: Introduction: There is paucity of data related to potential gender differences in the use of interventions to prevent and treat cardiovascular disease (CVD) among HIV‐positive individuals. We investigated whether such differences exist in the observational D:A:D cohort study. Methods: Participants were followed from study enrolment until the earliest of death, six months after last visit or February 1, 2015. Initiation of CVD interventions [lipid‐lowering drugs (LLDs), angiotensin‐converting enzyme inhibitors (ACEIs), anti‐hypertensives, invasive cardiovascular procedures (ICPs) were investigated and Poisson regression models calculated whether rates were lower among women than men, adjusting for potential confounders. Results: Women (n = 12,955) were generally at lower CVD risk than men (n = 36,094). Overall, initiation rates of CVD interventions were lower in women than men; LLDs: incidence rate 1.28 [1.21, 1.35] vs. 2.40 [2.34, 2.46]; ACEIs: 0.88 [0.82, 0.93] vs. 1.43 [1.39, 1.48]; anti‐hypertensives: 1.40 [1.33, 1.47] vs. 1.72 [1.68, 1.77] and ICPs: 0.08 [0.06, 0.10] vs. 0.30 [0.28, 0.32], and this was also true for most CVD interventions when exclusively considering periods of follow‐up for which individuals were at high CVD risk. In fully adjusted models, women were less likely to receive CVD interventions than men (LLDs: relative rate 0.83 [0.78, 0.88]; ACEIs: 0.93 [0.86, 1.01]; ICPs: 0.54 [0.43, 0.68]), except for the receipt of anti‐hypertensives (1.17 [1.10, 1.25]). Conclusion: The use of most CVD interventions was lower among women than men. We present two studies that investigated women's motives to pursue a career in top management. The central hypothesis was that masculine culture preferences are Important predictors for career motives. Women were expected to have less masculine preferences than men do, which is assumed to be a deternriinant of tiieir relative absence in management positions. In Study I (N = 327), we investigated gender differences in organizational culture preferences, both in a managerial sample and a sample of non-managerial professionals in private sector organizations. It was shown that gender differences only existed in the non-managerial groups, with women showing less masculine culture preferences than men did. In Study 2 (N=350), we examined the effects of organizational culture preferences on the ambitions of staff employees and middle-level managers to pursue a career at a top management level in one governmental organization. The results showed that organizational culture preferences were predictive for ambition of non-managerial employees, but not for that of middle management employees. Overall, women were less ambitious than men. and even ambitious women perceived work-home conflict as an important barrier to career advancement. [ABSTRACT FROM AUTHOR]

When in distress, people often seek help in regulating their emotions by sharing them with others. Paradoxically, although people perceive such social sharing as beneficial, it often fails to promote emotional recovery. This may be explained by people seeking-and eliciting-emotional support, which offers only momentary relief. We hypothesized that (1) the type of support sharers seek shapes corresponding support provided by listeners, (2) the intensity of sharers' emotions increases their desire for emotional support and decreases their desire for cognitive support, and (3) listeners' empathic accuracy promotes support provision that matches sharers' desires. In 8-min interactions, participants ( N = 208; data collected in 2016-2017) were randomly assigned to the role of sharer (asked to discuss an upsetting situation) or listener (instructed to respond naturally). Next, participants watched their video-recorded interaction in 20-s fragments. Sharers rated their emotional intensity and support desires, and listeners rated the sharer's emotional intensity and their own support provision. First, we found that the desire for support predicted corresponding support provision. Second, the intensity of sharers' emotions was associated with an increased desire for emotional and cognitive support. Third, the more accurately listeners judged sharers' emotional intensity, the more they fulfilled sharers' emotional (but not cognitive) support desire. These findings suggest that people have partial control over the success of their social sharing in bringing about effective interpersonal emotion regulation. People elicit the support they desire at that moment, explaining why they perceive sharing as beneficial even though it may not engender emotional recovery. Previous research has found that populist attitudes and conspiracy mentality - here summarised as anti-establishment attitudes - increase when people feel threatened. Two types of intergroup threat have been distinguished, namely realistic threats (pertaining to socio-economic resources, climate, or health), and symbolic threats (pertaining to cultural values). However, there is no agreement on which types of threat and corresponding appraisals would be most important in predicting anti-establishment attitudes. We hypothesise that it is the threat itself, irrespective of its cause, that predicts anti-establishment attitudes. In the current paper, we conducted new (multilevel) regression analyses on previously collected data from four high-powered studies with multiple time points (Study 1) or collected in multiple nations (Studies 2-4). All studies included a populist attitudes scale, a conspiracy mentality scale, and different types of threat and emotion measures, reflecting both realistic and symbolic threats. Across studies, both realistic and symbolic threats positively predicted anti-establishment attitudes. The results support an emotional appraisal approach to anti-establishment attitudes, which highlights the importance of anxiety and feeling threatened regardless of what type of event elicits the threat.

An oxygen sensor-mounted fine-needle biopsy tool was used for in vivo measurement of oxygen levels in tumor xenografts. The system provides a means of measuring the oxygen content in harvested tumor tissue from specific locations. Oxygen in human tumor xenografts in a murine model was observed for over 1 min. Tissues were mapped in relation to oxygen tension (pO 2 ) readings and sampled for conventional cytological examination. Careful modeling of the pO 2 readings over 60 seconds yielded a diffusion coefficient for oxygen at the sensor tip, providing additional diagnostic information about the tissue before sampling. Oxygen level measurement may provide a useful adjunct to the use of biomarkers in tumor diagnosis.

Past research investigating the relation between social anxiety (SA), empathy and emotion recognition is marked by conceptual and methodological issues. In the present study, we aim to overcome these limitations by examining whether individuals with high (n = 40) vs. low (n = 43) social anxiety differed across these two facets of empathy and whether this could be related to their recognition of emotions. We employed a naturalistic emotion recognition paradigm in which participants watched short videos of individuals (targets) sharing authentic emotional experiences. After each video, we measured self-reported empathic concern and distress, as well as their ability to recognize the emotions expressed by the targets in the videos. Our results show that individuals with high social anxiety recognized the targets' emotions less accurately. Furthermore, high socially anxious individuals reported more personal distress than low socially anxious individuals, whereas no significant difference was found for empathic concern. The findings suggest that reduced recognition of emotions among SA individuals can be better explained by the negative effects of social stress than by a general deficit in empathy., Competing Interests: The authors declare that they have no conflict of interest. Background: Facial weakness is a key feature of facioscapulohumeral muscular dystrophy (FSHD) and may lead to altered facial expression and subsequent psychosocial impairment. There is no cure and supportive treatments focus on optimizing physical fitness and compensation of functional disabilities., Objective: We hypothesize that symptomatic treatment options and psychosocial interventions for other neurological diseases with altered facial expression could be applicable to FSHD. Therefore, the aim of this review is to collect symptomatic treatment approaches that target facial muscle function and psychosocial interventions in various neurological diseases with altered facial expression in order to discuss the applicability to FSHD., Methods: A systematic search was performed. Selected studies had to include FSHD, Bell's palsy, Moebius syndrome, myotonic dystrophy type 1, or Parkinson's disease and treatment options which target altered facial expression. Data was extracted for study and patients' characteristics, outcome assessment tools, treatment, outcome of facial expression and or psychosocial functioning., Results: Forty studies met the inclusion criteria, of which only three studies included FSHD patients exclusively. Most, twenty-one, studies were performed in patients with Bell's palsy. Studies included twelve different therapy categories and results were assessed with different outcomes measures., Conclusions: Five therapy categories were considered applicable to FSHD: training of (non-verbal) communication compensation strategies, speech training, physical therapy, conference attendance, and smile restoration surgery. Further research is needed to establish the effect of these therapies in FSHD. We recommend to include outcome measures in these studies that cover at least cosmetic, functional, communication, and quality of life domains.

