Da Ruckenkrankheiten zu den haufigsten chronischen Leiden in der Bevolkerung zahlen, sind Patienten mit Ruckenschmerzen in jeder Praxis auserordentlich haufig. Fur den Arzt besteht die Hauptschwierigkeit darin, das sich hinter dem uniformen subjektiven Symptom „Ruckenschmerzen“ zahlreiche sehr differente Krankheitsbilder verstecken konnen. Das breite Gesamtspektrum der Erkrankungen der Wirbelsaule und ihrer Grenzgelenke ist aus Ubersicht 7-1 ersichtlich. In der Praxis bewahrt es sich, fur die Differentialdiagnose eine einfache Checkliste durchzugehen (Ubersicht 7–2).

A 51-year old patient with congenital basal cell naevus syndrome was treated with etretinate (50-100 mg/daily) over a period of 5 years (cumulative dose 110 g). Clinically he developed a stiff back with pronounced disability of both hips. Radiologically, axial involvement of the spine with severe diffuse idiopathic skeletal hyperostosis especially of the lumbar spine, and ossifications of both hips were documented. Rheumatological implications of long-term therapy with etretinate are discussed.

Chondroitin sulfate was administered orally to six healthy volunteers, six patients with rheumatoid arthritis and six patients with osteoarthritis. Blood was collected at intervals before and after treatment and the glycosaminoglycan concentration was analyzed in serum using a sensitive assay based on the metachromatic reaction with 1,9-dimethylmethylene blue. The glycosaminoglycan concentration in serum before and after ingestion of chondroitin sulfate was statistically unchanged in all of the subjects studied. We suggest that chondroprotection by orally administered chondroitin sulfate is a biologically and pharmacologically unfounded theory. Any possible benefit to osteoarthritic patients after ingestion of chondroitin sulfate should be sought at the gastrointestinal rather than at the plasmatic or articular cartilage level.

Langzeit-Therapie unterscheidet sich von der Kurzzeit-Therapie durch die besondere biologische Rolle der Zeit. Zeit ist ein physikalischer Parameter. Er dient der Quantifizierung von Abfolgen. Uhren sind Generatoren systematischer Ungleichzeitigkeiten; sie konnen daher als Mesinstrumente eingesetzt werden. Auch biologische Organismen sind Uhren, die sich als Instrumente eignen. Abbildung 1 gibt ihre Skalierung wieder. Sie reicht von den Relaxations-Zeiten einzelner Molekule bis zum Bestand sozialer Systeme. Ihre rechte Seite ordnet den Zeiten biologische Ablaufe zu, die zumeist zyklisch, also reversibel sind. Durch Wiederholung entstehen Rhythmen, deren Phasendauer eine Generation oder nur die Offenzeit eines Ionenkanals betragen kann. Aber diese biologischen Uhren haben eine Besonderheit. Wahrend die ideale physikalische Uhr stos- und schlagfest ist, nehmen biologische Uhren Impulse von ausen auf und stellen sich darauf ein. Anders als physikalische Uhren erinnern sie nicht nur andere an die Zeit, sondern sie erinnern auch an den Inhalt ihrer Zeit. Wenn die Zeit eine Arzneitherapie oder eine andere chemische Exposition enthalt, so wird die Uhr auch daran erinnern. Die verschiedenen Formen des biologischen Gedachtnisses, soweit sie in der praktischen Medizin gewichtig sind, sind auf der linken Seite der Abb. 1 dargestellt. Die Engramme konnen genetischer Art sein und erst mit dem Ende der Keimbahn verschwinden. Arzneitherapeutisch gesetzte Engramme auf neuronalen oder immunologischen Tragern enden spatestens mit dem Tode des Individuums. Aber auch Engramme in unserem Sozialsystem sind relevant; man denke an die nichtgenetische Tradition, welche den Umgang mit Arznei-, Genus- und Nahrungsmitteln mitbestimmt und zu haufig unerwunschten Langzeit-Expositionen beitragt.

