Abstract

Background & Aims: Two recently developed composite models, the alpha-fetoprotein (AFP) score and Metroticket 2.0, could be used to select patients with hepatocellular carcinoma (HCC) who are candidates for liver transplantation (LT). The aim of this study was to compare the predictive performance of both models and to evaluate the net risk reclassification of post-LT recurrence between them using each model's original thresholds. Methods: This multicenter cohort study included 2,444 adult patients who underwent LT for HCC in 47 centers from Europe and Latin America. A competing risk regression analysis estimating sub-distribution hazard ratios (SHRs) and 95% CIs for recurrence was used (Fine and Gray method). Harrell's adapted c-statistics were estimated. The net reclassification index for recurrence was compared based on each model's original thresholds. Results: During a median follow-up of 3.8 years, there were 310 recurrences and 496 competing events (20.3%). Both models predicted recurrence, HCC survival and survival better than Milan criteria (p <0.0001). At last tumor reassessment before LT, c-statistics did not significantly differ between the two composite models, either as original or threshold versions, for recurrence (0.72 vs. 0.68; p = 0.06), HCC survival, and overall survival after LT. We observed predictive gaps and overlaps between the model's thresholds, and no significant gain on reclassification. Patients meeting both models (“within-ALL”) at last tumor reassessment presented the lowest 5-year cumulative incidence of HCC recurrence (7.7%; 95% CI 5.1-11.5) and higher 5-year post-LT survival (70.0%; 95% CI 64.9-74.6). Conclusions: In this multicenter cohort, Metroticket 2.0 and the AFP score demonstrated a similar ability to predict HCC recurrence post-LT. Abstract

Background & Aims: Patients with hepatocellular carcinoma (HCC) are selected for liver transplantation (LT) based on pre-LT imaging ± alpha-foetoprotein (AFP) level, but discrepancies between pre-LT tumour assessment and explant are frequent. Our aim was to design an explant-based recurrence risk reassessment score to refine prediction of recurrence after LT and provide a framework to guide post-LT management. Methods: Adult patients who underwent transplantation between 2000 and 2018 for HCC in 47 centres were included. A prediction model for recurrence was developed using competing-risk regression analysis in a European training cohort (TC; n = 1,359) and tested in a Latin American validation cohort (VC; n=1,085). Results: In the TC, 76.4% of patients with HCC met the Milan criteria, and 89.9% had an AFP score of ≤2 points. The recurrence risk reassessment (R3)-AFP model was designed based on variables independently associated with recurrence in the TC (with associated weights): ≥4 nodules (sub-distribution of hazard ratio [SHR] = 1.88, 1 point), size of largest nodule (3–6 cm: SHR = 1.83, 1 point; >6 cm: SHR = 5.82, 5 points), presence of microvascular invasion (MVI; SHR = 2.69, 2 points), nuclear grade >II (SHR = 1.20, 1 point), and last pre-LT AFP value (101–1,000 ng/ml: SHR = 1.57, 1 point; >1,000 ng/ml: SHR = 2.83, 2 points). Wolber's c-index was 0.76 (95% CI 0.72–0.80), significantly superior to an R3 model without AFP (0.75; 95% CI 0.72–0.79; p = 0.01). Four 5-year recurrence risk categories were identified: very low (score = 0; 5.5%), low (1–2 points; 15.1%), high (3–6 points; 39.1%), and very high (>6 points; 73.9%). The R3-AFP score performed well in the VC (Wolber's c-index of 0.78; 95% CI 0.73–0.83). Conclusions: The R3 score including the last pre-LT AFP value (R3-AFP score) provides a user-friendly, standardised framework to design post-LT surveillance strategies, protocols, or adjuvant therapy trials for HCC not limited to the Milan

