have been randomized to receive either IHD or CRRT within twelve hours after the documentation of severe AKI. We determined each patient’s modality group as the first RRT modality they received. The primary outcome was 60-day overall survival. We used two propensity score methods to balance the differences in baseline characteristics between groups and the primary analysis relied on inverse probability of treatment weighting. Results A total of 543 patients were included. Continuous RRT was the first modality in 269 patients and IHD in 274. Patients receiving CRRT had higher cardiovascular and total-SOFA scores. Inverse probability weighting allowed to adequately balance groups on all predefined confounders. The weighted Kaplan–Meier death rate at day 60 was 54·4% in the CRRT group and 46·5% in the IHD group (weighted HR 1·26, 95% CI 1·01–1·60). Abstract

Introduction Expanded-criteria donors (ECDs) are used to reduce the shortage of kidneys for transplantation. However, kidneys from ECDs are associated with an increased risk of delayed graft function (DGF), a risk factor for allograft loss and mortality. HYPOREME will be a multicentre randomised controlled trial (RCT) comparing targeted hypothermia to normothermia in ECDs, in a country where the use of machine perfusion for organ storage is the standard of care. We hypothesise that hypothermia will decrease the incidence of DGF.Methods and analysis HYPOREME is a multicentre RCT comparing the effect on kidney function in recipients of targeted hypothermia (34°C–35°C) and normothermia (36.5°C–37.5°C) in the ECDs. The temperature intervention starts from randomisation and is maintained until aortic clamping in the operating room. We aim to enrol 289 ECDs in order to analyse the kidney function of 516 recipients in the 53 participating centres. The primary outcome is the occurrence of DGF in kidney recipients, defined as a requirement for renal replacement therapy within 7 days after transplantation (not counting a single session for hyperkalemia during the first 24 hours). Secondary outcomes include the proportion of patients with individual organs transplanted in each group; the number of organs transplanted from each ECD and the vital status and kidney function of the recipients 7 days, 28 days, 3 months and 1 year after transplantation. An interim analysis is planned after the enrolment of 258 kidney recipients.Ethics and dissemination The trial was approved by the ethics committee of the French Intensive Care Society (CE-SRLF-16-07) on 26 April 2016 and by the competent French authorities on 20 April 2016 (Comité de Protection des Personnes-TOURS-Région Centre-Ouest 1, registration #2016-S3). Findings will be published in peer-reviewed journals and presented during national and international scientific meetings.Trial registration number NCT03098706.

Abstract

Type-one cardiorenal syndrome (CRS) is defined by acute decompensated heart failure leading to secondary acute kidney injury. No study evaluates the reliability of transthoracic echocardiography as a help tool for diagnosis and optimization of CRS. Therefore, the aim of this study was to assess echocardiographic parameters in patients with CRS in the Intensive Care Unit.We conducted an observational, prospective, single-center study in the ICU department of a general hospital. Patients admitted in the ICU and presenting with type 1 CRS were included. Transthoracic echocardiography was performed at baseline and at day end after treatment by the same trained operator for the same patients. We report various echocardiographic indices at these two timepoints.Twenty-seven patients were included. At baseline 96.3% of patients had signs of congestion (IVC dilation2 cm), 76% had an altered S-wave (11.5 cm/s), 72.73% had an altered TAPSE (17 mm), 85.19% had an elevated RV/LV diameter ratio (0.6). Between baseline and D end, IVC size and, the number of patients with an elevated RV/LV diameter ratio significantly decreased. Weight decreased, whereas natriuresis significantly increased, and the amount of vasopressors support decreased. Diuresis, and S-wave velocity showed non-significant changes.Main echocardiographic findings at baseline in patients with type 1 CRS consist of a right ventricular dysfunction associated an IVC dilation. We report that weight, RV/LV diameter ratio, and IVC diameter might constitute good follow-up parameters to monitor treatment response.

Abstract

IntroductionExpanded-criteria donors (ECDs) are used to reduce the shortage of kidneys for transplantation. However, kidneys from ECDs are associated with an increased risk of delayed graft function (DGF), a risk factor for allograft loss and mortality. HYPOREME will be a multicentre randomised controlled trial (RCT) comparing targeted hypothermia to normothermia in ECDs, in a country where the use of machine perfusion for organ storage is the standard of care. We hypothesise that hypothermia will decrease the incidence of DGF.Methods and analysisHYPOREME is a multicentre RCT comparing the effect on kidney function in recipients of targeted hypothermia (34°C–35°C) and normothermia (36.5°C–37.5°C) in the ECDs. The temperature intervention starts from randomisation and is maintained until aortic clamping in the operating room. We aim to enrol 289 ECDs in order to analyse the kidney function of 516 recipients in the 53 participating centres. The primary outcome is the occurrence of DGF in kidney recipients, defined as a requirement for renal replacement therapy within 7 days after transplantation (not counting a single session for hyperkalemia during the first 24 hours). Secondary outcomes include the proportion of patients with individual organs transplanted in each group; the number of organs transplanted from each ECD and the vital status and kidney function of the recipients 7 days, 28 days, 3 months and 1 year after transplantation. An interim analysis is planned after the enrolment of 258 kidney recipients.Ethics and disseminationThe trial was approved by the ethics committee of the French Intensive Care Society (CE-SRLF-16-07) on 26 April 2016 and by the competent French authorities on 20 April 2016 (Comité de Protection des Personnes-TOURS-Région Centre-Ouest 1, registration #2016-S3). Abstract

