The objective of this study was to determine the influence of hemodynamic force on the development of type III endoleak and branch thrombosis after complex endovascular thoracoabdominal aortic aneurysm repair.Patients with thoracoabdominal aortic aneurysm, within surgical range, treated with a fenestrated or branched endovascular aneurysm repair from 2014 to 2018 and with 3-month control computed tomography angiography were selected. Demographic variables, aneurysm anatomy, and endograft conformation were analyzed retrospectively from a prospective registry. The hemodynamic force was calculated using the mass and momentum conservation equations.Twenty-eight patients were included; the mean follow-up period was 24.7 ± 19.3 months. There were 102 abdominal vessels successfully catheterized (19 celiac arteries, 29 superior mesenteric arteries, 27 right renal arteries, 26 left renal arteries, and 1 polar renal artery). The rate of type III endoleak was 11.5% (n = 12); six cases were associated with branches that received two stents (P .001). A higher rate of endoleak was observed with wider stents (8.50 ± 1.0 mm vs 7.17 ± 1.3 mm; P = .001) but not with longer stents (P = .530). All cases of type III endoleak affected visceral arteries (eight celiac arteries and four superior mesenteric arteries). The freedom from type III endoleak at 24 months was 86%. The rate of thrombosis was 5.9% (n = 6). A higher rate of thrombosis was observed in smaller vessels (5.00 ± 1.3 mm vs 7.16 ± 1.8 mm; P = .001), with higher stent oversizing (36.87% ± 23.6% vs 5.52% ± 15.0%; P .001), and with a higher angle of curvature (124.33 ± 86.1 degrees vs 57.71 ± 27.9 degrees; P .001). All cases of thrombosis were related to renal arteries (two left renal arteries, two right renal arteries, and two polar renal arteries). The freedom from thrombosis at 24 months was 92%. The area under the curve for the angle of curvature was 0.802 (95% confidence interval, 0.661-0.943; P = .013), and the cutoff point was established at 59.5 degrees (sensitivity, 100%; specificity, 60.4%). The receiver operating characteristic curve for the stent oversize showed an area under the curve of 0.903 (95% confidence interval, 0.821-0.984; P = .001), and the cutoff point was 14.5% (sensitivity, 100%; specificity, 77.1%). A higher hemodynamic force was associated with thrombosis (23.35 × 10Longer stents should be preferred to avoid type III endoleak. A higher angle of curvature leads to a higher hemodynamic force that results in a higher rate of thrombosis. Accordingly, we recommend maintaining the angle of curvature under 59.9 degrees. Small vessels and excessive stent oversizing entail a higher risk of thrombosis; as such, we advise a maximum stent oversize of 14.5%. Renal arteries are more susceptible to thrombosis, whereas visceral arteries are more prone to endoleak.

To the Editor, Surgery of aortic arch diseases like aneurysms and dissections require invasive procedures that include cardiopulmonary bypass, selective cerebral perfusion, aortic cross-clamping, and hypothermic circulatory arrest.1 Unfortunately, a large number of these patients are considered non-eligible for open surgical repair and receive medical therapy only. The NEXUS Stent Graft System (Endospan Ltd., Israel) is indicated to treat high-risk surgical patients with ascending aorta and aortic arch diseases.2 This device received European CE mark approval back in March 2019 for the endovascular repair of aortic arch disease including both aneurysms and dissections requiring landing in the zone 0. The NEXUS Stent Graft System is a dual module device that is inserted through a 20-Fr delivery system. The integrated brachiocephalic artery branch avoids the possibility of branch separation and the interlocking latch mechanism avoids the migration or separation of the modules. This article describes the implantation of a NEXUS branched aortic stent graft system in Spain for the first time. Patient's written informed consent for publication was obtained. In order for patients to comprehend health related information, it must be written at a level that can be readily understood by the intended population. During 2021 the European Society for Vascular Surgery (ESVS) published a sub-section about information for patients into its Guidelines on the Management of Venous Thrombosis.Nine readability measures were used to evaluate the patient educational material regarding venous thrombosis published by seven medical societies: ESVS, Society for Vascular Medicine (SVM), Society for Vascular Surgery (SVS), Vascular Society for Great Britain and Ireland (VS), Australia and New Zealand Society for Vascular Surgery (ANZSVS), Canadian Society for Vascular Surgery (CSVS) and American Heart Association (AHA).The mean reading grade level (RGL) for all the 58 recommendations was 10.61 (range 6.4-14.5) and the mean Flesch Reading Ease (FRE) was 56.10 (51.3-62.9), corresponding to a "fairly difficult" reading level. The mean RGL of the ESVS recommendations (11.45, 95% CI, 9.90-13.00) was significantly higher than the others. Post-hoc analysis determined a significant difference between the ESVS and the SVS (10.86, 95% CI, 9.84-11.91) recommendations (P=0.005). All the patient's education information published by the medical societies presented a RGL higher than recommended. The fifteen sub-sections of the information for patients included into the ESVS clinical guidelines presented a mean RGL above 9.5 points, revealing that no one (0%) was written at or below the recommended GRL. The mean FRE was 47.63 (28.2-61.6), corresponding to a "difficult" reading level.Venous thrombosis patient educational materials produced by leading medical societies have readability scores that are above the recommended levels. The innovative patient's information included into the ESVS venous thrombosis guidelines represents an important advance in the amelioration of the medical information for patients, but their readability should be improved to adapt the understanding to the general population.

Reports our experience for the first 50 cases of use of Supera stent for popliteal atherosclerotic lesions treatment.This prospective single-arm trial enrolled the first 50 limbs (46 patients) treated in our center with a Supera stent for symptomatic atherosclerotic disease of the popliteal artery. Also, a follow-up of 36 months is reported.The mean preoperative ankle-brachial index increased from 0.38 ± 0.37 before intervention to 0.63 ± 0.46 postoperatively at 12 months (P = 0.014), 0.66 ± 0.39 at 24 months (P = 0.023), and 0.74 ± 0.46 at 36 months (P = 0.029). Primary patency at 12, 24, and 36 months were 89.6%, 72.3%, and 70.2%, and primary assisted patency was 93.8%, 87.2%, and 85.1%, respectively, by Kaplan-Meier estimates. After 3 years of follow-up, age75 years (P = 0.034) and Trans-Atlantic Inter-Society Consensus (TASC)-II D lesions (P = 0.041) constituted risk factors for stent occlusion. During the follow-up of the patients, none of them developed stent fracture as evidenced on plain radiographs during follow-up. Implantation defects did not constitute a risk factor for stent restenosis.Based on the results of this study, Supera stent has shown an excellent 3-year patency in this difficult-to-treat anatomic segment. In our experience, younger patients and complex lesions (TASC-II type D) were positive significant predictors for restenosis. In our cohort, post deployment stent conformation did not influence patency. Nevertheless, further investigations, including randomized studies comparing the Supera stent with other stent platforms, drug eluting angioplasty, or atherectomy devices, are mandatory.