15. Estudio comparativo de micro, pequeñas y medianas empresas en Colombia

Abstract

Este trabajo busca realizar un análisis comparativo de las micro, pequeñas y medianas empresas en Colombia, a partir de características de Tecnologías de Información y Comunicaciones. Debido a la importancia para los países, la investigación de las MiPymes ha cobrado cada vez más interés tanto para la investigación académica como para instituciones que buscan fomentar el desarrollo de estas. Para este estudio se aplicó un estudio descriptivo, cuantitativo, con análisis de datos ANOVA de la aplicación de 1600 MiPymes de Colombia y se utilizaron 20 estratos sectoriales en 4 de las principales ciudades de Colombia: Barranquilla, Bogotá, Cali y Medellín. Los resultados obtenidos indican que, las MiPymes cuentan con la misma infraestructura, en términos de servicios de agua, sanitarios, electricidad y telefonía; mientras que, desde el punto de vista de implementación de las TICs, el uso del correo electrónico, la utilización de la banca electrónica y la disposición de una página web, están aún en un periodo de asimilación. Se concluye que el comportamiento y características de las micro, pequeñas y medianas empresas influye en el éxito sostenido o en el fracaso de estas.

Abstract

The International Severe Acute Respiratory and Emerging Infection Consortium (ISARIC) COVID-19 dataset is one of the largest international databases of prospectively collected clinical data on people hospitalized with COVID-19. This dataset was compiled during the COVID-19 pandemic by a network of hospitals that collect data using the ISARIC-World Health Organization Clinical Characterization Protocol and data tools. The database includes data from more than 705,000 patients, collected in more than 60 countries and 1,500 centres worldwide. Patient data are available from acute hospital admissions with COVID-19 and outpatient follow-ups. The data include signs and symptoms, pre-existing comorbidities, vital signs, chronic and acute treatments, complications, dates of hospitalization and discharge, mortality, viral strains, vaccination status, and other data. Here, we present the dataset characteristics, explain its architecture and how to gain access, and provide tools to facilitate its use.

Abstract

BACKGROUND: Up to 30% of hospitalised patients with COVID-19 require advanced respiratory support, including high-flow nasal cannulas (HFNC), non-invasive mechanical ventilation (NIV), or invasive mechanical ventilation (IMV). We aimed to describe the clinical characteristics, outcomes and risk factors for failing non-invasive respiratory support in patients treated with severe COVID-19 during the first two years of the pandemic in high-income countries (HICs) and low middle-income countries (LMICs). METHODS: This is a multinational, multicentre, prospective cohort study embedded in the ISARIC-WHO COVID-19 Clinical Characterisation Protocol. Patients with laboratory-confirmed SARS-CoV-2 infection who required hospital admission were recruited prospectively. Patients treated with HFNC, NIV, or IMV within the first 24 h of hospital admission were included in this study. Descriptive statistics, random forest, and logistic regression analyses were used to describe clinical characteristics and compare clinical outcomes among patients treated with the different types of advanced respiratory support. RESULTS: A total of 66,565 patients were included in this study. Overall, 82.6% of patients were treated in HIC, and 40.6% were admitted to the hospital during the first pandemic wave. During the first 24 h after hospital admission, patients in HICs were more frequently treated with HFNC (48.0%), followed by NIV (38.6%) and IMV (13.4%). In contrast, patients admitted in lower- and middle-income countries (LMICs) were less frequently treated with HFNC (16.1%) and the majority received IMV (59.1%). The failure rate of non-invasive respiratory support (i.e. HFNC or NIV) was 15.5%, of which 71.2% were from HIC and 28.8% from LMIC. Abstract

Introducción: La brucelosis es un enfermedad zoonotica relacionada al contacto con perros y con ganado vacuno, porcino y caprino, el agente etiológico pertenece al género Brucella con diez especies de importancia para el ser humano; su capacidad para evadir el sistema inmune y sobrevivir en el fago-lisosoma genera un cuadro clínico crónico en el ser humano caracterizado por fiebre ondulante y afectación osteoarticular. Su amplia variedad de expresión clínica genera un reto al momento del diagnóstico y tratamiento; por tal razón es relevante el reconocimiento de las pruebas diagnósticas que permitan establecer el posible contacto con el microorganismo. Objetivo: Establecer la seroprevalencia a Brucella spp en una población de estudiantes de Medicina Veterinaria con posible exposición ocupacional. Materiales y Métodos: Estudio descriptivo observacional donde se estudiaron 272 sueros obtenidos de estudiantes de medicina veterinaria mediante la técnica de rosa de bengala. Resultados: El 18,4% (50/272) de los sujetos presento positividad por rosa de bengala; 28 hombres (56%) y 22 mujeres (44%). Conclusiones: La serología mediante rosa de bengala en esta población de riesgo evidenció un 18,5% de sujetos seropositivos. Se sugiere el empleo de barreras efectivas de protección durante el contacto con animales portadores del microorganismo durante el entrenamiento como estudiantes de Medicina veterinaria.