International audience; Background: In patients with acute respiratory distress syndrome (ARDS), the National Heart, Lung, and Blood Institute ARDS Clinical Trials Network recommends a target partial pressure of arterial oxygen (Pao2) between 55 and 80 mm Hg. Prospective validation of this range in patients with ARDS is lacking. We hypothesized that targeting the lower limit of this range would improve outcomes in patients with ARDS.Methods: In this multicenter, randomized trial, we assigned patients with ARDS to receive either conservative oxygen therapy (target Pao2, 55 to 70 mm Hg; oxygen saturation as measured by pulse oximetry [Spo2], 88 to 92%) or liberal oxygen therapy (target Pao2, 90 to 105 mm Hg; Spo2, ≥96%) for 7 days. The same mechanical-ventilation strategies were used in both groups. The primary outcome was death from any cause at 28 days.Results: After the enrollment of 205 patients, the trial was prematurely stopped by the data and safety monitoring board because of safety concerns and a low likelihood of a significant difference between the two groups in the primary outcome. Four patients who did not meet the eligibility criteria were excluded. At day 28, a total of 34 of 99 patients (34.3%) in the conservative-oxygen group and 27 of 102 patients (26.5%) in the liberal-oxygen group had died (difference, 7.8 percentage points; 95% confidence interval [CI], -4.8 to 20.6). At day 90, 44.4% of the patients in the conservative-oxygen group and 30.4% of the patients in the liberal-oxygen group had died (difference, 14.0 percentage points; 95% CI, 0.7 to 27.2). Five mesenteric ischemic events occurred in the conservative-oxygen group.Conclusions: Among patients with ARDS, early exposure to a conservative-oxygenation strategy with a Pao2 between 55 and 70 mm Hg did not increase survival at 28 days. Abstract

IF 79.258 (2017); International audience; BackgroundAcute kidney injury is the most frequent complication in patients with septic shock and is an independent risk factor for death. Although renal-replacement therapy is the standard of care for severe acute kidney injury, the ideal time for initiation remains controversial.MethodsIn a multicenter, randomized, controlled trial, we assigned patients with early-stage septic shock who had severe acute kidney injury at the failure stage of the risk, injury, failure, loss, and end-stage kidney disease (RIFLE) classification system but without life-threatening complications related to acute kidney injury to receive renal-replacement therapy either within 12 hours after documentation of failure-stage acute kidney injury (early strategy) or after a delay of 48 hours if renal recovery had not occurred (delayed strategy). The failure stage of the RIFLE classification system is characterized by a serum creatinine level 3 times the baseline level (or ≥4 mg per deciliter with a rapid increase of ≥0.5 mg per deciliter), urine output less than 0.3 ml per kilogram of body weight per hour for 24 hours or longer, or anuria for at least 12 hours. The primary outcome was death at 90 days.ResultsThe trial was stopped early for futility after the second planned interim analysis. A total of 488 patients underwent randomization; there were no significant between-group differences in the characteristics at baseline. Among the 477 patients for whom follow-up data at 90 days were available, 58% of the patients in the early-strategy group (138 of 239 patients) and 54% in the delayed-strategy group (128 of 238 patients) had died (P=0.38). In the delayed-strategy group, 38% (93 patients) did not receive renal-replacement therapy. Criteria for emergency renal-replacement therapy were met in 17% of the patients in the delayed-strategy group (41 patients).ConclusionsAmong patients with septic shock who had severe acute kidney injury, there was no significant difference in overall mortality at 90 days between patients who were assigned to an early strategy for the initiation of renal-replacement therapy and those who were assigned to a delayed strategy. Abstract

Metformin Associates lactic acidosis (MALA) is a metabolic acidosis with higher anion gap, high levels of blood lactates and treatment by metformin. MALA is a very rare entity but is associated with high mortality (30 to 50%). The extrarenal blood purification may be necessary in emergency. Relatively good clinical tolerance contrasts with a very elevated serum lactate. There is always a trigger and hypovolemia is prevalent. MALA often occurs when its prescription is not indicated (renal failure, cardiac failure, hypovolemia, or patient aged over 80 years). Metformin must be stopped in situations of acute hypovolemia or frail patients as diuretics or blockers of the rennin-angiotensin-aldosterone system.