Type II endoleak relates to aneurysm perfusion through a patent branch vessel. Reintervention for type II endoleak should be considered in the presence of significant aneurysm growth. Recurrences and subsequent reinterventions are frequent by occult type II endoleaks through feeder arterial branches. We report a case of a patient with a type II endoleak due to inferior mesenteric artery (IMA) patency associated with aneurysm sac growth after an unsuccessfully attempt of transarterial embolization. Laparoscopic ligation of the IMA with direct sac puncture embolization was performed. The postoperative and 1-year follow-up computed tomography angiography scan demonstrated no endoleak signs and aneurysm sac shrinkage. The proposed modification of this technique constitutes a novel approach to this entity. Total laparoscopic IMA ligation and direct sac puncture embolization technique may increase the success rate for the treatment of endoleaks type II by excluding the recurrences. This technique may offer a safe, feasible, and minimally invasive approach for type II endoleaks when other endovascular techniques are unsuccessful.

BACKGROUND Intima media thickness (IMT), vascular calcifications and ankle-brachial pressure index (ABPI) were shown to be independent predictors of mortality in end-stage renal disease (ESRD) patients. METHODS Between January 2018 and March 2019, a physician-initiated, nonrandomized, prospective study was conducted. Carotid IMT, vascular calcifications analysis values and measurements of the ABPI, were made at baseline and after 1-year of follow-up. RESULTS A total of 284 patients, 152 patients with dialysis (100 hemodialysis (HD) and 52 CAPD (continuous ambulatory peritoneal dialysis)) and 132 patients with stage 4 chronic kidney disease (control group), were included (55% male, 67 [29-88] years). The values of carotid IMT at baseline were higher in the HD group than in the CAPD group (1.10±0.08 mm vs. 0.08±0.04 mm, P=0.004). BACKGROUND A pro-inflammatory state and a poor nutritional status have been associated with severity and prognosis of patients with peripheral arterial disease (PAD). The clinical applicability of the different pre-operative nutritional and inflammatory biomarkers in patients with critical limb-threatening ischemia (CLTI) was analyzed. METHODS A retrospective observational study was performed, that included all patients with CLTI revascularized from January 2016 to July 2019. The inflammatory state was calculated using neutrophil/lymphocyte (NLR), lymphocyte/monocyte (LMR) and platelet/lymphocyte ratios (PLR). For nutritional status, the Prognostic Nutritional Index (PNI) was calculated. Mortality and number of major amputations at 6 months and hospital length-of stay were studied. RESULTS 310 patients were included. Higher levels of NLR and lower levels of PNI were associated with mortality (6.61±5.6 vs. 3.98±3.27, P=0.034; 40.33±7.89 vs. 45.73±7.48, P=0.05, respectively). Lower levels of PNI and LMR (42.57±7.82 vs. 45.44±7.65, P=0.036; 2.77±1.61 vs. 3.22±1.75, P=0.013, respectively) and higher levels of NLR (6.91±7.85 vs. 3.94±2.57, P=0.023) were associated with major amputations. The mean hospital length-of-stay was higher in patients with lower levels of PNI and LMR (P=0.000 and P=0.003) and higher levels of NLR and PLR (P=0.001 and P=0.002). A PNI

The aim of the present study was to analyze and report the clinical presentation and treatment at a single center of bull horn vascular injuries (BHVIs) that had occurred during popular celebrations in the past four decades. Thus, we investigated BHVIs in a high-volume academic center in Spain.All the patients with a BHVI admitted between January 1980 and January 2021 were retrospectively enrolled in the present study. Data collection included demographics, injury profile, and outcomes. The primary outcome was in-hospital mortality. The hospital and intensive care unit lengths of stay, rates of reintervention, postoperative complications, and mortality were also analyzed. Data were collected from the electronic and/or digitized medical history records.A total of 296 patients were included in the present analysis. The mean patient age was 33.4 years (range, 17-91 years), and 93.9% were men. Of the 296 patients, 126 (42.6%) had experienced a complication, and 57 (19.3%) had required reoperation. The overall in-hospital mortality was 5.1%. The risk factors for mortality were arterial injury (odds ratio [OR], 5.11; 95% confidence interval [CI], 1.45-23.3; P = .009), injury to the abdominal region (OR, 3.45; 95% CI, 1.11-14.54; P = .038), American Society of Anesthesiologists classification IV (OR, 3.27; 95% CI, 1.87-17.83; P = .004), and age65 years (OR, 3.13; 95% CI, 1.38-10.04; P = .001). Statistically significant differences were found between bull horn arterial injuries and arterial plus concomitant venous injuries in the intensive care unit length of stay (4.9 ± 13.3 days vs 3.1 ± 12.6 days; P = .024), hospital length of stay (12.0 ± 8.2 days vs 10.1 ± 6.3 days; P = .007), postoperative complications (45.2% vs 30.6%; P = .002), and mortality (3.4% vs 1.9%; P = .001).The morbidity and mortality from BHVIs have remained high for the past 40 years. The objective assessment of the radiculopathy secondary to lumbar disc herniation is essential to optimize treatment. The quantitative sensory test (QST) is a useful tool to evaluate somatosensory nerves. The aim of our study is quantifying by QST the alterations of patients treated by epidural injections (EI) or surgical lumbar decompression (LD). A prospective, cohort study has done in Hospital Universitario Rio Hortega, Valladolid, Spain, between January 2014 and December 2016.The study includes 74 patients (40 men) who underwent EI (50) or LD (24) with lumbar disc herniation and treated by EI or LD. Participants underwent a brief battery of QST at baseline and after 1, 3 and 6 months of follow-up. QST threshold were measured in three series of five warm and cold stimuli (cold detection threshold, warm detection threshold, cold pain threshold, heat pain threshold) bilateral. Additionally, pain assessment (Visual Analogue Scale) and neurological examination was performed. Thermal thresholds were analysed and compared. In the EI group, warm detection threshold (WDT) measurements were significantly lower after 3 and 6 months of follow-up (40.44 ± 3.42°C vs. 38.30 ± 3.73°C and 37.48 ± 4.58°C respectively, p = 0.031 and p = 0.043). LD group showed lower WDT measurements at 1, 3 and 6 months of follow up (40.20 ± 2.97°C vs., 37.98 ± 2.04°C, 37.43 ± 3.80°C and 36.55 ± 2.77°C respectively, p = 0.049, p = 0.032 and p = 0.024) and lower heat pain threshold (HPT) levels after 3 and 6 months of follow-up (48.75 ± 1.37°C vs. 43.26 ± 0.60°C and 42.06 ± 1.37°C respectively, p = 0.037 and p = 0.021). QST explorations were compared between both groups. At 1-month follow-up only the WDT parameter was different, higher in EI group (40.98 ± 4.04°C vs. 37.98 ± 2.04°C, p = 0.043). There were no differences in any parameter measured by QST after 3 and 6-months follow-up between both groups. Epidural injection should be considered the first-step of treatment.