In an open three-center pilot study, 17 patients suffering from chronic persistent pain syndrome, due to osteoarthritis of the hip and knee or spondylarthrosis, were treated orally with 1800-2400 mg Ibuprofen per day for 3 weeks. The chronic pain syndrome and joint status were assessed by the physician at the beginning, and after 7, 14 and 21 days. Self-assessments were made by the patients six times daily during the full study period by means of battery-driven electronic diaries (E.D.). The data entered by the patients were stored in the E.D. and after online transfer, processed and evaluated in a PC using appropriate software programs. E.D. were returned by 16 of 17 patients for evaluation. The technical functioning of the instrument was perfect; its use was accepted by patients and physicians. The closely-meshed real-time recording of pain course and other subjective data, such as adverse events or medication, etc., enables the physician to calculate more exactly and reliably improvement rates, as well as to carry out prognostic trend analyses and individual benefit-risk-ratio estimates. By comparing different kinds of data, each entered at the same time, plausibility checks are possible. The procedure presented here is considered to be a new valuable tool for reviewing subjective data from clinical drug trials.

Arthrosis is a collective term the elucidation of which is of decisive importance for the clinical practitioner. The main definition (pathologic/anatomic, radiologic and clinical arthrosis) as well as the pertaining sub-definitions are presented and explained. Both condition definitions latent (dormant) and manifest (active and/or decompensated arthrosis) are clinically relevant. While the cause of primary arthrosis is--presently--still mostly unknown, causes of secondary arthrosis can be defined. Secondary arthrosis is the consequence of a pre-arthrosis. This definition nowadays comprises all kinds of joint lesions (not only of mechanical or traumatic kind) causing an irreversible lesion of the joint organs. Pains of manifest arthrosis are either due to activation (reactive secondary synovitis) or to decompensation (periarthropathy and painful muscular irritations as well as consecutive functional disorders). The distinction is decisive for applying therapeutic means. The clinically subjective and objective symptoms of arthrosis and considerations regarding differential diagnosis are described in detail.

Heat induced radial segmentation of leukocyte nuclei is an in vitro phenomenon accompanying inflammatory diseases. We studied 62 patients with suspected polymyalgia rheumatica (PMR) to determine, whether heat induced radial segmentation can help in discriminating PMR from other conditions. At the initial presentation patients with PMR had more radial segmentation formation than patients with other inflammatory conditions. Prednisone induced a rapid and marked decrease in radial segmentation formation in patients with PMR. This latter finding was much less marked in patients with other inflammatory conditions. We conclude that heat induced radial segmentation at the initial presentation and during prednisone treatment can help in discriminating PMR from other inflammatory conditions.

In 310 freshly analysed synovial fluid (SF) samples from patients with rheumatic and non-rheumatic diseases, activities of leukocyte elastase, collagenase and activator for human leukocyte latent collagenase were determined. These activities were investigated in relation to the number of polymorphonuclear leukocytes (PMN) and the activity of lactate dehydrogenase (LDH). Special care was taken to avoid artificial leakage of enzymes from SF cells after aspiration. The activator of latent collagenase was found in 86% of the SF tested and did not correlate with PMN counts and LDH activity, suggesting that this activity is not of leukocytic origin.

Die menschliche Haltung ist als Ergebnis des Kampfes zwischen Schwerkraft und Aufrichtung eine Resultante aus vielerlei morphologisch-statischen und funktionell-dynamischen Faktoren. Klinisch unterscheidet man zweckmasig zwischen normaler Haltung, bedingt pathologischen Fehlhaltungen und eigentlich krankhaften Fehlformen. Kriterien und klinische Bedeutung der einzelnen Haltungsformen werden eingehend beschrieben, ebenso die drei Verlaufsstadien der Scheuermannschen Krankheit.

One hundred and fifty patients with rheumatoid arthritis received 6 mg of auranofin daily for 2-24 months. Thirty patients were withdrawn from the study, and 82 patients were observed for 12 months. Statistical evaluation was available for 74 patients at weeks 24, 38, and 48, respectively. The major side effects observed were diarrhea, rash, and alopecia. Significant improvement was noted in the articular index, ESR, pain, and morning stiffness.

