Im Rahmen von onkologischen Operationen besteht immer die Abwagung zwischen onkologischer Sicherheit und dem Funktionserhalt. Dies gilt insbesondere bei Operationen im Becken, aufgrund der engen Lagebeziehung zur Muskulatur des Beckenbodens sowie Gefas und Nervenverlaufen. Aktuell dienen zumeist Risikomodelle, praoperative Bildgebung, die Einschatzung des Operateurs sowie der intraoperative Schnellschnitt als Entscheidungsgrundlage. Neue Bildgebungstechniken sowie Standardisierung im Schnellschnitt haben diese in den letzten Jahren deutlich verbessert. Es verbleiben jedoch weiterhin Einschrankungen durch zeitliche Verzogerung sowie erschwerte korrekte anatomische Zuordnung der zu untersuchenden Praparate. Alternative intraoperative Verfahren konnten diese Grenzen in der Zukunft uberwinden. Patientenabgeleitete Organoide haben sich in den letzten Jahren zu einem wichtigen neuen Forschungsvehikel entwickelt. Sie basieren auf Tumorstammzellen, die unter speziellen Kulturbedingungen dreidimensionale Replikate des Ursprungsgewebes formen. Damit sind sie optimal geeignet zur Testung von individuellen Systemtherapien aber auch als Validierungstechnik fur neue diagnostische Verfahren. Das durch die Deutsche Forschungsgemeinschaft geforderte Graduiertenkolleg 2543/I analysiert in einem interdisziplinaren Team das Potenzial neuer diagnostischer Methoden im Hinblick auf die intraoperative real-time Diagnostik in Erganzung zu konventionellen Schnellschnittdiagnostik

The classification of intraocular lymphomas is based on their anatomical location. They are divided into uveal lymphomas with involvement of the choroid, ciliary body or iris and vitreoretinal lymphomas with isolated or combined involvement of the vitreous body and/or retina. Over the last decades it has become increasingly possible to work out the clinical and pathobiological features of the various subtypes, thereby reducing the diagnostic hurdles and creating improved treatment options.A summary of the various types of intraocular lymphoma in terms of clinical features, diagnostics, treatment and prognosis is given as well as recommendations for follow-up care.A selective literature search was carried out on the subject of intraocular lymphomas using PubMed and Google Scholar.Intraocular lymphomas affect different structures, so that the symptoms can also be very different. The diagnostic spectrum ranges from typical ocular examination methods to sample biopsies with subsequent cytological, histological and molecular pathological processing. The treatment pillars available are percutaneous irradiation and intravitreal drug administration as local treatment and systemic treatment or a combination of systemic and local treatment. The prognosis depends mainly on the subtype of the lymphoma and the extent of the infestation when the diagnosis is confirmed. Even though some effective treatment options are now available, it has not yet been possible to significantly reduce the mortality rate.Many different options are available for the diagnostics and treatment of intraocular lymphomas, which require close interdisciplinary cooperation. The further developments in the field of molecular pathology allow a faster and more accurate diagnosis and could open up new treatment options in the future.HINTERGRUND: Die Einteilung der intraokulären Lymphome erfolgt in uveale Lymphome mit Beteiligung der Aderhaut, des Ziliarkörpers oder der Iris und die vitreoretinalen Lymphome mit isolierter oder kombinierter Beteiligung des Glaskörpers bzw. der Netzhaut. Über die letzten Jahrzehnte ist es zunehmend gelungen, die klinischen und pathobiologischen Merkmale der verschiedenen Typen herauszuarbeiten und damit die diagnostischen Hürden zu reduzieren und verbesserte Therapieoptionen zu schaffen.Es erfolgt eine Zusammenfassung der verschiedenen intraokulären Lymphomarten in Bezug auf Klinik, Diagnostik, Therapie und Prognose sowie Empfehlung der Nachsorge.Es wurde eine selektive Literaturrecherche zum Thema intraokuläre Lymphome mittels PubMed und Google Scholar durchgeführt.Die intraokulären Lymphome betreffen unterschiedliche Strukturen, sodass auch die Symptomatik sehr verschieden sein kann. Das diagnostische Spektrum reicht von typischen okulären Untersuchungsmethoden bis hin zur Probebiopsie mit anschließender zytologischer/histologischer und molekularpathologischer Aufarbeitung. Als therapeutische Säulen stehen zum einen die perkutane Bestrahlung und intravitreale Medikamenteneingabe als lokale und zum anderen die systemische Therapie bzw. eine Kombination aus System- und Lokaltherapie zur Verfügung. Die Prognose ist hauptsächlich vom Subtyp des Lymphoms und vom Ausmaß des Befalls bei Diagnosesicherung abhängig. Auch wenn mittlerweile z. T. effektive Therapien zur Verfügung stehen, ist es bisher nicht gelungen, die Mortalitätsrate deutlich zu senken.Für Diagnostik und Therapie der intraokulären Lymphome stehen viele verschiedene Optionen zur Verfügung, die eine enge interdisziplinäre Zusammenarbeit erfordern. Die Weiterentwicklungen im Bereich der Molekularpathologie erlauben eine schnellere und treffsichere Diagnosestellung und könnten in Zukunft neue Therapieoptionen eröffnen.