The etiology of bladder cancer among never smokers without occupational or environmental exposure to established urothelial carcinogens remains unclear. Urinary mutagenicity is an integrative measure that reflects recent exposure to genotoxic agents. Here, we investigated its potential association with bladder cancer in rural northern New England. We analyzed 156 bladder cancer cases and 247 cancer-free controls from a large population-based case-control study conducted in Maine, New Hampshire, and Vermont. Overnight urine samples were deconjugated enzymatically and the extracted organics were assessed for mutagenicity using the plate-incorporation Ames assay with the Salmonella frameshift strain YG1041 + S9. Logistic regression was used to estimate the odds ratios (OR) and 95% confidence intervals (CI) of bladder cancer in relation to having mutagenic versus nonmutagenic urine, adjusted for age, sex, and state, and stratified by smoking status (never, former, and current). We found evidence for an association between having mutagenic urine and increased bladder cancer risk among never smokers (OR = 3.8, 95% CI: 1.3-11.2) but not among former or current smokers. Risk could not be estimated among current smokers because nearly all cases and controls had mutagenic urine. Urinary mutagenicity among never-smoking controls could not be explained by recent exposure to established occupational and environmental mutagenic bladder carcinogens evaluated in our study. Our findings suggest that among never smokers, urinary mutagenicity potentially reflects genotoxic exposure profiles relevant to bladder carcinogenesis. Background: Acidic urine pH is associated with rapid hydrolysis of N-glucuronide conjugates of aromatic amines into metabolites that may undergo metabolism in the bladder lumen to form mutagenic DNA adducts. We previously reported that consistently acidic urine was associated with increased bladder cancer risk in a hospital-based case-control study in Spain. Here, we conducted a separate study in northern New England to replicate these findings., Methods: In a large, population-based case-control study conducted in Maine, New Hampshire, and Vermont, we examined bladder cancer risk in relation to consistent urine pH, measured twice daily by participants over 4 consecutive days using dipsticks. In parallel, we collected spot urine samples and conducted laboratory measurements of urinary acidity using a pH meter. Unconditional logistic regression was used to estimate associations, adjusting for age, gender, race, Hispanic status, and state. Analyses were further stratified by smoking status., Results: Among 616 urothelial carcinoma cases and 897 controls, urine pH consistently ≤ 6.0 was associated with increased bladder cancer risk (OR = 1.27; 95% confidence interval, 1.02-1.57), with the effect limited to ever-smokers. These findings were supported by analyses of a spot urine, with statistically significant exposure-response relationships for bladder cancer risk overall (Ptrend = 5.1×10-3) and among ever-smokers (Ptrend = 1.2×10-3)., Conclusions: Consistent with a previous study in Spain, our findings suggest that acidic urine pH is associated with increased bladder cancer risk., Impact: Our findings align with experimental results showing that acidic urine pH, which is partly modifiable by lifestyle factors, is linked to hydrolysis of acid-labile conjugates of carcinogenic aromatic amines., (©2023 American Association for Cancer Research.)

Introduction: Rapid onsite evaluation (ROSE) generally uses smears made at the site of the procedure ("smear-based ROSE"). It requires considerable time, generally 2 individuals, technical expertise, and it can be difficult to estimate material available for ancillary studies. We developed an alternative ROSE using liquid-based cytology ThinPrep with hematoxylin and eosin (H&E) stain ("liquid-based ROSE") and assessed its advantages., Materials and Methods: Clinicians rinse the sample(s) into CytoRich Red and send to Pathology. A defined proportion of the needle rinse is removed for a ThinPrep stained with a rapid H&E. Adequacy and diagnosis were compared to final outcome. Total time was recorded., Results: Among 52 liquid-based ROSE readings, 28 (53.8%) were interpreted as "adequate" with final as adequate; 17 (32.7%) were interpreted as "inadequate" with final as inadequate; 7 (13.5%) were interpreted as "inadequate" with final as adequate. Of 23 readings provided with onsite diagnosis, 15 (65.2%) were interpreted as definitive positive or negative diagnoses; 6 (26%) were interpreted as nondiagnostic; and 2 (8.7%) were interpreted as atypical. All definitive diagnoses were concordant with final diagnoses. The time for liquid ROSE performance ranges from 6 to 22 minutes (mean: 13 minutes) and required only 1 individual., Conclusions: Liquid-based ROSE allows accurate adequacy determination and diagnosis, takes about 15 minutes of cytologist time, and can be performed by just 1 person. Pyoderma gangrenosum (PG) has been linked to various underlying systemic diseases; many associations are based on case reports or small case series, including hidradenitis suppurativa. Literature examining systemic therapies according to underlying comorbid condition is limited. The study objective was to investigate comorbid diseases of PG and correlate disease associations with effectiveness of therapeutic interventions. Using Johns Hopkins Medical Institutions medical records, 220 patients had an ICD-9 code of 686.01 for PG between 1 January 2006 and 30 June 2015, of whom 130 patients met rigorous inclusion/exclusion criteria for PG (non-peristomal). The 130 PG patients in our study were 69% female, 58% Caucasian, and 35% African American. Documented comorbid conditions included inflammatory bowel disease (IBD; 35%), rheumatoid arthritis (RA; 12%), hidradenitis suppurativa (HS; 14%), and monoclonal gammopathy (12%). PG patients with HS versus without HS were more likely to be African-American (83% vs. 28%; P < 0.001) and had an earlier mean age of PG onset (38 vs. 48 years; P = 0.02). Strikingly, 53% of female African-American patients with PG onset prior to age 40 had comorbid HS. Comorbid inflammatory bowel disease was observed in 38% of PG patients with RA, 28% of PG patients with HS, and 27% of PG patients with monoclonal gammopathy. Of the 32 patients who received infliximab for active PG, complete ulcer healing was observed in 83% (5/6) of patients with comorbid HS versus 31% (8/26) of patients without HS (Fisher exact P = 0.03). Screening patients for associated systemic disease for multiple related illnesses is essential. Background: Diagnostic accuracy of the standard of care fine-needle aspiration cytology (FNAC) remains a significant problem in thyroid oncology. Therefore, a robust and accurate method for reducing uncertainty of cytopathological evaluation would be invaluable., Methods: In this double-blind study, we employed fluorescence emission and quantitative fluorescence polarization (Fpol) confocal imaging for sorting thyroid cells into benign/malignant categories. Samples were collected from malignant tumors, benign nodules, and normal thyroid epithelial tissues., Results: A total of 32 samples, including 12 from cytologically indeterminate categories, were stained using aqueous methylene blue (MB) solution, imaged, and analyzed. Fluorescence emission images yielded diagnostically relevant information on cytomorphology. Significantly higher MB Fpol was measured in thyroid cancer as compared to benign and normal cells. The results obtained from 12 indeterminate samples revealed that MB Fpol accurately differentiated benign and malignant thyroid nodules., Conclusions: The developed imaging approach holds the potential to provide an accurate and objective biomarker for thyroid cancer, improve diagnostic accuracy of cytopathology, and decrease the number of lobectomy and near-total thyroidectomy procedures.