Background The aim of this study was to analyze duplex ultrasound (DUS) and intraoperative angiography concordance for diagnosis of lower limb peripheral artery disease and its value for surgical planning. Materials and Methods This was a prospective, observational study, including patients who underwent revascularization of the lower limbs during 2018, diagnosed by DUS only or combined with preoperative computed tomography (CT) angiography. The concordance between preoperative DUS or CT angiography and the intraoperative angiography was studied using the Cohen kappa coefficient (k). The degree of agreement between the DUS-based surgical plan and the final surgical technique was also evaluated. Results Fifty-one patients were included, with mean age of 71.8 ± 11.96 years (46–94); 23 had chronic kidney disease (45%). In 17 patients (34%), preoperative CT angiography was also performed. DUS showed a sensitivity of 100% (95% confidence interval (CI) [83.3–100%]), 80% (95% CI [50.21–100%,]), and 100% (95% CI [96.43–100%]) at the iliac, femoral, and popliteal sector, respectively, and a specificity of 93.55% (95% CI [83.29–100%]), 95.45% (95% CI [84.48–100%]), and 90.48% (95% CI [75.54–100%]) at the iliac, femoral, and popliteal sector, respectively. The positive predictive value for DUS was 60% (95% CI [7.06–100%]), 88.9% (95% CI [62.8–100%]), and 87.5% (95% CI [68.17–100%]) for the iliac, femoral, and popliteal sectors, respectively, whereas the negative predictive value was 100% (95% CI [98.28–100%]), 91.3% (95% CI [77.61–100%]), and 100% (95% CI, [97.37–100%]). The concordance between DUS and intraoperative angiography showed a k index of 0.587 (P = 0.000) in the iliac sector, 0.799 in the femoral sector (P = 0.000), and 0.699 in the popliteal sector (P = 0.000). The concordance between CT angiography/intraoperative angiography had a k index of 0.71 in the iliac sector (P = 0.0093), 0.566 in the femoral sector (P = 0.006), and 0.5 in the popliteal sector (P = 0.028). DUS-based surgical plan was accurate in 86% of cases (n = 44). Conclusions Our experience suggests that DUS arterial mapping of the femoral and popliteal areas is better than CT angiography and can be considered as a unique preoperative imaging test during the surgical planning in patients undergoing a lower limb revascularization procedure.

Introduction Median arcuate ligament syndrome (MALS) is an uncommon condition caused by the extrinsic compression of the celiac trunk (CT) and celiac ganglion, secondary to an anatomical abnormality of the median arcuate ligament fibers. It is characterized by postprandial epigastric pain, chronic abdominal pain, weight loss, nausea and vomiting. MALS is typically diagnosed after the exclusion of other, more common conditions; however, a variety of imaging and diagnostic modalities, including Duplex ultrasonography, computed tomography angiography, magnetic resonance angiography, gastric tonometry and angiography can suggest findings consistent with MALS. Evidence acquisition Laparoscopic approach has been proposed as the option of choice for the treatment of these patients. A systematic review of the international literature regarding this modality of treatment is presented here. Evidence synthesis The review included 504 cases, and several principles are suggested to improve the management. The advantages of this minimally invasive technique include short operative time (136.0 minutes, range 70-242), shorter hospital stay (3.8 days, range 0.5-7) and low rate of conversion to open surgery (4.2%). Conclusions Various treatment modalities are available to decompress the celiac trunk. Although open surgery has been traditionally accepted as the gold standard, laparoscopic division of the MALS has proven equal results. Others have described different treatment modalities, including bypass surgery and endovascular procedures (angioplasty or stent placement). Laparoscopic approaches to correct MALS are feasible and safe. It may be the preferred modality of treatment in view of its lack of morbidity, shorter hospital stay and good results.

Vascular calcification represents a group of several pathological states of differing aetiologies. Mönckeberg medial sclerosis is considered to be more widespread in the lower abdominal region and lower limbs. We present a 59-years-old male patient presented right foot gangrene. At physical exploration, femoral and popliteal pulses were presented and the ankle-brachial pressure index was 0.45, and the toe-brachial index was 0.33. The patient underwent distal angioplasty of anterior and posterior tibial arteries and due to inaccurate evolution a transmetatarsal amputation was required. Mönckeberg’s medial sclerosis is diagnosed with an ABI>1.1, however, questions have been raised about the validity and the role of ABI in diagnosis of Mönckeberg’s medial sclerosis. Colour-doppler vascular ultrasound allow a non-invasive technique widely available to detect vascular calcification and to differentiation between Mönckeberg’s medial sclerosis and the atherosclerosis-related lesions.

Patients unfit for open repair after development a postdissection aortic arch and thoraco-abdominal aneurysm after open ascending aortic replacement for type A dissection would have no other treatment options. We report the case of a 75-year-old man with a type A aortic dissection treated by ascending aortic replacement. The patient presented an enlargement of the downstream aorta and thoraco-abdominal aneurysm 6-months after. This patient underwent a three-stage surgical sessions, open surgery for extraanatomical supra-aortic trunks reconstruction, endovascular branched treatment of ascending, arch and descending aorta and another one to perform an endovascular fenestrated reconstruction of thoraco-abdominal and infrarenal aorta.