In situ follicular neoplasia (ISFN) is a t(14;18)(q32;q21)To investigate the clonal evolution of ISFN to DLBCL - transformed from FL and de novo.Identification of ISFN lesions in patients with DLBCL was performed by BCL2 staining of reactive lymphoid tissues. ISFN and DLBCL were subsequently analyzed by fluorescence in situ hybridization, clonality analyses, sequencing of the t(14;18) breakpoint, and targeted next-generation sequencing.10 cases with paired ISFN and DLBCL samples were identified, 6 of which were de novo DLBCL and 4 transformed from FL. 3 DLBCL carried MYC-rearrangements in addition to the t(14;18) and were classified as high-grade B‑cell lymphoma (HGBL). The clonal relationship of ISFN and DLBCL/HGBL was confirmed for all cases. CREBBP, KMT2D, EZH2, TNFRSF14, and BCL2 were the genes most frequently mutated, with the distribution of private and shared mutations pointing to 2 different scenarios of clonal evolution. In most cases, DLBCL/HGBL, ISFN, and, if also present, FL had evolved divergently from a common progenitor, whereas linear evolution was less frequent.We show for the first time that t(14;18)HINTERGRUND: Die in situ follikuläre Neoplasie (ISFN) ist eine t(14;18)(q32;q21)Erforschung der klonalen Evolution der ISFN zum DLBCL – transformiert aus einem FL und de novo.Die Identifikation von ISFN-Läsionen bei Patienten mit DLBCL erfolgte durch BCL2-Färbung reaktiver lymphatischer Gewebe. ISFN und DLBCL wurden anschließend mittels Fluoreszenz-in-situ-Hybridisierung, Klonalitätsanalysen, Sequenzierung des t(14;18)-Bruchpunktes und gezieltem Next Generation Sequencing analysiert.Es wurden 10 Fälle mit gepaarten ISFN- und DLBCL-Proben identifiziert, davon 6 de novo DLBCL und 4 transformiert aus einem FL. Neben der t(14;18) trugen 3 DLBCL auch MYC-Rearrangements und wurden als high-grade B‑Zell-Lymphom (HGBL) klassifiziert. In allen Fällen konnte die klonale Verwandtschaft von ISFN und DLBCL/HGBL bestätigt werden. CREBBP, KMT2D, EZH2, TNFRSF14 und BCL2 waren die am häufigsten mutierten Gene, wobei die Verteilung privater und gemeinsamer Mutationen auf 2 unterschiedliche Szenarien der klonalen Evolution schließen ließ. Meist hatten sich DLBCL/HGBL, ISFN und, falls ebenfalls vorhanden, FL divergent aus einem gemeinsamen Progenitor entwickelt, während eine lineare Evolution selten war.Wir zeigen erstmals, dass t(14;18)