Envy shapes social hierarchies. To protect their rank, envied persons react to the threat posed by enviers. Doing so requires that envied persons initially perceive who envies them. However, a common perspective is that envy lacks a unique expression and that enviers disguise their experience, preventing the social perception of envy. In contrast to this perspective, recent evidence indicates that observers perceive benign and malicious forms of envy accurately when they can integrate information about targets. These findings suggest that observers infer envy based on multiple, contextual cues. We hypothesized that observers infer envy from facial and bodily expressions in comparison situations. Specifically, observers should infer benign envy when a target, who encounters an advantaged person, turns with disappointment toward the advantage. Conversely, observers should infer malicious envy when the target turns with anger toward the advantaged person. Three preregistered studies tested these hypotheses (total N = 693). In Studies 1 and 2, targets turned with an emotional or neutral expression either toward a person silhouette or a valuable object, and participants rated targets' envy. In Study 3, participants performed the same task with more realistic stimuli. Across studies, emotional display and head turning had independent effects on inferences of benign and malicious envy. Furthermore, observers inferred envy more when the target expressed an emotion instead of remaining neutral. We discuss how the results inform research on the social perception of envy. Context: Pericyte populations abundantly express tyrosine kinases (eg, platelet-derived growth factor receptor-β [PDGFR-β]) and impact therapeutic response. Lenvatinib is a clinically available tyrosine kinase inhibitor that also targets PDGFR-β. Duration of therapeutic response was shorter in patients with greater disease burden and metastasis. Patients may develop drug resistance and tumor progression., Objectives: Develop a gene signature of pericyte abundance to assess with tumor aggressiveness and determine both the response of thyroid-derived pericytes to lenvatinib and their synergies with thyroid carcinoma-derived cells., Design: Using a new gene signature, we estimated the relative abundance of pericytes in papillary thyroid carcinoma (PTC) and normal thyroid (NT) TCGA samples. We also cocultured CD90+;PAX8- thyroid-derived pericytes and BRAFWT/V600E-PTC-derived cells to determine effects of coculture on paracrine communications and lenvatinib response., Results: Pericyte abundance is significantly higher in BRAFV600E-PTC with hTERT mutations and copy number alterations compared with NT or BRAFWT-PTC samples, even when data are corrected for clinical-pathologic confounders. We have identified upregulated pathways important for tumor survival, immunomodulation, RNA transcription, cell-cycle regulation, and cholesterol metabolism. Pericyte growth is significantly increased by platelet-derived growth factor-BB, which activates phospho(p)-PDGFR-β, pERK1/2, and pAKT. Lenvatinib strongly inhibits pericyte viability by down-regulating MAPK, pAKT, and p-p70S6-kinase downstream PDGFR-β. Critically, lenvatinib significantly induces higher BRAFWT/V600E-PTC cell death when cocultured with pericytes, as a result of pericyte targeting via PDGFR-β., Conclusions: This is the first thyroid-specific model of lenvatinib therapeutic efficacy against pericyte viability, which disadvantages BRAFWT/V600E-PTC growth. Assessing pericyte abundance in patients with PTC could be essential to selection rationales for appropriate targeted therapy with lenvatinib., (© The Author(s) 2021. Background: Periocular dark circles (PDCs) are a common cosmetic complaint. Grading systems based on objective measures have been used but no standard system is in place., Objective: To determine factors associated with subjective and objective PDC severity., Methods: Enrolled patients (n=100) completed a questionnaire comprised of demographic variables, medical history, and self-perception of PDC. Those perceiving PDC graded dissatisfaction on a 10-point scale. Clinical severity (grades 0~4) and subtype (constitutional, post-inflammatory, vascular, shadow effects, or others) were determined. A Konica Minolta CR-400 chromameter was used to obtain colorimetry measurements (L * a * b * values). The objective average difference in darkness (ΔL * ) between the periocular region and the cheek was determined. Comparisons were made using Spearman correlation coefficients (r)., Results: Patient dissatisfaction correlated with both clinical severity (r=0.46, p <0.001) and the ΔL * by colorimetry (r=0.35, p =0.004). Factors associated with subjective dissatisfaction were female sex (r=0.38, p =0.002), higher Fitzpatrick skin type (r=0.42, p =0.001), fewer hours of sleep (r=-0.28, p =0.03), and use of concealer (r=0.35, p =0.004). Factors associated with objective measures were higher Fitzpatrick skin type (r=0.36, p =0.0007 and r=0.28, p =0.009, respectively), family history of PDC (r=0.34, p <0.001 and r=0.20, p =0.05), and history of eczema (r=0.45, p <0.001 and r=0.20, p =0.0504). Clinical severity grading correlated with colorimetric severity (r=0.36, p =0.0003)., Conclusion: Overall, subjective dissatisfaction was associated with clinical severity. However, factors associated with subjective severity did not necessarily overlap with factors associated with objective severity. These findings highlight the importance of patient-reported grading. Background: Urine cytology can reliably diagnose high-grade urothelial carcinoma (HGUC) but not low-grade urothelial carcinoma (LGUC), and a more sensitive test is needed. Previously, a pilot study highlighted the possible diagnostic utility of next-generation sequencing (NGS) in identifying both LGUC and HGUC in urine cytology specimens., Methods: Twenty-eight urine ThinPrep cytology specimens and preceding or subsequent bladder tumor biopsy/resection specimens obtained within 3 months were included in the study (LGUC, n = 15; HGUC, n = 13). A customized, bladder-specific NGS panel was performed; it covered 69 frequently mutated or altered genes in urothelial carcinoma (UC) that were reported by The Cancer Genome Atlas and the Catalogue of Somatic Mutations in Cancer., Results: The sequencing results were compared between the urine cytology specimens and the corresponding bladder tumor biopsies/resections. TP53 was the most frequently identified mutation in HGUC cases (11 of 13 [85%]). PIK3CA and KDM6A were the most frequently identified mutations in LGUC: they occurred in 7 of 15 cases (47%) and in 6 of 15 cases (40%), respectively. Additional frequent mutations identified in the panel included ARID1A (n = 5), EP300 (n = 4), LRP1B (n = 3), ERBB2 (n = 2), STAG2 (n = 2), FGFR3 (n = 3), MLL (n = 2), MLL3 (n = 2), CREBBP1 (n = 1), RB1 (n = 1), and FAT4 (n = 1). Overall, the concordance between the cytology and surgical specimens was 75%. The sensitivity and specificity for identifying mutations in urine cytology specimens were 84% and 100%, respectively., Conclusions: A bladder-specific NGS panel increases the sensitivity and specificity of urine cytology's diagnostic utility in both low- and high-grade tumors and may serve as a noninvasive surveillance method in the follow-up of patients with UC harboring known mutations., (© 2021 American Cancer Society.)

