This case study focuses on developing an objective, baseline understanding of the opinions and behaviors of residents currently residing in the developing neighborhood of Livingston (Calgary, Alberta, Canada). Livingston is unique because it is a new community composed of mostly young couples from diverse backgrounds who have collectively chosen to be part of a community-focused developing neighborhood. More specifically, researchers from Mount Royal University (MRU) explored the sense of belonging, physical activity behaviors, and level of engagement of the pioneer residents (first families who opted to move in to this brand-new community). This baseline knowledge can be used by the developer (Brookfield Residential) and local recreation leaders (such as Vivo for Healthier Generations and City of Calgary Recreation) to understand the current and anticipated expectations of residents living in Livingston, including opportunities for active and healthy living. Discoveries from this study can also provide evidence to help inform future planning and decision making related to the built environment and vibrancy within a community as it is being built. A combination of semi-structured interviews with families living in Livingston along with personalized physical activity (PA) behavior assessments and validated questionnaires for both children and adults living in Livingston were used. Several themes emerged from the data analysis including (but not limited to); neighborhood vibrancy, environmental appeal, health and happiness, harmonious living, belonging, and infrastructure. Background: Despite extensive research on HIV and hepatitis C (HCV), there remains a gap in knowledge on the burden. Objectives: to assess the prevalence of hepatitis C virus (HCV) among positive human immunodeficiency virus patients and to study the relationships between these infections and intravenous drug addiction. Methodology: a cross sectional study has been conducted among total of 325 positive immunodeficiency patients (HIV) diagnosed to have HCV infection in the gastroenterology clinic at King Abdul-Aziz hospital and oncology center in Jeddah, Saudi Arabia between January 2015 and January 2016. Conclusion: The high prevalence of these major virulent infections among drug addiction is a sign of a major public health problem. Other than complicating addiction illnesses of patients, they could play major roles in spreading these infections to other intravenous drug addictors.

Pengukuran dilakukan pada kanal Line of Sight (LOS), menggunakan dua buah laptop yang dihubungkan dengan Access Point (AP) standar IEEE802.11g dan IEEE802.11a. Hasil penelitian diperoleh, pada standar IEEE802.11g, transfer rate download dan upload data tertinggi 2.662,54 . KB/s dan 2.549,60 KB/s, dan terendah 484,50 . KB/s dan 477,40 KB/s, sedangkan pada standar IEEE802.11a, transfer rate download dan upload data tertinggi 8.104,68 KB/s dan 5.744,24 KB/s, dan terendah 872,24 KB/s dan 465,38 . KB/s. Pada standar IEEE802.11g, transfer rate download dan upload data pada sinyal terendah hingga di bawah 35% dan 30%, dari transfer rate download dan upload data tertinggi. Pada standar IEEE802.11a, transfer rate download dan upload data pada kualitas sinyal terendah hingga di bawah 20% dan 30%, dari transfer rate download dan upload data tertinggi. Tuberculosis is a public health problem in Morocco, which is difficult to eradicate despite the recognized efficiency of health policies. Aortic aneurysm is rare and lethal complication of spontaneous evolution. Pathophysiological characteristics and the difficulty of early diagnosis worsen the prognosis.

We recently reported that viral DNA could be the primary target of raltegravir (RAL), an efficient anti-HIV-1 drug, which acts by inhibiting integrase. To elucidate this mechanism, we conducted a comparative analysis of RAL and TB11, a diketoacid abandoned as an anti-HIV-1 drug for its weak efficiency and marked toxicity, and tested the effects of the catalytic cofactor Mg(2+) (5 mm) on drug-binding properties. We used circular dichroism and fluorescence to determine drug affinities for viral DNA long terminal repeats (LTRs) and peptides derived from the integrase active site and DNA retardation assays to assess drug intercalation into DNA base pairs. We found that RAL bound more tightly to LTR ends than did TB11 (a diketo acid bearing an azido group) and that Mg(2+) significantly increased the affinity of both RAL and TB11. We also observed a good relationship between drug binding with processed LTR and strand transfer inhibition. This unusual type of inhibition was caused by Mg(2+)-assisted binding of drugs to DNA substrate, rather than to enzyme. Notably, while RAL bound exclusively to the cleavable/cleaved site, TB11 further intercalated into DNA base pairs and interacted with the integrase-derived peptides. These unwanted binding sites explain the weaker bioavailability and higher toxicity of TB11 compared with the more effective RAL.