In the context of cancer surgery, there is always a trade-off between oncological safety and preservation of function. This is especially true in pelvic surgery due to the close relationship to the pelvic floor muscles, blood supply and nerves. Currently, risk models, preoperative imaging, the surgeon's assessment, and the intraoperative frozen section serve as the basis for decision-making. New imaging techniques and standardization in frozen section have significantly improved this in recent years. However, limitations remain due to time delays as well as more difficult correct anatomical assignment in the follow-up. Alternative intraoperative techniques may overcome this limitation in the future. Patient-derived organoids have emerged as an important new research vehicle in recent years. They are based on tumor stem cells that, under special culture conditions, form three-dimensional replicas of the original tissue. This makes them ideally suited for testing individual system therapies but also as a validation technique for new intraoperative diagnostic procedures. The CT and MRI scans of a 70-year-old male patient revealed a mass in the pancreatic head and a 2.8-cm peripancreatic lymph node. Under steroid therapy the mass did not show regression. Finally, a pancreatoduodenectomy was performed. Histologically, Rosai-Dorfman disease (RDD) was diagnosed. RDD is a rare histiocytic disorder with usually nodal but sometimes also extranodal involvement. Herein we report a rare case of extranodal RDD with intrapancreatic localization.Bei einem 70-jährigen Patienten fielen in der Bildgebung eine Raumforderung im Pankreaskopf und ein 2,8 cm messender peripankreatischer Lymphknoten auf, die unter Steroidtherapie nicht regredient waren. Schließlich wurde eine partielle Duodenopankreatektomie durchgeführt und am OP-Präparat eine Rosai-Dorfman-Erkrankung (RDE) mit Lymphknotenbeteiligung diagnostiziert. Die RDE ist eine seltene histiozytäre Erkrankung, die in erster Linie die Lymphknoten betrifft, sich jedoch auch extranodal manifestieren kann. Fälle mit Infiltration des biliodigestiven Trakts sind selten und treten häufig zusammen mit einer Vaskulopathie oder IgG4-assoziierten Läsionen auf.

The identification and molecular characterisation of premalignant precursor lesions of lymphomas, such as monoclonal gammopathy of unknown significance (MGUS) and the so-called in situ lymphoproliferations, has made significant progress in the recent years. The in situ follicular neoplasia (ISFN), the best-characterised entity, is by definition not identifiable by morphology and represents a t(14;18)+ precursor lesion of follicular lymphoma with characteristic immunophenotype, low potential for progression, and already identifiable secondary genetic alterations. The use of high-throughput genetic techniques on microdissected tissues has generated novel insights into clonal evolution and biological progression of early lesions and documented that an isolated genetic analysis is insufficient to understand the complexity of proliferations.

After 8 years, the WHO has now published the updated version of the 4th edition of the classification of hematopoietic and lymphoid tumors. This update provides a conceptual rewrite of existing entities as well as some new provisional entities and categories, particularly among the aggressive B‑cell lymphomas. Important new diagnostic categories include the high-grade B‑cell lymphomas, the large B‑cell lymphoma with IRF4 rearrangement, and the Burkitt-like lymphoma with 11q aberrations. Of particular importance, new concepts concerning the taxonomy and classification of early lymphoid lesions or precursor lesions are included, such as the in situ follicular neoplasia or the in situ mantle cell neoplasia. In addition, the concept of indolent lymphoproliferations, such as breast-implant-associated anaplastic large cell lymphoma and the indolent T‑cell lymphoproliferative disorder of the gastrointestinal tract, has been strengthened. Finally, diagnostic criteria for existing lymphoma entities have been refined.