Die Arthrose ist ohne Zweifel so alt wie die Menschheit, sie hinterlies ihre Unausloschlichen Spuren an den Skelettfunden der Vor- und Fruhmenschen (McCARTY 1979). Aber erst 1877 wies der Wiener Pathologe Weichselbaum erstmals darauf hin, das degenerative Gelenkleiden - er nannte sie Arthritis deformans — als ein krankmachendes morphologisches Geschehen eigengesetzlicher Art zu deuten sei und erst 1913, im selben Jahr, da Nils Bohr sein Atommodell entwarf, hielt der grose Internist Friedrich Von Muller am 17. Internationalen Internistenkongres in London einen Vortrag mit dem Titel: „Differenzierung der unter der Bezeichnung chronische Arthritis zusammengefasten Krankheiten“. Ihm fallt das grose Verdienst zu, erstmals einen klaren klinischen Trennstrich zwischen entzundlichen und degenerativen Gelenkerkrankungen gezogen zu haben. Trotzdem spielte in der Schulmedizin die Arthrose noch bis vor relativ kurzer Zeit die Rolle eines Aschenbrodels. Glucklicherweise haben die moderne pathologische Anatomie und Patho-Physiologie mit ihren verfeinerten morphologischen und vor allem biochemischen Methoden neues Licht auf das dustere Gebiet der Arthrosen-Pathogenese geworfen und damit ein ganz neues Interesse am Arthrosenproblem geweckt. Der Aufschwung der modernen Orthopadie und Rheumatologie brachte zudem neue therapeutische Moglichkeiten mit sich. Grundlagen- und klinische Forschung wurden in den letzten Jahren zunehmend intensiviert, woraus auch eine kaum mehr ubersehbare Flut von entsprechender Literatur resultierte.

A comparative study of the Crithidia luciliae immunofluorescence (CL-IF) assay and an adapted Farr radioimmunoassay (RIA), for the measurement of antibodies to native deoxyribonucleic acid, was performed using forty-two sera from patients with systemic lupus erythematosus (SLE) and another forty-two from patients with rheumatoid arthritis. Both assays were specific for SLE. The CL-IF assay was statistically significantly more sensitive than the adapted RIA assay. This significant difference was due to greater sensitivity of the CL-IF assay in the cases of sera from patients with SLE of slight activity. Additional advantages of the CL-IF assay were its use to classify the immunoglobulin types of the antibodies (most commonly IgG or IgM) and to measure complement-fixing antibodies to native deoxyribonucleic acid; it affords a simple method of selecting and following SLE patients at risk of developing severe renal disease. These advantages plus the simplicity and inexpensiveness of the CL-IF assay make it a useful tool, especially for use in small laboratories, for the study of antibodies to native deoxyribonucleic acid in patients with SLE.

In addition to the habitual skin manifestations, the following forms of organic involvement were found (with decreasing frequency) in 33 patients with progressive systemic sclerosis treated from 1972-1986 at the Clinic of Rheumatology, University Hospital of Zürich: blood vessels 29, lungs 18, digestive system 11, joints 11, bones 8, lacrimal and salivary glands 7, musculature 3, heart 3, kidneys 2, cranial nerves 1 and peripheral neuropathy 1.-A 65-year-old patient with classic progressive systemic sclerosis and visceral involvement of the esophagus and the lungs showed, as a principal sign, unilateral neuropathy of the trigeminal nerve involving all three branches as well as absent corneal reflex. Electromyographic examination afforded evidence of a sensory neuropathy of the trigeminal nerve. Chemical laboratory tests revealed a typical high titre of antinuclear antibodies. In addition to autoimmunological causes, proliferation of fibrous structures of the perineurium and vascular lesions are discussed in connection with these complications.-The subsequent history of 37 patients with localized scleroderma and 4 patients suffering from progressive systemic sclerosis, treated at the Clinic of Dermatology, University Hospital of Zürich from 1980-1985 provided no evidence of a cranial nerve involvement. The fact that during the period 1975 to 1985 only one patient treated at the Clinic of Neurology of Zürich University Hospital showed signs of disturbances of pupil motility confirms the very rare involvement of the nervous system in progressive systemic sclerosis.