The chromosomal abnormalities of number and structure or the 22q11.2 and 10p13-14 microdeletions are considered the main causes of congenital heart disease. In our best knowledge, cytogenetics studies on congenital heart diseases (CHD) have not been performed in Algeria. In this study, we will screen for chromosomal abnormalities and microdeletions of 22q11.2 and 10p13 in a cohort of Algerian patients. G-banded by trypsin Giemsa (GTG) and Fluorescent In Situ Hybridization (FISH) techniques have been performed to screen for chromosomal abnormalities and a critical regions 22q11.2 and 10p13-14 respectively in seventy patients with non syndromic congenital heart. GTG technique visualized no chromosomal abnormalities of number and structure in our patients. Moreover, FISH visualizing critical regions 22q11.2 and 10p13-14 respectively did not detect any microdeletion in the chromosomes 10 and 22 respectively of our patients. Our study could suggest that congenital heart defects observed in Algerian patients are not due to chromosomal abnormalities of number and structure nor the 22q11.2 and 10p13-14 microdeletions. For the fist time, we report here cytogenetics analysis of chromosomal abnormalities and the 22q11.2 and 10p13-14 microdeletions in Algerian patients with congenital heart disease. Genetic testing for screening for deletion 22q11.2 and 10p13-14 is not indicated in all patients with isolated conotruncal defects. In addition, conotruncal heart diseases have a multifactorial background like consanguinity and recessive mutations in some genes involved in cardiac morphogenesis. A genetic study to screen for the role of consanguineous marriages and some genes linked to CHD in Algerian population is on going. This study will focus also on health education for the families at risk about the importance of pre-marital genetic counseling. Хромосомные аномалии числа и структуры или микроделеции 22q11.2 и 10p13-14 считаются главными причинами врожденного порока сердца. Насколько нам известно, цитогенетические исследования врожденного порока сердца (CHD) в Алжире не проводились. В настоящей работе проведен скрининг хромосомных аномалий и микроделеций 22q11.2 и 10p13-14 в группе алжирских пациентов. Методы окраски по Гимза (GTG) и FISH были использованы для скрининга хромосомных аномалий и критичных участков 22q11.2 и 10p13-14 соответственно у 70 пациентов с несиндромным врожденным пороком сердца. GTG не выявило хромосомных аномалий по числу и структуре. Более того, изучение участков 22q11.2 и 10p13-14 не показало никаких микроделеций в хромосомах 10 и 22. Наши исследования позволяют сделать вывод, что дефекты, вызванные врожденным пороком сердца у алжирских пациентов, не связаны ни с хромосомными аномалиями числа и структуры, ни с микроделециями 22q11.2 и 10p13-14. Впервые проведен цитогенетический анализ хромосомных аномалий и микроделеций 22q11.2 и 10p13-14 у алжирских пациентов с врожденным пороком сердца. Генетическое тестирование скрининга на наличие делеций 22q11.2 и 10p13-14 не показано у всех пациентов с врожденным пороком сердца. Кроме того, конотрункальные болезни сердца имеют многофакторную основу, такую как генетическое родство и рецессивные мутации некоторых генов, вовлеченных в кардиальный морфогенез. Проводится генетическое исследование роли близкородственных браков и некоторых генов, связанных с врожденным пороком сердца, в алжирской популяции. Это исследование будет также сфокусировано на профилактической работе в семьях с факторами риска и на важности генетического консультирования перед вступлением в брак.

