2. Real-life use of anticoagulants in venous thromboembolism with focus on patients with exclusion criteria for direct oral anticoagulants.

- Abstract
We assessed the real-life use of direct oral anticoagulants (DOACs) in patients with venous thromboembolism (VTE) and exclusion criteria for randomized trials. From 2013 to 2016, 3,578 of 18,853 patients (19%) had exclusion criteria. Irrespective of which anticoagulant was chosen, they had more VTE recurrences (hazard ratio (HR): 3.10; 95% confidence interval (CI): 2.47-3.88), major bleeds (HR: 4.10; 95% CI: 3.38-4.96), and deaths (HR: 9.47; 95% CI: 8.46-10.6) than those without exclusion criteria. During initial therapy, no patient with exclusion criteria on DOACs (n = 115) recurred, but those on rivaroxaban bled less often (adjusted HR: 0.18; 95% CI: 0.04-0.79) than those on unfractionated heparin (n = 224) and similar to those (n = 3,172) on low-molecular-weight (LMWH) heparin. For long-term therapy, patients on rivaroxaban (n = 151) had nonsignificantly fewer VTE recurrences (adjusted HR: 0.74; 95% CI: 0.08-1.32) and major bleeds (adjusted HR: 0.41; 95% CI: 0.15-1.15) than those on LMWH (n = 2,071). The efficacy and safety of DOACs were similar to standard therapy.© 2017 American Society for Clinical Pharmacology and Therapeutics.

In the Part 1 paper (Gawin, et al. 2005) some experimental results concerning micro-structuralrntests, permeability measurements and stress-strain tests of four types of High Performance Concrete, exposed to elevated temperatures (up to 700oC) are presented and discussed. On the basis of these experimental results parameters of the constitutive relationships describing influence of damage and temperature upon material intrinsic permeability at high temperature were determined. In this paper the effects of various formulations of damage-permeability coupling on results of computer simulations are analysed and compared with the results obtained by means of the previously proposed approach, that does not take into account thernthermo-chemical concrete damage directly. Numerical solutions are obtained using the recently developedrnfully coupled model of hygro-thermal and damage phenomena in concrete at elevated temperatures. Highrntemperature effects are considered by means of temperature and pressure dependence of several material parameters. Based on the mathematical model, the computer code HITECOSP was developed. Materialrnparameters of the model were measured by several European laboratories, which participated in thern?ITECO?research project. A model problem, concerning hygro-thermal behaviour and degradation of a HPC structure during fire, is solved. The influence of two different constitutive descriptions of the concrete permeability changes at high temperature, including thermo-chemical and mechanical damage effects, upon the results of computer simulations is analysed and discussed.

The aim of this paper was to assess quality of life, possible presence of depressive symptomatology and their possible relationship during normal and high-risk pregnancy.A total of 100 women (50 experiencing a normal pregnancy and 50 experiencing a high-risk pregnancy) were studied. They completed the World Health Organization Quality of Life (WHOQOL) to assess the quality of life and the Beck Depression Inventory (BDI) to assess the level of depressive symptomatology. The mean of the scores obtained in both the entire sample and the 2 subgroups was calculated, and then the results obtained were compared.The study shows that the women with normal pregnancy have a good perception of their quality of life, instead women with high-risk pregnancy do not think so. They consider it more unfavourable then the others. It also emerged, that the 12% of our sample (all women with high-risk pregnancy) shows a significant level of depressive symptomatology.It is very important to identify early women at risk of depressive symptoms or mental disorders, so that it is possible to offer an adequate psychological support during pregnancy, to prevent the worsening of the symptoms or the development of a real mental disorder in the postnatal period.

This paper presents an analysis of some experimental results concerning micro-structural tests, permeability measurements and strain-stress tests of four types of High-Performance Concrete, exposed to elevated temperatures (up to 700∘C). These experimental results, obtained within the "HITECO" research programme are discussed and interpreted in the context of a recently developed mathematical model of hygro-thermal behaviour and degradation of concrete at high temperature, which is briefly presented in the Part 2 paper (Gawin, et al. 2005). Correlations between concrete permeability and porosity micro-structure, as well as between damage and cracks' volume, are found. An approximate decomposition of the thermally induced material damage into two parts, a chemical one related to cement dehydration process, and a thermal one due to micro-cracks' development caused by thermal strains at micro- and meso-scale, is performed. Constitutive relationships describing influence of temperature and material damage upon its intrinsic permeability at high temperature for 4 types of HPC are deduced. In the Part II of this paper (Gawin, et al. 2005) effect of two different damage-permeability coupling formulations on the results of computer simulations concerning hygro-thermo-mechanical performance of concrete wall during standard fire, is numerically analysed.

The Schottky barrier heights of scandium deposited on n- and p-type silicon have been measured by the capacitance versus voltage technique. Respective values of about 0.4 and 0.7 eV were determined; they do not change upon the formation of the monosilicide, ScSi, after annealing at 500°C. These results are similar to those obtained with yttrium and the rare-earth metals, in agreement with the respective positions of these elements in the Periodic Table: same column, but fourth, fifth and sixth periods, respectively.

Some rare‐earth elements, Tb, Er, Yb (including Y) and some of their respective silicides were found to make ohmic contacts to n‐type silicon. Forward I/V and photoresponse measurements give values of about 0.7 eV for the Schottky‐barrier height to p‐type silicon. The sum of this value and of the experimentally estimated barrier height to n‐type silicon, 0.4 eV, corresponds to the band gap of silicon.