Resumen Introduccion Conocer la utilidad de las escalas de riesgo de mortalidad para el tratamiento endovascular de los pacientes con aneurisma de aorta abdominal roto. Disenar una escala de riesgo especifica. Metodos Estudio retrospectivo de 61 pacientes intervenidos mediante reparacion endovascular de aneurisma de aorta abdominal roto entre 2009 y 2014. Se recogieron variables preoperatorias y de mortalidad intrahospitalaria, asi como las escalas Hardman, GAS, Vancouver y ERAS. Resultados La mortalidad intrahospitalaria fue del 45,9%. El estudio univariante obtuvo como factores pronosticos la edad, el sexo varon, la hipertension arterial, el habito tabaquico, la enfermedad pulmonar obstructiva cronica, la tension arterial sistolica 80 anos) + 0,701 (si tension arterial Conclusiones Edad, presion sistolica y frecuencia cardiaca constituyen factores predictores de mortalidad intrahospitalaria de los pacientes con aneurisma de aorta abdominal roto tratados mediante exclusion endovascular. Introduction To determine the usefulness of mortality risk scores for the endovascular treatment of ruptured abdominal aortic aneurysms. This work evaluates the mid-term impact of the addition of large amounts of an organic amendment on the recovery of the physical, chemical and, particularly, the microbiological properties of a marginal semiarid degraded soil and on increasing the soil organic C pool. In order to perform this study, a semiarid degraded soil was treated with composted urban waste at doses equivalent to the addition of 1% (S + CCD1) and 3% (S + CCD2) of organic C (Corg). Changes in soil characteristics in the amended soils were evaluated with respect to a control soil without organic amendment for a period of 5 years after the organic amendment was applied. A spontaneous vegetal cover developed on both amended and unamended soils 3–4 months after the organic amendments were added, yet the level of vegetal biodiversity was lower in the amended plots. Compost-amended soils showed higher concentrations of Corg, water-soluble C and water-soluble carbohydrates than the control soil throughout the experimental period. Furthermore, all of these C fractions were significantly higher (p ≤ 0.05) in S + CCD2 than in S + CCD1 and the control soil. However, compost addition also increased soil electrical conductivity and nitrate content, particularly at the higher dose. Likewise, compost addition produced a 4- to 10-fold increase in soil heavy metal concentrations, although the levels of heavy metal were under the limits allowed in soils. Five years after the organic amendment was added, the soil water holding capacity, stable aggregate percentage, porosity and nutrient and humic substance and humic acid content were greater in amended soils than in control soil, and the higher dose produced greater increases than the lower dose. Soils receiving the highest dose of compost also showed the highest values of basal respiration, dehydrogenase activity and β-glucosidase and phosphatase activity, as well as a greater abundance of total PLFAs, bacterial and fungal PLFAs, and saturated and monounsaturated fatty acids. A greater level of functional diversity was also observed in amended soils, particularly in the soil receiving the higher dose of compost. It can be concluded that the addition of high doses of compost can be a suitable strategy for restoring semiarid degraded soils and for fixing C in these soils, provided that the organic material is of high quality and has a low concentration of heavy metals.

Background Endovascular procedures come with a potential risk of radiation hazards both to patients and to the vascular staff. Classically, most endovascular interventions took place in regular operating rooms (ORs) using a fluoroscopy C-arm unit controlled by a third party. Hybrid operating rooms (HORs) provide an optimal surgical suit with all the qualities of a fixed C-arm device, while allowing the device to be controlled by the surgical team. The latest studies suggest that an operator-controlled system may reduce the radiation dose. The purpose of the present study is to determine the amount of absorbed radiation using an HOR in comparison with a portable C-arm unit and to assess whether the radioprotection awareness of the surgical team influences the radiation exposure. The primary end point was the effective dose in milliSievert (mSv) for the surgical team and the average dose-area product (ADAP) in Gray-meters squared (Gym2) for patients. Methods The values of absorbed radiation of the surgical team's dosimeters were collected from January 2015 to May 2016. The HOR was installed in June 2015, and a radioprotection seminar was given in October 2015. The HOR-issued radiation, measured by the maximum dose-area product, ADAP, average dose (AD) per procedure, maximum dose per procedure per month, maximum fluoroscopy time, average fluoroscopic time, peak skin dose, and average skin dose (ASD), was collected monthly from September 2015 to July 2016. The timeline was divided into 3 periods: 5 months pre-HOR (Pre-HOR), 5 months after the HOR installation (PreS-HOR), and 5 months after a radioprotection seminar (PostS-HOR). Results The average number of procedures per month was 22.55 (±4.9), including endovascular aneurysm repair/thoracic endovascular aneurysm repair, carotid, visceral, and upper and lower limb endovascular revascularization. The average amount of absorbed radiation by the surgeons during PreS-HOR was 1.07 ± 0.4 mSv, which was higher than the other periods (Pre-HOR 0.06 ± 0.03 mSv, P = 0.002; PostS-HOR 0.14 ± 0.09 mSv, P = 0.000, respectively). The ADAP during PreS-HOR was 0.016 ± 0.01 Gym2, which was lower than the PostS-HOR (0.001 ± 0.002 Gym2) (P = 0.034). The AD during PreS-HOR was 0.78 ± 0.3 Gy and 0.39 ± 0.3 Gy during PostS-HOR (P = 0.098). The ASD during PreS-HOR was 0.40 ± 0.2 Gy and 0.20 ± 0.1 Gy during PostS-HOR (P = 0.099). Conclusions In our experience, the HOR increases the amount of absorbed radiation for both patients and surgeons. The radioprotection seminars are of utmost importance to provide a continued training and optimize the use of ionizing radiation while using an HOR. Despite the awareness of the surgical team in the radioprotection field, the amount of absorbed radiation using an HOR is higher than the one using a C-Arm unit.

Objective Few cases of muscle arteriovenous malformations have been reported in literature to date. Case report We report the case of a 32-year-old man presenting a muscle arteriovenous malformation involving the vastus lateralis muscle with recurrent episodes of pain. The patient was treated by transcatheter embolization with Glubran 2 acrylic glue. There were no periprocedural or subsequent clinical complications, the glue resulted in successful selective occlusion and the patient showed resolution of symptoms at the six-months follow-up. Conclusions Endovascular therapy has been shown to be beneficial in patients with high surgical risks and is the treatment of choice for arteriovenous malformation lesions that extend beyond the deep fascia and involve muscle, tendon, and bone. Glubran 2 constitutes a useful tool to attempt embolization of the muscle arteriovenous malformation nidus, with easier handling and promising results.