Marfan syndrome is an autosomal dominant disorder with rheumatoid, ophthalmological, neurological, cutaneous and cardiovascular manifestations. Aneurysmal lesions affecting both the abdominal aorta and the peripheral arteries are not often described in the literature. We report a case associating a bilateral popliteal aneurysm and an aneurysm of the infra-renal abdominal aorta.

Poractant-α is the most common surfactant in Europe:1 its recommended dose is 200 mg/kg, according to pharmacokinetic and clinical data.2 We hypothesised that the correct dose is not often followed at the bedside as some clinicians may be tempted to administrate a rounded dose. In fact, a recent multicentre randomised clinical trial has been based on a rounded dose, as the authors acknowledge among the study limitations.3 We conducted a retrospective (2011–2013), population-based, cohort study performed on the formally established neonatal network in Parisian area,4 following Strengthening the Reporting of Observational Studies in Epidemiology guidelines. Eligible babies were fulfilling the following conditions: (1)

OBJECTIVE To verify if preterm neonates transferred between tertiary referral centers have worse outcomes than matched untransferred infants. DESIGN Cohort study with a historically matched control group. SETTING Two tertiary-level neonatal ICUs. PATIENTS Seventy-five neonates per group. INTERVENTIONS Transfer between tertiary-level neonatal ICUs carried out by a fully equipped transportation team. MEASUREMENTS AND MAIN RESULTS We measured in-hospital mortality, frequency of intraventricular hemorrhage greater than 2nd grade, periventricular leukomalacia, necrotizing enterocolitis greater than or equal to grade 2, bronchopulmonary dysplasia, composite outcomes (in-hospital mortality/bronchopulmonary dysplasia, in-hospital mortality/intraventricular hemorrhage > 2nd grade, and bronchopulmonary dysplasia/periventricular leukomalacia/intraventricular hemorrhage > 2nd grade), length of neonatal ICU stay, weight at discharge, and time spent on ventilatory support. Seventy-five similar (except for antenatal steroids administration) neonates were enrolled in each cohort. Cohorts did not differ in mortality, bronchopulmonary dysplasia, intraventricular hemorrhage greater than 2nd grade, periventricular leukomalacia, necrotizing enterocolitis greater than or equal to grade 2, any composite outcomes, neonatal ICU stay, weight at discharge, and duration of respiratory support. Results were unchanged adjusting for antenatal steroids. CONCLUSIONS Neonatal transfer between tertiary-level centers does not impact on clinical outcomes, if performed under optimal conditions.

Resume Les malformations arterio-veineuses (MAV) du membre superieur sont rares. Leur localisation habituelle pres des articulations et leur association aleatoire a des variations anatomiques et a des anomalies nerveuses, musculaires et articulaires rendent leur chirurgie extremement delicate. Les risques esthetiques et les exigences fonctionnelles des resections chirurgicales obligent une prise en charge par des praticiens experimentes. Nous rapportons le cas d’un jeune sportif de vingt ans, qui presentait une association originale d’une MAV du pli du coude et de l’avant-bras avec une exostose epicondylienne et epitrochleenne et un kyste synovial calcifie du coude. Il a beneficie d’une exerese chirurgicale en deux temps avec un bon resultat fonctionnel et sans prejudices esthetiques importants.

Venous aneurysms are a relatively rare pathology, far less common than arterial aneurysms. Unrelated to either age or gender, they can affect any vein, including cervical, thoracic, visceral, and lower limb veins. Aneurysmal dilatations in cervical veins are rare due to low pressure in the vena cava system; they can involve any vein but most frequently are observed on the internal and external jugular veins. This report of three patients highlights some of the specific diagnostic and therapeutic features of this pathology.