Chronic obstructive pulmonary disease (COPD) increases the risk of mortality in non-valvular atrial fibrillation (NVAF) patients. Data on the relationship of COPD to major cardiovascular events (MACE) in AF have not been defined. The aim of the study is to assess the predictive value of COPD on incident MACE in NVAF patients over a 3-year follow-up. In the Atrial Fibrillation Registry for Ankle-Brachial Index Prevalence Assessment-Collaborative Italian Study (ARAPACIS) cohort, we evaluate the impact of COPD on the following clinical endpoints: MACE (including vascular death, fatal/non-fatal MI and stroke/TIA), cardiovascular (CV) death and all-cause mortality. Among 2027 NVAF patients, patients with COPD (9%) are more commonly male, elderly and at higher thromboembolic risk. During a median 36.0months follow-up, 186 patients experienced MACE: vascular death (n = 72), MI (n = 57), stroke/TIA (n = 57). All major outcomes (including stroke/TIA, MI, vascular death, and all-cause death) are centrally adjudicated. Kaplan–Meier curves show that NVAF patients with COPD are at higher risk for MACE (p < 0.001), CV death (p < 0.001) and all-cause death (p < 0.001). On Cox proportional hazard analysis, COPD is an independent predictor of MACE (Hazard ratio [HR] 1.77, 95% Confidence Intervals [CI] 1.20–2.61; p = 0.004), CV death (HR 2.73, 95% CI 1.76–4.23; p < 0.0001) and all-cause death (HR 2.16, 95% CI 1.48–3.16; p < 0.0001). COPD is an independent predictor of MACE, CV death and all-cause death during a long-term follow-up of NVAF patients.

OBJECTIVE: Carotid intima-media thickness (cIMT) is a surrogate marker of subclinical atherosclerosis and it is able to predict both coronary and cerebral vascular events. No data exist on the association between cIMT and non valvular atrial fibrillation (NVAF) type. We conduct this study with the aim to analyze the association between abnormal cIMT and NVAF type. METHODS: A cross-sectional study of the "Atrial fibrillation Registry for Ankle-brachial index Prevalence Assessment-Collaborative Italian Study (ARAPACIS)" has been performed. Among 2027 patients enrolled in the ARAPACIS, 673 patients, who underwent carotid ultrasound examination to assess cIMT, were included in the study. RESULTS: Among the entire population, 478 patients (71%) had cIMT > 0.90 mm. Patients with an abnormal cIMT (>0.90 mm) were significantly older and more likely hypertensive, diabetic and with a previous history of stroke than those with normal cIMT (≤0.90 mm). These patients had more permanent/persistent NVAF and CHA2DS2-VASc score ≥ 2 (p < 0.0001) compared to those with cIMT

Many problems are linked with the long-term behavior of both earthdams and concrete dams. They range from hydraulic fracturing to alkali-silica reaction (ASR) and to repair work in concrete dams, from seismic behavior to secondary consolidation in earthdams. A common framework for the simulation of such systems is shown, based on the mechanics of multiphase porous media. The general model is particularized to specific situations and several examples are shown.

14. Acupuncture in the treatment of Dry Eye Syndrome with anxiety symptoms. A case report.

- Abstract
Dry Eye Syndrome (DES) is an eye disorder caused by alteration of production or composition of tear film. Symptoms include dry eye dryness, burning, itching and pain, and often is associated with emotional stress, anxiety and depression. Evidence of the effectiveness of acupuncture to treat DES has recently appeared in literature but not on acupuncture treatment of DES related anxiety or depression symptoms.A 53-year-old male, good health, no serious medical conditions, with history of contact lenses wearing, anxious personality traits and anxiety symptoms, suffering from DES, was treated with acupuncture, twice a week for three weeks with somatic and ear acupoints selected from literature.Inferior marginal lacrimal meniscus, lipid layer interferometry, tear ferning test, psychiatric examination, State-Trait Inventory -Y (STAI-Y) questionnaire and Hamilton-Anxiety Rating Scale (HAM-A) scale was evaluated before and after treatment and at 15 , 30 , 60, and 90 days follow-up.Results show clear and lasting improvements with remission of eye symptoms, together with clear and stable improvements of anxiety symptoms.Acupuncture is an effective treatment also for DES related anxiety symptoms.

- Abstract
Aim: The aim of this paper was to assess quality of life, possible presence of depressive symptomatology and their possible relationship during normal and high-risk pregnancy., Methods: A total of 100 women (50 experiencing a normal pregnancy and 50 experiencing a high-risk pregnancy) were studied. They completed the World Health Organization Quality of Life (WHOQOL) to assess the quality of life and the Beck Depression Inventory (BDI) to assess the level of depressive symptomatology. The mean of the scores obtained in both the entire sample and the 2 subgroups was calculated, and then the results obtained were compared., Results: The study shows that the women with normal pregnancy have a good perception of their quality of life, instead women with high-risk pregnancy do not think so. They consider it more unfavourable then the others. It also emerged, that the 12% of our sample (all women with high-risk pregnancy) shows a significant level of depressive symptomatology., Conclusions: It is very important to identify early women at risk of depressive symptoms or mental disorders, so that it is possible to offer an adequate psychological support during pregnancy, to prevent the worsening of the symptoms or the development of a real mental disorder in the postnatal period.