To assess the readability of informed consent documents (IC) of the different national surgical societies.During January 2012 we collected 504 IC protocols of different specialties. To calculate readability parameters the following criteria were assessed: number of words, syllables and phrases, syllables/word and word/phrase averages, Word correlation index, Flesch-Szigriszt index, Huerta Fernández index, Inflesz scale degree and the Gunning-Fog index.The mean Flesch-Szigriszt index was 50.65 ± 6,72, so readability is considered normal. There are significant differences between specialties such as Urology (43.00 ± 4.17) and Angiology and Vascular Surgery (63.00 ± 3.26, P.001). No IC would be appropriate for adult readability according to the Fernández-Huerta index (total mean 55.77 ± 6.57); the IC of Angiology and Vascular Surgery were the closest ones (67.85 ± 3.20). Considering the Inflesz scale degree (total mean of 2.84 ± 3,23), IC can be described as «somewhat difficult». There are significant differences between the IC of Angiology and Vascular Surgery (3.23 ± 0.47) that could be qualified as normal, or Cardiovascular Surgery (2.79 ± 0.43) as «nearly normal readability»; and others such as Urology (1, 70 ± 0.46, P.001) and Thoracic Surgery (1.90 ± 0.30, P.001), with a readability between «very» and «somewhat» difficult. The Gunning-Fog indexes are far from the readability for a general audience (total mean of 26.29 ± 10,89).IC developed by scientific societies of different surgical specialties do not have an adequate readability for patients. We recommend the use of readability indexes during the writing of these consent forms.

Resumen Introduccion Evaluar la legibilidad de los documentos de consentimiento informado ( CI ) de las diferentes sociedades nacionales quirurgicas. Metodos Durante enero de 2012 se recogieron los 504 protocolos de CI de las diferentes especialidades. Para el calculo de legibilidad se valoraron los parametros: numero de palabras, silabas y frases, promedio silabas/palabra y palabras/frase, indice de correlacion Word, indice de Flesch-Szigriszt, indice de Fernandez-Huerta, grado en la escala Inflesz y el indice de Gunning-Fog. Resultados La media del indice de Flesch-Szigriszt fue de 50,65 ± 6,72, por lo que se consideran de legibilidad normal. Existen diferencias significativas entre especialidades como Urologia (43,00 ± 4,17) y Angiologia y Cirugia Vascular (63,00 ± 3,26; p Conclusiones Los CI desarrollados por las sociedades cientificas nacionales de las diferentes especialidades quirurgicas no poseen una legibilidad adecuada para los pacientes. Es recomendable el empleo de indices de legibilidad durante la redaccion de los mismos.

To evaluate the safety and feasibility of using drug-eluting balloons (DEBs) in the treatment of infrainguinal bypass graft stenoses.A nonrandomized prospective study evaluated the feasibility of DEB treatment for intragraft/anastomotic stenoses arising1 month after infrainguinal bypass grafting; stenoses due to graft/technical problems (e.g., vein torsion) were excluded, as were failed grafts that could not be successfully recanalized with catheter-directed thrombolysis. Between February 2010 and February 2012, 41 patients (35 men; mean age 71 years, range 46-87) were treated with DEBs for 63 anastomotic/intragraft stenoses in vein or prosthetic grafts. Follow-up was performed with duplex ultrasonography. The primary endpoint at 12 months was graft occlusion or50% restenosis at the DEB treatment site.DEB treatment was technically successful in 61 (96.8%) of the 63 lesions with no complications other than one instance of vasospasm; one totally occluded segment and one restenosis were treated surgically. The mean follow-up was 16.7 months (range 3-24). The estimated cumulative target site primary and secondary patency rates at 6 months were 91% and 96%, respectively, and 70% and 90%, respectively, at both 12 and 18 months (no restenoses after 12 months). The estimated mean durations of primary and secondary treatment site patency were 20.3 and 22.7 months, respectively (p=0.033). At 6 and 12/18 months, the cumulative rates were 96% and 90%, respectively, for graft patency and 98% and 93% for freedom from amputation.DEBs proved to be a feasible, safe, and effective treatment for vein and prosthetic bypass graft stenoses, with excellent technical success and acceptable short and midterm patency.

OBJECTIVES: Ischaemia–reperfusion (I/R) injury is encountered in conditions that diminish intestinal blood flow. There is no clinically feasible technique available for mucosal preservation. METHODS: One hundred Wistar rats were subjected to intestinal ischaemia for 15 and 60 min (I15 0 , I60 0 ), followed by 1 and 7 days of reperfusion (R1d, R7d). Rats were subjected to ischaemia by clamping the superior mesenteric artery. Prostaglandin E1 (PGE1) (2.500 ng/kg intra-arterial bolus or 20 ng/kg intravenous infusion) was administered immediately prior to the commencement of the experimental period. Animals were divided into 20 groups: sham (laparotomy alone), sacrificed at 1 or 7 days; saline administration, 15 or 60 min of ischaemia, 1 or 7 days of reperfusion; prostaglandin E1 administration, 15 or 60 min of ischaemia, 1 or 7 days of reperfusion, each one for intra-arterial or intravenous administration. Ileal segments were excised and assessed for histopathological score, polymorphonuclear (PMN) leucocytes encountered and myeloperoxidase (MPO) activity measurement. RESULTS: I/R caused deterioration of histological characteristics. Prophylactic administration of PGE1 resulted in a significant decrease in the histological score compared with the respective saline group (analysis of variance, P< 0.005). In groups treated with PGE1, PMN leucocyte infiltration was lower for the 60 min of ischaemia group (I60 0 /R1d *P= 0.026; I60 0 /R7d P= 0.015). I15 0 /R7d did not lead to a significant reduction in PMN infiltration (P= 0.061). Pretreatment with PGE1 attenuates MPO levels after intestinal I/R injury (P< 0.05). No differences were encountered between types of administration. CONCLUSIONS: Results of this study showed that administration of prostaglandin E1 prevents I/R injury by diminishing histological damage parameters, inhibiting PMN leucocyte infiltration and attenuating MPO activity.

The Supera peripheral stent system has a woven wire design that may confer a potential advantage and has higher resistive radial strength of traditional stents, which gives it the ability to avoid kinking. Because a stent is a permanent implant in the target vessels and they would be subjected to a wide variety of torsion, compression, and stress, a rupture is not surprising. Only three cases of ruptured Supera stents have been published in literature. We present a case of P2 popliteal segment thrombosis secondary to Supera stent fracture. A 51-year-old male patient presented with lifestyle-limiting intermittent claudication after 6 months of popliteal recanalization and Supera stent implantation. The computed tomography angiography demonstrated a popliteal artery occlusion secondary to stent fracture. The lesion was satisfactorily treated by endovascular recanalization and with a new Supera stent deployment. The duplex examination after 3 and 6 months demonstrated stent's permeability, and the plain anteroposterior and lateral radiographs did not reveal new stent fractures. Our data suggested that Supera stents are not free of fractures during follow-up and their treatment depends on the type of fracture, with the possibility for endovascular treatment in type III fractures.