Differentiating adenoid cystic carcinoma (AdCC) from other basaloid neoplasm in a fine needle aspiration (FNA) sample can be challenging. Activation of MYB in AdCC by the fusion transcript MYB-NFIB has been recently demonstrated in salivary gland and other organs. The aim of this study is to evaluate the utility of MYB immunohistochemistry (IHC) in distinguishing AdCCs and other basaloid neoplasm in cytology specimens. Eighteen FNA cases, from salivary gland and other sites, and their subsequent surgical resection specimens were included in the study. Eight cases were confirmed AdCC on resection. MYB IHC was performed on slides made from cytology cell block and surgical resection paraffin blocks. Percentage and intensity of nuclear staining in tumor cells was scored as 0 to 3. The staining results were concordant between cytology specimens and their corresponding surgical resection tumors. Strong diffuse nuclear staining (score 3, N = 5) was exclusively observed in AdCC, both in cytology and surgical specimens. Only one pleomorphic adenoma and one poorly differentiated basaloid carcinoma were positive for MYB staining (score 1 to 2). Any degree of nuclear MYB labeling was seen in 100% AdCC cases (N = 8/8) compared with of 20% (N = 2/10) of all other non-AdCC cases (P = < 0.001). The sensitivity and specificity of any degree MYB positivity for AdCC in cytology specimen is 100% and 78%. The sensitivity and specificity of strong diffuse MYB labeling (score 2 to 3) for AdCC is 83% and 100% in cytology specimen. Strong diffuse nuclear staining of MYB is valuable in supporting a cytologic diagnosis of AdCC. However, weak and focal labeling of MYB should be interpreted with caution as it can be seen in benign and other malignant basaloid lesions.

While new regulations obligate or recommend people to wear medical masks at public places to prevent further spread of the Covid-19 virus, there are still open questions as to what face coverage does to social emotional communication. Previous research on the effects of wearing veils or face-covering niqabs showed that covering of the mouth led to the attribution of negative emotions and to the perception of less intense positive emotions. The current study compares a sample from the Netherlands with a sample from the United Arab Emirates on their perception of emotions from faces covered by a niqab, censoring black bars, or uncovered faces. The results show that covering the mouth area leads to greater anxiety in participants in both countries. Furthermore, although participants did not report greater decoding difficulties for faces that were covered as compared to fully visible, results show that face coverage did influence emotion perception. Specifically, happiness and anger were perceived as being less intense. Further, face coverage by a niqab, as compared to black bars, yielded lower emotional intensity ratings. Individual differences in understanding other people's emotions have typically been studied with recognition tests using prototypical emotional expressions. These tests have been criticized for the use of posed, prototypical displays, raising the question of whether such tests tell us anything about the ability to understand spontaneous, non-prototypical emotional expressions. Here, we employ the Emotional Accuracy Test (EAT), which uses natural emotional expressions and defines the recognition as the match between the emotion ratings of a target and a perceiver. In two preregistered studies (N total = 231), we compared the performance on the EAT with two well-established tests of emotion recognition ability: the Geneva Emotion Recognition Test (GERT) and the Reading the Mind in the Eyes Test (RMET). We found significant overlap ( r > 0.20) between individuals' performance in recognizing spontaneous emotions in naturalistic settings (EAT) and posed (or enacted) non-verbal measures of emotion recognition (GERT, RMET), even when controlling for individual differences in verbal IQ. On average, however, participants reported enjoying the EAT more than the other tasks. Thus, the current research provides a proof-of-concept validation of the EAT as a useful measure for testing the understanding of others' emotions, a crucial feature of emotional intelligence. Further, our findings indicate that emotion recognition tests using prototypical expressions are valid proxies for measuring the understanding of others' emotions in more realistic everyday contexts.