Zusammenfassung: Primäre T-Zell-Lymphome des zentralen Nervensystems (ZNS) sind selten. Sie müssen differenzialdiagnostisch von reaktiven Läsionen unterschieden werden. Die Diagnosestellung sollte integrativ unter Verwendung von immunhistochemischen, molekulargenetischen und/oder zytogenetischen Methoden erfolgen. Wir beschreiben den Fall eines 50-jährigen Mannes, bei welchem ein primäres zerebelläres T-Zell-Lymphom diagnostiziert und eine klonale T-Zell-Rezeptorgen-Umlagerung nachgewiesen wurde. Nach 2Zyklen Chemotherapie entwickelte der Patient eine Pneumozystis-carinii-Pneumonie und verstarb 10Wochen nach Diagnosestellung. Die Autopsie ergab keinen Residualtumor im ZNS

The update of the 4th edition of the WHO classification for hematopoietic neoplasms introduces changes in the field of mature aggressive B‑cell lymphomas that are relevant to diagnostic pathologists. In daily practice, the question arises of which analysis should be performed when diagnosing the most common lymphoma entity, diffuse large B‑cell lymphoma. We discuss the importance of the cell of origin, the analysis of MYC translocations, and the delineation of the new WHO entities of high-grade B‑cell lymphomas.

Das Peritoneum ist nicht nur eine Membran mit einem gemischten mesothelialen und epithelialen Phanotyp, sondern vielmehr ein Organ mit mehreren lebenswichtigen Funktionen. Die funktionelle Anatomie des Peritoneums bildet die pathophysiologische Basis der Entwicklung benigner und maligner Peritonealerkrankungen. Die zur tumorspezifischen Therapie notwendige Ursprungsdiagnostik peritonealer Tumoren bleibt wegen embryologischer und morphologischer Ahnlichkeiten eine Herausforderung. Bei Peritonealtumoren tragt die histologische Beurteilung der Tumorregression wesentlich zur Evaluation des Therapieansprechens bei. Molekulare Untersuchungen von Peritonealbiopsien nach Laparoskopie werden fur die Entwicklung patientenspezifischer Therapieverfahren eine zunehmende Rolle spielen.

The eye and the ocular adnexae are rare sites for malignant non-Hodgkin lymphoma (NHL). Based on their anatomical location, intraocular lymphomas must be discerned from NHL of adnexal structures including conjunctiva, lacrimal gland, and orbit. Whereas the latter group mostly consists of indolent extranodal marginal zone B‑cell lymphomas of mucosa-associated lymphoid tissue (MALT) type or secondary manifestations of systemic NHL, most primary intraocular lymphomas are classified as diffuse large B‑cell lymphomas (DLBCL) and are considered a variant of primary DLBCL of the central nervous system. The most common form is primary vitreoretinal lymphoma (PVRL), which presents with nonspecific symptoms and is difficult to discern from uveitis. Diagnosis of PVRL is usually made by cytological, immunocytochemical, and molecular analysis of vitreous aspirates. Degenerative changes, limited material, and the occurrence of pseudoclonality in the molecular analysis of B‑cell clonality can hamper diagnostic assessment. Novel techniques such as detection of MYD88 mutations common in PVRL can increase diagnostic sensitivity. Close cooperation with clinical colleagues and rapid specimen processing are fundamental for successful diagnosis.

Odontogene Kieferhohlenerkrankungen konnen in der differenzialdiagnostischen Abgrenzung signifikante Schwierigkeiten bereiten. Die korrekte histopathologische Einordnung ist aber unerlassliche Voraussetzung fur eine adaquate Therapie, die zwar i.d.R. in einem chirurgischen Eingriff besteht, aber von einer minimal-invasiven Masnahme bis zu einer kontinuitatsunterbrechenden En-bloc–Resektion mit notwendigen Sicherheitsabstanden und ggf. Lymphknotendissektion reichen kann. Nur durch die Integration klinischer, radiologischer und histopathologischer Befunde kann eine ausreichende Entscheidungsbasis fur eine adaquate individuelle Therapie sichergestellt werden. In diesem Beitrag werden wichtige Entitaten, die es in histopathologischer Hinsicht voneinander abzugrenzen gilt, in ihrer klinischen Bedeutung dargestellt.