Resume L’atteinte arterielle au cours de la neurofibromatose de type 1 est de survenue relativement rare et potentiellement grave de consequences pour le patient. Nous rapportons les observations cliniques de deux cas d’atteintes arterielles peripheriques au cours de la neurofibromatose de type 1. Le premier cas est celui d’une patiente de 25 ans, connue porteuse de la maladie de Von Recklinghausen, qui a presente une augmentation spontanee et douloureuse du volume du bras gauche. L’angioscanner du membre superieur gauche a revele la presence d’un faux anevrisme de l’artere humerale. Le geste chirurgical a consiste en une mise a plat du faux anevrisme et la ligature de l’artere humerale gauche. L’hemostase a ete realisee difficilement. Notre patiente a presente d’importantes hemorragies et une instabilite hemodynamique en per- et en postoperatoire. Le deces est survenu a j2 postoperatoire. Le deuxieme cas est celui d’un patient de 17 ans, porteur de la maladie de Von Recklinghausen, qui a presente une arterite stade II du membre inferieur droit, l’arteriographie a revele la presence d’une stenose longue de l’artere femorale superficielle droite etendue a l’artere poplitee, avec une reprise au niveau des arteres de jambe par une importante circulation collaterale arterielle. L’abstention therapeutique a ete de mise avec une surveillance clinique reguliere.

Resume L’osteochondrome constitue la tumeur osseuse la plus frequente. Exceptionnellement, elle est responsable de complications vasculaires dont les faux-anevrismes constituent l’entite la plus frequente. Nous rapportons le cas d’un jeune patient âge de 20 ans qui presentait une douleur brutale du genou gauche a la suite d’un accident de sport. L’echodoppler arteriel objectivait un faux anevrisme rompu de l’artere poplitee basse. Une arteriographie a permis de confirmer le diagnostic en montrant une extravasation du produit de contraste au niveau de l’artere poplitee basse en regard d’une image radiologique d’osteochondrome. Le traitement etait chirurgical et a consiste en une mise a plat du faux anevrisme avec un remplacement veineux, complete par une resection de la tumeur osseuse. Malgre la rarete de ses complications vasculaires, le diagnostic de l’exostose osteogenique doit etre evoque devant tout anevrisme poplite chez un sujet jeune. Le traitement des faux anevrismes sur exostose est plutot chirurgical.

Resume Nous rapportons le cas d’un patient de 20 ans admis aux urgences pour une masse de la cuisse droite d’apparition spontanee evoluant dans un contexte d’apyrexie. L’examen clinique objectivait une masse d’allure vasculaire. L’angioscanner arteriel des membres revelait un faux-anevrisme aux depens d’une branche de l’artere femorale profonde droite. Le diagnostic de syndrome d’Ehlers-Danlos de type vasculaire a ete retenu sur des criteres cliniques. Le traitement etait chirurgical avec une mise a plat et ligature de la branche nourriciere du faux-anevrisme. Les suites operatoires etaient simples avec une evolution favorable.

Large vessel thrombosis is a very rare clinical presentation of acute leukemia which is usually revealed by hemorrhagic complications or thrombosis of small vessels. We present here the case of a patient with previously undiagnosed acute myeloid leukemia who was referred to our hospital with symptoms of acute ischemia of the left lower limb. Occlusion of the left popliteal artery due to a leucostasis was noted and successfully treated with emergency surgical thromboembolectomy and chemotherapy.

Resume Nous presentons l’observation d’un patient de 42 ans admis en urgence pour une paraplegie d’installation brutale, l’examen trouvait une froideur et abolition des pouls des membres inferieurs. L’echo-Doppler arteriel de l’aorte et des membres inferieurs a objective la presence du materiel embolique au niveau de l’aorte sous-renale. Une embolectomie aorto-iliaque par sonde de Fogarty par abord des deux scarpas a ete realisee. L’echocardiographie transthoracique realisee en postoperatoire a montre une image tissulaire au niveau de l’oreillette gauche suspecte de myxome, l’examen histologique du materiel embolique a confirme le diagnostic d’occlusion aigue sur myxome de l’oreillette gauche. L’evolution a ete marquee par l’installation d’un syndrome de revascularisation, le deces est survenu au quatrieme jour d’hospitalisation. Ce cas souligne l’interet d’une prise en charge precoce et adaptee de l’occlusion aigue de l’aorte, evitant ainsi les consequences de la revascularisation des membres en ischemie severe et prolongee (l’insuffisance renale, l’hyperkaliemie).

