The right not to know is underappreciated in policy-making. Despite its articulation in medical law and ethics, policy-makers too easily let other concerns override the right not to know. This observation is triggered by a recent decision of the Dutch government on embryo selection for Huntington's disease. This is a monogenetic debilitating disease without cure, leading to death in early middle age, and thus is a likely candidate for preimplantation genetic diagnosis (PGD). People possibly affected with the Huntington gene do not always want to know their genetic status themselves, which is very burdensome knowledge. For prospective parents, this creates a difficulty in having unaffected children through embryo selection without knowing their own genetic status. A solution is provided by the exclusion test: it allows embryo selection while honouring the parents' right not to know. The Dutch government however disallowed the exclusion test and chose to allow PGD on Huntington only for parents who test themselves first. To avoid 'unnecessary' in-vitro fertilisation procedures for unaffected parents, prospective parents are 'forced to know' before they can conceive through embryo selection. This article analyses the scope of the right not to know in the context of embryo selection against Huntington's disease. The human zona pellucida is a structure consisting of carbohydrate and protein that surrounds the unfertilized egg and embryo. This structure enhances the binding of a single sperm to the egg and prevents attachment of additional sperm. It may also serve as a physical barrier to protect the egg and embryo from immunological, physical, or biochemical damage. The role of the zona pellucida has become more important since the development of artificial fertilization techniques. The zona pellucida must be penetrated to allow the sperm to come into contact with the egg. Holes that have been made in the zona pellucida of fertilized eggs for purposes of genetic analysis have not prevented live births. It was suggested that a hole in the zona pellucida may help the embryo to shed its zona pellucida structure when it implants in the uterus during in vitro fertilization. In vitro fertilization is the fertilization of the egg outside of the female genital tract, usually in a test tube. This is then followed by placement of the fertilized egg back into the uterus. The effect of partially dissecting the zona pellucida of fertilized eggs on the success of implantation in the uterus was assessed. Of 65 attempts at implantation, 32 involved partial dissection of the zona pellucida (also referred to as assisted hatching). Implantation of the embryo into the uterus was successful in 16 percent of embryos treated by assisted hatching and in 9 percent of untreated embryos. Larger study groups are needed to confirm the benefits of assisted hatching in improving implantation rates for in vitro fertilization. In vitro fertilization is a method in which the ovaries are stimulated to produce eggs, which are then retrieved from the ovaries, fertilized outside of the body and reintroduced into the uterus to achieve pregnancy. Originally, eggs were retrieved by inserting a small tube called a laparoscope into the abdomen while the patient was under anesthesia. Ultrasound directed follicle aspiration (UDFA), a method of retrieving eggs using the guidance supplied by high frequency sound to visualize the ovarian egg follicles, is now widely used since it does not require anesthesia and can be performed without an overnight hospital stay. This ultrasound method can image the ovaries via the bladder (transabdominal) or the vagina (transvaginal). A variation of the abdominal approach, which entails inserting a needle to aspirate the eggs from the ovaries by entering the bladder through the abdominal wall, approaches the bladder via the urethra, the tube leading from the bladder. Follicles are emptied, the needle withdrawn and the guiding catheter is removed. The women can leave the hospital within two hours. The experience with the perurethral technique of UDFA performed between 1984 and 1988 on 545 women having 636 in vitro fertilization cycles is reported. The average follicle aspiration rate was 93 percent and the average number of eggs retrieved was around six. Small follicles and difficult access were the main reasons for failure to retrieve an egg. Minor complications were experienced by 34 women (six percent), and included blood in the urine (hematuria), escape of urine and puncture of nearby blood vessels. In 91 patients (14 percent) another route was needed to retrieve the eggs because it was difficult to reach all the follicles. The transvaginal route and the perurethral route have similar rates of retrieval success. The transvaginal route for egg retrieval is more advantageous because it uses transvaginal ultrasound, an improved ultrasound technique in which sound transduced through a probe inserted into the vagina produces a clearer image of the ovarian follicles. However, the perurethral route will be useful where practitioners do not yet have transvaginal ultrasound equipment or is cases where ovaries are not accessible using the vaginal route or in women with current vaginal infections. In vitro fertilization (IVF) involves several procedures. The woman receives a course of drugs to control egg production. Drugs may include human menopausal gonadotrophin and urofollitrophins. Fertility specialists collect the eggs using a vaginally-inserted needle. They inseminate the eggs with sperm, repeating the procedure if fertilization does not occur, and transfer the embryos to the uterus between 47 and 72 hours after insemination. IVF may involve gamete or sperm donation. Other techniques related to IVF include intracytoplasmic sperm injection and gamete intra-fallopian transfer.