Abstract

International audience; Abstract Background The COVID-19 pandemic is a heavy burden in terms of health care resources. Future decision-making policies require consistent data on the management and prognosis of the older patients (> 70 years old) with COVID-19 admitted in the intensive care unit (ICU). Methods Characteristics, management, and prognosis of critically ill old patients (> 70 years) were extracted from the international prospective COVID-ICU database. A propensity score weighted-comparison evaluated the impact of intubation upon admission on Day-90 mortality. Results The analysis included 1199 (28% of the COVID-ICU cohort) patients (median [interquartile] age 74 [72–78] years). Fifty-three percent, 31%, and 16% were 70–74, 75–79, and over 80 years old, respectively. The most frequent comorbidities were chronic hypertension (62%), diabetes (30%), and chronic respiratory disease (25%). Median Clinical Frailty Scale was 3 (2–3). Upon admission, the PaO 2 /FiO 2 ratio was 154 (105–222). 740 (62%) patients were intubated on Day-1 and eventually 938 (78%) during their ICU stay. Overall Day-90 mortality was 46% and reached 67% among the 193 patients over 80 years old. Mortality was higher in older patients, diabetics, and those with a lower PaO 2 /FiO 2 ratio upon admission, cardiovascular dysfunction, and a shorter time between first symptoms and ICU admission. Abstract

Introduction : le don d'organe est un enjeu de santé publique avec cependant une grande disparité entre les besoins et le nombre de greffons disponibles. La principale source de greffon provient actuellement de donneurs en état de mort encéphalique. L’Hypertension intracrânienne (HTIC), l’évolution vers la mort encéphalique, et la mort cérébrale installée provoquent des altérations hémodynamiques assez stéréotypées. Une des autres conséquences de la mort encéphalique est l’apparition d’un diabète insipide, pouvant aggraver l’hémodynamique déjà précaire du patient. Ça prise en charge consiste en l’introduction de desmopressine. Initialement considérée comme délétère pour les fonctions rénales des greffons dans les années 1990, elle fait maintenant partie de la prise en charge des donneurs. Il n’existe à ce jour pas d’étude, se concentrant sur la posologie ainsi que le délai d’administration de la desmopressine, aux donneurs d’organes en état de mort encéphalique. Il est donc possible que l’administration la plus précoce possible par rapport au diagnostic de diabète insipide central et à des doses importantes de desmopressine influence l’hémodynamique des donneurs, leur fonction rénale et celle des receveurs à court et long terme. L’objectif de notre étude est de comparer entre deux périodes, les modalités d’administration de la desmopressine en termes de délai d’introduction et de dosage puis l’impact sur l’hémodynamique, la fonction rénale du donneur, ainsi que l’impact sur la fonction rénale des receveurs de greffons rénauxMatériels et Méthodes : nous avons inclus 196 patients hospitalisés au centre hospitalier d’Avignon, avec un codage de mort encéphalique. Les données cliniques et biologiques ont été recueillis. Concernant les données des receveurs, elles nous ont été transmises par l’agence de biomédecine.Résultats : nous avons retrouvé une différence statistiquement significative du délai d’introduction de desmopressine (4H groupe 2015-2017 vs 1H groupe 2018-2020), ainsi que de la posologie totale administrée (1 µg 2015-2017 vs 4 µg 2018-2020). Ayant pour conséquences une différence statistiquement significative d’administration de soluté glucosé, (4L groupe 2015-2017 vs 2,5L groupe 2018-2020)Nous n’avons pas retrouvé de différences concernant les fonctions rénales des donneurs, ni des receveurs. Le délai médian de sortie des patients ayant reçu ces greffons était significativement plus élevé chez les patients ayant reçu des greffons provenant du groupe 2015-2017 (p=0.025). Conclusion : l’utilisation d’un protocole de desmopressine avec une administration continue, permet une augmentation significative des posologies ainsi qu’une diminution du délai d’introduction. Ayant pour conséquences, une diminution des apports de solutés chez les patients éligibles pour un don d’organe. Nous n’avons pas retrouvé de différence concernant les fonctions rénales des donneurs, ni des receveurs, cependant les deux groupes avaient des réanimations bien conduites avec des patients stabilisés, des natrémies maximales contrôlées. Dans ce contexte une différence est difficile à retrouver. Il serait intéressant de réaliser une étude à plus grande échelle, prospective. Un des points intéressants de cette étude, est l’absence d’effets indésirables de l’augmentation des posologies de desmopressine.