Resume La maladie de Behcet est une vascularite d’etiologie inconnue, l’atteinte vasculaire au cours de la maladie est le plus souvent veineuse sous forme de thrombophlebite superficielle ou profonde, l’atteinte arterielle est rare, souvent anevrismale. Nous rapportons deux observations de deux patients dont la maladie de Behcet a ete revelee par un anevrisme du tronc cœliaque. La symptomatologie est non specifique, l’echodoppler et l’angioscanner permettent souvent le bilan lesionnel, les deux malades ont beneficie d’un traitement medicochirurgical. A la lumiere de ces deux observations, nous insistons sur la necessite d’evoquer la maladie de Behcet chez tout patient jeune, porteur d’anevrisme arteriel.

La localisation primaire retroperitoneale du kyste hydatique au niveau du muscle psoas reste exceptionnelle. Les auteurs rapportent l’observation d’un patient de 41 ans, qui presente une masse de la fosse iliaque gauche associee a une fievre. L’imagerie a montre la presence d’une masse kystique developpee aux depens du muscle psoas gauche. Il s’agissait d’un kyste hydatique infecte du psoas. Une perikystectomie partielle a ete realisee grâce a une voie d’abord retro peritoneale. A partir de cette observation et des donnees de la litterature, les auteurs discutent les problemes diagnostiques et therapeutiques du kyste hydatique du psoas.

Resume Introduction L’anevrisme geant de l’artere splenique est exceptionnel. Les auteurs rapportent, a travers une revue de la litterature, les particularites epidemiologiques, cliniques et therapeutiques de cette pathologie. Observation Nous rapportons le cas d’un patient de 62 ans admis pour douleurs epigastriques. L’examen clinique trouve une masse battante de l’hypocondre gauche. L’angioscanner abdominal a retrouve un gros anevrisme partiellement thrombose de l’artere splenique mesurant 10 cm de diametre. L’arteriographie a montre un anevrisme de 10 cm aux depens du tiers moyen de l’artere splenique. Le patient a beneficie d’une mise a plat avec ligature de l’artere splenique proximale et distale. Les suites operatoires etaient simples. Conclusion Les anevrismes geants de l’artere splenique (AAS) sont une entite clinique rare. Ils different des AAS ordinaires par leur terrain de survenue, leur localisation sur l’artere splenique, leurs manifestations cliniques et leur prise en charge therapeutique.

Resume Introduction Les anevrismes de l’artere hepatique sont rares. Le diagnostic est souvent fait au stade de rupture, avec une importante mortalite. Observations Une patiente âgee de 70 ans souffrait de vomissements et de douleurs de l’hypocondre droit. A l’echographie, un anevrisme asymptomatique de l’artere hepatique a ete diagnostique. Le traitement a consiste en une mise a plat de l’anevrisme suivi d’un pontage splenohepatique droit avec une evolution postoperatoire satisfaisante. Le second cas concerne un patient de 54 ans, pour lequel le diagnostic d’un anevrisme de l’artere hepatique droite a ete fait au stade de rupture dans la vesicule biliaire. Discussion A la lumiere de ces 2 cas et des donnees de la litterature, nous analysons l’incidence, les causes, les moyens diagnostiques et therapeutiques de cette affection.

