In this study, we used whole-exome sequencing of a cohort of 45 advanced-stage, treatment-naïve Nigerian (NG) primary prostate cancer tumors and 11 unmatched nontumor tissues to compare genomic mutations with African American (AA) and European American (EA) The Cancer Genome Atlas (TCGA) prostate cancer. NG samples were collected from six sites in central and southwest Nigeria. After whole-exome sequencing, samples were processed using GATK best practices. BRCA1 (100%), BARD1 (45%), BRCA2 (27%), and PMS2(18%) had germline alterations in at least two NG nontumor samples. Across 111 germline variants, the AA cohort reflected a pattern [BRCA1 (68%), BARD1 (34%), BRCA2 (28%), and PMS2 (16%)] similar to NG samples. Of the most frequently mutated genes, BRCA1 showed a statistically (P ≤ 0.05) higher germline mutation frequency in men of African ancestry (MAA) and increasing variant frequency with increased African ancestry. Disaggregating gene-level mutation frequencies by variants revealed both ancestry-linked and NG-specific germline variant patterns. Driven by rs799917 (T>C), BRCA1 showed an increasing mutation frequency as African ancestry increased. BRCA2_rs11571831 was present only in MAA, and BRCA2_rs766173 was elevated in NG men. A total of 133 somatic variants were present in 26 prostate cancer–associated genes within the NG tumor cohort. BRCA2 (27%), APC (20%), ATM (20%), BRCA1 (13%), DNAJC6 (13%), EGFR (13%), MAD1L1 (13%), MLH1 (11%), and PMS2 (11%) showed mutation frequencies >10%. Compared with TCGA cohorts, NG tumors showed statistically significant elevated frequencies of BRCA2, APC, and BRCA1. The NG cohort variant pattern shared similarities (cosign similarities ≥0.734) with Catalogue of Somatic Mutations in Cancer signatures 5 and 6, and mutated genes showed significant (q < 0.001) gene ontology (GO) and functional enrichment in mismatch repair and non-homologous repair deficiency pathways. Here, we showed that mutations in DNA damage response genes were higher in NG prostate cancer samples and that a portion of those mutations correlate with African ancestry. Moreover, we identified variants of unknown significance that may contribute to population-specific routes of tumorigenesis and treatment. These results present the most comprehensive characterization of the NG prostate cancer exome to date and highlight the need to increase diversity of study populations. Significance: MAA have higher rates of prostate cancer incidence and mortality, however, are severely underrepresented in genomic studies. This is the first study utilizing whole-exome sequencing in NG men to identify West African ancestry-linked variant patterns that impact DNA damage repair pathways.

Background There is an urgent need to elucidate the epidemiology of the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV2) and characterize its potential impact. Investing in characterising the SARS-CoV2 will help plan and improve the response to the pandemic. Furthermore, it will help identify the most efficient ways of managing the pandemic, avoiding public health policies and interventions that may be unduly restrictive of normal activity or unnecessarily costly. This paper describes the design and reports findings of a population based epidemiological study undertaken to characterise SARS-CoV2 in Qatar using limited resources in a timely manner. Methods Asymptomatic individuals ≥10 years registered with Qatar’s publicly funded primary health provider were eligible. A stratified random sampling technique was utilized to identify the study sample. Participants were invited to an appointment where they completed a questionnaire and provided samples for polymerase chain reaction and Immunoglobulin M and G immunoassay tests. Data collected were analyzed to calculate point and period prevalence by sociodemographic, lifestyle and clinical characteristics. Results Of 18,918 individuals invited for the study, 2084 participated (response rate 10.8%). The overall point prevalence and period prevalence were estimated to be 1.6% (95% CI 1.1–2.2) and 14.6% (95% CI 13.1–16.2) respectively. Period prevalence of SARS-CoV2 infection was not considerably different across age groups (9.7–19.8%). It was higher in males compared to females (16.2 and 12.7% respectively). A significant variation was observed by nationality (7.1 to 22.2%) and municipalities (6.9–35.3%). Conclusions The study provides an example of a methodologically robust approach that can be undertaken in a timely manner with limited resources. It reports much-needed epidemiological data about the spread of SARS-CoV2. Given the low prevalence rates, majority of the population in Qatar remains susceptible. Enhanced surveillance must continue