Introduction: Colposcopic endocervical brushing cytology (CEB) is more sensitive than endocervical curettage (ECC) for detecting squamous intraepithelial lesions. There are no data on performance of CEB for detecting endocervical adenocarcinoma., Materials and Methods: A total of 151 patients were identified in a word search for "endocervical adenocarcinoma" in surgical pathology reports from January 2007 to June 2019. To measure sensitivity, reports of CEB or ECC samples within 1 year preceding the first surgical pathology diagnosis of at least endocervical adenocarcinoma in situ (AIS+) were examined. Specificity was measured in a cohort in which at least atypical glandular cells (AGC+) was reported in CEB or ECC., Results: Seven CEB preceding diagnosis of AIS were identified: 6 of 7 were positive or suspicious for AIS+. One of 7 was negative and it was negative on re-review. Three of 6 positive CEB cases used cell blocks with immunohistochemistry. Seventy ECC samples preceding diagnosis of AIS were identified: 40 of 70 were diagnosed as AGC+. The sensitivities of CEB and ECC for detecting AIS+ at a threshold of AGC+ are 86% and 57% (too few patients for statistics), respectively. For specificity, 12 of 18 CEB and 9 of 25 ECC reports with AGC+ were false positive by follow-up surgical pathology. The specificities of CEB and ECC are 99.4% and 99.9%, respectively., Conclusion: Sensitivity of CEB for detecting AIS+ (86%) is at least as high as ECC (57%). Specificity of CEB is similar to ECC. Addition of a cell block to CEB may be useful. CEB appears to be an appropriate test for follow-up of atypical glandular cells reported on Papanicolaou tests., (Copyright © 2021. Roux-en-Y gastric bypass (RYGB) is the most effective treatment for morbid obesity and results in rapid remission of type 2 diabetes (T2D), before significant weight loss occurs. The underlying mechanisms for T2D remission are not fully understood. To gain insight into these mechanisms we used RYGB-operated diabetic GK-rats and Wistar control rats. Twelve adult male Wistar- and twelve adult male GK-rats were subjected to RYGB- or sham-operation. Oral glucose tolerance tests (OGTT) were performed six weeks after surgery. RYGB normalized fasting glucose levels in GK-rats, without affecting fasting insulin levels. In both rat strains, RYGB caused increased postprandial responses in glucose, GLP-1, and GIP. RYGB caused elevated postprandial insulin secretion in Wistar-rats, but had no effect on insulin secretion in GK-rats. In agreement with this, RYGB improved HOMA-IR in GK-rats, but had no effect on HOMA-β. RYGB-operated GK-rats had an increased number of GIP receptor and GLP-1 receptor immunoreactive islet cells, but RYGB had no major effect on beta or alpha cell mass. Furthermore, in RYGB-operated GK-rats, increased Slc5a1, Pck2 and Pfkfb1 and reduced Fasn hepatic mRNA expression was observed. In summary, our data shows that RYGB induces T2D remission and enhanced postprandial incretin hormone secretion in GK-rats, without affecting insulin secretion or beta cell mass. Thus our data question the dogmatic view of how T2D remission is achieved and instead point at improved insulin sensitivity as the main mechanism of remission., (Copyright © 2020 Elsevier Inc. Introduction: Cytology and cystoscopy are used to detect urothelial carcinoma (UC), but together they still fail to detect some UC cases and are not suitable for screening asymptomatic individuals. Mutations are present in more than 98% of UC, mutations have therapeutic significance, and they can be detected by next generation sequencing (NGS) in urine samples. We review the role of NGS in UC detection., Materials and Methods: Comprehensive literature review on UC genetics, economics of NGS, and previous reports of UC detection by NGS., Results: The raw costs of NGS have decreased to about 14,000 base pairs per penny, making it appear economically feasible to use NGS widely. Reported NGS assays fall short of predicted sensitivity. Decreased sensitivity is attributed to a low frequency of mutant alleles in many urine samples. Attempts to increase the percentage of mutant alleles, by using cell-free urinary DNA, or by using cell sorting and microfluidics, have been unsuccessful or remain unproven. However, cytologic examination can immediately enable NGS: Urine cytologies with sufficient proportions of abnormal cells could be directly triaged to NGS with high sensitivity for UC detection. For cases with a low proportion of abnormal cells, cytologically targeted microdissection of cells for NGS should maintain sensitivity and decrease sequencing costs. Cytologically targeted urothelial cells for NGS could allow a screening test for low grade UC., Conclusions: Cytology is immediately poised to allow NGS to improve the diagnosis of UC, allowing NGS to be an ancillary test for atypical cytologies, and potentially allowing a screening test for low-grade UC., (Copyright © 2020. Background: Culturally responsive teaching (CRT) has been associated with increased student engagement and achievement. Its practice in classrooms, however, has been shown to be less than optimal. Nonetheless, certain teacher qualities have been suggested to facilitate its practice., Aims: The current study sought quantitative evidence in support of two of these teacher qualities, namely teachers' multicultural attitudes, and their perspective taking abilities. By identifying the strength of the suggested relationships, we aimed to examine the generalizability of previous findings in the literature and inform teachers' professional development and interventions., Sample: A total of 143 primary school teachers from different parts of the Netherlands responded to our online survey., Methods: We conducted a multivariate multiple regression analysis to investigate the relationship between these qualities and teachers' engagement in two separate but related components of CRT (i.e., socially sensitive and culturally sensitive teaching)., Results: Results of our analysis yielded significant relationships between the two teacher qualities and the frequency with which teachers engage in socially and culturally sensitive teaching. Perspective taking was a stronger predictor for both aspects of CRT., Conclusion: These findings signal the significance of incorporating especially perspective taking experiences and exercises into teacher education and professional development programmes, which could benefit all students regardless of their backgrounds. Researchers examining nonverbal communication of emotions are becoming increasingly interested in differentiations between different positive emotional states like interest, relief, and pride. But despite the importance of the voice in communicating emotion in general and positive emotion in particular, there is to date no systematic review of what characterizes vocal expressions of different positive emotions. Furthermore, integration and synthesis of current findings are lacking. In this review, we comprehensively review studies (N = 108) investigating acoustic features relating to specific positive emotions in speech prosody and nonverbal vocalizations. We find that happy voices are generally loud with considerable variability in loudness, have high and variable pitch, and are high in the first two formant frequencies. When specific positive emotions are directly compared with each other, pitch mean, loudness mean, and speech rate differ across positive emotions, with patterns mapping onto clusters of emotions, so-called emotion families. For instance, pitch is higher for epistemological emotions (amusement, interest, relief), moderate for savouring emotions (contentment and pleasure), and lower for a prosocial emotion (admiration). Some, but not all, of the differences in acoustic patterns also map on to differences in arousal levels. We end by pointing to limitations in extant work and making concrete proposals for future research on positive emotions in the voice.

Empathizing with others is widely presumed to increase our understanding of their emotions. Little is known, however, about which empathic process actually help people recognize others' feelings more accurately. Here, we probed the relationship between emotion recognition and two empathic processes: spontaneously felt similarity (having had a similar experience) and deliberate perspective taking (focus on the other vs. oneself). We report four studies in which participants (total N  = 803) watched videos of targets sharing genuine negative emotional experiences. Participants' multi-scalar ratings of the targets' emotions were compared with the targets' own emotion ratings. In Study 1 we found that having had a similar experience to what the target was sharing was associated with lower recognition of the target's emotions. Study 2 replicated the same pattern and in addition showed that making participants' own imagined reaction to the described event salient resulted in further reduced accuracy. Studies 3 and 4 were preregistered replications and extensions of Studies 1 and 2, in which we observed the same outcome using a different stimulus set, indicating the robustness of the finding. Moreover, Study 4 directly investigated the underlying mechanism of the observed effect. Findings showed that perceivers who have had a negative life experience similar to the emotional event described in the video felt greater personal distress after watching the video, which in part explained their reduced accuracy. These results provide the first demonstration that spontaneous empathy, evoked by similarity in negative experiences, may inhibit rather than increase our understanding of others' emotions., (© 2019 The Authors.)