Zielsetzung: Endometriumkarzinome werden derzeit anhand FIGO-Stadium, Grade und histologischem Typ in prognostisch und therapeutisch relevante Gruppen mit niedrigem, intermediarem und hohem Risiko stratifiziert. Trotz exzellenter Prognose kommt es in der Gruppe mit niedrigem/intermediarem Risiko zu nicht vorhersagbaren Rezidiven. Das Zelladhasionsmolekul L1CAM wurde in diesem Zusammenhang als moglicher Biomarker fur die Identifikation bislang nicht erkannter high-risk Karzinome beschrieben. Materialien und Methoden: Identifikation samtlicher in den Jahren 2003 – 13 an der Universitatsfrauenklinik Tubingen behandelter Patientinnen mit Endometriumkarzinom und niedrigem (Typ I, FIGO IA, G1/2) bzw. intermediarem (Typ I, FIGO IA, G3 oder FIGO IB, G1/2) Risikoprofil. Spezialisierte histopathologische Zweitbegutachtung und L1CAM Expressionsanalyse mittels Immunhistochemie. Tumoren mit ≥10% angefarbten Tumorzellen wurden als L1CAM positiv gewertet. Multivariate Survivalanalysen mittels Cox-Regression Modellen. Ergebnisse: Insgesamt standen 344 Falle fur die immunhistochemische Untersuchung zur Verfugung. 29/344 (8,5%) Falle waren L1CAM positiv, 315/344 (91,5%) Falle waren L1CAM negativ. In multivariaten Analysen, welche neben L1CAM Expression auch Alter, FIGO Stage und Grade berucksichtigten, zeigte sich L1CAM sowohl im Gesamtkollektiv als auch in der Untergruppe der low-risk Karzinome mit einer Hazard-Ratio von 4,58 (CI 1,72 – 10,83; p = 0,003) bzw. 5,12 (CI 1,39 – 15,35; p = 0,017) als signifikanter Prognosefaktor fur das Gesamtuberleben. Zusammenfassung: Die Ergebnisse unserer Arbeit bestatigen L1CAM als einen fur das Endometriumkarzinom unabhangig prognostisch relevanten Biomarker. Die Berucksichtigung entsprechender L1CAM Expressionsanalysen konnte in Zukunft geeignet sein, Patientinnen mit bislang falsch eingeschatztem Risikoprofil zu identifizieren. Entgegen der heutigen Leitlinien konnten diese Patientinnen („low-risk, L1CAM+“) von der Entwicklung individualisierter und risikoadaptierter Therapiekonzepte profitieren.

Female adnexal tumors of probable Wolffian origin (FATWO) are rare tumors, which are mostly localized in the broad ligament or the mesosalpinx. They show high intratumor and intertumor variability of histological patterns (e.g. solid, tubular, cribriform and cystic) with usually unremarkable cellular and nuclear morphology and a lower mitotic rate. In general, they behave in a benign fashion but there are rare cases with malignant transformation, so that careful examination and surveillance are necessary. Differential diagnoses include Sertoli-Leydig cell tumors, metastasized endometrioid carcinoma and the FATWO-like variant of the endometrioid carcinoma of the fallopian tubes. The FATWOs express pancytokeratin, CD10, vimentin, calretinin and inhibin A. Estrogen and progesterone receptors are expressed in a minority of cases, whereas epithelial membrane antigen (EMA) is not detectable.

Die rasche methodische Weiterentwicklung immunhistochemischer und molekularpathologischer Verfahren hat auch betrachtliche Auswirkungen auf die Diagnostik an der Knochenmarkstanze. Die Erzielung einer moglichst guten Morphologie bei gleichzeitigem Erhalt der Gewebsantigenitat und der Integritat der DNS fur molekulare Untersuchungen einerseits und der Wunsch auf zeitnahe Bearbeitung andererseits erfordern einen sorgfaltig geplanten Arbeitsablauf fur Fixierung, Entkalkung und Einbettung des Trepanats. Dabei stellt die Fixierung in gepuffertem Formalin kombiniert mit der EDTA-Entkalkung einen sehr guten Kompromiss dar, der einen Einsatz aller modernen Techniken ohne Einschrankungen erlaubt. Obwohl sehr viele molekulargenetische Untersuchungen am Aspirat oder an Zellen aus dem peripheren Blut durchgefuhrt werden, gibt es doch Fragestellungen, bei denen molekularpathologische Analysen wie die Klonalitatsbestimmung bei lymphatischen Knochenmarkinfiltraten oder der Nachweis spezifischer Mutationen am Trepanat notwendig werden. Aufgrund der besonderen Materialgegebenheiten sind insbesondere fur die Klonalitatsbestimmung eine zuverlassige Qualitatskontrolle und eine genaue Kenntnis der biologischen und technischen Fehlerquellen notwendig. Diese Ubersichtsarbeit gibt einen Uberblick uber technische Aspekte der Aufarbeitung und diskutiert die Anwendung und Einsatzmoglichkeiten molekularpathologischer Methoden an der Knochenmarkbiopsie.

