Objectives: This study aims to evaluate self-care practices, sociodemographic and clinical factors that affect self-care and patient education among women with breast cancer-related lymphedema (BCRL). Patients and methods: This descriptive, cross-sectional study included a total of 102 women with BCRL (median age: 59 years; range, 35 to 80 years) who received lymphedema (LE) treatment at least once between July 2014 and May 2016. A Sociodemographic and Clinical Characteristics Form and the Lymphedema Self-care Survey were used to collect data via face-to-face interviews. Results: The median LE self-care practices score for women was 10 (range, 5 to 14). A total of 39.1% of the women implemented regular self-care. A statistically significant relationship was found between the score for perceived benefit of LE self-care and the score for self-care practice. No statistically significant difference was found among the self-care scores of the women with LE in terms of sociodemographic and clinical factors, except for education status. A total of 90.2% of the women with LE received self-care education, mostly from a physical therapy specialist and a physiotherapist. There was a statistically significant difference among self-care scores between patients who were educated and uneducated about LE. Conclusion: It is recommended that healthcare professionals should educate patients diagnosed with breast cancer to reduce LE risk and promote the implementation of self-care practices following the breast cancer surgery. Interventions should be made to increase the perceived benefits and reduce the perceived barriers and burden towards self-care behaviors to prevent and manage LE.

Objectives: This study aims to investigate the effect of complex decongestive therapy (CDT) on the kinesthetic sense of hands, upper extremity function, and the quality of life in patients with breast cancer-related lymphedema (BCRL). Patients and methods: Between August 2018 and August 2019, total of 50 women with BCRL (mean age: 56.5±9.6 years; range, 36 to 71 years) were included in the study. Kinesthetic sense of the hand, upper extremity function (Disabilities of the Arm, Shoulder and Hand [DASH]), quality of life (European Organization for Research and Treatment of Cancer Quality of Life Core Questionnaire [EORTC QLQ-C30]), and arm volume of all patients were evaluated before and after the treatment. All patients received CDT for 20 sessions for 1 h over a total of four weeks. Results: A statistically significant decrease in the volume of the involved extremity was observed after the treatment (p Conclusion: Our study results show that BCRL is a serious complication which may lead to impairment in the kinesthetic sense of hand and upper extremity function with the increased arm volume. The CDT is an effective and safe method not only to achieve significant volume reduction in the extremities, but also to achieve favorable results in managing these problems.

Giriş: Koronavirüs tüm dünyada yaşamları olumsuz etkilediği gibi, kronikhastalıkların yönetimini de etkilemiştir.Amaç: Meme kanserine bağlı lenfödem (MKBL)gelişen kadınlarda Covid-19 korkusu ile lenfödem öz bakım ilişkisininincelenmesidir.Gereç ve Yöntem: Araştırma tanımlayıcıve kesitsel tiptedir. Evrendeki 235 kadından MKBL gelişen, 18 yaş üzeri,okuryazar 103 kadın örneklemi oluşturmuştur. Dışlanma kriterleri; işitme,kognitif ve psikiyatrik problemlerin olması, kemoterapi ve radyoterapi alıyorolmasıdır. Veriler sosyodemografik-klinik özellikler formu, Covid-19 Korkusu Ölçeği(CKÖ) ve Meme Kanserine Bağlı Lenfödem Özbakım Ölçeği (MKBLÖÖ) kullanılaraktelefonla toplanmıştır. Yanıt verme oranı %43.83’tür. Çalışma için etik kurul,kurum, Sağlık Bakanlığı izinleriyle katılımcılarınonamı alınmıştır. Veriler normal dağılıma sahip (ShapiroWilk:.934,p sayı, yüzde, ortanca, MannWithney U, Kruskal Wallis, spearman korelasyon testleriyle değerlendirilmiştir.Bulgular: Katılımcıların yaş medyanı 54(min:35-max:76), %47.60’ı üniversite mezunu, %63.10’u