During the years 1984-1986 we saw 22 patients with cryoglobulinemia, 8 of whom had rheumatoid arthritis, 1 psoriasis arthritis, 2 Sjögren syndrome, 1 progressive systemic sclerosis, 5 different arthralgias, 1 soft tissue rheumatism, 2 vasculitis and arthralgias and 2 purpura-arthralgia-nephritis syndrome. The measurement of cryoglobulins is done only in special cases of diagnostic importance. The prognostic outlook in patients with nephritis is bad. A monoclonal cryoglobulinemia is always suspicious for a lymphoproliferative syndrome.

In 95 Swiss patients with classical ankylosing spondylitis (AS) the tissue antigen HLA-B27 was present in 92.6%, compared with 7.7% in healthy Swiss blood donors. Assuming the prevalence of ankylosing spondylitis in Switzerland to be 1.9 promille, the chance of a Swiss carrier of HLA-B27 to develop a classical form of AS would be only some 2.2%. For diagnostic purposes, HLA typing thus seems to be of very little value, as among the 462 000 Swiss carriers of HLA-B27 there seem to exist no more than 10 800 classical cases with clinically manifest AS. Absence of HLA-B 27 does not exclude ankylosing spondylitis, as 7.4% of the classical cases are HLA-B27-negative. However, the crudely calculated risk to develop AS is 160 times smaller compared to a carrier HLA-B27. Corner stone of the diagnosis therefore remains careful case history and radiological features of a bilateral sacroileitis of at least grade II.

Der klinische Begriff der Tendopathie im allgemeinen und der Insertionstendopathie (Enthesiopathie) im besonderen, umfast schmerzhafte Phanomene im Bereiche der Sehnen bzw. der epiphysaren oder apophysaren Sehnenansatze. Fuhrendes Leitsymptom dieser Krankheitsbilder sind lokalisierbare, subjektiv oft ausstrahlende und durch mechanische Beanspruchung auslosbare oder verstarkbare Schmerzen im Bereich von Sehnenansatzen und -verlaufen. Meist liegt zusatzlich eine schmerzhafte Funktionseinschrankung des Bewegungs- und Halteapparates vor. „Tendopathie“ und „Insertionstendopathie“ („Enthesiopathie“) sind nach modernen Nomenklaturregeln Sammelbegriffe, die sowohl entzundliche, wie degenerative Veranderungen der Sehnen und deren Ansatzzonen umfassen (Abb. 1.) In der weitaus uberwiegenden Anzahl der klinischen Falle mussen die Tendopathien in die Gruppe der degenerativen Bindegewebserkrankungen eingereiht werden. Die Bezeichnung „Tendinose“ und „Insertionstendinose“, sind dann nicht nur berechtigt, sondern einzig korrekt. Entzundliche Vorgange spielen bei diesen Krankheitsbildern keine primare pathogenetische Rolle, sie treten hochstens zeitweilig als sekundare vorubergehende Kollateral-Phanomene (Peritendinitis, Tendovaginitis, Bursitis) in Erscheinung. Die entzundlich-rheuma tischen Krankheiten hingegen fuhren haufig zu einem Befall der Sehnen und deren Insertionszonen. Echte „Tendinitiden“ und „Insertionstendi-nitiden“ — praktisch immer begleitet von Tenosynovitiden — treten vor allem bei der chronischen Polyarthritis, der Psoriasis-Arthritis und dem Reiter-Syndrom auf, bei der Spondylitis ankylosans kommt es haufig zu Insertionstendinitiden, deren klinische Symptomatik eine wichtige Rolle fur die Fruhdiagnose spielt.