Plasma cell myeloma (PCM) and related immunosecretory disorders are a group of B-cell proliferations with a wide clinical and prognostic spectrum, characterized by the production of monoclonal immunoglobulin by immortalized plasma cells. Recent years have seen an explosion in knowledge on the genetic basis and biology of these diseases, followed by improved clinical risk stratification and the introduction of novel therapeutic concepts, such as treatment with proteasome inhibitors or immunomodulatory substances. PCM is a common malignancy, accounting for approximately 10% of all hematological neoplasms. There is good evidence to support a multistep transformation process in plasma cell neoplasms, which corresponds to clinically discernible disease stages. Monoclonal gammopathy of unknown significance is a common asymptomatic precursor lesion for PCM which carries an approximately 1% annual risk for progression. Terminal disease stages are characterized by increasing genetic complexity and independence from bone marrow stromal cells and show a rapidly increasing tumour load with severe clinical symptoms. Modern diagnostics of plasma cell neoplasms require inclusion of clinical, morphological, immunophenotypical and cytogenetic features to allow for individual risk assessment and therapy planning.

Non-Langerhans cell histiocytoses (N-LCH) of adulthood are rare disorders with heterogeneous pathogenesis, morphology and clinical presentation. In this review two disorders are presented, which predominantly develop in extracutaneous sites in adults. Erdheim-Chester disease is a rare nonhereditary clonal disorder of lipid storing histiocytes most commonly presenting as osseous involvement of the long bones. Other organ manifestations include the central nervous system (CNS), the cardiovascular system, the retroperitoneum and kidneys and less commonly the skin and the lungs. Immunohistochemical staining reveals positivity for the macrophage markers CD163, CD68 and lysozyme but CD1a and langerin are negative, in contrast to Langerhans cell histiocytosis. Rosai-Dorfman disease is considered to be a reactive histiocytic proliferation occurring mainly in lymph nodes. Prominent sinuses filled with commonly multinucleated, S100-positive histiocytes with emperipolesis are a characteristic feature and develops particularly as extensive lymphadenopathy with massive sinus histiocytosis but can also occur extranodally. Painless bilateral cervical lymph node enlargement is the most common clinical presentation. This review summarizes the clinical, radiological and histopathological findings and discusses the recent molecular advances in these rare disorders.

A 72-year-old male patient presented with multiple erythematous plaques on the lower arms, lower legs and feet. The patient suffered from rheumatoid arthritis and accompanying interstitial granulomatous dermatitis under treatment with tocilizumab. Several months prior to presentation a chronic myelomonocytic leukemia (CMML) had been diagnosed. The skin biopsy showed a perivascular infiltration of medium-sized cells with positivity for CD123, CD303 and CD4 with a low proliferation activity so that a diagnosis of a CMML-associated proliferation of plasmacytoid dendritic cells was made. The differential diagnosis of specific cutaneous infiltrates in CMML is discussed.