La osteotomía Le Fort I es un procedimiento rutinario en cirugía ortognática. Los autores reportan la ocurrencia de complicaciones intra- y posquirúrgicas. La osteotomía Le Fort I está relacionada con gran variedad de complicaciones. Un conocimiento preciso de las complicaciones debería ayudar al cirujano, al ortodoncista y al paciente para estimar los beneficios de una cirugía electiva versus los riesgos. El conocer las complicaciones también debería ayudar a prevenir su ocurrencia y facilitar el tratamiento. El propósito de esta revisión fue reportar las complicaciones quirúrgicas y posquirúrgicas que están relacionadas con la osteotomía Le Fort I. Se presentan las complicaciones en el siguiente orden: oftalmológicas, vasculares, del seno cavernoso, necrosis avascular, edema, infecciosas, neurológicas, resultados estéticos desfavorables, cambios pulpares y otras complicaciones de rara ocurrencia.Le Fort I osteotomy has become a routine procedure in elective orthognathic surgery. The authors report the occurrence of intra and post operative complications in Le Fort I osteotomy. The Le Fort I osteotomy is related to a variety of complications. A precise assessment of complications might be helpful for the patient, the orthodontist, and the surgeon in order to estimate the benefit of an elective operation versus its immanent risks. Knowledge of complications also might help to prevent their occurrence and facilitate their management. The aim of this review was to report the types and frequencies of intra- and post operative complications related to Le Fort I osteotomy. This review presents surgical complications in the following order: Ophthalmic complications, Vascular complications, Cavernous sinus fistula, Ischemic complications including aseptic necrosis, swelling, significant infections such as abscesses or maxillary sinusitis, Nerve injures, unfavorable esthetic results, unfavorable pulpal reactions and other unusual complications.

Abstract

The purpose of this longitudinal descriptive study was to evaluate the behavior of soft tissues in patients who underwent bimaxilar ortognatic surgery at the Hospital University San Vicente de Paul (HUSVP) by means of lateral cephalic headplates. A total of twenty one patients selected by convenience, with an average age of 22,8 years, were treated , all of them received lateral cephalic headplates pre and post surgery. All of the subjects had received pre-surgical orthodontic treatment, they also received LeFort I surgery of impactation and maxilar advance, the mandible was retruded using a sagittal technique of the ramus. A different behaviour to the one found in different ethnic groups was found in this population. The movement in the horizontal plane showed an ascending path from the nasal point up to the upper superior point of the lip. For the nasal point a 46.9% movement was found. For the sub nasal and the superior anterior point of the lip a movement of 63.5% and 73% respectively. For the mandible the movement was51.0 % for labralis superius, point point B followed with 82.9 % and pogonion with 75%. In the vertical direction the movement was 67% for the nasal point, 64% for sub nasal point and 50% for the labralis superior. The mandible showed a movement of 34, 7% for labralis inferior and an average of 67,7% for the remaining points of the mandible. In conclusion, it is important to consider the ethnic characteristics of our population and the type of surgery, since they presented a different behavior than the one reported in the literature.