to be in place, particularly due to the large number of asymptomatic cases observed. Background: Prostate cancer (PCa) is the leading cause of cancer death among Nigerian men, with most men presenting late and with high grade disease. Furthermore, improved PCa diagnosis and treatment could rely on genomics-driven precision approaches. Mounting evidence supports potential population enriched molecular features of cancer, however the extent of which is hampered due to a dearth of genomic data in diverse cohorts. This study, therefore, aims at utilizing transcriptomics analysis to deduce transcriptional states in PCa from Nigerian men. Methodology: RNA was extracted from macro-dissected prostate tumor and adjacent normal tissue from 35 archival formalin fixed paraffin-embedded (FFPE) blocks from Gleason 8-9 cancers, of which 25 had cribriform morphologic architecture. High resolution hybrid-capture mRNA-seq libraries were sequenced using Illumina NovaSeq 6000 system and generated an average of 124 million read pairs per sample. Raw reads were processed using a custom pipeline based upon industry standard algorithms along with the deduction of genetic ancestry. Molecular annotation was also performed to investigate tumor and immune cell heterogeneity. Result: The Nigerian data were compared with high grade PCa data of Caucasian obtained from TCGA. The Nigerian PCa tumors where characterized with uniquely enriched immune-inflammatory microenvironment which include crosstalk between dendritic cells and natural killer cells, and natural killer cell signaling. While the Nigerian tumors share more features of distal metastatic tumor, their enrichment in NKT Cells is a unique feature. Conclusion: This represents the first report of RNA-seq analysis of PCa in Sub-Saharan Africa and the unique transcriptomics features of Nigerian PCa presents opportunities for precision intervention in addressing its burden in Nigeria. The African continent is facing enormous challenges from cancer crisis, and the lack of reliable population database on cancer incidence and mortality hinders high quality decision on prevention and management modalities. The objective of this study is to analyze the trend of malignant tumors in a population of rural Africans attending Ahmadu Bello University Teaching (ABUTH) Hospital Zaria, Nigeria with particular emphasis on the first eighteen (18) frequently diagnosed cancers over a period of 46 consecutive years (1972 - 2019) excluding the year 1988. Histologically diagnosed cancers from the population-based cancer registry of the ABUTH Zaria were included in this study. The results show unusual change in trend with: 2 fold increase for cervical cancer, 6 fold increase for Breast cancer, 8 fold increase for Prostate cancer, 3 fold increase for colorectal cancer; 6 fold increase for Pancreatic cancer, 13 fold increase for Nasopharyngeal cancer and 16 fold increase for Oesophageal cancer when comparing incidence of cancer from 2011-2019 with 1972-1980. However, the results show 12 fold decrease for Liver cancer and 3 fold decrease each for Lymphoma and Non-melanoma skin cancer when comparing cancer cases from 2011-2019 with 1972 - 1980. Understanding the phenomenon behind the rapid change in trend of different cancer types in sub Saharan Africa may contribute to addressing cancer challenges and better policy decision on cancer in the continent. Colorectal cancer incidence rates are rising among individuals age younger than 50 years (early-onset CRC) globally with causes unknown. Racial and ethnic disparities in early-onset CRC have also grown more pronounced, as blacks have higher early- onset CRC incidence and poorer survival compared with whites. We described the prevalence and burden of early-onset CRC among Africans in Nigeria and African Americans in the United States. We identified black individuals diagnosed with a first primary colorectal cancer ages 18 to 49 years between 1989 and 2017 at Ahmadu Bello University Teaching Hospital in Zaria, Nigeria using the Zaria Cancer Registry (Nigerians), and from population-based cancer registries across the US (African Americans) using the NIH/NCI’s Surveillance, Epidemiology, and End Results (SEER) program. Multivariable logistic regression models were used to investigate clinicopathologic and demographic differences between Nigerians and African Americans with early-onset CRC adjusted for age, sex, tumor site and grade. A total 566 and 31,284 colorectal adenocarcinoma cases were diagnosed among Nigerians and African Americans, respectively, in hospitals and clinics over the 28-year study period. More than 60% of Nigerian patients (n=354) were diagnosed with early-onset CRC. This is largely due to the lack