Emotions are part and parcel of the human condition, but their nature is debated. Three broad classes of theories about the nature of emotions can be distinguished: affect-program theories, constructionist theories, and appraisal theories. Integrating these broad classes of theories into a unifying theory is challenging. An integrative psychometric model of emotions can inform such a theory because psychometric models are intertwined with theoretical perspectives about constructs. To identify an integrative psychometric model, we delineate properties of emotions stated by emotion theories and investigate whether psychometric models account for these properties. Specifically, an integrative psychometric model of emotions should allow (a) identifying distinct emotions (central in affect-program theories), (b) between- and within-person variations of emotions (central in constructionist theories), and (c) causal relationships between emotion components (central in appraisal theories). Evidence suggests that the popular reflective and formative latent variable models-in which emotions are conceptualized as unobservable causes or consequences of emotion components-cannot account for all properties. Conversely, a psychometric network model-in which emotions are conceptualized as systems of causally interacting emotion components-accounts for all properties. The psychometric network model thus constitutes an integrative psychometric model of emotions, facilitating progress toward a unifying theory.

Background: The efficacy of antibiotics in rosacea treatment suggests a role for microorganisms in its pathophysiology. Growing concern over the adverse effects of antibiotic use presents a need for targeted antimicrobial treatment in rosacea., Objective: We performed a case-control study to investigate the skin microbiota in patients with rosacea compared to controls matched by age, sex, and race., Methods: Nineteen participants with rosacea, erythematotelangiectatic, papulopustular, or both, were matched to 19 rosacea-free controls. DNA was extracted from skin swabs of the nose and bilateral cheeks of participants. Sequencing of the V3V4 region of the bacterial 16S ribosomal RNA gene was performed using Illumina MiSeq and analyzed using QIIME/MetaStats 2.0 software., Results: Compared with controls, skin microbiota in erythematotelangiectatic rosacea was depleted in Roseomonas mucosa (p = 0.004). Papulopustular rosacea was enriched in Campylobacter ureolyticus (p = 0.001), Corynebacterium kroppenstedtii (p = 0.008), and the oral flora Prevotella intermedia (p = 0.001). The highest relative abundance of C. kroppenstedtii was observed in patients with both erythematotelangiectatic and papulopustular rosacea (19.2%), followed by papulopustular (5.06%) and erythematotelangiectatic (1.21%) rosacea. C. kroppenstedtii was also associated with more extensive disease, with the highest relative abundance in rosacea affecting both the cheeks and nose (2.82%), followed by rosacea sparing the nose (1.93%), and controls (0.19%)., Conclusions: The skin microbiota in individuals with rosacea displays changes from that of healthy skin, suggesting that further studies examining a potential role for the skin microbiota in the pathophysiology of rosacea may be warranted.

Introduction: Because of the high rates of false-negative or nondiagnostic ureteral Piranha microbiopsies associated with low cellularity, we assessed the effect of processing these using cytology., Materials and Methods: We included 2 groups of 44 consecutive microbiopsies processed from formalin as a standard surgical biopsy and 22 processed by cytology. All samples were from the ureter or renal pelvis or calyx. The cytology samples were collected in alcohol-based media and were prepared with a Cellient cell block only (n = 9) or with a Cellient cell block for the visible particles, together with ThinPrep, to capture the remaining desquamated cells (n = 13)., Results: Malignancy was diagnosed in 5 of 44 conventionally processed microbiopsies (11%) compared with 14 of 22 cytologically processed microbiopsies (64%; P < 0.001), including 1 case with invasion. Nineteen site-matched biopsies from 2 patients had undergone both cytologic and surgical processing, with 8 of 8 cytologically processed biopsies diagnosed as malignant. None of the 11 surgically processed biopsies from the same patients matched for site were diagnosed as malignant. Of the 11, 2 (18%) were suspicious for high-grade urothelial carcinoma and 6 (55%) were considered atypical. Increased sensitivity from cytologic processing appears related to increased cell recovery; large numbers of well-preserved urothelial cells were identified in the ThinPrep (range, 1000-25,000 cells/slide), and a nonsignificant trend was found toward increased urothelium (defined as >200 cells/profile) in the Cellient cell blocks (14 of 22 [64%]) compared with the histologic biopsies (17 of 44 [39%]; P = 0.070)., Conclusions: Cytologic processing of ureteral microbiopsies showed superior sensitivity for detecting high-grade urothelial carcinoma, apparently owing to the increased cellular recovery., (Copyright © 2020. Crying is a common response to emotional distress that elicits support from the environment. People may regulate another's crying in several ways, such as by providing socio-affective support (e.g. comforting) or cognitive support (e.g. reappraisal), or by trying to emotionally disengage the other by suppression or distraction. We examined whether people adapt their interpersonal emotion regulation strategies to the situational context, by manipulating the regulatory demand of the situation in which someone is crying. Participants watched a video of a crying man and provided support by recording a video message. We hypothesised that when immediate down-regulation was required (i.e. high regulatory demand), participants would provide lower levels of socio-affective and cognitive support, and instead distract the crying person or encourage them to suppress their emotions, compared to when there is no such urgency (i.e. low regulatory demand). As predicted, both self-reported and behavioural responses indicated that high (as compared to low) regulatory demand led to a reduction in socio-affective support provision, and a strong increase in suppression and distraction. Cognitive support provision, however, was unaffected by regulatory demand. When the context required more immediate down-regulation, participants thus employed more regulation strategies aimed at disengaging from the emotional experience. This study provides a first step in showing that people take the context into account when attempting to regulate others' emotions.

When in emotional distress, people often turn to others for support. Paradoxically, even when people perceive social support to be beneficial, it often does not result in emotional recovery. This paradox may be explained by the fact that the sharing process disproportionately centres on support that is not helpful in the long run. A distinction has been made between two types of support that are differentially effective: Whereas socio-affective support alleviates momentary emotional distress, cognitive support fosters long-term recovery. But can listeners tell what support the sharer needs? The present study examines the hypothesis that sharers communicate their support goals by sharing in such a way that it allows listeners to infer the sharer's needs. In Experiment 1, we manipulated participants' support goals, and showed that socio-affective support goals led participants to express more emotions, whereas cognitive support goals resulted in greater use of appraisals. In Experiments 2 and 3, we tested whether these differential expressions would affect the support goals that listeners inferred. We found no evidence for such an effect: Listeners consistently perceived the sharer to predominantly want socio-affective support. These findings help explain why many social sharing instances revolve around socio-affective support, leading to subjectively experienced benefits, but not to genuine recovery.

