Abstract

Interleukin-23 inhibition is effective in treating ulcerative colitis. Guselkumab is a dual-acting, human IgG1, interleukin-23p19 subunit inhibitor that potently neutralises interleukin-23 and can bind to CD64. We aimed to evaluate the efficacy and safety of guselkumab as induction and maintenance therapy in patients with ulcerative colitis.

Abstract

Summary Background Etrolizumab is a gut-targeted anti-β7 integrin monoclonal antibody. In an earlier phase 2 induction study, etrolizumab significantly improved clinical remission relative to placebo in patients with moderately to severely active ulcerative colitis. The HIBISCUS studies aimed to compare the efficacy and safety of etrolizumab to adalimumab and placebo for induction of remission in patients with moderately to severely active ulcerative colitis. Methods HIBISCUS I and HIBISCUS II were identically designed, multicentre, phase 3, randomised, double-blind, placebo-controlled and active-controlled studies of etrolizumab, adalimumab, and placebo in adult (18–80 years) patients with moderately to severely active ulcerative colitis (Mayo Clinic total score [MCS] of 6–12 with an endoscopic subscore of ≥2, a rectal bleeding subscore of ≥1, and a stool frequency subscore of ≥1) who were naive to tumour necrosis factor inhibitors. All patients had an established diagnosis of ulcerative colitis for at least 3 months, corroborated by both clinical and endoscopic evidence, and evidence of disease extending at least 20 cm from the anal verge. In both studies, patients were randomly assigned (2:2:1) to receive subcutaneous etrolizumab 105 mg once every 4 weeks; subcutaneous adalimumab 160 mg on day 1, 80 mg at week 2, and 40 mg at weeks 4, 6, and 8; or placebo. Randomisation was stratified by baseline concomitant treatment with corticosteroids, concomitant treatment with immunosuppressants, and baseline disease activity. All patients and study site personnel were masked to treatment assignment. The primary endpoint was induction of remission at week 10 (defined as MCS of 2 or lower, with individual subscores of 1 or lower, and rectal bleeding subscore of 0) with etrolizumab compared with placebo. Pooled analyses of both studies comparing etrolizumab and adalimumab were examined for several clinical and endoscopic endpoints. Efficacy was analysed using a modified intent-to-treat population, defined as all randomly assigned patients who received at least one dose of study drug. These trials are registered with ClinicalTrials.gov , NCT02163759 (HIBISCUS I), NCT02171429 (HIBISCUS II). Findings Between Nov 4, 2014, and May 25, 2020, each study screened 652 patients (HIBISCUS I) and 613 patients (HIBISCUS II). Each study enrolled and randomly assigned 358 patients (HIBISCUS I etrolizumab n=144, adalimumab n=142, placebo n=72; HIBISCUS II etrolizumab n=143; adalimumab n=143; placebo n=72). In HIBISCUS I, 28 (19·4%) of 144 patients in the etrolizumab group and five (6·9%) of 72 patients in the placebo group were in remission at week 10, with an adjusted treatment difference of 12·3% (95% CI 1·6 to 20·6; p=0·017) in favour of etrolizumab. In HIBISCUS II, 26 (18·2%) of 143 patients in the etrolizumab group and eight (11·1%) of 72 patients in the placebo group were in remission at week 10, with an adjusted treatment difference of 7·2% (95% CI –3·8 to 16·1; p=0·17). In the pooled analysis, etrolizumab was not superior to adalimumab for induction of remission, endoscopic improvement, clinical response, histological remission, or endoscopic remission; however, similar numerical results were observed in both groups. In HIBISCUS I, 50 (35%) of 144 patients in the etrolizumab group reported any adverse event, compared with 61 (43%) of 142 in the adalimumab group and 26 (36%) of 72 in the placebo group. In HIBISCUS II, 63 (44%) of 143 patients in the etrolizumab group reported any adverse event, as did 62 (43%) of 143 in the adalimumab group and 33 (46%) in the placebo group. The most common adverse event in all groups was ulcerative colitis flare. The incidence of serious adverse events in the pooled patient population was similar for etrolizumab (15 [5%] of 287) and placebo (seven [5%] of 144) and lower for adalimumab (six [2%] of 285). Two patients in the etrolizumab group died; neither death was deemed to be treatment related. Interpretation Etrolizumab was superior to placebo for induction of remission in HIBISCUS I, but not in HIBISCUS II. Etrolizumab was well tolerated in both studies. Funding F Hoffmann-La Roche.