of a large offline gold-standard benchmark of relevant documents that cover a variety of research fields such that newly developed literature search techniques can be compared, improved and translated into practice. To overcome this bottleneck, we have established the RElevant LIterature SearcH consortium consisting of more than 1500 scientists from 84 countries, who have collectively annotated the relevance of over 180 000 PubMed-listed articles with regard to their respective seed (input) article/s. The majority of annotations were contributed by highly experienced, original authors of the seed articles. The collected data cover 76% of all unique PubMed Medical Subject Headings descriptors. No systematic biases were observed across different experience levels, research fields or time spent on annotations. More importantly, annotations of the same document pairs contributed by different scientists were highly concordant. We further show that the three representative baseline methods used to generate recommended articles for evaluation (Okapi Best Matching 25, Term Frequency-Inverse Document Frequency and PubMed Related Articles) had similar overall performances. Additionally, we found that these methods each tend to produce distinct collections of recommended articles, suggesting that a hybrid method may be required to completely capture all relevant articles. The established database server located at https://relishdb.ict.griffith.edu.au is freely available for the downloading of annotation data and the blind testing of new methods. We expect that this benchmark will be useful for stimulating the development of new powerful techniques for title and title/abstract-based search engines for relevant articles in biomedical research. Colorectal cancer (CRC) has disproportionately overburden Africans with advanced stage at diagnosis and higher mortality rate due in part to issues that include grossly inadequate resources for management and poor understanding of the pathobiology of the disease. Accumulating evidences support the effectiveness of Aspirin in prevention and inhibition of CRC occurrence and cellular growth respectively. The utilisation of 5-fluorouracil (5-FU)-based regimens which include folinic acid, 5-FU and oxaliplatin (FOLFOX) combination in CRC management have not completely halt the distant recurrences and poor prognosis of CRC. In this study, we highlighted evidences that prove Aspirin plus FOLFOX to be promising alternative for clinical trial in Black men with CRC. Our study aims in this research were: to identify the expression pattern of BIRC7 in human CRC cells, SW480 cell line and N-methyl-N-nitrosourea (NMU)-induced CRC in rat; to determine the relationship between BIRC7 and BCL2, p53, PDL-1, MSH1, PMS2, DARC and Annexin V expression in CRC. to examine the efficacy of Aspirin plus neoadjuvant FOLFOX on the BIRC7 in CRC cells in-vivo in rat. Results show significant (P = 0.0001) expression of the BIRC7 in the CRC tissues compared to normal sections. When comparing this expression with Data from TCGA data bank, we observed a significant increase in BIRC7 expression in Caucasian than the Black and Indian and The expression pattern of the BIRC7 was significantly higher (P = 0.0001) in CRC not otherwise specified compared to mucinous CRC histology. The SW480 cell line show increased expression of the BIRC7 protein in-vitro. There was significant expression of the BIRC7 in the NMU-induced CRC cells compared the normal colonic lesion. There was increased expression of the BCL-2 and reduced expression of p53 in the human CRC cases and in the NMU-induced CRC cells model. There was increased expression of the Annexin V, absence of the MSH1, PMS2, PDL1 and DARC protiens in the CRC sections. Aspirin plus FOLFOX may serve as an important adjuvant treatment component for colorectal cancer metastasis and local recurrence in Blacks, Caucasian and Asians. There is need to explore the potential of Aspirin plus FOLFOX for clinical trial in colorectal cancer management in Blacks and other population using approach from our findings. Prostate cancer (CaP) is the most common male gender cancer and present with a 5-year prevalence, incidence and mortality rate in Nigerian Black men. There is disproportionate prevalence and poor understanding of CaP in Black men globally. The Prostate Cancer Transatlantic Consortium (CaPTC) has documented significant CaP burden among Nigerian black men in Nigeria and in the Diaspora. Notably, African ancestry is one of the most important risk factors of CaP globally. Increased levels of cortisol, a stress-related catabolic hormone is significantly associated to prostate specific antigen and poor prognosis in prostate cancer