Die folgenden Ausfuhrungen konzentrieren sich auf das Problem der diagnostischen Abklarung bei Erstuntersuchung in der Praxis eines Nicht-Facharztes. Erste Voraussetzung fur eine moglichst zielgerichtete und damit zweckmasige und kostensparende diagnostische Laboruntersuchung sowie fur den Einsatz der technischen Diagnostik ist eine gut fundierte Kenntnis der Nosologie der verschiedenen rheumatischen Krankheitsbilder. Nicht nur in einer Spezialklinik oder beim Facharzt, sondern auch in jeder Allgemeinpraxis, steht an der Spitze des Abklarungsprocedere die klinische Diagnostik, mit eingehender Anamnese sowie einer technisch einwandfreien, sorgfaltigen palpatorischen und funktionellen klinischen Untersuchung des Bewegungsapparates, kombiniert mit den internistischen klinischen Routineuntersuchungen. Auf dem gesicherten Fundament der klinischen Diagnostik mus anschliesend die Frage entschieden werden, welche zusatzlichen Untersuchungen notwendig, bedingt notwendig oder entbehrlich sind. In den Literaturangaben sind jene Publikationen angefuhrt, die vor allem dem Nicht-Facharzt den Schritt von der klinischen Krankheitsdiagnostik zur Indikation zusatzlicher Abklarungsmethoden, deren Interpretation und Aussagewert erleichtern. Von entscheidender Bedeutung ist, das der abklarende Arzt ein rationelles diagnostisches Stufenprogramm entwirft und einhalt. Es ist vollig unzweckmasig, ja oft sogar irrefuhrend, wenn schon im Anschlus an eine Erstuntersuchung bei einem Patienten mit nicht eindeutig klaren rheumatischen Beschwerden samtliche heute zur Verfugung stehenden, diagnostischen Laboruntersuchungen und diagnostische technische Untersuchungsmoglichkeiten angeordnet werden.

The question, whether HLA-B27-negative patients with classical ankylosing spondylitis (AS) belong to a separate nosological entity, was studied by a standardized analysis of 12 clinical, 8 radiological and 6 laboratory criteria in 95 cases, including 7 with HLA-B27-negativity, who reinforced international criteria for classical AS. The results showed neither definite clinical nor radiological, or laboratory differences between the HLA-B27 negative and positive group. We conclude, that existence or absence of the tissue antigen HLA-B27 has no influence on inflammatory activity of skeletal or soft tissue manifestations of the disease. The group of HLA-B27-negative patients who fulfill the criteria of classical AS does therefore not seem to form a separate nosological entity.

Da die Arthrosen zu den haufigsten chronischen Erkrankungen uberhaupt zahlen, stellen sie auch fur jeden Internisten und Allgemeinpraktiker ein diagnostisches und thera-peutisches Alltagsproblem in der Praxis dar, mit dem er vertraut sein mus. Epidemiolo-gische und klinische Analysen (Abb. 1a-c, Abb. 2) fuhren ubereinstimmend zum Ergebnis, das der Oberbegriff Arthrose weiter aufgeschlusselt werden mus, so das er auch fur den praktisch tatigen Kliniker fasbar und anwendbar wird (Tabelle 1).

Immunohistochemical studies were performed on the temporal artery of 34 patients with clinically established polymyalgia rheumatica (PR) or temporal arteritis, 6 patients with vasculitis, and 25 patients with various diseases. The combined immunofluorescence and peroxidase-anti-Peroxidase Methode zeigte Immunoglobulin- und C3-Ablagerunin histologically affected and to some degree also in unaffected arteries of patients with PR and in all patients with temporal arteritis. The deposits were found both inter- and intracellularly, and contained IgA and to a lesser extend IgG, IgM, and C3. Linear deposits of leukocyte elastase were found along the fragmented internal lamina, and decaying polymorphonuclear (PMN) leukocytes surrounded by elastase-containing inclusions were found in the neighborhood of zones rich in elastic material. These findings suggest that immune complex deposition is a prominent feature of temporal arteritis and that the PMN elastase is probably involved in the destruction of elastic fibers. The combined immunohistochemical investigation appears to increase the diagnostic value of temporal artery biopsy.

Als 1973 die ersten Berichte uber eine hochsignifikante Haufung des angeborenen Gewebsantigens HLA-B27 bei Spondylitis ankylosans (Bechterew) erschienen (21), sah es aus, als ob eine neue Ara der Bechterewdiagnostik ihren Anfang genommen hatte.