Abstract

La clasificación basada en rasgos y grupos funcionales establecidos en la morfología del fitoplancton posibilitó una aproximación factible y objetiva en la evaluación de seis sistemas de diferente tipo, origen, morfología y condición limnológica pertenecientes a las regiones amazónica, andina y caribe de Colombia. Se estableció específicamente la relación entre las variables continuas y categóricas del fitoplancton (volumen, superficie, dimensión lineal máxima, presencia de aerótopos, mucílago, flagelo y exoesqueleto de sílice) con los parámetros limnológicos de transparencia, profundidad máxima, temperatura y concentración de iones inorgánicos. Se seleccionaron entre dos y ocho estaciones durante tres o cuatro periodos climáticos en series anuales entre el 2005 y el 2007 tomando las muestras en la zona limnética. Los taxones identificados se clasificaron en uno u otro grupo funcional basado en la morfología (GFBM) según las dimensiones morfológicas y la presencia de rasgos especializados de cada taxón. Entre los resultados más relevantes se destacó el grupo V (flagelados unicelulares de tamaño medio a grande) en ambientes poco profundos y turbios, con excepción de la llanura del Bajo Sinú en el Caribe, donde dominaba el grupo VII (colonias mucilaginosas con los lípidos y aerótopos). En los lagos profundos y de gran transparencia en montañas sobresalieron los grupos IV (tamaño medio y ausencia de rasgos especializados) y VII. Los grupos propuestos en la clasificación permitieron exponer los patrones funcionales del ensamblaje del fitoplancton y explicar significativamente su variación en respuesta a las características de los sistemas. The classification based on traits and functional groups of phytoplankton proposed by Kruk et al., (2010) allowed a feasible and objective approach in the evaluation of six tropical systems of different types, origin, shape, size and limnological condition in relation to the variation of the morphology of phytoplankton in response to the environment. This study established the morphological variability of phytoplankton associated with physical, chemical and climatic characteristics in six systems with different environmental conditions, belonging to the Amazonian, Andean and Caribbean areas of Colombia. The samples were taken in the limnetic area establishing between two and eight stations during three to four climatic periods in an annual series between 2005 and 2007. The identified taxa are included in one or another GFBM using a "script" designed by Kruk and Segura (2011). The inclusion of the groups was based on the morphological dimensions and the presence of special characteristics of each taxon. Group V excelled in shallow and turbid environments, except for the plain of the Caribbean Bajo Sinú where Group VII dominated. In the deep and high transparency lake of high mountain, the outstanding groups correspond to Group IV and Group VII. The groups proposed by the classification of Kruk et al. (2010) were useful and objective as a tool for the study of the assembly of phytoplankton because they significantly explained the variation of phytoplankton morphology in response to the environment.

Abstract

ObjectivesThis study aimed to obtain a consensus on oral health policy, access to dental care, oral hygiene measures and training levels.BackgroundPoor oral health is widespread in care-dependent older people, but no consensus for a minimum standard of oral health care exists yet.MethodsThe e-Delphi approach was applied to a selected panel of interdisciplinary experts. Data analysis was based on three measurements: (a) ≥70% of experts’ opinion fall into category “agree or strongly agree,” (b) median score on the 5-point Likert scale ≥4, (c) interquartile range ≤1.ResultsA total of 31 experts from 17 European countries participated in this survey. Agreement was achieved for a compulsory dental examination when an elder is admitted to a long-term care (LTC) facility. Older people should brush their teeth twice/day and regularly clean interproximal spaces and oral mucosa. Dentures should be rinsed after meals and cleaned twice/day. The use of denture cleansing tablets was considered necessary. Dentures should be removed before sleeping and stored dry. A 5000 ppm fluoride toothpaste should be applied daily in elder with high caries risk. A short report on the oral health status of the elder should be included in the geriatric assessment. All experts concluded that the knowledge and the training in oral health care for caregivers and family members of care-dependent older people were imperative.ConclusionsUsing the e-Delphi method, multidisciplinary healthcare professionals from different countries agreed on certain cardinal recommendations for a standard oral health care for care-dependent older people.

Abstract

For each patient there is a natural head position which responds to individual and environmental factors, like survival, independent from occlusion. The purpose of this study was to value the radiographicdiagnostic differences between lateral cephalometric radiographs taken in natural head position (NHP) and in a standard position with cephalostatic ear rods in the craneocervical angulation of each patient. Thirty patients were evaluated and two lateral cephalometric radiographs were taken for each subject, one in natural head position and the other in standard position with the cephalostatic ear rods. They were classified as biotype I, II, III according to their facial patterns and the ANB angle. The measurements were compared using both techniques and biotypes. The results showed statistical significant differences between both techniques on biotype III, but for biotype I and II the results were similar. It is suggested that the lateral cephalometric radiographs in the biotype III subjects be taken in natural head position.