Context.—: Thyroid nodules are a common clinical problem. Cytologic evaluation via fine-needle aspiration is often employed in the diagnostic workup, and rapid on-site assessment of adequacy can help ensure an adequate sample is obtained. However, rapid on-site assessment of adequacy only examines part of the sample, a part that may not then be available for ancillary testing. Moreover, the procedure is time-consuming and poorly reimbursed., Objective.—: To develop an automatable fluorescence-based image analysis system for assessing the adequacy of thyroid fine-needle aspirations that uses the entire aspirated sample., Design.—: There were 12 previously diagnosed cases that served as a training set, and 11 cases were used for validation of an image analysis algorithm. The samples were fluorescently stained and imaged using a fluorescent microscope. The images were assessed for adequacy by an image analysis algorithm. Following image analysis, a ThinPrep slide was prepared and blindly scored by a cytopathologist. The standard and computer-derived results were then compared., Results.—: The algorithm was optimized using the 12 cases in the training set and then applied to the 11 test cases. A total of 8 of 8 adequate samples in the test group were correctly scored as adequate, and 2 of 3 cases that were inadequate were correctly scored as inadequate by the algorithm. One case was erroneously designated as not adequate by the algorithm., Conclusions.—: Our results demonstrate the feasibility of automating thyroid adequacy assessment using a fluorescent labeling technique followed by computer image analysis.

Impaired incretin effect is a culprit in Type 2 Diabetes. Cocaine- and amphetamine-regulated transcript (CART) is a regulatory peptide controlling pancreatic islet hormone secretion and beta-cell survival. Here we studied the potential expression of CART in enteroendocrine cells and examined the role of CART as a regulator of incretin secretion and expression. CART expression was found in glucose-dependent insulinotropic polypeptide (GIP)-producing K-cells and glucagon-like peptide-1 (GLP-1)-producing L-cells in human duodenum and jejunum and circulating CART levels were increased 60 min after a meal in humans. CART expression was increased by fatty acids and GIP, but unaffected by glucose in GLUTag and STC-1 cells. Exogenous CART had no effect on GIP and GLP-1 expression and secretion in GLUTag or STC-1 cells, but siRNA-mediated silencing of CART reduced GLP-1 expression and secretion. Furthermore, acute intravenous administration of CART increased GIP and GLP-1 secretion during an oral glucose-tolerance test in mice. We conclude that CART is a novel constituent of human K- and L-cells with stimulatory actions on incretin secretion and that interfering with the CART system may be a therapeutic avenue for T2D., (Copyright © 2018 Elsevier B.V. When in emotional distress, people often turn to others for social support. A general distinction has been made between two types of support that are differentially effective: Whereas socio-affective support temporarily alleviates emotional distress, cognitive support may contribute to better long-term recovery. In the current studies, we examine what type of support individuals seek. We first confirmed in a pilot study that these two types of support can be reliably distinguished. Then, in Study 1, we experimentally tested participants' support evaluations in response to different emotional situations using a vignette methodology. Findings showed that individuals perceived any type of reaction that included socio-affective support as preferable. The evaluation of cognitive support, however, was dependent on the specific emotion: Unlike worry and regret, anger and sadness were characterised by a strong dislike for purely cognitive support. Using different materials, Study 2 replicated these findings. Taken together, the findings suggest that individuals evaluate different types of support in a way that is unlikely to benefit emotional recovery in the long run.

Introduction: Scarring is an unfortunate clinical outcome of acne. Current treatment options for atrophic acne scars are dominated by non-pharmacological, invasive procedures which may not be suitable or affordable to all patients. This phase II, single-center, open-label, exploratory study assessed the efficacy, safety and subject-reported outcomes of adapalene 0.3% gel in the treatment of atrophic acne scars., Methods: The study included subjects aged 18-50 years with past history of acne and moderate to severe facial atrophic acne scars. Subjects received adapalene 0.3% gel once daily for the first 4 weeks and twice daily for the following 20 weeks. Assessments were performed at baseline, day 10 and weeks 4, 8, 16 and 24, and at post-treatment follow-ups (weeks 36 and 48-72)., Results: At week 24, investigator and subject assessments reported improvement in skin texture/atrophic scars in 50% and > 80% of subjects, respectively. Subjects were satisfied with the treatment and reported improvements in quality of life., Conclusion: Daily use of adapalene 0.3% gel for the treatment of atrophic acne scars showed promising clinical efficacy, a favorable tolerability profile, and improvement in quality of life., Funding: Nestlé Skin Health-Galderma R&D., Trial Registration: ClinicalTrials.gov Identifier NCT01213199.

Posed stimuli dominate the study of nonverbal communication of emotion, but concerns have been raised that the use of posed stimuli may inflate recognition accuracy relative to spontaneous expressions. Here, we compare recognition of emotions from spontaneous expressions with that of matched posed stimuli. Participants made forced-choice judgments about the expressed emotion and whether the expression was spontaneous, and rated expressions on intensity (Experiments 1 and 2) and prototypicality (Experiment 2). Listeners were able to accurately infer emotions from both posed and spontaneous expressions, from auditory, visual, and audiovisual cues. Furthermore, perceived intensity and prototypicality were found to play a role in the accurate recognition of emotion, particularly from spontaneous expressions. Our findings demonstrate that perceivers can reliably recognise emotions from spontaneous expressions, and that depending on the comparison set, recognition levels can even be equivalent to that of posed stimulus sets.

Recognising emotions from faces that are partly covered is more difficult than from fully visible faces. The focus of the present study is on the role of an Islamic versus non-Islamic context, i.e. Islamic versus non-Islamic headdress in perceiving emotions. We report an experiment that investigates whether briefly presented (40 ms) facial expressions of anger, fear, happiness and sadness are perceived differently when covered by a niqāb or turban, compared to a cap and shawl. In addition, we examined whether oxytocin, a neuropeptide regulating affection, bonding and cooperation between ingroup members and fostering outgroup vigilance and derogation, would differentially impact on emotion recognition from wearers of Islamic versus non-Islamic headdresses. The results first of all show that the recognition of happiness was more accurate when the face was covered by a Western compared to Islamic headdress. Second, participants more often incorrectly assigned sadness to a face covered by an Islamic headdress compared to a cap and shawl. Third, when correctly recognising sadness, they did so faster when the face was covered by an Islamic compared to Western headdress. Fourth, oxytocin did not modulate any of these effects. Implications for theorising about the role of group membership on emotion perception are discussed.