çekirdek ailesiyleyaşıyordu. %7.80’i Covid-19 geçirmişti, %55.30’unun yakınlarında Covid-19 tanı/şüphesivardı, %10.70’i yakınını Covid-19 nedeniyle kaybetmişti, %51.50’si pandemi sonrası kontrollerinisürdürmemişti. Yaşanılan yere (KW:7.776,p:.02), birlikte yaşadığı kişilere(KW:10.812,p:.01), pandemi sonrası kontrollere gitme sıklığına(KW:18.884,p:.001), yakınlarında Covid-19 tanısı/şüphesi olma durumuna(KW:2.272,p:.025), ve pandemi süresince hastalığı yönetme algısına(KW:16.599,p:.002) göre CKÖ ortalamalarında; algılanan maddi duruma(KW:13.714,p:.001), lenfödemin bölgesine (z:2.085,p:.04), kontrolleri sürdürmeşekline (KW:8.724,p:.01) ve sıklığına (KW:20.890,p:.00) ve pandemi süresincehastalığı yönetme algısına (KW:10.010,p:.04) göre MKBLÖÖ puan ortalamalarıarasında istatistiksel 0anlamlı farklılık vardı. CKÖ ile MKBLÖÖ puanortalamaları arasında anlamlı ilişki saptanmamıştır. Açık uçlu sorulardanpandemi süresince kendinizi nasıl hissedersinize 43 kişi iyi hissettiği yönündebeyan verirken, diğerleri kötü, endişeli, korkmuş, yorgun ve yalnız hissettiğinibelirtmiştir. Lenfödem özbakımını pandemi süresince 51 işi olumsuzetkilendiğine dair ifadelerde bulunurken, diğerleri pandeminin ekstra biretkisinin olmadığını belirtmişlerdir. Pandemi sürecinde yaşanılan zorluklarsorulduğunda 22 kişi zorluk yaşamadığından bahsederken, hastaneden korkma,hastanede randevu bulamama, kontrole gidememe, lenfödemin kötüleşmesi, gelirdurumunda sıkıntı yaşama gibi birçok problemi sıralamışlardır.Sonuç: Koronaviruskorkusu ile MKBL’li kadınlarda özbakım arasında anlamlı ilişki olmadığı, ancakkadınların pandemi süresince lenfödem takibi ile ilgili çeşitli zorluklarlakarşılaştıkları bulunmuştur. Amaç: Bu çalışma, farklı parsiyel larenjektomi (PL) tekniklerinin yutma üzerindeki etkilerini değerlendirmek ve karşılaştırmak amacıyla tasarlandı. Gereç ve Yöntemler: On hastada kordektomi ile laringofissür, onunda epiglotik rekonstrüksiyon (FAL) ile frontal anterior larenjektomi, on hastada frontolateral larenjektomi (FLL), on hastada krikohiyoidopeksi (CHP), on hastada krikohiyoidoepiglottopeksi (CHEP) ve diğer onunda supraglotik larenjektomi operasyonu yapılmıştı. Yutma fleksible endoskopi ile değerlendirildi. Bulgular: Katı yiyecekler için hafif veya orta derecede disfaji, CHP hastalarında FLL ve FAL hastalarına kıyasla anlamlı olarak daha sık olduğu bulundu (p0,05). Objectives: the aim of this study was to evaluate the validity and reliability of the Turkish version of the Brief Pain InventoryShort Form for patients with chronic nonmalignant pain. Methods: An analytical design was used. A total of 192 patients were included in the study. A demographic questionnaire and the Brief Pain Inventory-Short Form were used to collect data. Content validity was assessed by experts and construct validity was tested using exploratory factor analysis. Reliability analyses estimated the internal consistency and test-retest reliability. Cronbach’s alpha and the item-total correlations were calculated for the subscales to examine internal consistency. Results: Exploratory factor analysis yielded 2 factors: pain severity and pain interference, which accounted for 68.81% of the total variance. the coefficient alpha of both subscales demonstrated good internal consistency. the item-total correlations of the scale ranged between 0.56 and 0.87. the test-retest reliability was r=0.774 for pain severity and r=0.808 for pain interference (p=0.001). Conclusion: the Turkish version of the Brief Pain Inventory-Short Form is a valid and reliable instrument to assess chronic nonmalignant pain., Amaç: Bu çalışmanın amacı, kronik nonmaling ağrısı olan hastalarda Brief Ağrı Envanteri Kısa Formu’nun Türkçe versiyonunun geçerliğini ve güvenirliğini değerlendirmektir. Gereç ve Yöntem: Analitik çalışma deseni kullanıldı. Çalışmaya 192 hasta dahil edildi. Veri toplamada sosyodemografik soru formu ve Brief Ağrı Envanteri Kısa Formu kullanıldı. Envanterin geçerliğini değerlendirmek için içerik ve yapı geçerliği kullanılmıştır. Içerik