MEDICAL HISTORY AND CLINICAL COURSE: A 42-year-old patient with hairy cell leukemia had been treated for 3 years by a hematologist in private practice. Initially the patient received 1 course of cladribine upon which the disease went into complete remission. 6 weeks ago a relapse was diagnosed and combination therapy with cladibrin and rituximab was initiated. Now the patient presented to the emergency room with shortness of breath and pain when breathing.In the chest x-ray, patchy infiltrates and pleural effusions were found on both sides. The subsequently performed computed tomography showed bilateral compactions with an Halo suspicious for fungal infiltrates. Upon admission to the hospital, an empirical antibiotic therapy with clarithromycin and piperacillin/tazobactam was initiated, which was later escalated to meropenem and linezolid. Additionally, an antifungal therapy with voriconazole was started and later switched to liposomal amphotericin B. At his admission, a positive aspergillus antigen could be detected in the microbiological laboratory. Under antimycotic treatment the aspergillus antigen was repeatedly negative. The patient presented with pronounced cytopenias and after a switch of therapy to vemurafenib and filgrastim, the hematopoiesis could only be stimulated insufficiently. The patient was transferred to the intensive care unit three days after admission with severe respiratory failure. He died on day 8 after admission. AUTOPSY AND DIAGNOSIS: Diagnosis was consistent with relapse of hairy cell leukemia with positive BRAF mutation and a bone marrow infiltration 80 %. Autopsy revealed a significant hepato-splenomegaly, a lack of erythro-, granulo- and thrombopoiesis. Clots interspersed with fungal hyphae were found in both lungs and an infarction of the spleen with evidence of fungal hyphae was detected. The cultural findings post mortem on yeast or mold were negative.Patients with refractory hairy cell leukemia and prolonged neutropenia are at increased risk for systemic fungal infections. Therefore, prohylactic antimycotic therapy should be considered early in this group of patients. The therapeutic approach of vemurafenib in treatment-refractory hairy cell leukemia is promising and offers an additional treatment option. In the present case, the patient could unfortunately not be stabilized due to the septic complications.

Pulmonary involvement in ulcerative colitis may manifest as a variety of disorders. Ulcerative colitis-related interstitial lung disease is exceedingly rare and has been reported to be steroid-responsive. We describe the first case of a patient with acute exacerbation of ulcerative colitis-induced usual interstitial pneumonia, who did not respond to corticosteroid therapy and died 12 weeks after the onset of pulmonary symptoms. Early recognition of pulmonary disease in patients with ulcerative colitis is necessary to initiate further diagnostic work-up and may aid treatment decisions.

This study documents the characteristics of a large series of spontaneously occurring thymomas in a laboratory colony of European hamsters (Cricetus cricetus). Thymomas are rare organotypic neoplasms originating from the thymic epithelial compartment. Because the hamster thymomas largely resembled their human counterparts, the recent World Health Organization (WHO) classification of human thymic epithelial tumors was used. Forty hamsters of both sexes aged 3-29 months were examined macroscopically and histo- logically. In 22 (55%) of the 40 animals, necropsy revealed enormous whitish masses in the anterior medias- tinum, with a diameter ranging from 0.5 to 4.5 cm and a lobulated structure. The anatomy of the thymus region was normal in the remaining 18 hamsters. Histologically, the tumors presented as thymuslike organoid structures with areas of medullary and cortical differentiation and a predominance of lymphoid cells. A network of epithelial cells in the cortical areas, demonstrated immunohistochemically with a cross-reactive antibody against pancytokeratin, supported the diagnosis of thymoma. Cortical lymphocytes showed positive staining with cross- reacting antibodies against CD3 and terminal deoxynucleotidyl transferase, characteristic of immature T cells. On the basis of these findings, the tumors were classified as B1 thymomas, in some cases with AB or B2 components, according to the new WHO classification for human thymic epithelial tumors. Several possible differential diagnoses must be kept in mind when tumorous masses are found in the an- terior mediastinum. The most frequent neoplasms are lymphomas, followed by thymomas, tumors of ectopic thyroid tissue, and neuroendocrine or mesenchymal tu- mors. Thymomas are rare malignancies both in human and veterinary medicine. They have been described in a wide range of domestic and laboratory animals and occur more frequently in older animals. In humans, thymomas usually present in the fourth to fifth decade but can occur at all ages without sex predisposition. 28