patients. Cortisol causes degradation of muscle proteins into amino-acids. Amino acids such as leucine, serves as a source of fuel for aggressive prostate cancer cells in a Warburg effect model through the L-amino acid transporters (LATs). The annexins are calcium and phospholipid binding proteins implicated in cancer development and progression. Overexpression of annexin 1 is one of the mechanisms by which cortisol inhibit inflammation. Annexin V has a high affinity to phosphatidylserine and play important role of inhibiting engulfment of apoptotic cells by macrophages to increase the immunogenicity of tumor cells undergoing apoptosis. PD-L1 is an important membrane-bound costimulatory molecule that inhibits immune responses through its receptor, PD-1. Overexpression of PD-L1 results to cancer cells aggressively evading the host immune system. The Duffy Antigen/Receptor for Chemokines (DARC) is a 7-transmembrane protein atypical chemokine receptor expressed on erythrocytes and vascular endothelial cells that binds to and clears angiogenic chemokines. Expression pattern of DARC is associated to cancer growth and metastasis. In this study, we report the expression pattern of tissue Cortisol, Annexin V, PD-L1 and DARC proteins in relationship to Gleason score in Nigerian Black men with Prostate cancer using immunohistochemistry and immunofluorescence. In addition, we assessed salivary cortisol levels by enzyme immunoassay and physical activity by a standardized CaPTC-AC3-MADCaP (C.A.M) CaP measures from 500 subjects recruited by via CaPTC cohort. Results showed significant expression of tissue and salivary cortisol in the CaP patients compared to the normal subjects (p > 0.05). The Tissue cortisol protein expression was higher in CaP cases with Gleason score 8. There was significant overexpression of Annexin protein in virtually the prostate cancer samples studied. We found a negative expression of PD-L1 and DARC proteins in all the CaP tissues studied. Cortisol and Annexin may serve as an important biomarker for prostate cancer diagnosis and prognosis in Nigerian Blacks. Citation Format: Faruk Mohammed, Folakemi T. Introduction Nigeria is Africa's most populous country and the 9th most populous country in the world. The country is the 6th largest producer of petroleum in the world. Health care and general living conditions in Nigeria are poor, especially for children and women. At least 70 to 75 percent of health expenditure comes from out of pocket expenses. Childhood cancers as a growing public health challenge and is increasingly being recognized worldwide, including the developing nations. Tremendous progress has been made in the treatment and cure of childhood cancers mostly in the developed world, most cases die of childhood cancers in our society. We look at fifty two cases of small round blue cell tumour of childhood (SRBCT) a diverse group of childhood cancers that have considerable overlap in epidemiology, morphology and immunophenotype over a three year period. Methods We reviewed the histology report and paraffin blocks of patients diagnosed as SRBCT of childhood in the department of pathology, from 2014-2017. We identified fifty two patients diagnosed as SRBCT during the study period. The information extracted includes age, sex, cancer type, treatment modality and outcome of all patients. We subjected the blocks to nine immunohistochemical stains to rule out differentials using Genemed biotechnology protocol. The stains include Cytokeratin, EMA, S-100, CD99, Desmin, CD10, CD20, BCL2 and Synaptophysin.. Results The age range in this study was 2-15 years with a mean of 7.2 years. The most common site for SRBCT was the head and neck region accounting for more than 46%. This is followed by the abdomen accounting for 19%. The commonest SRBCT after immunohistochemical stains was Burkitt's lymphoma 16(30.7%), retinoblastoma 8(15.4%), alveolar rhabdomyosarcoma 7(13.5%), Follicular lymphoma 5(9.6%), ES/PNET 5(9.6%), nephroblastoma 4(7.7%), neuroblastoma 4(7.7%) and synovial sarcoma 3(5.8%). All patients 52(100%) undergo chemotheraphy, 36(69%) undergo both surgery and chemotheraphy while 8(15.4%) sign against medical advice. All patients pay through out of pocket expences with no access to insurance. Thirty three patients (63%) died during chemotheraphy. Most patients are lost to follow up due to financial burden and unavailability of standard oncology services. Conclusion While tremendous progress has been made in the treatment of childhood cancers in the developed world, it still remains an important health problem in our societies largely because of multifactorial reasons creating a huge health disparity and contributing to under five mortality in our environment.
