Purpose In light of the international evolutions to establish inclusive trauma systems and to concentrate the care for the most severely injured in major trauma centres, we evaluated the degree of dispersion of trauma care in Belgium. Methods We used descriptive statistics to illustrate the dispersion of major trauma care in Belgium based on two independent administrative databases: the registry of Mobile Intensive Care Units (2009–2015) and the Belgian Hospital Discharge Dataset (2009–2014). Results Patients with a severe trauma (n = 3856 in 2015) were transported towards 145 different hospital sites (on a total of 198 hospital sites) resulting in a median of 17 cases per hospital site (min = 1; P25 = 4; P75 = 30; max = 165). A minority of major trauma patients is after admission transferred to another hospital (8%) with a median of 10 days after admission to the hospital (IQR 3.5–24). Conclusions The dispersion of care for major trauma patients in Belgium is so high that a reorganisation of care for severe injured patients in major trauma centres concentrating professional expertise

39 p., ill., De geestelijke gezondheidszorg van kinderen en volwassenen is in afzonderlijke systemen georganiseerd, maar hoe staat het met de ouderen? Hoort hun mentale zorg ook in een afzonderlijk netwerk, of kan deze worden geïntegreerd in het bestaande systeem voor ‘volwassenen’? Volgens het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) mag de leeftijd niet bepalend zijn om iemand als ‘oudere’ te beschouwen. De ouderen vormen immers een zeer heterogene bevolkingsgroep, met mensen die nog in goede vorm zijn en met andere, zeer kwetsbare personen. Daarnaast stelt het KCE dat de huisarts een zeer belangrijke rol moet blijven spelen bij de behandeling van lichamelijke en psychische problemen. Voor sommige psychische problemen is er echter een specifieke kennis vereist. Daarom pleit het KCE voor een verruiming van de geestelijke gezondheidszorg van ‘volwassenen’ naar de ouderen. Dans de tels modèles, les blessés graves sont centralisés dans quelques hôpitaux stratégiquement répartis sur l’ensemble du territoire. Ces « Centres majeurs de traumatologie » répondent à des exigences extrêmement élevées en termes d’équipement, de qualification du personnel et de disponibilité (24h/24 et 7j/7). Le Centre fédéral d’Expertise des Soins de Santé (KCE) a étudié ces exemples étrangers et analysé de quelle manière ils pourraient être transposés à la réalité belge. 38 p., ill., Vandaag is er een internationale tendens om de zorg voor zwaar gewonde patiënten te organiseren in ‘geïntegreerde systemen’. In deze systemen wordt de zorg voor de slachtoffers gecentraliseerd in een aantal ziekenhuizen die strategisch verspreid zijn over het gehele land. BACKGROUND: Informal caregivers are essential figures for maintaining frail elderly at home. Providing informal care can affect the informal caregivers' physical and psychological health and labour market participation capabilities. They need support to prevent caregiver burden. A variety of existing support measures can help the caregiver care for the elderly at home, but with some limitations. The objective of this review was to explore the experiences of informal caregivers caring for elderly in the community with the use of supportive policy measures in Belgium and compare these to the experiences in other European countries. METHODS: An empirical qualitative case study research was conducted in five European countries (Belgium, The Netherlands, Luxembourg, France and Germany). Semi-structured interviews were conducted with informal caregivers and their dependent elderly. Interview data from the different cases were analysed. In particular data from Belgium was compared to data from the cases abroad. RESULTS: Formal services (e.g. home care) were reported to have the largest impact on allowing the caregiver to care for the dependent elderly at home. One of the key issues in Belgium is the lack of timely access to reliable information about formal and informal services in order to proactively support the informal caregiver. Compared to the other countries, informal caregivers in Belgium expressed more difficulties in accessing support measures and navigating through the health system. In the other countries information seemed to be given more timely when home care was provided via care packages. 24 p., ill., L’hospitalisation à domicile (HAD) consiste à délivrer à domicile des soins spécialisés qui ne sont normalement délivrés qu’à l’hôpital. C’est un concept qui s’inscrit dans la tendance actuelle à raccourcir les durées de séjour à l’hôpital, voire à éviter l’hospitalisation, mais qui répond aussi aux préférences de beaucoup de patients qui souhaitent recevoir des soins, même complexes, dans leur environnement familier. Il existe déjà quelques initiatives en Belgique, mais elles sont isolées et sans règles communes de fonctionnement. 22 p., ill., Bij thuishospitalisatie (TH) wordt bij de patiënt thuis zorg verleend, die normaal enkel in het ziekenhuis wordt aangeboden. TH is een concept dat aansluit bij de huidige tendens om het ziekenhuisverblijf in te korten, of zelfs te vermijden. Anderzijds beantwoordt het ook aan de wens van vele patiënten om in hun vertrouwde omgeving verzorgd te worden, zelfs al is die zorg complex. Er bestaan al een aantal initiatieven in België, maar ze zijn geïsoleerd en hebben geen gemeenschappelijke werkingsregels. Er is dus nood aan een duidelijk referentiekader, waarbij wordt bepaald wie er wat doet en wie ervoor verantwoordelijk is. 37 p., ill., Familieleden, buren en vrienden die de zorg opnemen voor een oudere of zieke zijn zeer belangrijk in ons gezondheidszorgsysteem, en zij dragen een groot stuk van alle zorg die wordt geleverd. Ons sociaal en gezondheidssysteem zou dan ook veel beter voor deze mantelzorgers zelf moeten zorgen. Het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) bekeek de ondersteuning die de overheid vandaag biedt aan mantelzorgers, in samenwerking met UA, UCL en Yellow Window. Er zijn weinig financiële vergoedingen voor hen. De mogelijkheden om sociaal verlof te nemen om zorg te verlenen worden dan weer wel als positief ervaren. Respijtzorg moet minder duur zijn, meer aangepast aan de behoeften en, net zoals psychosociale ondersteuning, proactief worden aangeboden. De bestaande diensten en ondersteuning moeten op elkaar afgestemd worden en informatie erover is best beschikbaar via één enkel informatiekanaal.