Abstract

The objective of this study is to present a descriptive and retrospective analysis of the behavior of trauma to the craniofacial complex produced by firearms in patients that consulted the Maxillofacial Surgery and Estomatologic Service of the San Vicente de Paul University Hospital (HUSVP) and the School of Dentistry of the University of Antioquia between the years1998 and 2003. The parameters considered were age, sex, type of firearm used, motive of aggression, and number of impacts received. If the patients were under the effects of alcohol or psychoactive drugs at the moment of injury were also evaluated. The type of injury caused to hard and soft tissues, type of treatment performed, complications and type of healing processes were studied as well. 137 patients examined with this type of traumatism presented the following findings: the most frequent age interval reported for this type of injury was between 21 and 30 years of age with 37% of the registered cases, 89% of the patients were males, 90% of the wounds were caused by pistols, assault was the motive in 58% of the aggressions, 89% of the patients never had antecedents of this type of traumatism, 50% of the patients presented a single bullet impact and 92 of the 137 patients had never consumed alcohol or drugs. The most common treatment was closing the injury, 39% of the cases and 28% of the patients received a combination of opening and closing, 88% of the patients didn’t present an infection during treatment and a correct healing process was observed in 72% of the patients. 60% of the patients required treatments for more than ten weeks before final discharge and 70% were see within the first five days at the Maxillofacial Surgery service after being injured. The most affected anatomical regions in the midface were the malar and dentoalveolar processes and the mandibular corpus and dentoalveolar process in the mandible.

A clinical case report of a fifteen-year-old female with an Angle class III malocclusion and with maxillary deficiency and mandibular prognathism is presented. The patient initially consulted at an early age and a decision was made to delay comprehensive treatment until her skeletal growth ended. Alexander’s prescription (Ormco) was used for orthodontic treatment and a mandibular surgical set back only with oblique ramous osteotomy was performed achieving an adequate occlusal relationship and facial balance. This patient was treated according to the protocol established for the orthodontic and oral and maxillofacial surgery postgraduate programs of the Dental Faculty of the University of Antioquia.

Abstract

Ameloblastoma is the most common benign tumor, It represents 1% of all odontogenic tumors and comprises 11 to 18% of all maxillary lesions. Eighty percent of the cases described are located in the body of the mandible and its angle; it affects males and females equally, and its more prevalent during the third and fourth decades of life. This tumor has diverse clinical, radiographical and histo-pathological aspects. Although it is considered a benign pathology, it is locally invasive and has a high rate of recurrence. The objective of this article is to present the experience obtained with the diagnosis and treatment of ten cases of ameloblastoma in patients ranging from 12 and 73 years of age. Eighty percent of theses cases were located in the mandible and were causing different clinical manifestations such as: facial asymmetry, dental displacement, root resorption, alteration in the occlusion, pain, masticatory difficulty and pathological fractures. According to their image a radiographic classification was made, identifying them as unilocular and multilocular. Histopathologically, they were grouped according to the observed variant. Different treatment alternatives are described, as well as the complications encountered. Two patients had previous surgery for dentigerous cysts in the same area. Two cases had recurrent ameloblastoma five years after their surgical interventions performed in other institutions.

37. Confiabilidad de las predeterminaciones cefalométricas en cirugía ortognática

Abstract

La predeterminacion cefalometrica en cirugia ortognatica es una aproximacion al resultado final que hacen el cirujano maxilofacial y el ortodoncista en la radiografia cefalica lateral prequirargica, con el fin de prever la dimension de los movimientos en los tejidos duros y blandos que se realizaran en el paciente durante la cirugia. Esta tecnica se ha utilizado como una herramienta diagn6stica valiosa permitiendo al cirujano, al ortodoncista y al paciente tener una vision de los resultados que se van a obtener. En esta investigacion se calculO el porcentaje de confiabilidad de las predeterminaciones cefalometricas hechas manualmente en treinta y tres pacientes sometidos a cirugia ortognirtica, comparando el namero de variables que se aproximaran a lo esperado en un rango de 95%. Se utilizaron las radiografias prequirargicas (T1), sobre las cuales se hizo el trazado cefalometrico, la prediccion de los movimientos de los maxilares necesarios para cada paciente (T2), y la comparaciOn con el trazado cefalometrico sobre las radiografias postquirOrgicas (T3). Para determinar el porcentaje de confiabilidad se midio la distancia en puntos cefalometricos de tejidos duros y blandos, entre ambos trazos superimpuestos (T2-T3). Estas medidas se compararon tanto en sentido horizontal (Plano de Frankfort), como en sentido vertical (Plano de McNamara). Los resultados muestran como las predeterminaciones hechas sobre acetatos tienen alto porcentaje de confiabilidad en los movimientos del maxilar superior y de la mandibula, pero mas en el piano vertical que en el horizontal. Por el contrario, la predeterminacion del menton se comporta de una manera menos confiable, siendo menor la confiabilidad en el piano anteroposterior, que en el vertical. Los resultados obtenidos estan en concordancia con los reportados por otros autores, quienes consideran la predeterminaci6n cefalometrica como una herramienta vita previa a la realizacion del plan de tratamiento ortodoncico quirúrgico.