Osteoarthritis (OA) is one of the most common diseases requiring treatment. A clear clinical and pathophysiological understanding of the disease is required before treating a patient with OA. Drug therapy of patients with OA is mainly divided into two groups: a group receiving symptomatic therapy and a group receiving drugs intended to modify or improve the disease. Symptomatic therapy includes pure analgesics as well as non-steroidal antirheumatic drugs and muscle relaxants. Their use depends on the activity as well as on the form of clinical manifestation (activated/decompensated disease). If there are signs of severe activation, intra-articular steroids, orgotein or even synoviorthesis with yttrium are administered as local therapy. All of these therapies are used in the sense of trouble shooters, as they cannot alter the course of the disease in the long run. The disease-modifying substances have well been investigated in vitro and in animal models. Furthermore, some clinical trials have shown evidence for the usefulness of these substances in the therapy of OA.

Die Arthritis (Synovitis) villonodularis pigmentosa (A.v.p.) ist eine chronische, gutartige, atiologisch ungeklarte Erkrankung von Gelenken, Bursen und Sehnenscheiden. Sie ist charakterisiert durch eine teils zottige, teils knotige, stark hamosiderinhaltige expansive Proliferation synovialer Gewebe. Das histologische Bild ist an sich typisch, aber sehr variabel und weist Merkmale sowohl eines blastomatosen (sarkomartigen) als auch eines chronisch-entzundlichen Prozesses auf. Obwohl das Krankheitsbild bereits seit etwa 120 Jahren in seiner Symptomatik bekannt ist (Chassaignac 1852; Simon 1865), erstaunt es daher nicht, das es unter sehr zahlreichen, unterschiedlichen Synonyma erwahnt und beschrieben wurde (Tabelle 1). Es ist das Verdienst von Jaffe et al.(1941), diese Sonderform der Synovitis nicht nur treffend beschrieben, sondem auch als nosologische Einheit erkannt zu haben. Von diesen Autoren stamnit denn auch die inzwischen allgemein akzeptierte, deskriptive Bezeichriung „villonodulare pigmentierte Synovitis“. 1958 hat Uehlinger die Darstellung von Jaffe wesentlich erweitert und erganzt und die typischen Krankheitserscheinungen herausgearbeitet. Da die Synovialisproliferation mit ihrem Fortschreiten spater alle gelenknahen Strukturen, insbesondere den Knochen mit einbezieht, hat die Arbeitsgemeinschaft „Klassifikation“ der Deutschen Gesellschaft Fur Rheumatologie in ihrer Klassifikation der Erkrankungen des Bewegungsapparates (1974) fur das Krankheitsbild die Bezeichnung „Arthritis“ villonodularis pigmentosa gewahlt.

The new antirheumatic agent diclofenac sodium (Voltaren) has been compared with indometacin with respect to tolerance and effect on osteoarthritic symptoms. In a double blind trial two groups of 30 arthritic patients each received Voltaren and indometacin respectively for 12 weeks. The initial dose for both drugs was 75 mg per day. Where the effect was inadequate the dose could be raised to 125 mg daily. The outcome revealed no significant differences of the effect and tolerance between the two drugs. An earlier onset of effect, however, was observed in the Voltaren group.

A case of polymyalgia rheumatica in a 70 year old woman is described. The patient was treated for 6 years with corticosteroids. Seven months after withdrawal of treatment, a severe relapse of polymyalgia rheumatica appeared which was connected with a histologically and immunohistochemically proved temporal arteritis. The problem of the duration of steroid treatment and the need for continued clinical as well as laboratory follow-up after withdrawal of therapy are discussed.

Seit der Entdeckung der statistisch hoch signifikanten Assoziation des angeborenen Gewebsantigens HLA-B27 mit Spondylitis ankylosans drangte sich die Frage auf, ob die, je nach Nation, 4–12% HLA-B27-negativen Trager einer klinisch eindeutigen Spondylitis ankylosans eine eigenstandige nosologische Entitat darstellten, wie dies unter anderem am HLA-Workshop des B. Europaischen Kongresses fur Rheumatologie aufgeworfen worden war (116).

Pathologisch-anatomisch, klinisch und radiologisch ist die Gonarthrose die haufigste aller Arthrosen: Heine (1926), Lawrence (1966, 1969, 1977), Moring (1966, 1967), Rimann (1906), De Seze und Ryckewaert (1954), Wagenhauser (1969a, 1969d), Waterman (1955), Weichselbaum (1877).