geçerliği uzman görüşü ile değerlendirildi. Yapı geçerliği için açımlayıcı faktör analizi kullanılmıştır. Güvenirlik analizi için iç tutarlılık ve test tekrar test güvenirlik analizleri yapılmıştır. İç tutarlılık için alt boyutların Cronbach alfa ve maddetoplam korelasyonları hesaplandı. Bulgular: Açımlayıcı faktör analizi 2 faktör göstermiştir, ağrı şiddeti ve ağrı girişimi. İki faktör toplam varyansin %68.81’ini açıklamıştır. Her iki alt boyutun alfa katsaysı iyi iç tutarlılık göstermiştir. Ölçeğin madde toplam korelasyonları 0.56 ile 0.87 arasındadır. Test–retest güvenirliği ağrı şiddeti için r=0.774 ve ağrı girişimi için r=0.808’dir (p=0.001). Sonuç: Brief Ağrı Envanteri Kısa Formu’nun Türkçe versiyonu, kronik nonmaling ağrıyı değerlendirmek için güvenilir ve geçerli bir araçtır.

Osteoarthritis (OA) is the most common form of arthritis. Despite the many challenges presented by clinical trials in OA, improvements in our understanding of disease pathogenesis and a move to treat pain, as well as underlying disease process, mean there are now many new pharmacological therapies currently in various stages of clinical trials. Some challenges remain, but opportunities for improving symptoms and disease process in OA in the clinic with new pharmacological agents would appear to be on the close horizon, Osteoartroz (OA) en yaygın artrit şeklidir. OA'da klinik çalışmaların sunduğu birçok zorluğa rağmen, hastalık patogenezi, hastalık süreci ve ağrının tedavisinin yanı sıra altta yatan hastalığın tedavisinde birçok yeni farmakolojik tedaviler ve klinik araştırmalar vardır. Yeni farmakolojik ajanlar ile klinikte OA'da semptomların ve hastalık sürecinin iyileştirilmesine yönelik fırsatlar ufukta görünmektedir.

Objective: To investigate the outcome of complete decongestive therapy (CDT) on primary and secondary lymphedema of the lower limb (LL). Material and Methods: Included 40 patients with LL lymphedema were retrospectively analyzed. Age, gender, diagnosis, extremity volume, subtype of lymphedema, radiotherapy chemotherapy history of all the patients were noted. the amount of limb edema was calculated with volumetric measurements before and after the treatment for each patient. Bilateral circumferential measurements were carried out at the level of metatarsophalangeal joints, mid-dorsum of the feet, ankle and every 10 cm till the inguinal level. Afterwards, a computer program was used to convert these values into limb volumes in milliliters. Results: It is found out that CDT causes a significant improvement in the average volume of the LL p=0.01). Nonetheless, the statistics showed significance (p= 0.008) only in secondary group. Moreover, there were no significant difference in percentage changes betwen both groups showed no significance after treatment (p>0.05). We also determined that initial volumes were significantly correlated with volume reduction rates (r= 0.670, p= 0.000). Conclusion: CDT is an effective, safe and well tolerated treatment for lymphedema of lower extremity limb. There was significant improvement in clinical outcomes particularly in secondary lymphedema group; whereas, percentage change in limb volumes of both groups showed no significance. Hence, our results suggest that the volume reductive effects of CDT is not correlated with the lymphedema (ethiology). Future studies comprising greater population with various ethiology are needed., Amaç: Primer ve sekonder alt ekstremite lenfödeminde kompleks dekonjestif tedavinin (KDT) etkinliğini araştırmak. Gereç ve Yöntemler: Çalışmaya dahil edilen 40 alt ekstremite lenfödemi tanılı hastanın verileri retrospektif olarak analiz edildi. Hastaların yaş, cinsiyet, tanı, ektremite volümü, lenfödem subtipleri ve kemoterapi/radyoterapi öyküleri kayıt edildi. Bacaklardaki ödem tedavi öncesi ve tedavi sonrasında volümetrik ölçümler ile hesaplandı. Ayak sırtından başlayarak 10'ar cm aralıklarla inguinal seviyeye kadar bilateral olarak çevresel ölçümler yapıldı. Ardından bilgisayar programı ile bu