Abstract

Etrolizumab is a gut-targeted anti-β7 integrin monoclonal antibody. In an earlier phase 2 induction study, etrolizumab significantly improved clinical remission relative to placebo in patients with moderately to severely active ulcerative colitis. The HIBISCUS studies aimed to compare the efficacy and safety of etrolizumab to adalimumab and placebo for induction of remission in patients with moderately to severely active ulcerative colitis.

29. Entropic Equations of State and Their Application to Shock Wave Phenomena in Solids.

Abstract

The front of a shock wave in an ideal fluid is characterized by a discontinuous transition from the undisturbed state to a highly compressed state in which the entropy of the fluid has increased. The relations between pressure, density, propagation velocity, and particle velocity behind the shock front are represented by the equations for conservation of mass, momentum, and energy and by the equation of state of the fluid. The entropy change produced by the transition is third order in the compression for weak shocks. The propagation of shock waves through metals can be described by a hydrodynamic theory which assumes that the metal is an ideal fluid behind the shock front, and that the hydrostatic equation of state is applicable there. Three hydrostatic equations of state for Al, Cu, Fe, and Pb are compared with experimental data at low pressures and quantum mechanical calculations pressures up to several hundred megabars. With these equations of state the hydrodynamic theory is used to calculate explicitly the entropy increase across the shock front, the amount by which the pressure exceeds the adiabatic pressure for a given compression, and the temperature rise after the shock pressure has been relieved for the metals mentioned above. Abstract

A series of experiments is described in which the time resolve spectra of several liquids have been recorded as they were compressed by shock waves reverberating between two sapphire plates. The extent of chemical reaction is reported as indicated by red shift of the uppermost absorption band in the UV or visible. Most of the liquids were organic materials containing 0,1 or 2 carbon double bonds. Unsaturated substances are, on the whole, more reactive than saturated ones, but the relation is not simple. One of the few inorganics studied was HI, which reacted very rapidly an in two stages. Some comments on possible mechanism are included, along with cautions about the interpretation of this kind of experiment. Abstract

The response of bimetallic junctions to shock loading was investigated using a new simplified geometry in which the circuit of interest is completed by impact. Techniques for measuring the 1 millivolt level signals were developed within this geometry with the extensive use of null experiments in which both parts of the junction were of the same material. It was shown that the use of a ferromagnetic material introduces a demagnetization signal that is not well characterized, which puts measurements of thermal E.M.F.s measured in nickel under shock loading conditions in some doubt. Results of experiments involving a nickel 80% chromium 20% alloy indicate that the E.M.F.s involved are anomalously high, in agreement with previous work done elsewhere. The dependence of the thermal E.M.F.s generated on the geometry or the assumptions of one- dimensional conditions has not been completely characterized but the indication is that there is indeed some dependence.

36. Optical Spectroscopy of Dynamically Compressed Liquids.

Abstract

The objectives of this work are described in the Introduction. Accomplishments are listed and experimental approaches are described briefly. A summary is given of spectroscopy experiments in CS2 and other materials. Conclusions are: i) there is no evidence in CS2 of an unique shock effect other than that resulting from rapid increase of pressure and temperature which accompanies compression, ii) metastable states, which can be sampled only by rapid compression, are achieved in these experiments, iii) chemical reaction rates can be measured and critical pressures and temperatures can be determined from spectral measurements in this regime, iv) changes in the UV and visible spectra of liquids studied vary from no observable effect to shifts of thousands of angstroms in absorption band edges for pressure less than 130 kbars, v) observed spectral changes in liquid CS2 are cooperative in nature and become very small or zero at infinite dilution, vi) a combination of static high pressure and shock compression measurements is particularly valuable in unravelling the physical processes, involved, vii) shock experiments appear to represent a particularly useful adjunct to and extension of chemical processes in liquids under conditions of high temperatures and pressures. Abstract