Reported incidences of peripherally inserted central catheter–associated DVT in the general population are between 1.6% and 3.5%, with upper-extremity DVT causing clinically significant PE in 2%–3% of cases (1). Normal lungs have a dual blood supply. The bronchial arteries arise from the aorta and distally perfuse the submucosal plexus of the airways. At this level, they collateralize with the pulmonary arteries. In a transplanted lung, the donor bronchi depend on collateral perfusion from the pulmonary arteries. Revascularization of the donor organ by the recipient bronchial arteries takes at least 2–4 weeks (2). Therefore, the viability of the allograft during this time is dependent on retrograde, low-pressure, poorly oxygenated blood flow from the recipient pulmonary arteries. Compromise of the pulmonary arterial circulation during this period results in pulmonary infarction and loss of the transplant, with high mortality rates (3). Bousamra et al (4) also showed that acute PE in lung transplant recipients resulted in pulmonary infarction, and further suggested that catheterdirected thrombolysis in this setting may be superior to systemic thrombolysis. The case presented here demonstrates that locoregional catheter-directed pharmacologic thrombolysis across an immature pulmonary artery anastomosis can be performed safely in a patient at high risk with acute PE. This case also suggests that a perfect postthrombolysis angiographic result may not be necessary to achieve a good outcome, and that any residual thrombus should be assessed in the clinical context of the individual patient. Finally, the brief review of transplant lung perfusion presented here highlights the importance of intervening in patients with acute PE after lung transplantation, particularly in the immediate postoperative period.

Thoracic endovascular aortic repair (TEVAR) is a minimally invasive, and generally excellent, technique to treat thoracic aortic aneurysms, dissections, or traumatic aortic transections. Aortoesophageal fistula (AEF) is a highly lethal cause of massive bleeding and a catastrophic complication after TEVAR. The incidence of AEF after TEVAR has been reported as between 5% and 10.53%. We present 3 cases of AEF after TEVAR, and our experience with management and outcome of these patients. A review of the recent literature has been included. These findings emphasize the importance of mediastinal infection, esophageal surgical management, and aorta replacement. Management should be surgical, since the outcome under conservative management seems almost invariably fatal. Future developments in endoprosthesis design and material, and avoidance of oversizing will hopefully reduce the occurrence of this disastrous complication.

Background This study reports our experience during the first 50 cases of use of Supera stent for popliteal atherosclerotic lesions treatment. Methods This prospective single-arm trial enrolled the first 50 limbs (46 patients) treated in our center with a Supera stent for symptomatic atherosclerotic disease of the popliteal artery and a follow-up longer than 12 months. Results Tissue loss (43.5%) was the most common primary indication for intervention, followed by claudication (37.0%) and rest pain (19.5%). Stent implantation was successful in all patients. The majority of lesions were Trans-Atlantic Inter-Society Consensus (TASC) II type D lesions (64%). The lesions treated were localized in P3 zone in 64%, P2 zone in 28%, and P1 zone in 8%. One runoff vessel was the most common situation (42%). Mean follow-up was 16.5 months (range 12–46). The mean preoperative ankle-brachial index increased from 0.38 + 0.37 before intervention to 0.63 + 0.86 postoperatively at 12 months ( P = 0.014). Primary patency rates at 3, 6, 9, and 12 months were 95.9%, 91.7%, 89.6%, and 89.6%, and primary assisted patency rates were 95.9%, 93.8%, 93.8%, and 93.8%, respectively. In-stent restenosis was associated with the following: patient P = 0.023), female sex ( P = 0.032), and TASC-II D lesions ( P = 0.041). Implantation defects did not constitute a risk factor for stent restenosis. Conclusions The implantation of the interwoven nitinol stents in patients with popliteal occlusive disease is safe and effective, with encouraging patency rates and clinical results after a 12-month follow-up. In our experience, we have not seen significant differences in patency at 1-year follow-up in patients with implantation defects.

Abstract. Background: Statins have been reported to help prevent the development and the recurrence of deep vein thrombosis (DVT). We conducted a prospective randomized clinical trial to compare the effects of rosuvastatin plus a low-molecular-weight heparin (LMWH), bemiparin, with conventional LMWH therapy in the treatment of DVT. Patients and methods: In total, 234 patients were randomized into two groups, 116 in the LMWH group and 118 in the statin plus LMWH group. All patients underwent lower limb duplex ultrasound and analytic markers at diagnosis and three months of follow-up. The final analysis included 230 patients. Results: No significant differences were observed in D-dimer levels after three months of follow-up between patients treated with LMWH+rosuvastatin compared to the LMWH group (802.51 + 1062.20 vs. 996.25 + 1843.37, p = 0.897). The group of patients treated with statins displayed lower levels of CRP (4.17 + 4.27 vs. 22.39 + 97.48, p = 0.018) after three months of follow-up. The Villalta scale demonstrated significant differences between groups (3.45 + 6.03 vs. 7.79 + 5.58, p = 0.035). There was a significant decrease in PTS incidence (Villalta score> 5) in the rosuvastatin group (38.3 % vs. 48.5%, p = 0.019). There were no differences in EuroQol score between groups. Conclusions: Adjuvant rosuvastatin treatment in patients diagnosed of DVT improve CRP levels and diminish PTS incidence.

To determine the usefulness of mortality risk scores for the endovascular treatment of ruptured abdominal aortic aneurysms.Retrospective study of 61 patients undergoing endovascular repair between 2009 and 2014. Preoperative variables and in-hospital mortality were collected. The Hardman, GAS, Vancouver and ERAS scales were calculated.In-hospital mortality was 45.9%. The univariate predictors obtained were age, male sex, hypertension, smoking, chronic obstructive pulmonary disease, systolic blood pressure90mmHg, heart rate and loss of consciousness. After completing the multivariate analysis, significant variables were age (P=.021), systolic blood pressure90mmHg (P=.004) and heart rate (P=.050). The GAS (76.79±9.88 vs. 90.43±14.76, P=.001), Vancouver (4.41±0.62 vs. 4.83±0.55, P=.007) and ERAS scales (0.06±0.24 vs. 0.86±0.76, P=.001) were statistically different between the groups. The scale resulting from the following formula: 0.083+0.158 (if age80 years)+0.701 (if systolic blood pressure80mmHg)+0.598 (if heart rate70 beats/min); obtained an area under the curve of 0.95.Age, systolic pressure and heart rate, are predictors of hospital mortality of patients treated with endovascular repair of ruptured abdominal aortic aneurysms. Applying the scale proposed in this study, in combination with GAS, Vancouver and ERAS scales, allows the detection of patients who would not benefit from endovascular treatment.