Abstract

Objectives: This study aimed to obtain a consensus on oral health policy, access to dental care, oral hygiene measures and training levels. Background: Poor oral health is widespread in care-dependent older people, but no consensus for a minimum standard of oral health care exists yet. Methods: The e-Delphi approach was applied to a selected panel of interdisciplinary experts. Data analysis was based on three measurements: (a) ≥70% of experts’ opinion fall into category “agree or strongly agree,” (b) median score on the 5-point Likert scale ≥4, (c) interquartile range ≤1. Results: A total of 31 experts from 17 European countries participated in this survey. Agreement was achieved for a compulsory dental examination when an elder is admitted to a long-term care (LTC) facility. Older people should brush their teeth twice/day and regularly clean interproximal spaces and oral mucosa. Dentures should be rinsed after meals and cleaned twice/day. The use of denture cleansing tablets was considered necessary. Dentures should be removed before sleeping and stored dry. A 5000 ppm fluoride toothpaste should be applied daily in elder with high caries risk. A short report on the oral health status of the elder should be included in the geriatric assessment. All experts concluded that the knowledge and the training in oral health care for caregivers and family members of care-dependent older people were imperative. Conclusions: Using the e-Delphi method, multidisciplinary healthcare professionals from different countries agreed on certain cardinal recommendations for a standard oral health care for care-dependent older people. © 2020 Gerodontology Association and John Wiley & Sons Ltd

Abstract

PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks.