Abstract

Introduction : les pneumopathies interstitielles diffuses (PID) forment un groupe hétérogène de maladies caractérisées par une association d’anomalies inflammatoires et fibrotiques du poumon. Une détresse respiratoire aiguë peut être la présentation initiale de la maladie. Cette étude vise à mettre en lumière le diagnostic de PID en réanimation et à décrire l’épidémiologie, le pronostic et la sémiologie scanographique des patients atteints de PID qui ont été diagnostiqués dans le service de réanimation du centre hospitalier d’Avignon (CHA). Matériels et méthode : nous avons réalisé une étude mono-centrique rétrospective. Les dossiers de tous les patients admis en réanimation entre octobre 2017 et février 2020 ont été analysés. Sur 2547 patients, 716 ont été admis pour détresse respiratoire aiguë. Les patients étaient inclus dans la cohorte si la cause de leur détresse respiratoire était une PID. Les critères d’exclusion étaient la présence d’une autre cause de détresse respiratoire aiguë (SDRA, infectieuse, œdème cardiogénique). Pour chaque patient, les données cliniques et scanographiques ont été analysées.Résultats : un diagnostic principal de PID a été retenu pour 26 patients. La plupart étaient des hommes (73%), avec un âge moyen de 65,3 ans. Les diagnostics le plus fréquents étaient les PID associées aux connectivites (31%), les PID idiopathiques (23%), et les lymphangites carcinomateuses (19%). Le score SOFA moyen était de 4,9. La plupart des patients étaient intubés (81%). Le ratio moyen de la pression partielle en oxygène sur la fraction inspirée en oxygène (PaO2/FiO2) était de 165 mmHg. La mortalité toutes causes confondues était de 65,4%. En analyse univariée, la mortalité était plus faible dans le groupe des PID associées aux connectivites (37,5%) et dans le sous-groupe des patients traités par hautes doses de corticoïdes. A contrario, la ventilation mécanique était associée à un fort taux de mortalité (76,2%). Vingt-cinq patient sur 26 (96,2%) ont bénéficié d’un scanner thoracique. Ces scanners ont été analysés et la fréquence relative des patterns lésionnels est présenté ci-dessous :Conclusion : nous avons constitué une cohorte mono-centrique rétrospective de patients dont le diagnostic de PID a été réalisé en réanimation. Cette cause rare de détresse respiratoire aiguë est associée à un mauvais pronostic en réanimation, mais les PID associées aux connectivites semblent avoir un meilleur pronostic. Le scanner haute résolution et l’identification des patterns scanographiques constituent la pierre angulaire du diagnostic. Une connaissance approfondie des PID par les réanimateurs pourrait amener à un diagnostic plus précoce et ainsi à une meilleure prise en charge.

Abstract

Introduction : les besoins en greffons vont croissant. L’hématome intraparenchymateux est devenu la première cause de passage en mort encéphalique. La population de donneurs et de receveurs est de plus en plus âgée. Seulement 50% des prélèvements aboutissent chez les patients de plus de 75 ans.Objectif : notre objectif était d’étudier le devenir des patients très âgés (> 80 ans) pris en charge aux urgences pour une hémorragie intraparenchymateuse grave.Méthode : étude de cohorte rétrospective monocentrique du 1er Janvier 2013 au 17 Juillet 2019 réalisée dans le C.H. d’Avignon. Les patients inclus devaient passer par les urgences de l’hôpital d’Avignon, avoir plus de 80 ans, présenter un Glasgow strictement inférieur à 9 et un hématome intraparenchymateux. Les données simples du scanner et de l’examen clinique étaient recueillies. Nous suivions ensuite leur devenir afin de dénombrer le nombre de patients admis en réanimation, le nombre de passage en état de mort encéphalique puis le nombre de prélèvement et enfin le nombre de greffes réalisées.Résultats : trente six patients répondaient aux critères d’inclusion. Vingt et un patients ont été placés en soins palliatifs aux urgences. 16 patients sont passés en mort encéphalique, tous avaient une anomalie du réflexe pupillaire. Quatre ont été contre-indiqués d’emblée à un prélèvement multi-organes et treize sont passés en réanimation. Quatre patients ont été prélevés ce qui a donné lieu à 8 greffes (3 foies, 4 reins et 1 greffe bi-rénale).Conclusion : le prélèvement chez les patients très âgés fonctionne. Son importance est capitale. Il est important de sélectionner correctement ces patients à l’entrée de façon à éviter une admission futile en réanimation. Il faut donc éduquer les professionnels de santé à la réflexion du prélèvement chez le patient âgé.