Genetic events mediating transformation from premalignant monoclonal gammopathies (MG) to multiple myeloma (MM) are unknown. To obtain a comprehensive genomic profile of MG from the early to late stages, we performed high-resolution analysis of purified plasma cells from 20 MGUS, 20 smoldering MM (SMM) and 34 MM by high-density 6.0 SNP array. A progressive increase in the incidence of copy number abnormalities (CNA) from MGUS to SMM and to MM (median 5, 7.5 and 12 per case, respectively) was observed (P=0.006). Gains on 1q, 3p, 6p, 9p, 11q, 19p, 19q and 21q along with 1p, 16q and 22q deletions were significantly less frequent in MGUS than in MM. Although 11q and 21q gains together with 16q and 22q deletions were apparently exclusive of MM status, we observed that these abnormalities were also present in minor subclones in MGUS. Overall, a total of 65 copy number-neutral LOH (CNN-LOH) were detected. Their frequency was higher in active MM than in the asymptomatic entities (P=0.047). A strong association between genetic lesions and fragile sites was also detected. In summary, our study shows an increasing genomic complexity from MGUS to MM and identifies new chromosomal regions involved in CNA and CNN-LOH.

The objective of the work is to study the clinical and haemodynamic evolution, over 1 year, in patients with femoropopliteal arterial pathology treated by means of atherectomy with the SilverHawk device. Materials and methods Nineteen (19) patients were treated between December 2008 and May 2009, collecting data on sex, age, comorbidity and clinical degree, with prospective monitoring over 12 months of clinical symptoms, physical examination and ecodoppler, obtaining results on diameter and peak systolic velocity at different arterial levels. Results Of the 19 patients, 14 were men and 5 women, with a mean age of 70 years, hypertensive (73%), diabetic (63%) and smokers (63%). Six (6) presented disabling claudication and 13 critical ischemia with advanced distal trophic lesions in 5. A good arteriographic result was obtained in 12 cases, a stent was placed on the superficial femoral artery in 5 due to suboptimal outcome. Contrast extravasation was observed in 2, with femoropopliteal bypass performed and one exclusion with endoprosthesis for repair. In the ecodoppler after 1, 3, 6 and 12 months, a progressive reduction in lumen diameter and peak intraarterial systolic velocity was observed, particularly on the distal superficial femoral artery. After one year, 7 patients (36.8%) were symptom-free, 5 (26.3%) presented mild or moderate intermittent claudication and 1 patient (5.3%) presented localised distal trophic lesion. Four (4) major amputations were performed, in 2 the knee was preserved, there were 3 thromboses due to the procedure, a secondary endovascular procedure was performed in one case and a femoropopliteal bypass in another, and there were 2 non procedure-related deaths. Discussion atherectomy with SilverHawk achieves an improvement in clinical degree, with a good rate of extremity salvage in patients with critical ischemia. In the first year, the ecodoppler shows evolution of the arteriopathy, without this necessarily meaning a clinical worsening.

Introduction and objectives To evaluate the readability, accessibility, usability, and reliability of information available on the Internet in the Spanish language about aortic aneurysm and its endovascular treatment, and to determine whether this information source provides comprehensible material that will enable patients to participate in decisions regarding their condition. Methods In November 2010, information from the Internet was compiled by entering the terms “ aneurisma aorta” (aortic aneurysm) and “ endoprotesis aorta” (aortic endoprosthesis) in the most widely used search engines: Google, Yahoo, and MSN/Bing. The first 30 pages provided by each search engine were analyzed. The Inflesz software was used to calculate the readability of the information retrieved and the LIDA instrument, a validated tool to evaluate the quality of health-related Web sites, was used to assess accessibility, usability, and reliability. Results The results for Web pages containing the terms aneurisma aorta and endoprotesis aorta indicated that the readability of the material retrieved was “somewhat difficult” based on the Flesch index within Microsoft Word (48.3 ± 11.42 and 50.11 ± 9.33, respectively; P =.87), Flesch-Szigriszt index (52.69 ± 8.86, 49.31 ± 7.24; P =.87), Fernandez-Huerta index (58.05 ± 8.5, 54.44 ± 7.19; P =.82), and Gunning-Fog index (22.03 ± 2.05, 23.86 ± 1.59; P =.83), as well as the Inflesz grading scale (2.39 ± 0.7, 2.08 ± 0.64; P =.28). The LIDA values for accessibility (82.28 ± 14.14, 77.77 ± 12.64; P =.98), usability (72.28 ± 16.67, 72.28 ± 26.61; P =.08), and reliability (46.17 ± 28.69, 56.38 ± 16.17; P =.06) and the total score (70.22 ± 16.85, 72.15 ± 14.93; P =.52), yielded an evaluation of “moderate”. Introduccion y objetivos Evaluar legibilidad, accesibilidad, utilidad y fiabilidad de la informacion existente en internet sobre el aneurisma de aorta y su tratamiento endovascular, para comprobar su funcion como vehiculo de informacion comprensible que capacite al paciente para participar en decisiones respecto a su enfermedad. Metodos En noviembre de 2010 se recopilo informacion en internet sobre los terminos «aneurisma aorta» y «endoprotesis de aorta» empleando los buscadores mas extendidos: Google, Yahoo y MSN/Bing. Se analizaron las primeras 30 paginas ofrecidas por cada uno. Para calcular la legibilidad de los textos, se empleo el programa informatico Inflesz y el instrumento LIDA como metodo validado de evaluacion de paginas web relacionadas con la salud sobre accesibilidad, utilidad y fiabilidad. Resultados Los indices de correlacion Word (48,3±11,42; 50,11±9,33; p=0,87), Flesch-Szigriszt (52,69±8,86; 49,31±7,24; p=0,87), Fernandez-Huerta (58,05±8,5; 54,44±7,19; p=0,82) y Gunning-Fog (22,03±2,05; 23,86±1,59; p=0,83) y el grado de la escala Inflesz (2,39±0,7; 2,08±0,64; p=0,28) de las paginas web sobre «aneurisma de aorta» y «endoprotesis de aorta», respectivamente, reflejaron una legibilidad «algo dificil». Los valores de accesibilidad (82,28±14,14; 77,77±12,64; p=0,98), utilidad (72,28±16,67; 72,28±26,61; p=0,08), fiabilidad (46,17±28,69; 56,38±16,17; p=0,06) y valoracion total LIDA (70,22±16,85; 72,15±14,93; p=0,52) resultaron «moderados». Conclusiones La informacion disponible en internet sobre el aneurisma de aorta y su terapeutica mediante implantacion de endoprotesis presenta deficiencias en cuanto a accesibilidad, utilidad y fiabilidad, con la dificultad anadida de una complicada legibilidad. Creemos que los indices de legibilidad deber ser utilizados en la elaboracion o la mejora de las paginas web sobre conocimientos medicos cardiovasculares