Some background information is given on fast reactions, detonations, shock waves. Research at Washington State University on liquid CS2 is summarized. Techniques for and results of measurements of characteristic reaction times are given in Section IV. A double shock method was used, a double wave observed, and times ranged from less than 10 nsec at 150 kb to several hundred at 80-90 kbars. An induction time was observed in some experiments and a quasi-coherent electrical signal appears to have generated in the reaction process. Reflection and absorption spectroscopy are reported in Section V. Reflection measurements show that the shocked liquid behaves like a poor conductor in the spectral range 2500-4000 A for shock pressures greater than about 20 kb. Transmission measurements show that extinction extends into the visible as shock pressures increase. The shift is linear with pressure about 20 kbars, but the dependence is more complicated at higher pressures. The observations do not appear to be compatible with static pressure and temperature measurements, but this observation requires further substantiation. (Author)

38. Shock-Induced Reaction in Carbon Disulfide.

Abstract

In the studies of the shock-induced chemical decomposition of liquid CS2, it has been shown that the liquid under compression transforms to a higher density form and that this transformation occurs under static pressure at 45 kbar and 175 C. Previous shock experiments have shown that a similar transformation occurs in a single shock at approximately 60 kbar and 1000 C. Sheffield at WSU has shown that the transformation occurs in a reflected shock at pressures greater than 75 kbar and at temperatures greater than 750 C. The transformation or reaction time was found to decrease from about 400 nsec at 79 kbar to less than 10 nsec at 130 kbar. At lower pressures there is an induction time decreasing from 300 nsec at 79 kbar to less than 6 nsec at 106 kbar. We have finished and reported the work on shock-induced light emission in CS2 experiments and gone on to redesign the absorption experiment to overcome two problems. One was the inadequacy of the light source being used. The present source has an effective temperature of about 15000 K. With this the spectrograph-streak camera produces a satisfactory image with time and spectral resolutions of about 30 nsec and 30 A, respectively. The other difficulty lay with the effects of shock on the light reflection. This has been resolved by placing the reflector in the projectile, where it is shock-free during the significant part of the experiment.

Abstract

It is pointed out that variations of shear stress in a solid undergoing transition may have an important effect on the conditions under which transitions occur. Such variations are not calculable from present knowledge, and for practical reasons description of the state of shear during the transition must be made part of the constitutive relations. The difficulty of defining a Gibbs energy in a solid is pointed out and the thermodynamic problems are indicated in some details. Experiments are described which promise to reveal the state of shear in the alpha-gamma transition in Fe/30%Ni at high temperatures using shock wave methods. Attempts to carry out these experiments are described, and numerical calculations of phase transitions with and without shear stress show that characteristic shock wave structures do indeed result from various shear conditions during transformation. Transformation experiments at various initial temperatures suggest that there may be a third phase--perhaps epsilon--which is reached in shock experiments for modest driving pressures and for initial temperatures less than 200 C. A description of the metallographic state after shock transition is given. There is minimal evidence that the shear stresses were sharply reduced for initial temperatures of 400 C. (Author)

Abstract

Liquid carbon disulfide (CS2) was shocked by reflection to pressures where reaction occurs. Progress of shocks and reaction was monitored with electromagnetic particle velocity probes. Particle velocity waveforms show that a two-wave structure evolves in the reflected shock wave at pressures above about 75 kbar, with the character depending upon the pressure. Two wave evolution is accompanied by an induction time, followed simultaneously by particle velocity decay and a burst of decaying electrical noise; the noise decay takes longer than the particle velocity decay. Equation of state models were developed and used to calculate temperatures both before and after reaction. From these calculations and observations, it is hypothesized that a decomposition chemical reaction occurs in the shock. Abstract

The electrical response of a copper-constantan junction to shock compression has been studied over a pressure range from 145 kbars to 360 kbars. Four possible sources of anomalous response were found; electrical noise due to circuit closure at impact, shock demagnetization of a ferromagnetic material, high local temperature at the junction interface due to shock compression of a surface damage layer, and two-dimensional flow in the pressurized region due to pressure relief from the edges. Using a diffusion welded junction the emf measured is within 20% of the predicted emf based on normal thermocouple response to shock compression temperatures. In a geometry in which a guard ring was used to maintain the pressure behind the shock front, the observed voltage-time profile was a step with constant plateau. In a similar configuration where radial pressure relief was allowed behind the shock front, the initial step, which compares directly in amplitude to the above results, is followed by a marked positive ramping. This ramping was directly correlated with the two-dimensional flow due to radial pressure relief. The results indicate that any fundamental anomaly which exists for one-dimensional strain is less than 10-20% of the observed signals in the pressure range studied. (Author)