ölçümler volümetrik değerlere çevrildi. Bulgular: KDT'nin alt ekstremite lenfödem tedavisinde anlamlı gelişmeler sağladığı görüldü (p=0,01). Ancak istatiksel anlamlılığa yalnızca sekonder lenfödem grubunda ulaşıldı (p=0,008). Bununla birlikte yüzde değişim oranlarına bakıldığında iki grup arasında anlamlı farkın olmadığı saptandı (p>0,05). Tedavi öncesindeki volümlerin, volüm azalma oranları ile anlamlı oranda korele olduğu da (r=0,670, p=0,000) saptandı. Sonuç: KDT, alt ekstremite lenfödem tedavisinde etkili, güvenilir ve iyi tolere edilebilen bir tedavidir. Ancak her nekadar istatiksel anlamlılığa sekonder lenfödem grubunda ulaşılmış olsa da yüzde değişim oranlarına bakıldığında gruplar arasında anlamlı farklılığın olmadığı görülmüştür. Bu nedenle KDT'nin volüm azaltıcı etkisinin lenfödem etyolojisi ile korele olduğunu söyleyememekteyiz. Değişik etyolojilere sahip bireylerle yapılacak daha çok sayıda hastayı içeren prospektif çalışmalara ihtiyaç olduğunu düşünmekteyiz.

The number of individuals with serious or life-threatening diseases is gradually increasing because of the aging population and technologies that enable longer life expectancy. Palliative care programs are developed in order to improve quality of life during the course of these diseases. In palliative care, service is provided by experienced and educated healthcare personnel and a team of volunteers, and in hospital, first line treatment environment, house or hospices, depending on the conditions of each country. Intensive care units (ICUs) are unique environments where patients with critical condition receive rapid and aggressive life-saving interventions. In developed countries physical therapy and rehabilitation programs are integrated parts of intensive care units in many hospitals. Considering patient needs and complications in intensive care units, the importance of improving patients’ quality of life emerges as an issue to be kept in mind. Physical therapy and rehabilitation programs aim to maintain optimum respiratory and circulatory function in these patients, prevent muscular atrophy, muscular problems and joint contractures, control pain, optimize functioning and independency, educate patients and enable the participation of the caregiver. Physical therapy and rehabilitation specialists, physiotherapists and occupational therapies work together in a multidisciplinary palliative care team in order to increase patients’ mobility, independency and quality of life. Unfortunately it is observed that physical therapy and rehabilitation programs are largely ignored during the distribution of palliative care resources and that necessary transfer of funds is not provided. In this study we aim to highlight the role, necessity and benefit of rehabilitation, one of the important components of palliative care, in intensive care units. Yaşlanan popülasyon ve yaşam süresinin artmasını sağlayan teknolojiler nedeniyle ciddi veya yaşamı tehdit edici hastalıklara sahip bireylerin sayısı giderek artmaktadır. Bu hastalıklar sırasında yaşam kalitesini arttırmak için artan bir şekilde palyatif bakım programları geliştirilmektedir. Palyatif bakımda hizmet, deneyimli ve eğitimli sağlık personeli ve gönüllülerden oluşan bir ekip tarafından sunulmakta ve her ülkenin kendi koşulları içerisinde; hastane ortamında, birinci basamak sağlık alanlarında, ev ortamında ve son dönem evleri veya hospislerde verilmektedir. Yoğun bakım üniteleri, kritik hastaların hızlı ve hayat kurtarıcı müdahaleler aldığı özgün bakım ortamlarıdır. Fizik tedavi ve rehabilitasyon programları gelişmiş ülkelerde pek çok hastanede yoğun bakım ünitelerine entegre olmuş bir bölüm olarak yer almaktadır. Yoğun bakım ünitelerinde hastaların ihtiyaçları ve komplikasyonları göz önünde bulundurulduğunda, bu hastaların yaşam kalitelerinin iyileştirilmesinin önemi akılda tutulması geren bir durum olmalıdır. Fizik tedavi ve rehabilitasyon programları ile hastalardaki optimum respiratuar ve sirkulatuar fonksiyonun sürdürülmesi, kas atrofisi ve kas