Introduction Le but de cette etude etait de comparer les resultats du traitement endovasculaire (stenting couvert) et du traitement chirurgical de la coarctation aortique chez l’adulte. Methodes Une etude prospective de 11 patients traites pendant les 10 dernieres annees a ete effectuee. Parmi eux, cinq patients ont ete traites par endoprothese (groupe A) et six par chirurgie ouverte (groupe B). Le suivi comportait une surveillance de la pression arterielle, une echocardiographie, et des angioscanners our angioIRMs. Resultats L’âge moyen des patients etait de 46 ans (extremes : 17 a 67 ans) et le suivi moyen etaient de 52,6 mois (extremes : 1 a 117 mois ; 32,3 pour le groupe A contre 69,7 pour le groupe B ; p = 0,01). Deux cas du groupe A etaient des recoarctations apres angioplastie dans l’enfance. Le taux de complications postoperatoires etait de 27,7% (un hemothorax dans le groupe A contre un pneumothorax et un hemothorax dans le groupe B) ; aucun deces ne s’est produit. Le taux de succes de la technique endovasculaire etait de 80%. La duree de sejour en unite de soins intensifs etait de 2,3 jours avec une difference significative (1 pour le groupe A contre 3 pour le groupes B ; p = 0,01), tandis que la duree du sejour hospitalier etait de 11 jours (7,8 pour le groupe A contre 11,83 pour le groupe B ; p = 0,17). Le gradient de pression a travers la stenose etait diminue de 21,9 ± 3,7 mmHg (24,5 ± 4,3 dans le groupe A contre 33 ± 3,2 dans le groupe B). Six patients (54,5%) avaient une hypertension persistante (80% du groupe A contre 33% du groupe B), avec un gradient de pression de residuel moyen de 23,4 ± 4,3 mmHg (22,5 ± 5,4 dans le groupe A contre 22 ± 2,1 dans le groupe B.; p = 0,58). Conclusions Les resultats a court et a moyen terme du traitement endovasculaire sont comparables, avec un sejour plus court en unite de soins intensifs et un recours plus frequent a un traitement antihypertenseur. Les gradients aortiques de la coarctation en echocardiographie et en Doppler etaient legerement superieurs au 20 mmHg habituels au cours du suivi.

Background The aim of this study was to compare the results of endovascular therapy (covered stenting) with surgical technique to repair aortic coarctation in adults. Methods A prospective study of 11 patients who were treated during the past 10 years was carried out. Of these, five patients underwent endoprosthesis (group A) and six an open surgical repair (group B). Follow-up comprised monitoring of the blood pressure, echocardiography, and computed tomography and magnetic resonance angiographic studies. Results The mean age of the patients was 46 years (range: 17-67 years) and the mean follow-up was 52.6 months (range: 1-117 months; 32.3 for group A vs. 69.7 for group B; p = 0.01). Two cases in group A were recoarctations after child angioplasty. The rate of postoperative complications was 27.7% (one hemothorax for group A vs. one pneumothorax and one hemothorax for group B); however, mortality did not occur. The success rate of the endovascular technique was 80%. The stay in the intensive care unit was 2.3 days with significant differences (one group A vs. three group B; p = 0.01), whereas length of hospital stay was 11 days (7.8 group A vs. 11.83 group B; p = 0.17). The pressure gradient across the stenosis decreased by 21.9 ± 3.7 mm Hg (24.5 ± 4.3 group A vs. 33 ± 3.2 group B). Six patients (54.5%) showed persistent hypertension (80% group A vs. 33% group B), with a mean residual pressure gradient of 23.4 ± 4.3 mm Hg (22.5 ± 5.4 group A vs. 22 ± 2.1 group B; p = 0.58). Conclusions Short- and medium-term results of the endovascular therapy are similar, with shorter stay in the intensive care unit and higher necessity of antihypertensive treatment. Echocardiography and Doppler aortic coarctation gradients slightly higher than 20 mm Hg are usual during follow-up.

The non-vitamin K antagonist oral anticoagulant rivaroxaban is indicated in prevention and treatment of venous thromboembolism (VTE). A 60-year-old male patient complained of bilateral ptosis after administration of rivaroxaban for deep vein thrombosis (DVT). Myasthenia gravis (MG) was confirmed by positive serum antiacetylcholine receptor antibody test. No mediastinal thymoma was found. The ocular myasthenia reversed after discontinuing rivaroxaban treatment. Nevertheless, ptosis recurred and chronic oral pyridostigmine bromide treatment was necessary. The mechanism of MG development by rivaroxaban therapy is not completely understood. The development of rivaroxaban-induced autoimmune disease could be based on cross-reactivity between antibodies against rivaroxaban-derived antigens or by T-cell activation. To our knowledge, this report of ocular myasthenia by rivaroxaban administration is the first in the literature. Despite the benefits of rivaroxaban, it is important to recognize unexpected immune-related adverse events.

True brachial artery aneurysms are. Recent case reports have suggested aneurysmal degeneration of brachial artery in kidney transplant receptors after arteriovenous fistula (AVF) ligation. We present a study on the evolution of the brachial artery in this context in our center.This is a descriptive study in kidney transplant receptors in whom AVF was ligated between 2008 and 2015. Patients with AVF in both upper limbs were excluded. Diameters of axillary artery, brachial artery in middle portion and its bifurcation, as well as brachial artery flow were measured using Dupplex ultrasound in AVF and contralateral limb. Both groups were compared using the Student t-test for paired samples.20 patients were included in the study and had a mean age of 59.35 ± 2.49 years. The median time of use of AVF for hemodialysis was 729 days (range 120-6117) and the median time in which AVF was patent was 2261 days (range 791-7091). Mean diameters (in mm) of axillary artery, brachial in middle portion and bifurcation were respectively 9.33 ± 1.07, 7.5 ± 0.61 and 5.81 ± 0.43 in AVF arm and 5.6 ± 2.8, 4.4 ± 0.1, 4.9 ± 0.15 in control limb, finding statistically significant differences (p0.01) in brachial and axillary arteries. 5 patients (25%) developed aneurysm, 2 of which (10%) underwent surgery and 3 are on follow up.True incidence of brachial artery aneurysm in kidney receptors following AVF ligation is high. Careful follow up with physical examination and dupplex scanning are needed to find this complication.
