Résumé: La neurofibromatose de Von Recklinghausen est une maladie génétique qui peut toucher tous les systèmes de l'organisme. Chez l'enfant, ses complications sont nombreuses : elles sont surtout neurologiques et orthopédiques. L'atteinte vasculaire est exceptionnelle mais grave, et peut engager le pronostic fonctionnel et vital. Les atteintes les plus fréquentes sont les sténoses des artères rénales et la coarctation de l'aorte. L'atteinte des artères périphériques est encore plus rare. Nous rapportons le cas unique d'un enfant de 7 ans avec hypoplasie du membre inférieur gauche associée à un trouble trophique au gros orteil, secondaires à une occlusion de l'artère iliaque externe en rapport avec une neurofibromatose de type 1, qui a bénéficié d'un pontage veineux ilio-fémoral, avec bonne évolution clinique. Von Recklinghausen neurofibromatosis is a genetic disease that can affect all systems of the body. In children, its complications are numerous, they are especially neurological and orthopedic. The vascular involvement is exceptional but serious, and can engage the functional and vital prognosis. The most common are renal artery stenosis and aortic coarctation. Peripheral vascular lesions are even more rare. We report a unique case of a 7-year-old child with left lower limb hypoplasia associated with a trophic disorder of the big toe, secondary to occlusion of the external iliac artery related to neurofibromatosis type 1, who benefited from ilio-femoral venous bypass, with good clinical outcomes. Résumé: La maladie de Behçet est une vascularite systémique avec un grand polymorphisme clinique. L'atteinte vasculaire est souvent grave et peut engager le pronostic vital. Les artères et les veines de tout calibre peuvent être touchées. L'atteinte des vaisseaux digestifs est souvent grave. Nous rapportons le cas d'un jeune patient, de sexe masculin, dont la maladie de Behçet a été révélée par une ischémie digestive secondaire à une thrombose de la veine mésentérique supérieure étendue à la veine porte. Behçet's disease is a systemic vasculitis with a large clinical polymorphism. Vascular involvement is often severe and can be life-threatening. Arteries and veins of any size can be affected. The involvement of the digestive vessels is often severe. We report a case of a patient whose Behçet's disease was revealed by digestive ischemia secondary to superior mesenteric vein thrombosis extended to the portal vein. [ABSTRACT FROM AUTHOR]

Brachial artery aneurysmal degeneration is an exceptional complication of distal native fistulas. Chronic mechanical stresses due to high flow, as well as immunosuppressants drugs following renal transplantation, are the mains factors implicated in the pathophysiological mechanism. We report a case of a transplant patient with a true, symptomatic, brachial artery aneurysm, 8 years after transplantation and 5 years after radiocephalic wrist fistula ligation. The patient underwent open surgical repair, with aneurysm resection and end-to-end anastomosis. The creation and preservation of vascular accesses, in patients with end-stage renal failure, remains a challenge for nephrologists and vascular surgeons. Native fistula is the best vascular access, humeral-basilic fistula is a precious access in patients who have exhausted their venous capital in the forearm and in whom the cephalic vein of the arm is small or damaged. Given its deep location, any puncture of this vein is prohibited before its superficialization, even if it is of good caliber, because it can have dramatic consequences, in particular the loss of the limb or even death. Native arteriovenous fistula is the best available vascular access for hemodialysis in end-stage renal failure. It is characterized by higher patency rates and lower rates of morbidity, mortality and complication compared to prosthetic bypass grafts and central venous catheters. Aneurysmal complications remain the main complications of these access with a high risk of rupture and fatal hemorrhage. Their management varies from case to another and several methods are reported in the literature.