kısalmasının, eklem kontraktürlerinin önlenmesi, ağrının kontrol edilmesi ve giderilmesi, fonksiyonun ve bağımsızlığın optimize hale getirilmesi, hastaların eğitimi ve bakıcının katılımı hedeflenmektedir. Fizik tedavi ve rehabilitasyon uzmanları, fizyoterapistler ve iş-uğraşı terapistleri multidisipliner palyatif bakım ekibi içerisinde hastaların mobilitesinin, bağımsızlığının ve yaşam kalitesinin arttırılmasına yardımcı olmak için birlikte çalışmaktadırlar. Maalesef palyatif bakım kaynaklarının dağıtımı sırasında fizyoterapi ve rehabilitasyon olasılıklarının yüksek oranda göz ardı edilmekte olduğu ve gerekli kaynak aktarımının yeterince yapılmadığı gözlenmektedir. Biz bu yazımızda palyatif bakımın önemli bir bileşeni olan rehabilitasyonun, yoğun bakım ünitesindeki rolünü, gerekliliğini ve sağladığı faydanın vurgulanması amaçlamaktayız.

Objective: in our study, we aimed to evaluate the influence of training on compliance and persistencewith bisphosphonate treatment given on a weekly vs. monthly basis in postmenopausal osteoporosispatients.Methods: A total of 979 patients with postmenopausal osteoporosis (mean age: 63.2± 7.2 years) wereincluded in this national, multicenter, prospective non-interventional observational cohort registrystudy. Patients were randomized into training (n¼ 492, 50.3%, mean age: 63.4 ± 7.2 years) and control(n¼ 487, 49.7%, mean age: 63.0 ± 7.1 years) groups. Patients in each intervention group were givenweekly (44.9% and 44.6% for training and control subjects, respectively) or monthly (55.1% and 55.4%,respectively) bisphosphonate regimens. After the initial visit, patients were followed up at three-monthintervals throughout 12 months of treatment for evaluation of persistence, compliance and adverseevents.Results: on average, 79.4% of the patients were persistent with the treatment with a mean of 350.4 daysof duration during the 12-month follow-up period. the mean compliance in the compliant and fullycompliant group remained at an average of 86.6%. No signiŞcant difference was detected between thetraining and control groups in terms of compliance and persistence. SigniŞcantly longer persistence(360.0± 89.0 vs. 345.0 ± 108.0 days; p ¼ 0.035), higher percentage of persistent patients (83.4% vs. 74.2%;p¼ 0.012) and higher compliance rates (88.8% vs. 83.3%; p ¼ 0.002) were noted in monthly regimenpatients in comparison to those given weekly regimen

For effective pain treatment, it is important to determine pain prevalence, its nature, and the factors affecting it. However, epidemiologic information on neuropathic pain in the elderly is inadequate. In our cross-sectional multicenter study, we aimed to determining the prevalence of neuropathic pain in elderly patients and the relationship of neuropathic pain with socio-demographic and clinical factors. Materials and Method: Thirteen centers in different regions of Turkey. The study included 1163 individuals over age 65. Physicians conducted face-to-face interviews to obtain clinical and socio-demographic data and The Douleur Neuropathic 4 (DN4) and The Self-completed Leeds Assessment of Neuropathic Symptoms and Signs (S-LANSS) pain scales were used to assess neuropathic pain. Patients who scored >= 4 or >= 12 on the DN4 and S-LANSS scales, respectively, were determined to be experiencing neuropathic pain. Results: Neuropathic pain was found in 52.5% of the patients (n=610) in this study. Approximately 67.5% of the patients with neuropathic pain were in the 65-74 age group, and 72.1% (n=440) were females. Of the patients who were experiencing neuropathic pain, 48.4% were graduates of primary school, 91.6% engaged in very little or no physical activity, and 56.7% were taking four or more medications. Conclusions: Neuropathic pain prevalence was 52.5% in the elderly over age 65 who had presented with pain complaints. Neuropathic pain was more frequently seen in women, patients with comorbidities, those with poor levels of ambulation, those using walking aids, and those using multiple drugs.