Abstract

Background & Aims: Good outcomes after liver transplantation (LT) have been reported after successfully downstaging to Milan criteria in more advanced hepatocellular carcinoma (HCC). We aimed to compare post-LT outcomes in patients receiving locoregional therapies (LRT) before LT according to Milan criteria and University of California San Francisco downstaging (UCSF-DS) protocol and ‘all-comers’. Methods: This multicentre cohort study included patients who received any LRT before LT from Europe and Latin America (2000–2018). We excluded patients with alpha-foetoprotein (AFP) above 1,000 ng/ml. Competing risk regression analysis for HCC recurrence was conducted, estimating subdistribution hazard ratios (SHRs) and corresponding 95% CIs. Results: From 2,441 LT patients, 70.1% received LRT before LT (n = 1,711). Of these, 80.6% were within Milan, 12.0% within UCSF-DS, and 7.4% all-comers. Successful downstaging was achieved in 45.2% (CI 34.8–55.8) and 38.2% (CI 25.4–52.3) of the UCSF-DS group and all-comers, respectively. The risk of recurrence was higher for all-comers (SHR 6.01 [p <0.0001]) and not significantly higher for the UCSF-DS group (SHR 1.60 [p = 0.32]), compared with patients remaining within Milan. The all-comers presented more frequent features of aggressive HCC and higher tumour burden at explant. Among the UCSF-DS group, an AFP value of ≤20 ng/ml at listing was associated with lower recurrence (SHR 2.01 [p = 0.006]) and better survival. However, recurrence was still significantly high irrespective of AFP ≤20 ng/ml in all-comers. Conclusions: Patients within the UCSF-DS protocol at listing have similar post-transplant outcomes compared with those within Milan when successfully downstaged. Meanwhile, all-comers have a higher recurrence and inferior survival irrespective of response to LRT. RESUMEN Antecedentes: España por su situación geopolítica no está exenta de amenazas y desafíos asociados a enfermedades infecciosas víricas como las «fiebres hemorrágicas víricas». Implementar acciones para disminuir estos riesgos frente a agentes víricos con el desarrollo de un tratamiento de urgencia por vía intravenosa resulta de gran interés. Objetivos: Desarrollar dos formulaciones de Ribavirina 100 mg/ml con distintos excipientes y condiciones de fabricación para buscar la formulación más estable. Material y métodos: Se ha utilizado ribavirina, distintos tampones y agua para inyectables. Se ha evaluado alteraciones físicas, riqueza y pH siguiendo especificaciones de la monografía de la Farmacopea Europea. Resultados: Se exponen resultados de pH y riqueza de ambas formulaciones con y sin esterilización terminal. Conclusiones: Los resultados obtenidos cumplen especificaciones para ambas formulaciones, demostrando una estabilidad química tras su conservación en condiciones de estudios a largo plazo durante 12 meses y acelerados durante 6 meses. El desarrollo de la ribavirina 100 mg/ml (10% w/v) inyectable satisface una necesidad terapéutica relacionada con el tratamiento de primera elección en enfermedades víricas altamente contagiosas que carecen de tratamientos eficaces. SUMMARY Background: Spain, due to its geopolitical situation, is not exempt from the threats and challenges associated with viral infectious diseases such as the «viral hemorrhagic fevers». Implementing actions to reduce these risks with the development of an emergency treatment against these viral agents for intravenous is of great interest. Objective: Develop two formulations of Ribavirin 100 mg/ml with different excipients and manufacturing conditions to look for the most stable formulation. Material and methods: Ribavirin, various buffers and water for injections have been used. Physical alterations, richness and pH have been evaluated following specifications of European Pharmacopoeia monograp. Results: Results of pH and richness of both formulations are exposed with and without terminal sterilization. Conclusions: The results obtained meet specifications for both formulations, demonstrating chemical stability after preservation under long-term study conditions for 12 months and accelerated for 6 months. The development of injectable 100 mg/ml ribavirin (10% w/v) satisfies a therapeutic need related to the treatment of first choice in highly contagious viral diseases that lack treatments.

Abstract

A demência é uma síndrome neurológica de agravamento progressivo, sem cura, cuja prevalência tem vindo a aumentar devido ao envelhecimento da população. Existe um grande desconhecimento entre profissionais de saúde e cuidadores relativamente à melhor abordagem da alimentação nos doentes com demência avançada. Dado não existirem recomendações nacionais acerca deste tema, foi elaborado um documento de consenso da Sociedade Portuguesa de Medicina Interna e da Associação Portuguesa de Nutrição Entérica e Parentérica que explicita as orientações existentes relativas à abordagem dos problemas alimentares nos doentes com demência avançada. A demência avançada é uma condição terminal em que deve ser privilegiado o conforto do doente, frequentemente acamado, incapaz de comunicar verbalmente e com dificuldade na alimentação. Nesta população, a literatura atual não recomenda o uso de alimentação por sonda (nasogástrica, nasojejunal, gastrostomia percutânea ou jejunostomia percutânea), que está associada a maior risco de infeção, maior utilização de meios de contenção e de- senvolvimento de úlceras de decúbito. Como alternativa, recomenda-se a alimentação por via oral de acordo com a tolerância e vontade do doente (alimentação de conforto). Do ponto de vista ético e legal, é legítimo não proceder à artificialização da alimentação na fase terminal da demência caso este procedimento seja contrário aos valores da pessoa e não se objetivem benefícios. Esta decisão deve ser tomada após discussão multidisciplinar incluindo o doente (se possível), representante legal, cuidadores, família e equipa de profissionais de saúde envolvidos, elaborando um plano individual de cuidados que permita a tomada de decisões no melhor interesse do doente., Medicina Interna, Vol. 27 N.º 1 (2020): Janeiro / Março

Abstract

Material and methods: Experts in each GBM treatment field have briefly described it based in their experience and the reviewed of the literature. Results: Each area has been summarized and the consensus of the brain tumor group has been included at the end. Conclusions: GBM are aggressive tumors with a dismal prognosis, however accurate treatments can improve overall survival and quality of life. Neurosurgeons must know treatment options, indications and risks to participate actively in the decision making and to offer the best surgical treatment in every case.