Research under this grant was directed to two distinct tasks. First was study of the kinetics of shock-induced phase transitions in solids; second was measurement of precursor amplitudes in aluminum under carefully controlled conditions. Shock-induced stresses were measured in single crystals of KCl with shock waves propagating in <100>, <110>, and <111> directions and in single crystals of BaF2 with propagation in <111> and <100> directions. KCl measurements give clear evidence of the initiation of transformation at the established (fcc-bcc) transformation pressure. Stress-time profiles in BaF2 with <111> orientation have a complex structure which apparently results from interaction between the shear yielding process and phase transformation. Precursor amplitudes in aluminum are uniformly low for 1060-0 commercial aluminum, 5-9's single crystals, and single crystals doped with .01 and 0.1 atomic percent copper. Variation of heat treatment to change precipitate structure affects precursor amplitude as it does static yield stress.

45. WASTE HEAT IN LIQUID SPHERICAL BURSTERS

Abstract

Special Technical Report No. 3 (hereafter referred to as STR-3) dealt with energy loss through shock heating in a bomblet with an explosive radius of 1.6 cm and an outer liquid radius of 3.85 cm. Detailed calculations were made for glycerin, water, and ethyl ether; the first and last representing practical extremes in the mechanics of organic liquid behavior. In this supplement, attention is focused on the effects of changing burster size. The validity of linear scaling is examined and is found to be adequate to burster radii of 2.56 cm. New equations of state are used for the three liquids mentioned above, and the sensitivity of waste heat to changes in equation of state is examined., Supplement to special technical report no. 3.

Abstract

After a brief review of the history of material failure studies and of measurements of stored energy in plastically deformed bodies, energy and stress and strain relations are developed for both elastic and plastic deformation in uniaxial strain. The physical effects of plastic deformation are discussed and foundations are laid for thermodynamic calculations of plastically deformed materials. Calculations of both thermodynamic and mechanical parameters are described for uniaxial strain with and without energy storage in internal strains. Conditions of uniaxial strain are found to yield new thermodynamic relations which are useful for synthesizing constitutive relations.

Abstract

A procedure was developed for synthesizing a complete, solid equation of state and applied to iron. An experimental technique was developed for measuring shock induced magnetic anisotropy in YIG5 An improved procedure was developed for averaging magnetic moment in polycrystalline material. A theory of the asymptotic behavior of magnetization with field was developed for ferromagnetic materials. A code was written for 2-D elastic-plastic wave propagation in a bar. Analytic solutions for finite amplitude waves in lattices were developed.

Abstract

Possible practical effects of phase transitions in structural materials are discussed and a rationale is developed for investigating the kinetics of transitions induced by shock waves. A gas gun experiment in two-dimensional steady plane flow is described. Atomic motions required for transition from the NaCl form of KCl to the CsCl lattice are described in detail. Some experiments with KCl which were intended to relate ease of transition to atomic motions are described. They suggest that those transitions involving minimal motions may occur most rapidly, but they are not conclusive. They also show evidence of thermal-activated diffusion at 85C. Effects of shear stress on the equilibrium of two phases are discussed for a very simple case and are shown to be sensitive to shear stress. A more general formulation shows that conditions for phase equilibrium, even without surface energies, can differ markedly for equality of the simple Gibbs functions. An extensive bibliography is appended.

49. Some Experimental Manifestations of Instabilities in Shock Waves

Abstract

Instabilities which occur in shock wave or impact experiments are noted to be of two kinds. In one-dimensional experiments the wave may break up into multiple waves due to plastic yield or phase transitions. The indication that such an instability will occur is normally in a cusp in the P-v relation for the material. In two-dimension problems such as impact of two plates at some non-zero angle, ripples in the impact surface may develop. These may be related to jetting effects at the interface. Abstract

The report describes a program for computing equation of state parameters for a material which undergoes a phase transition, either rate-dependent or reversible. The FORTRAN program is described in detail and a sample calculation is made for iron. A modification is described which makes it possible to compute the rarefaction shock arising from a phase transition. This too is computed for iron with and without rate-dependence in the transition. Next, a FORTRAN program is described for deriving crystalline elastic constants from sonic measurements at high pressure; it is capable of obtaining explicit temperature dependence if measurements are made at three temperatures. It is applied to data from calcite. Finally a theoretical equation of state is described for solid iron. (Author)