Resume Propos L'atteinte des troncs supra-aortiques dans la maladie de Takayasu realise souvent des lesions diffuses et complexes inaccessibles au traitement endovasculaire. La chirurgie occupe par consequent une place importante dans la prise en charge de l'ischemie cerebrale dans cette maladie. Le but de ce travail est de preciser les indications et les techniques chirurgicales en cas d'atteinte des arteres a destinee encephalique dans cette maladie et d'exposer nos resultats. Materiel et methodes Nous rapportons une serie de sept cas de maladie de Takayasu avec atteinte des arteres carotides extracrâniennes traites chirurgicalement dans le service de chirurgie vasculaire de l'hopital Ibn-Sina de Rabat sur une periode de 11 ans. Resultats Il s'agit de six femmes et un homme d'âge moyen au moment du diagnostic de 33,8 ans. Les signes revelateurs etaient essentiellement d'ordre neurologique et oculaire. Six de nos patients etaient en poussee inflammatoire au moment du diagnostic, et ont necessite un traitement medical premier a base de corticoides. Il a ete realise une revascularisation carotidienne bilaterale dans trois cas, unilaterale dans quatre cas. L'axe donneur etait l'aorte ascendante dans six cas et le tronc arteriel brachiocephalique dans un cas. Un deces en rapport avec une hemorragie cerebrale est survenu deux jours apres la revascularisation. Une nette amelioration des signes fonctionnels etait notee chez trois patients, alors que l'amelioration etait partielle chez deux autres. Une thrombose secondaire du pontage est survenue dans trois cas. Conclusion L'histoire naturelle de la maladie de Takayasu et son evolution sont mal connues. Les indications operatoires ne doivent pas reposer uniquement sur le seul bilan anatomique, mais basees sur un faisceau d'arguments dans lesquels, l'evaluation du debit sanguin cerebral serait utile. Le syndrome d'hyperperfusion cerebrale constitue un risque majeur qui peut etre reduit par des revascularisations par etapes.

A novel, low-profile filter embolic protection device was deployed in 26 patients who underwent stent deployment for saphenous vein graft stenoses in a multicenter trial. Major adverse cardiovascular events were observed in only 2 patients (7.7%) and angiographic flow grades were improved.

Resume La plupart des recommandations concernant la reanimation du nouveau-ne en salle de naissance reposent sur l’experience et n’ont pas ete scientifiquement etablies. C’est le cas de l’utilisation de l’oxygene pur. Aujourd’hui, ces pratiques sont remises en question, a la lumiere des consequences de l’hyperoxie et de plusieurs meta-analyses recentes. En cas d’anoxie perinatale moderee chez le nouveau-ne a terme, une ventilation avec de l’air peut ainsi etre proposee en premiere intention. Cependant, de nouvelles etudes randomisees multicentriques, avec stratification selon la cause de l’asphyxie et l’âge gestationnel, sont necessaires avant d’envisager des recommandations du meme type pour les nouveau-nes presentant les formes les plus severes d’asphyxie perinatale ou chez les prematures. Dans les situations les plus graves, l’introduction secondaire d’oxygene, a une concentration mesuree et sous controle permanent de la saturation par oxymetrie pulsee, parait pour l’instant de bonne pratique.

A thermodynamic study of 4 liquid crystals with DOE substituents using gas chromatography are carried out. The 4 compounds have the same mesogenic part; they differ by the partial or total substitution of the terminal chain heptyloxy with 2-(2-methoxyethoxy)ethoxy (DOE). The thermal properties of the 4 compounds are determined by differential scanning calorimetry. The knowledge of the gas chromatographic specific retention volume of a serial geometric isomers obtained by using the 4 liquid crystals as stationary phases allow to calculate their thermodyamic data: activity coeffients, enthalpies and entropies of solution. In the other hand, the support influence on the thermal properties are investigated. The results founds show that this parameter is not negligible but disappear when the coating increases. For silica, the transitions (melting, clarification) are apparent only when the coating is higher than 50%. With Chromosorb P, 5% of coating are sufficient to observe the transitions.

The thermodynamics of four new liquid crystals were investigated in order to understand their selectivity as stationary phases in gas chromatography. In this case study, liquid crystals with a benzoyloxy azobenzene mesogenic core substituted with heptyloxy (C7) and/or dioxyethylene ether (DOE) groups, were used. The chromatographic separations of linalool and citronellal, and of xylene, tetraethylbenzene and cresol isomers, which were achieved with the liquid crystal stationary phases, have been related to the dissolution thermodynamics of the solutes.