Previous meta-analyses and reviews on gender differences in emotion recognition have shown a small to moderate female advantage. However, inconsistent evidence from recent studies has raised questions regarding the implications of different methodologies, stimuli, and samples. In the present research based on a community sample of more than 5000 participants, we tested the emotional sensitivity hypothesis, stating that women are more sensitive to perceive subtle, i.e. low intense or ambiguous, emotion cues. In addition, we included a self-report emotional intelligence test in order to examine any discrepancy between self-perceptions and actual performance for both men and women. We used a wide range of stimuli and models, displaying six different emotions at two different intensity levels. In order to better tap sensitivity for subtle emotion cues, we did not use a forced choice format, but rather intensity measures of different emotions. We found no support for the emotional sensitivity account, as both genders rated the target emotions as similarly intense at both levels of stimulus intensity. Men, however, more strongly perceived non-target emotions to be present than women. In addition, we also found that the lower scores of men in self-reported EI was not related to their actual perception of target emotions, but it was to the perception of non-target emotions.

Socially anxiety may be related to a different pattern of facial mimicry and contagion of others' emotions. We report two studies in which participants with different levels of social anxiety reacted to others' emotional displays, either shown on a computer screen (Study 1) or in an actual social interaction (Study 2). Study 1 examined facial mimicry and emotional contagion in response to displays of happiness, anger, fear, and contempt. Participants mimicked negative and positive emotions to some extent, but we found no relation between mimicry and the social anxiety level of the participants. Furthermore, socially anxious individuals were more prone to experience negative emotions and felt more irritated in response to negative emotion displays. In Study 2, we found that social anxiety was related to enhanced mimicry of smiling, but this was only the case for polite smiles and not for enjoyment smiles. These results suggest that socially anxious individuals tend to catch negative emotions from others, but suppress their expression by mimicking positive displays. This may be explained by the tendency of socially anxious individuals to avoid conflict or rejection.

I focus on some differences between negative emotional states and how they are coped with in explaining different cardiac risks. The different cognitive, motivational, and physiological characteristics of emotions imply different appraisals of the negative event, and different resources to cope with the event. Cardiovascular activity depends on these different appraisals and coping strategies. For example, cortisol levels have shown to be differently associated with anger and fear responses to social stress. In addition, different ways to regulate one's emotions are also associated with different bodily responses that may increase or decrease cardiac risks. Future research should not only examine different emotions to stressors, but also more long-term regulation strategies and coping resources, such as self-esteem.

Background: Eczema is a common chronic inflammatory disease of the skin. Studies suggest differences in disease prevalence and severity by race/ethnicity. Our knowledge of health care utilization for eczema among different racial/ethnic groups remains limited., Objective: To evaluate health care utilization for childhood eczema among different racial/ethnic groups in the United States., Methods: We performed a cohort study of non-Hispanic white (reference), non-Hispanic black, and Hispanic white individuals under the age of 18 years with caregiver-reported eczema (N = 2043) pooled from the 2-year longitudinal cohorts of the 2001-2013 Medical Expenditure Panel Surveys. Health care utilization outcomes were evaluated over the 2-year follow-up period by race/ethnicity using multivariable regression., Results: Among all children with eczema, non-Hispanic blacks were less likely than whites to report an ambulatory visit for eczema (adjusted odds ratio [OR adj ] 0.69; 95% confidence interval [CI] 0.51-0.92). Among those with ≥1 ambulatory visit for eczema, non-Hispanic blacks reported more visits (adjusted incidence rate ratio [IRR adj ] 1.68; 95% CI 1.10-2.55) and prescriptions (IRR adj 1.22; 95% CI 1.01-1.46) than whites and were more likely than whites to report a dermatology visit (OR adj 1.82; 95% CI 1.06-3.14) for eczema., Limitations: We used caregiver- or self-reported data., Conclusion: Our findings suggest disparities in health care utilization for eczema among non-Hispanic black children despite utilization patterns suggestive of more severe disease., (Copyright © 2017 American Academy of Dermatology, Inc. Choosing the correct statistical test to analyze results is essential in interpreting the validity of the study and centers on defining the study variables and purpose of the analysis. PD-L1 expression in the tumor immune microenvironment is recognized as both a prognostic and predictive biomarker in patients with cutaneous melanoma, a finding closely related to its adaptive (IFN-γ-mediated) mechanism of expression. Approximately 35% of cutaneous melanomas express PD-L1, however, the expression patterns, levels, and prevalence in rarer melanoma subtypes are not well described. We performed immunohistochemistry for PD-L1 and CD8 on 200 formalin-fixed paraffin-embedded specimens from patients with acral (n=16), mucosal (n=36), uveal (n=103), and chronic sun-damaged (CSD) (n=45) melanomas (24 lentigo maligna, 13 'mixed' desmoplastic, and 8 'pure' desmoplastic melanomas). CD8+ tumor-infiltrating lymphocyte (TIL) densities were characterized as mild, moderate, or severe, and their geographic association with PD-L1 expression was evaluated. Discrete lymphoid aggregates, the presence of a spindle cell morphology, and the relationship of these features with PD-L1 expression were assessed. PD-L1 expression was observed in 31% of acral melanomas, 44% of mucosal melanomas, 10% of uveal melanomas, and 62% of CSD melanomas (P<0.0001). Compared to our previously characterized cohort of cutaneous melanomas, the proportion of PD-L1(+) tumors was lower in uveal (P=0.0002) and higher in CSD (P=0.0073) melanomas, while PD-L1 expression in the acral and mucosal subtypes was on par. PD-L1 expression in all subtypes correlated with a moderate-severe grade of CD8+ TIL (all, P<0.003), supporting an adaptive mechanism of expression induced during the host antitumor response. The tumor microenvironments observed in CSD melanomas segregated by whether they were the pure desmoplastic subtype, which showed lower levels of PD-L1 expression when compared to other CSD melanomas (P=0.047). The presence of lymphoid aggregates was not associated with the level of PD-L1 expression, while PD-L1(+) cases with spindle cell morphology demonstrated higher levels of PD-L1 than those with a nested phenotype (P<0.0001). Our findings may underpin the reported clinical response rates for anti-PD-1 monotherapy, which vary by subtype.

Embarrassment displays show others that one is aware of one's own misbehavior and willing to make up for it. The facial actions of embarrassment, however, are partly similar to those of disinterest, which has an opposite function, signaling that one is not concerned about one's self in relation to others. In the context of negative intergroup relations, embarrassment displays of outgroup members may therefore be misinterpreted as disinterest. In the present research, the authors predicted that Whites would perceive Arab expressions of embarrassment more as disinterest, but embarrassment displays of Whites more as embarrassment. Aggregated Study 1 (N = 1,154) confirms this hypothesis showing that White participants perceived more intense embarrassment in Whites than in Arabs and more intense disinterest in Arabs than in Whites. Studies 2 (n = 193) and 3 (n = 260) include methodological improvements and either largely or fully replicated our findings. Based on this evidence in an Arab-White context, the authors conclude that the affiliative function of embarrassment perception is dependent on the nature of the group context. Finally, they discuss the generalizability of this intergroup emotion bias in which emotional expressions may be perceived as the opposite of what they are intended to display.
